Description
Foreliggende oppfinnelse vedrører anvendelse av 5-halo-2-pyrimidin-deoksyribose eller 5-halo-pyrimidinon for fremstilling av et medikament for behandling av kreft.
Iododeoksyuridin (IUdR) ble syntetisert som et anti-neo-plastisk middel i 1959 av Prusoff (Prusoff; W.H., (1959), Biochem. Biophys. Acta, 32, 295-296) og var den første thymidinanalogen som klinisk ble anvendt som et anti-herpes-middel (Kaufman, H.E., Martola, E.L. og Dohlman, C, (1962), Archs. Ophthalmol., 68, 235-239). Toksisitetene forbundet med IUdR når det ble anvendt systemisk, begrenset dens kliniske anvendelse. IUdR ble også erkjent å være et potensielt klinisk radiosensitiseringsmiddel for kjemoterapi ved kreft (Kinsella, T.J., Mitchell, J.B., Russo, A., Morstyn, G. og Glatstein, E., (1984), J. Radiation Oncology Biol. Phys., 10, 1399-1406). Graden av radiosensitisering er direkte avhengig av mengden thymidinfortrengning i DNA ved denne analogen (Speth, P.A.J., Kinsella, T.J., Chang, A.E. , Klecker, R.V/., Belanger, K. og Collins, J.M., (1988), Clin. Pharmacol. Ther., 44. 369-375). Intraheptisk infusjon av IUdr etterfulgt av stråling for behandlingen av tumorceller i lever har hatt et visst hell (Remick, S.C., Benso III, A.B., Weese, J.L., Willson, J.K.V., Tutsch, K.D., Fischer, P.H. og Trump, D.L.,
(1989), Cancer Res., 49. 6437-6442).
I et forsøk på å utvikle selektive anti-herpes-simpleksvirus (HSV) midler basert på det brede spekteret av substrat spesifisitet av thymidinkinase av herpes-simpleksvirus sammenlignet med den humane thymidinkinase ble 5-iodo-2-pyrimidinon-deoksyribose (IPdR) - som adskiller seg fra IUdR ved et dobbeltbundet oksygen ved 4-posisjonen av basen-syntetisert. IPdR ble funnet å ha potent aktivitet mot HSV-1 og HSV-2 i cellekultur og mot HSV-2 i mus (Lewandowski, G.A., Grill, S.P., Fisher, M.H., Dutschman, G.E. , Efange, S.M. , Bardos, T.J. og Cheng, Y.C., (1989), Antimicrob. Agents Chemother., 33, 340-344). Dette midlet var ikke toksisk for uinfiserte celler og heller ikke for mus når det ble tilført oralt ved den anvendte dosen (Lewandowski, G.A., Grill, S.P., Fisher, M.H., Dutschman, G.E., Efange, S.M., Bardos, T.J. og Cheng, Y.C., (1989), Antimicrob. Agents Chemother., 33, 340-344 ). Siden IPdR og IUdR er strukturelt beslektet, ble den mulige omdanningen av IPdR til IUdR undersøkt. Det var tidligere vist at IPdR ikke kunne omdannes til IUdR ved hjelp av xantinoksydase (Lewandowski, G.A., Grill, S.P., Fisher, M.H., Dutschman, G.E., Efange, S.M., Bardos, T.J. og Cheng, Y.C., (1989), Antimicrob. Agents Chemother., 32, 340-344).
US-patent 4.895.937 beskriver nukleosidet l-(2-deoksy-g<->D-ribofuranosyl)-5-(iod)-2-pyrimidinon (IPdR) for anvendelse som et middel mot herpesviruser, f.eks. HSV-2. Hele innholdet av US-patent 4.895.937 er innbefattet heri som referanse.

Foreliggende oppfinnelse angår anvendelse av 5-halo-2-pyrimidin-deoksyribose eller 5-halo-2-pyrimidinon for fremstilling av et medikament for behandling av kreft.
Oppfinnelsen angår særlig anvendelse av en 5-halogen-substituert pyrimidin-deoksyribose for fremstilling av et medikament for behandling av leverkreft.
Figur 1 viser HPLC-profiler for omdanningen av IPdR til IUdR
ved leverhomogenat. IPdR ble inkubert med
Totteleverhomogenat. For kontrollreaksjoner ble en porsjon av supernatanten kokt i 5 minutter for å inaktivere alle enzymer før anvendelse. Analyse-betingelsene var som beskrevet nedenfor bortsett fra at 60 jjI av et 100.000 g x 60 minutt supernatanten (ekvivalent med ca. 0,5 mg protein/ml) ble anvendt i et reaksjonsvolum på 1500 pl. Porsjoner (300 pl) ble fjernet ved forskjellige tidspunkter (0 minutter, 15 minutter og 30 minutter fra henholdsvis (1) til (3)) under inkuberingsperioden ved 37°C. Retensjonstidene var henholdsvis ca. 9,5 minutter for IPdR(A), 8,0 minutter for IUdR(B) og 6,5 minutter for IU(C).
Figur 2 viser overlevelsesraten for mus tilført 100 mg/kg
daglige orale doser av FU og FP i 5 dager.
Figur 3 viser overlevelsesraten for mus behandlet med
varierende doser av FU og FP tilført oralt.
Figur 4 viser overlevelsesraten for mus på forhånd injisert med leukemiceller og senere administrert i varierende konsentrasjoner av FU og FP tilført oralt. Figur 5 viser endringen i vekt av en tykktarmstumor behandlet med varierende doser av FU og FP tilført oralt over tid. Alle vekttall er gitt sammenlignet med en innledende kontroll. Figur 6 viser den relativt lave IPdR-inkorporeringen i vev av athymisk nakenmus i benmarg og tarm, hvori thymidin-fortrenging flater ut ved ca. 250 mg/kg/d. Figur 7 angir sammenlignende IPdR fortrengning i lever og tumorvev av athymisk nakenmus mellom levervev og metaboliske tumorer, hvor lever-inkorporering var neglisjerbar sammenlignet med økende tumor-inkorporering. Figur 8 viser resultater av FP-behandling av transgeniske hunnmus som uttrykker SV40 stort tumorantigen og utvikler forskjellige krefttyper, hvor FP-behandlede mus levde lenge og hadde en langsommere økning i kroppsvekt enn et kontrolldyr. Figur 9 viser resultater av FP-behandling av transgeniske hannmus som uttrykker SV40 stort tumorantigen og utvikler forskjellige kreftformer, hvor FP-behandlede mus hadde en langsommere økning i kroppsvekt, hvilket indikerer lengre overlevelse enn kontrolldyr.
Foreliggende oppfinnelse er basert på den oppdagelse at promedikamenter aktiveres ved hjelp av leveraldehydoksydase in vitro og in vivo og blir aktive legemidler eller metabolitter for å oppnå en høy selektivitet og terapeutisk indeks. Det ovenfor angitte kan angis ved følgende reaksjonsskjerna: Hepatisk aldehydoksydase er utbredt blant pattedyrspecies. Dette enzymet katalyserer oksydasjonen av en rekke alifatiske og aromatiske aldehyder, så vel som et antall ikke-aldehyd-heterocykliske forbindelser såsom N<1->metylnikotinamid, 4—amino-antifolater og metotreksat og dets analoger. Funnet av at oksydasjonen av 5—substituerte pyrimidinoner til deres uracil- eller uridinmotstykker ved human eller rottealdehyd-oksydase har resultert i en fullstendig ny kategori av substrater som virkes på av dette enzymet. Denne oppdagelsen tillater også utformingen av legemidler, som metaboliseres i leveren, som kan undertrykke og ødelegge kreftceller, viruser, parasitter og andre uønskede mikrobielle patogener.
Farmasøytisk akseptable salter av de ovenfor omtalte analoger innbefatter de som er avledet fra farmasøytisk akseptable uorganiske syrer og baser. Eksempler på egnede syrer innbefatter saltsyre, hydrobromsyre, svovelsyre, salpeter-syre, perklorsyre, fumarsyre, maleinsyre, fosforsyre, glykolsyre, melkesyre, salicylsyre, ravsyre, toluen-p-sulfonsyre, vinsyre, eddiksyre, sitronsyre, metansulfonsyre, maursyre, benzosyre, malonsyre, naftalen-2-sulfonsyre og benzensulfonsyre.
Slik betegnelsen her benyttes innbefatter betegnelsen "analog" (eller "aktiv bestanddel") analogen selv, så vel som en ester eller et salt derav.
Salter avledet fra egnede baser innbefatter alkalimetall (f.eks. natrium), jordalkalimetall (f.eks. magnesium), ammonium og NR4<+> (hvor R er C^^-alkyl )-salter.
Terapeutisk behandling av lever-forbundede sykdommer er en spesielt foretrukket anvendelse av de tidligere nevnte promedikamentene på grunn av det faktum at den hepatiske aldehydoksydasen blir funnet i det vesentlige utelukkende i leveren. Følgelig er det en spesiell fordel ved disse promedikamentene ved elimineringen og reduksjonen av bivirkninger som ellers ville forårsakes ved den systemiske administreringen av den bio-påvirkende forbindelsen når det er ønsket å ha dens effekt bare i leveren. Mange forbindelser, såsom IUdR, som er nyttige ved behandling av prolifererende celler, har betydelig systemisk toksisitet som har begrenset deres kliniske anvendelse. Det tilsvarende promedikamentet vil generelt ikke ha den samme aktiviteten som den bio-påvirkende forbindelsen. Følgelig er slike promedikamenter generelt ikke-toksiske, bortsett fra når de metaboliseres til den ønskede bio-påvirkende forbindelsen. F.eks. er de 5-substituerte pyrimidinon-forstadiene for IUdR eller FU ikke substrater for human thymidinkinase og thymidinfosforylase og er i det vesentlige ikke-toksiske. Når de anvendes for behandlingen av hepatisk karsinom, f.eks. vil IPdR metaboliseres i leveren ved den hepatiske aldehydoksydasen til IUdR som deretter fortrinnsvis opptas av tumor-cellene i leveren før vesentlig spredning av IUdR til annet vev kan finne sted. Følgelig vil den terapeutiske indeksen for de 5-substituerte PdR-forbindelsene for primær leverkreft eller metastatisk leverkreft, være langt bedre enn deres UdR-motstykker.
Det synes andre tilfeller ved siden av behandlingen av leverforbundede sykdommer når administreringen av et promedikament vil være foretrukket fremfor administreringen av den bio-påvirkende forbindelsen som dannes fra slike promedikamenter. F.eks. er mange legemidler ikke oralt administrerbare av forskjellige grunner. Oral administrering av et promedikament hvorfra den aktive forbindelsen frigjøres i leveren vil unngå de fleste av ulempene ved oral administrering av den aktive forbindelsen. Følgelig er f.eks. FU et velkjent anti-kreftmedikament som ikke kan administreres oralt. FP er et promedikament som er metaboliserbart ved hepatisk aldehydoksydase til FU. FP kan tilføres oralt til et behandlingsobjekt. Som vist i eksemplene nedenfor har FP demonstrert terapeutisk effektivitet ved leukemi og tarm-kreft. Effektiviteten av FP er i det vesentlige den samme som for FU, hvilke indikerer effektiv avlevering av FU fra hepatocyttene til tumoren.
I dag utforskes IUdR og BUdR som radiosensitiserende midler. Imidlertid er deres effektive anvendelse begrenset av cytotoksisiteten og den raske katabolismen til den frie basen, etterfulgt av dehalogenering (Speth, P.A.J., Kinsella, T.J., Chang, A.E., Klecker, R.W., Belanger, K. og Collins, J.M., (1988), Clin. Pharmacol. Ther., 44. 369-375). Siden IPdR og dets analoger er i det vesentlige ikke-toksiske, og ikke er substrater for thymidinfosforylase, vil anvendelsen av IPdR og dets analoger istedenfor IUdR og BUdR omgå vanskelighetene med toksisitet og nedbrytning forbundet med strålingsterapi.
"Lever-forbundede sykdommer" innbefatter viral hepatitt, f.eks. hepatitt A, hepatitt B, hepatitt C og hepatitt D; hepatomer; kreftformer som er metastasert til og fra leveren; infeksjon med cytomegalovirus eller andre viruser, para-sittiske infeksjoner, f.eks. Schistosomiasis, Clonorchiasis, Fascioliasis, Opisthorchiasis; og infeksjoner forårsaket av et assortiment av ikter og bendelormer, mikrobielle midler, f.eks. sopp eller bakterielle infeksjoner, såsom Paracocci-dioides brasiliensis og andre leversykdommer, såsom cirrhose i leveren, eller avvisning av levertransplanter.
Metaboliske tilstander eller sykdommer kan også behandles ved selektiv anvendelse av et promedikament som er metaboliserbart til en aktiv forbindelse ved den hepatiske aldehydoksydasen. Legemidlet kan være de som er målrettet til reseptorer i eller på leverceller så vel som ikke-leverceller er også av verdi. Disse legemidlene kan også ha enten fullstendig eller delvis agonist- eller antagonistaktivitet. En lang rekke reseptor-virkende legemidler kan dannes ved administrering av det tilsvarende promedikamentet til et behandlingsobjekt eller til et preparat av aldehydoksydasen in vitro. Følgelig kan promedikamenter av legemidler som virker ved fjerne seter såsom hjerte eller hjernen, også anvendes ifølge foreliggende oppfinnelse. Dersom videre leverceller mangler målresep-torene, reduseres potensialet for uønsket toksisitet på grunn av høye leverkonsentrasjoner. Anvendelsen av promedikamenter som er metaboliserbare ved hjelp av et leverenzym til aktive legemidler har stor anvendelighet.
Avhengig av nivået av enzymaktivitet av aldehydoksydasen på et promedikament, kan det oppnås en lav hastighet for dannelse eller langsom frigjøring av aktivt legemiddel i kroppen. Selv om forbindelsene som her er eksemplifisert metaboliseres raskt, kan de modifiseres for å retardere metabolismen. Dette skulle resultere i et behov for mindre hyppig doseringer av promedikament sammenlignet med medi-kamentet, med alle de kjente fordelene i form av pasient compliance og konstant dose. Dersom metabolismen til det aktive legemidlet er tilstrekkelig langsom, kan helt nye klasser av forbindelser anvendes terapeutisk som hittil ikke har kunnet anvendes på grunn av toksisitetsproblemer med doser i store piller. Det fremgår f.eks. fra tabell V nedenfor at naturen av R-gruppen på pyrimidinonsubstratet i vesentlig grad påvirker omdannelsesraten. Det er følgelig åpenbart at promedikamenter kan utformes med en på forhånd bestemt metabolismerate.
Et eksempel på et legemiddel som kunne forbedres ved langsom omdanning in vivo er ketoner som inneholder legemidlets suramin. Den effektive anti-kreftdosen for metastatisk prostatakreft er meget nær dosen som resulterer i paralyse, hvilket uheldigvis og uventet ble oppdaget ved forsøk på mennesker. Stor pilledose ble funnet å være betydelig mindre effektiv. Som en følge hospitaliseres pasienter som har behov for slik behandling og overvåkes kontinuerlig og infuseres for å opprettholde et trangt område av effektive og akseptable konsentrasjoner. Ved å anvende et aldehyd eller annen promedikament i form av suramin metaboli serbar ved aldehydoksydase til aktivt suramin, bør det være mulig å gi pasienten stor pilledose uten et behov for konstant hospita-lisering.
Som angitt ovenfor kan de oralt akseptable promedikamentene anvendes istedenfor deres aktive produkt som ikke er oralt akseptabelt. Et promedikament kan atskille seg fra det aktive produktet ved forøkt stabilitet i sure betingelser, motstand mot nedbrytende enzymer, bedre adsorpsjon eller mindre irritasjon eller toksisitet for fordøyelsessystemet. Når det er adsorbert, omdannes promedikamentet til det aktive legemidlet ved leveraldehydoksydase, derved unngås for-døyelseskanalen. Fordelene ved oral mot parenteral administrering fremgår lett for fagmannen.
Uavhengig av administreringsmåten har et hvilket som helst legemiddel halveringstidbegrensninger på grunn av renal clearance, enzymatisk nedbrytning og for mye eller for lite binding til serumproteiner og lignende. Anvendelsen av promedikamenter utvider mulighetene for utvikling av legemidlet ved å kompensere for slike problemer.
Mengden av analogen beskrevet ovenfor for anvendelse i foreliggende oppfinnelse vil variere ikke bare med den spesielle forbindelsen som velges, men også med administreringsmåten, naturen av tilstanden som behandles og alderen og tilstanden for pasienten og vil i siste instans bestemmes av den behandlende lege eller veterinærs skjønn. Generelt vil imidlertid en egnet dose være i området fra ca. 1 til ca. 100 mg/kg kroppsvekt pr. dag, fortrinnsvis ca. 2 til ca. 50 mg/kg kroppsvekt pr. dag, mest foretrukket 2 til 10 mg/kg/dag.
Den ønskede dosen kan hensiktsmessig presenteres i en enkeltdose eller som oppdelte doser administrert ved egnede intervaller, f.eks. ved 2, 3, 4, eller flere underdoser pr. dag.
Analogen som beskrevet ovenfor administreres hensiktsmessig i enhetsdose; f.eks. inneholdende 0,5 til 50 mg, fortrinnsvis 20 til 1000 mg, mest foretrukket 50 til 700 mg aktiv bestanddel pr. enhetsdoseform.
Ideelt bør den aktive bestanddelen administreres for å oppnå topp plasmakonsentrasjoner av den aktive bestanddelen på fra ca. 1 til 75 uM, fortrinnsvis ca. 2 til 50 pM, mest foretrukket ca. 3 til ca. 30 jjM. Dette kan f.eks. oppnås ved intravenøs injeksjon av 0,1 til 5% oppløsning av promedikamentet, eventuelt i saltvannsoppløsning, eller administreres som en stor pille inneholdende ca 0,1 til 50 mg/kg av den aktive bestanddelen.
Det vil være åpenbart at forskjellige promedikamenter kan kreve s°vært forskjellige doser. Videre kan behandlingen av sykdommer av forskjellig vev eller organer også kreve forskjellige doser. Disse dosene kan lett bestemmes av en av vanlig kunnskap innen teknikken ved å anvende kjente fremgangsmåter.
Selv om det er mulig at analogen beskrevet ovenfor kan anvendes ved behandling og kan administreres som råkjemi-kalie, er det foretrukket å presentere promedikamentet i forbindelse med en farmasøytisk akseptabel bærer som et farmasøytisk preparat. Bæreren(e) må være "akseptable" i den forstand at de er kompatible med de andre bestanddelene av preparatet og ikke bryter ned mottageren.
Farmasøytiske preparater innbefatter de som er egnede for oral, rektal, nasal, topisk (innbefattende bukal, sub-lingual og transdermal), vaginal eller parenteral (innbefattende intramuskulær, subkutan og intravenøs) administrering eller i en form egnet for administrering ved inhalering eller insufflasjon. Preparatene kan, der hvor det er hensiktsmessig, presenteres i atskilte doseenheter og kan fremstilles ved en hvilken som helst av fremgangsmåtene som er velkjente innen farmasien. Alle fremgangsmåter innbefatter trinnet med å bringe i forbindelse den aktive bestanddelen med flytende bærere eller finfordelte faste bærere eller begge og deretter, om nødvendig, forme produktet til det ønskede preparatet. Farmasøytiske preparater som er egnede for oral administrering kan hensiktsmessig presenteres som atskilte enheter såsom kapsler, drops eller tabletter hver inneholdende en på forhånd bestemt mengde av den aktive bestanddelen; som et pulver eller korn; som en oppløsning; som en suspensjon eller som en emulsjon. Den aktive bestanddelen kan også være presentert som en stor pille, "electuary" eller pasta. Tabletter og kapsler for oral administrering kan inneholde konvensjonelle hjelpestoffer såsom bindemidler, fyllstoffer, smøremidler, sprengmidler eller fuktemidler. Tablettene kan være belagt i henhold til velkjente fremgangsmåter innen teknikken. Orale, flytende preparater kan foreligge i form av f.eks. vandige eller oljeformige suspensjoner, oppløs-ninger, emulsjoner, siruper eller eliksirer, eller kan være til stede som et tørt produkt for oppbygning med vann eller annen egnet bærer før anvendelse. Slike flytende preparater kan inneholde konvensjonelle additiver såsom suspensjonsmidler, emulgeringsmidler, ikke-vandige bærere (som kan innbefatte spiselige oljer) eller konserveringsmidler.
Den aktive bestanddelen kan også formuleres for parenteral administrering (f.eks. ved injeksjon, f.eks. bolusinjeksjon eller kontinuerlig infusjon) og kan være til stede i enhetsdoseform i ampuller, på forhånd fylte sprøyter, små volum-infusjons- eller flerdosebeholdere med et tilsatt konserveringsmiddel. Preparatene kan anta slike former som suspensjoner, oppløsninger eller emulsjoner i oljeformige eller vandige bærere, og kan inneholde formuleringsmidler såsom suspensjons-, stabiliserings- og/eller dispergerings-midler. Alternativt kan den aktive bestanddelen foreligge i pulverform, oppnådd ved aseptisk isolering av sterilt, fast stoff eller ved lyofilisering fra oppløsning, for oppbygning med en egnet bærer, f.eks. sterilt, pyrogenfritt vann før anvendelse.
Farmasøytiske preparater egnede for rektal administrering, hvor bæreren er et fast stoff, presenteres mest hensiktsmessig som enhetdosesuppositorier. Egnede bærere innbefatter kakaosmør og andre materialer som er vanlig anvendte innen teknikken; og suppositoriene kan hensiktsmessig dannes ved blanding av den aktive forbindelsen med den eller de myknede eller smeltede bærerne etterfulgt av avkjøling og forming i former.
Preparater egnede for vaginal administrering kan foreligge som pessarer, tamponger, kremer, geler, pastaer, skum eller sprayer inneholdende i tillegg til den aktive bestanddelen, slike bærere som er kjent innen teknikken som hensikts-messige .
For intra-nasal administrering kan den aktive bestanddelen anvendes som en flytende spray eller et dispergerbart pulver eller i form av dråper.
Dråper kan formuleres med en vandig eller ikke-vandig basis innbefattende et eller flere dispersjonsmidler, oppløselig-gjørende midler eller suspensjonsmidler. Flytende sprayer avleveres hensiktsmessig fra beholdere under trykk.
For administrering ved inhalering avleveres av den aktive bestanddelen hensiktsmessig fra en insufflator, en forstøver eller en pakke under trykk eller ved hjelp av en annen hensiktsmessig innretning for avlevering av en aerosolspray. Pakker under trykk kan innbefatte et egnet drivmiddel såsom diklordifluormetan, triklorfluormetan, diklortetrafluoretan, karbondioksyd eller annen egnet gass. I tilfellet med en aerosol under trykk kan doseringsenheten bestemmes ved å tilveiebringe en ventil som avleverer en utmålt mengde.
Alternativt kan den aktive bestanddelen, for administrering ved inhalering eller insufflering, anta formen av en tørr pulversammensetning, f.eks. en pulverblanding av forbindelsen og en egnet pulverbasis såsom laktose eller stivelse. Pulversammensetningen kan være til stede i enhetsdoseform i f.eks. kapsler eller patroner eller f.eks. gelatin eller blærepakker hvorfra pulveret kan administreres ved hjelp av en inhalator eller insufflator.
Når det er ønsket, kan de ovenfor omtalte formuleringene utformes for å gi vedvarende frigivelse av den aktive bestanddelen.

De farmasøytiske preparatene for anvendelse ifølge oppfinnelsen kan også inneholde andre aktive bestanddeler, såsom antimikrobielle midler eller konserveringsmidler.
Det farmasøytiske preparatet for anvendelse ifølge foreliggende oppfinnelse kan også inneholde inerte bestanddeler såsom tørkemidler, stoffer som letter håndteringen, farge-stoffer, smaksstoffer og belegg for enkel svelging.
Den aktive bestanddelen kan også anvendes i kombinasjon med andre terapeutiske midler, f.eks. andre anti-infeksjons-midler. Spesielt kan forbindelsene av formel (I) anvendes sammen med velkjente antivirale midler, f.eks. adenin-arabinosid eller interferon-a.
Når den aktive bestanddelen generert fra promedikamentet er et anti-kreftmiddel, kan andre anti-kreft- eller immuno-modulerende midler anvendes sammen, på samme måte som en hvilken som helst annen kompatibel kombinasjon av legemidler enten aktiviteten er synergistiske, komplementære eller separate.
Kombinasjonene referert til over kan hensiktsmessig foreligge for anvendelse i form av et farmasøytisk preparat.
De individuelle komponentene av slike kombinasjoner kan administreres enten trinnvis eller samtidig i separate eller kombinerte farmasøytiske preparater.
Når en analog som beskrevet ovenfor anvendes i kombinasjon med et andre terapeutisk middel for den samme sykdommen, f.eks. aktivt mot det samme viruset, kan dosen av hver forbindelse enten være den samme eller forskjellig fra den som anvendes når analogen anvendes alene. Den egnede dosen vil lett kunne fastslås av fagmannen.
Aldehydoksydasen renses fordelaktig fra leveren og kan være immobilisert på en fast fase for enkel separering av enzymkatalysator fra en reaksjonsblanding. De ønskede bio-påvirkende optisk aktive forbindelsene separeres deretter fra proforbindelsesforstadiene og utvinnes. Teknikken for enzymrensing er velkjente innen teknikken, en hvilken som helst egnet av disse kan anvendes. Teknikker for enzym-immobi 1 iser ing til en fast fase enten den er adsorbert, fanget, kjemisk bundet eller bevart bak en semipermeabel membran er også velkjente innen teknikken.
EKSEMPLER
Eksempel 1: Vevspreparering
Rottehepatiske vev ble vasket med iskald 1,15$ KC1 og tørket tørt. Vevet ble deretter homogenisert med en vevshomogeni-sator, i et volum på 1, 15% KC1 som var 3 ganger vevsvekten, for å danne et 25$ (vekt/volum) homogenat. Homogenatet ble deretter sentrifugert ved lO.OOOg i 10 minutter ved 4°C. Den resulterende supernatanten ble filtrert gjennom "Miracloth"
(tilsvarende osteklede), deretter dialysert over natten mot 50 mM Tris-HCl-buffer, pH 7,5 og lagret ved -80 ° C før anvendelse. Eksempet 2: Analysebetingelser
For standard analysebetingelsen inneholdt reaksjonsblandingen 50 mM Tris-HCl, pH 7,5, 1 mM EDTA, 180 pM IPdR (eller dens analoger) og ca. 0,01 mg protein av 10,000g supernatant av vevshomogenat i et endelig volum på 500 pl og inkuberingen var ved 37°C i 10 minutter med mindre annet er angitt. 300 pl av reaksjonsblandingen ble fjernet ved avslutningen av inkuberingen og blandet med 630 pl acetonitril, deretter omrørt. Det utfelte proteinet ble fjernet ved sentrifugering og supernatanten ble lyofilisert til tørrhet. Eksempel 3: Omdanning av 5- iodo- 2- pyrimidinon- 2'- deoksyribose ( IPdR) til 5- iodo- deoksvuridin ( IUdR) For å undersøke omdanningen av IPdR til IUdR ved leverenzym ble metabolitter av IPdR analysert etter inkubering med en supernatant av rotteleverhomogenat ved anvendelse av en reversfase-HPLC-teknikk. IPdR og IUdR kunne detekteres ved en absorpsjonsbølgelengde på 230 nm, men bare i IPdR kunne detekteres ved 335 nm. For å begrense den fosforolytiske spaltingen av IUdR til iodouracil (IU) ved thymidinfosforylase Kinsella, T.J., Mitchell, J.B., Russo, A., Morstyn, G. og Glatstein, E. , (1984 ), J. Radiation Oncology Biol. Phys.„ 10, 1399-1406) ble Tris-HCl-buffer anvendt i analysen. Som vist i figur 1 var det en tidsavhengig omdanning av IPdR til IUdR og IUdR synes å være det eneste produktet som ble produsert ved IPdR. Identifikasjonen av IUdR ble bekreftet basert på retensjonstiden på en C-18-kolonne (8 minutter mot 9,5 minutter for IPdR) og UV-spekteret, så vel som ved kjernemagnetisk resonansspektroskopi (resultater er ikke vist).
Eksempel 4: Egenskaper av " IPR- oksydase"- aktivitet
Denne "IPdR-oksydase"-aktiviteten krever ikke eksogene kofaktorer, er langt mindre aktivt i ekstrakter av nyre og milt enn lever (tabell I) og kan ikke detekteres i lunge-eller innvollsekstrakter fra rotter. Den humane leveren inneholder en tilsvarende mengde av dette enzymet. Ekstrakter av hepatocytter, som representerer ca. 80$ av cellepopula-sjonen i lever, hadde en meget lignende spesifikk aktivitet sammenlignet med hele leverekstraktet, hvilket antyder at IPdR-oksydasjonen hovedsakelig er til stede i denne cel.le-populasjonen. Differensiell fraksjoneringssentrifugering ble anvendt i et forsøk på å lokalisere denne IPdR-omdannings-enzymaktiviteten i leverhomogenatet. Enzymaktivitetsanalysen ble utført på hver fraksjon. Den eneste fraksjonen som viste enzymaktivitet kom til syne i lOO.OOOg x 60 minutter supernatanten; dette antydet at dette enzymet bare er lokalisert i den oppløselige fraksjonen av cytosol. Ytter-ligere rensing ble oppnådd ved å anvende DEAE-cellulose-kolonnekromatografi, blå Sepharose-kolonnekromatografi og glyserolgradient-sentrifugering i denne rekkefølgen. Denne IPdR-oksydase-aktiviteten ble renset 380 ganger med start fra de rå ekstraktene av rottelever. Det delvis rensede enzymet katalyserte IUdR-syntesen ved en rate på 3,8 umol pr. minutt pr. mg protein ved 37"C under disse betingelsene. Den tilsynelatende molekylvekten av dette enzymet i både rotter og mennesker, som bestemt ved sentrifugering på en 20 til 40$ glyserolgradient er ca. 280.000 dalton. Hverken kofaktor eller toverdig kationkrav er funnet for rotteleverenzymet eller det humane leverenzymet.
Eksempel 5: Identifikasjon av " IPdR- oksydase"
For å bestemme hvilket enzym som er ansvarlig for katalysa-torene av denne IPdR-oksydasen, ble serier av kjente oksido-reduktaser som har tilsvarende evne til å oksydere et karbonatom med en nabostilt aminogruppe til en karbonyl-funksjonalitet undersøkt med henblikk på deres evne til å katalysere IPdR-oksydasjon. Xantinoksydase (Boehringer Mannheim) isolert fra kumelk som katalyserer oksydasjonen av hypoxantin til xantin var ikke i stand til å omdanne IPdR til IUdR. Et blandet funksjon-oksydasesystem fremstilt fra mikrosomet av rottelever et bredt spektrum av substratet og er NADPH-avhengig. Imidlertid viste denne mikrosome fraksjonen fra rottelever ingen IPdR-oksydaseaktivitet uavhengig av om NADPH ble tilsatt eller ikke. Alkoholdehydrogenase (som kan oppnås fra Sigma) og alkoholoksydase (som kan oppnås fra Sigma) finnes begge i den oppløselige fraksjonen av lever-celleekstrakter, men omdanningen av IPdR til UdR ble ikke detektert med renset preparat av noe enzym under de samme betingelsene hvorunder de omdannet de naturlige substratene effektivt. Sarkosinoksydase (Boehringer Mannheim) som katalysérer omdanningen av N^-metylglysin til glysin hadde ingen IPdR-oksydase-aktivitet. Chem., 239, 2022-2035; Stanulovic, M. og Chaykin, S., (1971), Archs. of Biochem. and Biophy., 145, 35-42; Badway, J.A., Robinson, J.M., Karnovsky, M.J. og Karnovsky, M.L., (1981), J. Biol. Chem., 256. 3479-3486). Derfor ble en serie forbindelser undersøkt med henblikk på deres effekter på oksydasjonen av IPdR til IUdR og det ble funnet at inhiberingsprofilen for forbindelsene med "IPdR-oksydase"-aktivitet (tabell II) var i det vesentlige identisk med egenskapsmønsteret for aldehydoksydase. Videre kunne, under hvert trinn av rensingen, aldehydoksydase ikke separeres fra "IPdR-oksydase"-aktivitet.
Eksempel 6: Substratspesif isitet
Flere 2-pyrimidinondeoksyriboseanaloger ble undersøkt med henblikk på omdanningen til deres deoksyuridin-motstykker. Michaelis-konstanten Km for IPdR i reaksjonen ved pH 7,5 og pH 9,5 er henholdsvis 150 pM og 87 pM, og Km for 5-etynyl-2-pyrimidinondeoksyribose (EPdR) i reaksjonen ved pH 7,5 og pH 9,5 er henholdsvis 77 pM og 46 pM. Ikke desto mindre er den relative Vmaks. for IPdR i reaksjonen ved pH 7,5 og pH 9,5 den samme. 5-iodo-2-pyrimidinon (IP), aglykosen av IPdR var et utmerket substrat for aldehydoksydase. De syntetiske substratene for aldehydoksydasen synes å være bedre enn de naturlige substratene, N^-metylnikotinamid og acetaldehyd, bedømt ved potensen av inhibering av N-^-metylnikotinamid og acetaldehyd for IPdR-oksydasjonsreaksjonen (tabell IV). Raten for reaktivitet av leverenzymet med forskjellige IPdR-analoger følger rekkefølgen EPdR, IP, IPdR, 5-bromo-2-pyrimidinondeoksyribose (BDdR) og 5-metyl-2-pyrimidinon-deoksyribose (MPdR) eller 5-etyl-2-pyrimidinondeoksyribose (EtPdR) (tabell III). Elektronegative substituenter i 5-posisjonen synes å øke substrataktiviteten i denne oksyda-sj onsreaksj onen.
Eksempel 7: Toksisitet av FU og FP
BDFl-mus ble daglig administrert orale doser på enten 50, 75 eller 100 mg/kg av FU eller 100, 150 eller 200 mg/kg FP. Overlevelsesratene for hver er presentert i kurve- og grafisk form i tabell VI og figurer 2 og 3. Toksisiteten av FP var betydelig mindre, selv når høyere doser ble administrert.
Eksempel 8: Virkning av FU og FP på leukemiceller
Mus ble injisert med 100.000 P388-R-leukemiceller for å indusere en leukemi. Disse cellene var resistente overfor Adriamycin. Daglige behandlinger på 25 og 50 mg/kg FU og 50 og 100 mg/kg FP ble tilført oralt til disse leukemiske musene og varigheten av overlevelsen ble målt. Resultatene er gitt i tabell- og grafisk form i tabell V og figur 4. Overlevelses-tiden var like lang ved anvendelse av FP sammenlignet med FU. Eksempel 9: Virkninger av FU og FP på tykktarmskarslnomer Mus ble injisert med tykktarms-38-celler og enten ikke behandlet eller behandlet med daglige behandlinger av 25 og 50 mg/kg FU og 50 og 100 mg/kg FP som ble tilført oralt til musene og endringen i tumorvekt ble målt over tid. Dataene er angitt i figur 5. Reduksjonen i tumorstørrelse eller reduksjonen i dens vektrate er sammenlignbar for FU og FP.
Eksempel 10: Virkninger av IPdR- inkorporering i vev av
athvmiske nakenmus
Mus ble daglig oralt administrert doser på 0, 100, 250 og 500 mg/kg. Prosent dThd-fortrengning som indikasjon på IPdR-inkorporer ing i benmarg, tarm og levervev ble analysert ved kjente fremgangsmåter. Som angitt i figur 6 ble relativt små mengder av thymidin fortrengt ved IPdR i benmarg og tarm, med thymidin-fortrengningsplatåer som opptrådte ved behandlings-nivåer over 250 mg/kg/dag. Ingen inkorporering av IUdR i lever ble funnet. Resultatene angitt i figur 6 er gjennom-snittlig prosent thymidin-fortrengning ± standardfeil for gjennomsnittet. N er større eller lik 3 for hver dose. Som vist for disse resultatene fant det ikke sted noen betydelig thymidin-fortrengning ved IUdR i leveren, og en meget liten prosent fortrengning ble funnet i benmarg og tarm. Følgelig fastslår disse resultatene at administreringen av IPdR anvendt som et promedikament ifølge foreliggende oppfinnelse bør være egnet for behandling av pattedyr, innbefattende mennesker.
Eksempel 11; IPdR- inkorporering i vev av athymisk nakenmus
som har metastatlske tumorer
Mus ble daglig administrert doser på 0, 100, 250 og 500 mg/kg/dag for å bestemme thymidinfortrengning i lever og tumorvev. Resultatene angitt i figur 7 er angitt som gjennom-snittlig prosent thymidinfortrengning ± et standardavvik for gjennomsnittet. N > 3 (kontrolltumorer N = 2). Mens en meget lav prosent inkorporering ble detektert i normal lever, ble det vist en økende prosent fortrengning i tumoren, følgelig demonstreres at anvendelsen av IPdR som et promedikament for tumorbehandling ventes å gi gode resultater.
Eksempel 12: Behandling av transgeniske mus med FP
Transgeniske mus oppnådd i henhold til fremgangsmåten til Sepulveda et al., Cancer Research, 49:6108-6117 (1989) ble administrert ved begynnelse 9 uker etter fødsel i en hunn og en hann, hvor behandlingsgruppen mottok 100 mg/kg to ganger pr. dag, en gang pr. uke. Figurer 8 og 9 viser endringene i kroppsvekt over uker 9-17 for henholdsvis hunn- og hannmus. Både behandlede hannmus og hunnmus viste en betydelig redusert vektøkning etter uke 13, hvilket korrelerer med økede overlevelsestider. Den behandlede hunnrotten overlevde kontrollrotten som døde av kreftkomplikasjoner ved uke 16. Følgelig antydet dataene ovenfor at anvendelsen av FP som en tumorbehandling ifølge foreliggende oppfinnelse ventes å gi gode resultater.
