본 발명은 유일한 활성성분으로 쿠르쿠마 종의 추출물, 선택적으로 인지질과 복합체화된 형태의 쿠르쿠민, 및 에키네시아 종 추출물 또는 산초나무 종의 친유성 추출물을 함유하는 조성물에 관한 것으로서, 상기 조성물은 말초성 통증 및 표재성 및 심부 염증성 통증 상태의 국소 및 전신적 치료에 유용하다. 본 발명에 따른 조성물은 NSAID나 스테로이드로 장기간 치료할 수 없는 환자에서 골관절염 및 류마티스성 관절염을 치료하는데 특히 효과적이다.

본 발명은 유일한 활성성분으로 쿠르쿠마 종(Curcuma spp)의 추출물, 선택적으로 인지질과 복합체화된 형태의 쿠르쿠민, 및 에키네시아 종(Echinacea spp) 추출물 또는 산초나무 종(Zanthoxylum spp)의 친유성 추출물을 함유하는 조성물에 관한 것으로서, 상기 조성물은 말초성 통증 및 표재성 및 심부 염증성 통증 상태의 국소 및 전신적 치료에 유용하다. 본 발명에 따른 조성물은 NSAID나 스테로이드로 장기간 치료할 수 없는 환자에서 골관절염 및 류마티스성 관절염을 치료하는데 특히 효과적이다.
골관절염은 노인에서 신체 장애를 일으키는 주요한 원인 중 하나이다. 관절염은 케어에 드는 비용이 매우 비싸기 때문에, 세계 도처에서 수백만명의 개인의 삶을 질을 저하시킬 뿐만 아니라, 가족과 커뮤니티 둘다에 재정적인 손실 또한 야기한다.
골관절염의 통상적인 치료법은 진통제로 증상을 치료하는 것과 비스테로이드성 항염증제로 치료법적으로 치료하는 것을 수반하며, 이들 치료는 종종 치료의 중단을 초래하는 심각한 부작용을 유발하는 것으로 알려져 있다. 류마티스성 관절염에도 또한 유사한 사항이 적용된다. 이러한 이유로, 일반적으로 파이토테라피(phytotherapy) 약제 및 천연 식물 치료에 대해 연구의 초점이 맞춰져 왔다.

강황(turmeric)(쿠르쿠마: Curcuma)의 뿌리 및 근경(rhizome)은 아득한 옛날부터 인도에서 향신료로 사용되어 왔고, 또한 현재 많은 산업화된 국가에서 사용되고 있다. 이의 활성 성분인 쿠르쿠민(curcumin)은 카레 및 기타 전통적인 인도 음식의 주요 성분을 구성하는 노란색 화합물이다. 전통 의약에 기재된 용도는 소화불량, 고창(속이 부글거림) 및 설사를 포함하며, 특히 장기 치료 후의 염증성 상태 및 관절 통증을 포함한다. 전통적인 용도 중 많은 것들이 시험관내(in vitro) 생화학적 시험 또는 약리학에 의해 확인되어 왔다.
쿠르쿠민은 현재 생화학적 관점에서 가장 빈번히 연구되는 분자 중 하나이며, 최근 몇년간 과학적 가치가 확실하지 않은 수많은 간행물에 추가하여, 현대의 임상 약물학의 프로토콜에 따라 조절된 이중맹검 임상 시험들도 또한 수행되어 왔다.
예비 임상 시험들은 장내 pH에서 불안정한(pH 7에서의 반감기가 10분 미만) 분자의 낮은 생체이용률 및 이의 낮은 경구 흡수율을 증명하였고, 이는 시험관내(in vitro) 시험에서 나타난 적용의 많은 것을 제한한다. 12g의 화합물을 투여한 후 50ng의 혈장 농도가 보고되었고, 2차 대사산물, 글루쿠로니드(glucuronides) 및 황산염(sulphates) 또한 측정되었다.
많은 폴리페놀(polyphenol)처럼, 쿠르쿠민은 물 및 지방에서 잘 용해되지 않는다(poorly soluble). 따라서, 본 출원인은 쿠르쿠민의 생체이용률을 개선시키기 위해 쿠르쿠민과 인지질의 복합체를 기술하고 있는 국제특허공개공보 제WO 2007/101551호를 개발하였다. 인지질-복합체화된 형태의 쿠르쿠민은 개선된 전신 및 국소 흡수를 제공하여 인간에 투여될 수 있도록 한다.
골관절염을 앓는 환자에 대해 수행된 임상시험은 인지질-복합체화된 쿠르쿠민으로 장기간 치료하는 것이 유리함으로 보여주었지만(Belcaro 등의 문헌 [Alternative Medicine Review, 4, 338, 2010] 참조), 낮은 수준의 통증 감소 때문에 여전히 부족한 결과였고, 이는 환자들이 어쩔 수 없이 NSAID를 복용하도록 만든다.
그러나, 8개월 치료후에야 상당한 개선이 얻어졌고, 많은 환자들은 통증을 경감시키기 위해 NSAID나 다른 진통제를 사용해야만 했다.
본 발명에서는 놀랍게도, 유일한 활성성분으로, 쿠르쿠마 종의 추출물, 및 에키네시아 종 추출물 또는 산초나무 종의 친유성 추출물로부터 선택된 추출물을 함유하는 조성물이 강력한 진통 및 항염증 활성을 가지며, 이러한 활성은 상기 추출물을 따로 사용하는 경우에 비해 더 크다는 점을 발견하였다.
인지질과 복합체화된 쿠르쿠민 및 에키네시아 안구스티폴리아의 친유성 추출물을 함유하는 조성물에서 특히 좋은 결과가 관찰되었다.
또한, 에키네시아 추출물을 이소부틸아미드 함량이 높은 다른 식물의 추출물, 특히 산초나무 종의 친유성 추출물로 대체한 경우에도 또한 양호한 결과가 얻어졌다.
따라서, 본 발명의 목적은 허용가능한 부형제와 혼합된, 쿠르쿠마 종 추출물 또는 쿠르쿠민, 및 에키네시아 안구스티폴리아 추출물 또는 산초(Zanthoxylum bungeanum)의 친유성 추출물로부터 선택된 추출물로 실질적으로 구성된 조성물을 제공하는 것이다.
본 발명은 특히 허용가능한 부형제와 혼합된, 쿠르쿠마 종 추출물 또는 쿠르쿠민, 및 에키네시아 안구스티폴리아 추출물 또는 산초의 친유성 추출물로부터 선택된 추출물로 실질적으로 구성된 조성물에 관한 것이다.
본 발명에 따른 조성물은 경구적으로 또는 국소적으로 투여될 수 있다.
바람직한 양태에 의하면, 경구 투여용 조성물은, 경구용 단위용량당, 인지질과 복합체화된 쿠르쿠민 100 내지 1,000 mg, 및 에키네시아 추출물 1 내지 200 mg 또는 대안적으로(alternatively) 산초나무 종의 친유성 추출물 5 내지 25 mg을 함유한다. 특히 바람직한 양태에 의하면, 경구 투여용 조성물은, 경구용 단위용량당, 인지질과 복합체화된 쿠르쿠민 500 mg, 및 에키네시아 추출물 5 mg 또는 대안적으로 산초나무 종의 친유성 추출물 10 mg을 함유할 것이다.
바람직한 양태에 의하면, 국소 투여용 조성물은 인지질과 복합체화된 쿠르쿠민 0.1 내지 0.5 중량%, 및 에키네시아 추출물 0.05 내지 0.5 중량% 또는 대안적으로 산초나무 종의 친유성 추출물 0.1 내지 1 중량%를 함유할 것이다. 특히 바람직한 양태에 의하면, 국소 투여용 조성물은 인지질과 복합체화된 쿠르쿠민 0.2 중량%, 및 에키네시아 추출물 0.2 중량% 또는 대안적으로 산초나무 종의 친유성 추출물 0.5 중량%를 함유할 것이다.
본 발명에 따른 조성물은 당뇨성 신경병증에서부터 다양한 기관의 관절 및 근육 통증에 이르기까지 모든 종류의 말초성 통증을 치료하는데 사용될 수 있다. 본 발명에 따른 조성물은 위장관에 부작용을 일으키기 않으므로 NSAID를 견딜 수 없는 위 문제를 갖는 환자에 사용가능하므로 특히 중요하다.
에키네시아 종의 친유성 추출물은 뿌리 또는 근경을 알코올, 케톤 또는 지방족 에테르로 추출하거나, 바람직하게는 유럽 특허출원공개 제464298호에 따라 초임계 조건하에서 이산화탄소로 추출함으로써 얻을 수 있다.
약물학적으로 활성인 양의 이소부틸아미드를 여전히 함유하는 에키네시아 종의 알콜성 추출물은 최소 투여량인 이소부틸아민 1mg 이상이 환자에 투여될 때 효과적인 것으로 증명되었다. 활성인 임상 용량은 평균 체중 70 Kg에 대해 0.5 mg 내지 10 mg, 바람직하게는 5 mg이다.
산초나무 종의 친유성 추출물은 국제특허공개공보 제WO 00/02570호에 따라 제조할 수 있다.
그러나, 본 발명에 따른 조성물은 환자들의 증상을 거의 즉시 완화시키므로 다른 약제를 사용할 필요가 없다. 위 식도 역류증(gastro-oesophageal reflux)을 앓고 궤양 형성 전 단계(the pre-ulcerative stage)에 있는 환자들 또한 본 발명에 따른 조성물을 복용할 수 있다.
약물학적 시험
1. 래트에서의 꼬리 도피 반사(tail-flick test)
본 발명에 따른 조성물(인지질-복합체화된 쿠르쿠민과 에키네시아 추출물의 조합)의 진통 활성을 개개 성분을 래트에 별도로 투여한 경우와 비교하였다. 시험 결과, 하기 표 1에 나타낸 바와 같이, 본 발명에 따른 조성물의 2가지 성분 사이에 명확한 상승효과가 입증되었다.
진통 활성은 래트에서 꼬리 도피 반사로 평가하였다. 동물들이 취급 및 사용한 기기에 적당한지 확인하기 위해 치료 전에 3가지 기본적인 측정을 수행하였다. 꼬리 도피를 평가함에 있어 15V의 복사열(radiant heat)과 15초 컷오프(동물들에게 비가역적인 해를 주는 것을 방지하기 위함)를 파라미터로 사용하였다. 동물은 꼬리의 기저(base)로부터 5cm를 실시예 4에 기재된 조성물의 연고 0.1ml로 치료하였다. 진통 효과는 투여후 15분 및 30분에 측정하였다.
조성물의 2개의 개별 성분은 동일한 실험 모델에서 실시예 4에 기재된 조성물과 동일한 양의 하나의 활성 성분을 함유하는 별도의 2개의 제제로 평가하였다. 대조 동물은 2개의 성분을 용해시키기 위해 사용된 오일(담체) 0.1ml로 처치하였다.

2. 인간에서의 진통 활성
무릎에 만성적인 통증(constant pain)을 갖는 뼈 질환을 앓고 있는 40명의 환자를 무작위로 실시예 1에 따라 하루에 2정, 즉 오전에 1회 및 저녁에 1회로 치료하거나, 위약(담체만으로 구성됨)으로 치료하거나, 실시예 1에 기재된 제제와 동일한 농도로 위약 제제에 개별 성분을 첨가한 것으로 치료하였다.
약효는, 10은 최대 통증을 나타내고 0은 통증이 사라진 것을 나타내는, 0 내지 10으로 점수를 매기는 국제 통증 평가 스케일(International analogue pain scale)로 점수를 매겼다. 효과는 정제 투여후 2일째에 오전 치료후 60분 및 120분에 평가하였다.
결과를 하기 표 2에 나타내었다.

3. 골관절염에 대한 효과
모집한 환자를 본 발명에 따른 조성물로 3개월 이하로 치료한 후에 얻은 골관절염에 대한 전반적인(global) 효과 결과를 표 3에 나타내었으며, 환자의 선택 및 효능 평가 모두 Karnofsky Scale에 따랐다(문헌 [J.Clin.Oncology 1984; 2:187-193] 참조).
10% 경사도(inclination) 및 3 Km/h로 설정된 트레드밀(treadmill) 상에서 통증 없이 이동한 거리를 다른 통증 정도로 측정하는 것에 의해 평가를 수행하였다. 무릎의 골관절염을 앓는 80명의 환자를 2개의 그룹으로 나누었다. 무작위로 하나의 그룹은 위약으로 처치하고, 다른 그룹은 실시예 1에 기재된 조성물로 처치하였다. 통증은 WOMAC 지수로 치료하는 동안 매주 평가하였고, 염증성 파라미터의 표지(indexes)를 구성하는 체액성 파라미터(humoral parameters)를 매달 평가하였다(표 4).

겔, 크림 및 연고 형태의 약학 조성물은 말초성 통증과 표재성 및 심부 염증성 통증 상태의 국소적 치료에 특히 유용한 것으로 입증되었다. 조합물은 조합물이 가용화되는 오일 상태로 피부에 바로 적용하거나 또는 투여에 적합한 크림 또는 연고에 혼입될 수 있다. 하루 1회 내지 3회 치료를 수행할 수 있으며, 통증성 질환에 영향을 받는 신체 부위에 국소 제제를 0.5-5 g의 투여량으로 적용할 수 있다.
추가의 양상에 의하면, 본 발명에 따른 조성물은 유용한 또는 상호 보완적인 활성을 갖는 다른 물질과 함께 투여할 수 있다.
본 발명에 따른 조성물은 통상적인 기술, 예컨대 문헌 [Remington's Pharmaceutical Handbook, Mack Publishing Co., N.Y., USA.]에 기술된 방법에 따라 제형화할 수 있을 것이다.
하기 실시예들을 통해 본 발명을 추가로 설명한다.

Claims (11)

  1. 허용가능한 부형제와 혼합된, 쿠르쿠마 종 추출물 또는 쿠르쿠민, 및 에키네시아 안구스티폴리아(Echinacea angustifolia) 추출물 또는 산초(Zanthoxylum bungeanum)의 친유성 추출물로부터 선택된 추출물로 실질적으로 구성된 조성물.
  2. 청구항 1에 있어서, 쿠르쿠민이 인지질과 복합체화된 조성물.
  3. 청구항 1에 있어서, 에키네시아 안구스티폴리아 추출물 및 산초의 친유성 추출물이 초임계 이산화탄소(supercritical carbon dioxide)로 추출하는 것에 의해 제조된 조성물.
  4. 청구항 1 내지 청구항 3 중 어느 한 항에 있어서, 경구 투여용인 조성물.
  5. 청구항 4에 있어서, 유일한 활성 성분으로, 경구용 단위용량당, 인지질과 복합체화된 쿠르쿠민 100 내지 1,000 mg, 및 에키네시아 추출물 1 내지 20 mg 또는 대안적으로(alternatively) 산초나무 종(Zanthoxylum spp .)의 친유성 추출물 5 내지 25 mg을 함유하는 조성물.
  6. 청구항 5에 있어서, 유일한 활성 성분으로, 경구용 단위용량당, 인지질과 복합체화된 쿠르쿠민 500 mg, 및 에키네시아 추출물 5 mg 또는 대안적으로 산초나무 종의 친유성 추출물 10 mg을 함유하는 조성물.
  7. 청구항 1 내지 청구항 3 중 어느 한 항에 있어서, 국소 투여용인 조성물.
  8. 청구항 7에 있어서, 인지질과 복합체화된 쿠르쿠민 0.1 내지 0.5 w/w%, 및 에키네시아 추출물 0.05 내지 0.5 w/w% 또는 산초나무 종의 친유성 추출물 0.1 내지 1 w/w%를 함유하는 조성물.
  9. 청구항 8에 있어서, 인지질과 복합체화된 쿠르쿠민 0.2 w/w%, 및 에키네시아 추출물 0.2 w/w% 또는 대안적으로 산초나무 종의 친유성 추출물 0.5 w/w%를 함유하는 조성물.
  10. 청구항 1 내지 청구항 9 중 어느 한 항에 있어서, 말초성 통증, 관절 및 근육 통증, 염증성, 표재성 및 심부 통증 상태(inflammatory and superficial and deep pain conditions)의 치료에 사용하기 위한 조성물.
  11. 청구항 10에 있어서, 골관절염 및 류마티스성 관절염을 치료하기 위한 조성물.
