Abstract
본 발명은 (a) 카르복시 폴리메틸렌 중합체; (b) 글리신; (c) 칼슘 이온 공급원; 및 (d) 물을 포함하는 겔 조성물에 관한 것이다. 또한 본 발명은 상기 성분 (a), (b) 및 (c)를 포함하는 무수 조성물에 관한 것이다. 본 발명의 겔 조성물은 코피의 예방 및 치료에 유용하다. 겔 조성물은 비강 내로 쉽게 투여할 수 있으며, 사용 후에 제거할 필요가 없다.
Description
본 발명은 코피(비출혈) 치료에 유용한 신규 조성물에 관한 것이다. 특히, 본 발명은 비강에 쉽게 투여할 수 있고 사용 후에 제거할 필요가 없는 코피 예방 및 치료용 겔 조성물에 관한 것이다.
코피(비출혈)는 매우 흔하다. 대부분의 경우에, 증상은 일시적이고 재발하지 않지만, 일부 환자는 재발과 심각한 증상을 겪게 된다. 중증의 코피를 치료하는 가장 일반적인 방법은 출혈 혈관을 소작하는 것이다. 출혈 근원지를 볼 수 없는 경우에는, 순면 탐폰 또는 팽창성 고무 풍선을 이용한 비내 충전물을 사용할 수 있지만, 이러한 방법은 환자를 불편하고 불쾌하게 한다. 통상적으로, 순면 탐폰은 폭이 2 cm이고 길이가 40∼100 cm이다. 이 탐폰을 코로 밀어넣어 비점막을 압박하여 출혈을 멈추게 하고, 적소에 1∼4일 동안 둘 수 있다. 팽창성 고무 풍선도 비점막을 압박하여 작용하며, 적소에 1∼2일 동안 유지시킬 수 있다. 탐폰을 사용하든 풍선을 사용하든, 삽입할 때 아프고, 정상적인 호흡을 방해하며, 국소 감염을 유발할 수 있다. 이러한 단점이 없는 치료법이 필요하다.
WO 01/00218호는 멸균 수용액 ℓ당 약 0.5∼5 g 농도로 사해 소금을 포함하고, 완충액을 함유하며, 유해한 유기 불순물을 실질적으로 함유하지 않는 비강 스 프레이 조제물을 개시하고 있다. 이 조제물은 비염, 부비동염, 코피 및 수술후 자극 치료에 사용되는 것으로 기재되어 있지만, 코피에 관한 실험은 보고된 바 없다.
상이한 비강 겔 도포제 및 혈관 수축제가 실험적으로 연구된 바 있다. 문헌[Bende et al, Acta Otolaryngol (Stockh) 88, 459-461 (1979); Bende et al, Acta Otolaryngol (Stockh) 102, 488-493 (1986); 및 Bende et al, Acta Otolaryngol (Stockh) 110, 124-127 (1990)] 참조. 초기 임상 연구 결과, 섬유소 분해가 재발성 코피에 관여하는 것으로 나타났다. 문헌[Petruson et al, Acta Otolaryngol (Stockh) suppl. 317 (1974)] 참조. Tibbelin 등[ORL (Basel) 57, 207-209 (1995)]은, 무작위의 이중맹 다기관 임상 시험에서 트라넥삼산 겔과 플라시보 겔을 국소 도포한 경우의 지혈 효과를 연구하였다. 이들 2가지 겔은 둘다 유리한 효과를 발현하는 것으로 나타났으며, 놀랍게도 플라시보 겔이 트라넥삼산(섬유소분해 시스템의 억제제)을 함유한 것보다 (비록 유의적이지는 않지만) 약간 더 양호한 것으로 나타났다.
발명의 개요
본 발명자들은 놀랍게도 비강 겔로서 사용하기 위한 개선된 조성물을 발견하였는데, 이 조성물은 상기 언급한 기존의 플라시보 겔의 모든 유리한 성질을 보유하면서 코피 치료에 더욱 효과적이다.

카르복시 폴리메틸렌 중합체(카보머라고도 알려짐)는 폴리알케닐 에테르 또는 디비닐 글리콜과 가교된 아크릴산의 중합체이다. 카보머는 기본 구조 단위 를 가진다. 카보머는 미국 오하이오주 클리블랜드 소재의 노베온에서 일정 범위 등급으로 상품명 카보폴(Carbopol)로 입수가능하다. 상기 등급은 가교도 및 제조 조건에 따라 달라진다. 미정제 카보머 중합체는 백색 분말이지만, 친수성이 강하며, 본래 부피의 최대 1000배로 물에서 팽윤하여 겔을 형성할 수 있다. 카보머는 실질적으로 생물학적으로 비활성이며, 오랫 동안 각종 종류의 국소 조제물에 안전하게 사용되어 왔다. 카보머는 상피 표면에 부착된 점막층으로부터 물을 유인함으로써, 점막 표면에 부착하는 것으로 알려져 있다.
카보머를 비강 내에 넣으면, 점막과 출혈 중인 혈관을 압박하는 효과가 발현된다. 이러한 방식으로, 카보머는 점막을 압박하고 코피 치료품으로서 사용되는 탐폰과 유사하게 작용한다(상기 배경 기술 참조). 그러나, 겔은 점막섬모 수송 체계에 의해 코 뒤쪽을 통해 인후로 수송되어 투여 1 내지 2 시간 내에 삼켜지기 때문에 일반적으로 겔을 제거할 필요가 없다는 점에서 겔이 탐폰보다 유리하다.
본 발명에 사용하기 위한 바람직한 카르복시 폴리메틸렌 중합체는 노베온에서 입수가능하며 상표명 카보폴로 시판되는 것들이다. 카보폴 974P NF 및 카보폴 971P NF가 특히 바람직한 중합체이다.
사용되는 카르복시 폴리메틸렌 중합체의 양은 중합체(들)가 무엇인지에 따라 달라진다. 통상적으로 카르복시 폴리메틸렌 중합체는 총 무수 구성성분의 5∼50 중량%, 바람직하게는 10∼35 중량%, 더욱 바람직하게는 15∼30 중량%, 가장 바람직하게는 약 20 중량%를 구성한다.
카르복시 폴리메틸렌 중합체를 통상적으로 겔 1 ℓ당 10∼100 g 건조 중량, 바람직하게는 20∼50 g/ℓ, 더욱 바람직하게는 30∼40 g/ℓ, 가장 바람직하게는 약 35 g/ℓ의 농도로 사용하여 본 발명의 겔 조성물을 형성할 수 있다.
b) 글리신
겔의 제2 성분은 아미노산 글리신(아미노아세트산)이다. 이것은 소위 말하는 "필수 아미노산"이 아니며, 인체 자체에서 생산될 수 있으며, 모든 20종의 아미노산 중 가장 단순한 것이다. 글리신은 각종 대사 반응에 연루되어 있으며, 인체의 단백질 형성에 관여한다.
본 발명자들은 글리신이 겔에 고삼투압성 작용을 부여하는 것을 발견하였다. 비점막으로부터 물이 빠지는데, 이것이 의미하는 바는 점막의 출혈이 완화되고 점막이 수축하며, 혈관으로의 혈류가 감소하고, 출혈이 정지될 수 있다는 것이다. 글리신은 인간 세포의 빌딩 블록이기 때문에, 비점막과 혈관벽의 복구에도 역할을 할 수 있다.
글리신은 상업적으로 널리 입수가능한 물질이다.
본 발명의 무수 조성물에 있어서, 글리신은 통상적으로 총 구성성분의 30∼90 중량%, 바람직하게는 50∼80 중량%, 더욱 바람직하게는 약 70 중량%를 구성한다.
본 발명의 겔 조성물에 있어서, 글리신의 농도는 통상적으로 겔 1 ℓ당 50∼200 g 건조 중량, 바람직하게는 80∼150 g/ℓ, 더욱 바람직하게는 110∼120 g/ℓ, 가장 바람직하게는 약 117 g/ℓ이다.
c) 칼슘 이온 공급원
제3의 필수 성분은 칼슘 이온(Ca2+) 공급원이다. 칼슘 이온은 생물학적 응고 과정에 연루되어 있다. 혈액 응고의 최종 단계에서, 트롬빈은 단백질 피브리노겐(분자량 340,000)을 가용성 피브린으로 변환시킨다. 마지막으로, 가용성 피브린은 휠씬 더 효과적으로 물리력을 견딜 수 있는 불용성 피브린으로 전환될 수 있으며, 그 결과 재출혈이 예방될 수 있다. 어떤 이론에 구애받고자 하는 것은 아니지만, 피브린의 불용성 형태로의 전환은 칼슘 이온의 존재 하에 더욱 빠르게 진행되며, 이의 흡수는 비점막이 매우 얇기 때문에 증진될 수 있는 것으로 생각된다.
칼슘 이온은 임의의 편리한 형태로 존재할 수 있지만, 바람직하게는 가용성 염의 형태, 예를 들어 염화물 또는 또 다른 염의 형태, 예컨대 아세테이트, 포스페이트, 카르보네이트 또는 글루코네이트로 존재한다. 염화칼슘이 가장 바람직하다.
칼슘 이온의 총량은, 무수 구성성분의 총 중량을 기준으로 Ca2+ 이온 단독의 중량으로 표시한 경우, 통상적으로 총 무수 구성성분의 0.005∼0.1 중량%, 바람직하게는 0.01∼0.05 중량%, 더욱 바람직하게는 0.025∼0.040 중량%, 가장 바람직하게는 약 0.035 중량%이다.
본 발명의 겔 조성물에 있어서, 칼슘 이온의 농도는 통상적으로 겔 1 ℓ당 10∼200 ㎎, 바람직하게는 20∼100 ㎎/ℓ, 더욱 바람직하게는 40∼70 ㎎/ℓ, 가장 바람직하게는 약 58 ㎎/ℓ이다.
기타 성분
겔 형태로 조성물을 조제할 때, 예컨대 환자에게 투여하기 전에 물을 사용한다. 사용되는 물의 양은 기타 성분과 혼합시 적절한 점도의 겔을 형성하는 데 필요한 양이다. 그 양은 사용되는 카르복시 폴리메틸렌 중합체(들)가 무엇인지에 따라 달라진다. 통상적으로, 물은 겔 1 kg당 750∼950 g, 바람직하게는 800∼900 g/kg, 더욱 바람직하게는 약 850 g/kg를 차지한다.
상기 언급한 3종의 필수 성분 외에, 조성물의 조제 분야에 통상적인 기타 추가 재료, 예컨대 pH 조정제(산, 알칼리, 완충제), 보존제(예, 메타르겐, 프로파긴), 항산화제, 색소 및 염료, 향료 물질, 부형제, 담체 등을 사용할 수 있다.
처리 조건
임상용 겔로서 본 발명의 조성물을 조제하기 위해서, 하기 단계를 실시한다.
칼슘 이온 공급원, 예컨대 염화칼슘을 물에 용해시킨다. 칼슘 이온 공급원과 혼합하기 전 또는 후에, 적량의 산, 알칼리 또는 완충 물질을 물에 첨가하여 최종 겔의 pH가 적당하도록, 예컨대 6.5∼7.5의 범위가 되도록 한다. 이어서, 완전히 용해될 때까지 교반하면서 글리신을 첨가한다. 그 다음 교반하면서 카르복시 폴리메틸렌 중합체를 서서히 조심스럽게 (예컨대 수성 액체에 분말을 점차적으로 뿌려서) 첨가한다. 모든 중합체가 용해될 때까지 계속 교반한다. pH가 필요한 범위 6.5∼7.5 이내가 되도록 겔의 pH를 검사할 수 있다.
겔의 성분을 배합하는 순서는 앞의 단락에서 기재한 것과 다를 수 있다. 예를 들어, 무수 구성성분을 먼저 함께 혼합할 수 있으며, 그 다음 적량의 물을 첨가하여 적당한 점도의 겔을 형성한다. 무수 구성성분의 임의의 소정 배합에 대해서, 당업자는 예컨대 시행착오에 의해 적당한 점도의 겔을 형성하는데 적당한 물의 양을 결정할 수 있다.
임의의 적당한 온도에서 겔 제조를 실시할 수 있다. 그러나, 실온에서의 제조가 가장 편리하다.
전술한 처리 조건은, 당업자에게 익숙한 방식으로 변화시켜 최적화할 수 있다.
투여 방식
조성물은 겔 형태로 사용된다. 사용 전에, 환자에게 코를 풀라고 해야 한다. 바람직하게는, 코의 내부 표면을 살짝 세척해내서, 겔과 비점막의 접촉을 최대화해야 한다. 그 다음, 예를 들어 사전 충전한 주사기를 사용하여, 겔을 출혈 콧구멍(들)에 삽입해야 한다. 바람직하게는 전체 비강을 충전해야 한다. (이것은 겔이 비강의 뒤에서 인후로 흘러내리기 시작한다고 환자가 표시하는 경우 겔의 투여를 중단함으로써 실시할 수 있다.) 그러나, 충전이 완료되기 전에 출혈이 확실히 멈춘 경우 전체 비강을 채울 필요는 없을 수 있다. 이어서, 작은 순면 조각 또는 다른 적절한 마개를 환자의 콧구멍에 넣을 수 있다. 겔이 효과를 나타내도록 충분한 시간 동안, 예컨대 30분 동안 환자가 코를 풀지 않도록 한다.
이하의 본 발명의 제조 및 사용 실시예는, 단지 본 발명을 예시하기 위해서 제공되는 것이며, 발명의 범위를 제한하는 것으로 간주되어서는 안된다.
순수 504 g 및 1 M 수산화나트륨 용액 352 ㎖(366 g)을 유리 용기에 함께 두었다. 염화칼슘(CaCl2·2H2O) 185 ㎎을 첨가하고, 염화칼슘이 용해될 때까지 혼합물을 교반하였다. 용해가 완료될 때까지 교반하면서 글리신 100 g을 첨가하였다. 15 g의 카보폴 PNF 974 및 15 g의 카보폴 PNF 971를 교반하면서 첨가하였다. 생성되는 겔이 더욱 점섬이 됨에 따라서 교반 속도를 계속 증가시켰다. 모든 카보머가 용해될 때까지 계속 교반하였다. 전술한 모든 단계를 실온에서 실시하였다. pH가 필요한 범위 6.5∼7.5 내가 되도록 겔의 pH를 조사하였다.
Claims (22)
- (a) 카르복시 폴리메틸렌 중합체;(b) 글리신;(c) 칼슘 이온 공급원; 및(d) 물을 포함하는 겔 조성물.
- 제1항에 있어서, 상기 카르복시 폴리메틸렌 중합체는 카보폴(Carbopol) 974P NF 및/또는 카보폴 971P NF인 겔 조성물.
- 제1항 또는 제2항에 있어서, 상기 카르복시 폴리메틸렌 중합체의 농도는 겔 1 ℓ당 10∼100 g 건조 중량인 겔 조성물.
- 제3항에 있어서, 상기 카르복시 폴리메틸렌 중합체의 농도는 겔 1 ℓ당 20∼50 g 건조 중량인 겔 조성물.
- 제4항에 있어서, 상기 카르복시 폴리메틸렌 중합체의 농도는 겔 1 ℓ당 30∼40 g 건조 중량인 겔 조성물.
- 제5항에 있어서, 상기 카르복시 폴리메틸렌 중합체의 농도는 겔 1 ℓ당 약 35 g 건조 중량인 겔 조성물.
- 제1항 내지 제6항 중 어느 하나의 항에 있어서, 상기 글리신의 농도는 겔 1 ℓ당 50∼200 g 건조 중량인 겔 조성물.
- 제7항에 있어서, 상기 글리신의 농도는 겔 1 ℓ당 80∼150 g 건조 중량인 겔 조성물.
- 제8항에 있어서, 상기 글리신의 농도는 겔 1 ℓ당 110∼120 g 건조 중량인 겔 조성물.
- 제9항에 있어서, 상기 글리신의 농도는 겔 1 ℓ당 약 117 g 건조 중량인 겔 조성물.
- 제1항 내지 제10항 중 어느 하나의 항에 있어서, 상기 칼슘 이온의 농도는 겔 1 ℓ당 10∼200 ㎎인 겔 조성물.
- 제11항에 있어서, 상기 칼슘 이온의 농도는 겔 1 ℓ당 20∼100 ㎎인 겔 조성물.
- 제12항에 있어서, 상기 칼슘 이온의 농도는 겔 1 ℓ당 40∼70 ㎎인 겔 조성물.
- 제13항에 있어서, 상기 칼슘 이온의 농도는 겔 1 ℓ당 약 58 ㎎인 겔 조성물.
- 물과 무수 성분 (a), (b) 및 (c)를 임의의 원하는 순서로 혼합하는 것을 포함하는, 제1항 내지 제14항 중 어느 하나의 항의 조성물의 제조 방법.
- 제1항 내지 제14항 중 어느 하나의 항의 겔 조성물의 유효량을 인간 또는 인간 이외의 동물의 비강 내로 삽입하는 것을 포함하는, 예방 또는 처치에 의한 코피 치료 방법.
- 제1항 내지 제14항 중 어느 하나의 항에 있어서, 처치에 사용하기 위한 겔 조성물.
- 제1항 내지 제14항 중 어느 하나의 항에 있어서, 예방 또는 처치에 의한 코피 치료에 사용하기 위한 겔 조성물.
- (a) 카르복시 폴리메틸렌 중합체;(b) 글리신; 및(c) 칼슘 이온 공급원을 포함하는 무수 조성물.
- 예방 또는 처치에 의한 코피 치료용 약제의 제조에 있어서의, 제1항 내지 제19항 중 어느 하나의 항의 조성물의 용도.
- 실질적으로 실시예와 관련하여 본원에 개시된 바와 같은 겔 조성물.
- 실질적으로 실시예와 관련하여 본원에 개시된, 겔 조성물의 제조 방법.
