Description
Titel: Probiotische hydrolysaatvoeding voor kinderen
De uitvinding heeft betrekking op voedingssamenstellingen, zoals kindervoeding en voedingssupplementen voor kinderen. In het bijzonder betreft de uitvinding formuleringen welke, ook bij langdurig gebruik, geschikt zijn voor kinderen die (over)gevoelig zijn voor bestanddelen uit 5 melk van herkauwers (zoals kinderen met koemelkallergie) of een verhoogde kans hebben om een dergelijke gevoeligheid te ontwikkelen.
In Nederland ontwikkelt tussen de 2 en 3% van de flesgevoede zuigelingen een allergie voor koemelkeiwit. Een primaire oorzaak voor het ontstaan van koemelkallergie is een combinatie van erfelijkheid en 10 omgevingsfactoren. Kinderen uit een gezin waarin allergische aandoeningen zoals astma, atopisch eczeem, hooikoorts en/of voedselallergie voorkomen hebben een hoger risico op het ontwikkelen van een koemelkallergie. Normaal gesproken wordt door de zuigeling na de geboorte tolerantie voor voedingseiwitten opgebouwd. Het blijkt dat bij zuigelingen met 15 koemelkallergie dit opbouwproces nog niet optimaal verloopt.
Behalve erfelijkheid spelen ook andere factoren een rol. Zo is de verteringscapaciteit ten aanzien van eiwitten bij zuigelingen nog niet optimaal. Dit wordt veroorzaakt door een relatief laag niveau van eiwitsplitsende enzymen en een relatief hoge pH in de maag. Verder heeft 20 de darm vaak nog een verhoogde permeabiliteit waardoor de eiwitmoleculen de darmwand kunnen passeren en na resorptie als lichaamsvreemd worden herkend. Deze factoren verklaren de relatief hoge prevalentie van koemelkallergie op de zuigelingenleeftijd.
Daarnaast blijkt dat microbiële stimulatie gedurende het eerste 25 levensjaar een belangrijke rol speelt bij de ontwikkeling en rijping van het immuunsysteem. Niet allergische kinderen hebben meer bifidobacteriën en minder clostridia in de ontlasting dan allergische kinderen. Opmerkelijk is 1032840, 2 dat de verandering in darmflora voorafgaat aan de ontwikkeling van allergische symptomen, hetgeen suggereert dat de samenstelling van de darmflora van invloed kan zijn op (het risico op) de ontwikkeling van allergie.
5 Bij kinderen manifesteren de verschijnselen van koemelkallergie zich vooral in de huid, het maagdarmkanaal en ook wel de luchtwegen. Geen van deze symptomen zijn bewijzend of specifiek voor koemelkallergie, wat de diagnostiek moeilijk maakt. De meest voorkomende symptomen bij jonge kinderen zijn diarree, voedingsproblemen (zoals onrustig drinken), braken, 10 kolieken, eczeem, urticaria, benauwdheid, hoesten of piepen na de inname van voedsel.
Preventie wordt aanbevolen voor zuigelingen met een positieve gezinsanamnese: dit zijn zuigelingen van wie één ouder, broertje of zusje een bewezen koemelkallergie heeft. Tot op heden wordt voor kinderen met 15 een positieve gezinsanamnese, een voeding op basis van partieel of vergaand melkeiwithydrolysaat geadviseerd; de antigene eiwitepitopen zijn door enzymatische behandeling gedeeltelijk (partieel hydrolysaat) of vrijwel geheel (vergaand hydrolysaat) vernietigd.
Bekende hydrolysaten van koemelkeiwitten zijn wei-eiwit- en caseïne-20 hydrolysaten. Voor toepassing bij kinderen met een (over)gevoeligheid voor melkvetten van ruminanten, zoals bij voorbeeld Infantile Refsum Disease (IRD) patiënten, wordt bij voorkeur gebruik gemaakt van melkeiwitbevattende grondstoffen welke arm zijn aan melkvet, zoals caseïne of caseïnaat.
25 Vergaand gehydrolyseerde melkeiwitproducten, ook wel eerste generatieproducten genoemd, bevatten voornamelijk peptiden met een molecuulgewicht beneden 1500 dalton. Het is gebleken dat dit gunstig is voor een maximale verlaging van de allergeniciteit. Voedingsmiddelen op basis van vergaand caseïnehydrolysaat hebben een bewezen gunstig effect 30 op kinderen die gevoelig zijn voor ruminant-eiwitten. Bij voorbeeld, 3 aanvraagster publiceerde op basis van een dubbelblind onderzoek in kinderen met koemelkallergie over de veiligheid van een dergelijk hypoallergeen product (bekend onder de handelsnaam Frisolac Allergycare ®) (Terheggen- Lagro et al. BMC Pediatrics (2002), 2: 10).
5 Alhoewel het product veilig is voor de doelgroep heeft het nog wel een aantal nadelen welke met name tot uiting komen bij langdurig gebruik, bij voorbeeld gedurende ten minste enige maanden. De nadelen betreffen onder meer de relatief lage voedingswaarde van een vergaand hydrolysaat. Zo rapporteert Gioviannini et al. (J. American College of Nutr. (1994) Vol. 13, 10 No. 4, 357-363) dat kinderen die enkele maanden zijn gevoed met een vergaand melkeiwithydrolysaat een lagere gewichtstoename vertonen in vergelijking met kinderen die met een partieel melkeiwithydrolysaat waren gevoed. Daarnaast hebben ze een hogere concentratie ureum in het bloed, hetgeen een indicatie kan zijn van een overbelaste aminozuurstofwisseling. 15 In tegenstelling tot vetzuren en glucose kunnen aminozuren niet worden opgeslagen. Aminozuren kunnen ook niet uitgescheiden worden. Bij de stofwisseling van eiwitten (aminozuren) komen kooldioxide en ammoniak vrij. De ureumcyclus produceert ureum uit ammoniak en kooldioxide en stelt ons in staat via de urine overtollige stikstof in de vorm van ureum uit 20 te scheiden in een niet giftige en goed oplosbare vorm.
Guesry et al. (Food Intolerance in Infancy (1989), pg. 257) merkt op dat het verlagen van de allergeniciteit door vergaande hydrolyse (bij voorbeeld tot vrije aminozuren) gepaard gaat met een verlaagde voedingswaarde, zodat een dergelijk hypoallergeen product ongeschikt is 25 voor langdurig gebruik.
Daarnaast heeft een vergaand hydrolysaat geen of een zeer laag vermogen om (orale) tolerantie tegen allergie voor melkeiwitten te induceren (zie JP-03-181681; EP 629350; Schmidt et al., Intestinal Immunology and Food Allergy, 34th Nestle Nutrition Workshop, pg. 24-28). 30 Dit is mogelijk gerelateerd aan vertraagd en/of niet tijdig sluiten van de 4 zogenaamde "tight junctions" in de darmwand en de daarmee gepaard gaande permeabiliteit van de darmwand voor allergenen. De bekende formuleringen op basis van vergaande melkeiwithydrolysaten zijn eveneens niet optimaal om aan te wenden bij kinderen met IRD.

De onderhavige uitvinding beoogt derhalve een bestanddeel voor een voedingsmiddel, bij voorbeeld een kindervoeding, te verschaffen waarbij één of meerdere van bovengenoemde nadelen niet optreden. In het bijzonder heeft de uitvinding als doel om een optimale voeding te verschaffen welke 10 geschikt is voor langdurig gebruik door kinderen met een overgevoeligheid voor melkeiwitten van herkauwers, dan wel een optimale voeding voor preventief langdurig gebruik bij kinderen welke een verhoogde kans hebben om deze gevoeligheid te ontwikkelen. Voorts is de voeding bij voorkeur geschikt voor kinderen, die gevoelig zijn voor melkvet van ruminanten, zoals 15 kinderen met IRD.
Aanvraagster heeft gevonden, dat het verlies aan tolerantie-inducerende eigenschappen gedeeltelijk of geheel kan worden hersteld door toevoeging van één of meerdere probiotica welke de tolerantie voor melkeiwitten kan 20 bevorderen. In een ander aspect van de uitvinding verschaft de aanvraagster het inzicht dat de voedingswaarde van een product dat is gebaseerd op vergaand caseïnehydrolysaat kan worden verhoogd door de ammoniakovermaat in het lichaam, welke bij dit type voeding bijzonder in het geding is, weg te nemen. Hiertoe wordt aan de samenstelling één of 25 meerdere melkzuurproducerende probiotica toegevoegd welke in staat zijn om de overmaat aan ammoniak te reduceren.
De uitvinding verschaft derhalve een samenstelling geschikt als voedingsmiddel of therapeutisch voedingssupplement voor kinderen, 30 omvattende een lipidebron, een koolhydraatbron en een laag-allergeen 5 caseïnehydrolysaat met peptiden van maximaal 3000 dalton, met het kenmerk dat de samenstelling ten minste één melkzuurproducerende probiotische bacteriestam bevat welke de tolerantie voor melkeiwitten kan bevorderen en/of ten minste één melkzuurproducerende probiotische 5 bacteriestam welke de ammoniakbelasting kan verlagen.
In een uitvoeringsvorm bevat de samenstelling ten minste één melkzuurproducerende probiotische bacteriestam welke de ammoniakbelasting kan verlagen. In een voorkeursuitvoeringsvorm bevat 10 de samenstelling ten minste één melkzuurproducerende probiotische bacteriestam welke de tolerantie voor melkeiwitten kan bevorderen.
Bij voorkeur bevat een samenstelling van de uitvinding zowel ten minste één probiotische bacteriestam welke, na opname, de tolerantie voor 15 melkeiwitten kan bevorderen als ten minste één probiotische bacteriestam die de ammoniakbelasting in het lichaam kan verlagen.
Geschikte tolerantieverhogende probiotische stammen kunnen worden gekozen uit stammen van Lactobacillus casei en Lactobacillus paracasei. De 20 L.paracasei stam ssp. paracasei gedeponeerd onder ATCC nummer 55544 heeft de voorkeur. Deze stam is ook bekend onder de code CRL 431 van fabrikant Chr. Hansen.
Bifidobacteria blijken geschikt te zijn voor het verlagen van de 25 ammoniakbelasting, in het bijzonder Bifidobacterium animalis, ssp. lactis zoals gedeponeerd onder ATCC nummer 27536 (B. lactis 27536). Laatstgenoemde is ook bekend onder code BB 12 van fabrikant Chr. Hansen.

In een uitvoeringsvorm bevat een samenstelling zowel Lactobacillus paracasei, bij voorkeur L. paracasei ATCC-55544, als B. lactis, bij voorkeur B. lactis ATCC-27536.
5 Aanvullend op de probiotische stam welke in staat is om de ammoniakbelasting van het lichaam te verlagen kan de samenstelling ook andere componenten bevatten welke kunnen bijdragen aan het verwerken van de overmaat aan ammoniak, zoals het aminozuur arginine dat een belangrijke rol speelt in de ureumcyclus.

De hoeveelheid probiotische bacteriën in een samenstelling van de uitvinding kan variëren, en kan afhangen van het type probioticum en/of de gewenste eigenschappen van de samenstelling in relatie tot de beoogde toepassing. Bij voorkeur bevat de samenstelling ten minste 106 tot 107 15 kolonie vormende eenheden (cfu) per gram van een probiotische melkzuurproducerende stam. De samenstelling bevat bij voorbeeld per gram 106 tot 108 kolonievormende eenheden L. paracasei ATCC-55544 en 106 tot 108 kolonievormende eenheden B. lactis ATCC-27536.
20 Een samenstelling volgens de uitvinding bevat bij voorkeur ten minste één component die een prebiotisch effect heeft op een of meerdere in de samenstelling aanwezige probiotica. Voorbeelden van geschikte prebiotica zijn fructo- en/of galacto-oligosacchariden, met korte of lange ketens, inuline, fucose-bevattende oligosacchariden, beta-glycanen, 25 johannesbroodpitmeel, gommen, pectinen, sialyloligosacchariden, sialyllactose, galactanen met korte of lange ketens, en nucleotiden.
Verrassenderwijs is het laag allergene eiwithydrolysaat in een samenstelling volgens de uitvinding tevens in staat gebleken om de uitgroei 30 van de melkzuurproducerende probiotische stammen in het darmkanaal te 7 stimuleren. Het hydrolysaat vervult dus zowel een nutritionele als een prebiotische rol.
Door de orale opname van een samenstelling met genoemde probiotica en de 5 prebiotische effecten van peptiden uit het laag-allergene eiwithydrolysaat kan de disbalans in de darmflora van kinderen met een allergie voor koelmelkeiwitten ten minste gedeeltelijk worden hersteld.
Het laag allergene caseïnehydrolysaat is bij voorkeur vetarm. Het bevat bij 10 voorkeur voornamelijk peptiden met een molecuulgewicht lager dan 1500 dalton. Bijzonder geschikt is een hydrolysaat dat voor ten minste ongeveer 80 mol% uit peptiden van maximaal 3 aminozuren bestaat en/of geen peptiden met een lengte van meer dan 6 aminozuren bevat. Het gehalte aan peptiden kleiner dan 0.5 dalton is bij voorbeeld 40-60%, waarbij het aandeel 15 vrije aminozuren bij voorkeur zo laag mogelijk is in verband met hun bittere smaak. Bijzonder geschikt is een hydrolysaat met een peptide-lengte-verdeling zoals beschreven in Terheggen- Lagro et al. (BMC Pediatrics (2002), 2: 10).
20 Met het oog op een optimale sluiting van de "tight junctions" en/of het voorkomen van een ongewenste darmpermeabiliteit (en daarmee verhoogde kans op allergie) kunnen bovendien één of meerdere niet-essentiële aminozuren worden toegevoegd die daarvoor een extra ondersteuning bieden. Zoals beschreven in WOOl/58283, NL-1025900 en NL-1027262, op 25 naam van aanvraagster, zijn hiertoe onder meer glutamine, glutaminezuur, arginine en proline geschikt. Glutaminezuur heeft hierbij de voorkeur boven glutamine aangezien glutamine gewoonlijk een verminderde stabiliteit heeft in producten en tevens, na enzymatische hydrolyse in de darm, kan bijdragen aan een ongewenste ammoniakbelasting.

Zoals hierboven vermeld bevat een vergaand gehydrolyseerde caseïnehydrolysaat waarin allergene peptiden afwezig zijn ook nauwelijks of geen tolerantie-inducerende peptiden meer. Teneinde de tolerantie· inducerende eigenschappen geheel of deels te ’herstellen' kan een 5 samenstelling volgens de uitvinding, naast de probiotica, tevens een tolerantie-inducerend melkeiwit bevatten, bij voorbeeld in de vorm van een melkeiwit- of wei-eiwithydrolysaat. Tolerantie-inductie uit zich gewoonlijk door de inductie en handhaving van een gebalanceerd immuunsysteem wanneer een organisme wordt belast met antigene epitopen. Dit komt bij 10 voorbeeld tot uitdrukking in de T-cel balans, zoals de relatieve hoeveelheden T-helpercellen (Thl, Th2 en Th3 cellen).
Bij voorkeur betreft het tolerantie-inducerend melkeiwit een melkvetarm partieel gehydrolyseerd wei-eiwithydrolysaat. In het bijzonder is het wei-15 eiwithydrolysaat rijk aan alfa-lactalbumine. Geschikte wei- eiwithydrolysaten hebben bij voorbeeld een hydrolyse graad (DH) welke kan variëren tussen ongeveer 5 en 25%. Hiermee wordt tevens bereikt dat het aminozuurpatroon van de samenstelling meer gaat lijken op die van moedermelk.

Als koolhydraatbron kan dienen elk type koolhydraat, of mengsel van verschillende koolhydraten, dat gewoonlijk in kindervoedingformuleringen wordt gebruikt. Geschikte koolhydraatbronnen zijn disacchariden zoals lactose en saccharose, monosacchariden, zoals glucose, en maltodextrinen, 25 zetmeel en koolhydraatbronnen met een prebiotische werking.
De lipidebron in een samenstelling volgens de uitvinding kan elk lipide of combinatie van lipiden zijn die geschikt is voor toepassing in kindervoeding. Voorbeelden van geschikte lipidebronnen zijn tri-, di- en monoglyceriden, 30 fosfolipiden, sfingolipiden, vetzuren, en esters of zouten daarvan. De lipiden 9 kunnen een dierlijke, plantaardige, microbiële of synthetische oorsprong hebben. Van bijzondere interesse zijn meervoudig onverzadigde vetzuren zoals gamma-linoleenzuur (GLA), di-homo-gamma-linoleenzuur (DHGLA), arachidonzuur (AA), stearidonzuur (SA), eicosapentaeenzuur (EPA), 5 docosahexaeenzuur (DHA), docosapentaeenzuur (DPA) en geconjugeerd linolzuur (CLA). CLA is van betekenis bij de bescherming tegen eczeem en respiratoire aandoeningen bij kinderen. Het betreft hier in het bijzonder de cis-9,trans-11 en trans-10, cis-12 isomeren van CLA. AA kan aan de samenstelling worden toegevoegd voor een optimale tolerantie-inductie.

Verder kan de samenstelling één of meer conventionele microingrediënten bevatten, zoals vitaminen, antioxidanten, mineralen, vrije aminozuren, nucleotiden, taurine, carnitine en polyaminen. Voorbeelden van geschikte antioxidanten zijn BHT, ascorbylpalmitaat, vitamine E, alfa- en beta-15 caroteen, luteïne, zeaxanthine, lycopeen en fosfolipiden.
In een uitvoeringsvorm betreft de samenstelling een voedingsmiddel of voedingsupplement voor zuigelingen. In een specifieke vorm betreft het een volledige zuigelingenvoeding tot aan de leeftijd van 6 maanden. In een 20 andere specifieke vorm betreft het een (opvolg-)zuigelingenvoeding voor zuigelingen met een leeftijd vanaf 6 maanden. Het eiwitgehalte van de samenstelling bedraagt dan gewoonlijk minder dan 3,5 gram per 100 kcal.
Zoals eerder genoemd beoogt de onderhavige uitvinding onder meer het 25 verschaffen van een hypoallergene samenstelling welke, ook op langere termijn, geschikt is voor kinderen met een gevoeligheid voor melkvet van ruminanten. Bij voorbeeld voor kinderen met IRD waarbij langdurig gebruik van genoemd melkvet leidt tot ophoping van fytaanzuur in het vetweefsel, in het bijzonder het zenuwweefsel.

Fytaanzuur in melkvet betreft een C20 vetzuur met vertakte ketens en is afkomstig uit chlorofyl, dat met het gras of gewonnen ruwvoer door ruminanten wordt opgenomen. Door een stoornis in de afbraak van fytaanzuur welke plaatsvindt in de peroxisomen, bij voorbeeld door 5 afwezigheid of inactiviteit van een of meerdere enzymen en/of afwezigheid of een tekort aan peroxisomen, hoopt fytaanzuur zich in het lichaam op.
Dit probleem is in het bijzonder bekend bij patiënten die lijden aan de ziekte van Refsum, een peroxisomale stofwisselingsziekte. Bij de infantiele vorm 10 van de ziekte van Refsum (ook wel "Phytanic Acid Storage Disease" (PASD) genoemd) is er sprake van een gebrek aan peroxisomen, waardoor stapeling ontstaat van verschillende vetzuren in bloed en weefsels. De eerste symptomen zijn al bij de geboorte aanwezig. Er is sprake van verstandelijke achteruitgang, licht afwijkende gelaatstrekken, doofheid, osteoporose en een 15 vertraagde groei. Door schade aan het netvlies ontstaat nachtblindheid en toenemende slechtziendheid (retinitis pigmentosa). De lever is vergroot, en het cholesterolgehalte in het bloed is verhoogd (hypercholesterolemie). Voor de ziekte van Refsum, infantiele vorm, is geen genezing mogelijk. De behandeling van de ziekte van Refsum bestaat uit het volgen van een strikt 20 fytaanzuur-arm dieet en richt zich voor zover mogelijk op verlichting van klachten.
In een specifieke uitvoeringsvorm verschaft de uitvinding een samenstelling geschikt als voeding of therapeutisch voedingssupplement voor kinderen die 25 een stoornis hebben in fytaanzuur metabolisme, zoals kinderen met de infantiele vorm van de ziekte van Refsum. Een dergelijke "IRD-samenstelling" omvat een fytaanzuurarme lipidebron, een koolhydraatbron en een a-allergeen (niet-allergeen) caseïnehydrolysaat met peptiden van maximaal 3000 dalton, met het kenmerk dat de samenstelling ten minste 30 één melkzuurproducerende probiotische bacteriestam bevat welke na 11 opname in het lichaam de tolerantie voor melkeiwitten kan bevorderen en/of ten minste één melkzuurproducerende probiotische bacteriestam bevat die de ammoniakbelasting kan verlagen. Bij voorkeur is de lipidebron fytaanzuurvrij. De samenstelling bevat bij voorbeeld ten hoogste 0.1 5 gewichtsprocent (gew%) fytaanzuurhoudend (koemelk)vet, bij voorkeur ten hoogste 0.05 gew%, bij grotere voorkeur ten hoogste gew 0.01%.
Het is aangetoond dat Refsum patiënten, behalve de ophoping van fytaanzuur, een verlaagd gehalte aan docosahexaeenzuur (DHA) en 10 arachidonzuur (AA) in hun circulatie hebben (Moser et al., Neurochem. Res. 1999, 187-197). Suppletie van DHA en AA via de voeding, in het bijzonder bij kinderen, wordt derhalve aangeraden. Daarom bevat een IRD-samenstelling bij voorkeur DHA en/of AA, bij grotere voorkeur zowel DHA als AA. DHA en AA zijn gevoelig voor oxidatie en daarom worden deze 15 langketenige meervoudig onverzadigde vetzuren bij voorkeur in combinatie met antioxidanten toegepast in een samenstelling volgens de uitvinding.
De uitvinding verschaft voorts de toepassing van een samenstelling volgens de uitvinding voor de bereiding van een voedingsmiddel of therapeutische 20 samenstelling voor het behandelen of voorkomen van symptomen die gepaard gaan met allergie voor melkeiwitten van ruminanten. Het te bereiden voedingsmiddel of de samenstelling is met name geschikt voor langdurig gebruik, bij voorkeur gedurende ten minste enige maanden.
25 De symptomen die gepaard gaan met allergie voor melkeiwitten van ruminanten zijn zeer divers en omvatten onder meer een vertraagde sluiting van de "tight junctions" van de darmwand, een disbalans in de darmflora en /of verhoogde ureumspiegels in het bloed.

Door haar verhoogde voedingswaarde kan een samenstelling volgens de uitvinding met voordeel worden toegepast ter verbetering van de groei van kinderen, in het bijzonder als (langdurig) therapeuticum voor kinderen met een allergie voor melkeiwitten van ruminanten of als (langdurige) profylaxe 5 voor kinderen die een verhoogde kans hebben om een dergelijke allergie te ontwikkelen.
Daarnaast kan door het gebruik van een samenstelling het remissiepercentage van kinderen met (kans op) allergie worden verhoogd.
10 Onder remissiepercentage wordt verstaan: het wegblijven van allergie bij langdurig gebruik, uitgedrukt als percentage van de gevolgde "probleemkinderen". Als referentiepunt bij deze parameter geldt dat het merendeel van de zuigelingen gevoed met moedermelk in de praktijk uiteindelijk over de allergie heen groeit, met ongeveer 50% in het eerste jaar, ongeveer 70% 15 in het tweede jaar en ongeveer 85% in het derde jaar. Met een samenstelling volgens de uitvinding kan het remissiepercentage na 1 jaar met ten minste 5% verbeterd worden, bij voorkeur met ten minste 10%.
Een a-allergene samenstelling volgens de uitvinding kan een gunstig effect 20 hebben op de ammoniakbelasting, de darmwandsluiting, het verhinderen van de hechting van pathogenen aan de darmwand en/of het herstel van de darmflora en is daarom bijzonder geschikt voor langdurig preventief gebruik waar het kinderen betreft met een allergische familieanamnese, dat wil zeggen kinderen met een verhoogde kans om allergie voor melkeiwitten te 25 ontwikkelen.
De uitvinding wordt geïllustreerd aan de hand van de volgende Voorbeelden.

Voorbeeld 1 Stimulering van de groei van melkzuurproducerende probiotica door vergaand gehvdrolvseerd caseïne-hvdrolvsaat.

Uitgaand van een minimaal medium voor de groei werd nagegaan of in aanmerking komende melkzuurproducerende probiotica zich wat betreft hun groei lieten stimuleren door een a-allergeen vergaand gehydrolyseerd caseïne-hydrolysaat. Het minimale medium omvatte per liter: 10 35 g lactose, 25 g natriumacetaat, 954 mg imidazol, 100 mg L-asparagine, 200 mg L-cystine, 200 mg DL-tryptofaan, 10 mg uracil, 10 mg guanine, 10 mg adenine, 10 mg xanthine, 200 mg alanine, 10 mg natriumchloride, 200 mg riboflavine, 200 mg thiamine HC1, 10 microgram foliumzuur, 600 15 microgram niacine, 1,2 mg pyridoxamine. 2 HC1, 200 microgram calciumpanthothenaat, 10 mg p-aminobenzoëzuur, 5 microgram biotine, 21 g L-aminozuren Sigma LAA21, 10 ml zoutoplossing.
Het caseïnehydrolysaat betrof het type, waarvan het molecuulgewicht- en 20 het peptiden-lengte-profïel werd gepubliceerd in BMC Pediatrics 2002 (2) 10.
Voor de voorkeursstammen L. paracasei ATCC-55544 en Bifidobacterium lactis ATCC-27536 werd een duidelijke prebiotische werking op de groei 25 uitgaand van genoemd caseïne-hydrolysaat vastgesteld.

Voorbeeld 2 Basisreceptuur voor een voedingsmiddel of therapeuticum voor kinderen, welke belast ziin met allergie of een verhoogde kans hebben om allergie te ontwikkelen.
5 Bereid werd een voeding op basis van vergaand gehydrolyseerde caseïne, glucosestroop, maltodextrinen, plantaardige oliën, arginine en micro-ingrediënten, tevens verrijkt met de probiotica Lactobacillus paracasei ATCC-55544 en Bifidobacterium lactis (ATCC-27536).
10 Het maximale molecuul gewicht van de peptiden in het caseïne- eiwithydrolysaat bedroeg 2,5 kDa. Het caseïne-hydrolysaat stimuleerde de groei van beide probiotica volgens de test naar voorbeeld 1.
De voeding bevatte 2 x 107 kolonievormende eenheden van beide probiotica 15 per gram poeder.
De samenstelling van de poedervormige probiotische hydrolysaatvoeding betrof per 100 gram poeder (alles in grammen): 20 Caseïnehydrolysaat 12

In een vergelijkende test met de voeding van voorbeeld 2, met en zonder de 30 toegevoegde probiotica bij kinderen belast met allergie, bleek de gunstige 15 werking van de probiotica op zowel de groei, de darmflora als het bereikte remissiepercentage.
De voeding verrijkt met beide cultures kan met voordeel en zonder 5 bijzondere risico's aanbevolen worden voor langdurig preventief gebruik bij kinderen met een allergische familieanamnese.
Voorbeeld 4 Basisreceptuur, tevens verrijkt met DHA-. AA- en CLA.
10 Bereid werd 100 gram hydrolysaat-voeding geschikt voor IRD-kinderen, bevattend L. paracasei ATCC-55544 + B. lactis ATCC-27536, met als samenstelling (alles in grammen):

Claims (18)
1. Samenstelling omvattende een lipidebron, een koolhydraatbron en een laag-allergeen caseïnehydrolysaat met peptiden van maximaal 3000 5 dalton, met het kenmerk dat de samenstelling ten minste één melkzuurproducerende probiotische bacteriestam bevat welke de tolerantie voor melkeiwitten kan bevorderen en/of ten minste één melkzuurproducerende probiotische bacteriestam welke de ammoniakbelasting kan verlagen. 10
2. Samenstelling volgens conclusie 1, waarin het caseïnehydrolysaat een a-allergeen vetarm vergaand caseïnehydrolysaat betreft.
3. Samenstelling volgens conclusie 1 of 2, waarbij de tolerantie-15 verhogende en/of de ammoniak-verlagende melkzuurproducerende probiotische bacteriestam zich in zijn groei laat stimuleren door een vergaand gehydrolyseerd caseïne-eiwit.
4. Samenstelling volgens één der conclusies 1-3, waarbij de 20 ammoniak-verlagende probiotische bacteriestam is gekozen uit Bifidobacterium lactis stammen, bij voorkeur de B. lactis stam gedeponeerd onder ATCC nummer 27536.
5. Samenstelling volgens één der conclusies 1-4, waarbij de tolerantie-25 verhogende probiotische bacteriestam is gekozen uit Lactobacillus casei en Lactobacillus paracasei stammen.
6. Samenstelling volgens conclusie 5, waarbij de tolerantie verhogende probiotische bacteriestam een L. paracasei stam is, bij voorkeur

7. Samenstelling volgens één der conclusies 1-6, waarbij de samenstelling minimaal 106 - 107 kolonievormende eenheden per bacteriestam bevat.
8. Samenstelling volgens één der conclusies 1-7, waarbij de samenstelling voorts een tolerantie-inducerend melkeiwit of melkeiwithydrolysaat omvat.
9. Samenstelling volgens conclusie 8, waarbij het melkeiwit 10 (hydrolysaat) een wei-eiwit(hydrolysaat) betreft, bij voorkeur melkvet-arm alfa-lactalbumine-hydrolysaat.
10. Samenstelling volgens één der conclusies 1-9, waarbij de samenstelling een aanvullende component bevat die bijdraagt aan het 15 verwerken van ammoniak, bij voorkeur arginine.
11. Samenstelling volgens één der conclusies 1-10, waarbij lipidebron één of meerdere componenten betreft gekozen uit tri-, di- en monoglyceriden, fosfolipiden en sflngolipiden, vetzuren, en esters of zouten 20 daarvan,

Samenstelling volgens één der conclusies 1-11, waarbij de samenstelling ten minste één meervoudig onverzadigd vetzuur bevat, bij voorkeur gekozen uit gamma-linoleenzuur (GLA), di-homo-gamma- 25 linoleenzuur (DHGLA), arachidonzuur (AA), stearidonzuur (SA), eicosapentaeenzuur (EPA), docosahexaeenzuur (DHA), docosapentaeenzuur (DPA) en geconjugeerd linolzuur (CLA), bij grotere voorkeur ten minste DHA en AA.
13. Samenstelling volgens één der conclusies 1-12, omvattende een prebioticum, bij voorkeur gekozen uit fructo- en/of galacto-oligosacchariden met korte of lange ketens, inuline, fucose-bevattende oligosacchariden, beta-glycanen, johannesbroodpitmeel, gommen, pectine, sialyloligosacchariden, 5 sialyllactose, galactanen met korte of lange ketens en nucleotiden.
14. Samenstelling volgens één der conclusies 1-13, omvattende de combinatie van ten minste één Bifidobacterium lactis stam en één of meer prebiotica voor deze stam, bij voorkeur gekozen uit nucleotiden, sialyl- 10 oligosacchariden en siaallactose.
15. Samenstelling volgens één der conclusies 1-14, waarbij de samenstelling maximaal 0.1 gew% fytaanzuurhoudend vet bevat, betrokken op de uiteindelijke samenstelling, bij voorkeur maximaal 0.05 gew%, bij 15 grotere voorkeur maximaal 0.01 gew%.
16. De toepassing van een samenstelling volgens één der conclusies 1-15 voor de bereiding van een voedingsmiddel of therapeutische samenstelling voor het behandelen of voorkomen van symptomen die 20 gepaard gaan met allergie voor melkeiwitten van ruminanten.
17. De toepassing van een samenstelling volgens conclusie 1-15 voor de bereiding van een voedingsmiddel of therapeutische samenstelling voor kinderen met een stoornis in fytaanzuurmetabolisme, bij voorkeur voor 25 kinderen met de ziekte van Refsum.
18. Toepassing volgens conclusie 16 of 17, waarbij het voedingsmiddel of de samenstelling bedoeld is voor langdurig gebruik, bij voorkeur gedurende ten minste 3-6 maanden. 30 1032840
