d - 1 - r
Nitrosoureumderivaten met anti-tumorwerkzaamheid, alsmede daarmee samengestelde farmaceutische samenstelling.
De uitvinding heeft betrekking op nieuwe nitroso-ureumderivaten, die bruikbaar zijn in verband met hun anti-tumorwerkzaamheid. De uitvinding om tevens farmaceutische samenstellingen, die deze verbindingen bevatte^, 5 als methodes om ze te gebruiken.
De afgelopen tien jaar zijn de nitrosourea aanvaard als krachtige anti-tumormiddelen (Johnston, e.a., J. Med.
Chem., 14:600 (1971)). Daarbij wordt aangenomen, dat de werking plaatsvindt door het vrijgeven van isocyanaat 10 in vivo. De twee verbindingen, die het meest frequent klinisch worden gebruikt, zijn l-cyclohexyl-3-(-chloorethyl)--3-nitrosoureum (CCNU) en 1,3-bis(2-chloorethyl)-1-nitroso-ureum (BCNU), dat in vivo een isocyanaat vrijgeeft, afgeleid van de niet genitroseerde zijde van het molecuul, en een 15 alkyleringsmiddel van de andere zijde.
Tal van studies zijn gericht op de metabolische produkten, voortgebracht in vivo en in vitro in waterige media, bestaande in hoofdzaak uit 2-chloorethanol, vinyl-chloride, acetaldehyde,en dichloorethaan (Johnston, e.a., 20 J. Med. Chem., 18:634 (1975)). Het is verder bekend, dat het N-nitroso-N-alkylureido-gedeelte van het molecuul DNA (deoxyribonucleïnezuur) alkyleert in vivo en in vitro (Frei, e.a., Biochem. J., 174:1031 (1978)). In feite is aangetoond, dat de carcinogene werkzaamheid van middelen 25 zoals N-methyl-N-nitrosoureum in correlatie staat met de mate van alkylering van het guaninedeel in DNA van target-weefsels aan het C-6 atoom.
Alkylering van het DNA treedt op binnen een uur na toediening van het nitrosoureum, en de halfwaarde van de 30 levensduur van gealkyleerde produkten is ongeveer 24 tot 28 uur ((Reed, e.a., (Cancer Res., 35:568 (1975)). Onderzoek gaf aan, dat lage doses van nitrosoureum slechts een geringe bedreiging geven als mutagens, en zijn derhalve niet belangrijk carcinogeen. Hieruit kan worden geconclu-35 deerd, dat, des te meer transglutaminase-specifiek (zie beneden) het isocyanaat, dat resulteert van ontleding van 800 1 6 02 - 2 - τ het nitrosoureum is, des te lager de vereiste dosis is, resulterende in een verminderd risico van kankervorming van het anti-tumormiddel.
Er is recentelijk aangetoond, dat een aantal 5 isocyanaten krachtige inhibitoren zijn van het enzym transglutaminase (Gross, e.a., J. Biol, Chem., 250:7693 (1975)}, een calcium-afhankelijk enzym, dat de lysine-glutamine vernetting van bepaalde proteïnes, aanwezig op neoplastische celoppervlakken, katalyseert. Dit enzym 10 is betrokken bij de ongecontroleerde proliferatie van kankercellen (Yancey and Laki, Ann. N.Y. Acad. Sci, 202:344 (1972)). Er wordt aangenomen, dat deze vernette proteïnen een extra cellulaire bekleding vormen, die er voor zorgt, dat de cel niet wordt herkend door het cellulair 15 immune systeem, waardoor de normale destructie van vreemd neoplastisch weefsel wordt verhinderd. Het enzym is vrij specifiek ten opzichte van glutamineresiduën,daar substraten, en isocyanaten, die op deze residuen lijken, zijn gebleken de meest effektieve inhibitoren te zijn (Gross, e.a., 20 J. Biol, Chem., 250:7693 (1975)).
De struktuur van de aktieve plaats van het transglutaminase is gebleken de pentapeptidereeks —Tyr-Gly-Gln-Cys-Trp— te bevatten en heeft de vorm van een zak van ongeveer 5 x 5 R (Folk and Cole, J. Biol. Chem., 25 241:3238 (1966)).
Volgens de hypothese, waarop de onderhavige uitvinding zich baseert, dient een superieure inhibitor van transglutaminase gezien de bovengegeven reeks en grootte hydrofobe eenheden te bevatten, afgericht van, maar in de 30 nabijheid van de zak aan de actieve plaats. Twee mogelijke inhibitoren, die aan deze criteria voldoen, zijn neopentyl-isocyanaat (formule 1 van het formuleblad), en neohexyl-isocyanaat (formule 2) die in grootte overeen schijnen te komen met glutamine (formule 3).
35 - De hypothese van de uitvinding wordt gesteund door het feit, dat tert-butylgroepen, of andere hydrofobe groepen, bevestigd aan het a- of β-koolstofatoom van glutaminezuur, superieure substraten produceren voor transglutaminase (Gross and Folk, J. Biol. Chem., 248:130 (1973)). Ester-40 analogen van deze verbindingen zijn eveneens substraten 80 0 1 6 02 - 3 - * τ (Grossand Folk, J. Biol- Chem., 249:3021 (1974)).
Er is aangetoond, dat isocyanaten de vorming van transglutaminase via alkylthiocarbamaatester inhibiteren door de enkele sulfhydrylgroep aan de actieve plaats (Gross, 5 e.a., J. Biol. Chem., 250:7693 (1975)): H 0

R-SH + R' -N=C=0 -> R'-N-C-SR
Bis-(neopentyl)-N-nitrosoureum (formule 4) werd gesynthetiseerd als een verbinding, die neopentylisocyanaat 10 (formule 1) in vivo zou kunnen vrijgeven, resulterende in inhibitie van transglutaminase. De beperkte oplosbaarheid van deze verbinding verhinderde evenwel de klinische evaluatie ervan.
Recente studies hebben laten zien, dat de activi-15 teit van nitrosourea sterk wordt verhoogd door de aanwezigheid van de 2-chloorethylgroep aan de genitroseerde zijde van de verbinding (Montgomery, Cancer Treat. Rep., 60:651 (1976)? Johnston, e.a., J. Med. Chem., 9:892 (1966); Farmer e.a., J.Med. Chem., 21:514 (1978)). Aangezien de 2-chloorethylgroep 20 eveneens de oplosbaarheid verhoogt, werd het de keuzegroep voor de 3-positie bij dit onderzoek.
Zoals in de verdere beschrijving in de voorbeelden zal worden behandeld, werden volgens de uitvinding verbindingen gesynthetiseerd met formule 5 van het formuleblad, 25 waarbij R resp. neopentyl, neohexyl, isopentyl, en isobutyl is, en X Cl of Fl.
Uit chemische onderzoeken bleek, dat de neopentyl-en neohexylisocyanaten opmerkelijk stabiel zijn in water-aceton (1:3) bij 57°C zonder dat aanmerkelijke hoeveelheden 30 worden gehydrolyseerd, zelfs na een drie dagen periode bij een pH 6,0. Beide bezitten halfwaardetijden van levensduur . van groter dan 30 minuten bij fysiologische pH en temperatuurwaarde .
Zodoende werden de vertakte alkylgroepen van de 35 verbinding volgens de uitvinding geselecteerd onder gebruikmaking van het criterium, dat zij substraten zouden kunnen 900 1 6 02 i' - 4 - vormen voor en zodoende selectieve inhibitoren zijn voor transglutaminase. Zoals boven opgemerkt, vermeldt het werk van Montgomery een enkel l-vertakt-alkyl-3-(2-chloorethyl)- 3-nitrosoureum, nl. 1-(1-methyl-hexyl)-3-(2-chloorethyl)-3-5 nitrosoureum (formule 6).
Hoewel deze verbinding· een l-vertakte-alkyl-3-(2-chloorethyl)-3-nitrosoureum is, voldoet zij niet aan de sterische criteria, boven uiteengezet voor een optimale inhibiterende werkzaamheid.
10 Het is het doel van de uitvinding om nieuwe verbin dingen te verschaffen met een anti-tumoractiviteit.
Het is een verder doel van de uitvinding om nieuwe verbindingen te verschaffen, die de activiteit van transglutaminase inhibiteren.
15 Nog een ander doel van de uitvinding is het ver schaffen van nieuwe verbindingen, die effektieve anti-tumorverbindingen vormen bij lage doses, waardoor nadelige mutagene en/of carcinogene effekten tot een minimum worden teruggebracht.
20 Een additioneel doel van de uitvinding is het verschaffen van samenstellingen, die de nieuwe verbindingen bevat, alsmede toepassingsmethodès.
Experimenteel gedeelte.
Er werden smeltpunten bepaald onder gebruikmaking 25 van een Thomas Hoover capillair smeltpunt-apparaat, waarbij de smeltpunten niet waren gecorrigeerd. Infrarood (IR) spectra werden verkregen onder gebruikmaking van ëen Perkin Elmer 397 spectrofotometer, en NMR (magnetische kernresonantie) werden opgenomen op een Bruker WP80DS 30 systeem met tetramethylsilaan als inwendige 'standaard. Elementair-analyses werden uitgevoerd door Galbraith Laboratories, Knoxville, Tennessee. Tert-butylacetylchloride en 3,3-dimethy1-1-butanol werden verkregen van Aldrich Chemical Co., en werden gebruikt zonder verdere zuivering 35 als gevolg van een bevredigende NMR analyse.
In de onderstaande voorbeelden zijn bereidingen gegeven van verbindingen volgens de uitvinding.
VOORBEELD I
l-Neopentyl-3-(2-chloorethyl)ureum:Neopentylamine (8,7 g, 40 0,1 mol) werd opgelost in 50 ml anhydrisch diethylether 8001602 - 5 - r en gekoeld tot 5°C. Vervolgens werd 2-choorethylisocyanaat (10,5 g, 0,1 mol), opgelost in een additionele 50 ml ether, toegevoegd gedurende 30 min. onder het handhaven van een temperatuur van minder dan 10°C. Het roeren werd 5 voortgezet voor nog een uur, en het koude mengsel werd afgefiltreerd en gewassen met gekoelde ether. De verkregen verbinding werd gedroogd in een desiccator over natrium-hydroxyde gedurende een nacht, waarbij 15,4 g (80 %) wit poeder werd verkregen met een smeltpunt van 90-91°C 10 (ontleed). Een elementair-analyse gaf de volgende resultaten:

l-Neopentyl-3-(2-chloorethyl)-3-nitrosoureum: De gehele 20 opbrengst van het op bovenstaande wijze verkregen 1-neopentyl- 3-(2-chloorethyl)ureum (0,08 mol) werd opgelost-in 120 ml geconcentreerd HC1:ethanol, 2:1 bij 5°C in een 500 ml ronde bodemfles, uitgerust met magnetische roerorganen. Vervolgens werd natriumnitriet (5,5 g, 0,08 mol) opgelost in 30 ml 25 water en toegevoegd aan de oplossing gedurende een periode van 10 min. Het roeren werd voortgezet gedurende 2 uur, het gele, kristallijne precipitaat werd afgefiltreerd en gewassen met 5 delen van elk 100 ml van gekoeld gedestilleerd water. Het produkt werd in vacuum gedroogd gedurende 8 uur, 30 waardoor men 15 g (84 %) gezuiverd produkt verkreeg, dat onder ontleding smolt bij 52-53°C. IR-analyse gaf pieken bij 1730/cm (C=0) en 1520/cm (C-N-H). NMR gaf een singlet (9H) bij 1 ppm, een doublet (2H, J=6,5Hz) bij 3,3 ppm, een triplet (2H, J=6,5Hz) bij 3,6 ppm, en een triplet (2H, 35 J=6,5Hz) bij 4,2 ppm.
VOORBEELD II
l-Neohexyl-3-(2-chloorethyl)ureum: Een hoeveelheid van 5 g van 3,3-dimethylbutylamine (0,5 mol) werd opgelost in 100 1 6 02 f - 6 - 30 ml anhydrisch diethylether en geroerd op een ijsbad. Vervolgens werd 2-chloorethylisocyanaat (5,25 g, 0,5 mol) opgelost in 15 ml ether, toegevoegd onder het handhaven van een temperatuur van 10°C of minder, terwijl het roeren 5 werd voortgezet gedurende 2 uur. Na affiltreren van het produkt en wassen met vier porties van 5 ml gekoeld ether werd 6,2 g gezuiverd produkt (60 %) verkregen, dat smolt bij 84°C (onder ontleding). IR analyse gaf pieken bij 1625/cm (C=0) en 1580/cm (N-C=0). NMR gaf een singlet 10 (9H) bij 0,9 ppm, een multiplet (2H) bij 1,3 ppm, een multiplet (2H) bij 3,2 ppm, een multiplet (4H) bij 3,5 ppm, een singlet (1H) bij 5,5 ppm, en een singlet (1H) bij 5,7 ppm. l-Neohexyl-3-(2-chloorethyl)-3-nitrosoureum: Een hoeveelheid van 2,07 g (10 mmol) van het op bovenstaande wijze verkregen 15 l-neohexyl-3-(2-chloorethyl)ureum werd opgelost in 15 ml geconcentreerd HC1:ethanol, 2:1, bij 10oc, en daaraan werd 690 mg (10 mmöl) matriumnitriet, opgelost in 3 ml water, toegevoegd. Het roeren werd voortgezet bij 5°Q gedurende 1½ uur, en het precipitaat werd gewassen met vier maal 20 25 ml gedestilleerd water. Na een nacht drogen in vacuum werd 1,7 g (74 %) van het gezuiverde produkt verkregen, dat smolt bij 43,0-43,5°C (onder ontleding). IR analyse gaf pieken bij 1705/cm (C=0) en 1525/cm (C-N=0). NMR gaf een singlet (9H) bij 0,9 ppm, een multiplet (2H) bij 1,3 25 ppm, en een symmetrische multiplet (6H) bij 3,8 ppm.
Er werd geen aanwezigheid van isomeer materiaal gedetecteerd. VOORBEELD III
l-Isopentyl-3-(2-chloorethyl)ureum: Isopentylamine (8,7 g, 0,1 mol) werd opgelost in 100 ml anhydrische ether en gekoeld 30 tot minder dan 10°C. Vervolgens werd 2-chloorethylisocyanaat (10,5 g, 0,1 mol) opgelost in 20 ml ether en toegevoegd aan het reactiemengsel onder snel roeren bij een temperatuur van minder dan 10°C. Het roeren werd 1 uur langer voortgezet, en het precipitaat werd afgefiltreerd en gewassen met vier 35 maal 20 ml gekoelde ether. Na gedurende de nacht drogen in vacuum, werd 16,5 g (85 %) gezuiverd produkt verkregen als een wit poeder met een smeltpunt van 58-59°C. IR analyse gaf pieken bij 1620/cm (C=0) en 1575/cm (N-C=0). NMR gaf een doublet (6H, J=7Hz) bij 0,9 ppm, een triplet (2H, J=7HZ) 40 bij 1,4 ppm, een multiplet (1H) bij 1,8 ppm, een multiplet 8001602 - 7 - ï (1H) bij 3,2q?pm, een multiplet (4H) bij 3,6 ppm, een singlet (1H) bij 5,7 ppm, en een singlet (1H) bij 6,0 ppm. l-Isopentyl-3-(2-chloorethyl)-3-nitrosoureum: Van het aldus bereide 1-isopenty1-3-(2-chloorethyl)ureum loste 5 men een hoeveelheid van 1,92 g (10 mmol) op in 15 ml HC1:ethanol, 2:1, en koelde tot 5°C. Natriumnitriet (690 mg, 10 mmol), opgelost in 3 ml water, werd toegevoegd in gedeelten in roeren, waarna het roeren werd voortgezet voor nog eens twee uur. Het mengsel werd afgefiltreerd 10 en 10 ml water werd toegevoegd. Door tweemaal te extraheren met 20 ml ethylacetaat, gevolgd door verdamping van het oplosmiddel, werd het produkt verkregen als een olie, die niet vast werd bij staan. Overeenkomstige resultaten werden verkregen, wanneer 58 %'s mierezuur als oplosmiddel 15 werd gebruikt. IR analyse gaf pieken bij 1705/cm (C=0) en 1525/cm (N-C=0). NMR analyse gaf een doublet (6H, J=7Hz) bij 0,9 ppm, een triplet (2H, J=7Hz) bij 1,4 ppm, een multiplet (1H) bij 1,8 ppm, een multiplet (6H) bij 3,8 ppm, en een singlet (1H) bij 7,4 ppm.
20 VOORBEELD IV
l-Isobutyl-3-(2-chloorethyl)ureum: Isobutylamine (7,3 g, 0,1 mol) werd opgelost in 20 ml diethylether en gekoeld tot minder dan 5°C. Chloorethylisocyanaat (10,5 g, 0,1 mol), opgelost in 15 ml ether, werd toegevoegd onder het handhaven 25 van een temperatuur tussen 0 en 5°C en met krachtig roeren.
Het roeren werd nog eens 2 uur voortgezet, waarna het mengsel werd gekoeld tot -5°C en het produkt (4,1 g, 46 %) werd afgefiltreerd en gedroogd in vacuum gedurende 18 uur over KOH; smeltpunt 79,5°C (ontleding). IR analyse gaf pieken bij 30 1630/cm (C=0) en 1585/cm (N-C=0). NMR analyse gaf een doublet bij 0,9 ppm (J=6,4Hz), een multiplet (1H) bij 2,1 ppm, een quartet (2H) bij 2,9 ppm (J=6,4Hz), en een multiplet (4H) bij 3,5 ppm.
l-Isobutyl-3-(2-chloorethyl)-3-nitrosoureum: Van het op 35 bovenstaande wijze verkregen l-isobutyl-3-(2-chloorethyl)-ureum werd een hoeveelheid van 2 g (11,2 mmol) opgelost in 25 ml geconcentreerd HC1:ethanol, 2:1, en gekoeld tot 5°C. Een oplossing van 773 mg (11,2 mmol) natriumnitriet in 5 ml water werd toegevoegd in gedeelten, en het mengsel 40 werd bij deze temperatuur gedurende 2 uur geroerd. De 8001602 * - 8 - kristallijne gele vaste stof werd afgefiltreerd en in vacuum gedroogd gedurende de nacht, waardoor 1/6 g (70 %) van het produkt werd verkregen, dat een smeltpunt had van 510C (ontleding). IR analyse gaf pieken bij 1705/cm (C=0) 5 en 1525/cm (C-N=0). NMR analyse gaf een doublet (6H, J=6,8Hz) bij 0/9 ppm, een multiplet (1H) bij 2,0 ppm, en een multiplet (6H) bij 3,7 ppm.
Op dezelfde wijze als in de bovenstaande voorbeelden beschreven voor l-vertakt-alkyl-3-(2-chloorethyl)-3-nitroso-10 ureau is het ook mogelijk om l-vertakt-alkyl-3-(2-fluorethyl)- 3-nitrosourea te bereiden uitgaande van corresponderende 2-fluorethylverbindingen.
KLINISCHE ONDERZOEKINGEN Proef 1 15 Een enkelvoudige dosis van l-neopentyl-3-(2-chloor- ethyl)-3-nitrosoureum (NCNU) werd intraperitoneaal (ip) geïnjecteerd in CDF1 muizen, waarin twee dagen eerder 5 bij benadering 10 x 10 murine-leukemia LI210 tumorcellen i.p. waren ingeplant. Het NCNU was opgelost in emulphor EL-620 20 (gepolyoxyethyleerd; GAF Corporation, New York, N.Y.) en aangevuld tot het gewenste volume met 0,85 %'s natrium-chloride. De toename in gemiddelde levensduur (ILS) van de proefdieren in vergelijking met controle (onbehandelde) dieren is weergegeven in onderstaande tabel A.

Proef 3
Het effekt van NCNU werd getest ten opzichte van Yancey's lymfocytische leukemia (YLL) , ddle onderhuids was ingeplant twee dagen voor injectie van het medicijn.
20 De enkelvoudige injectie van 0,1 ml van een oplossing van een milt van een YLL dragende muis, gehomogeniseerd in 10 ml Locke's oplossing, werd gegeven of intraperitoneaal of onderhuids. In onderstaande tabel C is de toename in gemiddelde levensduur gegeven van proefdieren in vergelijking 25 met controledieren.

Proef 4 25 Proef 3 werd herhaald met een NCNU behandeling, die werd aangevangen 2 of 7 dagen na implanting van YLL tumoren. NCNU injectie was ip of sc op een schema van een keer per week gedurende 4 weken, tweemaal per week gedurende 4 weken, of een enkele dosis. De resultaten zijn gegeven in 30 onderstaande tabel D.

Farmaceutische samenstellingen
De verbindingen volgens de uitvinding kunnen worden gebruikt in bruikbare farmaceutische samenstellingen 25 in doseringsvormen zoals tabletten, capsules, poeder-pakketten, vloeibare oplossingen, suspensies, of elixers voor orale toediening; vloeistof voor parenteraal gebruik, en in bepaalde gevallen suspensies voor parenteraal gebruik. In dergelijke samenstellingen is het actieve bestanddeel 30 gewoonlijk aanwezig in een hoeveelheid van ten minste 0,5 gew. % gebaseerd op het totaal gewicht van de samenstelling, en niet meer dan 95 gew. %.
Behalve de actieve ingrediëntverbinding van de uitvinding, kan de anti-tumorsamenstelling een vaste of 35 vloeibare niet-toxische farmaceutische drager bevatten voor het actieve ingrediënt.

De capsules, tabletten, en poeders zullen algemeen 1 tot ongeveer 95 % en bij voorkeur ongeveer 5 tot 90 gew. % aan actief ingrediënt bevatten. Deze doseringsvormen bevatten, bij voorkeur ongeveer 5 mg tot ongeveer 500 mg 5 aan actief bestanddeel, met bij voorkeur ongeveer 7 mg tot ongeveer 250 mg.
De farmaceutische drager kan een steriele vloeistof zijn zoals water, of een geschikte olie, waaronder die van petroleum, dierlijke of plantaardige olieën, of olieën 10 van synthetische oorsprong, bijv. pinda-olie, soja-olie, minerale olie, sesamolie, en dergelijke. In het algemeen bezitten water, een zoutoplossing, waterige dextrose (glucose) en verwante suikeroplossingen, en glycols zoals propeenglycol en polyetheenglycol de voorkeur als vloeibare 15 dragers, in het bijzonder voor injecteerbare oplossingen. Steriele injecteerbare oplossingen zullen gewoonlijk van ongeveer 0,5 tot 25 gew. % en bij voorkeur van. ongeveer 1 tot Ongeveer 10 gew. % van het actieve ingrediënt bevatten.
20 Orale toediening kan door middel van een geschikte suspensie of stroop, waarin het actieve ingrediënt gewoonlijk ongeveer 0,7 tot ongeveer 10 gew. % en bij voorkeur 'ongeveer 1 tot ongeveer 5 gew. % zal vormen. De farmaceutische drager in de samenstelling kan een waterige 25 drager zijn zoals een aromatisch water, een stroop, of een farmaceutische gom.
Geschikte farmaceutische dragers zijn beschreven in Remington's Pharmaceutical Sciences, van E.W. Martin, hetgeen een bekende referentietekst op dit gebied vormt.
30 In de onderstaande voorbeelden zal de preparatie van farmaceutische samenstelling volgens de uitvinding nader worden toegelicht.
VOORBEELD V
Een groot aantal eenheidscapsules werden geprepareerd 35 door het vullen van standaard tweedelige harde gelatine-capsules elk met 250 mg gepoederd l-neopentyl-3-(2-chloorethyl).-3-nitrosoureum, 110 mg lactose, 32 mg talk, en 8 mg stearaat.
VOORBEELD VI
40 Een mengsel van l-neopentyl-3-(2-fluorethyl)-3- 800 1 β 02 £ - 14 -
nitrosoureum in sojabonenolie werd geprepareerd en geïnjecteerd door middel van een positieve verplaatsingspomp in gelatine voor het vormen van zachte gelatinecapsules, die elk 35 mg van het actieve ingrediënt bevatten. De 5 capsules werden gewassen in petroleumether en gedroogd. VOORBEELD VII
Een groot aantal tabletten werd geprepareerd door gebruikelijke procedures op zodanige wijze, dat de doserings-eenheid 100 mg aan actief bestanddeel, 7 mg ethylcellulose, 10 0,2 mg colloidaal siliciumdioxyde, 5 mg magnesiumstearaat, 11 mg microkristallijne cellulose, 11 mg stijfsel (cornstarch) en 98,8 mg lactose bevatte. Geschikte bekledingen werden aangebracht voor het verhogen van de smakelijkheid of het vertragen van de absorptie.
15 VOORBEELD VIII
Een parenterale samenstelling, geschikt voor toediening’ per injectie, werd geprepareerd door het roeren van 1,5 gew. % l-isopentyl-3-(2-chloorethyl)-3-nitroso-ureum in 10 vol. % propyleenglycol en water. De oplossing 20 werd gesteriliseerd door filtratie.
VOORBEELD IX
Een waterige suspensie werd geprepareerd voor orale toediening zodanig, dat elke 5 ml 50 mg fijnverdeeld l-neohexyl-3-(2-chloorethyl)-3-nitrosoureum, 500 mg 25 acacia, 5 mg natriumbenzoaat, 1,0 g sorbitoloplossing, U.S.P., 5 mg natriumsaccharine, en 0,025 ml vanille-tinctuur bevatte.
VOORBEELD X
Een parenterale samenstelling, geschikt voor 30 toediening per injectie, werd geprepareerd door het oplossen van 1 gew. % l-neopentyl-3-(2-chloorethyl)-3-nitrosoureum in natrumchloride-injectie U.S.P. XV, waarbij de pH van de oplossing werd ingesteld tussen 6 en 7. De oplossing werd gesteriliseerd door filtratie.
35 Een grote verscheidenheid van samenstellingen, die omvat zijn binnen de uitvinding, kunnen worden geprepareerd door het substitueren van andere verbindingen, die onder de uitvinding vallen voor de specifieke verbindingen, die in de bovengegeven voorbeelden V t/m X 40 zijn gegeven, en door andere geschikte farmaceutische dra- 800 1 6 02 - 15 - 'Γ gers, beschreven in"Remington1s Pharmaceutical Sciences" te substitueren.
De verbindingen volgens de uitvinding kunnen worden toegediend in de behandeling van één van de verschillende 5 vormen van kanker door elk geschikt middel, dat zorgt voor contact van de werkzame verbinding met de plaats in het lichaam van een warmbloedig dier, waar de werking dient plaats te vinden. Zo kan bijv. de toediening parenteraal zijn, d.w.z. onderhuids, intraveneus, intramusculair, of 10 intraperitoneaal; alternatief of tegelijk kan de toediening oraal plaatsvinden. In het hier gegeven verband wordt onder een warmbloedig dier elk dier verstaan, dat een homeostatisch mechanisme bezit, en deze definitie omvat zoogdieren en vogels.
15 De dosis, die wordt toegediend, hangt af van de leeftijd, gezondheid, en het gewicht van de ontvanger, de mate van ziekte, het soort bijkomende behandeling, de frequentie van de behandeling, en het gewenste effekt.
Gewoonlijk kan een dagelijkse dosis aan actief bestanddeel 20 lopen van ongeveer 0,1 tot 150 mg per kg lichaamsgewicht. Gewoonlijk is van 2,0 tot 75, en bij voorkeur van 10 tot 50 mg per dag, toegediend in één of meer doses dagelijks effektief om de gewenste resultaten te verkrijgen.
Onderzoek op mensen 25 Patiënten met gevorderde stadia van verschillende vormen kanker werden behandeld door onderhuidse injectie van twee 25 mg/kg doses van NCNU in water, of door intraveneuze toediening van 1 mg NCNU/ml 0,9 %'s zoutoplossing voor het verkrijgen van een totale dagelijkse 30 dosis van 50 mg/kg; een volledig klinisch onderzoek werd uitgevoerd voorafgaand aan en na toediening. Tumorgroei werd waar mogelijk gemeten door palpiteren, röntgenstralen en fotografie.
Na de behandeling vertoonden patiënten opmerkelijke 35 klinische verbetering binnen een paar dagen. De ontsteking slonk geleidelijk, en de tumorgrootte nam veelal af.
De volgende soorten kanker in toevoeging aan anderen kunnen worden behandeld met verbindingen volgens de uitvinding : 40 1) ongedifferentieerde carcinomen van de (R) nier 800 1 δ 02 - 16 - / met metastasen, 2) adenocarcinomen van de dikke darm met levermeta- stasen, 3) intraduct carcinomen van de borst (fase IV) met 5 beenmetastasen, 4) telkens voorkomende melanomen, 5) adenocarcinomen van de borst met hersenmetastasen, en 6) geschubde celcarcinomen van de long.
10 De hierboven gegeven klinische onderzoeken bevestigen, dat de α-vertakte alkyl-3-(2-halogeenethyl)-3-nitrosoureum-verbindingen volgens de uitvinding bijzonder bruikbaar zijn bij de behandeling van verschillende vormen kanker. Voortgezette intraveneuze toediening inhibiteert tumoractiviteit 15 en leidt veelal tot een algemene teruggang van de ziekte.
De werkingswijze van de verbindingen volgens de uitvinding is tot op heden niet geheel bekend. De empirische observatie, dat cellen, indien blootgesteld aan deze verbindingen, stoppen met proliferatie, is evenwel voldoende 20 om hun nuttige gebruik in de behandeling van dergelijke ernstige, tot nog toe niet te behandelen, vaak fatale ziektes, zoals kanker, te waarborgen.
