Description
Denne oppfinnelse vedrører farmasøytiske formuleringer som omfatter amoxycillin og et salt av clavulansyre (heretter betegnet "clavulanat" hvis ikke et spesifikt salt er identifisert).
Kombinasjonen av amoxycillin og clavulanat er en empirisk effektiv behandling for bakterielle infeksjoner og kan bli gitt ved oral dosering, for eksempel i form av tabletter, og, i formuleringer for barn, som vannholdige løsninger eller suspensjoner, typisk som er en smakstilsatt sirup.
Clavulanat er en p-laktamasehemmer og er inkludert med p-laktam antibiotisk amoxycillin for å motvirke en p-lactamasemediert resistensmekanisme. Noen mikroorganismer slik som Stæptococcus pneumoniae har resistensmekanismer som ikke er p-laktamasemediert. Generelt viser W094/16696 at kaliumclavulanat kan øke effektiviteten av beta-laktam antibiotika slik som amoxycillin mot mikroorganismene som har en resistensmekanisme som ikke er p-laktamasemediert.
Stæptococcus pneumonia er en viktig patogen i luftveisinfeksjon i samfunnet. S. pneumoniae er den mest vanlige impliserte bakterie i de viktige luftveisinfeksjoner som betennelse i mellomøre hos barn og bihulebetennelse i pasienter i alle aldre og forverring av bronkitt og pneumococcal lungebetennelse hos voksne. I rapporter i Europa og i USA er det en økning av tilsynekomsten av DRSP (drug-resistant Stæptococcus pneumoniae) med synkende mottakelighet for p-laktam og andre antibiotika.
Mens bekreftede tilfeller av DRSP-infeksjon kan bli behandlet med relativt høye mengder av amoxycillin, gjenstår det fortsatt et behov for å utvikle empiriske behandlinger hvor man har mistanke til DRSP, for eksempel i et område med en høy prevalens av DRSP, men hvor, organismer som produserer p-laktamase også kan være tilstede.
Det er nå blitt oppdaget at empirisk behandling av infeksjoner som kan være forårsaket av DRSP med hell kan bli behandlet med formuleringer av co-amoxiclav som har en relativt stor mengde av amoxycillin.
I overensstemmelse med det, fremskaffer denne oppfinnelsen et farmasøytisk preparat inneholdende amoxycillin og clavulanat, kjennetegnet ved at det er i form av et pulver eller granulert produkt for rekonstituering til en multippeldosesuspensjon eller -oppløsning og som etter rekonstituering inneholder 500 til 700 mg amoxycillin pr. 5 ml og fra 35 til 50 mg clavulanat pr. 5 ml, slik at forholdet mellom amoxycillin og clavulanat er 14:1.
I henhold til et annet aspekt, omfatter foreliggende oppfinnelse en farmasøytisk formulering inneholdende amoxycillin og clavulanat, kjennetegnet ved at det er i form av en rekonstituert multippeldosesuspensjon eller oppløsning som inneholder 500 til 700 mg amoxycillin pr. 5 ml og fra 35 til 50 mg clavulanat pr. 5 ml, slik at forholdet mellom amoxycillin og clavulanat er 14:1.
Slike formuleringer er i bruk for den empirisk behandling av infeksjoner, som kan være forårsaket av DRSP, spesielt i luftveisinfeksjoner slik som mellomørebetennelse hos barn og bihulebetennelse i pasienter i alle aldre og akutt forverring av bronkitt og pneumococcal lungebetennelse hos voksne.
Oppfinnelsen fremskaffer også anvendelse av amoxycillin og clavulanat i et nominelt forhold på 14:1 ved fremstilling av et legemiddel for oral administrering til pediatriske pasienter i form av et pulver eller granulert produkt for rekonstituering til en suspensjon eller oppløsning, for behandling av luftveisinfeksjoner i en dose på 75 til 115 mg/ amoxycillin pr. kg pr. dag og fra 5 til 7,5 mg clavulanat pr. kg pr. dag, i alt administrert bid (to ganger om dagen).
Spesielt anvendes amoxycillin og clavulanat i fremstillingen av et medikament for oral administrasjon for empirisk behandling av infeksjoner som kan være forårsaket av DRSP i humane pasienter.
Oppfinnelsen fremskaffer også en fremgangsmåte for fremstilling av en farmasøytisk formulering, kjennetegnet ved at formuleringens bestanddeler blandes i passende rekkefølge.
Formuleringen av denne oppfinnelsen er passende til bruk i pasienter i alle aldre, inkludert voksne, eldre barn og barn.
Vektratioene av amoxycillin:clavulanat er her uttrykt som frie syre-ekvivalenter. Foretrukket amoxycillin:clavulanat ratioer er ca. 14:1 ±5%.
I formuleringene ifølge oppfinnelsen, er amoxycillin foretrukket i form av amoxycillintrihydrat selv om natriumamoxycillin, for eksempel den krystallinsk form av natriumamoxycillin som er innbefattet i EP 0131147 A, også kan benyttes.
Clavulanat er fortrinnsvis i form av kaliumclavulanat. Kaliumclavulanat er ekstremt fuktighetssensitivt og bør bli lagret og håndtert under forhold med 30% RH eller mindre, helst så lavt som mulig. Faste doseringsformer burde bli pakket i atmosfæriske fuktighetssikre beholdere, og slike former og/eller deres beholdere bør inneholde et dehydreringsmiddel.
Formuleringene ifølge oppfinnelsen kan bli laget i faste doseringsformer for oral administrasjon ved en fremgangsmåte som er vanlig i farmasøytisk teknologi, f.eks tabletter eller pulver eller granuløse produkter som kan bli løst opp i en suspensjon eller løsning. Passende reagenser og passende fremgangsmåter for å lage slike tabletter er fremsatt i for eksempel GB 2 005 538-A, WO 92/19227 og WO 95/28927. Pulver eller granuløse formuleringer, slik som barnesuspensjons-formuleringer, kan bli fremstilt ved å bruke teknikker som er generelt vanlige i feltet av fremstilling av farmasøytiske formuleringer og i fremstillingen av tørre formuleringer som blir løst i slike suspensjoner. For eksempel er en passende teknikk å blande tørrpulveriserte eller granuløse ingredienser for så å helle dette over i en passende beholder.
For barnedosering, er formuleringene ifølge oppfinnelsen helst fremstilt i en søtkrydret vannholdig sirupformulering av generelt vanlig formulering (unntatt for dets nye amoxycillin:clavulanat ratio og tilsiktet anvendelse) som inneholder en passende vekt av amoxycillin og clavulanat i et enhetsdosevolum, f.eks., 5 ml eller 2,5 ml av sirupen. På grunn av vannsensitiviteten av clavulanat er det foretrukket å fremskaffe en slik sirupformulering som tørt pulver eller granulater som inneholder en atmosfærisk fuktighetssikker beholder eller pose for å blande i vann eller andre passende vannholdige media korttid før bruk.
Formuleringen ifølge denne oppfinnelsen vil normalt, i tillegg til de aktive stoffene amoxyciliintrihydrat og kaliumclavulanat, også inkludere hjelpestoffer som er standard i feltet av formuleringer for oral dosering og brukt generelt i standard proporsjoner, og ved generelt standard partikkelstørrelse og graderinger etc.
I tilfelle av orale barnesuspensjoner, kan disse eksperimentene omfatte suspensjonshjelpestoffer, glidemiddel (for å hjelpe fylling), fortynningsmiddel, svellemiddel, smaksstoffer, søtningsstoffer, stabilisatorer, og i tilfelle av tørre formuleringer for å lage en vannholdig suspensjon, et spiselig dehydreringsmiddel for å sikre preserveringen av kaliumclavulanat mot hydrolyse ved atmosfærisk fuktighet på lageret. Kaliumclavulanat er normalt levert i blanding med silisium-dioksid som fortynningsmiddel.
Passende hjelpestoffer til anvendelse inkluderer xantangummi (supensjons-hjelpemiddel, kolloidalsilika (glidemiddel), ravsyre (stabilisator), aspartam
(søtningsstoff), hydroksypropylmetylcellulose (suspensjonshjelpestoff) og silisium-dioksid (dehydreringsstoff, fortynner for kaliumclavulanat og svellemiddel). Smaksstoffer kan omfatte vanlige smaksstoffer slik som appelsin, banan, bringebær og sirup, eller blandinger derav, som passer til lokale krav.
Generelt kan proporsjonene av aktive stoffer amoxycillintrihydrat og kaliumclavulanat i en tørr formulering for tilbereding med vannholdigmedium i en løsning, suspensjon eller sirupformulering ifølge oppfinnelsen være ca. 30-80 vt%.
Derfor fremskaffer denne oppfinnelsen en prosess for fremstilling av en formulering som beskrevet ovenfor.
Formuleringene kan ifølge oppfinnelsen bli tilpasset barnedosering, dvs. til pasienter i alderen mellom 3 måneder til 12 år. Slike formuleringer kan bli dosert i daglige mengder opp til maksimum normalt tillatt dose av amoxycillin og clavulanat.
En passende doseringsmengde av formuleringen ifølge oppfinnelsen er for barn 75 til 115 mg/kg amoxycillin per dag og 5 til 7,5 mg/kg av clavulanat per dag. Passende doseringer er gitt, for eksempel i to ,helst like doseenheter per dag, passende med rundt 12 timers mellomrom. En passende dosering til bruk i et slikt regime er 90±10%, helst ±5%, mg/kg amoxycillin og 6,4 ±10%, helst ±5%, mg/kg clavulanat (dvs. nominelt en 14:1 ratio) per dag.
Passende, barneformuleringer som er beskrevet i det foregående og er fremskaffet omfatter 500 til 700, helst ca. 600 mg av amoxycillin/5 ml av formulering når oppløst og fra 35 til 50 mg, helst ca 43 mg av clavulanisk syre/5 ml av formulering når den er oppløst.
For eldre barn og voksne pasienter kan disse mengdene bli øket pro rata. En passende dosering for anvendelse i et slikt regime er 3500±10%, helst ±5%. mg amoxycillin og 250+10, helst 5%, mg clavulanat (dvs. nominelt en 14:1 ratio) per dag, fortrinnsvis gitt i, for eksempel i to, helst like, doseenheter per dag, passende rundt 12 timers mellomrom.
Formuleringen ifølge oppfinnelsen kan for eksempel bli fremskaffet i faste doseenhetsformer utformet i passende mengder for administreringen av en slik daglig dose. For eksempel kan en doseenhetsform være tabletter, eller poser som inne-holder granula eller pulver for oppløsning, en eller to som er gitt ved hvert dose-interval. Alternativt kan en doseenhet bli fremskaffet som en "bulk" av fast stoff eller løsning eller suspensjon, f. eks som en sirup for barneadministrasjon, sammen med en passende måleenhet av kjent type for å lette administrasjonen av en passende doseenhetsmengde av formuleringen. En passende doseenhetsmengde er en som tillater administrasjon av den ovenfornevnte daglige dose-mengde delt mellom to ganger daglige doser.
For barn, er en passende doseenhetsmengde en som tillater administrasjon av den ovenfornevnte daglige doseringsmengde, delt mellom to doser, f.eks., halvparten av den ovenfornevnte daglige dose i et volum av en løsning eller suspensjon passende for oral administrasjon til et barn, helst mellom 2,5 til 10 ml, helst som sirup. En barneformulering kan derfor omfatte en "bulk" av en løsning eller suspensjon, f.eks., en sirup eller granula eller pulver som kan bli fremstilt i en løsning eller suspensjon, ved en konsentrasjon av løsning eller suspensjon som inneholder en slik dose i et slikt volum.
Denne oppfinnelsen fremskaffer derfor også den formuleringen som er beskrevet ovenfor til administrasjon av slike doser.
For voksne, kan en passende doseenhet bli gitt i en tablett. Passende for et to ganger daglig doseregime basert på 1750 mg amoxycillin/125 mg clavulanat per doseenhet, kan dette beleilig bli gitt som to tabletter, en bestående av amoxycillin og clavulanat og en annen bestående av amoxycillin alene. På den måten, i et videre aspekt, fremskaffer denne oppfinnelsen en doseenhet på 1750 mg amoxycillin og 125 mg clavulanat gitt som to tabletter, en som består av 875 mg amoxycillin og 125 mg clavulanat og en annen som består av 875 mg amoxycillin. En passende tablett som består av 875 mg amoxycillin og 125 mg clavulanat er markedsført av SmithKline Beecham i flere land og er også beskrevet i WO 95/28927 (SmithKline Beecham).
Oppfinnelsen vil nå bli beskrevet ved eksempler bare med referanse til fig. 1,2 og 3 som grafisk viser resultatene i eksempel 3 nedenfor.
Fig. 1,2 og 3 viser henholdsvis Log™ av "colony forming units" ("cfu") av S. pneumoniae stammene N1387,14319 og 410101 per lunger observert i rotter etter dosering med en amoxycillin:kaliumclavulanat ("AMX:CA") formulering av denne oppfinnelsen gitt ved 45:3,2 mg/kg amoxycillin : clavulanisk syreekvivalent, en sammenlignbar formulering gitt ved 22,5:3,2 mg/kg, og en ikke-behandlet kontroll ("NTC") som beskrevet nedenfor.
Eksempel 1 - Formulering for barn
Følgende formuleringer for barn når oppløst omfatter 600 mg amoxycillin og 42,9 mg clavulansyre i 5 ml suspensjon.
Basert på 86% styrke som amoxycillin fri syre
basert på 41% styrke som clavulansyre i kaliumclavulanat/Syloid 1:1
blanding, inkludert 8 % overskudd
mengde av silisiumdioksyd (Syloid)varianter, i forhold til mengdene av amoxycillintrihydrat og kaliumclavulanat/Syloidblanding, slik at total fyllvekt forsatt er konstant på 1100,00 mg.
Flaskene er fylt med 23,92 g av formulert pulver og deretter løst med 84 ml av vann rett før bruk, for å gi en 100 ml suspensjon.
Eksempel 2- Tablettformulering
En tablettformulering som omfatter 875 mg amoxycillin og 125 mg clavulanat ble laget med følgende sammensetning:
Tablettene er laget ved å blande amoxycillinet, kaliumclavulanatet, og en porsjon av mikrocrystallincellulose og magnesiumstearat, valset for å sammenpresse denne blandingen, deretter blandet med de andre bestandelene, før tablettene ble laget på en vanlig tablettpresse og overtrukket. Tablettkjernen ble overtrukket med en film (Opadry White YS-1 7700/Opadry White OY-S-7300 ex Colorcon) fra et vannholdig løsningssystem, for å gi tabletter med en nominell overtrukketvekt av 1482 mg. Ytterligere detaljer av hvordan tablettene er fremstilt er gitt i WO 95/28927 (SmithKline Beecham).
Like tabletter kan bli laget hvor valsekomprimeringstrinnet er byttet ut med slenging og/eller et siste belegg er tilført fra et organisk løsningssystem slik som diklormetan heller enn et vannholdig løsningssystem.
En tablett formulering som omfatter 875 mg amoxycillin ble laget ved å ha følgende sammensetning:
Kjernebestandeler (mg/tablett)
Tablettene er laget ved å blande amoxycillin og deler av mikrocrystallincellulose og magnesiumstearat, valsekomprimering av denne blandingen, deretter blanding med andre bestandeler, før tablettfremstilling på en vanlig tablettpresse og overtrekking.
Eksempel 3- Biologiske data - In vivo rottemodell:
Metodikk
Dyrene ble anestesert og i den ytre halsvene ble det lagt inn en kanyle for administrasjon av forbindelsene. Minst 48 timer senere ble dyrene infisert ved intrabronkiale drypp av en 50 mikroliter inokulat av S. pneumoniae ved en ikke-kirurgisk intubasjon. Inokulatet ble laget i avkjølt smeltet næringsagar med et sluttrnokulat på ca. 10<6>cfu i 50 mikroliter agar. Doseringsstart var 24 timer etter infeksjon og forbindelsene ble gitt som en kontinuerlig infusjon inn i halsvenen designet for å simulere konsentrasjonen i rotteplasma versus tidskurve oppnådd i humant serum etter oral administrasjon av amoxycillin/clavulanat. For hver organisme som ble testet, ble det brukt tre grupper av dyr. De første to gruppene fikk amoxycillin og clavulanat for å stimulere to ganger daglig dosering av denne kombinasjonen ved enten 22,5/3,2 mg/kg (en 7:1 ratio) eller 45/3,2 mg/kg en 14:1 ratio) til barn. Den gjenværende gruppe mottok en infusjon av saltvann ved en hastighet lik som de doserte gruppene og opptrådde som infiserte ikkebehandlete kontroller. Doseringen fortsatte i 2-5 dager, og etter 14 dagers behandling ble dyrene drept og lungene fjernet aseptisk for bakteriell bestemmelse.
Resultater
Tabell 1 viser MIC"s av amoxycillin, amoxycillin:clavulanat og penicillin G for de tre resistente stammene av S. pneumoniae som ble testet.
Steptococcus Pneumoniae N1387:
Bakterielt antall i lungene av saltvannsbehandlete dyr var 6,97±0,30 logio cfu/lunger. Begge dosene av amoxycillin : clavulanat reduserte betydelig antallet av levende bakterier i lunger sammenlignet med kontrolldyr (4,37+0,93 logio cfu/lunger og 2,62+0,85 logio cfu/lunger for henholdsvis 7:1 og 14:1 ratio; p<0,01). Men som vist i fig. 1 amoxycillin : calvulanate ved 14:1 to ganger daglig ratio var betydelig mer effektiv enn når det ble administrert ved en lavere ratio av 7:1.
Streptococcus Pneumoniae 14319:
Bakterielt antall i lungene av saltvannsbehandlete dyr var 6,8±0,62 logio cfu/lunger. Amoxycillin : clavulanat ved 7:1 ratio reduserte antallet av levende bakterie i lungene (6,26±0,47 logio cfu/lunger), men denne reduksjonen nådde ikke signifikans sammenlignet med kontrolldyr. Imidlertid som vist i fig.2 amoxycillin : calvulanate ved 14:1 to ganger daglig ratio reduserte bakterieantallet til 4,28±0,82logio cfu/lunger slik at denne dosen var betydelig mere effektiv enn kontrolldyr og dyrene behandlet med den lavere ratio 7:1.
Streptococcus Pneumoniae 410101.
Bakterielt antall i lungene i saltvannsbehandlete dyr var 7,11±0,45logi0 cfu/lunger. Amoxycillin : clavulanat ved 7:1 ratio reduserte antallet av levende bakterier i lungene (6,14 ± 0,47logio) betydelig sammenlignet med kontrolldyr (p, 0,05). Imidlertid som vist i fig.3 amoxycillin : clavulanat ved 14: 1 ratio to ganger daglig reduserte bakterietallet til 3,91+0,81 logio cfu/lunger og var betydelig mere effektivt enn dyr som ble behandlet med den lavere ratio på 7:1.
Claims (16)
1. Farmasøytisk preparat inneholdende amoxycillin og clavulanat, karakterisert ved at det er i form av et pulver eller granulert produkt for rekonstituering til en multippeldosesuspensjon eller -oppløsning og som etter rekonstituering inneholder 500 til 700 mg amoxycillin pr. 5 ml og fra 35 til 50 mg clavulanat pr. 5 ml, slik at forholdet mellom amoxycillin og clavulanat er 14:1.
2. Formulering som angitt i krav 1, karakterisert ved at det etter rekonstituering inneholder 600 mg amoxycillin pr. 5 ml og 43 mg clavulanat pr. 5 ml.
3. Farmasøytisk formulering inneholdende amoxycillin og clavulanat, karakterisert ved at det er i form av en rekonstituert multippeldosesuspensjon eller oppløsning som inneholder 500 til 700 mg amoxycillin pr. 5 ml og fra 35 til 50 mg clavulanat pr. 5 ml, slik at forholdet mellom amoxycillin og clavulanat er 14:1.
4. Formulering som angitt i krav 3, karakterisert ved at den inneholder 600 mg amoxycillin pr. 5 ml og 43 mg clavulanat pr. 5 ml.
5. Formulering som angitt i et av kravene 1til4, karakterisert ved at amoxycillin er i form av amoxycillintrihydrat.
6. Formulering som angitt i ett av kravene 1 til 5, karakterisert ved at clavulanat er i form av kaliumclavulanat.
7. Formulering som angitt i et av kravene 1 tit 6, karakterisert ved at det som hjelpestoff inneholder et suspenderingshjelpemiddel som er hydroksypropylmetylcellulose.
8. Farmasøytisk formulering inneholdende amoxycillin og clavulanat, karakterisert ved at det er i form av et pulver eller granulert produkt for rekonstituering til en multippeldosesuspensjon eller oppløsning og som, etter rekonstituering, inneholder de følgende bestanddeler i 5 ml suspensjon:
9. Fremgangsmåte for fremstilling av en farmasøytisk formulering ifølge et av kravene 1,2 og 5 til 8, karakterisert ved at formuleringens bestanddeler blandes i passende rekkefølge.
10. Fremgangsmåte for fremstilling av en farmasøytisk formulering ifølge krav 3 og 4, karakterisert ved at vann settes til en formulering som angitt i krav 1.
11. Anvendelse av amoxycillin og clavulanat i et nominelt forhold på 14:1 ved fremstilling av et legemiddel for oral administrering til pediatriske pasienter i form av et pulver eller granulert produkt for rekonstituering til en suspensjon eller oppløsning, for behandling av luftveisinfeksjoner i en dose på 75 til 115 mg/ amoxycillin pr. kg pr. dag og fra 5 til 7,5 mg clavulanat pr. kg pr. dag, i alt administrert bid.
12. Anvendelse ifølge krav 11 for fremstilling av et medikament for behandling av infeksjoner forårsaket av legemiddelresistent Strep pneumoniae.
13. Anvendelse som angitt i krav 12 for fremstilling av et medikament for behandling av infeksjoner forårsaket av penicillinresistent Strep pneumoniae.
14. Anvendelse som angitt i et av kravene 11 til 13, for fremstilling av et medikament for behandling av mellomørebetennelse.
15. Anvendelse som angitt i et av kravene 11 til 13 for fremstilling av et medikament for behandling av en infeksjon som er bihulebetennelse.
16. Anvendelse som angitt i et av kravene 11 til 15, hvor dosemengden er 90 10% mg amoxycillin pr. kg og 6,4 ± mg clavulanat pr. kg.
