SOLUTION STABLE D'ERYTHROMYCINE L'érythromycine a une stabilité limitée en solution aqueuse selon Chemical Abstracts, 60, 14337 (1964). Le brevet des E.U.A. N° 4.000.263 décrit une solution stable 5 d'érythromycine contenant du propylèneglycol, de l'alcool * éthylique et un éther éthoxylé d'alcool laurylique (Brij-30).
La quantité de propylèneglycol varie de 30 à 45 %. Selon le - brevet des E.U.A. N° 4.000.263, les solutions d'érythromycine décrites perdent environ 5 % de leur stabilité en 10 semaines 1Q à 45°C ou en 18 semaines à 37°C.
Mills, dans Cutis, 15, 93 (1975), décrit l'utilisation d'une solution à 2 % d'érythromycine dans un mélange solvant 1:1 d'éthanol et d'eau, pour le traitement de l'acné. J.E. Fulton Jr., Dermat, 110, 83 (1974) utilise une ]_5 solution ä 1 % d'érythromycine dans un solvant contenant deux parties d'éthanol, deux parties d'éther monomëthylique d'éthylèneglycol et une partie de propylèneglycol. L'auteur indique que cette préparation est tout à fait efficace pour le traitement de l'acné.
20 Les antibiotiques, y compris les éléments de la famille de la lincomycine, la tétracycline et 1'épi-tétracycline, ont été utilisés pour traiter l'acné ; voir les brevets des E.U.A. N° 3.969.516 et 3.952.099. Ce dernier brevet concerne plus particulièrement la pénétration accrue de 25 l'antibiotique dans la peau par addition de décylméthyl-sulfoxyde à une solution alcoolique de l'antibiotique.
L'invention fournit une composition pharmaceutique comprenant par ml de 1 à 100 mg de l'ingrédient actif, l'érythromycine,et des excipients, l'éthanol, un acide non 30 toxique en quantité suffisante pour donner un pH de 8 à 10 après dilution avec de l'eau, et du propylèneglycol en quantité suffisante pour amener le volume à 1 ml.
" L'invention fournit également un procédé de préparation d'une composition pharmaceutique contenant comme 35 ingrédient actif de l’érythromycine, procédé qui consiste à dissoudre, par ml de volume final, 1 à 100 mg d'érythromycine dans 70 % du volume final d'éthanol, à ajouter suffisamment d'acide non toxique pour obtenir un pH compris entre 8 et 10 2 après dilution avec de l’eau, et à ajouter suffisamment de propylèneglycol pour atteindre le volume final.
La concentration préférée de l'érythromycine dans la solution est de 20 mg/ml. Dans la préparation de la 5 solution, on ajoute suffisamment d'acide pour obtenir un pH . final compris entre 8 et 10 par dissolution d'une partie aliquote dans de l’eau et détermination du pH sur un pH-mètre « équipé d'une électrode de verre. Tout acide non toxique pharmaceutiquement acceptable peut être utilisé mais l'acide 10 préféré est l'acide citrique et l'on a trouvé qu'une concentration finale de 0,04 S d'acide citrique (0,4 mg/ml) donne une solution qui, par dilution dans l’eau, donne un pH d'environ 9 sur une électrode de verre.
Pour préparer la solution d'érythromycine de cette 15 invention, il est courant de dissoudre l'érythromycine-base et l'acide organique, de préférence l’acide citrique, dans de l'éthanol puis d'ajouter suffisamment de propylèneglycol pour amener la solution finale à 1 ml ou un de ses multiples. On mélange ensuite soigneusement les ingrédients.
20 La quantité de propylèneglycol présente doit être suffisante pour maintenir l'érythromycine en solution et disponible pour l'absorption dans la peau mais doit être maintenue à un minimum en raison des problèmes d'irritation.
Une double épreuve ä l'insu a été effectuée sur des 25 humains souffrant d'acné, oü l'on utilise comme témoin le véhicule lui-même sans érythromycine. Les patients recevant une composition préparée selon l'invention présentent une diminution de 56 % du nombre des papules pendant la période d'essai tandis que dans le groupe témoin qui reçoit le véhicule de la 30 composition seule, il y a seulement une diminution de 31 % des papules. Le nombre de patients utilisés était suffisant pour que la différence soit statistiquement significative. i Les solutions de cette invention sont très stables.
Par exemple, une solution contenant 22 mg d'érythromycine 35 (excès de 10 %), 0,7 ml d'éthanol USP, suffisamment d'acide citrique pour obtenir une concentration finale de 0,04 % et du propylèneglycol en quantité suffisante pour faire 1 ml, s’est révélée titrer 83,2 % de l'antibiotique initial (92,9 % de ce 3 » * qui est indiqué sur l'étiquette) à 25°C après 30 mois de conservation. D'autres solutions confirment cette tendance à la stabilité. Bien que la quantité de propylèneglycol présente soit à la limite de la quantité qui peut irriter la peau 5 extérieurement et intérieurement, les quantités inférieures donnent une poudre par évaporation de l'éthanol, composition qui est alors évidemment non satisfaisante car l'érythromycine , doit être en solution pour traiter les papules d'acné.

Claims (8)
1. Composition pharmaceutique, caractérisée en ce qu'elle contient par ml de 1 à 100 mg de l'ingrédient actif, l'érythromycine, et les excipients suivants : éthanol, 5 suffisamment d'acide non toxique pour obtenir un pH compris entre 8 et 10 après dilution avec de l'eau , et suffisamment - de propylèneglycol pour porter le volume à 1 ml.
2. Composition pharmaceutique selon la * revendication 1, caractérisée en ce que l'acide non toxique 10 est l'acide citrique.
3. Composition pharmaceutique selon la revendication 1, caractérisée en Ge que l'acide non toxique „ est l'acide citrique et que la quantité d'acide citrique présente est d'environ 0,4 mg/ml.
4. Composition pharmaceutique selon la revendication 1, caractérisée en ce que sont présents 20 mg d'érythromycine par ml.
5. Composition pharmaceutique selon la revendication 1, caractérisée en ce qu'elle contient 0,7 ml d'éthanol par 20 ml.
6. Procédé de préparation d'une composition pharmaceutique comportant comme ingrédient actif de l'érythromycine, caractérisé en ce qu'on dissout, par ml de volume final, 1 à 100 mg d'érythromycine dans 70% du volume 25 final d'éthanol ; on ajoute suffisamment d'acide non toxique * pour obtenir un pH compris entre 8 et 10 après dilution avec de l'eau ; et l'on ajoute suffisamment de propylèneglycol pour atteindre le volume final.
7. Procédé selon la revendication 6, caractérisé en 30 ce que l'acide non toxique est l'acide citrique.
8. Procédé selon la revendication 6, caractérisé en ce qu'on dissout 20 mg d'érythromycine par ml de volume final. j v
