Description
Foreliggende oppfinnelse angår en analogifremgangsmåte for fremstilling av terapeutisk aktive forbindelser, idet slike forbindelser generelt er farmakologisk anvendbare som anti-blodplateaggregeringsmidler i forskjellige vaskulære patologier. De foran nevnte, farmakologiske aktiviteter er anvendbare ved behandling av pattedyr. Mere spesielt virker forbindelsene fremstilt ifølge foreliggende oppfinnelse ved å blokkere proteinfibrinogenets blodplatereseptorsete. Fibrinogen er et glykoprotein som sirkuleres i blodplasmaer, og hvis blodplatereseptorsete er glykoprotein Ilb/IIIa. Ved å blokkere virkningen av fibrinogen ved reseptoren (glykoprotein Ilb/IIIa) innvirker forbindelsene fremstilt ifølge foreliggende oppfinnelse på blodplateaggregering, hvilken er en årsak til mange vaskulære patologier. På nåværende tidspunkt er det et behov på området for vaskulære terapeutika, for et slikt fibrinogenreseptorblokkerende middel. Ved innvirkning på hemostase ville en slik terapi minske sykeligheten og dødeligheten av trombotisk sykdom.
Hemostase er den spontane prosess for stansing av blødning fra ødelagte blodkar. Prekapillære kar trekker seg umiddelbart sammen når de overskjæres. I løpet av sekunder bindes thrombocytter, eller blodplater, til det skadede kars eksponerte matriks ved en prosess som kalles blodplateadhe-sjon. Blodplatene kleber også til hverandre i et fenomen som er kjent som blodplateaggregering for å danne en blodplateplugg. Denne blodplateplugg kan stanse blødningen raskt, men den må forsterkes med proteinfibrin for langvarig effektivitet inntil blodkarets skade permanent kan repareres ved vekst av fibroblaster, som er spesialiserte vevreparasjonsceller.
En intravaskulær trombe (klump) oppstår ved en pato-logisk forstyrrelse av hemostase. Tromben kan vokse til en tilstrekkelig størrelse til å blokkere arterielle blodkar. Tromber kan også dannes i områder med stase eller langsom blodstrøm i vener. Venetromber kan lett avgi deler av seg selv kalt emboli som beveger seg gjennom sirkulasjonssystemet og kan resultere i blokade av andre kar, som for eksempel lunge-arterier. Arterietromber kan således forårsake alvorlige sykdommer ved lokal blokade, mens venetromber primært gjør dette ved fjerntliggende blokade, eller embolisering. Disse sykdommer omfatter venøs trombose, tromboflebitt, arteriell emboli, koronar og cerebral, arteriell trombose, myocardialt infarkt, slag, cerebral emboli, nyreemboli og lungeemboli.
Det er et behov på området cardiovaskulære og cerebrovaskulære terapeutika for et middel som kan anvendes for å hindre og behandle tromber, med minimale bivirkninger, inkludert uønsket forlengelse av blødning i andre deler av sirkulasjonen, mens måltromben hindres eller behandles. Forbindelsene fremstilt ifølge foreliggende oppfinnelse tilfreds-stiller dette behov på fagområdet ved å tilveiebringe tera-peutiske midler for å hindre og behandle tromber.
Forbindelsene fremstilt ifølge foreliggende oppfinnelse oppviser effektivitet som antitrombotiske midler på grunn av deres evne til å blokkere fibrinogen fra å virke ved dets blodplatereseptorsete, og således hindre blodplateaggregering.
Foreliggende oppfinnelse angår en analogifremgangsmåte for fremstilling av terapeutisk aktive forbindelser av formelen

Uttrykket "farmakologisk effektiv mengde" skal bety den mengde av en medisin eller et farmasøytisk middel som vil fremkalle den biologiske eller medisinske respons i et vev, system eller dyr som ettersøkes av en forsker eller lege. Uttrykket "anti-koagulant" skal omfatte heparin og warfarin. Uttrykket "trombolyttisk middel" skal omfatte streptokinase og vevplasminogenaktivator. Forbindelsene fremstilt ifølge foreliggende oppfinnelse kan administreres i slike orale doseringsformer som
tabletter, kapsler (som hver omfatter blandinger med forlenget frigjøring eller fastsatt frigjøring), piller, pulvere, granuler, eliksirer, tinkturer, suspensjoner, siruper og emulsjoner. De kan likeledes også administreres i intravenøs (bolus eller infusjon), intraperitoneal, subkutan eller intra-muskulær form, alle bruksformer som er kjent for fagmannen. En effektiv, men ikke-toksisk mengde av den ønskede forbindelse kan anvendes som et anti-aggregeringsmiddel.
Forbindelser fremstilt ifølge foreliggende oppfinnelse kan administreres til pasienter der det ønskes å inhibere trombose ved å inhibere binding av fibrinogen til blodplate-membranglycoproteinkompleks Ilb/IIIa-reseptor. De er anvendbare i kirurgi på perifere arterier (arterietransplantasjoner, carotid endarterectomi) og i cardiovaskulær kirurgi hvor mani-pulasjon av arterier og organer, og/eller innvirkningen av blodplater og kunstige overflater, fører til blodplateaggregering og -kompensasjon. De aggregerte blodplater danner tromber og tromboemboli. De kan administreres til disse kirurgiske pasienter for å hindre dannelse av tromber og tromboemboli.
Sirkulasjon utenfor kroppen anvendes rutinemessig for cardiovaskulær kirurgi for å oxygenere blod. Blodplater fester seg til overflatene i kretsen utenfor kroppen. Adhesjon er avhengig av samvirkningen mellom GPIIb/IIIIa på blodplatemembranene og fibrinogen som er adsorbert til over-flaten av kretsen. (Gluszko et al., Amer. J. Physiol., 1987, 252:H, sider 615 - 621). Blodplater som er frigjort fra kunstige overflater oppviser svekket hemostatisk funksjon. Andre anvendelser av disse forbindelser omfatter hindring av blodplatetrombose, tromboemboli og reokklusjon under og etter trombolyttisk terapi og hindring av blodplatetrombose, tromboemboli og reokklusjon etter angioplasti av koronare og andre arterier og etter kransarterie-bypass-prosedyrer. De kan også anvendes for å hindre myocardialt infarkt.

N-CBZ-tyrosin (3 g, 9,9 mmol) (fra Bachem Chemical Supply, California), ble oppløst i DMF og behandlet med NaH (50 % dispersjon i olje, 0,95 g, 19,8 mmol) i 1 time, så ble 1,3-bromklorpropan (1 ml, 9,9 mmol) tilsatt og reaksjonsblandingen omrørt i 16 timer. DMF ble fjernet i vakuum og resten oppløst i vann, ble surgjort til pH 3 og ekstrahert med ethylacetat. Ethylacetatsjiktet ble tørket med MgSO^, filtrert og inndampet. Kolonnekroaratografi (Si02, 97:3:1 (CHCl3/CH3OH/HOAc) ga 2,42 g produkt som en gul olje.

Den resulterende suspensjon ble omrørt ved 0 - 10° C i 1,0 time og så ble 6-(t-butyloxycarbonylamino)hexylbromid (12,6 g, 0,045 mol) i 25 ml DMF tilsatt dråpevis ved 0 - 5° C og den klare, mørke reaksjonsblanding ble omrørt ved romtemperatur over natten.
Etter løsningsmiddelfjerning ble resten tatt opp i EtOAc og dette ble gjort surt med 10 % KHS04~løsning. Den organiske fase ble fraskilt, vasket med saltløsning, tørket (Na2S04) og løsningsmidlet fjernet for å gi en olje. Denne ble renset ved hjelp av kolonnekrpmatografi på silicagel ved eluering med 98:2:1 CHCl3/CH3OH/HOAc til å gi ren 2-1 som en klar olje.

Oxalylklorid (55,8 ml, 0,64 mol) ble oppløst i 1 liter CH2C12 og avkjølt til -78° C under N2. DMSO (54,2 ml,
>0,76 mol) ble tilsatt dråpevis. Etter at gassutviklingen hadde stanset ble 2-10 (102,5 g, 0,45 mol) oppløst i 200 ml CH2C12 tilsatt i løpet av 20 minutter. 0,152 g (0,4 mmol) av forbindelse 2-la ble tilsatt til en løsning av IN NaOH (0,4 ml) i 10 ml H20 og denne ble omrørt ved 0 - 5° C i 10 minutter mens det meste av faststoffet ble oppløst. Til denne kraftig oppløste suspensjon ble det tilsatt benzoylklorid (0,062 g, 0,44 mmol) fulgt av fast natriumbicarbonat (0,037 g, 0,44 mmol) og den resulterende blanding ble omrørt ved 0 - 5° C i 1 time.

Til en omrørt løsning av 1,2 ml IN NaOH i 15 ml
H20 avkjølt til 0 - 5° C ble det tilsatt 0,457 g (1,2 mmol)
av forbindelse 2-la og den resulterende blanding ble omrørt i 10 minutter hvorved det meste av faststoffet ble oppløst. Til denne kraftig. omrørte suspensjon ble det så tilsatt 3-fenylpropanoylklorid (0,223 g, 1,32 mmol) fulgt av fast natriumcarbonat (0,111 g, 1,32 mmol). Den resulterende, hvite blanding ble omrørt kraftig ved 0 - 5° C i 1,5 timer. Reaksjonsblandingen ble så fortynnet med 40 ml H20 og denne ble surgjort til pH 2 - 3 med en 10 %-ig KHS04~løsning.
Den resulterende, vandige fase ble så ekstrahert med 4 x 50 ml porsjoner av EtOAc, og den kombinerte, organiske fase ble vasket med 50 ml H20, 50 ml saltløsning og tørket (Na2S04). N-B0C-4-jod-L-fenylalanin (4-1) (1,0 g, 2,55 mmol) ble oppløst i diethylamin under N2 og behandlet med 3-butylen-l-ol (0,23 ml, 3,06 mmol), [Pd (P (CgH5) 3]2C12 (°/089 <3 * 0,127 mmol) og Cul (0,012 g, 0,064 mmol). Etter 3 timer ble løs-ningsmidlet fordampet, resten ble oppløst i vann (pH =11) og ekstrahert med ethylacetat. Vannsjiktet ble så surgjort til pH 3 og ekstrahert med ethylacetat. Dette organiske ekstrakt ble tørket og inndampet for å gi 0,8 g rå 4-2. R_ = 0,47 i 97/3/1CHC13/CH3OH/HOAc, ninhydrin-farve.

4-2 (0,40 g, 1,2 mmol) ble oppløst i en ethanol/ vannoppløsning (25 ml) og ble behandlet med 10 %-ig Pd/C
(0,1 g) og H2 på et Parr-apparat. Etter 2 timer ble løsningen filtrert og inndampet. Kolonnekromatografi på silicagel (94:5:1 CHCl3/CH3OH/HOAc) ga 0,321 g (80 %) av 4-3. Rf = 0,57 i 97:3:1 CHCl3/CH30H/HOAc, hinhydrinfarve.

4-3 (0,285 g, 0,85 mmol) ble oppløst i CH2C12
3 (10 ml) avkjølt til 0° C, og ble behandlet med CH2N2~løsning. Etter 10 minutter ble reaksjonen stanset med MgSO^, filtrert og inndampet for å gi den ester som anvendes i den neste reaksjon. Rf = 0,5 i 92:8:1 CHCl3/CH3OH/HOAc, ninhydrin-farve.

Denne ester ble oppløst i 10 ml CH2C12 og tilsatt ved -78° C til en løsning fremstilt ved å behandle p-toluen-sulfonylklorid (0,14 g, 0,67 mmol) i CH2C12 ved -78° C med
pyridin (0,1 ml, 1,35 mmol) i 10 minutter. Reaksjonsblandingen fikk varme seg opp til romtemperatur i løpet av 1,0 time og så ble vannet tilsatt. Det organiske sjikt ble separert, tørket og fordampet. Kolonnekromatografi 97:3:1 på silicagel og eluering med CHCl3/CH30H/HOAc ga 4-4 (0,27 g, 70 %).

ble fjernet ved høyvakuum (30° C). Resten ble renset ved flashkromatografi på silicagel og eluering med 98:2 CHCl^
(mettet med NH3)/CH3OH for å gi methyl-2-S-(N-t-butyloxycarbonylamino) -3-[4-(4-t-butylaminobutyl)fenyl]propionat (0,11 g, 52 %) som en olje.

Denne ester (0,10 g, 2,7 mmol) ble oppløst i 1:1:1 THF/CH3OH/H20 (10 ml) og LiOH-H20 (0,033 g, 1,38 mmol) ble tilsatt ved romtemperatur. Etter omrøring i 2 timer ble løs-ningsmidlet fjernet og resten kromatografert på silicagel ved eluering med 9:1:1 C2H5OH/H20/NH4OH for å gi ren 4-5.

Eksempel 51
2- S-( pentanoyloamino)- 3-[ 4-( 4- N- t- butyloxycarbonylpiperidin-4- yl) butyloxyfenyl] propionsyre ( 5- 2)
5-1 (1,05 g, 2,5 mmol) ble tilsatt til en kald løs-ning av IN NaOH (2,5 ml) i 20 ml H20 og omrørt ved 0 - 10° C
i 5 minutter for å gi en klar løsning. Så ble pentanoylkloridet (0,332 g, 2,75 mmol) tilsatt dråpevis fulgt av NaHC03 (0,231 g, 2,75 mmol) og den resulterende blanding ble omrørt kraftig ved 0 - 10° C i 1 time. Reaksjonsblandingen ble fortynnet med H20 (75 ml), surgjort til pH 2 - 3 med 10 %-ig KHS04 og ekstrahert med EtOAc. Denne ekstrakt ble filtrert, vasket med saltløsning, tørket (Na2S04) og løsningsmidlet fjernet for å gi en olje. Denne ble renset ved flashkromatografi på silicagel og eluering med 97:3:1 CHCl3/CH3OH/HOAc for å gi ren 5-2 (0,44 g) som en klar olje.

De testfremgangsmåter som anvendes for å måle anti-blodplate-aggregeringsaktiviteten for forbindelsene fremstilt ifølge oppfinnelsen er beskrevet nedenfor.
Eksempel 55
Blod ble ført i 0,1 volumer av syre-citrat-dextrose (85 mM natriumcitrat, 64 mM citronsyre, 110 mM dextrose) ved venepunktur fra normale frivillige mennesker. Blodplaterik plasma ble fremstilt ved sentrifugering ved 400 x g i 12 minutter. PGEl (5 mg/ml) ble tilsatt og blodplater ble oppsamlet ved sentrifugering ved 800 x g i 12 minutter. Blodplatepelleten ble resuspendert i human blodplatebuffer (140 mM NaCl, 7,9 mM KC1, 3,3 mM Na2HP04, 6 mM HEPES, 2 % kvegserumalbumin, 0,1 % dextrose, pH 7,2) og ble filtrert gjennom "Sepharose 2B" som på forhånd var bragt i likevekt i human blodplatebuffer. Blodplatene ble tellet og justert til 2 x 108/ml med human blodplatebuffer. Humant fibrinogen (10 - 100 mg/m]) og CaCl2 (1 mM) ble tilsatt og aggregering ble startet ved tilsetning av 10 mM ADP. Aggregering ble fulgt ved starthastighet for økning av lysgjennomgang.
Inhibering av blodplateaggregering for utvalgte forbindelser fremstilt ifølge oppfinnelsen er vist i den følgende tabell og angitt som IC50, hvilken representerer den nødvendige konsentrasjon (dose) av testforbindelsen for å inhibere aggregering med 50 % i forhold til en kontroll som mangler forbind-elsen.
Claims (3)
1. Analogifremgangsmåte for fremstilling av terapeutisk aktive forbindelser av formelen
og de farmasøytisk akseptable salter derav, hvori
R<1> er en eventuelt med lavere alkyl substituert pyrroli-
dinyl-, piperazinyl- eller piperidinylring, eller NR<6>R<7>, hvori R6 og R<7> uavhengig er hydrogen eller usubsti-
tuert eller substituert C^Q-alkyl, hvori nevnte substituenter er dialkylaminoalkyl eller fenyl,
R4 er hydrogen,
C4_10- f enylalkyl,
fenylcarbonyl,
eventuelt med C^-C^o-alkoxycarbonyl substituert Cj_10-alkylcarbonyl, C^g-alkylaminocarbonyl, C-L.-Lo-alkoxycarbonyl,
eventuelt med fenyl-C^-CLo-alkoxycarbonyl eller carboxy substituert C4.10-fenylalkylcarbonyl,
eventuelt med fenyl-C-L-C^-alkoxycarbonyl eller
carboxy substituert C4_10-fenylalkoxycarbonyl,
m er et helt tall fra null til ti, og

karakterisert ved at en forbindelse av formelen

2-S-(N-t-butyloxycarbonylamino)-[4-(3-N-t-butylamino-propyloxy)fenyl]propionsyre;
2-S-(N-benzyloxycarbonylamino)-3-[4-(N,N,2,2-tetra-methyl-1,3-propandiamino)propyloxyfenyl]propionsyre;
2-S-(N-benzyloxycarbonylamino)-3-[4-(1,1,3,3-tetra-methylbutylamino)propyloxyfenyl]propionsyre;
2-S-(N-benzyloxycarbonylamino)-3-[4-(6-aminohexyl-oxyfenyl)]propionsyrehydroklorid;
2-S-(N-benzyloxycarbonylamino)-3-[4-(7-aminoheptyloxy)fenyl]propionsyrehydroklorid; og
