Abstract
골관절염(OA)의 치료 및 예방에 사용하기 위한 및 류머티스성 관절염(RA)의 치료를 위한, 히알루론산 및 카르노신을 함유하는 약학 조성물을 기재한다.
Description
본 발명은 히알루론산 및 카르노신을 함유하는 약학 조성물 및 그의 관련 용도에 관한 것이다.
골관절염(OA)은 기질의 분해 및 주요 연골질 세포 성분: 연골세포의 손실로 인한 관절 연골의 진행성 미란을 특징으로 하는 중증 불능성 병리이다.
병인은 여전히 부분적으로 알려지지 않고 있지만, 최근의 실험 결과는 복합적으로 관절을 수반할 수 있는 기계적 불균형이 또한 상기 언급한 병리의 개시의 초기 원인일 수도 있음을 입증하였다.
관절의 과도한 및/또는 잘못된 부하는 실제로 연골 자체의 분해에 원인인 효소의 합성에서 나타나는 연골세포 반응을 야기할 수 있다. 이러한 프로테아제 효소를 메탈로프로테아제(MMP)라 칭하며, 상기 효소는 무엇보다도 염증 병리의 개시로 인해 관절 강 중에서 생성되고 방출되는 염증-전 사이토킨, 예를 들어 IL-1, IL-6 및 TNF-α에 의해 자극시 연골세포에 의해 합성된다. 이들 사이토킨은 또한 높은 수준의 산화 질소(연골세포의 세포사멸에 의한 사망의 원인이다)의 합성을 자극하며, 더욱이 프로테오글리칸(기질의 구조 성분)의 합성을 억제한다(Dozin B. et al., Matrix Biology, 2002, 21:449-459).
높은 수준의 염증-전 분자는 또한 류머티스성 관절염(RA) 및 건선성 관절염을 앓고 있는 환자의 활액에서 발견되었다(Arend W.P. et al., Arthritis Rheum, 1995, 38:151-160).
류머티스성 관절염(RA)은, 주로 관절 기능의 상실시까지 기형 및 통증이 야기되는 활막 관절에 기인하는, 자가면역 발병 및 미지의 병인을 갖는 만성의 강직성이고 진행성인 염증성 다발관절염이다.
실제로, RA의 진행은, 결과적으로 세포가 팽창하는 활막의 강한 염증 반응, 과도한 활액 및 섬유성 조직의 발생을 결정한다. 보다 구체적으로, 활막은 활막세포(대식세포 및 피브리노이드 유형의)에 의해 형성된 간엽 기원의 막으로, 질병 중에 과형성 및 비대를 겪고 이어서 두께가 증가하여(비-병원성 상태의 2개/3개 층은 7개 이상의 층이 된다), 연골에 의해 덮이지 않은 뼈를 주변부에서 서서히 약화시키기 시작하는 활막 판누스를 형성한다.
동시에, T, B 림프구 및 형질 세포를 갖는 다형핵 세포가 활액으로 이동하여, 병든 관절의 염증을 증가시키고, 이어서 RA가 침범한 관절 연골(비교적 하부의 뼈)이 파괴가 진행되면서 얇아진다.
상기 병리는 일반적으로 자체가 전신 수준으로 나타나며 다른 장기 및 기관을 또한 수반한다. 류머티스양 결절이 전형적이며, 결과적으로 폐 수준에서 폐 섬유증(pulmonary fibrosis), 흉막염(pleurisy) 및 흉막심낭염(pleuropericarditis)이 형성될 수 있고, 심장 수준에서 관상동맥 죽상동맥경화증(coronary atherosclerosis)의 가속화가 있을 수 있는 반면, 눈 수준에서 안구건조증(xerophthalmia), 포도막염(uveitis) 및 공막염(scleritis)이 있을 수 있다. 아밀로이드증(Amyloidosis) 및 골다공증(osteoporosis)이 또한 RA의 합병증이다(Cecil, TEXTBOOK of MEDICINE, 1988).
류머티스성 관절염은 전 세계 성인의 0.5 내지 1%가 걸리며, 통상적으로 40 내지 50세에 시작한다.
RA의 치료는 어려우며, 그의 중증도 및 관련 장기의 유형에 따라 변하고, 장애의 발생을 늦추는 목적과 함께 관절 손상(다른 장기에 더하여)의 예방 및/또는 치료를 위해 염증을 억제하기에 적합한 약물의 사용을 포함한다. 스테로이드 및 NSAID를 포함한 통증-경감 및 소염 약물은 증상은 억제하고 상태의 진행은 멈추지 못하지만, 질병을 변형시키는 항-류머티스 약물(DMARD)은 이를 늦출 수 있다.
보다 공격적인 형태로, 저 용량으로 면역억제제로서 사용되는 폴산 합성의 대사길항성 억제제, 메토트렉세이트가 일반적으로 사용되나, 최근에 보다 선택적이고 특이적인 방식으로 작용하는 생물학적 약물(Canete JD et al. Expert Opinion Biol Ther, 2017, 17:1-15), 예를 들어 염증-전 TNF 사이토킨을 길항함으로써 작용하는 융합 단백질인 에타너셉트가 도입되었다.
TNF는 전신 염증의 급성기의 원인인 상기 사이토킨 그룹의 부분이며; 다수의 과정, 예를 들어 세포 증식, 분화 및 세포사멸, 발암 및 바이러스 복제에 관련된다. 상기는 대개 대식세포에 의해 생산되며; 그의 합성은 세균 내독소에 의해 자극되고 스테로이드에 의해 억제될 수 있다.
상기는 다수의 장기 및 시스템상에서 다른 사이토킨과 함께 작용하여, 염증 반응을 촉진하고, 차례로 다수의 병리(자가면역 질병 포함), 예를 들어 RA 및 OA, 크론병, 건선 및 천식을 점화하며; 상기 사이토킨은 또한 파골세포를 활성화시킬 수 있고 따라서 골 재흡수를 유도할 수 있으며, 산화 작용과 함께 분자의 대식세포 생산을 자극할 수 있고, 죽상동맥경화증 및 혈관염의 발병에서 정맥 혈전의 형성에 관여하는 심혈관계의 특정한 병리와 관련되며(Alam J., Biomed Pharmacother, 2017, 92:615-633), 마지막으로 인슐린에 대한 조직의 내성을 증가시켜 II형 당뇨병의 개시를 촉진할 수 있다(Nicolau J et al., Joint Bone Spine, 2017, 84 (4):411-416).
OA 및 RA의 경우 상기 언급한 병리의 진행을 늦추거나 또는 관련 증상의 치료에 유효한 다양한 유형의 약물이 존재하지만, 이들은 모두 합성 분자로서(따라서 천연이 아님), 매우 장기간 동안 지속되어야 하는 약물학적 치료 섭생, 및 또한 투여 방법, 일반적으로는 비경구, 그러나 특히 경구 및 따라서 다수의 경우 단지 병든 관절만을 치료하고자 하더라도 전체 유기체를 수반하는 방법에 중요한 부작용, 종종 독성을 가질 수 있다.
따라서 본 발명의 목적은 질병의 초기의 관절에서 또는 같은 관절의 강화 국소화된 단계에서, 바람직하게는 경구 투여(특히 상기 질병이 후천적인 분명한 전신 발현을 갖는 경우) 및/또는 관절-내 투여에 의해, OA의 치료 및 예방 및 RA의 치료(RA에 의해 직접적으로 야기되거나 또는 RA에 간접적으로 관련된/RA에 의존하는 모든 결과적인 질병에 대한)에 사용하기에 특히 유효한 히알루론산(HA)/카르노신 접합체를 포함하는 약학 조성물을 규명하는 것이다.
카르노신은 체내에서 β-알라닌 및 L-히스티딘간의 축합 반응으로부터 수득되는 디펩티드이다. 상기는 근육 및 뇌에서 다량으로 발견되며; 과학 문헌은 상기가 항산화, 항-라디칼 및 소염 활성을 제공하는데 만장일치로 동의한다(Budzen S. et al., Adv Clin Exp Med, 2013, 22 (5): 739-44).
전-임상 연구는 백내장의 예방 및 치료에서 그의 항-라디칼 능력을 입증한 반면(Babizhayev M. et al., Biochim, Biophys, Acta, 1989, 1004(3):363-371), 그의 소염 활성은 소화계의 일부 조직, 눈 및 피부에서 입증되었으며(US4508728, WO01/52808); 라디칼 하이드록실에 대한 스캐빈저로서의 그의 특이 활성이 또한 입증되었다(La Mendola D. et al., Helv. Chim. Acta, 2002, 85 (6):1633-1643).
그러나, 카르노신의 펩티드 성질은 그의 용도에 대해 일부 제한, 그의 열불안정성과 관련된 제한 및 특정한 펩티다제에 기인한 그의 높은 생체내 분해성을 부과한다. 이러한 제한을 극복하기 위해서, N-아세틸-카르노신의 사용(WO95/10294)이 제안되었으며, 상기는 유리 버전보다 훨씬 더 느리게 분해되고(Babizhayev MA et al., Clin. Chim. Acta, 1996, 254 (1):1-21), 동시에 카르노신의 화학적 유도체가 또한 약물 담체로서 약물에 사용되는 천연 사이클릭 올리고사카라이드인 사이클로덱스트린(EP1176154)에 의해 합성되었다. 이 경우에, 사이클로덱스트린은 디펩티드를 안정화시켜, 상기를 카르노시나제의 가수분해 활성으로부터 보호하고(Schaschke N. et al., JACS, 1998, 120 (28):7030-7038), 따라서 상기 언급된 분자가 그의 생물학적 활성을 발휘할 수 있게 한다. 같은 원리에 근거하여, 트레할로스와의 카르노신 접합체가 항-산화, 항-당화 및 항-응집 활성을 갖는 시스템으로서 후속적으로 특허를 받았다(EP1860116).
HA의 유도체가 또한 공지되어 있으며, 이는 카르노신을, HA의 카르복실 및 디펩티드의 아미노기를 수반하는 아미드 결합에 의해 상기 폴리사카라이드와 접합시킴으로써 수득된다(WO2016016847).
그러나, 상기 유도체에 대한 RA및 OA의 치료에서의 용도는 고려되지 않았으며; 더욱 또한, 상기 언급된 참고문헌에 따르면, 상기 유도체는 단지 25%(HA 및 카르노신간의 몰/몰 비)까지만 유도체화될 수 있었다.
본 발명은 25% 초과의 유도체화 정도를 갖는 카르노신에 의한 HA의 아미드 유도체, 따라서 당해 시점의 기술적 수준의 교시를 근거로 당해 분야의 숙련가에 의해 합성이 가능한 것으로 간주되지 않는 접합체를 포함하는 조성물을 기재한다.
HA는 D-글루쿠로산 및 N-아세틸-D-글루코스아민의 교번 잔기로 구성된 헤테로-폴리사카라이드이다. 상기는 상기가 수득되는 공급원 및 사용되는 제조 방법에 따라, 50,000 내지 13 x 106 Da 범위의 분자량을 갖는 직쇄 중합체이다. 상기는 실제로 주위세포 겔, 척추동물 유기체의 결합 조직의 기본 물질, 관절의 활액, 유리체액 및 탯줄 중에 존재한다. 따라서 HA는 피부, 건, 근육 및 연골과 같은 다수 조직의 세포의 기계적 지지체로서, 및 또한 세포 생리학 및 생물학, 예를 들어 증식, 이동, 세포 분화 및 혈관형성과 관련된 다수의 과정을 조절할 수 있는(CD44 수용체를 통해) 활성제 모두로서 생물학적 유기체에서 중요한 역할을 하며; 조직의 수화 및 관절의 윤활의 유지에서 그의 역할이 또한 공지되어 있다. HA는 실제로 특정한 점탄성 성질을 갖고, 주로 활막세포에 의해 관절 강에서 합성되고 분비되는 폴리사카라이드이며(Asari A. et al., Arch. Histol. Cytol., 1995, 58 (1):65-76) 활액의 주성분 중 하나이다. 관절이 느리게 움직이는 동안 HA는 점성 윤활제로서 작용하는 반면, 빠르게 움직이는 동안에는 탄성 성질과 함께, 관절을 때릴 수 있는 임의의 가능한 외상 또는 미세외상을 흡수한다.
비-병적인 활액에서의 HA의 교환은 일반적으로 대단히 빠른 반면, OA에서는 평균 분자량(MW)의 강하와 함께 농도의 감소, 특히 그의 교환 흐름의 급격한 감소가 발견되었다(Balazs EA, et al., J Rheumatol, Suppl., 1993, 12: 75-82).
이러한 이유로 인해, 발라즈(Balazs)는 처음으로 관절강에서 직접적인 외인성 HA(특히 고 MW를 갖는)의 기여를 통해 골관절염 과정의 발생을 변형시킬 가능성을 제시하였으며, 이러한 치료법 덕분에 염증성 질병 또는 외상에 의해 손상된 관절의 연골에 관련된 퇴행에 대한 HA의 보호 효과를 입증할 수 있었다.
본 발명의 목적은 OA의 예방 및 치료, RA 및 RA에 의해 직접적으로 야기되거나 RA와 간접적으로 관련된/RA에 의존하는 병리의 치료에 사용하기 위한, 특히 폐 섬유증, 흉막염 및 흉막심낭염, 안구건조증, 포도막염 및 공막염을 포함하는 그룹 중에서 선택된 RA에 의해 직접적으로 야기되거나, 또는 병적인 조기 노화(premature pathological aging), 심장마비(heart attack), 뇌졸중(stroke), 동맥경화증(arteriosclerosis) 및 관상동맥 죽상동맥경화증(coronary atherosclerosis), 동맥성 고혈압(arterial hypertension), 치매, 당뇨병, 골다공증 및 아밀로이드증(amyloidosis), 암, 단백뇨(proteinuria) 및 신염(nephritis), 위 병변(gastric lesions), 백내장, 건선성 관절염, 위염(gastritis), 혈관염(vasculitis), I형 당뇨병 및 자가면역 갑상선염(autoimmune thyroiditis)을 포함하는 그룹 중에서 선택된 RA와 간접적으로 관련된/RA에 의존하는 질병의 치료에 사용하기 위한, HA의 카르복실기 및 카르노신의 아미노기 간의 직접적인 아미드 결합에 의한 히알루론산(HA)/카르노신 접합체를 포함하는 약학 조성물에 관한 것이다. 상기 용도는 이들 질병이 유래되는 RA의 직접적인 치료 결과로서 확실한 치료 이득을 도출할 것이다.
본 발명에 따라 사용하기 위한 약학 조성물은 바람직하게는 25% 초과(25% 내지 100%), 바람직하게는 30% 내지 100%의 범위, 보다 바람직하게는 30% 내지 60%의 범위, 및 훨씬 더 바람직하게는 35±3%의 범위(즉 32% 내지 38%) 내지 45±5%(즉 40% 내지 50%)의 범위내에서 변하는 HA의 카르복실기의 유도체화(아미드화) 정도(DS)를 갖는 HA/카르노신 접합체를 포함한다.
카르노신을 갖는 HA의 상기 아미드 유도체(즉 HA/카르노신 접합체)는 디펩티드(즉 카르노신)를, HA의 카르복실 및 카르노신의 아민기 간의 직접적인 아미드 결합(영구적인 이격자의 도움 없이)의 형성에 의해 히알루론산(HA)과 공유 결합시킴으로써 수득된다.
하기에 입증되는 바와 같이, 출원인은 실제로 놀랍게도 본 발명의 목적 접합체가 어떻게 RA 및 OA의 치유적 치료에서 카르노신과 HA간에 뜻밖의 상승작용 효과를 갖고, 염증 및 지질 산화 모두의 혈장 지표의 현저한 감소와 함께 상기 언급된 병리의 현저한 임상적/조직학적 개선을 성취하는 지를 발견하였다. 본 발명의 목적 접합체의 유도체화 정도를 하기 실시예 4에 기재된 방법에 의해 측정한다.
본 발명의 HA/카르노신 접합체의 합성에 사용되는 HA는 임의의 공급원으로부터, 예를 들어 수탉 벼슬로부터의 추출에 의해(EP138572), 발효에 의해(스트렙토코커스 에퀴이(Streptocuccus Equi) 또는 주에피데미쿠스(Zooepidemicus)로부터)(EP716688), 또는 생합성에 의해(바실러스(Bacillus)로부터)(EP2614087) 유도될 수 있으며, 400 내지 3x106 Da, 특히 1x 105 Da 내지 1x106 Da의 범위, 훨씬 더 특히 130-220 kDa의 범위 이내 및/또는 500-750 kDa의 범위 이내의 평균 분자량(MW)을 가질 수 있다.
HA의 유도체화 정도가 30 내지 60%의 범위 이내로 다양하고 HA의 평균 분자량이 1x 105 Da 내지 1x106 Da의 범위인 약학 조성물이 바람직하며; HA의 DS가 30 내지 60%의 범위 이내로 다양하고 HA의 평균 분자량이 130-220 kDa, 또는 500-750 kDa의 범위 이내인 약학 조성물 및 관련 혼합물이 보다 바람직하다.
HA의 DS가 31 내지 34%(35±3%)의 범위 이내 또는 40 내지 50%(45±5%)의 범위 이내로 다양하고, HA의 평균 분자량이 130-220 kDa 또는 500-750 kDa의 범위 이내인 약학 조성물 및 관련 혼합물이 훨씬 더 바람직하며; 특히, HA의 DS가 35±3%의 범위 이내로 다양할 때, HA의 평균 분자량이 500-750 kDa의 범위 이내이고, HA의 DS가 45±5%의 범위 이내로 다양할 때, HA의 평균 분자량이 130-220 kDa의 범위 이내인 약학 조성물이 바람직하다.
평균 분자량은 "고유 점도" 방법으로 계산된 중량 평균 MW를 지칭함에 유의해야 한다(Terbojevich et al., Carbohydr Res, 1986, 363-377).
문제의 접합체의 합성에 사용되는 히알루론산은 약 40% 이하(따라서 25% 내지 40%)의 DS로 HA/카르노신 접합체의 합성에 바람직한 HA의 나트륨 염(EP138572), 및 40% 초과(따라서 40% 내지 100%)의 DS를 갖는 HA/카르노신 접합체의 합성에 바람직한 HA의 테트라부틸암모늄(TBA) 염 중에서 선택되며, 최종 생성물(하기에 제공된 제조 실시예에 기재됨)은 (바람직하게는) 시약으로서 사용된 HA 염과 관계 없이, HA/카르노신 접합체의 나트륨 염으로서 제조된다.
본 발명의 추가의 목적은 본 명세서에서 "HA/카르노신 생체적합물질"로서 정의된, HA에 대해 기재된 방식 및 정도로 카르노신과 접합된 HA 유도체(다시 아미드 결합을 통해)로 이루어지는 생체적합물질에 관한 것이다.
상기 언급된 생체적합물질의 형성에 사용될 수 있는 HA 유도체를 하기에 나열한다:
HYAFF®: 사용되는 알콜의 유형 및 길이에 따라, 바람직하게는 1 내지 70%로 변할 수 있는 에스테르화 백분율을 갖는 지방족, 아르지방족, 지환족, 방향족, 사이클릭 및 헤테로사이클릭 시리즈의 알콜과의 HA의 에스테르, 반면에 에스테르화되지 않은 HA의 나머지 백분율은 유기 및/또는 무기 염기로 염화될 수 있다(EP216453);
HYADD®: 0.1 내지 50% 범위의 아미드화 백분율을 갖는 지방족, 아르지방족, 지환족, 방향족, 사이클릭 및 헤테로사이클릭 시리즈의 아민과의 HA의 아미드, 반면에 아미드화가 가해지지 않은 HA의 나머지 백분율은 유기 및/또는 무기 염기로 염화될 수 있다(EP1095064);
ACP®: 20% 이하의 에스테르화 백분율, 바람직하게는 0.05 내지 10%의 에스테르화 범위를 갖는 HA의 내부 에스테르, 반면에 에스테르화되지 않은 HA의 나머지 백분율은 유기 및/또는 무기 염기로 염화될 수 있다(EP341745);
HYOXX®: 0.1 내지 100%, 및 바람직하게는 25 내지 75% 범위의 퍼카르복실화 정도를 갖는 N-아세틸-글루코스아민 분획의 1차 하이드록실의 산화에 의해 수득된 HA의 퍼카르복실레이트의 유도체. HA의 카르복실기는 유기 및/또는 무기 염기로 염화될 수 있다(EP1339753).
4차 황산화도 이하의 HA의 O-황산화된 유도체(EP702699).
또한 HA/카르노신 접합체로부터 출발하여 상기 언급된 HA/카르노신 생체적합물질을 합성하여(당해 분야의 숙련가에게 공지된 방법에 따라) HA를 추가로 유도체화하는 것이 또한 가능하다: 따라서 출발 물질로서 문제의 접합체를 사용하여(HA 나트륨 염 또는 TBA 염 대신에) 당해 분야의 숙련가에게 공지된 방법에 따라 HYAFF, ACP, HYADD, HYOXX 또는 황산화된 유도체의 합성을 진행하여, 앞서 HA/카르노신 생체적합물질로서 정의된 생성물을 수득하는 것이 가능하다.
HA의 유도체 중에서, 에스테르 및 아미드가 본 발명의 목적 생체적합물질의 형성 공정에 특히 중요한 것들이며; 상기 프로파일하에서 특히 중요한 것은 바람직하게는 5 내지 50% 범위의 에스테르화 백분율을 갖는 벤질 에스테르(HYAFF) 및 5% 이하(HPLC에서 측정됨)의 유도체화 정도를 갖는 HA의 헥사데실 아미드(HYADD4)이며, 상기 아미드는 관절 손상의 경우에 활막 대용물로서 임상적으로 사용될 수 있는 점탄성 겔로서의 그의 역할에 특히 적합한 것으로 입증되었다(EP1853279).
다수의 과학 실험은 HYAFF가 어떻게 완전히 생체적합성 생분해성 중합체인지(Campoccia D. et al., Biomaterials, 1998, 19:2101-2127) 및 부직포 형태로 또는 스펀지로서 가공되는 섬유의 형성을 위한 HA의 에스테르 유도체의 용도(EP618817)가 어떻게 피부과학 및 정형외과학 분야에 사용될 수 있는 3차원 기질을 구성하는 지를 상세히 입증하였다.
ACP 유도체는 피부 충전제로서 및 부착-방지 겔로서 성공적으로 사용된 반면(EP0850074), HA의 황산화된 유도체의 경우, 피부과학용 조성물의 국소 성분으로서 그의 소염 효과(EP2429515), 또는 경구 또는 관절-내 조성물의 경우에 그의 관절 기질의 보호/재생 효과(EP2021078, EP2786782)가 연구 중에 있다.
결과적으로, 카르노신과 상기 언급된 HA 유도체의 카르복실의 아미드 결합에 의해 형성된 신규의 생체적합물질은 본 발명에 따라, 섬유, 겔, 하이드로겔, 미소구, 스펀지, 직물 또는 부직포, 필름의 형태로 가공되는 생체적합물질로서, RA(RA에 의해 직접적으로 야기되거나 또는 RA와 간접적으로 관련된/RA에 의존하는 모든 질병에 더하여) 및 OA의 치료(및 또한 OA의 예방에서)에 유효하게 사용된다.
RA에 의해 직접적으로 야기되는 질병은 관절 이외의 장기 및 기관을 수반하는 동일한 병리의 전신적인 임상 발현이다. 구체적으로, 류머티스양 결절이 폐 수준에서 형성된 RA(결과적으로 폐 섬유증, 흉막염 및 흉막심낭염)가 존재하는 반면, 눈 수준에서 안구건조증, 포도막염 및 공막염이 존재한다.
RA와 간접적으로 관련된/RA에 의존하는 병리는 본 명세서에서, 상기 병리가 변경된 면역 프로파일의 심각한 상황 및/또는 높은 산화적 스트레스의 상태와 연계되므로, RA에 의해 간접적으로 야기된 병리로서 정의된다.
변경된 유리 라디칼 생성은 실제로 결합 조직의 구조, 단백질의 구조, 세포막 및 DNA 유전자에 심각한 손상을 야기할 수 있다. 따라서 산화적 스트레스는 유리 라디칼의 생성과 폐기간의 복잡한 균형의 파괴에 의해 야기되는 병적인 상태로서 간주될 수 있다.
산화적 스트레스는 현재 RA, 및 결과적으로 RA에 전후관계로 빈번히 발생하고/하거나 RA에 의존하는 다른 중요한 질병, 예를 들어 병적인 조기 노화, 심장마비, 뇌졸중, 동맥경화증 및 관상동맥 죽상동맥경화증, 동맥성 고혈압, 치매, 당뇨병, 골다공증 및 아밀로이드증, 다수 형태의 암, 몇몇 간 또는 신장 질환, 예를 들어 단백뇨 및 신염, 위 병변, 백내장의 개시에 능동적으로 기여하는 것으로서 간주된다.
다른 한편으로, 변경된 면역 프로파일에 연계된 질병은 건선성 관절염, 위염, 혈관염, I형 당뇨병, 및 예를 들어 자가면역 갑상선염을 포함한 자가면역 질병을 포함한다.
상술한 병리 및 관련된/의존적인 질병의 예방 또는 치료와 관련하여 투여가 요구되고 약물학적으로 및/또는 생물학적으로 활성인 물질, 예를 들어 스테로이드, 소염성 사이토킨, 인터페론, 성장 인자(예를 들어 BMP2 및 BMP7), NSAID, 및/또는 조절된 약물 전달 시스템, 예를 들어 사이클로덱스트린, 및/또는 천연 중합체, 예를 들어 HA 및 그의 유도체, 바람직하게는 HYADD4(EP1095064)라 칭하는 헥사데실아민과의 HA의 아미드 유도체, 또는 합성 성질의 상기 유도체와 관련될 수 있는 모든 분야에서, 국소, 경구, 관절-내, 비경구 또는 외과적 적용을 위한 HA/카르노신 접합체 및 HA/카르노신 생체적합물질을 포함하는 약학 조성물을 기재한다.
본 발명의 목적인 HA/카르노신 접합체의 합성을 하기에 기재하며, 카르노신 메틸 에스테르 및 히알루론산 나트륨 염 또는 테트라부틸암모늄 염 간의 상기 접합체의 접합 과정은 주 용매로서 DMSO 또는 DMSO/H2O를 포함하는 매질 중에서 일어난다.
상기 논의된 특허 출원 WO2016016847에, THF 중에서의 HA의 나트륨 염 및 카르노신 메틸-에스테르 간의 접합 과정이 독점적으로 기재되었다.
따라서, 본 발명에 따른 방법에서, 상기 합성은 비양성자성 및 강한 흡습성의 이극성 용매 중에서 실온에서 발생한다. 당해 시점의 기술적 수준에 따른 THF 중에서의 합성은 보다 낮은 극성의, 비양성자성, 휘발성 용매 중에서 수행되었으며 따라서 4℃에서 수행되었다.
나트륨 염과 상이한 HA의 염의 사용을 또한 허용하는 상이한 반응 온도와 관련된, 상이한 용매의 화학적 특성은 하기에 기재되고 특허청구된 카르노신에 의한 HA의 유도체화 정도가 획득되게 하였다.
본 발명의 목적인 HA/카르노신 접합체의 제조 방법
실시예 1
카르노신 메틸-에스테르(하이드로클로라이드 염으로서)의 합성
반응을 무수 환경하에서 수행하였다. 1.5 g의 카르노신을 250 ㎖ 플라스크 내부에서, 1:20 비(v/v)의 무수 메탄올 중의 염화 아세틸 용액(예비-혼합된) 50 ㎖로 12시간 동안 처리하였으며, 그 후에 상기 용매의 약 90%를 진공하에서 증발에 의해 제거하였다. 20 ㎖의 무수 메탄올을 반응 잔사에 가하고 다시 상기 용매의 약 90%를 증발에 의해 제거하였다. HCl(반응 중 형성되었다)이 전부 제거될 때까지 상기 작업을 반복하였으며; 이어서 생성물을 진공하에서 건조시켰다.
생성물을 페이퍼 상에서 전기영동에 의해 조절하고 샘플의 다양한 성분을 닌하이드린을 사용하여 검출하였다.
실시예 2
35±3% mol/mol의 범위내에 포함되는 DS를 갖는 HA/카르노신 접합체의 합성
700 kDa의 MW를 갖는 히알루론산 나트륨 염(HANa) 1.1 g을 반응기에 도입시키고 여기에 80 ㎖의 H2O:DMSO(1:1 v/v의 비)의 혼합물을 적어도 4시간 동안 일정하게 교반하면서(실온에서) 후속적으로 가하였다. 이어서 480 ㎕의 트리스 [2-(2-메톡시에톡시)에틸]아민(TMEA)를 함유하는 H2O:DMSO(1:1 v/v) 용액 40 ㎖, 분말 형태의 400 ㎎의 3-하이드록시-1,2,3-벤조트리아진-4(3H)-온(HOOBt), 및 240 ㎎의 N-(3-디메틸아미노프로필)-N'-에틸카보디이미드 하이드로클로라이드(EDC HCl)를 함유하는 H2O 20 ㎖을 차례로 가하였다.
30분 후에, DMSO 중의 365 ㎎의 카르노신 메틸 에스테르를 함유하는 용액 20 ㎖을 가하였다. 이어서 상기 혼합물을 25℃에서 4일 동안 격렬히 교반하였다.
이어서 상기 반응의 생성물을 에탄올(1 L)을 가하여 침전시키고 후속적으로 카르노신의 메틸 에스테르의 탈보호(염기성 가수분해)를 25℃에서 2시간 동안 일정한 교반하에 0.1 N NaOH로 수행하였다.
이어서 pH를 1 N HCl을 가하여 중화시키고 생성물을 800 ㎖의 에탄올로 침전시켰다. 이어서 상기 침전물을 물에 용해시키고, 2일 동안 수 중에서 투석시키고 후속적으로 동결건조시켰다.
실시예 3
50±5% mol/mol의 범위내에 포함되는 DS를 갖는 HA/카르노신 접합체의 합성
200 kDa의 MW를 갖는 히알루론산 테트라부틸암모늄 염(HATBA) 1.5 g을 반응기에 도입시키고 여기에 50 ㎖의 DMSO를 적어도 4시간 동안 일정하게 교반하면서(실온에서) 후속적으로 가하였다. 이어서 2.4 ㎖의 트리스 [2-(2-메톡시에톡시)에틸]아민(TMEA), 분말 형태의 400 ㎎의 3-하이드록시-1,2,3-벤조트리아진-4(3H)-온(HOOBt), 및 240 ㎎의 N-(3-디메틸아미노프로필)-N'-에틸카보디이미드 하이드로클로라이드(EDC HCl)를 함유하는 DMSO 20 ㎖을 차례로 가하였다.
30분 후에, DMSO 중의 365 ㎎의 카르노신 메틸 에스테르를 함유하는 용액 10 ㎖을 가하였다. 이어서 상기 혼합물을 25℃에서 1일 동안 격렬히 교반하였다.
최종적으로 240 ㎎의 N-(3-디메틸아미노프로필)-N'-에틸카보디이미드 하이드로클로라이드(EDC, HCl)를 함유하는 DMSO 10 ㎖, 및 DMSO 중의 365 ㎎의 카르노신 메틸-에스테르를 함유하는 용액 10 ㎖의 두 번째 첨가를 수행하였다.
상기 혼합물을 25℃에서 3일 동안 격렬히 교반하면서 방치하였다.
이어서 상기의 반응 생성물을 5 ㎖의 NaBr의 포화 용액 및 800 ㎖의 에탄올을 가하여 침전시키고 이어서 0.1 N NaOH에 의한 카르노신의 메틸 에스테르의 탈보호(염기성 가수분해)를 25℃에서 2시간 동안 일정한 교반하에 수행하였다.
이어서 pH를 1 N HCl을 가하여 중화시키고 생성물을 800 ㎖의 에탄올로 침전시켰다. 이어서 상기 침전물을 물에 용해시키고, 2일 동안 수 중에 다시 투석시키고 후속적으로 동결건조시켰다.
실시예 4
실시예 2 및 3에 따라 제조된 샘플의 HPLC 분석
(HA의 카르복실에 결합된 카르노신 중에 존재하는 β-알라닌의 정량 분석)
샘플의 제조:
- HA/카르노신 접합체를 70℃에서 5분간 25 ㎎/㎖의 농도로 6 M HCl 중에 용해시키고, 이어서 6 M HCl 중에서 1/10 희석시키고;
- 이어서 0.1 ㎖의 샘플 용액을 시험-튜브로 옮기고 여기에 1.9 ㎖의 6 M HCl을 가하고;
- 샘플을 165℃에서 2시간 동안 가수분해시키고, 냉각되게 하고, 이어서 8 ㎖의 H2O MilliQ를 가하고, 전체 샘플을 완전히 재현탁될 때까지 혼합하고 여과하고;
- 이어서 8 ㎖의 상기 여과된 샘플을 20 ㎖ 플라스크로 옮기고, 여기에 10 ㎖의 1 M NaOH를 가하고;
- 상기와 같이 제조된 샘플 0.2 ㎖을 바이알로 옮기고 이어서 0.2 ㎖의 0.2 M 보레이트 완충제, pH 9.3 및 0.2 ㎖의 FMOC-Cl(ACN(아세토니트릴) 중에 1 ㎎/㎖의 농도로 제조됨)을 가하고; 반응시간: 적어도 30분;
- 최종적으로 샘플을 UV 검출기와 함께 하기의 HPLC 방법을 사용하여 HPLC에 주입하였다:

결과: 실시예 2의 샘플은 35% mol/mol의 HA의 카르복실에 대한 유도체화 정도를 나타낸 반면, 실시예 3의 샘플은 50% mol/mol과 같은 유도체화 정도를 나타내었다.

실시예 5
RA의 실험 모델: 마우스에서 콜라겐-유발된 관절염(CIA)
HA/카르노신 접합체에 의한 경구 치료
CIA는 동물에서 RA의 체액, 세포, 조직학 및 병리학적 특징을 결정하므로(Holmdahl et al., Immunological reviews, 1990, 118: 193-232), 마우스 또는 래트에서 RA의 유효한 실험 모델로서 수년간 인정되어 왔다. 실제로 마우스에서 CIA의 도입에 이어서, 다수의 활성화된 호중구/대식세포/림프구가 마우스의 관절 중에 존재하며, 결과적으로 RA의 활막 판누스가 생성된다.
CIA 유도: II형 치킨 콜라겐(CII)을 0.01 M 아세트산 중에 2 ㎎/㎖의 농도로 용해시키고, 이어서 완전한 프로인트 항원보강제(CFA)를, 불완전 항원보강제(수/무기 오일 유화액으로 구성됨)에 마이코박테리움 투베르쿨로시스 H37Ra(2 ㎎/㎖)를 첨가하여 제조하고, 이어서 상기 두 성분(CII 및 CFA)을 혼합하고 동일한 비 v/v로 유화시켰다. 상기와 같이 수득된 유화액 100 ㎕(100 ㎍의 CII 함유)를 피내 주사에 의해 마우스의 꼬리 기부에 주사하였다(1일). 두 번째의 동일한 주사를 21일 후에 수행하였다.
실험 그룹:
각각 30 g의 9-주 된 DBA/IJ 수컷 마우스를 그룹 당 20마리의 마우스로 사용하였다.
그룹 1: CIA-대조군: 동물을 CIA 유도에 의해 처리하고 후속적으로 증류수를 CIA 후 25일째부터 시작하여 실험이 종료되는 35일까지 24시간마다 경구 투여하였다.
그룹 2: CIA + HA/카르노신 접합체: 동물을 CIA 유도에 의해 처리하고 후속적으로 CIA 후 25일째에 시작하여 35일까지 24시간마다 상기 언급된 접합체로 처리하였다. 시험에 사용된 접합체를 실시예 3에 따라 제조하였으며, 따라서 상기는 50% mol/mol로 유도체화되었고, 경구 투여된 용량은 81 ㎎/㎏의 접합체(20 ㎎의 카르노신 및 61 ㎎의 HA)였다.
그룹 3: CIA + HA/카르노신 혼합물: 동물을 CIA 유도에 의해 처리하고 후속적으로 CIA 후 25일째에 시작하여 35일까지 24시간마다 HA/카르노신으로 처리하였다. 상기 제제는 수 중의 카르노신과 혼합된 HA 나트륨 염의 혼합물로 이루어졌으며, 61 ㎎/㎏의 HA와 함께 20 ㎎/㎏의 카르노신의 용량으로 경구 투여되었다.
임상 평가
질병의 발생을 각각의 단일의 발에 대해 하기의 평가 계획에 따라 첫 번째 주사 후 20일째에 매일 평가하였다:
0 = 관절염 발생의 표시 없음
1 = 발의 팽창 및/또는 발적
2 = 적어도 2개의 관절에서 발의 팽창 및/또는 발적
3 = 적어도 3개의 관절에서 발의 팽창 및/또는 발적
4 = 모든 손가락을 포함하여 전체 발의 심한 형태의 관절염의 발생.
각각의 동물에 대해서, 전체 CIA 발생 지수(관절염 발 점수로서 정의됨)를 각각의 발에 대해 획득된(및 상기와 같이 평가된) 값을 합하여 계산하였다.
임상적 중증도 지수는 질병의 중증도를 근거로 전체 분석 기간 전체에 걸쳐 점차적으로 진행된 발의 부피(체적변동유량계에 의해 2일마다 측정됨)에 의해 제공된다: 발 증가.
조직학적 평가
평가 기간의 끝에서, 즉 실험 시작 후 35일째에, 동물을 죽이고, 처리된 발을 제거하여(무릎 관절 포함) 10% 포르말린 중에 고정시켰으며; 후속적으로 칼슘을 제거하고 5 ㎛ 마이크로톰 절편의 제조를 위해 파라핀에 담그고, 이어서 헤마톡실린/에오신으로 염색하였다. 광학 현미경 분석을 하기의 조직학 점수에 따라 수행하였다:

혈장 분석:
TNF-α의 혈장 정량분석
TNF-α 수준의 측정을 위해서(판독 한계 10 pg/㎖), 칼바이오켐-노바바이오켐 코포레이션(Calbiochem-Novabiochem Corporation, IT)의 ELISA 키트를 사용하여 실험 종료 시 동물 혈장에서 TNF-α 수준을 측정하였다.
지질 과산화 지수의 혈장 정량분석
생물막 중에 존재하는 지질의 유리 라디칼에 의한 공격은, 생물막의 완전성의 손상, 및 말론디알데히드(MDA)를 포함한 알데히드와 같은 부산물과 함께, 산화된 지질 단백질 및 지질 과산화물의 형성을 유도하는 산소-의존적인 악화 과정(지질 과산화)의 시작을 결정한다. 이들 과정은 전부 산화적 스트레스를 유도한다. CIA의 개시로 인해 마우스에서 야기된 유리 라디칼 손상을 측정하기 위해서, 실험의 종료 시 채취한 처리된 그룹의 혈장 샘플(따라서 상기 알데히드를 함유한다)을, MDA에 의해 제공된 표준(즉 99% 테트라메톡시프로판 용액, 시그마(Sigma), 이탈리아 밀라노 소재)과 비교하여, 바르비투르산(TBAR)과 반응시켰다. 결과는 분광광도계 OD650 ㎚로 용이하게 검출가능한 부가물이었다(Ohkawa H et al., Anal. 임상 평가
도 1에 나타낸 바와 같이(관절염 발 점수), 병리학적 발현의 최대 지수를 CIA 처리 후(대조군) 30 내지 35일째의 발의 관절주위 홍반 및 부종의 분명한 징후로 나타내며; HA/카르노신 접합체의 경구 투여는 또한 CIA 그룹 + HA/카르노신 혼합물에 비해, 특히 30일째에 관절 염증의 현저한 감소를 야기하였다. 효능, 실험된 DS에 공유 결합시 HA 및 카르노신 간의 상승작용 지수의 이러한 현저한 차이는 실험이 끝날때까지 훨씬 더 오랜 시간 동안 유지된다.
도 2는 발 증가의 경향을 도시한다: 또한 이 경우에 본 발명의 목적 접합체에 의한 처리는 도 1의 놀라운 효과를 입증하며, 여기에서 실험 종료 시, 상기 접합체로 처리된 발의 팽창은 처리되지 않은 대조군 및 또한 HA/카르노신 혼합물로 처리된 그룹 모두에 비해 현저하게 더 낮았다.
조직학적 평가
도 3: 실험 35일(종료)째에, CIA-대조용 동물의 조직학적 평가는 골 미란 및 관절 조직의 보통의/중증의 괴사와 함께 심각한 형태의 RA의 발생의 분명하고 중요한 조직학적 징후를 밝혀내었다. 다른 한편으로, HA/카르노신 접합체에 의한 처리는 상기 미란을 대조군 및 HA/카르노신 혼합물로 처리된 그룹 모두에 비해 현저하게 감소시켰다.
혈장 분석: TNF-α의 혈장 정량분석
실험 종료 시, 염증-전 사이토킨 TNF-α의 혈장 수준을 CIA가 가해진 및 가해지지 않은(모의 샘플, 즉 비교로서 수술되지 않은 건강한 동물의 혈장 TNF 값을 갖도록 CIA가 유도되지 않았다) 동물 모두에서 분석하였다.
도 4에 도시된 바와 같이, CIA-대조군에서 상기 언급된 염증성 사이토킨의 값은 대단히 높아, 모의 대조군보다 거의 5배 더 높은 반면, 본 발명의 목적 접합체에 의한 처리는 TNF-α 값을 절반 넘게 줄였으며, 또한 상기 값을 HA/카르노신 혼합물이 투여된 그룹에 비해 현저하게 감소시켰다.
실시예 6
OA의 실험 모델: 래트에서 일나트륨 요오도아세테이트(MIA) 골관절염의 유도

HA/카르노신 접합체에 의한 관절-내 처리
실험 그룹:
각각 약 230 g의 수컷 스프래그-다우리 래트를 그룹 당 20마리의 래트로 사용하였다.
그룹 1: MIA-대조군: 동물을 MIA 유도에 의해 처리하고 후속적으로 25 ㎕의 증류수를 MIA 후 7, 10, 13, 16 및 19일째에 오른쪽 무릎에 관절-내 투여하고, 21일째에, 동물을 안락사시켜 실험을 종료하였다.
그룹 2: MIA + HA/카르노신 접합체: 동물을 MIA 유도에 의해 처리하고 후속적으로, 실시예 2에 따라 제조되고 이어서 35% mol/mol로 유도체화된 접합체 25 ㎕로, 21일까지 MIA 후 7, 10, 13 및 16 19일째에 오른쪽 무릎에 관절-내 처리하였다. 25 ㎕의 HA/카르노신 접합체는 250 ㎍의 접합체, 즉 45.75 ㎍의 카르노신 및 204.25 ㎍의 HA를 함유하였다.
그룹 3: MIA + HA/카르노신 혼합물: 동물을 MIA 유도에 의해 처리하고 후속적으로 25 ㎕의 HA 및 카르노신으로 21일까지 MIA 후 7, 10, 13, 16 및 19일째에 오른쪽 무릎에 관절-내 처리하였다. 상기 제제는 수 중의 카르노신과 혼합된 HA 나트륨 염의 혼합물로 이루어졌으며, 따라서 25 ㎕의 상기 제제는 45.75 ㎍의 카르노신 및 204.25 ㎍의 HA를 함유하였다.
TNF-α의 혈장 정량분석
실험 종료 시, 상기 TNF-α 수준의 측정을 위해서(판독 한계 10 pg/㎖) 칼바이오켐-노바바이오켐 코포레이션(이탈리아 소재)의 ELISA 키트를 사용하여 동물의 혈장에서 TNF-α 수준을 측정하였다.
결과
도 6은 처리 그룹, 및 동물에게 MIA 처리가 가해지지 않은 그룹으로서 정의된 모의 그룹에서 획득된 결과를 도시한다: 또한 이 경우에, MIA-대조군의 TNF 사이토킨의 혈장 수준은 대단히 높은 것으로 입증된 반면, HA/카르노신 접합체 처리는 TNF-α 값을 상기 대조군에 비해서뿐만 아니라, 어떠한 효능도 보이지 않은 HA/카르노신 혼합물이 투여된 그룹에 비해서 현저하게 낮추었으며, 이는 본 발명의 목적 접합체의 현저한 효과, 및 따라서 당해 시점의 기술적 수준에 관하여 특히 높은 그의 유도체화 수준을 다시 한번 입증한다.
HA/카르노신 접합체에 의한 경구 처리
실험 그룹:
각각 약 230 g의 수컷 스프래그-다우리 래트를 그룹 당 20마리의 래트로 사용하였다.
그룹 1: MIA-대조군: 동물을 MIA 유도에 의해 처리하고 후속적으로 증류수를 MIA 후 3일째부터 실험이 종료되는 20일까지 24시간마다 경구 투여하였다.
그룹 2: MIA + HA/카르노신 접합체: 동물을 MIA 유도에 의해 처리하고 후속적으로 MIA 후 3일째부터 20일까지 24시간마다 상기 언급된 접합체로 처리하였다. 실험에 사용되는 접합체를 실시예 2에 따라 제조하였으며, 이어서 상기를 35% mol/mol로 유도체화하였고, 경구 투여된 용량은 88.5 ㎎/㎏의 접합체(16.2 ㎎의 카르노신 및 72.3 ㎎의 HA)와 같았다.
그룹 3: MIA + HA/카르노신 혼합물: 동물을 MIA 유도에 의해 처리하고 후속적으로 MIA 후 3일째에 시작하여 20일까지 24시간마다 HA/카르노신으로 처리하였다. 상기 제제는 수 중의 카르노신과 혼합된 HA 나트륨 염의 혼합물로 이루어졌으며, 72.3 ㎎/㎏의 HA와 함께 16.2 ㎎/㎏의 카르노신의 용량으로 경구 투여되었다.
그룹 4: MIA + 나프록센: 동물을 MIA 유도에 의해 처리하고 후속적으로 MIA 후 3일째에 시작하여 20일까지 24시간마다 10 ㎎/㎏의 용량의 나프록센을 경구 투여하였다.
조직학적 평가
실험 기간의 끝에서, 즉 MIA 유도 시작으로부터 21일째에, 동물을 죽이고, 처리된 다리를 제거하고(무릎 관절 포함) 10% 포르말린 중에 고정시켰다. 상기 다리를 후속적으로 칼슘 제거하고 5 ㎛ 마이크로톰 절편의 제조를 위해 파라핀에 담그고, 이어서 헤마톡실린/에오신으로 염색하였다. 광학 현미경 분석을 0 내지 12의 점수 범위, 즉 정상적인 세포 구성에서부터 완전한 해체 및 저세포상태까지의 범위를 갖는 맨킨(Mankin)에 의한 변형된 조직학적 점수에 따라 수행하였다; (Mankin HJ et al., J Bone Joint Surg Am., 1971, 53 (3): 523-537).
결과:
그룹 1의 샘플의 조직학적 검사는 표면층의 피브릴화, 감소된 세포상태 및 연골층의 퇴행을 갖는 조직 구성의 불규칙성을 보였다. HA/카르노신 접합체에 의한 경구 처리는 조직학적 손상 및 연골 퇴행을 매우 현저하게 감소시켰으며, 이는 나프록센(문서로 기록된 유효도를 갖는 공지된 NSAID)에 의해 유도된 효과에 전적으로 필적하는 반면, HA/카르노신 혼합물에 의한 처리는 그룹 1(MIA-대조군)의 처리되지 않은 샘플에 관하여 어떠한 조직학적 개선도 도출하지 않았다(도 7).
Claims (11)
- 히알루론산(HA)의 카르복실기 및 카르노신의 아민기 간의 직접적인 아미드 결합에 의한 히알루론산/카르노신 접합체를 포함하는,
골관절염의 예방 및 치료, 및 류머티스성 관절염(RA)의 치료에 사용하기 위한 약학 조성물. - 제1항에 있어서,
폐 섬유증(pulmonary fibrosis), 흉막염(pleurisy) 및 흉막심낭염(pleuropericarditis), 안구건조증(xerophthalmia), 포도막염(uveitis) 및 공막염(scleritis)을 포함하는 그룹 중에서 선택된, RA에 의해 직접적으로 야기되는 병리의 치료, 또는
병적인 조기 노화(premature pathological aging), 심장마비(heart attack), 뇌졸중(stroke), 동맥경화증(arteriosclerosis) 및 관상동맥 죽상동맥경화증(coronary atherosclerosis), 동맥성 고혈압(arterial hypertension), 치매, 당뇨병, 골다공증(osteoporosis) 및 아밀로이드증(amyloidosis), 암, 단백뇨(proteinuria) 및 신염(nephritis), 위 병변, 백내장(cataract), 건선성 관절염(psoriatic arthritis), 위염(gastritis), 혈관염(vasculitis), I형 당뇨병 및 자가면역 갑상선염(autoimmune thyroiditis)을 포함하는 그룹 중에서 선택된, RA와 간접적으로 관련된/RA에 의존하는 병리의 치료에 사용하기 위한 약학 조성물. - 제1항 또는 제2항에 있어서,
히알루론산(HA)/카르노신 접합체가, 25% 초과, 바람직하게는 30 내지 100%의 범위, 보다 바람직하게는 30 내지 60%의 범위, 및 훨씬 더 바람직하게는 35±3% 또는 45±5%의 범위 내에서 변하는 HA의 카르복실기의 유도체화 정도, 즉 아미드화로, HA의 카르복실 및 카르노신의 아민기 간의 직접적인 아미드 결합의 형성에 의해 카르노신의 디펩티드를 히알루론산과 공유 접합시킴으로써 수득되는 약학 조성물 및 그의 혼합물. - 제1항 내지 제3항 중 어느 한 항에 있어서,
HA가 400 내지 3x106 Da의 범위, 바람직하게는 1x 105 Da 내지 1x106 Da의 범위, 훨씬 더 바람직하게는 130-220 kDa의 범위 이내 또는 500-750 kDa의 범위 이내의 평균 분자량을 갖는 약학 조성물 및 그의 혼합물. - 제1항 내지 제4항 중 어느 한 항에 있어서,
HA의 유도체화 정도가 30 내지 60%의 범위이고 HA의 평균 분자량이 1x 105 Da 내지 1x106의 범위이고, 바람직하게는 130-220 kDa 또는 500-750 kDa의 범위 이내인 약학 조성물 및 그의 혼합물. - 제1항 내지 제5항 중 어느 한 항에 있어서,
카르노신과의 HA의 아미드 유도체에 사용되는 히알루론산이 HA의 나트륨 염 또는 HA의 테트라부틸암모늄 염인 약학 조성물. - 제1항 내지 제6항 중 어느 한 항에 있어서,
국소, 경구, 관절-내, 비경구 또는 외과적 적용을 위한 약학 조성물. - 카르노신과 접합된 HA의 유도체로 이루어지는 생체적합물질로, 상기 유도체가 HA의 에스테르, HA의 아미드, HA의 내부 에스테르, HA의 퍼카르복실레이트 및 HA의 설페이트 중에서 선택되는 생체적합물질.
- 제8항에 있어서,
골관절염의 예방 및 치료; 류머티스성 관절염(RA)의 치료; 및 폐 섬유증, 흉막염 및 흉막심낭염, 안구건조증, 포도막염 및 공막염을 포함하는 그룹 중에서 선택된 RA에 의해 직접적으로 야기되는 병리, 또는 병적인 조기 노화, 심장마비, 뇌졸중, 동맥경화증 및 관상동맥 죽상동맥경화증, 동맥성 고혈압, 치매, 당뇨병, 골다공증 및 아밀로이드증, 암, 단백뇨 및 신염, 위 병변, 백내장, 건선성 관절염, 위염, 혈관염, I형 당뇨병 및 자가면역 갑상선염을 포함하는 그룹 중에서 선택된 RA와 간접적으로 관련된/RA에 의존하는 병리의 치료에 사용하기 위한, 카르노신과 접합된 HA의 유도체로 이루어지는 생체적합물질. - 제9항에 있어서,
국소, 경구, 관절-내, 비경구 또는 외과적 적용을 위한, 섬유, 겔, 하이드로겔, 미소구, 스펀지, 직물 또는 부직포, 필름 형태의, 카르노신과 접합된 HA의 유도체로 이루어지는 생체적합물질. - 제1항 내지 제10항 중 어느 한 항에 있어서,
약물학적으로 및/또는 생물학적으로 활성인 물질, 및/또는 약물 및/또는 천연 중합체 또는 합성 성질의 중합체의 조절된 방출 시스템과 관련된 약학 조성물 또는 생체적합물질.
