족관절 내부 인공 삽입물 {ANKLE-JOINT ENDOPROSTHESIS}
거골에 연결되는 구성요소, 경골에 연결되는 구성요소, 및 중간부로 이루어진, 족관절을 대체하기 위해 사용되는 내부 인공 삽입물에 관한 것이다.
거골에 연결되는 구성요소, 경골에 연결되는 구성요소, 및 중간부로 이루어진, 족관절을 대체하기 위해 사용되는 내부 인공 삽입물은 공지되어 있다[DE-U-88 12 806호, 함부르크(Hamburg)에 사업장을 둔 주식회사 발데마르 링크 게엠베하 앤 코.(Waldemar Link Gmbh & Co.)의 하. 코푀드(H. Kofoed)가 저술한 팜플릿 "링크 에스.테.아.에르. 총체적인 족관절 인공 삽입물(LINK S.T.A.R. Totale Spunggelenkprothese)]. 거골 구성요소와 중간부는, 시상면(sagittal plane)에서 굽히기와 펴기를 가능하게 하는 슬라이딩 면을 통해 상호 작용한다. 경골 구성요소와 중간부는, 종축에 대해 회전을 허용하는 상호 작용 슬라이딩 면을 형성한다. 상기 슬라이딩 면은 AP 방향 및 LM 방향으로 보상 운동이 가능할 수 있도록 구성될 수 있다. 천연 인대 장치에 의해 안정화될 수 있다.
공지된 인공 삽입물에서 중간부의 상측 및 하측 슬라이딩 면은 전면에 서로 평행하게 배치되는데, 이는 관절이 인공 삽입물에 의해 대체됨으로써 방향 변화가 야기되지 않아야 하기 때문이다. 그러나 관절의 측부 인대 및 내측 인대는 수술 후에 종종 상이한 인장을 나타내어, 이로써 통증을 유발할 수 있는 것으로 나타났다. 이는 해부학 상의 불규칙성에 근거하거나, 수술의사가 인공 삽입물을 경골에 연결시키기 위해서 바람직하지 않은 절제면을 선택하는 것에 근거한다.
본 발명의 목적은 한편으로는 해부학적 또는 수술 상황과, 다른 한편으로는 인공 삽입물과의 사이의 불균형을 방지하거나 완화시키기 위한 것이다.
본 발명에 따른 해결 방법은 특허청구범위 제1항 또는 제6항의 특징부에 나타나 있다. 본 발명에 따라 중간부 및/또는 경골 구성요소는 웨지 형태로 형성된다. 교정되지 않은 통상의 인공 삽입물이 사용된 경우, 수술의사가 인대 인장이 상이하다는 것을 확인하게 되면, 수술의사는 전면에 웨지 형태를 갖는 교정 구성요소를 사용함으로써 이를 보상할 수 있다. 웨지 형태의 교정판 중에서 보다 두꺼운 측면은, 통상의 구성요소를 사용하는 경우 인대 인장이 불충분할지도 모르는 측면에 배치시킨다. 수술의사가, 경골의 절제면이 경골 방향에 대해 수직이 아니라는 것을 인지한 경우 또는 다른 이유로 인해 인공 삽입물의 면들이 경골 축에 대해 수직으로 연장되지 않는 것을 원할 경우, 수술의사는 시상면에 웨지 형태가 위치하는 교정 구성요소를 삽입할 수 있다. 상부 구성요소의 하부 슬라이딩 면이 경골 방향 또는 부하 방향에 대해 특정한 방향을 갖기를 원하는 경우, 수술의사는 일반적으로 교정 구성요소로서 설계된 경골 구성요소를 사용하게 된다. 그러나 이와는 달리, 경골의 절제면의 방향이 정확하고 발의 해부학을 고려하는 경우, 수술의사는 교정 구성요소로서 설계된 중간부를 선호한다.
상술된 본 발명은, 본 발명에 선행되지만 공개되지 않은 특허원 PCT/EP02/02573호가 대상으로 하는, 전면에서 중간부의 웨지 형태의 설계에 관한 것이 아니다.
상부 구성요소가 웨지 형태인 것은, 상측의 연결면 뿐만 아니라 하측의 슬라이딩 면이 평평하거나 거의 평평하기 때문에 실제로 전혀 곤란하지 않다. 시상면에서 중간부가 임의의 웨지 형태로 결정되기 위해서 중간부와 비교하는 것이 중요하다. 게다가, 중간부의 대략 평면인 상측 슬라이딩 면의 방향 뿐만 아니라 하측 슬라이딩 면의 전체적인 방향도 간단하게 결정된다. 중간부가, 통상의 중간부에 비해 전방으로 또는 후방으로 점점 두꺼워지는 경우, 본 발명에 따른 의미에서 시상면에서 중간부가 웨지 형태라는 것이다.
시상면에서의 웨지 형태는 본 발명의 범위 내에서 전면에서의 웨지 형태와 조합될 수 있다. 그러나 시상면 또는 전면에서의 웨지 형태는 전면 또는 시상면에서 웨지 형태가 아니라도 상관없다.
웨지각은 유리하게 1° 내지 16°, 바람직하게 3° 내지 8°이다.
중간부의 웨지 형태의 방향이 종축 주위로의 중간부의 회전에 의해 변경될 수 없도록 하기 위해, 그 방향은 거골에 의해 또는 경골에 의해 어느 방향을 결정하도록 (예컨대, 원통형으로) 설계된 상호 작용하는 슬라이딩 면의 쌍에 의해서 강제적으로 고정되는 것이 바람직하다. 이를 위해서는 거골과 중간부 사이의 관절이 적합하다.
수술의사가 다양한 웨지각을 선택할 수 있도록, 의사는 각각의 인공 삽입물에 대해 복수의 다양한 교정 구성요소를 이용할 수 있어야 한다. 이와 관련되는 비용을 절감하기 위해, 교정 구성요소는 가능한 한 간단하게 설계되는 것이 바람직하다. 따라서 항상 일정하게 유지되는 표준 부품 및 웨지 부품으로 조립되는 것이 바람직할 수 있다. 표준 부품은 오직 하나의 구조로 제공된다. 단지 웨지 형태만 다양한 변형으로 제공될 필요가 있다. 이는 특히, 상부 구성요소가 교정 구성요소로서 사용되고 표준 부품이 경골상의 고정을 위해 필요한 수단을 형성하는 경우에 적합하다.
본 발명에 의하면, 내부 인공 삽입물의 상부 구성요소 또는 중간부가 웨지 형태이므로, 수술의사가 인공 삽입물이 삽입될 경골의 절제면의 각도에 따라 다양한 웨지각을 선택할 수 있어, 인공삽입물의 해부학적 또는 수술 상의 불규칙성이 보상될 수 있다.

도7은 두 부분으로 형성되는 상부 교정 구성요소가 도시된 도면이다.
상부 구성요소(3), 하부 구성요소(4) 및 중간부(5)로 구성되는 인공 삽입물이 경골(1)과 거골(2) 사이에 배치된다. 상부 구성요소(3)는 판 형태의 부분(6)을 포함하고, 이의 하측(7)은 평평한 슬라이딩 면을 형성한다. 돌출부(8)는 경골(1)의 상응하는 절제 함요(9) 내의 고정을 위한 기능을 한다.
하부 구성요소(4)는 볼록하게 만곡된, 실린더형 또는 웨지형으로 형성될 수 있는 슬라이딩 면(10)을 형성한다. 이 구성요소는 리브(11)를 포함하며, 리브는 굽히기 운동 및 펴기 운동 시에 중간부의 상대 운동 방향을 향하여 존재한다. 또한, 하부 구성요소는 경골(1) 및 비골(13)의 상응하는 슬라이딩 면과 상호 작용하기 위한 하부 구성요소의 측면(12)을 포함한다.
중간부(5)는 슬라이딩 면(7)에 맞는 평면인 상측(15) 및 하부 구성요소(4)의 슬라이딩 면(10)을 보완하도록 설계된 하부 슬라이딩 면(16)을 갖는다. 중간부의 하측은 리브(11)를 수용하기 위한 홈(17)을 갖는다. 이로써 중간부(5)는 하부 구성요소(4)에 대해 측면을 따라 안내된다. 오로지 굽히기 운동과 펴기 운동만이 허용된다.
바람직하게 상부 구성요소(3) 및 하부 구성요소(4)는 금속으로 이루어지고, 중간부(5)는 폴리에틸렌과 같이 슬라이딩에 유리한 플라스틱으로 이루어진다. 그러나 또한, 강도와 슬라이딩 성능이 충분한 다른 재료, 예를 들면 세라믹도 사용될 수 있다.
슬라이딩 면(10 및 16)의 보완 형태에 의해, 그리고 홈(17)과 상호 작용하는 리브(11)에 의해, 중간부(5)는 족관절 구성요소(4)에 대해 종축 주위로 회전 불가능하다. 또한, 그 방향은 하부 구성요소의 방향에 의해 고정된다. 도시된 실시예는 하부 구성요소와 중간부 사이에서 종축에 대한 이러한 회전 운동을 완전히 배제하는 반면, 사전에 결정된 한계 내에서 회전 운동이 허용되거나, 슬라이딩 면의 설계에 의해서만 제한되거나 또는 회전운동이 배제되지 않는 실시예도 고려할 수 있다.
도1 내지 도3을 참조로 하여 상술된 실시예는 통상의 구성요소를 사용하는 실시예에 대해서 뿐만 아니라 교정 구성요소를 사용하는 실시예에 대해서도 해당된다.
도4 내지 도6에서는 교정 구성요소에 대한 실시예가 도시되어 있다. 도4는 교정 구성요소로서 설계된 상부 구성요소(3)의 전면도이다. 이 구성요소는 도면에서 왼쪽에 나타나 있는 가장자리(20) 근처에서 반대편에 위치한 가장자리 보다 더 두껍게 형성되어 있다. 이 구성요소는 두꺼운 측면(20)이 수술의사의 선택에 따라 관절의 측면 또는 중간면에 위치할 수 있도록 이의 전면 중간평면에 대해 대칭적으로 설계된다.
도5는 상부 구성요소(3)의 시상 절편을 도시한다. 이 구성요소는 도면에서 왼쪽에 나타나 있는 단부(21)에서 웨지형으로 두껍게 형성된다. 이 구성요소의 상측은 전면에 대해 대칭적으로 설계된다. 따라서 두꺼운 단부는 수술의사의 선택에 따라 관절 내에서 전방 또는 후방에 배치될 수 있다. 상부 고정면(23)과 하부 슬라이딩 면(7) 사이의 웨지각(22)은 두 실시예 모두에서 5°의 범위를 나타낸다.
도6은 교정 중간부(5)의 시상 절편을 도시한다. 슬라이딩 면으로서 설계되는 이의 하측(16)은 전체적으로, 상부 슬라이딩 면(15) 근처에서의 웨지각(19)을 나타내기 위해 그려 넣은 보조 라인(24)에 대해 대략 평행하게 연장되는 방향을 가진다. 이러한 경우, 통상의 중간부에서의 라인(24)이 하측(16)의 전체 방향에 대해 평행하에 연장되는 것을 전제로 한다. 중간부의 웨지 형태를 결정함에 있어서 중요한 요인은 항상 인공 삽입물 시스템의 통상의 구성요소와의 비교이다.
교정은, 각각의 경우에 있어서 단 하나의 구성요소에 제한할 필요는 없다. 오히려 교정 구성요소는, 상부 구성요소를 위해서 뿐만 아니라 중간부를 위해서도 사용될 수 있다. 도1은 이러한 가능성을 도시한다.
수술 의사는 하부 구성요소(4)를 이식하자마자, 의사는 적합한 기구를 사용하여, 측부 인대가 인장되었을 때, 경골(1)의 절제면이 하부 구성요소(4)에 대해 정상적으로 연장되는지 또는 교정이 필요한지의 여부를 확인할 수 있다. 후자의 경우, 의사는 상부 구성요소 또는 중간부를 위해 둘 다 교정 구성요소를 선택해야만 하는지의 여부 및 각각의 웨지 형태가 얼마나 강해야 하는지 및 어떤 방향이어야 하는지를 결정한다. 또한, 상부 구성요소(3)를 삽입하는 경우, 상응하는 측정이 가능하다. 그리고 나서 중간부로서 교정 구성요소를 사용할지의 여부를 결정할 수 있다.
도7은 표준 부품(25) 및 웨지 부품(26)으로 이루어진 상부 구성요소(3)의 조립상태를 도시하고 있다. 표준 부품(25)은 고정 부재(돌출부, 8)를 형성하기 때문에, 복수의 실시예에서 다양한 웨지각으로 사용될 수 있는 웨지 부품(26)이 더욱 간단하게 형성될 수 있다. 두 부분은 임의의 공지된 방식으로 서로 연결될 수 있다. 예를 들면 이들 둘은 서로 나사로 연결될 수 있다. 또한, 두 부분이 사실상의 소모 없이 서로 삽입되는 것이 가능하게 되는, 서로 보완하면서 맞물리는 돌출부와 수용부가 제공될 수 있다.

Claims (7)

  1. 족관절을 대체하기 위해 사용되는 내부 인공 삽입물이며, 상부 슬라이딩 면(10)을 형성하며 거골(2)에 연결되는 하부 구성요소(4)와, 하부 슬라이딩 면(7)을 형성하며 경골의 절제면과 연결되기 위해 상부 연결 면(23)을 포함하는 상부 구성요소(3)와, 상부와 하부 구성요소(3, 4)의 슬라이딩 면(7, 10)과 상호 작용하는 두 개의 슬라이딩 면(15, 16)을 포함하는 중간부로 이루어진 내부 인공 삽입물에 있어서,
    상기 상부 구성요소(3)는 이의 하부 슬라이딩 면(7)과 이의 상부 연결면(23) 사이의 전면 절편, 시상 절편, 또는 전면과 시상 절편에서 웨지 형태이거나,
    상기 중간부(5)는 이의 슬라이딩 면(15, 16) 사이의 시상 절편에서 웨지형태이거나,
    상기 상부 구성요소(3)는 이의 하부 슬라이딩 면(7)과 이의 상부 연결면(23) 사이의 전면 절편, 시상 절편, 또는 전면과 시상 절편에서 웨지 형태이고 상기 중간부(5)는 이의 슬라이딩 면(15, 16) 사이의 시상 절편에서 웨지 형태인 것을 특징으로 하는 내부 인공 삽입물.
  2. 제1항에 있어서, 하부 구성요소(4) 및 중간부(5) 상에서 상호 작용하는 슬라이딩 면(10, 16)은 종축에 대해 회전 불가능하게 상호 작용하는 것을 특징으로 하는 내부 인공 삽입물.
  3. 제1항에 있어서, 상부 구성요소(3) 및 중간부(5) 상에서 상호 작용하는 슬라이딩 면(7, 15)은 종축에 대해 회전 가능하게 상호 작용하는 것을 특징으로 하는 내부 인공 삽입물.
  4. 제1항 내지 제3항 중 어느 한 항에 있어서, 웨지각(19, 22)은 1° 내지 16°인 것을 특징으로 하는 내부 인공 삽입물.
  5. 제1항 내지 제3항 중 어느 한 항에 있어서, 웨지 형태의 구성요소(3)는 다양한 웨지각을 갖는 웨지 부품(26) 및 표준 부품(25)으로 이루어진 것을 특징으로 하는 내부 인공 삽입물.
  6. 제4항에 있어서, 웨지 형태의 구성요소(3)는 다양한 웨지각으로 사용될 수 있는 웨지 부품(26) 및 표준 부품(25)으로 이루어진 것을 특징으로 하는 내부 인공 삽입물.
  7. 족관절을 대체하기 위해 사용되는 내부 인공 삽입물 시스템이며, 상부 슬라이딩 면(10)을 형성하며 거골(2)에 연결되는 하부 구성요소(4)와, 하부 슬라이딩 면(7)을 형성하며 경골(1)의 절제면과 연결되기 위해 연결 면(23)을 포함하는 상부 구성요소(3)와, 상부와 하부 구성요소(3, 4)의 슬라이딩 면(7, 10)과 상호 작용하는 두 개의 슬라이딩 면(15, 16)을 포함하는 중간부(5)로 이루어진 내부 인공 삽입물이며, 상기 시스템은 비교정 상부 구성요소와 중간부를 포함하며, 중간부의 상측과 하측은 전체적으로 평행한 시스템에 있어서,
    상기 시스템은 비교정 상부 구성요소(3)에 대해 교환 가능한 교정 요소를 포함하며 교정 요소는 교정 요소의 상측과 하측(7, 23) 사이에서 시상면, 전면, 또는 시상면과 전면에서 웨지 형태로 형성되거나, 상기 시스템은 비교정 중간부(5)에 대해 교환 가능한 교정 중간부를 포함하며 교정 중간부는 교정 중간부의 상측(15)과 하측(16)의 전체 면 사이에서 시상면에서 비교정 중간부(5)에 비해 웨지 형태이거나, 상기 시스템의 교정 요소와 교정 중간부 모두 웨지 형태인 것을 특징으로 하는 내부 인공 삽입물 시스템.
