본 발명은 일반적으로 면역학, 염증, 관절염 및 의약 분야에 관한 것이다. 더 상세하게는, 본 발명은 하나 이상의 관절염 증상을 치료하기 위하여 인터루킨-1α(IL-1α)에 특이적으로 결합하는 항체(Ab)의 용도에 관한 것이다.
미국에서 장애의 가장 흔한 원인인 관절염은 골관절염, 류마티스 관절염, 통풍, 건선성 관절염, 패혈성 관절염 및 반응성 관절염과 같은 여러 가지 질환의 집합이다. 모든 유형의 관절염은 환부에 통증, 부기, 발적, 경직 및 체온 상승을 유발하는 관절 염증을 특징으로 한다. 괴로워하는 대상자들은 통증과 경직 탓에 덜 움직이므로, 관절염은 간접적으로 비만, 고 콜레스테롤 및/또는 심장 질환을 초래할 수 있다. 관절염은 홍채염, 포도막염, 입 궤양, 위장관 염증, 비뇨생식관 염증 및 피부병변과 같은 관절 외 질병도 유발할 수 있다.
대부분 유형의 관절염에 대해, 치유법은 존재하지 않으며, 치료는 주로 증상에 관하여, 예를 들어, 진통제와 소염제 투여 식으로 이루어진다. 염증과 통증을 줄이기 위하여 비스테로이드성 항염제(non-steroidal anti-inflammatory drugs, NSAIDs)를 이용할 수 있다. 비스테로이드성 항염제는 일반적으로 효과가 있지만, 복통, 출혈, 궤양 및 간과 신장 손상과 같은 부작용을 유발할 수 있다. 코르티코스테로이드는 염증 및 관절 손상을 줄이는 데 효과적이지만, 타박상, 체중 증가, 백내장, 골 손실(bone thinning), 당뇨 및 고혈압을 포함한 여러 가지 관련 부작용을 유발할 수 있다. 관절염 치료에 흔히 사용하는 기타 약물은 메토트렉세이트, 시클로스포린, 시클로포스파미드, 레플루노마이드, 하이드록시클로로퀸, 술파살라진 및 미노사이클린이다. 이것들 역시 간 손상 및 면역억제와 같은 부작용을 유발할 수 있다. 에타너셉트(etanercept, 엔브렐(Enbrel)), 인플릭시맙(infliximab, 레미케이드(Remicade)) 및 아달리무맙(adalimumab, 후미라(Humira))와 같은 종양 괴사 인자(TNF) 억제제 또한 관절염을 치료하는 데 유용하다. 종양 괴사 인자 억제제의 부작용은 주사 부위 반응, 심장기능상실, 림프종 및 감염 위험 증가를 포함한다.

본 발명은 관절염을 앓는 인간 대상자에서 특이적으로 IL-1α를 표적화하는 항체(Ab) 투여 시, 대상자의 CD14+IL-1α+ 말초 혈액 단핵세포의 수를 줄이고, 관절 및 관절 외 부위 모두에서 현저하게 염증을 개선하며, 이 모든 것이 투여 부위의 통증 이외에는 관찰된 부작용 없이 이루어진다는 발견을 기초로 한 것이다.
따라서 본 발명은 약학적으로 허용 가능한 담체와 인간 대상자의 염증성 병적 측면의 적어도 하나의 증상을 줄이기에 효과적인 양의 항 IL-1α 항체를 포함하는 제약 조성물을 대상자에 투여하여 인간 대상자에서 관절염과 관련된 염증성 병적 측면을 치료하는 방법을 특징으로 한다. 증상은 손목 또는 어깨의 관절과 같은 관절 염증 또는 포도막염과 같은 눈의 염증일 수 있다. 항 IL-1α 항체는 IgG1과 같은 단일클론항체일 수 있다. 항 IL-1α 항체는 MABp1으로 지정한 단일클론항체 또는 MABp1의 하나 이상의 상보성 결정 부위(CDR)를 포함하는 단일클론항체일 수 있다.
제약 조성물은 피하로, 정맥 내로, 근육 내로, 안구 내로, 또는 염증이 생긴 관절에 직접 주사하여 대상자에게 투여될 수 있다. 항체는 눈에 국소적으로 투여될 수도 있을 것이다. 방법에서, 대상자의 염증성 병적 측면의 적어도 하나의 증상을 줄이기에 효과적인 양의 항 IL-1α 항체는 대상자의 항 IL-1α 항체의 말초 혈액 농도를 적어도 4㎍/㎖까지 상승시키기에 충분하고; 및/또는 대상자의 CD14+IL-1α+ 말초혈액단핵세포의 수를 적어도 5% 줄이기에 충분할 수 있다.
또한, 방법은 제약 조성물 투여 후에 대상자의 말초 혈액 내의 CD14+IL-1α+ 단핵세포의 수를 측정하는 단계를 포함할 수 있고, 예를 들어 여기서 대상자의 말초 혈액 내의 CD14+IL-1α+ 말초혈액단핵세포의 수를 측정하는 단계는 제약 조성물 투여 후에 적어도 두 번의 다른 시점에 이루어진다.

관절염 증상의 치료
본원에 기술한 조성물 및 방법은 대상자의 염증성 병적 측면의 적어도 하나의 증상을 줄이기에 효과적인 양의 항 IL-1α 항체를 포함하는 제약 조성물을 대상자에 투여하여 포유류 대상자의 관절염과 관련된 염증성 병적 측면을 치료하는 데 유용하다. 포유류 대상자는 인간, 개, 고양이, 말, 소, 양, 염소 및 돼지를 포함하는, 관절염을 앓는 임의의 대상자일 수 있다. 인간 대상자는 남성, 여성, 성인, 아동, 노인(65세 이상) 및 다른 질병을 앓는 이들일 수 있다. 관절염과 관련된 특정 증상 또는 병리학적 과정은 염증, 통증, 경직, 또는 관절(예를 들어, 손목, 손발가락[손허리뼈 또는 발허리뼈 관절] 내), 팔꿈치, 어깨, 엉덩이, 무릎, 발목, 발, 목 또는 등)의 퇴화 또는 관절 외 조직의 퇴화(예를 들어, 홍채염, 포도막염, 입 궤양, 위장관의 염증, 비뇨생식관의 염증 또는 피부병변)일 수 있다.
IL-1α를 대상으로 하는 항체와 기타 작용제
IL-1α에 특이적으로 결합하여 대상자에서 관절염으로 유발된 증상 또는 병리학적 과정을 줄이는 임의의 적합한 유형의 Ab가 본 발명에서 이용될 수 있다. 예를 들어, 이용되는 항 IL-1α Ab는 mAb, 다중클론성 Ab, mAb의 혼합물 또는 scFv와 같은 Ab 조각 또는 조작된 Ab 유사 분자일 수 있다. Ig은 H2L2 배열에 무거운 사슬(H)과 가벼운 사슬(L)을 포함하므로, 각각을 암호화하는 유전자를 별도로 분리하여 서로 다른 벡터에서 발현시킬 수 있다.
대상자가 항 Ab 반응을 발달시킬 가능성이 커서 일반적으로 더 적게 선호되지만, 서로 다른 동물 종에서 유래된 서로 다른 부분(예를 들어, 인간 Ig의 불변영역에 융합된 마우스 Ig의 가변영역)이 있는 항원 결합 분자인 키메라 mAb(예를 들어, "인간화" mAb)가 본 발명에 이용될 수 있다. 그러한 키메라 Ab는 당해 분야에 공지된 방법으로 제조할 수 있다. 예를 들어, Morrison et al., Proc. 본 명세서에 기술된 mAb는 VH 및 VL 도메인 셔플링(domain shuffling: Marks et al. Bio/Technology 10:779-783, 1992), 고도가변 영역(hypervariable regions, HVRs) 및/또는 골격 잔기(framework residue)의 무작위 돌연변이법(Barbas et al. Proc Nat. Acad. Sci. USA 91:3809-3813, 1994; Schier et al. Gene 169:147-155, 1995; Yelton et al. J. Immunol. 155:1994-2004, 1995; Jackson et al., J. Immunol. 154(7):3310-9, 1995; and Hawkins et al, J. Mol. 바람직하게는, 최소의 부작용으로 고 역가의 인간 IL-1α 특이적인 mAb가 대상자에게 투여될 수 있도록, 본 발명의 mAb 조성물은 (임의의 부형제를 제외한) 순수 중량을 기준으로 적어도 0.5, 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, 13, 14, 15, 20, 25, 30, 35, 40, 45, 50, 60, 70, 80, 90, 95, 96, 97, 98, 99, 99.9 이상의 퍼센트이다. 본 발명의 mAb 조성물은 mAb의 단일 유형만을 포함할 수 있거나(즉, 단일 클론성 B 림프구계(lymphocyte line)로부터 생성된 mAb) 둘 이상의(예를 들어, 2, 3, 4, 5, 6, 7, 8, 9, 10 이상) 상이한 유형의 mAb의 혼합물을 포함할 수 있다.
기능을 수정하거나 향상하고자, 인간 IL-1α mAb는 세포 독소와 같은 다른 분자에 접합될 수 있다. 위에서 기술한 IL-1α 특이적인 Ab가 본 발명에 이용하기 바람직하지만, 일부 경우에는 특이적으로 IL-1α를 대상으로 하는 기타 작용제를 투여한 결과, 하나 이상의 관절염 증상 개선으로 이어지는 한, 이러한 기타 작용제가 이용될 수 있다. 이러한 기타 작용제는 특이적으로 IL-1α를 결합하는 작은 유기 분자, 앱타머, 펩티드 및 단백질을 포함할 수 있다.
제약 조성물 및 방법
항 IL-1α Ab 조성물은 투여의 방식과 경로 및 표준 약제업무(pharmaceutical practice)를 기초로 하여 선택되는, 약학적으로 허용 가능한 담체(예를 들어, 살균 식염수)로 동물 또는 인간에 투여될 수 있다. 제약 제제뿐만 아니라 약학적으로 허용 가능한 담체의 목록은 이 분야의 표준 교재인 Remington’s Pharmaceutical Sciences와 미국약전(USP/NF)에서 볼 수 있다. 기타 물질이 조성물에 추가될 수 있으며, 기타 단계가 조성물을 안정화 및/또는 보존하고자 및/또는 대상자에 대한 투여를 촉진하고자 취해질 수 있다.
예를 들어, Ab 조성물은 동결 건조(Draber et al., J. Immunol. 실시예
실시예 1 - 실로닉스(Xilonix™)
실로닉스(Xilonix™)는 안정화 등장 완충액(pH 6.4)에 15mg/mL MABp1의 멸균 주사 가능한 액체 제제이다. 각각의 10mL 유형 I 붕규산 유리 혈청 바이알은 5mL의 제제를 포함하고, 20mm Daikyo Flurotec 부틸 고무 마개와 플립-오프 알루미늄 밀폐물로 밀폐되어 있다. 제품은 5±3℃에 보관되며, 실온으로 잠깐 벗어나는 것이 허용된다. 약 제품의 정확한 조성은 아래에 나타내었다.
실시예 2 - IL-1α 특이적인 단일클론항체를 이용한 반응성 관절염 치료
반응성 관절염을 앓는 48세의 남성 환자에 2009년 6월 1일 출원된 미국 특허출원 일련번호 제12/455,458호에 기술된 IL-1α 특이적인 단일클론항체인 MABp1을 총 220밀리그램 투여하였다. 그 환자는 왼쪽 무릎에 심각한 염증으로 입원하던 중, 라이터 증후군으로 진단 받았던 16세에 시작하여, 오랜 반응성 관절염 병력을 가졌다. 이 염증은 해소되었지만, 환자는 이십 대 중반까지 여러 관절에 주기적인 재발을 겪었다. 35세에 환자가 8주 동안 지속되는 심각한 한쪽 포도막염 삽화를 겪기 전까지는 더 이상의 삽화가 발생하지 않았었다. 포도막염은 안과용 코르티코스테로이드와 경구용 NSAIDS로 제대로 관리되지 않아서 약간 위협적인 상태로 되었다. 환자는 이후 강도를 달리 하는 적어도 세 차례의 추가적인 포도막염 삽화와 코르티코스테로이드의 각질 하 주사를 요하는 한 번의 삽화를 겪었다.
48번째 생일 직전에, 환자의 왼쪽 어깨와 손목에 심각한 통증이 생겼다. 거의 완전히 이동성을 상실한 채, 눈에 띄는 부기와 발적이 손목에 발생했다. 환자는 극심한 어깨 통증으로 왼쪽 팔을 약 20°보다 크게 벌릴 수 없었다. 그 날, 환자는 왼쪽 어깨에 코르티코스테로이드 견봉 하 주사를 맞았다. 환자는 그 상태가 계속하여 악화되었으며, 어깨와 손목의 통증이 계속 이어져 일을 방해하고 수면을 막았다고 보고했다. 또한, 왼쪽 눈의 통증과 자극이 뒤따라, 포도막염 삽화 개시를 알렸다. 이것이 처음으로 관절 염증과 포도막염이 함께 발생한 것이라고 한다. 환자는 거의 뚜렷한 이익 없이 안구용 코르티코스테로이드와 경구용 및 국소 안구용 NSAIDS를 받고 있었다.
0일째에(코르티코스테로이드 견봉 하 주사 후 42일 경과 시), 환자는 총 110mg의 MABp1을 (같은 투여량으로) 전달하는 네 번의 MABp1 피하 주사를 맞았다. 주사하는 동안 통증 이외에는 아무런 부작용도 보고되지 않았다. 두 개의 5ml 헤파린 나트륨 튜브로 주사하기 직전에 정맥천자로 채혈하였다. 기존의 내인성 항 IL-1α 항체 검출을 위한 효소결합면역흡착검사(ELISA)를 이용한 혈장 분석 결과, 이미 존재하는 항체가 없는 것으로 밝혀졌다.
1일째에, 환자는 그날 아침, “깨어날 때의 최초의 감각”이었던 욱신거리는 통증 없이 일어났다고 보고했다. 다음 며칠 동안, 이동성이 뚜렷하게 향상되었다. 주사 부위에는 아무런 경화 또는 발적도 나타나지 않았다. 백혈구 서브세트와 단핵구 상 IL-1α 발현을 평가하고자, 채혈하고 유동 혈구 계산 분석(FACS)을 하였다. MABp1 수준을 확인하고, MABp1에 대한 약물 동력학적(pK) 데이터 수집을 시작하고자 혈장에 대한 분석도 하였다. PBMC의 FACS 분석은 대부분의 CD14+ 단핵구(72.6%)가 IL-1α를 발현한다는 점을 보여주었다. 3.2㎍/ml의 MABp1 혈장 농도가 관찰되었다.
6일째에, 다른 혈액 시료를 채취하여 FACS를 이용하여 MABp1에 대해 분석하였다. MABp1으로 염색된 CD14+ 단핵구의 빈도가 47.3%로 줄어들었다. MABp1의 혈장 수준은 7㎍/ml로 늘어났다. 확인되지는 않았지만, MABp1 농도의 증가는 MABp1 피하 투여의 저장 효과(depot effect)를 반영하는 것으로 여겨졌다. 개선되긴 했지만, 환자는 여전히 상당한 압통과 움직임에 따른 통증을 나타냈고, 환자가 파티에 참가하여 알코올을 섭취했던 그 전 주말 이후로 포도막염이 확 발발하였다. 환자는 피하로 110mg의 MABp1을 더 투여 받았다.
14일째에, 혈액 시료를 채취하여 FACS를 이용하여 분석하였고, 혈장에 대해 pK 분석을 하였다. MABp1으로 염색된 CD14+ 단핵구 빈도는 21.7%까지 더 감소하였다. 그러나 MABp1의 혈장 수준도 5.8㎍/ml로 감소하였다. MABp1의 혈장 수준이 최초 주사 후 그 주에 걸쳐 증가했기 때문에, 이는 예상치 못한 것이었다.
MABp1을 최초 주사한 지 약 한 달 후에 환자를 다시 평가하였다. 이동성이 눈에 띄게 향상되었으며, 손목에 통증이 없었다. 어깨 통증은 90°까지 벌렸을 때에만 나타났다. FACS 분석 결과, MABp1으로 염색된 감지할 수 있는 CD14+ 단핵구가 없는 것으로 나타났다. MABp1의 혈장 수준은 1.6㎍/ml로 감소되어, MABp1의 반감기가 약 2주임을 시사하였다.
다음 몇 주 동안, 환자는 점진적이지만 지속적인 이동성 향상을 나타내었다. 포도막염은 완전히 해소되었다. MABp1의 최초 주사 이후 환자가 모든 약물 이용을 중단했음에도 향상됨이 목격되었다. MABp1의 최초 주사 이후 대략 3개월 경과 시, MABp1으로 염색된 CD14+ 단핵구 빈도가 치료 전 수준으로 되돌아갔다. 혈장 내 MABp1 수준은 0.07㎍/ml으로 감소되었다. 그러나 환자는 어깨 움직임을 계속 향상하게 하는 일을 잘 지속하였다.
실시예 3 - hIL-1α에 대한 내인성 자가항체를 위한 혈장 시료 선별 및 MABp1의 약물 동력학
직접 ELISA를 이용하여 인간 IL-1α(hIL-1α)에 대한 내인성 자가항체를 위한 혈장 시료 선별 방법을 개발하였다. 이 방법은 또한 높은 희석률로 혈장 시료가 만들어진 것을 제외하고, 투여 후의 MABp1의 약물 동력학(pK)을 확인하는 데에 이용하였다.
직접 ELISA는 폴리스티렌 마이크로플레이트 상에 재조합 인간 IL-1α를 코팅하는 것을 수반한다. 결합된 인간 IL-1α는 시험 시료로부터 내인성 항 인간 IL-1α 항체를 포획한다. 그런 다음, HRP가 콘쥬게이트된 Fc에 특이적인 마우스-항-인간 IgG가 포획된 내인성 항-인간 IL-1α 항체를 검출하는 데 이용되며, TMB 기질 처리가 뒤따른다. HRP 효소와 반응시킬 때, TMB 기질은 진한 푸른 빛깔의 가용성 생성물을 생산한다. 효소적 반응은 푸른 빛깔의 생성물을 노란색으로 바꾸는 종결 용액 첨가로 중단시킨다. 450nm의 마이크로플레이트 리더 상에서 비색측정을 하였다.
시료당 약 5ml 혈장 시료가 제공된다. 혈장은 분취 전 2~8℃로 유지하며, -80℃에 저장한다. 혈장 시료는 시료로 이용하기 위해 1:500, 1:1000 및 1:2000배 희석시킨다. 마이크로플레이트 상에 1:5,000 및 1:10,000배 희석액으로 20㎍/ml MABp1 항체주를 함유하는 완충액 내 양성 대조군이 이용된다. 완충액은 1:1,000, 1:2,000 및 1:5,000으로 희석되는 미리 결정된 음성 대조군 혈장뿐만 아니라 음성 대조군으로 이용된다. 추가적인 양성 혈장 대조군이 이용되며, 그것은 20㎍/ml MABp1 항체를 섞은 혈장으로, 마이크로플레이트 상 시료를 위해 1:5,000 및 1:10,000으로 희석된다.
양성 대조군 값이 ± 2 표준편차의 범위에 들어가면, ELISA 데이터는 허용 가능하다고 여겨진다. 그러나 QC 양성 대조군 값이 ± 2 표준편차의 범위 밖에 있으면, ELISA 데이터는 허용 불가능하다고 여기며, 실험은 반복될 것이다. 칼레이다그래프를 이용하여, 표준용액의 대수 평균 흡광도를 흡광도 에러 바와 함께 대수 농도의 함수로서 기록한다. 표준곡선은 선형 행동을 나타내야 한다. 실시예 2에서 기술한 바와 같이 환자에서 채취한 시료의 약물 동력학 분석 결과는 도 1에 나타내었다.
실시예 4 - 혈통 서브세트의 유세포 계측(FACS) 검사
FACS 절차는 전혈 염색과 전혈에서 강화된 말초 혈액 단핵구 세포(PBMC)의 염색 모두를 위해 기술된다. 전혈 염색 및 PBMC 염색 모두, 모든 시료에 대해 하였다. 이러한 FACS 분석은 혈통 서브세트인 B 림프구와 T 림프구, NK 세포, 단핵구, 호중구 및 IL-1α+ 세포의 상대적인 퍼센트를 결정할 수 있게 한다. 실시예 2에서 기술한 바와 같이 환자로부터 채취한 시료의 FACS 분석 결과를 도 2 및 도 3에 나타내었다. 혈액 도말의 현미경 사진은 투여 후 32일 경과 시 분석하였을 때, MABp1 투여가 말초 혈액 단핵구에서 광범위한 공포형성을 유발하였음을 보여주었다.
실시예 5 - IL-1α 특이적 단일클론항체를 이용한 포도막염 치료
실시예 2에서 기술한 포도막염이 해소된 지 약 2개월 뒤, 환자는 또 다른 포도막염(대부분 홍채염) 삽화를 겪었다. 환자는 코르티코스테로이드와 비스테로이드성 항염 점적약(NSAIDS) 처리를 개시했다. 경구용 NSAIDS 또한 이용되었다. 포도막염은 처리에 반응이 없었고, 진행되고 있었다. 그러나 어떠한 관절의 연루 증거가 없었고, 어깨는 계속하여 이동성 향상을 보였다. 환자는 병에 걸린 눈에 국소적으로 MABp1을 투여 받았다. MABp1(15mg/ml 용액)은 분당 한 방울의 속도로 10분 동안 병에 걸린 눈에 총 열 방울(0.25ml에 대략 3.75mg)을 투여하였다. 투여하는 동안, 환자는 어떠한 통증도 호소하지 않았다. 그러나 몇 시간 후, 환자는 불편과 작열감을 보고하였다. 경구용 NSAIDs를 복용하고 환자는 수면을 취했다. 다음날 아침, 환자는 투여 전보다 상당한 향상, 통증 감소 및 더 적은 염증을 보고했다. MABp1 점적약을 최초 투여하고 24시간 경과 후, 환자는 같은 방식으로 열 방울을 투여하였다. 다시, 불편과 작열감이 기록되었다. 경구용 NSAIDs를 복용하고, 다시 환자는 수면을 취했다. 포도막염이 저절로 완전히 해소되었다. 추가적인 약물은 복용하지 않았다. 향후 4개월 동안 포도막염 재발이 관찰되지 않았다.
