개관
손상, 질환, 상처, 또는 수술로 인한 신속하고도 효과적인 뼈 결손 복구는 정형외과의 목표이다. 이러한 목적을 위하여, 뼈 결손 복구를 위한 다양한 조성물 및 물질들이 사용되거나 제안되어 왔다. 조성물과 재료들의 생물학적, 물리적 및 기계적 특성은 다양한 정형외과적 적용에 있어서 적합성 및 수행 성과에 영향을 끼치는 주요한 인자들이다.
뼈는 여러 유형의 세포들과 풍부한 석회화 세포외 기질을 포함한다. 정상 신체에서 뼈 흡수 및 뼈 형성은 신체의 형태 형성 및 칼슘 항상성이 관여하는 과정들이다. 생리적 역할 이외에도, 뼈 흡수는 골다공증, 대사 뼈 질환들, 뼈 골절, 및 악성 과칼슘혈증과 같은 병리적 장애에서 역할을 한다.
자가이식편(autograft) 또는 자가 뼈(autogenous bone)로도 알려진 자가 해면골(autologous cancellous bone; ACB)은 뼈 이식편의 최고의 표준으로 인정되고 있다. 대체 뼈 이식재료들을 확인, 개발하는 데에 상당한 노력이 있어 왔다. 우리스트(Urist)는 뼈 유도 이론 및 뼈 탈회방법, 즉 탈석회화 뼈 기질(demineralized bone matrix; DBM)의 제조방법에 대한 중요 논문을 발표하였다[참고: Urist M.R., Bone Formation by Autoinduction, Science 1965; 150(698): 893-9; Urist M.R. et al., The Bone Induction Principle, Clin. Orthop. Rel. Res. 53:243-283, 1967]. 쥐의 이소 부위(ectopic site)에 이식될 때, DBM 내에 함유된 뼈 유도 인자들로 인하여, DBM은 뼈 성장을 유도하는 뼈 유도물질이다[참고: Honsawek et al. (2000)]. 예를 들면, 형질전환 성장인자-β(TGF-β) 수퍼패밀리(superfamily)의 일부인 BMP 1-18(뼈 형성 단백질)과 같은 많은 뼈 유도 인자들이 알려져 있다. BMP-2은 널리 연구되어 왔다. DBM에는 단독으로는 뼈 유도를 하지 않지만 뼈 성장에 기여하는 다른 단백질들이 존재하며, 예를 들면, 섬유모세포 성장인자-2(FGF-2), 인슐린-유사 성장인자-I 및 -II(IGF-I 및 IGF-II), 혈소판 유래 성장인자(PDGF), 형질전환 성장인자-β 1(TGF-β 1)이다[참고: Hauschka, et al., 1986; Canalis, et al., 1988; Mohan et al., 1996].

고유 강도, 즉 수령 부위(recipient site)에서 하중 지탱 성능으로 인하여 석회화 뼈는 부분적으로 뼈 이식체에 사용된다. 종래의 구조적 뼈 이식체는 이식 용도로 채취된 피질골을 처리하고 기계 처리하거나 형상화하여 제조되었다. 뼈 이식체들은 입자들이 응집된 단일 뼈로 구성될 수 있다. 또한, 뼈 이식체들은 실질적으로 고형성, 유동성 또는 성형성일 수 있다. 피질골은 구조적 뼈 이식체의 특정 용도에 따라 다양한 형상으로 형태화될 수 있다. 때로 구조적 뼈 이식체들은 복잡한 기하 형태들, 예를 들면, 일련의 계단들; 오목 또는 볼록 표면들; 경사면들; 평탄 표면들; 인접 뼈, 도구 또는 이식체와 상응하는 표면들, 육각형 홈, 나사식 홀들과 체결되기 위한 표면들; 톱니 형태 등으로 제공된다.

일반적으로, 뼈 치유 또는 재형성은 파골세포에 의한 기존 뼈 재흡수, 새로운 혈관 형성 및 조골세포에 의한 새로운 뼈 성장을 포함하는 다단계 과정을 포함한다. 따라서, 뼈 흡수 및 뼈 형성은 서로 연결되어 있다. 일반적으로, 뼈 흡수는 파골세포 보충 속도 및 파골세포 활성 정도 및 지속에 따라 결정된다. 파골세포 보강에서 출발하면, 파골세포는 조혈 전구체, 예를 들면, 단핵 대식세포 계통에서 분화된 CFU-M에서 유래된다. 생체내 뼈 흡수를 자극하고 생체내 골수 배양액으로부터 파골세포 형성을 증가시키고 골다공증 발병 메카니즘(mechanism)과 연관되는 제제는 부갑상선 호르몬 및, 소정 조건에서는, IL-6을 포함한다. 뼈 흡수 이후, 뼈 재형성 사이클은 조골세포를 이용한 뼈 형성으로 이어진다.

균형잡힌 뼈 흡수 및 형성을 유지하는 조절 시스템은 RANKL/OPG 시스템(핵 인자 κ B 리간드에 대한 활성화 수용체와 오스테오프로테게린(OPG) 조절 시스템)이다. RANKL, 316-아미노산 막횡단 단백질(transmembrane protein)은 해면(또는 갯솜) 뼈에서 조골세포/기질세포에 의해 상당히 발현된다. 정의
본원에 사용되는 "생체적합성(biocompatible)"이란 생체 내에 투여된 후 장기 부작용을 일으키지 않는 물질을 기술하는 것이다.
본원에 사용되는 "뼈(bone)"는 자가, 동종, 이종 또는 유전자이식체 기원의 피질골, 해면골, 또는 피질-해면골을 의미한다.
본원에 사용되는 "염증(inflammation)"이란 감염 또는 자극에 대한 면역 시스템의 1차 반응이다. 본원에 사용되는 염증은 감염이 존재하지 않고 다핵 거대세포 존재로 특징되는 조직반응을 의미한다.

I. 개관
뼈 형성 촉진방법이 제공된다. 더욱 상세하게는, 파골세포 형성 촉진, 또는 파골세포의 뼈 흡수 활성 개선에 의한 뼈 형성을 촉진하는 방법이 제공된다. 따라서 뼈 재형성을 자극하기 위한 파골세포 및 관련 흡수 자극방법이 제공된다.
파골세포는 조혈 단핵 줄기세포 또는 조혈 전구세포에서 유래되는 혈액-운반 세포이다. 전구세포들은 서로 융합되고 기능적으로 성숙한 뼈 세포들로 분화된다. 파골세포 및 조골세포 계통의 세포들은 세포-세포 연접, 확산성 측분비 인자 및 세포-뼈 기질 상호작용을 통하여 상호 소통한다. 파골세포-조골세포 소통은 뼈 재형성 개시, 전이 및 종료 단계들에서 기본 다세포 단위(BMU)에서 진행된다. 개시단계에서, 조혈 전구체는 BMU에 보강된다. 이들 전구체는 c-Fms, RANK, 및 공동자극 분자들, 예를 들면, 파골세포-연관 수용체(OSCAR)를 포함한 세포 표면 수용체를 발현하며, 파골세포로의 분화에 이어서 조골세포와 세포-세포 연접하여 리간드를 발현한다. 양방향성 신호(bidirectional signal)는 파골세포에 있는 ephrinB2 및 조골세포 전구체에 있는 EphB4 간의 상호 작용에 의해 발생되고 전이를 가능하게 한다[참고: Matsuo K., Irie N., Osteoclast - Osteoblast Communication, Arch Biochem Biophys. 2008 May 15; 473(2):201-91].

또한, 거스텐펠드(Gerstenfeld) 등은, 파골세포 억제제(알렌드로네이트 및 데노수마브)는 대퇴부 편측 횡 골절 쥐에서, 연골 제거 및 골절 가골 재형성을 지연시킨다는 것을 보고했다[참고: Gerstenfeld LC, et al., J. Bone. Miner. Res., 2009 Feb; 24(2): 196-208].

. 적어도 하나의 자극물질 제공
자극물질이 제공된다. 자극물질은 파골세포 형성에 이르는 신호 단계들에 참여하여 자극 또는 영향을 주는 물질, 예를 들면, 단백질, 펩티드, 호르몬, 뉴클레오티드, 소분자, 거대분자 또는 이들의 조합물일 수 있다. 또한, 자극물질은 파골세포의 뼈 흡수 활성을 증강시키는 물질일 수 있다. 일부 양태들에서, 자극물질은 선택적으로 또는 추가적으로 조골세포 형성을 자극할 수 있다. 또한, 자극물질은 파골세포 분할 또는 융합을 증가시킬 수 있다. 일부 양태들에서 자극물질은 파골세포 성숙을 개선하거나 파골세포 수명을 연장시킬 수 있다. 자극물질은 파골세포 활성을 직접 자극하거나 파골세포 형성 또는 활성을 억제하는 인자 또는 세포와 경쟁, 비활성, 결합 또는 하향-조절하는 능력에 따라 선택될 수 있다. 예를 들면, 물질은 OPG 또는 기타 파골세포-억제자 분비를 감소시킬 수 있고, 따라서 파골세포 형성 억제를 방지할 수 있다. 따라서, 일반적으로는 자극물질은 "파골세포 자극자" 또는 "항-파골세포-억제자"이다.

조골세포성 기질세포는 세포-세포 상호작용/신호화 메커니즘을 통하여 파골 전구세포의 파골세포로의 분화에 영향을 준다. 뼈흡수 호르몬 및 시토카인은 전구세포의 파골세포로의 분화를 조절한다. 파골 전구세포의 파골세포로의 분화는 최소한 3개의 독립적인 신호 전달 경로들에 의해 조절된다: 1α,25(OH)2D3-, cAMP-, 및 gp130-매개 신호들. 뼈흡수 호르몬 및 시토카인에 대한 표적 세포들은 조골세포들이다. 조골세포는 뼈흡수 호르몬 및 시토카인에 응답하여 파골세포 분화를 유도하거나 자극하는 인자들을 생성한다. 또한, 조골세포는 용해성 인자들, 예를 들면, M-CSF 및 보체 C3을 생성하며, 이들은 파골세포 발생을 조절한다. 일부 양태들에 있어서, 이식체는 최소한 하나의 자극물질을 가진다. 이러한 물질은 이식체 재료에 직접 또는 혼합물로서 부가된다. 물질은 적용 생체 부위에 물질을 유지할 수 있는 적합한 운반체 재료에 포함될 수 있다. 운반체(carrier)는 생체적합성, 생체내 생분해성 및 세포 침윤이 가능한 다공성일 수 있다.

형성
이식체는 형상화, 성형화 및/또는 구조 변경될 수 있다. 이식체는 일체형 이식체 또는 작은 요소들의 조합일 수 있다. 이식체는 결정 또는 규격화 형상 또는 형태, 예를 들면, 시트, 판, 원반, 터널, 원추, 관 등의 형상을 가질 수 있다. 조립 기하형태로는 제한적이지 않지만, 단일 부위 용도의 초승달 에이프런(crescent apron), 골 내 결손 용도로 치하 사이에 배치되는 I-형상, 구강 및 혀 치조제 결손 용도의 직사각 비브(bib), 중립 판, 재건 판, 지지 판, T-지지 판, 수저 형상 판, 클로버 잎 형상 판, 과상 판, 압축 판, 다리 형상 판, 또는 파 형상 판을 포함한다. 평판뿐만 아니라 부분 관형도 뼈 이식편에서 제작될 수 있다. 이러한 판들은, 예를 들면, 오목 윤곽, 우묵 형상 또는 결손 형상의 형태를 포함할 수 있다. 이식체는 임의의 적합한 형상화 수단으로 가공되거나 형태화될 수 있다. 뼈 복구 부위에 맞도록 주문되는 이식체의 복잡한 3차원 구조를 상당히 정밀하게 제작하기 위하여 컴퓨터 모델링이 이용될 수 있다. 이식체는 생체 내 이식체 배치에 맞는 고정 요소들 또는 기타 기하 구조를 가질 수 있다. 이식체가 형상화 또는 성형될 수 있는 양태들에서, 이식체는 유체 내에서의 일관성을 보유한다.

따라서, 다양한 양태들에 있어서, 이식체는 일체형 또는 입자 응집체로 이루어질 수 있다. 이식체는 실질적으로 고형성, 유동성 또는 성형성일 수 있다. 이식체는 실질적으로 응집성 또는 다수의 분절들로, 예를 들면, 외피에 의해 보유될 수 있다.

. 선택적 첨가제
선택적으로, 기타 첨가제가 이식체에 제공될 수 있다. 사용되는 첨가제 함량은 첨가제 유형, 적용되는 특정 첨가조제 특이적 활성, 및 이식체 적용 용도에 따라 다르다는 것을 이해할 것이다. 소망 함량은 사용자에 의해 용이하게 결정될 수 있다. 임의 다수의 의학적 및/또는 외과적 유용한 선택적 물질들이 처지 전, 동안 또는 후에 조직내에 또는 결합하여 포함될 수 있다.

적합한 첨가제는 혈관형성 촉진 물질, 생체활성 제제, 의학적/외과적 유용물질 및 뼈 유도 제제를 포함한다. 당업자는 본원에 포함된 선택적 물질들 목록들은 배제적인 것이 아니라 기타 재료들도 본원에 개시된 이식체에 제공될 수 있다는 것을 이해할 것이다.
뼈 유도 제제
자극물질 이외에 기타 뼈 유도 제제가 이식체에 첨가될 수 있다. 이러한 제제들은 활성화 또는 비-활성화 형태로 첨가될 수 있다. 이러한 제제들은 이식체 제조 과정에서 언제든지 첨가될 수 있다.

. 용도
이식체는 뼈 복구 부위, 예를 들면, 손상, 수술, 감염, 악성 종양, 또는 발생적 기형으로 인한 결손에 따른 부위에 적용될 수 있다. 일부 양태들에 있어서, 이식체는 하중 지탱 기능을 하여야 하는 부위에 적용될 수 있다. 이식체는 대사성 뼈 질환 치료, 뼈 치유, 연골 복구, 척추 원반 복구, 힘줄봉합, 질환 또는 수술로 발생된 결손 복구, 경막 봉합에 사용될 수 있고 또한 다양한 정형외과, 치주, 신경외과, 및 구강 및 턱 얼굴 수술에 사용될 수 있다. 구조적 이식편이 필요한 적용 분야의 예로는 분절 이식, 척추 융합, 관절 고평, 대규모 뼈 재건 등을 포함한다. 파골세포 자극 특성을 가지는 거대 이식체는 치유 과정에 기여할 수 있다. 이식체는 또한 수의학 분야에도 사용될 수 있다.
또한, 이식체는 약물 전달기구, 예를 들면, 상처 치유를 촉진하는 인자들 또는 제제들을 전달하기 위하여 사용될 수 있다. 이식체는 달리 또는 추가적으로 항생제, 항종양 제제, 성장인자, 조혈인자, 영양제, 기타 상기 생체활성 제제를 포함하는 다른 약학적 제제를 전달하기 위하여 사용될 수 있다. 이식체에 포함되는 생체활성 제제의 함량은 크게 다르며, 전달되는 제제, 투여 부위 및 환자의 생리적 조건과 같은 인자들에 따라 변동될 것이다. 최적 수준은 이식체가 적용되는 특정 케이스에서 결정된다.

본 실시예는 전-파골세포 제공에 따른 간엽 분화를 보인다. 파골세포는 조혈 단핵 줄기세포 또는 조혈 전구세포에서 유래하는 혈액-운반 세포이다. 파골세포 및 조골세포 계통의 세포들은 세포-세포 연접, 확산성 측분비 인자(diffusible paracrine factor) 및 세포-뼈 기질 상호작용을 통하여 상호 소통한다. 파골세포-조골세포 소통은 뼈 재형성 개시, 전이 및 종료 단계들에서 기본 다세포 단위(BMU)에서 진행된다. 개시단계에서, 조혈 전구체는 기본 다세포 단위에 보강되고, 조골세포와 세포-세포 연접을 통하여 파골세포로 분화되도록 유도된다. 양방향성 신호는 파골세포에 있는 ephrinB2 및 조골세포 전구체에 있는 EphB4 간의 상호 작용에 의해 발생된다.
간엽 세포 분화에 대한 파골세포 전구체 효과를 테스트하기 위하여, 파골세포 전구체 세포주 RAW가 간엽 세포주, C2C12와 공동 배양되었다. 세포들은 1:4 비율로 DMEM, L-글루타민, 아스코르브산, 베타-글리세로인산염 및 덱사메타손 함유 뼈 형성 분화 배지를 사용하여 접종되었다. C2C12 간엽 세포주 배양액이 대조군으로 사용되었다.
조골세포 분화는 마커로서 염기성 인산분해효소 염색[염기성 인산분해효소 염색 키트, 미주리주 세인트 루이스 소재 시그마-알드리히(Sigma-Aldrich) 제품]을 이용하여 경과 10일에 평가되었다. 또한, 세포들은 가시적 마커로서 알리자린 레드로 염색되었다. 염기성 인산분해효소 및 알리자린 레드는 590nm에서 측정되었다. 도 2는 이들 연구 결과를 나타낸다. 염기성 인산분해효소(ALP) 및 알리자린 레드 (AR)가 세포 염색에 사용되었다. A: C2C12 대조 배양액; AR. B: C2C12 대조 배양액; ALP. C: C2C12 및 RAW 공동 배양액(AR). D: C2C12 및 RAW 공동 배양액; ALP. E: ALP 흡광도, 590nm. F: AR 흡광도, 590nm. 독립 t-검증(Unpaired Student T-Test)을 적용하여 데이터를 분석하였다.
RAW와 C2C12 세포들과의 공동 배양액(도 2C 및 도 2D)은 C2C12 대조 배양액(도 2A 및 도 2B)에 비하여 상당히 높은 수준의 염기성 인산분해효소 및 알리자린 레드 염색을 나타낸다. 피-염색 배양액 흡광도는 590nm에서 측정되었고, 도 2E 및 도 2F에 도시되어 있다. 도 2E는 염기성 인산분해효소 염색 도표이며, RAW/C2C12의 배양액에서의 흡광도는 C2C12만의 배양액에 비하여 2배 이상 증가된 것을 나타낸다.
이러한 데이터는, C2C12 간엽 세포는, RAW 파골세포 전구체 세포의 존재하에서, 조골세포 세포로 분화되도록 유도된 것을 의미한다. 따라서, 이식체 재료에 자가 파골세포 전구체를 접종하면 뼈 유도 특징이 이식 재료에 부가될 것이다.

뼈 가골 형성은 조골세포성 새로운 뼈 증가 및 파골세포성 흡수 활성에 의존한다. 재형성 단계에서, 파골세포는 매우 활성적이다. 파골세포 생물학의 주요 틀은 핵 인자 κ B(RANK) 활성 수용체, 이의 리간드(RANKL) 및 오스테오프로테게린(OPG)의 발견에서 출발한다. 이들 인자는 뼈 흡수 및 파골세포 활성 조절에 관여한다. RANK은 파골세포 및 일부 면역 시스템 세포들에 있는 막 수용체이다. RANK는 파골세포 분화, 생존 및 활성을 촉진하기 위하여 RANKL와 결합한다.
최근, 마르첼리(Marchelli) 등은 간부 골절 위축 불유합 환자들에서, 평균 혈청 OPG 수준이 치유 및 치유 중 대조 대비 상당히 증가된 것을 보였다. 환자들의 상당히 높은 혈청 OPG 수준은 RANK/RANKL/OPG 시스템 불균형을 의미하며, 파골세포가 하향 조절된다[참고: Marchelli D., et al., J. Orthop. Traumatol., 2009 Jun; 10(2): 55].
DBM와 혼합된 재조합 RANKL가 뼈 치유에 조력하는지를 테스트하기 위하여, 쥐 대퇴골 치명 결손 모델이 이용되었다. 밀리미터 단위의 3개의 결손들을 무-흉선 쥐 대퇴골에 발생시켰다. 쥐들은 두 그룹으로 분류되었고, 1 그룹의 뼈 결손에는 150mg의 Grafton™ DBM FLEX[뉴 저지주 이튼타운 소재의 오스테오테크 제품]가 이식되었고, 2 그룹에는 2㎍의 재조합 RANKL가 혼합된 Grafton™ DBM FLEX이 처방되었다. 경과 3주에, 쥐 대퇴골 결손은 마이크로CT로 평가되었다. 도 2에 도시되어 있는 바와 같이, 대조군 대비 RANKL 처방 그룹은 유의하게 높은 수준(p< 0.05)의 새로운 뼈 형성을 나타내었다(도 3). 데이터는 종속 t-검증으로 분석되었다.
이러한 결과들은 뼈 이식체 재료와 혼합된 재조합 RANKL이 뼈 형성을 자극하는 자극물질로서 기능할 수 있다는 것을 시사한다.
