Abstract

Description
본 발명은 신경퇴행성 장애에 대한 치료법의 분야에 관한 것이다. 특히, 본 발명은 폴리글루타민(polyQ) 질병을 줄기 세포로 치료하기 위한 치료 요법(therapeutic regimen)에 관한 것이다.
운동 실조증(ataxia)은 소뇌, 뇌간, 및 척수소뇌로에 다양하게 영향을 미치는 임상적으로 및 유전적으로 이질적인 신경퇴행성 질병의 군이다. 척수소뇌 운동 실조증(SCA)은 진행적, 퇴행적, 및 치명적이다. SCA는 병리학적 변화의 주요 위치(locus)가 소뇌, 뇌간 또는 척수의 핵 또는 뉴런 경로에 존재하는 뉴런 조직의 퇴행을 포함한다. SCA의 태아 상태는 광범위한 뉴런 손실뿐만 아니라, 말기 질병에서 빈약한 호흡 부전에서 비롯된다. 가장 일반적인 SCA 유전 서브타입은 폴리-글루타민(polyQ) 매개 SCA, 즉, SCA1, SCA2, SCA3, SCA6, SCA7 및 SCA17이다.

그러나, SCA에 대한 효과적인 의학적 치료 또는 잠재적인 치유법이 존재하지 않는다. 이에 따라, SCA의 징후 및 증상을 완화시키기 위한 치료 방법이 요구된다.
본 발명은 대상체에 단위 투여량(unit dosage)으로서 유효량의 줄기 세포를 비경구적으로 또는 국소적으로 투여하는 것을 포함하는, 대상체에서 폴리글루타민(polyQ) 질병을 치료하는 방법으로서, 투여는 1회 이상의 치료 사이클로 수행되며, 여기서, 1회 치료 사이클은 각각 2 내지 6주의 투약 간격(dosing interval)에서 3회의 단위 투여량을 투약하는 것을 포함하는 방법을 제공한다.
일부 구현예에서, polyQ 질병은 척수소뇌 운동 실조증(SCA); 마카도-조셉병(MJD/SCA3); 헌팅턴병(HD); 치상핵적핵담창구시상하핵 위축증(DRPLA); 및 척수 및 연수 근육 위축증, X-연관 1(SMAX1/SBMA)을 포함하지만, 이로 제한되지 않는다. 일 구현예에서, SCA는 SCA1, SCA2, SCA3, SCA6, SCA7 또는 SCA17이다.

도 8은 IV 주입을 통한 두개내 국소화의 능력을 갖는 QPSC를 도시한 것이다. QPSC는 꼬리 정맥 주사를 통해 야생형 마우스 내에 이식되었으며, 뇌 조직은 정량적 실시간 RT-PCR 분석을 위해 이식 후 7일에 제거되었다. 인간 DNA(인간 β2 마이크로글로불린에 의해 검출됨) 대 마우스 DNA(마우스 18s rRNA에 의해 검출됨)의 비율은 대략 0.8%(번호 1 마우스) 내지 2.8%(번호 4 마우스)이었다.

도 12a 내지 도 12c는 다발성 파라크린 신경 영양 인자 및 조직 성장 인자를 발현시킴을 도시한 것이다. QPSC에서 NT-3, NT-4, NGF, CNTF, BDNF 및 GDNF를 포함하는 신경 영양 인자의 유전자 발현은 내부 대조군 18srRNA 유전자(도 12a)에 대한 정량적 PCR(qPCR)에 의해 검출되었다. 조직 인자, 예를 들어, QPSC에서 EGF, FGF-β 및 VEGF(도 12b) 및 PDGF 및 TGF-β1(도 12c)은 또한, ELISA에 의해 시험되었으며, 세포내 분획과 분비 분획 사이의 그러한 인자의 상이한 농도가 입증되었다.

ADMSC를 세포외 배양 확대시키고, PIC/S Good Manufactory Practice 가이드라인에 따라 확립된 세포 설비에서 SBI의 표준 작업 절차에 따라 품질 관리하였다. 간단하게, 지방 조직을 건강한 공여체로부터 수확하고, 저온(0 내지 5℃)에서 SBI의 가공 설비로 즉시 옮겼다. ADMSC를 단리시키고, 정제하고, 배양 확대 동안 SBI의 독점 배양 배지에서 유지시켰다. 12회의 계대배양된 ADMSC는 고도로 발현된 CD273, CD46, CD55 및 CXCR4를 갖는 QPSC이었고, 이후에, 냉동보존 백에서 패킹되었고, 생성물(Stemchymal®)은 품질 보증 및 냉동보존을 위해 송부되었다. 세포 투약
유의미한 운동 기능 저하가 없는 마우스를 이용한 연구를 위하여:
2.5×107개 세포/kg 체중의 Stemchymal®을 해동시키고, 준비하고, 1 ml 인슐린 시린지(29 1/2G) 내에 로딩하였다. 세포를 해동 1시간 내에 서서히(15 내지 20초 기간) 주사하였다.
유의미한 운동 기능 저하를 갖는 마우스를 이용한 연구를 위하여:
마우스를 4개의 그룹으로 무작위적으로 분배하였다: (1) SCA3+세포, (2) SCA3+PBS, (3) Wt+세포, (4) Wt+PBS. 2.5×107개 세포/kg 체중의 Stemchymal®을 그룹 1 및 그룹 3의 각 마우스에 정맥내로 주입하였다. 총 125 ㎕의 세포 현탁액(냉동용액(Biolife) 중 1:1) 또는 PBS(Gibco)를 각 마우스에 투여하였다.
두 연구 모두에 대하여, 동물을 4시간 동안 모니터링하고, 주사 후에 매일 관찰하였다. 세포 투약을 2주마다 1회 수행하고, 총 3회 수행하였다.
데이터 수집 및 분석
마우스를 마지막 시험 물품 주사 후 1개월에 희생시켰다. 마우스의 체중 및 낙하 지연시간(latency to fall)을 전체 연구 동안 기록하였다. 마우스 발자국을 또한 시험 물품 주사 후 보행 수행에 대해 분석하였다. 마우스 조직(피질, 소뇌, 심장, 신장, 간, 비장, 폐 및 꼬리)를 미래 병리조직학적 분석 및 생물-분류 연구를 위해 수집하였다.
통계학
데이터는 평균+SEM로서 제시된다. 로타로드 및 발자국 시험의 결과를 p < 0.05의 유의성 임계값과 함께 스튜던트 t-테스트를 이용하여 분석하였다.
운동 협응 및 균형 검정
운동 협응 및 균형을 로타로드 장비(MK-670, Muromachi Kikai Co., Ltd., Japan)에서 평가하였다. 마우스를 5분의 시간에 걸쳐 40 rpm까지 가속화되는 일정 속도(4 rpm)에서의 로타로드 상에 배치하였다. 낙하하거나 완전 수동 회전(full passive rotation)(완전 회전을 위해 봉(rod)에 달라붙음)을 완료하기 위한 지연시간(latency)을 기록하였다. 마우스를 각 시험 동안 3회 시험을 수행하였고, 시험 사이에 15분 휴지하였다. 각 시험으로부터 각 마우스에 대한 평균 지연시간을 계산하였다. 시험 결과를 t-테스트를 이용하여 통계학적으로 분석하였다.
SHIRPA 시험
변형된 SHIRPA 시험을 20, 24 및 28주령의 마우스에서 수행하였다. SHIRPA 프로토콜을 RIKEN BRC의 변형된 SHIRA 프로토콜로부터 변형하였다. 시험 항목 및 산정 기준은 하기 표에 나열되어 있다.
마우스를 이의 행동에 따라 산정하였다. 연구 그룹에서 전체 마우스 수를 100%로서 계산하였다. 결과를 특정 산정 수준과 함께 마우스 수의 백분율로서 나타내었다.
발자국 분석
마우스 발자국을 마지막 세포 투약 후 대략 1개월에 분석하였다. 2015년에 출판된 논문(1)을 참조하여, 마우스 발을 잉크(앞발: 적색, 뒷발: 녹색)에 담그고, 이에 따라, 마우스는 마우스가 복도를 따라 목표 박스(goal box)까지 걷거나 뛸 때 발자국의 흔적을 남기게 한다. 마우스를 터널 앞에서 한 장의 페이퍼(50 cm 길이, 10 cm 폭) 상에 놓았다. 보폭, 흔들림, 자세 길이, 및 앞뒤발 겹침의 측정은 보행의 징후를 제공한다(하기 도면 참조). 모든 마우스를 희생 전에 3회 실행(run)을 제공하였다.

T 세포 단리
인간 말초 혈액 단핵 세포(PBMC)를 Histopaque-1077(Sigma-Aldrich) 밀도 구배 원심분리에 의해 건강한 공여체로부터의 헤파린처리된 전혈로부터 단리하였다. CD3+ T 림프구를 이후에, 제조업체 설명서에 따라 항-인간 CD3 항체 컨쥬게이션된 자성 입자(BD Biosciences)를 사용하여 PBMC로부터의 양성 선택(positive selection)에 의해 정제하였다.
T 세포 증식 검정
정제된 인간 CD3+ T 세포(1×105개 세포)를 96-웰 플레이트에서 플레이트-결합된 항-CD3(2 ㎍/㎖) 및 항-CD28(2 ㎍/㎖) 모노클로날 항체(BD Biosciences)로 자극시키고, 10% 우태아혈청(FBS), 2 mM 1-글루타민, 100 U/ml 페니실린, 100 U/ml 스트렙토마이신 및 25 mM HEPES를 함유한 RPMI-1640 배지(Gibco)에서 상이한 수의 ADMSC와 동시-배양하였다. 48시간 후에, 5-브로모-2-데옥시우리딘(BrdU)을 각 웰에 첨가하고, 플레이트를 T 세포 증식의 측정을 위해 다른 18시간 동안 인큐베이션하였다. T 세포 내에 도입된 BrdU의 양을 제조업체 설명서에 따라 Cell Proliferation ELISA, BrdU 키트(Roche)를 이용하여 측정하였다.
면역조직화학(IHC)
QPSC의 신경보호 효과를 평가하기 위해, QPSC를 C57BL/6J SCA2 형질전환 마우스의 꼬리 정맥을 통해(IV hMSC-Tg 그룹) 또는 대후두공을 통해 소뇌의 위치내로(IC hMSC-Tg 그룹) 주사하였다. IHC에 의해 뮤린 뇌 조직에서의 인간 세포를 입증하기 위하여 인간 베타2 마이크로글로불린(Abcam, code: ab15976)과 반응한 특정 항체를 선택하였다. 뮤린 섹션(4 ㎛)을 절단하고, 현미경 슬라이드 상에 마운팅하였다. 자일렌, 100% 에탄올, 95% 에탄올 및 80% 에탄올 중에서 5분 간격으로 2회 세정함으로써 섹션을 탈수시켰다. 파라핀제거 후에, 섹션을 퍼옥시다아제 비활성화를 위해 3% H2O2로 처리하고, 항원 회수를 위해 10 mM 시트레이트 완충제(0.05% Tween20)로 처리하고, 1% 차단 용액(문헌[Chang et al. Journal of Biomedical Science 2011, 18:54 http://www.jbiomedsci.com/content/18/1/54 Page 3 of 9 PBS]에서 1% BSA 및 0.1% Triton X-100)으로 차단하였다. 섹션을 실온에서 40분 동안 차단 용액 중에 희석된(1:400) 특정 항-인간 b2 마이크로글로불린 폴리클로날 항체(Abcam)와 함께 인큐베이션하였다. PBS로의 광범위한 세척 후에, 섹션을 실온에서 40분 동안 차단 용액 중에 희석된(1:1000) 2차 항체와 함께 인큐베이션하였다. 1차 항체를 EnVision Detection System(DAKO)을 이용하여 검출하고, 디아미노벤지딘(DAB; DAKO)으로 시각화하였다. 본 발명자는 수성 헤마톡실린(SigmaAldrich)으로 대조염색하였다. 직접 비교를 위해, 본 발명자는 변동성을 최소화하기 위해 모든 슬라이드를 단일 배치에서 처리하였다.
안전성 시험
동물
C57BL/6 마우스를 Insulin Syringe 1/2cc 30G x 3/8" Needle(Terumo, 또는 BD Bioscience)을 이용하여 꼬리에서 정맥내 주사를 통해 3회 용량의 QPSC를 수용하였다. 주사 전에, 동물의 꼬리 정맥을 팽창시키기 위해 15 내지 20분 동안 케이지 아래에 배치된 열 패드에 의해 동물을 가온시켰다. 부검 전에, 모든 동물을 우레탄(2 g/kg의 체중, Sigma-Aldrich)으로 마취시키고, 이후에, 턱밑 정맥으로부터 또는 심장 천자(cardiac puncture)를 통해 채혈하였다.
혈액 샘플 수집
혈액학 분석을 위해, 전혈 샘플을 EDTA 함유 혈액 수집 튜브(BD Bioscience, Cat No. 365974)에 수집하였다. 혈액 화학 분석을 위해, 전혈 샘플을 혈청 분리기를 함유한 혈액 수집 튜브(BD Bioscience, Cat No. 365967)에서 수집하였다. 실온에서 20분 동안 혈청 튜브를 정치시킨 후에, 혈청을 5분 동안 4℃, 6000 rpm에서 원심분리에 의해 분리하였다.
전체 부검 및 조직 수집
혈액 샘플링 후에, 동물 장기를 수집하였다. 이들 각각을 2 부분으로 나누었다: (1) -80℃ 냉동고에서 장기의 절반을 저장하고, 이후에, 옮기고 생물학적 분배 분석을 위해 액체 질소 용기에 저장함: (2) 나머지 절반을 고정시키고(5% 파라포름알데하이드, Sigma-Aldrich), 조직병리학적 분석을 위해 파라핀을 임베딩하였다.
정량적 PCR
QPSC 또는 뮤린 조직으로부터의 전체 RNA를 제조업체 설명서에 따라 Total RNA Miniprep Purification Kit(GMbiolab Cat#TR01)를 이용하여 추출하였다. 이후에, 2-단계 MMLV RT-PCR 키트(GMbiolab Cat#RP012-M)를 cDNA 분석을 위해 사용하였다. 선택된 유전자의 상대적 발현 분석을 위한 정량적 PCR을 The Fast SYBR® Green Master Mix(Thermo Cat#4385612)를 이용하여 수행하였다.
ELISA
QPSC의 EGF, FGF-b, VEGF, PDGF 및 TGF-b1의 세포내 및 분비 함량을 측정하기 위해, 세포 용해물 및 컨디셔닝된 배지의 샘플을 하기와 같이 제조하였다: QPSC를 냉동-해동 방법을 이용하여 용해시키고, 세포 용해물의 상청액을 초원심분리 후에 수집하였다. 그리고, 컨디셔닝된 배지 수집을 위해, 배지를 3일 QPSC 배양 후에 수집하였다. 마지막으로, 상기 성장 인자의 농도를 제조업체 설명서에 따라 ELISA(R&D systems)에 의해 결정하였다.
뉴런 세포 공동 배양 시험
인간 성상세포 세포주, SVG p12를 1250 uM 1-메틸-4-페닐피리딘(MPP+)으로 처리하고, 상이한 비율의 QPSC(SVG p12: QPSC=1:0.1 내지 1:10)로 공동 배양하였다. 24시간 후에, SVG p12의 세포 수를 카운팅하였다.
실시예 1 QPSC는 SCA3의 표현형을 변화시킴
QPSC는 SCA3 마우스의 표현형을 변화시켰다. 도 1에 도시된 바와 같이, SCA3 마우스는 야생형 마우스와 비교하여 약간 더 넓은 아래 부분(base)을 나타내며, QPSC 치료 후에, SCA3 마우스의 겉모습은 야생형 마우스와 유사하게 보였다. 유사한 개선 결과는 다양한 기능성 시험, 예를 들어, 변형된 SHIRPA(도 4, 도 5), 발자국(도 6, 도 7) 및 로타로드 실행 분석(도 5c)에서 관찰되었다.
실시예 2 본 발명에 치료에 의한 체중 손실의 중지 및 장기 조직의 부작용 없음
QPSC는 또한, 질병의 진행 동안 SCA3의 체중 손실을 중지시켰다(도 3). 그럼에도 불구하고, 3회 용량의 QPSC는 SCA3 개체에서 완전한 혈액 총수(표 1) 또는 혈액 생화학(표 2)의 프로파일에 영향을 미치지 않았다. 표 1 및 표 2는 1주 간격으로 3회 용량 동안 3회 용량의 QPSC와 염수 간에 25 내지 30주령의 야생형 마우스로부터 완전 혈액 총수/생화학 프로파일의 차이를 나타내지 않았다. 조직병리학적 분석은 3회 용량의 QPSC 주사 후에 다양한 중요한 장기 조직에서 정상적인 소견을 나타내었다(도 2).

실시예 3 본 발명의 치료를 이용한 마우스에서의 면역조절 및 항-ROS 용량 및 다수의 신경영양 인자 및 성장 인자를 발현시키는 발현
시험관내 연구에서는, QPSC가 면역조절 및 항-ROS 용량(도 10)을 지닐 뿐만 아니라 다수의 신경영양 인자 및 성장 인자를 발현시키는 능력을 가짐을(도 12)을 나타내었다. 생체내 연구에서는 QPSC 치료 후 산화적 스트레스 하에서 SCA 마우스에서의 로타로드 실행의 개선을 나타내었다(도 11). 또한, QPSC는 시험관내(도 13) 및 생체내(도 14) 둘 모두에서 뉴런 손실을 방지할 수 있다. 혈액 뇌 장벽(BBB)을 가로질러 이동하는 세포에 대한 가능성에 대한 질의가 제안되었지만, QPSC의 정맥내 주입을 통한 두개내 국소화의 능력이 나타났다(도 8 및 도 9).
이에 따라, 정맥내 주입을 통해, QPSC가 소뇌로 BBB를 진행시키고, ROS의 손상 및 면역 과잉반응으로부터 SCA 개체에서 뉴런 세포를 보호한다고 결론을 내는 것이 합리적이다. QPSC는 또한, 뉴런 세포의 수를 유지시키기 위해, 이에 따라, 글루타민 척수소뇌 운동 실조증, 마카도-조셉병, 헌팅턴병, DRPLA, 및 SMAX1/SBMA와 같은 poly-Q 질병의 진행을 지연시키기 위해 다수의 신경영양 및 성장 인자를 분비하였다.
II. 인간 임상 시험
인간 임상 시험은 무작위화, 이중-맹검, 플라시보-제어 연구 디자인에 의해 글루타민-매개 질병(예를 들어, 글루타민 척수소뇌 운동 실조증, 마카도-조셉병, 헌팅턴병, DRPLA, 및 SMAX1/SBMA)의 치료를 위한 Stemchymal® 주입의 치료 효능 및 안전성을 연구하는 것이다. 적합한 대상체는 정맥내 주입을 통해 Stemchymal®을 수용할 것이다.
글루타민 척수소뇌 운동 실조증에 대한 일 예에서, 시험을 받는 대상체는 유전형으로 척수소뇌 운동 실조증 타입 2 또는 척수소뇌 운동 실조증 타입 3임을 확인하였다. 대상체의 기준선 SARA 스코어는 5 내지 15의 범위에 있다.
2.5×107개 세포/kg 체중의 Stemchymal®을 해동시키고, 준비하고, 시린지 내에 로딩하였다. 세포를 해동 1시간 내에 서서히 주사하였다. Stemchymal®을 각 대상체에 정맥내로 주입하고, 세포 투약을 2주마다 1회의 간격으로 3회 수행하였다. 1회 이상의 치료 사이클 후에, 대상체의 SARA 스코어는 감소하였으며, SCA2 또는 SCA3 상태는 개선되었다.
Claims (8)
- 세포 마커 CD273, CD46, CD55 및 CXCR4를 발현하는 지방조직 유래 줄기세포(ADSC)의 유효량을 포함하는, 대상체의 폴리글루타민(폴리Q) 질병 치료용 조성물로서, 상기 폴리Q 질병은 척수소뇌실조증(SCA) 또는 헌팅턴병인, 조성물.
- 제1항에 있어서, 상기 SCA가 SCA1, SCA2, SCA3(마카도-조셉병), SCA6, SCA7 또는 SCA17인, 조성물.
- 제1항에 있어서, 상기 ADSC가 적어도 CD90, CD105, CD29, CD44, CD49b, CD49e, CD58 및 HLA-ABC를 발현하는, 조성물.
- 제1항에 있어서, 상기 ADSC가 비경구 투여 또는 국소 투여에 의해 투여되는, 조성물.
- 제4항에 있어서, 상기 비경구 투여가 근육내, 정맥내, 동맥내 또는 피하 투여인, 조성물.
- 제5항에 있어서, 상기 국소 투여가 뇌내(intrabrain) 또는 두개내(intracranial) 투여인, 조성물.
- 제1항에 있어서, 상기 ADSC가 하나 이상의 치료 주기로 투여되고, 여기서 하나의 치료 주기는 각각 2 내지 6주의 투여 간격으로 3개의 단위 투여량을 투여하는 것을 포함하고, 상기 단위 투여량이 0.5 x 105 내지 5 x 107 ADSCs/kg 체중인, 조성물.
