Abstract

The invention relates to medicine, namely to laparoscopic surgery, and can be used for treating acute or chronic cholecystitis with a pronounced adhesive process.Summary of the invention consists in that it is performed an oblique incision of a length of 2.0…2.5 cm of the skin and subcutaneous tissue 2 cm lower from the right costal edge along the medioclavicular line, and a 1.5 cm incision is performed on the aponeurosis, then with a blunt instrument is penetrated into the abdominal cavity and is introduced the trocar cannula, after which it is sealed with a purse-string suture, is temporarily introduced the optical device to view the adhesive process and determine the final approach place of the optical device 6…10 cm from the middle line on the right flank, in the first approach is introduced the first working instrument and is performed the adhesiolysis as necessary, depending on the spread of the adhesive process with the preparation of approaches to the gallbladder for the 2nd and 4th working instruments, then is mobilized the gallbladder, is ligated and cut off the bile duct and bile artery, is performed the cholecystectomy, after which is weakened the purse-string suture and removed the gallbladder, then the trocar, is closed the wound by tightening the purse-string suture, are removed the remaining instruments and the wounds are sutured in layers.
