본 특허 출원은 2015년 11월 10일에 출원된, "근적외선, 닫힌 사슬 설포-시아닌 염료의 약학 조성물"이란 제목의 미국 가특허출원 14/937,641의 출원일에 대한 우선권 및 유익을 주장하며, 상기 출원은 그 전문이 본원에 참조로 포함된다.

본 발명의 조성물은 멸균되고 일반적으로 바람직하지 못한 물질이 없다. 화합물 및 조성물은 열/압력, 가스 플라스마, 스팀, 방사선, 화학물질 및 여과 멸균 기법들을 포함하여, 종래의, 잘 알려져 있는 기법들에 의해 멸균될 수 있다.
예를 들어, 사후 열 멸균(terminal heat sterilization)이 최종 제형 내의 모든 생존 가능한 미생물을 파괴하기 위해 사용될 수 있다. 최종 포장 제품에서 약물 제품의 사후 열-멸균을 이루기 위해 오토클레이브가 통상 사용된다. 제약 산업에서 최종 제품의 사후 멸균을 이루기 위한 전형적인 오토클레이브 사이클은 15분 동안 121℃이다. 본 발명의 조성물은 115 내지 130℃의 범위의 온도에서 5 내지 40분의 범위의 기간 동안 안정성이 허용되면서 오토클레이빙될 수 있다. 오토클레이빙은 바람직하게는 119 내지 122℃의 범위의 온도에서 10 내지 36분의 범위의 기간 동안 수행된다.

조성물은 또한 Illum과 Moeller에 의해 Arch. Pharm. Chemi. Sci., Ed. 2, 1974, pp. 167-174에 기술된 것과 같이 방사선 조사에 의해 멸균될 수 있다. 조성물은 또한 UV, x-선, 감마선, 빔 방사선, 플래밍(flaming), 굽기 및 화학적 멸균에 의해 멸균될 수 있다.
대안으로, 본 발명에 따르는 멸균 약학 조성물은 무균 처리 기법을 사용하여 제조될 수 있다. 무균 충전은 가열 멸균을 견디지 못하는 약물 제품을 제조하기 위해 통상적으로 사용되지만, 성분들은 모두 멸균되어 있다. 멸균성은 멸균 물질 및 제어된 작업 환경을 사용함으로써 유지된다. 모든 용기 및 장치가, 바람직하게는 가열 멸균에 의해, 충전 전에 멸균된다. 용기 (예컨대 바이알, 앰플, 주입 백, 병 또는 주사기)는 다음에 무균 조건하에서 충전된다.
본 발명의 화합물 및 조성물은 비-독성이고 일반적으로 근적외선으로의 조명 하에서 조직의 가시화를 위해 효과적인 수준에서 척추동물에 투여될 때 유해한 효과가 없다. 본 발명의 화합물 및 조성물의 독성은 표적 (유기체, 기관, 조직 또는 세포)에 미치는 효과를 측정함으로써 평가될 수 있다. 개별적인 표적은 전형적으로 동일 용량의 화합물에 대해 상이한 수준의 반응을 가지기 때문에, 집단의 주어진 개체에 대한 결과의 가능성과 관련된, 독성의 집단-수준 측정이 종종 사용된다. 화합물의 독물학은 시험관 내 (살아있는 유기체 외부에서) 및 생체 내 (살아있는 유기체의 내부에서) 연구를 포함하여 종래의, 잘 알려져 있는 기법들에 의해 측정될 수 있다.

Ames 역돌연변이 분석 (Ames reverse mutation Assay)이 화합물의 독성을 평가하기 위한 또 다른 공통 독물학 분석이다. Ames 분석은 여러 상이한 시험용 스트레인을 활용하는데, 각 스트레인은 히스티딘 (his) 생합성 오페론을 포함하는 유전자들 중 하나에 분명한 돌연변이를 가지고 있다 (Ames, B.N., et al., (1975) Mutation Res. 31:347-363). 히스티딘이 없을 때 성장할 수 있는, 시험 샘플 중의 복귀 돌연변이체 (즉 돌연변이체) 박테리아의 검출은 평가 중인 화합물이 유전자독성 (즉 돌연변이 유발성) 활성에 의해 특성화되는 것을 나타낸다. 게다가, 인간 에테르-에이-고-고 관련 유전자 (hERG, human ether-a-go-go related gene) 분석은 화합물의 잠재적 심장독성을 평가하기 위해 사용될 수 있다. 심장독성은 QT 간격이 연장될 때 발생하여 생명을 위협하는 부정맥의 상승된 위험을 초래할 수 있다. QT 간격은 심장의 전기 사이클에서 Q 파가 시작될 때와 T 파가 끝날 때 사이의 시간의 측정이다. 일부 구체예에서, 본 발명의 조성물은 편리한 건조 보관 및 수송을 위해 멸균 용기에 동결건조될 수 있다. 사용할 준비가 된 제제는 계속해서 동결건조된 조성물을 멸균수로 복원시킴으로써 만들어질 수 있다. 용어 "동결건조", "동결건조된" 및 "냉동-건조된"은 건조될 물질이 먼저 냉동된 후에 얼음 또는 냉동 용매가 진공 환경에서 승화에 의해 제거되는 방법을 나타낸다. 부형제가 보관시 동결건조된 제품의 안정성을 향상시키기 위해 사전-동결건조 제형에 첨가될 수 있다.

C. 투여
본 발명의 화합물 및 조성물은 경구, 비경구 및 국소 방법을 포함하여, 임의의 적합한 수단에 의해 전달될 수 있다. 국소 경로에 의한 경피 투여 방법들은 도포용 스틱, 용액, 현탁액, 에멀션, 겔, 크림, 연고, 페이스트, 젤리, 페인트, 분말 및 에어로졸로서 제형될 수 있다.
약학적 제제는 바람직하게 단위 투여 형태이다. 그런 형태에서 제제는 적절한 양의 본 발명의 화합물 및 조성물을 함유하는 단위 용량으로 세분화된다. 단위 투여 형태는 포장된 제제일 수 있고, 포장은 포장된 정제, 캡슐 및 바이알 또는 앰플 중의 분말과 같이 개별적인 양의 제제를 함유한다. 또한, 단위 투여 형태는 캡슐, 정제, 카시에 또는 마름모꼴 약용 캔디 자체일 수 있거나, 또는 이것들 중 임의의 것의 적절한 수가 포장된 형태일 수 있다.
일부 구체예에서, 공동-투여는 공동-제형, 즉, 본 발명의 화합물 및 조성물 및 임의의 다른 제제를 포함하는 단일 약학 조성물에 의해 이루어질 수 있다. 대안으로는, 다양한 성분들이 별도로 제형될 수 있다.
본 발명의 조성물, 및 임의의 다른 제제들은 임의의 적합한 양으로 존재할 수 있고, 한정하는 것은 아니지만, 환자의 체중 및 연령, 질환의 상태 등을 포함하는 다양한 인자들에 따라 좌우될 수 있다. 적합한 투여량 범위는 약 0.05 내지 8 mg/kg 또는 약 0.05 내지 5 mg/kg 또는 약 0.05 내지 1 mg/kg을 포함한다. 적합한 투여량 범위는 또한 0.05, 0.10, 0.20, 0.30, 0.35, 0.40, 0.45, 0.50, 0.55, 0.60, 0.65, 0.70, 0.75, 0.80, 0.90, 1, 2, 4, 6 또는 8 mg/kg을 포함한다.

IV. 조직의 시각화 방법
본 발명은 일반적으로 근적외선, 닫힌 사슬, 설포-시아닌 염료의 신규한, 멸균 및 비-독성 약학 조성물 및 근적외선으로의 조명 하에서 조직의 시각화 방법을 제공한다.
제1 구체예에서, 본 발명은 다음 식을 가지는 화합물의 단위 투여 형태를 포함하는 멸균 약학 조성물을 제공한다:

R1, R2, R3 및 R4는 각각 독립적으로 수소, C1-3 알킬 또는 -CO2T이고;
X는 단일 결합, -O- 또는 -S-이며;
첨자 a, b, c 및 d는 각각 독립적으로 1 내지 6의 정수이고, 및
T는 금속 이온, H 또는 음전하이며;
조성물은 멸균 상태이며, 비-독성이고, 근적외선으로의 조명 하에서 조직의 시각화를 위해 환자에게 투여하기에 적합하며; 및
화합물의 단위 투여 형태는 0.05 내지 8 mg/kg이다.
일부 구체예에서, 본 발명은 근적외선으로의 조명 하에 조직의 시각화를 얻기 위해 환자에게 투여하기에 적합한 조성물의 사용을 제공하며, 이때 단위 투여 형태는 0.05 내지 8 mg/kg이다. 일부 경우에, 사용은 근적외선으로의 조명 하에 인간 조직의 시각화를 얻기 위해 인간 환자에게 투여하기에 적합하며, 이때 단위 투여 형태는 0.05 내지 8 mg/kg이다.
일부 구체예에서, 본 발명은 환자에게 조성물을 투여하는 단계, 근적외선으로의 조명 하에서 조직을 영상화하는 단계, 및 조성물을 사용하여 환자로부터 조직의 적어도 하나의 영상을 얻는 단계를 포함하는, 조직의 시각화 방법을 제공한다. 일부 경우에, 방법은 조성물의 투여 중에, 투여 후에, 또는 투여 중 및 후에 모두 영상을 얻는 단계를 더 포함한다.
일부 경우에, 방법은 환자에게 조성물을 정맥 내로 주사하는 단계를 더 포함한다. 일부 경우에 조성물은 순환계로 주사된다.
일부 경우에, 방법은 수술이 수행되거나 또는 수행될 환자 영역을 시각화하는, 또는 그렇지 않으면 의료 전문가에 의해 조사가 진행되는 환자 영역을 가시화하는 단계를 더 포함한다. 일부 경우에, 방법은 수술 영역의 시각화를 기반으로 하여 환자 영역에 수술 과정을 수행하는 단계를 더 포함한다. 일부 경우에, 방법은 안과, 관절경, 복강경, 흉부, 근육 또는 신경 수술이 수행되거나 또는 수행될 환자 영역을 보는 단계를 더 포함한다. 일부 경우에, 방법은 환자의 적어도 일부의 생체 외 영상을 얻는 단계를 더 포함한다.
일부 경우에, 방법은 외과 전문의에게 그/그녀가 손상을 유발하지 않을 자신감 및 안전성을 제공함으로써, 손상률을 감소시키고 가능하게는 수술의 속도를 증가시키도록 시각화가 어려운 조직을 향상시키는 증강 시야를 제공함에 있어서, 다빈치 수술 시스템의 파이어플라이 형광과 조합하여 본 발명의 조성물을 사용하는 것을 추가로 포함한다. 외과 전문의들은 자주 양성 및 악성 자궁 절제술과 같은 최소 침습 부인과 수술 중에, 관련된 해부학적 구조에 접근하고 그것을 시각화하는 방법으로 인해, 신장으로부터 방광으로 소변을 운반하는, 수뇨관으로서 알려져 있는 관형 구조를 배치시키는 힘든 시간을 가진다. 수뇨관 손상의 발생률은 미국에서 년간 1 내지 3%로, 5000 내지 15000명의 여성에 달한다. 손상률은 심지어 대장 수술에서보다 6 내지 8% 정도 높고 수뇨관은 외과 전문의가 세심한 절개를 수행할 때 충분한 주의를 기울이지 않는다면 자주 절단된다. 선행 수술, 방사선 치료법 또는 다른 개입으로부터 유의미한 부착 또는 괴사가 있는 경우에, 확인 작업은 훨씬 더 어렵고 시간 소모적일 수 있다. 신우요관 이행부 폐색을 완화시키기 위한 신우성형과 같은 복잡한 비뇨기과학 사례에서, 수뇨관의 시각화는 어려울 수 있고 본 발명의 조성물과 같은 영상화제가 외과 전문의가 정상적인 또는 일탈적인 해부학적 구조를 식별하는 것을 도울 수 있다.
V. 실시예
실시예 1. 조명하에서 신장 및 수뇨관의 시각화
10 ml의 멸균수 중의 20 mg의 식 III의 화합물을 5마리의 돼지에 정맥내로 투여하였다. 복강경 포트를 놓고 그 포트에 다빈치 수술 시스템을 연결시켰다. 시스템의 내시경을 돼지 신장에 겨냥하고, 대략 800 nm에서의 레이저 여기를 사용하여 수뇨관 내의 식 III의 조성물을 자극하였다 (도 1). 소량의 청색 및 녹색광을 또한 방출하여 배경의 해부학적 구조를 볼 수 있도록 하였다. 투여 후 대략 10분 후에, 식 III의 조성물은 수뇨관을 통해 신장을 빠져나가서, 배설을 위해 방광으로 가는 것으로 나타났다 (도 2A 및 2B).
실시예 2. 조명 하에 신장 및 수뇨관의 시각화
환자에서 시각화가 어려운 신장 수뇨관을 향상시키기 위한 증강 시야는 10 ml의 멸균수 중의 20 mg의 식 III의 화합물을 원하는 시각화 전 15분 전에 60 kg의 인간 환자에게 정맥내로 투여함으로써 이루어질 수 있다. 시스템의 내시경을 환자의 신장에 겨냥하고, 대략 800 nm의 레이저 여기를 사용하여 수뇨관 내의 식 III의 조성물을 자극하였다. 소량의 청색 및 녹색광을 또한 방출하여 배경의 해부학적 구조를 볼 수 있도록 하였다. 투여 후 대략 15분 후에, 수뇨관의 시각화를 식 III의 조성물이 수뇨관을 통해 신장을 빠져나가, 배설을 위해 방광으로 가는 것으로서 볼 수 있다.
Claims (21)
- 제1 항에 있어서, 조성물 단위 투여 형태는 0.05, 0.10, 0.20, 0.30, 0.35, 0.40, 0.45, 0.50, 0.55, 0.60, 0.65, 0.70, 0.75, 0.80, 0.90, 1, 2, 4, 6 또는 8 mg/kg인 것을 특징으로 하는 조성물.
- 제1 항에 있어서, 조성물은 근적외선으로의 조명 하에서 인간 조직의 시각화를 얻기 위해 인간 환자에게 투여하기에 적합하고, 이때 단위 투여 형태는 0.05 내지 8 mg/kg인 것을 특징으로 하는 조성물.
- 제1 항에 있어서, 조성물은 건조한 단일 용량 형태인 것을 특징으로 하는 조성물.
- 제1 항에 있어서, 조성물은 멸균 용기에서 동결건조되는 것을 특징으로 하는 조성물.
- 제1 항에 있어서, 조성물은 멸균 용기 내에 함유되고, 용기는 의료 장치에 의해 판독 가능한 기계 검출용 식별자를 가지는 것을 특징으로 하는 조성물.
- 제1 항에 있어서, 경구 투여 형태로 약학적으로 허용되는 부형제와의 조합물을 더 포함하는 것을 특징으로 하는 조성물.
- 제1 항에 있어서, 주사 투여 형태로 약학적으로 허용되는 담체와의 조합물을 더 포함하는 것을 특징으로 하는 조성물.
- 제1 항에 있어서, 수술 부위로의 직접적인 적용을 위한 투여 형태로 약학적으로 허용되는 부형제와의 조합물을 더 포함하는 것을 특징으로 하는 조성물.
- 근적외선으로의 조명 하에서 조직의 시각화를 얻기 위해 환자에게 투여하기에 적합하고, 이때 단위 투여 형태는 0.05 내지 8 mg/kg인, 제1 항의 조성물의 사용.
- 제12 항에 있어서, 근적외선으로의 조명 하에서 인간 조직의 시각화를 얻기 위해 인간 환자에게 투여하기에 적합하고, 이때 단위 투여 형태는 0.05 내지 8 mg/kg인 것을 특징으로 하는 사용.
- 조직의 시각화 방법으로서, 환자에게 제1 항의 조성물을 투여하는 단계, 근적외선으로의 조명 하에 조직을 영상화하는 단계 및 조성물을 사용하여 환자로부터 조직의 적어도 하나의 영상을 얻는 단계를 포함하는 방법.
- 제14 항에 있어서, 조성물의 투여 중에, 투여 후에 또는 투여 중과 후에 모두 영상을 얻는 단계를 더 포함하는 것을 특징으로 하는 방법.
