Abstract
본 발명은 알파-2 아드레날린 수용체 효능제를 유효성분으로 포함하는 항암 효과 증진용 조성물에 관한 것으로, 보다 상세하게는 본 발명에 따른 조성물을 암세포에 처리하였을 때에 암세포 유래의 엑소좀 분비가 억제되고, T 세포의 면역 회피를 유도하는 PD-L1의 발현이 억제되는 항암효과가 증진되는 것을 확인함으로써, 알파-2 아드레날린 수용체 효능제를 면역관문 억제제 및 세포 표면 또는 엑소좀 PD-L1의 발현에 대한 억제제로써 암 질환의 예방 또는 치료를 증진할 수 있는 항암 효과 증진용 제제 개발에 유용하게 활용될 수 있다.
Description
본 발명은 알파-2 아드레날린 수용체 효능제를 유효성분으로 포함하는 면역항암 효과 증진용 조성물로, 구체적으로, 알파-2 아드레날린 수용체 효능제에 의한 세포 표면 PD-L1 및 엑소좀 PD-L1 발현을 억제하는 새로운 기전을 이용한 항암 효과 증진제에 관한 것이다.
암 치료를 위한 일반적인 방법으로서 외과적 수술을 통한 종양 조직의 절제술이 가장 선호되고 있다. 그러나, 최근 외과적 수술을 포함한 침습적 치료법의 후유증 등의 문제로 인한 비침습적 암 치료법이 주목받고 있으며, 활발한 연구가 이루어지고 있다. 특히, 면역관문(Immune checkpoint) 억제용 항체를 이용한 항암 치료가 효율적인 암 치료 방법으로 각광 받고 있다.
면역관문 억제제를 활용한 항암 면역치료란 인체 내의 면역반응을 저해하는 PD-1(Programmed cell death 1)/PD-L1(PD-1 ligand 1) 등의 면역관문 상호작용을 해당 항체(항-PD-1 항체, 항-PD-L1 항체 등)를 사용하여 억제함으로써 세포독성 T 세포 등을 활성화하고, 암세포의 사멸을 유도하는 방법으로, 기존 치료법과 비교하여 뛰어난 치료효능으로 임상에서 폭넓게 활용되고 있다.
그러나, 상당수의 환자에서(>70%) 면역관문 억제용 항체를 이용한 치료에 효과가 없는 것으로 나타나는데, 이는 암이 암 엑소좀(Exosome)을 분비하여 면역 억제가 용이한 암 미세환경(tumor microenvironment)을 구축하는 등 능동적 면역억제 기작을 가지고 있기 때문이다.
따라서, 본 발명에서는 알파-2 아드레날린 수용체 효능제를 사용하여 암세포 표면 PD-L1 또는 엑소좀 PD-L1이나 암세포 유래 엑소좀 단백질의 발현을 억제시켜 보다 효과적인 면역항암 치료가 가능함을 제시한다.
본 발명은 알파-2 아드레날린 수용체 효능제를 유효성분으로 포함하는 면역항암 효과 증진용 조성물에 관한 것으로, 구체적으로 알파-2 아드레날린 수용체 효능제를 암세포에 처리하였을 때에 암세포 유래의 엑소좀 분비가 억제되고, T 세포의 면역 회피를 유도하는 세포 표면 PD-L1 또는 엑소좀 PD-L1의 발현이 억제되는 효과로 인해 암 질환의 예방 또는 치료를 증진할 수 있는 면역항암 효과 증진용 제제로 제공하는 것이다.
본 발명은 알파-2 아드레날린 수용체 효능제를 유효성분으로 포함하는 면역항암 효과 증진용 조성물을 제공한다.
또한, 본 발명은 알파-2 아드레날린 수용체 효능제 및 면역항암제를 유효성분으로 포함하는 병용투여용 약학조성물을 제공한다.
또한, 본 발명은 알파-2 아드레날린 수용체 효능제를 유효성분으로 포함하는 PD-L1 억제용 조성물을 제공한다.
또한, 본 발명은 암 질환이 의심되는 대상체로부터 분리된 시료에 후보 약물을 처리하는 제1단계; 상기 후보 약물이 처리된 시료에서 암세포 표면 또는 엑소좀 PD-L1이나, 암세포 유래 엑소좀 단백질의 발현 또는 활성 수준을 측정하는 제2단계; 및 대조군 시료와 비교하여 상기 암세포 표면 또는 엑소좀 PD-L1이나, 암세포 유래 엑소좀 단백질의 발현 또는 활성 수준을 감소시키는 후보 약물을 선별하는 제3단계;를 포함하는 면역항암 효과 증진제 스크리닝 방법을 제공한다.
또한, 본 발명은 알파-2 아드레날린 수용체 효능제를 대상체에 투여하는 단계를 포함하는 면역항암 효과 증진 방법을 제공한다.
또한, 본 발명은 알파-2 아드레날린 수용체 효능제를 대상체에 투여하는 단계를 포함하는 암세포 유래의 엑소좀 분비 및 암세포 표면 또는 엑소좀 PD-L1 발현을 억제하는 방법을 제공한다.
본 발명은 알파-2 아드레날린 수용체 효능제를 암세포에 처리하였을 때에 암세포 유래의 엑소좀 분비가 억제되고, T 세포의 면역 회피를 유도하는 PD-L1의 발현이 억제되는 항암효과가 증진되는 것을 확인함으로써, 알파-2 아드레날린 수용체 효능제를 면역관문 억제제와 세포 표면 및 엑소좀 PD-L1의 발현에 대한 억제제로써 암 질환의 예방 또는 치료를 증진할 수 있는 면역항암 효과 증진용 제제 개발에 유용하게 활용될 수 있다.

이하, 본 발명을 보다 상세하게 설명한다.
본 발명의 발명자들은 로펙시딘(LFX)과 같은 알파-2 아드레날린 수용체 효능제를 암세포에 처리 시 암세포 표면의 PD-L1 발현과 암세포 유래의 엑소좀 PD-L1 발현을 감소시키는 동시에 엑소좀 분비를 억제하는 현상을 통해 T 세포의 활성 억제를 막을 수 있으므로, 기존의 면역항암제와 동시 사용 혹은 약물 단독 사용 시 암 치료에 효과적이라는 점을 확인함으로써 본 발명을 완성하였다.
이에, 본 발명은 알파-2 아드레날린 수용체 효능제를 유효성분으로 포함하는 면역항암 효과 증진용 조성물을 제공한다.
상기 알파-2 아드레날린 수용체 효능제는 로펙시딘, 브리모니딘(Brimonidine), 클로니딘(Clonidine), 목소니딘(Moxonidine), 아프라클로니딘(Apraclonidine), 티자니딘(Tizanidine), 릴메니딘(Rilmenidine), 노르에피네프린(Norepinephrine), 드록시도파(Droxidopa), 레이스에피네프린(Racepinephrine), 에토미데이트(Etomidate) 및 덱스메데토미딘(Dexmedetomidine)으로 이루어진 군에서 선택된 어느 하나 이상일 수 있다.
상기 알파-2 아드레날린 수용체 효능제는 암세포 표면 PD-L1 및 엑소좀 PD-L1의 발현을 감소시켜 면역 회피를 억제하고 면역항암 효과를 증대시킬 수 있다.
또한, 상기 알파-2 아드레날린 수용체 효능제는 암세포 유래 엑소좀 분비를 감소시켜 면역 회피를 억제하고 면역항암 효과를 증대시킬 수 있다.
상기 조성물은 흑색종, 피부암, 폐암, 간암, 위암, 췌장암, 골암, 두경부암, 자궁암, 난소암, 유방암, 삼중음성유방암, 나팔관암종, 자궁내막암종, 자궁경부암종, 질암종, 음문암종, 호킨스씨병, 식도암, 소장암, 대장암, 결장암, 직장암, 항문부근암, 내분비선암, 갑상선암, 부갑상선암, 부신암, 연조직 육종, 요도암, 음경암, 전립선암, 만성 또는 급성 백혈병, 림프구 림프종, 방광암, 신장 또는 수뇨관암, 신장세포 암종, 신장골반 암종, 중추신경계 종양, 1차 중추신경계 림프종, 척수 종양, 뇌간신경교종 및 뇌하수체 선종으로 이루어진 군에서 선택된 암질환의 면역항암 효과를 증대시킬 수 있으나, 이에 제한되는 것은 아님을 명시한다.
상기 조성물은 면역항암제와 병용요법으로 투여될 수 있으나, 이에 제한되는 것은 아님을 명시한다.
상기 면역항암제는 PD-1 억제제, PD-L1 억제제 및 CTLA-4 억제제로 이루어진 군에서 선택된 하나 이상일 수 있다. 구체적으로, 상기 면역항암제는 이필리무맙(ipilimumab), 니볼루맙(nivolumab), 펨브롤리주맙(pembrolizumab), 아테졸리주맙(atezolizumab), 아벨루맙(avelumab) 및 더발루맙(durvalumab)으로 이루어진 군에서 선택된 1종 이상일 수 있다.
또한, 본 발명은 알파-2 아드레날린 수용체 효능제 및 면역항암제를 유효성분으로 포함하는 병용투여용 약학조성물을 제공한다.
상기 약학조성물은 통상적인 방법에 따라 겔제, 유제, 주사제, 산제, 과립제, 에어로솔제, 페이스트제, 경피흡수제 및 패치제로 이루어진 군에서 선택된 하나 이상의 제형으로 제공될 수 있으나, 이에 제한되는 것은 아니다.
본 발명의 다른 구체예에서, 상기 약학조성물은 약학조성물의 제조에 통상적으로 사용하는 적절한 담체, 부형제, 붕해제, 감미제, 피복제, 팽창제, 윤활제, 활택제, 향미제, 항산화제, 완충액, 정균제, 희석제, 분산제, 계면활성제, 결합제 및 윤활제로 이루어진 군에서 선택되는 하나 이상의 첨가제를 추가로 포함할 수 있다.
구체적으로 담체, 부형제 및 희석제는 락토즈, 덱스트로즈, 수크로스, 솔비톨, 만니톨, 자일리톨, 에리스리톨, 말티톨, 전분, 아카시아 고무, 알지네이트, 젤라틴, 칼슘 포스페이트, 칼슘 실리케이트, 셀룰로즈, 메틸 셀룰로즈, 미정질 셀룰로스, 폴리비닐 피롤리돈, 물, 메틸히드록시벤조에이트, 프로필히드록시벤조에이트, 탈크, 마그네슘 스테아레이트 및 광물유를 사용할 수 있으며, 경구투여를 위한 고형제제에는 정제, 환제, 산제, 과립제, 캡슐제 등이 포함되며, 이러한 고형제제는 상기 조성물에 적어도 하나 이상의 부형제, 예를 들면, 전분, 칼슘카보네이트, 수크로스 또는 락토오스, 젤라틴 등을 섞어 조제할 수 있다. 또한 단순한 부형제 이외에 마그네슘 스티레이트, 탈크 같은 윤활제들도 사용할 수 있다. 경구를 위한 액상제제로는 현탁제, 내용액제, 유제, 시럽제 등이 있으며 흔히 사용되는 단순 희석제인 물, 리퀴드 파라핀 이외에 여러 가지 부형제, 예를 들면 습윤제, 감미제, 방향제, 보존제 등이 포함될 수 있다. 비경구 투여를 위한 제제에는 멸균된 수용액, 비수성용제, 현탁제, 유제, 동결건조제제, 좌제 등이 포함된다. 비수성용제, 현탁제로는 프로필렌글리콜, 폴리에틸렌 글리콜, 올리브 오일과 같은 식물성 기름, 에틸올레이트와 같은 주사 가능한 에스테르 등이 사용될 수 있다. 좌제의 기재로는 위텝솔(witepsol), 마크로골, 트윈(tween) 61, 카카오지, 라우린지, 글리세로제라틴 등이 사용될 수 있다.
상기 약학조성물은 정맥내, 동맥내, 복강내, 근육내, 동맥내, 복강내, 흉골내, 경피, 비측내, 흡입, 국소, 직장, 경구, 안구내 또는 피내 경로를 통해 통상적인 방식으로 대상체로 투여할 수 있다.
상기 화합물의 바람직한 투여량은 대상체의 상태 및 체중, 질환의 종류 및 정도, 약물 형태, 투여경로 및 기간에 따라 달라질 수 있으며 당업자에 의해 적절하게 선택될 수 있다. 본 발명의 일실시예에 따르면 이에 제한되는 것은 아니지만 1일 투여량이 0.01 내지 50 mg/kg, 구체적으로는 0.1 내지 50 mg/kg, 보다 구체적으로는 0.1 내지 20 mg/kg 일 수 있다. 투여는 하루에 한 번 투여할 수도 있고 수회로 나누어 투여할 수도 있으며, 이에 의해 본 발명의 범위가 제한되는 것은 아니다.
본 발명에 있어서, 상기 '대상체'는 인간을 포함하는 포유동물일 수 있으나, 이들 예에 한정되는 것은 아니다.
또한, 본 발명은 알파-2 아드레날린 수용체 효능제를 유효성분으로 포함하는 PD-L1 억제용 조성물을 제공한다.
상기 PD-L1 억제용 조성물은 상기 알파-2 아드레날린 수용체 효능제가 암세포 표면 PD-L1 및 엑소좀 PD-L1의 발현을 감소시켜 면역 회피를 억제하고 면역항암 효과를 증대시킬 수 있다.
또한, 본 발명은 암 질환이 의심되는 대상체로부터 분리된 시료에 후보 약물을 처리하는 제1단계; 상기 후보 약물이 처리된 시료에서 암세포 표면 또는 엑소좀 PD-L1이나, 암세포 유래 엑소좀 단백질의 발현 또는 활성 수준을 측정하는 제2단계; 및 대조군 시료와 비교하여 상기 암세포 표면 또는 엑소좀 PD-L1이나, 암세포 유래 엑소좀 단백질의 발현 또는 활성 수준을 감소시키는 후보 약물을 선별하는 제3단계;를 포함하는 면역항암 효과 증진제 스크리닝 방법을 제공한다.
상기 엑소좀 단백질은 CD63, Alix, TSG101 및 Synthenin-1로 이루어진 군에서 선택된 1종 이상을 포함한다.
상기 제2단계의 암세포 표면 또는 엑소좀 PD-L1이나, 암세포 유래 엑소좀 단백질의 발현 또는 활성 수준은 역전사 중합효소 연쇄반응(reverse transcription-polymerase chain reaction, RT-PCR), 효소면역분석법(enzyme-linked immunosorbent assay, ELISA), 방사능면역분석(radioimmunoassay), 면역조직화학(immunohistochemistry), 마이크로어레이(microarray), 웨스턴 블랏(western blotting) 및 유세포 분석법(FACS)으로 이루어지는 군에서 선택된 어느 하나 이상으로 측정할 수 있으나, 이에 제한되는 것은 아님을 명시한다.
또한, 본 발명은 알파-2 아드레날린 수용체 효능제를 대상체에 투여하는 단계를 포함하는 면역항암 효과 증진 방법을 제공한다.
또한, 본 발명은 알파-2 아드레날린 수용체 효능제를 대상체에 투여하는 단계를 포함하는 암세포 유래의 엑소좀 분비 및 암세포 표면 또는 엑소좀 PD-L1 발현을 억제하는 방법을 제공한다.
이하, 실시예를 통하여 본 발명을 더욱 상세히 설명하고자 한다. 이들 실시예는 오로지 본 발명을 보다 구체적으로 설명하기 위한 것으로, 본 발명의 요지에 따라 본 발명의 범위가 이들 실시예에 의해 제한되지 않는다는 것은 본 발명이 속하는 기술분야에서 통상의 지식을 가진 자에 있어서 자명할 것이다.
<실시예 1>
약물 스크닝에 적합한 세포주(cell line)를 선정하기 위해서 인간 암세포 용해물(human cancer cell lysate)에서 PD-L1 발현을 확인하는 실험을 하기와 같이 진행하였다. PD-L1 치료법(therapy)에 많이 이용하고 있는 대표적인 암 형태(cancer type)인 결장암(colon cancer), 삼중음성 유방암(triple negative breast cancer), 악성 흑색종(melanoma), 폐암 세포주(lung cancer cell line)인 HCT116, MDA-MB231, SK-MEL-28, A375, A549에서 각각의 세포 용해물(cell lysate)를 얻었다. PD-L1 발현 가장 높게 나타나는 MDA-MB231 세포를 스크리닝에 사용했고, 실험 결과를 도 1에 나타냈다.
구체적으로, 도 1의 (b)에 나타낸 것과 같이 가장 PD-L1 발현이 높은 MDA-MB231 세포를 24 웰 플레이트(well plate)에 시딩한 후 FDA 승인약물 중 지표(indication)이 대사질환(metabolic disease)과 심장혈관계 질병(cardiovascular disease) 두 가지인 것을 처리 후 유세포 분석기(flow cytometry)를 통해 PD-L1 발현을 확인했다.
도 1을 살펴보면, IFN-γ에 의해 증가된 PD-L1 발현을 낮추는 약물을 유세포 분석을 통해 스크리닝 후 히트(hit) 중에 세포독성, 약물투여경로 등을 고려하여 로펙시딘(Lofexidine)이라는 약물을 선정했다.
<실시예 2>
세포 표면(cell surface)의 PD-L1 발현을 확인하기 위해서, 인간 암세포MDA-MB231, A375, HCT116, A549) 및 마우스 암세포(CT26)에 로펙시딘을 농도별로 처리한 후 유세포 분석기로 측정하였고, 그 결과를 도 2에 나타냈다.
24 well 플레이트에 1.5 X 105 cells/well(MDA-MB231), 1.3 X 105 cells/well(A375, HCT116, A549), 1.0 X 105 cells/well(CT26) 시딩 후 24시간 뒤에 IFN-γ (50 ng/mg)와 로펙시딘(LFX)을 농도별(5, 20, 50 μM)로 처리 후 24시간 배양을 시키고 PBS로 워시 후, APC 형광이 붙은 PD-L1 항체(invitrogen, 17-5983-42)를 붙인 뒤 유세포 분석기로 세포 표면의 PD-L1을 나타내는 APC 형광 발현을 측정하였다.
도 2를 살펴보면, 4가지 인간 암세포(MDA-MB231, A375, HCT116, A549)와 마우스 암세포 (CT26)에서 LFX 처리시 세포 표면의 PD-L1 발현이 농도 의존적으로 감소되는 것을 확인하였다. 로펙시딘의 표적 수용체(target receptor) 알파-2 아드레날린 수용체 효능제(Alpha2 adrenergic receptor agonist)인 클로니딘(Clonidine)도 함께 처리하여 유세포 분석기를 통해 세포 표면 PD-L1 발현이 감소하는 것을 확인하였다. 세포 표면 PD-L1의 발현의 억제를 확인하기 위해 유세포 분석기를 통해 여러 가지 인간 암세포에서 농도 의존적인 PD-L1 발현 감소를 확인했다.
총 PD-L1 단백질(Total PD-L1 protein)의 발현을 확인하기 위해 로펙시딘을 농도별로 처리한 후 웨스턴 블랏(Western blot)을 수행하였고 3가지 인간 암세포와 1가지 마우스 암세포에서 총 PD-L1 단백질 발현이 농도 의존적으로 감소되는 것을 확인하였다.
인간 암세포와 마찬가지로 마우스 암세포(mouse cancer cell)인 CT26 세포에서도 로펙시딘에 의해 PD-L1 발현이 감소되는지 확인했다. CT26 세포에서 세포 표면 PD-L1 발현을 확인하기 위해 로펙시딘을 농도별로 처리한 후 유세포 분석기를 이용하여 측정하였고, 웨스턴 블랏을 수행하였고, 세포 표면 PD-L1 발현이 감소되는 것을 확인하였다.
이러한 결과를 통해 4가지 인간 암세포와 마우스 암세포에서 로펙시딘의 농도에 의존적으로 PD-L1 발현 감소하는 것을 알 수 있다.
<실시예 3>
로펙시딘의 PD-L1 발현 억제와 관련된 메커니즘을 확인하기 위해, MDA-MB231 세포와 A375 세포에 IFN-γ와 다른 농도의 로펙시딘(LFX)을 함께 처리 후 IFN-γ 하위 신호 단백질 발현을 웨스턴 블랏으로 실험을 수행하였고, 그 결과를 도 3에 나타냈다.
웨스턴 블랏은 12well 플레이트에 4 X 105 cells/well(MDA-MB231), 3 X 105 cells/well(A375) 세포를 시딩 후 24시간 뒤에 IFN-γ (50 ng/mg)와 로펙시딘(LFX)을 농도별(5, 20, 50 μM)로 처리 후 24시간 배양을 시키고, 세포 용해물에 단백질을 추출 후 SDS-PAGE에 의해 분리되고 니트로셀룰로오스 막(Nitrocellulose membrane)으로 이동하고, 각 1차 항체(IFN-γ 하위 신호전달 단백질 항체)와 결합하고 HRP가 붙은 2차 항체와 배양된 후 ECL 시약을 사용하여 검출된다.
도 3을 살펴보면, MDA-MB231 세포에 IFN-γ와 로펙시딘을 처리하는 경우 IFN-γ 의존 경로(IFN-γ dependent pathway)에 따라 IFN-γ 하위 신호(signaling)이 감소하는 것을 확인하였다. JAK-STAT 경로(JAK-STAT pathway), MEK-ERK(MAPK) 경로(MEK-ERK(MAPK) pathway)의 억제를 통해 PD-L1 발현을 감소시킨다.
도 3의 p-AKT의 발현에는 변화가 없으므로, PI3K-AKT 경로(PI3K-AKT pathway)는 연관이 없는 것을 알 수 있었다. A375 세포에서도 마찬가지로 IFN-γ와 로펙시딘을 처리하는 경우 IFN-γ 의존 경로에 따라 IFN-γ 하위 신호가 감소하는 것을 확인하였다. JAK-STAT 경로, MEK-ERK(MAPK) 경로의 억제를 통해 PD-L1 발현을 감소시키나, PI3K-AKT 경로는 연관이 없는 것을 알 수 있었다.
<실시예 4>
In vitro 실험에서 적절한 약물 처리 농도를 선정하기 위해 각각 세포주에서 로펙시딘을 농도별로 처리한 후 MTT assay 진행했으며, 그 결과는 도 4에 나타냈다.
MTT assay는 96well 플레이트에 2x104 cells/well (MDA-MB231) 1x104 cells/well (A375, CT26) 시딩 후 24시간 후에 로펙시딘을 5 μM부터 500 μM까지 처리를 하고 24시간 배양 후 세포 배양액을 제거하고 MTT 시약(500 μg/ml)을 넣고 37℃에서 2~4시간 배양 후 제거하고, 디메틸 설폭사이드(Dimethyl sulfoxide, DMSO)200 μl를 첨가한 후에 마이크로플레이트 리더기(Microplate reader)로 측정하였다.
도 4를 살펴보면, 로펙시딘(LFX) 5 내지 200 μM에서 모두 세포 생존능(cell viability)가 100% 인 것을 확인하였다. MDA-MB231 세포에 로펙시딘을 20 μM 및 50 μM로 각각 처리 후 초원심분리기(ultracentrifugation)을 이용한 엑소좀 분리(exosome isolation)한 후 천연 흉선세포독성 자가항체(natural thymocytotoxic autoantibody; NTA) 측정했을 때 IFN-γ 유무에 상관없이 엑소좀 분비(exosome secretion)이 각각 30%, 60% 감소되는 것을 확인하였다. IFN-γ 처리 시에는 Control과 비교했을 때 exosome 분비량에 차이가 없었다. 그리고 MDA-MB231 cell에 로펙시딘을 처리한 후 초원심분리기를 이용하여 엑소좀을 분리한 후 엑소좀으로 웨스턴 블랏을 진행하면 엑소좀 PD-L1(exosomal PD-L1) 발현은 감소되었다. 로펙시딘은 세포 PD-L1 뿐만 아니라 엑소좀 PD-L1의 발현 또한 억제한다는 것을 알 수 있었다.
동일한 세포수(cell number)에서 유래된 엑소좀 단백질(exosome protein)을 로딩(loading) 했을 때 엑소좀 PD-L1 발현이 감소되었다. CT26 세포에도 마찬가지로 로펙시딘을 처리한 후 초원심분리기를 이용하여 엑소좀을 분리한 후 NTA 측정했을 때 IFN-γ 유무에 상관없이 엑소좀 분비가 각각 30%, 60% 감소되는 것을 확인하였다.
<실시예 5>
MDA-MB231 세포에서 로펙시딘을 처리한 후 엑소좀 분비 억제와 관련된 매커니즘(mechanism)을 확인하기 위하여, Rab 단백질들과 엑소좀 마커 단백질(exosome marker protein)에 대하여 웨스턴 블랏을 수행하였고, 그 결과는 도 5에 나타냈다.
상기 웨스턴 블랏은 12well 플레이트에 4 X 105 cells/well(MDA-MB231), 1 X 105 cells/well(CT26) 세포를 시딩 후 24시간 뒤에 IFN-γ (50 ng/mg)와 로펙시딘(LFX)을 농도별(5, 20, 50 μM)로 처리 한 군과 로펙시딘(LFX)을 단독으로 농도별 (5, 20, 50 μM)로 처리 한 군을 나눠서 24시간 배양을 시키고, 1차 항체를 각각 Rab 단백질과 엑소좀 마커 단백질 항체를 사용하고, HRP가 붙은 2차 항체와 배양 후 실험을 수행하였다.
도 5를 살펴보면, Rab11, Rab27a 단백질들의 발현이 감소되는 경향을 보였다. 엑소좀 마커 단백질들에 대해서도 웨스턴 블랏을 진행했을 때 Alix의 발현이 로펙시딘에 의해 농도 의존적인 경향 에 따라 감소되는 결과를 보였다.
또한, CT26 세포에 로펙시딘을 처리한 후 Rab 단백질과 엑소좀 마커 단백질의 발현을 웨스턴 블랏을 통해 확인한 결과, Rab5, Alix가 감소되는 경향을 보였다.
<실시예 6>
In vivo 실험에 적합한 마우스 세포주(mouse cell line)를 선정하기 위해 로펙시딘의 표적 리셉터인 알파-2 아드레날린 수용체(alpha2 adrenergic receptor)의 발현을 마우스 세포주 5가지에서 확인하였고, 그 결과를 도 6에 나타냈다.
(웨스턴 블랏을 이용하여 5가지 마우스 세포주에서 추출한 단백질을 같은 양을 로딩한 후 1차 항체인 알파-2 아드레날린 수용체 단백질 항체(Invitrogen, PA1-048)를 사용하여 기본적으로 발현하는 알파-2 아드레날린 수용체 단백질 발현을 비교하였다. 또한 Balb/c 누드 마우스와 Balb/c 야생형 마우스에 CT26 세포(2 X 105 cells/mouse)를 주입하고, 12일 후에 무작위로 2개의 군으로 나눈 뒤 한 군은 PBS 다른 한 군은 로펙시딘(LFX)을 매일 4 mg/kg로 경구 투여하고, 종양 크기는 3일에 한번 씩 측정하였다.)
도 6을 살펴보면, CT26의 발현이 가장 높은 것을 확인하였다. 면역 시스템(Immune system)이 결핍된 누드 마우스(nude mouse)와 면역 시스템이 존재하는 야생형 마우스(WT mouse)에 각각 CT26 세포 주입 후 로펙시딘(LFX) 약물을 투여하면 면역 시스템이 존재하는 야생형 마우스에서만 항암 효과(anti tumor effect)가 나타나는 것을 확인할 수 있었다.
<실시예 7>
CT26 마우스 동종이식 모델(CT26 mouse syngeneic model)에 로펙시딘(LFX) 1 mg/kg, 2 mg/kg, 4 mg/kg를 각각 약물 투여하여 실험을 수행하였고, 그 결과는 도 7에 나타냈다.
도 7을 살펴보면, LFX에 의해 농도 의존적인 경향에 따라 종양 성장(tumor growth)가 감소되는 것을 확인할 수 있다. LFX에 의한 농도 의존적인 항암 효과를 확인할 수 있다. 각각에서 생존능(survival rate)을 확인했을 때 LFX 4 mg/kg에서 가장 높은 생존능을 보이는 것을 확인할 수 있다.
<실시예 8>
PD-L1 억제제 면역 치료법의 치료 효과를 확인하기 위해서, 야생형 마우스 CT26 마우스 동종이식 모델(WT mouse CT26 syngeneic model)에 항 PD-L1 항체(Bio X cell, BE0101)과 로펙시딘(LFX) 약물을 각각 단독 투여 또는 병용 요법(combination therapy)를 이용한 실험을 수행하였고, 그 결과를 도 8에 나타냈다.
야생형 마우스 모델(WT mouse model)에 CT26 세포(2 X 105 cells/mouse)를 주입 후 무작위로 4개의 군으로 나눈 뒤, 첫 번째 군인 대조군에서는 PBS 경구 투여(100μl/mouse)와 복강내 투여(100μl/mouse)를 진행하고, 두 번째 군은 로펙시딘(LFX, 4mg/kg/day) 경구 투여와 함께 PBS 복강내 투여(100μl/mouse), 세 번째 군은 PBS (100μl/mouse) 경구 투여와 3일에 1번씩 총 3번 항 PD-L1 항체(100μl/mouse)를 복강 내 투여, 네 번째 군은 로펙시딘(LFX, 4mg/kg/day) 경구 투여와 함께 3일에 1번씩 총 3번 항 PD-L1 항체(100μl/mouse)를 복강 내 투여 후, 3일에 1번 씩 종양 크기를 측정하였습니다.
도 8을 살펴보면, 상기 로펙시딘 약물 단독 투여에 의한 항암 효과도 나타나지만 PD-L1 억제제와 같은 면역관문억제제(Immune checkpoint blockade; ICB)와 같이 투여하는 병용 요법에서 더 효과적인 항암 효과가 나타났다. 이를 통해, 로펙시딘은 면역관문억제제의 효능을 증가시킨다는 것을 알 수 있다.
<실시예 9>
동종이식 마우스 모델(Syngeneic mouse model)에서의 로펙시딘과 PD-L1 억제제의 병용 요법의 효과를 확인하기 위해, 야생형 마우스 모델(WT mouse model)에 로펙시딘(LFX)과 항 PD-L1 항체(Bio X cell, BE0101)을 혼합하여 투여한 병용 요법을 수행하였으며, 그 결과는 도 9에 나타냈다.
CT26 동종이식 마우스 모델에 로펙시딘(LFX, 4mg/kg/day)과 항 PD-L1 항체100μl/mouse)를 단독 또는 병용 치료를 하고 17일 후 마우스 혈장에서 원심 분리기를 사용하여 1,500 x g /15분, 10,000 x g 30분 후에 초원심 분리기를 사용하여 160,000 x g 90분 후에 추출한 엑소좀 단백질과 마우스 종양에서 추출한 세포 단백질에서 PD-L1 발현 및 엑소좀 단백질들의 발현을 웨스턴 블랏으로 확인하였다.
도 9를 살펴보면, 야생형 마우스 모델(WT mouse model)에 로펙시딘(LFX) 약물 투여하면 약물 단독 투여에 의한 항암 효과도 나타나지만 항 PD-L1 항체와 같은 면역관문억제제(ICB)와 병용 요법에서 종양 PD-L1(tumor PD-L1) 및 엑소좀 PD-L1의 억제에 의해 더 효과적인 항암 효과가 나타났다. 야생형 마우스 모델에 PD-L1 억제제와 로펙시딘을 병용 투여하면 CD8+ T cell 등의 면역세포(immune cell)의 활성화를 통해 항암 효과를 나타낼 것을 알 수 있다.
<실시예 10>
로펙시딘(LFX)는 알파-2 아드레날린 수용체(α2 adrenergic receptor; A2AR)의 효능제(agonist)로 작용한다고 보고되었다. 로펙시딘이 기존에 알려진 표적(target)을 매개하여 세포외 소포체(EV) PD-L1을 억제하는지 확인하기 위해 A2AR의 효능제인 클로니딘을 MDA-MB231 세포에 처리하여 실험하였으며, 그 결과는 도 10에 나타냈다.
도 10을 살펴보면, 클로니딘은 IFN-γ 유도된 세포 표면(IFN-γ induced cell surface) PD-L1 레벨 및 세포외 소포체 분비를 모두 억제하였다. 이 결과는 로펙시딘의 표적 수용체인 A2AR이 세포외 소포체 PD-L1의 중요 조절자(regulator)인 것을 알 수 있다. 또한, 클로니딘도 로펙시딘과 같은 효과를 나타냄을 알 수 있다.
이상으로 본 발명의 특정한 부분을 상세히 기술하였는 바, 당업계의 통상의 지식을 가진 자에게 있어서, 이러한 구체적 기술은 단지 바람직한 실시예일 뿐이며, 이에 의해 본 발명의 범위가 제한되는 것이 아닌 점은 명백할 것이다. 따라서, 본 발명의 실질적인 범위는 첨부된 청구항들과 그것들의 등가물에 의하여 정의된다고 할 것이다.
Claims (13)
- 알파-2 아드레날린 수용체 효능제를 유효성분으로 포함하는 면역항암 효과 증진용 조성물.
- 제1항에 있어서, 상기 알파-2 아드레날린 수용체 효능제는 로펙시딘, 브리모니딘(Brimonidine), 클로니딘(Clonidine), 목소니딘(Moxonidine), 아프라클로니딘(Apraclonidine), 티자니딘(Tizanidine), 릴메니딘(Rilmenidine), 노르에피네프린(Norepinephrine), 드록시도파(Droxidopa), 레이스에피네프린(Racepinephrine), 에토미데이트(Etomidate) 및 덱스메데토미딘(Dexmedetomidine)으로 이루어진 군에서 선택된 어느 하나 이상인 것을 특징으로 하는 면역항암 효과 증진용 조성물.
- 제1항 또는 제2항에 있어서, 상기 알파-2 아드레날린 수용체 효능제는 암세포 표면 PD-L1 및 엑소좀 PD-L1의 발현을 감소시켜 면역 회피를 억제하고 면역항암 효과를 증대시키는 것을 특징으로 하는 면역항암 효과 증진용 조성물.
- 제1항 또는 제2항에 있어서, 상기 알파-2 아드레날린 수용체 효능제는 암세포 유래 엑소좀 분비를 감소시켜 면역 회피를 억제하고 면역항암 효과를 증대시키는 것을 특징으로 하는 면역항암 효과 증진용 조성물.
- 제1항 또는 제2항에 있어서, 상기 조성물은 흑색종, 피부암, 폐암, 간암, 위암, 췌장암, 골암, 두경부암, 자궁암, 난소암, 유방암, 삼중음성유방암, 나팔관암종, 자궁내막암종, 자궁경부암종, 질암종, 음문암종, 호킨스씨병, 식도암, 소장암, 대장암, 결장암, 직장암, 항문부근암, 내분비선암, 갑상선암, 부갑상선암, 부신암, 연조직 육종, 요도암, 음경암, 전립선암, 만성 또는 급성 백혈병, 림프구 림프종, 방광암, 신장 또는 수뇨관암, 신장세포 암종, 신장골반 암종, 중추신경계 종양, 1차 중추신경계 림프종, 척수 종양, 뇌간신경교종 및 뇌하수체 선종으로 이루어진 군에서 선택된 암질환의 면역항암 효과를 증대시키는 것을 특징으로 하는 면역항암 효과 증진용 조성물.
- 제1항 또는 제2항에 있어서, 상기 조성물은 면역항암제와 병용요법으로 투여되는 것을 특징으로 하는 면역항암 효과 증진용 조성물.
- 제6항에 있어서, 상기 면역항암제는 PD-1 억제제, PD-L1 억제제 및 CTLA-4 억제제로 이루어진 군에서 선택된 하나 이상인 것을 특징으로 하는 면역항암 효과 증진용 조성물.
- 알파-2 아드레날린 수용체 효능제 및 면역항암제를 유효성분으로 포함하는 병용투여용 약학조성물.
- 알파-2 아드레날린 수용체 효능제를 유효성분으로 포함하는 PD-L1 억제용 조성물.
- 암 질환이 의심되는 대상체로부터 분리된 시료에 후보 약물을 처리하는 제1단계;
상기 후보 약물이 처리된 시료에서 암세포 표면 또는 엑소좀 PD-L1이나, 암세포 유래 엑소좀 단백질의 발현 또는 활성 수준을 측정하는 제2단계; 및
대조군 시료와 비교하여 상기 암세포 표면 또는 엑소좀 PD-L1이나, 암세포 유래 엑소좀 단백질의 발현 또는 활성 수준을 감소시키는 후보 약물을 선별하는 제3단계;
를 포함하는 면역항암 효과 증진제 스크리닝 방법. - 제10항에 있어서, 상기 제2단계의 암세포 표면 또는 엑소좀 PD-L1이나, 암세포 유래 엑소좀 단백질의 발현 또는 활성 수준은 역전사 중합효소 연쇄반응(reverse transcription-polymerase chain reaction, RT-PCR), 효소면역분석법(enzyme-linked immunosorbent assay, ELISA), 방사능면역분석(radioimmunoassay), 면역조직화학(immunohistochemistry), 마이크로어레이(microarray), 웨스턴 블랏(western blotting) 및 유세포 분석법(FACS)으로 이루어지는 군에서 선택된 어느 하나 이상으로 측정하는 것을 특징으로 하는 면역항암 효과 증진제 스크리닝 방법.
- 알파-2 아드레날린 수용체 효능제를 대상체에 투여하는 단계를 포함하는 면역항암 효과 증진 방법.
- 알파-2 아드레날린 수용체 효능제를 대상체에 투여하는 단계를 포함하는 암세포 유래의 엑소좀 분비 및 암세포 표면 또는 엑소좀 PD-L1 발현을 억제하는 방법.
