Description

図面の簡単な説明
I.全般
本発明は、近IR閉鎖スルホ-シアニン染料の、新規で、無菌で、無毒の医薬組成物、および近赤外放射線による照射下で組織の可視化をするための方法を提供する。
本願において使用される略語は、化学および生物学分野内でのそれらの従来の意味を有する。
本願において使用される用語「金属イオン」は、金属性で、中性金属元素について存在するものより原子価殻中に少ない電子を有する結果として正に帯電した周期表の元素を指す。本発明において有用な金属には、薬学的に許容し得る組成物を形成することができる金属が含まれる。有用な金属には、Li、Na、K、Rb、Cs、Be、Mg、Ca、Sr、およびBaが含まれる。当業者は、上記の金属が、それぞれいくつかの異なる酸化状態をとり得ることを十分に理解するであろう。いくつかの例において、最も安定な酸化状態が形成されるが、他の酸化状態が本発明において有用である。

本願において使用されるとおり、用語「組織」には、これに限定されないが、同種または異種骨、神経組織、腱および靭帯を含む線維性結合組織、軟骨、硬膜、筋膜、心膜、筋肉、心臓弁、静脈および動脈および他の血管、真皮、脂肪組織、または腺組織が含まれる。
本願において使用されるとおり、用語「注射可能な投与形態」には、当該技術分野におけるその通常の意味を指す(すなわち、例えば水または水/プロピレングリコール溶液などの溶液、懸濁液、および乳化液の形態の医薬組成物を指す)。
A.化合物
本発明の化合物は、電磁スペクトルの近赤外領域における波長で光を吸収する式IおよびIIの染料を含む。本願における任意の複数形「(単数および複数の)波長(wavelength(s))」への言及は、放射線が単一波長または異なる波長を有する放射線のスペクトルであり得ることを示す。

本発明の組成物を、種々の広範な経口、非経口および局所投与形態で調製することができる。経口処方物には、患者による摂取に好適な、タブレット、ピル、粉末、糖衣錠、カプセル、液体、トローチ剤、カシェ剤、ジェル、シロップ、スラリー、懸濁液などが含まれる。本発明の組成物をまた、注射により投与することができ、すなわち、静脈内に、筋内に、皮内に、皮下に、十二指腸内に、または腹膜内に投与することができる。また、本願に記載される組成物を、例えば鼻腔によるなどの吸入により投与することができる。さらに、本発明の組成物を、経皮投与することができる。本発明の組成物をまた、眼内、吹き込み、粉末、およびエアロゾル処方物により投与することができる(例えば、ステロイド吸入剤など、Rohatagi, J. Clin. Pharmacol. 35:1187-1193, 1995; Tjwa, Ann. Allergy Asthma Immunol. 75:107-111, 1995を参照)。したがって、本発明はまた、薬学的に許容し得る担体または賦形剤を含む医薬組成物を提供する。

本発明は、一般に、近IR閉鎖スルホ-シアニン染料の、新規で、無菌で、無毒の医薬組成物、および近赤外放射線による照射下で組織の可視化をするための方法を提供する。

ここで、前記組成物は無菌で無毒であり、近赤外放射線による照射下で組織の可視化のための患者への投与に適合され;
ここで、前記化合物の単位剤形は、0.05~8mg/kgである。
いくつかの場合において、前記方法は、外科手術が行われるか、または外科手術が行われるであろう患者側領域を可視化すること、またはそうでなければ、医療専門家により検査されている患者側領域を観察するために患者側領域を可視化することをさらに含む。いくつかの場合において、前記方法は、外科的領域の可視化に基づいて患者側領域に外科的手法を行うことをさらに含む。いくつかの場合において、前記方法は、眼病の、関節鏡の、腹腔鏡の、心胸の、筋肉の、または神経的手法が行われるか、またはこれらが行われるであろう患者側領域を観察することをさらに含む。いくつかの場合において、前記方法は、患者の少なくとも一部の生体外での画像を得ることをさらに含む。
例1.照射下での腎臓および尿管の可視化
10ml中の滅菌水中の20mgの式IIIの化合物を、5匹のブタに静脈注射した。腹腔鏡のポートを配置して、da Vinci Surgical Systemをポートに接続した。システムの内視鏡をブタの腎臓に向け、およそ800nmのレーザー励起を使用して尿管内で式IIIの組成物を励起させた(図1)。バックグラウンド組織の可視化を可能にするために、少量の青色および緑色光をまた照射した。投与後およそ10分で、式IIIの組成物が、排出のために膀胱に向かって尿管を介して腎臓から出ていくことが見えた(図2Aおよび2B)。
患者内で可視化困難な腎臓尿管を強化するための拡張ビューを、所望の可視化の15分前に、10ml中の滅菌水中の20mgの式IIIの化合物を60kgのヒト患者に静脈投与することにより達成することができる。システムの内視鏡を患者の腎臓に向け、およそ800nmのレーザー励起を使用して尿管内で式IIIの組成物を励起させ得る。バックグラウンド組織の可視化を可能にするために、少量の青色および緑色光をまた照射し得る。投与後およそ15分で、式IIIの組成物が、排出のために膀胱に向かって尿管を介して腎臓から出ていくので、尿管の可視化が達成され得る。
Claims (14)
- 前記化合物の前記単位剤形は、0.05、0.10、0.20、0.30、0.35、0.40
、0.45、0.50、0.55、0.60、0.65、0.70、0.75、0.80
、0.90、1、2、4、6、または8mg/kgの量を前記患者に送達する、請求項1に記載の無菌医薬組成物。 - 前記無菌医薬組成物は、乾燥した単一剤形である、請求項1に記載の無菌医薬組成物。
- 前記無菌医薬組成物は、無菌容器中で凍結乾燥されている、請求項1に記載の無菌医薬組成物。
- 前記無菌医薬組成物は、無菌容器内に格納され、かつ、前記無菌容器は、医療デバイスにより読み出し可能な機械的検出可能な識別器を有する、請求項1に記載の無菌医薬組成物。
- 経口投与形態で、1又はそれ以上の薬学的に許容し得る賦形剤をさらに含む、請求項1に記載の無菌医薬組成物。
- 注射可能な投与形態で、1又はそれ以上の薬学的に許容し得る担体をさらに含む、請求項1に記載の無菌医薬組成物。
- 手術部位へ直接適用する投与形態で、1又はそれ以上の薬学的に許容し得る賦形剤をさらに含む、請求項1に記載の無菌医薬組成物。
- 近赤外線照射下で組織の少なくとも1つの画像を得るために患者に投与するように適合された請求項1に記載の無菌医薬組成物。
- 患者がヒトの患者であり、組織がヒトの組織である、請求項10に記載の無菌医薬組成物。
- 画像が、無菌医薬組成物の投与中、投与後、または投与中と投与後の両方で得られる、請求項10に記載の無菌医薬組成物。
- 前記無菌医薬組成物は、患者に静脈内投与するための注射剤の形態である循環系に注射される、請求項10~12のいずれか一項に記載の無菌医薬組成物。
- 前記無菌医薬組成物が循環系への注射用である、請求項13に記載の無菌医薬組成物。
