Description
Oppfinnelsen vedrører anvendelse av en bestemt forbindelse, N-metyl-N-[ (IS )-l-fenyl-2-( (3S)-3-hydroksypyrrolidin-l-yl)-etyl]-2,2-difenylacetamid, og/eller et av dens fysiologisk akseptable salter, til fremstilling av legemiddelpreparater for behandling av inflammatoriske tarmsykdommer.
Lignende forbindelser og egnede fremgangsmåter for fremstilling derav, er beskrevet i offentliggjørelsesskriftet DE 40 34 785.
Inflammatoriske tarmsykdommer fører ofte til tykk-tarmssmerter, fordøyelsesforstyrrelser og i verste fall til en tarmlukning. Det sistnevnte er forbundet med kolikklignende smerter som en følge av en kraftig sammentrekningsirritasjon, avførings- og vindforhold, oppkast og med tiltagende varighet av tilstanden til dehydratasjon, tilbakeholdingsspenning i bukhulen og til sist et sjokk.
Funksjonelle tarmsykdommer tilbakeføres til de forskjelligste årsaker; blant annet en unormalitet i de glatte tarmmusklenes sammentrekningsevne og i den motoriske aktivitet i mage- og tarmkanalen. En for stor sammentrekningsaktivitet og en forandret koordinasjon av den motoriske aktivitet kan fremkalle smerter gjennom aktiveringen av mekanoreseptorene og ved transportunormaliteter, noe som fører til utvidelse av tarmen. Disse årsakene ble tidligere også brukt til forklaring av brystsmerter som ikke stammer fra hjertet, og også til forklaring av smertene ved nervøst tarmsyndrom eller en ikke med et magesår forbundet dyspepsi. I alle fall ble denne sammen-hengen ytterligere understøttet gjennom 24-timers registre-ringer av den motoriske spiserørs- og mage- og tolvfinger-tarmfunksjon hos pasienter som lider under brystsmerter som ikke stammer fra hjertet, og av en dyspepsi som ikke er forbundet med magesår (Katz, P.O. et al Ann. Intern. Med. (1987) 106. 593-7). Motoriske unormaliteter kan opptre hos normale kontrollpersoner uten symptomer, men kan også forsvinne, hvorved det vil kunne påvises en tidsmessig korrelasjon mellom symptomene hos pasienten (Fefer, L et al Gastroenterology
(1992) 102: A447 (sammendrag)).
Behandlingen av de motoriske unormaliteter med forskjellige terapeutiske agenser, som f eks med midler som fører til bevegelser i mage- og tarmkanalen, med anticholi-nergika eller kalsiumkanal- og kolecystokininantagonister, er i de fleste tilfeller virksom ved korreksjon av de motoriske unormaliteter, den forbedrer imidlertid ikke alltid symptomene hos pasientene.
Det var således en oppgave ved oppfinnelsen å stille til rådighet en farmasøytisk aktiv forbindelse som er anvend-bar og er aktiv ved behandlingen av inflammatoriske tarmsykdommer, samtidig lindrer smertene som er forbundet med denne sykdommen, og som i akutte tilfeller igjen normaliserer eller igjen setter i gang tarmens motorikk ved en gjennom den inflammatoriske tarmsykdom truende eller fremkalt tarmlukning, uten å fremkalle påvisbare bivirkninger. Samtidig var det en oppgave ved oppfinnelsen å stille til rådighet en farmasøytisk aktiv forbindelse som ikke har noen innvirkninger på en normal tarmperistaltikk, men som likevel bevirker helbredelse av den inflammatoriske tarmsykdom.
Det ble nå funnet at forbindelsen N-metyl-N-[(1S)-1-fenyl-2-((3S)-3-hydroksypyrrolidin-l-yl)-etyl]-2,2-difenyl-acetamid og/eller dens fysiologisk akseptable salter er farma-søytisk aktive og egner seg som legemidler til behandling av inflammatoriske tarmsykdommer på en helt spesiell måte.
Som allerede anført ovenfor, er forbindelser med lignende strukturformer og deres fremstilling i og for seg kjent fra offentliggjørelsesskriftet DE 40 34 785. Deres terapeutiske aktivitet er derimot ny.
Forsøk har vist at forbindelsen som anvendes ifølge oppfinnelsen, virker ved "vridningstesten" på mus eller rotter (fremgangsmåte ifølge Siegmund et al, Proe Soc Biol 95,
(1957), 729-731). Den analgetiske virkning som sådan lar seg videre påvises i "haleslagstesten" på mus eller rotter (meto-dikk ifølge d'Amour og Smith, J Pharmacol Exp Ther 72, (1941), 74-79), videre ved "varmeplatetesten" (jf Schmauss og Yaksh, J Pharmacol Exp Ther 228, (1984), 1-12 og den der siterte litte-ratur ). Særlig sterke virkninger kan iakttas på rotter ved modellen med karrageninindusert hyperalgesi (jf Bartoszyk og Wild, Neuroscience Letters 101, (1989), 95). Forbindelsen utviser da ingen eller bare liten tilbøyelighet til fysisk av-hengighet .
Dessuten ble det, gjennom tilsvarende forsøk gjennom-ført ved vanlige metoder, påvist utpregete anti-inflammatoriske, diuretiske, antikonvulsive og neurobeskyttende virkninger. Forbindelsen utviser en høy affinitet med hensyn til bindingsforholdet til kappa-reseptorer.
Forbindelsen er, i motsetning til andre forbindelser med lignende virkningsspekter, særlig egnet for anvendelse i farmasøytiske preparater til behandling av inflammatoriske tarmsykdommer, ettersom den ved siden av den analgetiske eller antiinflammatoriske virkning er egnet til å normalisere for-styrrelser i tarmmotorikken som er fremkalt gjennom sykdommen. Særlig er den egnet til å bringe tarmbevegelsene i gang igjen når en tarmlukning truer eller allerede har inntrådt gjennom den inflammatoriske tarmsykdom. Denne virkningen kan også anvendes til behandling av en postoperativ tarmslyng og de derved forbundne smerter.
Som særlig fordelaktig har det ved forbindelsen
anvendt ifølge oppfinnelsen dessuten vist seg at den på grunn av sin struktur åpenbart kan passere blod-hjerne-barrieren og således ikke oppviser noe avhengighetspotensiale. Det ble tidligere heller ikke funnet noen virkninger som på noen måte ville begrense nytten av de fordelaktige virkningene for den krevde indikasjon.
Forbindelsen og dens fysiologisk akseptable salter kan således anvendes til fremstilling av farmasøytiske preparater ved at man bringer den sammen med minst ett bærer- eller hjelpestoff, og om ønsket med en eller flere ytterligere aktive forbindelser i den egnede doseringsform. De således erholdte preparater kan anvendes som legemidler innen human-eller veterinærmedisin. Som bærerstoff kommer det på tale med organiske eller uorganiske stoffer som egner seg for enteral (f eks oral eller rektal) eller parenteral applikasjon, og som ikke reagerer med de nye forbindelsene, f eks vann, plante-oljer, benzylalkoholer, polyetylenglykoler, glyseroltriacetat og andre fettsyreglyserider, gelatin, soyalecitin, karbohydra-ter som laktose eller stivelse, magnesiumstearat, talkum eller cellulose.
For oral anvendelse tjener særlig tabletter, drasjeer, kapsler, siruper, safter eller dråper. Av interesse er spesielt lakktabletter og kapsler med magesaftresistente belegg, henholdsvis kapselhylser. For rektal anvendelse tjener suppositorier og for parenteral applikasjon oppløsninger, fortrinnsvis olje- eller vannoppløsninger, videre suspensjoner, emulsjoner eller implantater.
Den aktive forbindelse som anvendes ifølge oppfinnelsen, kan også lyofiliseres og det erholdte lyofilisat anvendes f eks til fremstilling av injeksjonspreparater.
De angitte preparater kan være sterilisert og/eller inneholde hjelpestoffer som konserverings-, stabiliserings-og/eller fuktemidler, emulgatorer, salter for påvirkning av det osmotiske trykk, bufferstoffer, farge- og/eller aroma-stoffer. De kan, om ønsket, inneholde en eller flere aktive forbindelser, f eks et eller flere vitaminer, diuretika eller antiflogistika.
Forbindelsen som anvendes ifølge oppfinnelsen, admin-istreres som regel analogt med andre kjente preparater som lar seg erholde i handelen for den krevde indikasjon, fortrinnsvis ved doseringer mellom ca 1 mg og 50 mg, særlig mellom 5 og 30 mg pr doseringsenhet. Den daglige dosering ligger fortrinnsvis mellom ca 0,02 og 20 mg/kg, særlig 0,2 og 0,4 mg/kg kroppsvekt.
Den spesielle dose for hver enkelt pasient avhenger imidlertid av de forskjelligste faktorer, f eks av aktiviteten til den spesielle forbindelse som anvendes, av alderen, kroppsvekten, den generelle helsetilstand, arv, av kosten, av administreringstidspunkt og -vei, av utskillingshastigheten, legemiddelkombinasjonen og av alvorligheten av den enkelte sykdom som behandlingen gjelder for. Den orale applikasjon er foretrukket.
I det følgende er det gitt eksempler som tjener til belysning av oppfinnelsen.
Nedenunder er alle temperaturer angitt i °C.
Eksempel 1
N- metvl - N- T ( IS) - 1 - f envl - 2 - (( 3S) - 3 - hvdroksvpvrrolidin- 1 - yl ) etvll - 2 . 2- difenylacetamidhvdroklorid
I en 500 ml-apparatur ble 22 g (2S)-2-N-karboksyetyl-2-fenylglycin-N,N-[(3S)-3-hydroksy-tetrametylamid] plassert og oppløst i 150 ml tetrahydrofuran. Under omrøring ble det tildryppet en oppløsning bestående av 150 ml tetrahydrofuran og 24,1 g difenylacetylklorid ved 10-20 °C i løpet av en time, idet det til å begynne med ble dannet et bunnfall som imidlertid igjen gikk i oppløsning i løpet av reaksjonen. Mot slutten av reaksjonen ble det på nytt dannet et bunnfall. Det ble omrørt i ytterligere 12 timer ved romtemperatur. Deretter ble det avkjølt til ca 5 "C og det utfelte produkt ble frafiltrert under sug. Det fraskilte produkt ble ettervasket med ca 100 ml tetrahydrofuran og tørket. På denne måte ble det erholdt 39 g råprodukt. Dette ble omkrystallisert med 250 ml etanol og 1 g aktivt karbon.
Utbytte 33 g (73,2 % av det teoretiske).
Den farmasøytiske aktivitet av forbindelsen som anvendes ifølge oppfinnelsen, ved behandlingen av inflammatoriske tarmsykdommer, ble undersøkt etter den metode som er beskrevet i European J of Pharmacology 271 (1994) 245-251. Virkningen av perifert virkende kappa-agonister på aktiviteten til nervene som tar seg av tykktarmen, samt på den friske og også den betente tykktarmen, ble undersøkt. For dette formål ble svarene på sykliske trykkendringer registrert for i alt 14 sensoriske nervetråder, idet det dreide seg om 8 C-tråder (langsomt ledende) og om 6 A-tråder (hurtig ledende).
Betennelser i tykktarmsområdet ble fremkalt gjennom applikasjon av trinitrobenzensulfonsyre. Målingene ble gjen-nomført 4 dager etter applikasjonen av dette stoffet.
Virkningen av N-metyl-N-[(IS)-l-fenyl-2-((3S)-3-hydroksypyrrolidin-l-yl )-etyl] -2, 2-difenylacetamidhydroklorid på nerveutladningen ble sammenlignet f eks med standardsammen-ligningsstoffet ICI 204 488. N-metyl-N-[(IS)-l-fenyl-2-((3S )-3-hydroksypyrrolidin-l-yl )-etyl] -2,2-difenylacetamidhydro-klorid hemmet på doseavhengig måte nervesvaret på de sykliske trykkøkningene. I ikke betent tykktarm ble svaret etter til-setning av totalt 32 mg/kg hemmet med 75,4 %. I betent tarm hemmet denne dosen virkningen til og med 99,8 %. ED50-verdien for N-metyl-N- [ (IS ) -1-fenyl-2- ((3S) -hydroksypyrrolidin-l-yl) - etyl]-2,2-difenylacetamidhydroklorid utgjorde 13 +. 4 mg/kg i ikke betent tykktarm. ICI 204 488 var derimot til og med etter applikasjonen med i alt 32 mg/kg uten virkning.
Den hemmende virkning av N-metyl-N-[(IS)-l-fenyl-2-((3S)-3-hydroksypyrrolidin-l-yl)-etyl] -2, 2-difenylacetamid-hydroklorid ble således åpenbart fremkalt gjennom aktiveringen av kappa-opioidreseptorer på de sensoriske nerveendene, mens ICI 204 488 ikke oppviste noen virkning på disse sensorene.
De etterfølgende eksemplene vedrører farmasøytiske preparater:
Eksempel A: Injeksjonsglass
En oppløsning av 100 g av en aktiv forbindelse med formel I og 5 g dinatriumhydrogenfosfat ble innstilt på pH 6,5 med 2 n saltsyre i 3 1 dobbeltdestillert vann, sterilfiltrert, fylt i injeksjonsglass, lyofilisert under sterile betingelser og lukket sterilt. Hvert injeksjonsglass inneholdt 5 mg aktiv forbindelse.
Eksempel B: Suppositorier
En blanding av 20 g av en aktiv forbindelse med formel I og 100 g soyalecitin ble smeltet sammen med 1400 g kakaosmør, helt i former og fikk avkjøles. Hvert suppositorium inneholdt 20 mg aktiv forbindelse.
Eksempel C: Oppløsning
Det ble laget en oppløsning av 1 g av en aktiv forbindelse med formel I, 9,38 g NaH2P04-2H20, 28 , 48 g NA2HP04-12H20 og 0,1 g benzalkoniumklorid i 940 ml dobbeltdestillert vann. Det ble innstilt på pH 6,8, fylt opp til 1 1 og sterilisert under bestråling.
Eksempel D: Salve
500 mg aktiv forbindelse med formel I ble blandet med 99,5 g vaselin under aseptiske betingelser.
Eksempel E: Tabletter
En blanding av 1 kg aktiv forbindelse med formel I,
4 kg laktose, 1,2 kg potetstivelse, 0,2 kg talkum og 0,1 kg magnesiumstearat ble presset på vanlig måte til tabletter slik at hver tablett inneholdt 10 mg aktiv forbindelse.
Eksempel F; Drasjeer
Analogt med eksempel E ble det presset tabletter som deretter ble belagt på vanlig måte med et belegg av sakkarosé, potetstivelse, talkum, tragant og fargestoff.
Eksempel G; Kapsler
2 kg aktiv forbindelse med formel I ble fylt på vanlig måte i hardgelatinkapsler, slik at hver kapsel inneholdt 20 mg aktiv forbindelse.
Eksempel H; Ampuller
En oppløsning av 1 kg aktiv forbindelse med formel I i 60 1 dobbeltdestillert vann ble sterilfiltrert og fylt i ampuller, lyofilisert under sterile betingelser og lukket sterilt. Hver ampulle inneholdt 10 ml aktiv forbindelse.

- Anvendelse av forbindelsen N-metyl-N-[(IS)-l-fenyl-2-((3S)-3-hydroksypyrrolidin-l-yl)-etyl]-2,2-difenylacetamid samt fysiologisk akseptable salter derav, til fremstilling av legemiddelpreparater for behandling av inflammatoriske tarmsykdommer .
