Abstract

중합체 피복물이 스텐트 격자 틈새를 폐색시키지 않고 와이어의 교차점에 결합하거나, 또는 지그-재그 스텐트의 경우에는 인접한 와이어의 지그-재그에 결합하도록 중합체로 스텐트를 피복한다. 적합한 중합체로는 폴리우레탄, 폴리카보네이트 우레탄, 폴리우레탄 우레아, 실리콘 고무, 폴리이소부틸렌 공중합체(스티렌등과의 공중합체), 폴리올레핀, 폴리에스테르, 클리콜화된 폴리에스테르, 폴리아미드, 무정형 폴리아미드, 이들의 혼합물 등이 포함된다. 폴리이소부테레이트, 폴리 발렐레이트, 폴리락트산, 폴리클리콜산 및 이들의 혼합물과 같은 생분해가능한 중합체도 또한 적합하다. 중합체는 이성분 풀리우레탄 또는 실리콘 고무와 같이 용매 없이 그 장소에서 반응되거나, 또는 반응된 중합체는 적절한 용매, 예를 들면 폴리우레탄의 경우 디메틸아세타미드, 폴리오레핀의 경우 톨루엔, 또는 실리콘 고무의 경우 헵탄에 용해될 수 있다. 스텐트 와이어에 중합체가 결합되는 것을 증강시키기 위해 피복하기전에 먼저 금속 스텐트를 하도할 수 있다. 교차점이 고정되어 서로에 대한 스텐트 와이어들의 자유로운 운동이 저해됨으로써, 중합체 피복된 스텐트의 후프(hoop) 강도가 개선된다.

개선된 후프 강도를 갖는 스텐트 및 스텐트-이식편 및 이의 제조 방법

제1도는 응력을 받지않는 위치에서 팽창되는 종래의 브레이드(braid) 스텐트의 파쇄면 입면도이다, 제1a도는 제1도의 선 1A-1A를 따른 단면도이다, 제2도는 연신 및 수출된 제1도 및 제ㅣa의 종래 분야 스텐트의 파쇄면 입면도이다, 제2a도는 제2도의 선 2A-2A를 따른 단면도이다.

Claims (41)

  1. 와이어 필라멘트간의 틈새를 한정하는 교차점을 갖는 다수의 와이어 필라멘트로부터 형성된 방사상으로 및 축방향으로 가요성이며 실질적으로 원통형인 몸체를 포함하고, 상기 와이어 필라멘트가 교차점에서 중합체로 피복되어 와이어 필라멘트는 교차점에서 중합체에 의해 서로 결합되고 격자 틈새는 중합체에 의해 실질적으로 폐색되지 않도록 함을 특지으로 하는 보철물.
  2. 제1항에 있어서, 중합체가 80A 내지 100D의 쇼어 경도를 갖는 방향족 폴리카보네이트 우레탄인 보철물.
  3. 제2항에 있어서, 중합체가 55D 내지 75D의 쇼어 경도를 갖는 보철물.
  4. 제1항에 있어서, 필라멘트가 실질적으로 전체 길이를 따라 중합체로 피복된 보철물.
  5. 제1항에 있어서, 필라멘트 및 중합체가 피복된 몸체의 후프 강도가 중합체가 없는 유사한 몸체의 제2후프 강도보다 25% 이상 증가되도록 선택되는 보철물.
  6. 제1항에 있어서, 필라멘트가 여러층의 중합체로 피복된 보철물.
  7. 제1항에 있어서, 중합체가 약물 용출 중합체인 보철물.
  8. 제1항에 있어서, 몸체에 부착된 다공성 혈공성 혈관 이식편을 더욱 포함하는 보철물.
  9. 제8항에 있어서, 다공성 혈관 이식편이, 약 160℃의 융점을 갖는 스펀 폴리카보네이트 우레탄 라이너이고, 중합체가 약 240℃의 융점을 갖는 폴리카보네이트 유레탄인 보철물.
  10. 제8항에 있어서, 다공성 혈관 이식편이, 하나는 몸체의 내부에 위치되고 다른 하나는 몸체위에 위치되며 격자 틈새를 통해 서로 결합되는 2개의 스펀 폴리카보네이트 우레탄 라이너인 보철물.
  11. a) 필라멘트간의 격작 틈새를 한정하는 교차점을 갖는 다수의 필라멘트로부터 형성된 방사상으로 및 축방향으로 가요성이며 실질적으로 원통형인 몸체를 수득하는 단계, b) 중합체 용액을 교차점에 도포하는 단계, 및 c) 교차 필라멘트가 중합체에 의해 서로 결합되고 격자 틈새가 중합체에 의해 실질적으로 폐색되지않도록 중합체 용액을 경화시키는 단계를 포함하는 보철물 제조 방법.
  12. 제11항에 있어서, 중합체 용액이 고형분 약 5중량%의 농도로 디메틸아세타미드중에 용애된 약 55D 쇼어 경도의 폴리카보네이트 우레탄을 함유하는 방법.
  13. 제12항에 있어서, 중합체 용액을 필라멘트상에 분무하고 건조시킴으로써 경화시키는 방법.
  14. 제13항에 있어서, 단계 b) 및 c)를 5 내지 15회 반복하는 방법.
  15. 제11항에 있어서, 중합체 용액이 클로로포름중에 용해된 폴리부테레이트 및 폴리발레레이트의 생분해 가능한 혼합물을 함유하는 방법.
  16. 제11항에 있어서, d) 다공성 이식편을 필라멘트에 부착하여 스텐트-이식편을 형성하는 단계를 더욱 포함시키는 방법.
  17. 제16항에 있어서, 약 160℃의 융점을 갖는 폴리카보네이트 우레탄을 회전 맨드릴상에 방사하고, 이식편을 경화시키고, 경화된 이식편상에 제1추가 폴리카보네이트 우레탄을 방사하고, 스텐트를 이식편상에 위치시키고, 스텐트-이식편 조립체를 경화시킴으로써 다공성 이식편을 형성시키는 방법.
  18. 제17항에 있어서, 제2추가 폴리카보네이트 우레탄을 스텐트-이식편 조립체상에 방사시키고, 제2추가 폴리카보네이트 우레탄을 틈새를 통해 압착시켜 제2추가 폴리카보네이트 우레탄을 제1추가 폴리카보네이트 우레탄에 결합시킴으로써 이식편을 또한 형성하는 방법.
  19. 제14항에 있어서, d) 인지질 박층이 중합체 표면에 결합하도록 경화된 중합체를 인지질을 함유한 용액으로 피복시키는 단계를 더욱 포함하는 방법.
  20. 제11항에 있어서, 중합체 용액이 약물 5-플루오로우라신 약 10중량%, 및 디메틸 아세타미드중에 용해된(된 약 50D의 쇼어 경도를 갖는 폴리카보네이트 우레탄이 약 5중량%를 함유하여 라커를 형성하는 방법.
  21. 제11항에 있어서, d) 중합체 용액을 도포하기전에, 제1말단이 활짝 펴지도록 실질적으로 원통형인 몸체의 적어도 제1말단에 원추형 삽입체를 밀어넣는 단계를 더욱 포함하는 방법.
  22. 제11항에 있어서, d) 중합체 용액을 경화된 후에, 제1말단이 활짝 펴지도록 실질적으로 원통형인 몸체의 적어도 제1말단에 원추형 삽입체를 밀어넣는 단계를 더욱 포함하는 방법.
  23. 제22항에 있어서, e) 제1말단이 활짝 펴진 상태인 실질적으로 원통형인 몸체를 중합체가 부분적으로 용융될 때까지 약 170 내지 200℃로 가열는 단계, 및 f) 실질적으로 원통형인 몸체 및 맨드릴을 실온으로 냉각시키는 단계를 더욱 포함하는 방법.
  24. 제11항에 있어서, d) 중합체 용액을 도포하기전에, 실질적으로 97%/5%인 에탄올과 물의 혼합물 중에 용해된 2% 아미노프로필아미노에틸트리메톡시실란의 하도 용액중에 실질적으로 원통형인 몸체를 침지시키는 단계, 및 e) 중합체 용액을 도포하기전에 실질적으로 원통형인 몸체를 건조시키는 단계를 더욱 포함하는 방법.
  25. 제24항에 있어서, 중합체 용액이 약 73D 경도를 갖는 약 6중량%의 폴리카보네이트 우레탄 약 47중량%의 디메틸아세타미드, 및 약 47중량%의 테트라하이드로푸린을 함유하는 방법.
  26. 나선형으로 감긴 지그-재그 필라멘트로부터 형성된 방사상으로 및 축방향으로 가요성인 실질적으로 원통형인 몸체를 포함하고, 인접한 지그-재그가 격자 틈새를 한정하고, 상기 필라멘트가 지그-재그에서 중합체로 피복되어 인접한 지그-재그가 중합체에 의해 서로 결합되고 격자 틈새가 중합체에 의해 실질적으로 폐색되지 않도록함을 특징으로 하는 보철물.
  27. 제26항에 있어서, 중합체가 80A 내지 100D의 쇼어 경도를 갖는 방향족 폴리카보네이트 우레탄인 보철물.
  28. 제26항에 있어서, 필라멘트가 실질적으로 전체 길이를 따라 중합체로 피복된 보철물.
  29. 제26항에 있어서, 필라멘트 및 중합체가, 중합체로 피복된 몸체의 후프 강도가 중합체가 없는 유사한 몸체의 제2후프 강도보다 25% 이상 증가되도록 선택되는 보철물.
  30. 제26항에 있어서, 필라멘트가 여러 층의 중합체로 피복된 보철물.
  31. 제26항에 있어서, 중합체가 약물 용출 중합체인 보철물.
  32. 제26항에 있어서, 몸에 부착된 다공성 혈관 이식편을 더욱 포함하는 보철물.
  33. 제32항에 있어서, 다공성 혈관 이식편이 약 160℃의 융점을 갖는 스펀 폴리카보네이트 우레탄 라이너인 보철물.
  34. 제32항에 있어서, 다공성 혈관 이식편이, 하나는 몸체의 내부에 위치되고 다른 하나는 몸체위에 위치되며, 격자 틈새를 통해 서로 결합되는 2개으 스펀 폴리카보네이트 우레탄 라이너인 보철물.
  35. 제26항에 있어서, 필라멘트가 탄탈륨 와이어이고, 중합체가 약 420℃의 융점을 갖는 FEP 중합체이고 실질적으로 필라멘트의 전체 길리를 따라 필라멘트상으로 압출되는 보철물.
  36. a) 지그-재그의 인접한 부분이 격자 틈새를 한정하는, 나선형으로 감긴 지그-재그 필라멘트로부터 형성된 방사상으로 및 축방향으로 가요성인 실질적인 원통형인 몸체를 수득하는 단계, b) 중합체 용액을 지그-재그의 인접한 부분에 도포하는 단계, 및 c) 지그-재그의 인접한 부분이 중합체에 의해 서로 결합되고 격자 틈새가 중합체에 의해 실질적으로 폐색되지 않도록 중합체 용액을 경화시키는 단계를 포함하는 보철물 제조 방법.
  37. 제36항에 있어서, 중합체 용액이 디메틸아세타미드중에 용해된 폴리카보네이트 우레탄인 방법.
  38. 제36항에 있어서, d) 중합체 용액을 도포하기전에, 몸체를 지그-재그의 인접한 부분이 실질적으로 서로접촉하고 있는 맨드릴싱에 위치시키는 단계를 더욱 포함하는 방법.
  39. a) 피복된 와이어 필라멘트를 형성하기 위해 와이어 필라메트를 압출 다이를 통해 연신하여 중합체 용액을 와이어 필라멘트로 도포시키는 단계, b) 피복된 와이어 필라멘트로 피복된 와이어 필라멘트가 특정 위치에서 서로 인접하는 실질적으로 원통형인 몸체로 성형하는 단계, 및 c) 피복된 와이어 필라멘트가 특정 위치에서 서로 접착하여 비-폐색된 격자 틈새를 한정하도록 중합체 용액을 경화시키는 단계를 포함하는 보철물 제조 방법.
  40. 제39항에 있어서, 중합체 용액이 약 420℃의 융점을 갖는 FEP 중합체이고, 경화 단계가 약 420℃로 가열한 후 냉각시킴을 포함하는 방법.
  41. 제40항에 있어서, d) 실질적으로 원통형인 몸체를 FEP 중합체의 항복 강도를 넘어서까지 팽창시키는 단계를 더욱 포함하는 방법.
