Abstract
본 발명은 종양 괴사 인자-관련된 아폽토시스 유도 리간드(TRIAL)를 발현하는 종양-조직지향성 세포(tumor-homing cell), 특히, TRIAL을 코딩하는 아데노바이러스 벡터로 형질도입된 CD34+와 NK 세포에 관계한다. 본 발명의 이들 세포는 종양, 특히 림프종의 정맥내 치료에 유용하다.
종양-조직지향성 세포(tumor-homing cell)
Description
본 발명은 종양 괴사 인자-관련된 아폽토시스 유도 리간드(tumor necrosis factor-related apoptosis inducing ligand, TRAIL)를 발현하는 종양-조직지향성 세포(tumor-homing cell) 및 항-종양 요법에서 이들의 용도에 관계한다.
제어되지 않은 아폽토시스 기전은 림프증식성 질환(lymphoproliferative disorder)의 병인과 진행에서 핵심적인 역할을 수행하고, 신생물 세포가 정상적인 수명을 초과하여 생존하고 궁극적으로 화학-방사선내성(chemo-radioresistance)을 획득할 수 있도록 한다[1]. 따라서, 아폽토시스 경로는 악성 세포의 아폽토시스 감수성(apoptosis sensitivity)을 회복시키거나, 또는 아폽토시스의 항진제를 활성화시키기 위한 매력적인 치료 표적을 대표한다[2]. 아폽토시스 유전자와 단백질을 조절하기 위하여, 아폽토시스의 내인성(Bcl-2, Bcl-XL), 외인성(DR4, DR5, FLIP), 또는 수렴(cIAP2) 경로를 표적하는 여러 전략이 고려될 수 있다[3].
종양 괴사 인자-관련된 아폽토시스-유도 리간드(TRAIL)는 사멸 수용체 리간드(death receptor ligand)의 TNF 계통에 속한다[4].
TRAIL을 코딩하는 분리된 DNA 서열, 상기 DNA 서열을 포함하는 발현 벡터, 재조합 TRAIL 폴리펩티드는 WO 97/01633에서 기술한다.
TRAIL은 사멸 수용체 4(DR4)와 사멸 수용체 5(DR5)에 결합하는데, 이들 수용체는 여러 암 세포의 세포 표면에서 발현된다[5]. DR4 또는 DR5에 가용성 TRAIL의 결합은 카스파제 활성화(caspase activation)와 아폽토시스를 유도한다[6].
Liu Zhongyu et al., J Clin Invest. Vol 112, no.9, 9.11.2003, 1332-1341에서는 재조합 아데노바이러스에 의해 TRIAL로 형질도입된 수상돌기 세포 및 관절염의 치료에서 잠재적인 의학적 용도를 기술한다. 종양 치료를 위한 이들 세포의 잠재적 용도에 관해서는 전혀 언급되지 않았다.
시험관내와 생체내 연구에서는 높은 암세포-특이성과 강력한 항종양 활성으로 인하여, 가용성 재조합 TRAIL이 항암 치료에서 핵심적인 역할을 수행할 수 있음을 암시한다[7]. 실제로, TRAIL은 정상 세포에는 해를 입히지 않으면서 여러 형질전환된 세포에서 아폽토시스를 선택적으로 유도한다[4]. 부가적으로, 생쥐에서 TRAIL 투여는 결장 암종[8, 9], 유방암[10], 다발성 골수종[11] 또는 신경교종[12, 13]의 종양 이종이식편에 대하여 상당한 효능을 발휘한다.
생쥐와 비-인간 영장류에서 수행된 독성 연구에서, 정상 간세포와 각화세포는 TRAIL의 삼량체 형태에 저항하지만[14, 15], 비-최적화된 TRAIL 제조물 또는 상기 리간드의 항체-가교결합된 변이체에 의한 아폽토시스 유도에 현저한 감수성을 보이는 것으로 밝혀졌다[16]. TRAIL의 삼량체 형태를 고용량으로 사용하는 경우에 간 독성(hepatic toxicity)이 중요한 문제가 될 수 있는 지의 여부는 여전히 논쟁의 여지가 있다.
가용성 TRAIL의 이용에서 다른 단점은 아폽토시스가 진행되도록 하기 위하여 연장된 배양 시간 또는 가용성 TRAIL의 매우 높은 용량을 필요로 하는 상당한 수의 다양한 종양 세포주에서 관찰되는 손상된 아폽토시스 반응으로 대표된다[17, 18]. 정맥내 주입이후 TRAIL의 약물동태학 프로필(32분의 혈장 반감기(plasmatic half-life)와 4.8시간의 제거 반감기(elimination half-life))을 고려하면, 연장된 노출 시간과 높은 농도의 조건은 임의의 생체내 치료 전략에는 적합하지 않다[10]. 가용성 재조합 TRAIL과 관련된 단점을 극복하고 종양 세포를 특이적으로 표적하기 위하여, 상이한 동물 모형을 이용한 여러 아데노바이러스-매개된 TRAIL 유전자 전달 방법이 현재 개발되고 있다[19, 20].
하지만, 유효한 전략으로서 유전자 전달 방법은 종양의 효율적인 감염 및 면역 제거(immune clearance)의 회피에 거의 의존한다[21]. 이에 더하여, 아데노바이러스 벡터의 전신 감염과 관련된 여러 안정성 문제가 여전히 해결되어야 한다[22]. 가령, TRAIL의 아데노바이러스 유전자 전달은 간암 세포의 손상된 아폽토시스 반응을 극복하긴 하지만, 일차 인간 간세포(primary human hepatocyte)에서 심각한 아폽토시스를 유발한다[19]. 현재, 아데노바이러스-기초된 유전자 치료법은 TRAIL-인코딩 아데노바이러스의 종양내 전달에 거의 의존한다[23]. 국지적인 항종양 활성에도 불구하고, TRAIL의 종양내 전달은 전신 항종양 활성이 없기 때문에, 임상적 가 치가 부족하다.
다양한 세포 유형이 종양을 선호적으로 지향하고[24], 항종양 분자를 생산하는데 필요한 구조체로 적하될 수 있는 것으로 알려져 있다. CD34+ 세포[25]와 자연 킬러(NK) 세포[26]는 조직지향 특성(homing characteristics)으로 인하여, 항암 분자용 전달 운반체(delivery vehicle)로서 이용될 수 있다.
정맥내 주입이후, CD34+ 세포는 골수에 영구적으로 콜로니를 형성하고 간과 비장에 일시적으로 콜로니를 형성하는 능력을 비롯한 특이적인 조직지향 특성을 보인다[27-30]. 이들 조직지향 특성은 골수 미세환경(bone marrow microenvironment)과 종양 미세환경(tumor microenvironment) 내에 존재하는 기질 세포(stromal cell)에서 발현되는 특정 리간드(가령, SDF-1, VCAM-1 등)와 상호작용하는 유착 수용체(adhesion receptor)(가령, CXCR-4, VLA-4, VLA-5, CD44 등)의 발현과 엄밀하게 관련된다[31-34].
NK 세포는 주요 조직접합성 복합체(major histocompatibility complex) 비-제한된 기전을 통한 종양 세포에 대한 세포-기초된 면역 방어(cellular-based immune defense)에서 필수적인 역할을 수행하는 림프구의 하위집합이다[35]. 정맥내 주입이후, NK 세포는 골수와 림프 기관을 지향하고 적절한 사이토킨 신호의 영향아래 종양 부위에서 일혈(extravasation)된다. 이런 이유로, 많은 연구 그룹에서 치료 목적으로 종양을 지향하는데 NK 세포의 활용을 모색하였다[36-38].
NK-세포의 종양-조직지향 특성이 알려져 있긴 하지만, 유전적으로 변형된 세포는 형질도입된 유전자의 종류에 따른 스트레스를 받게 된다. 이러한 스트레스의 결과로써, 이들 세포는 TRIAL 유전자로 형질도입되는 경우에 최초의 종양-조직지향 특성을 상실하게 된다. 이런 이유로, 성공의 당연한 기대가 보장되지 않으며, 고려된 특정 방법이 실행 가능한 지를 확증하기 위하여 실제적인 실험 검사가 필요하다.
본 발명에서는 TRAIL를 발현하도록 조작된 CD34+ 세포와 NK 세포가 생체내에서 세포-매개된 항-종양 활성을 달성하는데 유익하게 이용될 수 있음을 확인하였다.
본 발명에 따라, 종양-조직지향성 세포는 아데노바이러스-매개된 유전자 전달에 의해 종양 괴사 인자-관련된 아폽토시스-유도 리간드(TRAIL)를 생산하도록 조작된다. 본 발명에 따른 세포는 상기한 단점을 유발하지 않으면서 종양 부위로 TRAIL을 전달하기 위하여 전신 투여될 수 있다.
따라서, 본 발명은 TRAIL을 종양-조직지향성 세포 및 이들을 함유하는 세포 제조물에 관계한다. 본 발명은 또한, 항암 치료, 특히, 인간 림프종의 치료를 위한 세포 조성물의 제조에서 이들 세포의 용도에 관계한다.
본 발명의 세포는 적절한 프로모터, 예를 들면, CMV 프로모터의 조절하에, 인간 TRAIL 유전자를 인코딩하는 복제-결함아데노바이러스(Ad-TRAIL)로 종양-조직지향성 세포를 형질도입함으로써 수득될 수 있다. 이런 형질도입은 분자 생물학자에게 공지된 방법, 바람직하게는, 실시예에 기술된 절차에 따라 수행될 수 있다.
본 명세서에서, “종양-조직지향성(tumor-homing)” 세포는 (1) 정맥내 주입이후 종양 조직을 지향하고, (2) 적어도 수일간 적당한 양의 막-결합된 TRAIL(mTRAIL)을 발현할 수 있는 세포를 의미한다.
혈액으로부터 종양 침전물(tumor deposit)로 지향할 수 있는 종양-조직지향성 세포의 실례는 조혈 세포(hematopoietic cell), 다시 말하면, 성장-인자 치료된 암 환자의 말초혈로부터 CD34+ 조혈 세포; 상기 말초혈로부터 NK 세포; 말초혈 단핵 세포의 체외 배양(ex vivo culture)을 통하여 수득된 사이토킨-유도된 킬러 세포(CIK); 내피 세포와 내피 전구 세포; 종양-침윤 림프구(TIL); 림포카인-활성화된 킬러 세포(LAK); 대식세포; 중간엽 줄기 세포(MSC)(말초혈이나 인간 조직으로부터 새로이 분리되거나, 또는 체외에서 증식된); 종양 추적 능력(tumor seeking ability)을 유지하고 mTRAIL을 발현하도록 조작된 인간 세포주 등이다. mTRAIL의 적절한 발현을 달성하기 위하여, 적절한 조절 유전자 요소, 예를 들면, 조직-특이적 프로모터 및/또는 인헨서를 보유하는 플라스미드와 바이러스 벡터의 이용을 비롯한 여러 방법이 고려될 수 있다.
CD34+와 NK 세포가 특히 선호된다.
NK 세포의 최적 형질도입 효율은 N-아세틸시스테인(10 mM)과 함께 18시간동안 NK 세포를 사전-배양함으로써 달성될 수 있다. 사전-배양이후, NK 세포는 37℃에서 혈청-없는 조건하에 단계적(50 내지 500) 감염 다중도(Multiplicity of Infection, MOI)의 Ad-TRAIL(50 내지 500)에 노출시킨다. 이후, 혈청-보충된 배지(RPMI-1640/FBS20%)를 첨가하고, 수시간후 배양액을 GeneBooster(1:200)로 보충하고 18시간동안 추가로 배양하였다.
N-아세틸시스테인과의 사전-배양을 제외하고, CD34+ 세포에 대하여 유사한 조건이 이용될 수 있다.
본 발명의 조성물은 REMINGTON’S PHARMACEUTICAL SCIENCES (Mack Pub. Co., N.J. 1991)에 보고된 바와 같이, 비경구, 특히, 정맥내 투여에 적합한 운반제를 이용함으로써 제조될 수 있다.
가령, 사용될 수 있는 부형제에는 보조제, 예를 들면, 적심제 또는 유화제, 완충제 등과 선택적으로 혼합되고 조성물을 섭취하는 개체에 무해한 임의의 약제, 예를 들면, 물, 염수, 글리세롤, 에탄올 등이 포함된다. 적절한 용량은 특히, 성별, 연령, 치료되는 개체의 상태, 질병의 심각도에 좌우된다. 적절한 효과량은 숙련된 전문의에 의해 편의하게 결정될 수 있고, 임상적 연구에 의해 결정될 수도 있다. 치료 효과량은 일반적으로, 대략 103 내지 1015개의 형질도입된 세포이다. 다른 용량은 통상적인 실험에 의해, 다시 말하면, 용량-반응 곡선(dose-response curve)에 의존하거나, 최대 허용 용량(maximal tolerable dose, MTD)을 결정하거나, 또는 한정된 수의 환자에서 임상 I기 시험을 수행함으로써 확립될 수 있다. 투약 치료(dosage treatment)는 단일 투약 또는 복수 투약 일정으로 달성될 수 있다. 더 나아가, 개체는 적절한 시점에 여러 번 투약될 수 있다. 숙련된 전문의는 가장 효과적인 투약 섭생(dosage regimen)을 용이하게 결정할 수 있다.
본 발명은 아래의 사실을 뒷받침하는 확실하고 명백한 증거를 제공하는 아래의 실시예에 의해 더욱 구체적으로 예시된다:
(i) CD34+ 줄기 세포와 NK 세포는 TRAIL을 발현하는 아데노벡터로 시험관내에서 효율적으로 형질도입될 수 있다;
(ii) 형질도입이후, 이들 세포 아형은 그들의 세포 표면에서 TRAIL을 일시적으로 발현한다;
(iii) TRAIL 발현의 이런 짧은 지속 기간은 형질도입된 세포와 동시-배양된 TRAIL-감수성 세포와 TRAIL-저항성 종양 세포 모두에서 검출되는 강력한 아폽토시스 효과와 연관한다;
(iv) 초기 또는 진행된 단계 종양을 보유하는 NOD/SCID에서 형질도입된 세포의 생체내 주입은 생쥐 생존의 현저한 연장과 연관하는데, 이는 아데노바이러스 형질도입된 CD34+ 세포와 NK 세포 모두 치료 목적의 전신 TRAIL 전달용 운반체로서 효율적으로 기능할 수 있음을 암시한다.
정맥내 투여의 효능은 종양내 전달(intra-tumor delivery)에 기초한 프로토콜에 비하여, 본 발명의 분명한 이점을 공여한다.
mTRAIL-발현 인간 세포의 시험관내 항종양 효과의 기전은 mTRAIL과 종양 세포에서 이의 동족 수용체(cognate receptor) 사이의 직접적인 상호작용에 의해 유인된 아폽토시스일 가능성이 가장 높지만, 이들의 생체내 항종양 활성의 기전은 알려져 있지 않다. 종양 세포의 직접적인 사멸은 아마도, TRAIL 수용체를 발현하고 종양으로 둘러싸인 비-종양 세포(종양 발생과 종양 세포 생존에서 중요한 역할을 수행하는 혈관, 혈관주위, 세포간 및 다른 세포 유형)의 아폽토시스에 기인한 간접적인 성장-저해 효과와 결부된다. 본 발명이 관찰된 활성의 실제 기전에 대한 임의의 가설에 의해 한정되진 않지만, 진행되는 실험은 생체내에서 관찰된 항종양 활성 기전의 더욱 명확한 이해를 목표로 한다.

실시예 1 - 아데노바이러스-매개된 유전자 전달이후 TRAIL을 발현하는 CD34+ 세포 또는 NK 세포와 동시-배양된 림프종 세포주에서 아폽토시스의 시험관내 유인
아데노바이러스-매개된 유전자 전달이후 TRAIL를 발현하도록 조작된 CD34+ 세포 또는 NK 세포와 림프종 세포주의 동시-배양(co-culturing)을 평가하였다.
최초 시험관내 실험에서, CD34+ 세포와 NK 세포의 아데노바이러스 감염을 위한 최적 조건을 설정하기 위하여, TRAIL을 발현하는 아데노바이러스 벡터(Ad-TRAIL)를 이용하였다. 이후, PE-공액된 항-TRAIL 단클론 항체(클론 Rik-2)를 이용함으로써, CD34+와 NK 세포 표면에서 TRAIL 발현의 시간-코스(time-course)를 모니터하였다. 최종적으로, Ad-TRAIL/CD34+ 세포 또는 Ad-TRAIL/NK 세포 및 림프종 세포주를 동시-배양함으로써, 림프종 세포주의 아폽토시스를 유도하는 막-결합된 TRAIL의 능력을 평가하였다.
방법
인간 TRAIL 유전자를 인코딩하는 아데노바이러스. CMV 프로모터로부터 발현된 인간 TRAIL 유전자를 인코딩하는 복제-결함 아데노바이러스(Ad-TRAIL)가 이들 실험에 이용되었다[39]. 상기 Ad-TRAIL은 pAd5CMVK-NpA의 XhoI와 NotI 부위에 클론된 인간 TRAIL의 전체 코딩 서열을 보유한다. 결과의 플라스미드 및 E1 유전자가 결실된 아데노바이러스 골격 서열(Ad5)은 인간 태아 신장 293 세포에 형질감염시키고, 바이러스 입자는 TRAIL 발현의 분석을 위하여 분리하고 증폭한다. 재조합 아데노바이러스는 A549 플라크 검사(plaque assay)로 복제 콤피던트 바이러스를 스크리닝하고, 바이러스 역가(virus titer)는 293 세포에서 플라크 검사로 결정한다. 정제된 바이러스는 이용 때까지 -80℃에서, 3% 수크로오스를 포함하는 PBS에 담아 보관한다. TRAIL 유전자 산물은 형질도입된 세포의 세포 표면에서 발현되고, 유세포분석법(flow cytometry)에 의해 검출될 수 있다.
CD34+ 세포의 아데노바이러스 형질도입. Ad-TRAIL로 형질도입되는 CD34+ 세포는 화학치료를 받고 조혈 성장 인자(hematopoietic growth factor)로 치료된 암 환자의 동의를 얻어 수집한 말초혈로부터 획득하였다. CD34+ 세포는 면역자기 기술(immunomagnetic technique)과 양성 선별(positive selection)(Miltenyi Biotech)로 류커퍼레틱(leukapheretic) 샘플로부터 농후하게 하였다. 아데노바이러스 형질도입을 위하여, CD34+ 세포는 35 ㎜ 페트리 접시에서, 100 내지 1,000 범위의 최종 감염 다중도(MOI)를 허용하는 Ad-TRAIL 원액의 적절한 희석액을 포함하는 1 ㎖ 혈청-없는 배지(IMDM)에 2 x 106/㎖로 도말하였다. 2시간동안 배양(37℃, 5% CO2)이후, 배양액은 1 ㎖ IMDM/FBS20%로 보충하고, 4시간후 GeneBooster(1:200)를 첨가하였다. 세포는 18시간동안 추가로 배양하고, 이후 혈청-함유 배지에서 완전하게 세척(3x)하며, 최종적으로, PE-공액된 항-TRAIL 항체를 이용한 직적접인 면역형광으로 형질도입 효율을 평가하였다.
NK 세포의 아데노바이러스 형질도입. Ad-TRAIL로 형질도입되는 NK 세포는 건강한 지원자의 동의를 얻어 수집한 말초혈로부터 획득하였다. NK 세포는 면역자기 기술과 양성 선별(Miltenyi Biotech)로 농후하게 하였다. 형질도입에 앞서, NK 세포는 10% FBS와 N-아세틸시스테인(10 mM)으로 보충된 RPMI-1640에서 배양(18 시간, 37℃)하였다. 아데노바이러스 형질도입을 위하여, NK 세포는 35 ㎜ 페트리 접시에서, 50 내지 500 범위의 최종 감염 다중도(MOI)를 허용하는 Ad-TRAIL 원액의 적절한 희석액을 포함하는 1 ㎖ 혈청-없는 배지(RPMI-1640)에 2 x 106/㎖로 도말하였다. 2시간동안 배양(37℃, 5% CO2)이후, 배양액은 1 ㎖ RPMI-1640/FBS20%로 보충하고, 4시간후 GeneBooster(1:200)를 첨가하였다. 세포는 18시간동안 추가로 배양하고, 이후 혈청-함유 배지에서 완전하게 세척(3x)하며, 최종적으로, PE-공액된 항-TRAIL 항체를 이용한 직적접인 면역형광으로 형질도입 효율을 평가하였다.
동시-배양. CD34+ 세포 또는 NK 세포의 막에서 발현된 TRAIL의 아폽토시스 유인 활성은 동시-배양 실험으로 시험관내에서 검사하였다. 간단히 말하면, Ad- TRAIL/CD34+ 세포 또는 Ad-TRAIL/NK 세포(효과기 세포)를 림프종 세포(표적 세포)와 동시-배양하였다. 동시-배양의 개시이후 24시와 48시간 시점에, 아폽토시스 유도를 평가하였다. 이들 실험에서, 효과기 세포 : 표적 세포 비율은 효과기 세포의 형질도입 효율에 기초하여 산정하였다.
결과
CD34+ 세포의 아데노바이러스 형질도입. 예비 실험에서, CD34+ 세포의 최적 형질도입 효율은 혈청-없는 조건(37℃)하에 2시간 동안 CD34+ 세포(2 x 106/㎖)를 단계적 MOI의 Ad-TRAIL(범위, 100 내지 1,000)에 노출시킴으로써 지속적으로 달성되는 것으로 밝혀졌다. 이후, 동등 부피의 혈청-보충된 배지(IMDM/FBS 20%)를 첨가하고, 4시간후 배양액을 GeneBooster(1:200)로 보충하고 18시간동안 추가로 배양하였다. 최종적으로, 이들 세포는 혈청-함유 배지에서 완전하게 세척(3x)하고, PE-공액된 항-TRAIL 항체를 이용한 직적접인 면역형광으로 유전자도입 발현(transgene expression)을 평가하였다.
대조 세포에서는 배경 TRAIL 신호가 검출되지 않는 반면, Ad-노출된 세포는 일정 비율의 TRAIL-양성 CD34+ 세포를 나타냈는데, 상기 비율은 MOI-의존성 방식으로 증가하였다. 대표 실험에서, TRAIL-양성 CD34+ 세포의 비율은 각각, 100과 1000의 MOI에서 25%와 84%이었다.
앞서 기술된 감염 프로토콜은 TRAIL을 발현하는 CD34+ 세포의 83± 8%(범위 70 - 95%, n = 5)의 평균(± SD)으로, 1,000의 MOI에서 최대 유전자 전달 효율을 지속적으로 유도하였다. Trypan blue 염료 배제 검사로 평가된 세포 생존능(cell viability)은 최대 1,000까지 MOI에 의한 영향을 받지 않았다(≥85% 생존 세포).
유전자도입 발현의 시간-과정을 평가하기 위하여, CD34+ 세포는 1,000 MOI로 형질도입하고, 형질도입이후 7일까지 TRAIL 발현을 분석하였다. 이들 실험에서, CD34+ 세포가 생존할 수 있도록, 배양액을 저용량(3 ng/㎖)의 과립구 콜로니-촉진 인자(granulocyte colony-stimulating factor, G-CSF)로 보충하였다. 표 1에 도시된 바와 같이, 상당한 비율의 CD34+ 세포(25%)가 형질도입이후 최대 120시간까지 TRAIL를 지속적으로 발현하였다.

NK 세포의 아데노바이러스 형질도입. 예비 실험에서, NK 세포의 최적 형질도입 효율은 N-아세틸시스테인(10 mM)과 함께 18시간동안 NK 세포를 사전-배양함으로써 지속적으로 달성되는 것으로 밝혀졌다. 사전-배양이후, NK 세포(2 x 106/㎖)는 혈청-없는 조건(37℃)하에 2시간 동안 단계적 MOI의 Ad-TRAIL(범위, 50 내지 500)에 노출시켰다. 이후, 동등 부피의 혈청-보충된 배지(RPMI-1640/FBS 20%)를 첨가하고, 4시간후 배양액을 GeneBooster(1:200)로 보충하고 18시간동안 추가로 배양하였다. 최종적으로, 이들 세포는 혈청-함유 배지에서 완전하게 세척(3x)하고, PE-공액된 항-TRAIL 항체를 이용한 직적접인 면역형광으로 유전자도입 발현(transgene expression)을 평가하였다.
Ad-노출된 세포는 일정 비율의 TRAIL-양성 NK 세포를 나타냈는데, 상기 비율은 MOI-의존성 방식으로 증가하였다. 대표 실험에서, TRAIL-양성 NK 세포의 비율은 각각, 50, 100, 500의 MOI에서 30%, 45%, 46%이었다.
앞서 기술된 감염 프로토콜은 TRAIL을 발현하는 NK 세포의 61± 18%(범위 45 - 84%, n = 4)의 평균(± SD)으로, 100의 MOI에서 최대 유전자 전달 효율을 지속적으로 유도하였다. Trypan blue 염료 배제 검사로 평가된 세포 생존능(cell viability)은 최대 100까지 MOI에 의한 영향을 받지 않았다(≥85% 생존 세포).
유전자도입 발현의 시간-과정을 평가하기 위하여, NK 세포는 100 MOI로 형질도입하고, 형질도입이후 7일까지 TRAIL 발현을 분석하였다. 표 2에 도시된 바와 같이, 형질도입된 NK 세포(25%)는 형질도입이후 최대 168시간까지 TRAIL를 지속적으로 발현하였다.

동시-배양. 최종적으로, Ad-TRAIL/CD34+ 세포 또는 Ad-TRAIL/NK 세포(효과기 세포) 및 종양 세포주(표적 세포)를 동시-배양함으로써, 림프성 세포주에서 막-결합된 TRAIL의 아폽토시스 유인 능력을 평가하였다. 이들 실험을 위하여, 가용성 TRAIL에 대한 감수성에 따라 2가지 세포주를 선별하였다. 특히, TRAIL-감수성 KMS-11 세포주와 TRAIL-저항성 JVM-2 세포주가 이용되었다.
앞서 기술된 감염 프로토콜에 따라 최적 조건하에 형질도입된 CD34+ 세포 또는 NK 세포는 아데노바이러스에 대한 최초 노출이후 24시간 시점에 수집하고 완전하게 세척하며 KMS-11 또는 JVM-2 세포와 동시-배양하였다.
Ad-TRAIL/CD34+ 세포를 0.8:1의 효과기 : 표적 세포 비율로 동시-배양하는 경우에, 24-시간 동시-배양이후 KMS-11 세포에서 상당한 비율(81%)의 아폽토시스 세포와 괴사 세포가 검출되었다. 아폽토시스 세포의 양은 48-시간 동시-배양이후 더욱 증가하였다(93% 아폽토시스 세포).
Ad-TRAIL/NK 세포를 0.6:1의 효과기 : 표적 세포 비율로 동시-배양하는 경우에, 24-시간 동시-배양이후 KMS-11 세포에서 상당한 비율(83%)의 아폽토시스 세포와 괴사 세포가 검출되었다. 아폽토시스 세포의 양은 48-시간 동시-배양이후 더욱 증가하였다(85% 아폽토시스 세포).
TRAIL-감수성 KMS-11 세포주에서, 막-결합된 TRAIL에 의해 발휘되는 아폽토시스 효과의 효능은 고용량(100 ng/㎖)의 가용성 TRAIL에 24-시간 내지 48-시간 노출에 의해 발휘되는 효능과 유사하였다(표 3). 하지만, 정맥내 주입이후 TRAIL의 약물동태학 프로필(32분의 혈장 반감기(plasmatic half-life)와 4.8시간의 제거 반감기(elimination half-life))을 고려하면, 100 ng/㎖에서 연속 48-시간 또는 24-시간 노출은 생체내에서 달성될 수 없다.

또한, Ad-TRAIL/CD34+ 세포 또는 Ad-TRAIL/NK 세포를 가용성 TRAIL-저항성 JVM-2 세포주와 함께 배양함으로써 동시-배양 실험을 수행하였다.
Ad-TRAIL/CD34+ 세포를 0.8:1의 효과기 : 표적 세포 비율로 동시-배양하는 경우에, 48-시간 동시-배양이후 TRAIL-저항성 JVM-2 세포에서 상당한 비율(51%)의 아폽토시스 세포와 괴사 세포가 검출되었다.
Ad-TRAIL/NK 세포를 0.6:1의 효과기 : 표적 세포 비율로 동시-배양하는 경우에, 48-시간 동시-배양이후 TRAIL-저항성 JVM-2 세포에서 상당한 비율(53%)의 아폽토시스 세포와 괴사 세포가 검출되었다.
TRAIL-저항성 JVM-2 세포주에서, 막-결합된 TRAIL에 의해 발휘되는 아폽토시스 효과의 효능은 고용량(100 ng/㎖)의 가용성 TRAIL에 48-시간 노출에 의해 발휘되는 효능보다 현저하게 높았다(표 4).

표 5에 도시된 바와 같이, Ad-TRAIL/CD34+ 세포는 0.4:1의 효과기 : 표적 비율의 동시-배양 설정에서, 상당한 비율(66%)의 KMS-11 세포에서 아폽토시스 또는 괴사를 유인하였다.

Ad-TRAIL/CD34+ 세포 또는 Ad-TRAIL/NK 세포의 치료 잠재력을 조사하기 위하여, NOD/SCID 생쥐에 TRAIL-감수성 KMS-11 다발성 골수종 세포주를 재주입하였다. 이후, 이들 생쥐는 Ad-TRAIL/CD34+ 세포 또는 Ad-TRAIL/NK 세포를 주입하고, 생쥐 생존은 세포-기초된 TRAIL 전달의 항종양 효능의 정보(readout)로서 이용하였다.
방법
Ad-TRAIL/CD34+ 세포 또는 Ad-TRAIL/NK 세포의 항종양 활성의 평가. 20 내지 25 g의 체중을 보유하는 6주 내지 8주령 암컷 NOD/SCID 생쥐를 Charles River(Milano, Italy)로부터 구입하였다. 이들 생쥐는 임상시험 가이드라인(institutional guideline)에 따른 표준 실험실 조건하에 사육하였다. 이들 동물에서 수행된 실험 절차는 Ethical Committee for Animal Experimentation of the Istituto Nazionale Tumori의 승인을 받았고, United Kingdom Coordinating Committee on Cancer Research의 가이드라인에 따라 실시되었다.
생쥐는 KMS-11 세포(0.5 x 106/생쥐)를 정맥내(IV) 주입하였다. Ad-TRAIL/CD34+ 세포 또는 Ad-TRAIL/NK 세포(1 x 106/생쥐)로 처리는 종양 세포 접종이후 7일 또는 17일 시점부터 시작하여 4주간 IV 주1회 주입으로 구성되었다. 재주입된 Ad-TRAIL/CD34+ 세포 또는 Ad-TRAIL/NK 세포의 평균 형질도입 효율은 각각, 83± 8%와 61± 18%이었다. 따라서, 각 생쥐는 4회 주입동안 평균적으로, 3.32 x 106개의 TRAIL-발현 CD34+ 세포와 2.44 x 106개의 TRAIL-발현 NK 세포가 주입되었다. 생쥐는 종양 외관(tumor appearance), 체중 척도(body weight measurement), 독성(toxicity)을 주2회 검사하였다. 각 군에서 동물의 생존 시간을 기록하고, 통계학적 분석으로 군간 차이를 평가하였다. 적절한 대조군은 (i) 종양 세포가 단독으로 주입된 생쥐, (ii) 종양 세포 + 비-형질도입된 CD34+ 세포 또는 NK 세포가 주입된 생쥐이었다.
결과
Ad-TRAIL/CD34+ 세포. KMS-11 세포(0.5 x 106/생쥐)로 이종이식된 생쥐는 2가지 상이한 일정, 다시 말하면, KMS-11 세포주 주입이후 7일부터 시작된 초기 단계 종양의 치료 및 17일에 시작된 진행된 단계 종양의 치료에 따라, 주1회 Ad-TRAIL/CD34+ 세포의 4회 IV 주입으로 치료하였다. 재주입된 Ad-TRAIL/CD34+ 세포의 평균 형질도입 효율이 83± 8%(평균± SD, n = 9)이기 때문에, 각 생쥐는 4회 주입동안 평균적으로, 3.32 x 106개의 TRAIL-발현 CD34+ 세포가 주입되었다.
도 1에 도시된 바와 같이, KMS-11 세포가 단독으로 주입된 NOD/SCID 생쥐의 평균 생존(median survival)은 56일이었다. 야생형 CD34+ 세포의 주입은 평균 생존(56일)에 어떤 영향도 주지 않는 반면, 초기 단계 종양에서 Ad-TRAIL/CD34+ 세포로 치료된 NOD/SCID 생쥐는 111일의 평균 생존을 보였다(P ≤ 0.0001).
Ad-TRAIL/CD34+ 세포로 진행된 단계 종양을 치료하는 효과는 도 2에 도시한다. KMS-11 세포가 단독으로 주입된 NOD/SCID 생쥐의 평균 생존은 56일이었다. 야생형 CD34+ 세포의 주입은 평균 생존(56일)에 어떤 영향도 주지 않는 반면, 진행된 단계 종양에서 Ad-TRAIL/CD34+ 세포로 치료는 평균 생존의 현저한 증가와 연관하였다(98일, P ≤ 0.007).
Ad-TRAIL/NK 세포. KMS-11 세포(0.5 x 106/생쥐)로 이종이식된 생쥐는 주1회 Ad-TRAIL/NK 세포의 4회 IV 주입으로 치료하였다. NK 세포의 재주입은 종양 성장의 초기 단계에서, KMS-11 세포 주입이후 7일 시점에 시작하였다. 이 실험에서, 재주입된 Ad-TRAIL/NK 세포의 평균 형질도입 효율은 61± 18%이었다. 따라서, 각 생쥐는 4회 주입동안 평균적으로, 2.44 x 106개의 TRAIL-발현 NK 세포가 주입되었다.
도 3에 도시된 바와 같이, KMS-11 세포가 단독으로 주입된 NOD/SCID 생쥐의 평균 생존(median survival)은 56일이었다. 야생형 NK 세포의 주입은 평균 생존(50일)에 어떤 영향도 주지 않는 반면, 초기 단계 종양에서 Ad-TRAIL/NK 세포로 치료된 평균 생존의 현저한 증가와 연관하였다(74일, P ≤ 0.03).
실시예 3 - 유방암 세포주에 대한 CD34/Ad-TRAIL 세포의 시험관내 항종양 활성
상당한 비율의 유방암 세포주가 TRAIL 미끼 수용체(decoy receptor)에 의한 TRAIL 격리(sequestering), R1과 R2 수용체의 발현 상실, FLIP 과다발현 등을 비롯한 다수의 기전에 기인한 TRAIL-유도된 아폽토시스에 저항한다[40]. sTRAIL-저항성 림프종 세포주가 mTRAIL에 노출되는 경우에, 실제로 TRAIL 반응성으로 변함을 입증하는 기존의 결과에 기초하여, sTRAIL과 mTRAIL에 대한 2가지 유방암 세포주의 감수성(sensitivity)을 조사하였다.
sTRAIL의 사멸 효과(killing effect)에 대한 유방암 세포주의 감수성은 CD34/Ad-TRAIL 세포와 동시-배양된 sTRAIL-감수성과 sTRAIL-저항성 유방암 세포주의 아폽토시스의 시험관내 유인의 평가와 비교하여 평가하였다.
2가지 유방암 세포주, 다시 말하면, MCF-7과 MDA-MB-361이 이용되었다. sTRAIL의 사멸 효과에 대한 개별 세포주의 감수성을 평가하기 위하여, 종양 세포 (5 10 x 104/㎖)를 저용량(10 ng/㎖)과 고용량(100 ng/㎖)의 sTRAIL에 72시간동안 노출시켰다. 이후, 안넥신 V/요오드화 프로피디엄 이중 염색(annexin-V/propidium iodide double staining)으로 아폽토시스를 평가하였다.
방법
세포주. 유방암 세포주 MCF-7과 MDA-MB-361은 각각, DSMZ(Braunschweig, Germany, EU)와 ATCC(Manassas, VA, USA)로부터 구입하였다. 이들 세포는 중합효소 연쇄 반응으로, 마이코플라즈마 오염을 주기적으로 검사하였다. 모든 시험관내 실험은 기하급수적으로 성장하는 세포로 수행하였다.
안넥신-V 발현. 안넥신 V-FITC 검사(PharMingen)를 이용하여 조기 또는 후기 아폽토시스와 괴사가 진행되는 세포의 비율을 정량적으로 결정하였다. 간단히 말하면, 분석되는 세포는 차가운 PBS로 2회 세척하고, 이후 결합 완충액(10 nM HEPES, 140 nM NaCl, 5 nM CaCl2, pH 7.4)에 재부유시켰다. 배양이후, 0.1 ㎖의 세포 현탁액을 5 ㎖ 배양 튜브로 이전하고 5 ㎕의 안넥신 V-FITC와 10 ㎕의 요오드화 프로피디엄을 첨가하였다. 교반이후, 이들 샘플은 어두운 상태로 실온에서 15분간 배양하였다. 배양의 종결 시점에서, 0.4 ㎖의 결합 완충액을 첨가하고 유세포분석법으로 이들 세포를 즉시 분석하였다.
결과
sTRAIL과의 배양이 아폽토시스 유인과 연관하는 지를 조사하기 위하여, MCF-7과 MDA-MB-361 세포주를 sTRAIL(10 100 ng/㎖, 72시간)에 노출시키고, 이후 FACS 분석으로 아폽토시스 세포와 괴사 세포의 비율을 결정하였다. 표 7에 도시된 바와 같이, sTRAIL에 대한 노출은 MCF-7 세포에서 아폽토시스 반응을 유도하는데 실패한 반면, 72시간동안 100 ng/㎖의 sTRAIL에 노출된 MDA-MB-361 세포는 현저한 세포 사멸 반응(cell death response)을 보였다. 이들 결과에 따라, MCF-7은 sTRAIL-저항성 세포주이고, MDA-MB-361은 sTRAIL-감수성 세포주이다.

이후, 아데노바이러스-매개된 유전자 전달이후 TRAIL을 발현하는 CD34+ 세포(CD34/Ad-TRAIL)와 동시-배양된 유방암 세포주에서 아폽토시스의 시험관내 유인을 평가하였다.
방법
인간 TRAIL 유전자를 인코딩하는 아데노바이러스. CMV 프로모터로부터 발현된 인간 TRAIL 유전자를 인코딩하는 복제-결함 아데노바이러스(Ad-TRAIL)가 이들 실험에 이용되었다[39]. 상기 Ad-TRAIL은 pAd5CMVK-NpA의 XhoI와 NotI 부위에 클론된 인간 TRAIL의 전체 코딩 서열을 보유한다. 결과의 플라스미드 및 E1 유전자가 결실된 아데노바이러스 골격 서열(Ad5)은 인간 태아 신장 293 세포에 형질감염시키고, 바이러스 입자는 TRAIL 발현의 분석을 위하여 분리하고 증폭한다. 재조합 아데노바이러스는 A549 플라크 검사(plaque assay)로 복제 콤피던트 바이러스를 스크리닝하고, 바이러스 역가(virus titer)는 293 세포에서 플라크 검사로 결정한다. 정제된 바이러스는 이용 때까지 -80℃에서, 3% 수크로오스를 포함하는 PBS에 담아 보관한다. TRAIL 유전자 산물은 형질도입된 세포의 세포 표면에서 발현되고, 유세포분석법(flow cytometry)에 의해 검출될 수 있다.
CD34+ 세포의 아데노바이러스 형질도입. Ad-TRAIL로 형질도입되는 CD34+ 세포는 화학치료를 받고 조혈 성장 인자(hematopoietic growth factor)로 치료된 암 환자의 동의를 얻어 수집한 말초혈로부터 획득하였다. CD34+ 세포는 면역자기 기술과 양성 선별(Miltenyi Biotech)로 류커퍼레틱(leukapheretic) 샘플로부터 농후하게 하였다. 아데노바이러스 형질도입을 위하여, CD34+ 세포는 35 ㎜ 페트리 접시에서, 500의 감염 다중도(MOI)를 허용하는 Ad-TRAIL 원액의 적절한 희석액을 포함하는 1 ㎖ 혈청-없는 배지(IMDM)에 2 x 106/㎖로 도말하였다. 2시간동안 배양(37℃, 5% CO2)이후, 배양액은 1 ㎖ IMDM/FBS20%로 보충하고, 4시간후 GeneBooster(1:200)를 첨가하였다. 세포는 18시간동안 추가로 배양하고, 이후 혈청-함유 배지에서 완전하게 세척(3x)하고, 최종적으로, PE-공액된 항-TRAIL 항체를 이용한 직적접인 면역형광으로 형질도입 효율을 평가하였다.
동시-배양. mTRAIL의 아폽토시스 유인 활성은 동시-배양 실험으로 시험관내에서 검사하였다. 간단히 말하면, CD34/Ad-TRAIL 세포(효과기 세포)를 유방암 세포(표적 세포)와 동시-배양하였다. 동시-배양의 개시이후 48시간 시점에, 아폽토시스 유도를 평가하였다. 이들 실험에서, 1:1과 4:1 효과기 세포 : 표적 세포 비율이 이용되었다.
결과
CD34+ 세포의 아데노바이러스 형질도입. CD34+ 세포의 최적 형질도입 효율은 혈청-없는 조건(37℃)하에 2시간 동안 CD34+ 세포(2 x 106/㎖)를 단계적 MOI의 Ad-TRAIL(500)에 노출시킴으로써 지속적으로 달성되었다. 대조 세포에서는 배경 TRAIL 신호가 검출되지 않는 반면, TRAIL-발현 세포는 93± 8%(평균± SD)의 TRAIL-양성 CD34+ 세포를 나타냈다. Trypan blue 염료 배제 검사로 평가된 세포 생존능(cell viability)은 최대 1,000까지 MOI에 의한 영향을 받지 않았다(≥85% 생존 세포).
동시-배양. CD34/Ad-TRAIL 세포(효과기 세포) 및 종양 세포(표적 세포)를 동시-배양함으로써, 유방암 세포주에서 mTRAIL의 아폽토시스 유인 능력을 평가하였다. CD34/Ad-TRAIL 세포는 아데노바이러스에 최초 노출이후 24시간 시점에 수집하고 완전하게 세척하며 MCF-7 또는 MDA-MB-361 세포와 동시-배양하였다. 이들 실험에서, 2가지 효과기 : 표적 세포 비율, 다시 말하면, 1:1과 4:1이 이용되었다. 결과는 표 8에 요약한다.

48시간동안 CD34/Ad-TRAIL 세포와 sTRAIL-감수성 MDA-MB-361 세포를 1:1 E:T 비율(세포 사멸 = 28%)과 4:1 E:T 비율(세포 사멸 = 50%)에서 동시-배양함으로써 상당한 비율의 세포 사멸이 검출되었다.
sTRAIL-감수성 MDA-MB-361 세포주에서, mTRAIL에 48-시간 노출로 발휘되는 아폽토시스 효과의 효능은 고용량(100 ng/㎖)의 sTRAIL에 72-시간 노출에 의해 발휘되는 효능과 유사하였다(표 7).
CD34/Ad-TRAIL의 세포독성 효과(cytotoxic effect)가 실제로, mTRAIL에 기인하는 지를 검증하기 위하여, MDA-MB-361 세포와 모의-형질도입된(mock-transduced) CD34+ 세포를 동시-배양하였다. 표 2에 도시된 바와 같이, MDA-MB-361 세포와 모의-형질도입된 CD34+ 세포의 동시-배양은 최대 E:T 비율에서만 검출되는 온건한 세포 사멸 효과와 연관하였다. 이런 온건한 세포 사멸 유도는 배양 과밀(culture overcrowding)과 관련된 것으로 생각된다.
CD34/Ad-TRAIL의 세포 사멸 활성이 Ad-TRAIL로 CD34 감염의 종결 시점에서 세척되지 않은 유리 아데노바이러스 입자에 기인할 가능성을 배제하기 위하여, MDA-MB-361 세포를 106 플라크 형성 단위(plaque forming unit, pfu)에 노출시켰다. 106개의 바이러스 입자에 대한 MDA-MB-361 세포의 노출에 의한 세포 독성의 증거는 관찰되지 않았다(표 8).
pfu의 총수는 아래와 같이 산정되었다.
500 MOI에서 5 x 105개의 CD34+ 세포를 감염시키기 위하여, 250 x 106 pfu를 1 ㎖ CD34+ 세포 현탁액에 첨가하였다.
감염의 종결 시점에서, CD34+ 세포는 배양 배지에서 3x 세척하였다. 각 세척 절차에서, 1:20 희석비(dilution factor)를 이용하였다, 다시 말하면, 현탁 배양액은 적어도 8,000-배 희석되었다.
세척 절차의 종결 시점에서 현탁 배양액에 250 x 106 pfu가 여전히 존재한다고 가정하고, 이들을 8,000-배 희석하였다, 다시 말하면, 동시-배양중에 <50,000 잔류 pfu가 추가될 것으로 예상되었다.
따라서, 동시-배양액에서 검출되는 세포 사멸 활성이 세척되지 않은 유리 아데노바이러스 입자에 기인하는 가능성을 배제하기 위하여, 유방암 세포주를 잔류 pfu의 이론적 수치(#3) X 20(즉, 50,000 x 20 = 106 pfu)에 필적하는 pfu에 노출시켰다.
이후, CD34/Ad-TRAIL 세포와 sTRAIL-저항성 MCF-7 세포주를 동시-배양함으로써 동시-배양 실험을 수행하였다. 이들 실험에서, 48시간동안 CD34/Ad-TRAIL 세포와 sTRAIL-저항성 MCF-7 세포를 4:1 E:T 비율(세포 사멸 = 64%)에서 동시-배양함으로써 상당한 비율의 세포 사멸이 검출된 반면, 1:1 E:T 비율에서는 세포 사멸이 검출되지 않았다.
sTRAIL-저항성 MCF-7 세포주에서, mTRAIL에 48-시간 노출로 발휘되는 아폽토시스 효과의 효능은 고용량(100 ng/㎖)의 sTRAIL에 72-시간 노출에 의해 발휘되는 효능보다 현저하게 높았다(표 7 & 8).
CD34/Ad-TRAIL의 세포독성 효과가 실제로, mTRAIL에 기인하는 지를 검증하기 위하여, MCF-7 세포와 모의-형질도입된 CD34+ 세포를 동시-배양하였다. 표 8에 도시된 바와 같이, MCF-7 세포와 모의-형질도입된 CD34+ 세포의 동시-배양은 배양 과밀(culture overcrowding)과 관련된 것으로 생각되는 온건한 세포 사멸 효과와 연관하였다.
CD34/Ad-TRAIL의 세포 사멸 활성이 Ad-TRAIL에 의한 CD34 감염의 종결 시점에서 세척되지 않은 유리 아데노바이러스 입자에 기인할 가능성을 배제하기 위하여, MCF-7 세포를 106 pfu에 노출시켰다. 106 pfu에 대한 MCF-7 세포의 노출에 의한 세포 독성의 증거는 관찰되지 않았다(표 8).

- 종양 괴사 인자-관련된 아폽토시스 유도 리간드(tumor necrosis factor-related apoptosis inducing ligand)를 발현하는 종양-조직지향성 세포(tumor-homing cell)에 있어서, 상기 종양-조직지향성 세포는 수상돌기 세포(dendritic cell)가 아닌 것을 특징으로 하는 종양-조직지향성 세포.
- 제 1항에 있어서, 종양 괴사 인자-관련된 아폽토시스 유도 리간드를 코딩하는 아데노바이러스 벡터로 종양-조직지향성 세포를 형질도입함으로써 수득되는 것을 특징으로 하는 종양-조직지향성 세포.
- 제 1항 또는 2항에 있어서, 성장-인자 치료된 암 환자의 말초혈로부터 CD34+ 조혈 세포; 말초혈로부터 NK 세포; 말초혈 단핵 세포의 탈체 배양(ex vivo culture)을 통하여 수득된 사이토킨-유도된 킬러 세포(CIK); 내피 세포와 내피 전구 세포; 종양-침윤 림프구(TIL); 림포카인-활성화된 킬러 세포(LAK); 대식세포; 중간엽 줄기 세포(mesenchymal stem cell, MSC)에서 선택되는 것을 특징으로 하는 종양-조직지향성 세포.
