Abstract

Description

바람직한 치료 조성물의 종양내 투여는 1형 항종양 면역의 발생 및 종양 면역기피 메커니즘의 하향조절을 위한 잠재적 보조 효과를 제공할 수 있다. 조성물의 보조 효과는 세포들의 세 가지 주요 특징을 기초로 한다: (1) 많은 양의 1형 사이토카인을 생산하는 능력; (2) CD40L의 표면 발현; 및 (3)세포들의 공용 본질. 이종(xeno-), 공동(allo-), 또는 바이러스 항원과 같은, 외부 항원들은 또한 잠재적 보조 효과를 제공할 수 있다.

종양괴사인자-알파의 중요성은 이러한 사이토카인의 주입만으로 특정한 확립된 동물 종양을 치료하기에 충분하다는 증거에 의해 설명될 수 있다. 종양괴사인자-알파는 1형 사이토카인 군의 일부이며 세포사멸(apoptosis)을 유도함으로써 암 세포들을 효과적으로 파괴할 수 있는 리간드들이다. 인터페론-감마 및 종양괴사인자-알파는 항종양 효과기 세포들 상에 보조 효과를 가질 뿐만 아니라, 종양 세포사멸을 직접 유도할 수 있다.

경피(percutaneous) 냉동절제술 또는 알코올 절제술(뚜렷한 병변의 절제에 최적)을 보여주는 이미지의 최소 침습 기법(minimally-invasive technique)이 사용된다. 제거에 적격인 종양 병변들은 예를 들면, 간, 피부, 머리/목, 림프절, 췌장, 골, 부신, 방광, 위장관(G1 tract) 또는 신장에 잔류할 수 있으며 필요할 때 전산화 단층촬영 또는 초음파 영상 이용을 사용하여 안전한 경피 액세스를 허용하는 이러한 조직 내의 위치에 위치될 수 있다.

실시 예들
환자
적어도 1 라운드(one round)의 화학치료에 난치성의 진행중인 전이성 암(단계 Ⅳ)을 갖는 환자가 연구에 참가할 자격을 주었다. 각각의 환자의 임상 단계는 완전한 병력, 신체 검사, 완전 혈구 측정, 임상 화학, 및 흉부, 복부와 골반의 전산화 단층촬영을 사용하여 평가되었다. 골 전이의 이력을 갖는 몇몇 환자들에 전산화 단층촬영/양전자 방사 단층 촬영(PET) 스캔이 또한 수행되었다. 모든 환자에 대한 임상 단계는 개정된 미국 공동 위원회(American Joint Committee) 시스템을 기초로 하여 결정되었다.

제외 기준은 계획된 치료를 받는데 손상을 줄 수 있는 이미 앓고 있는 질병, 동종 골수/줄기 세포 또는 고형 장기(solid organ) 이식, 연구 약물 치료의 첫째 날의 30일 내에 코티코스테로이드 작용제(corticosteroid agent)의 생리적 용량(프레드니손(prednisone)의 〉5 ㎎/일과 같은 용량)보다 큰 만성적 사용, 수반되는 활성 자기면역 질병(예를 들면, 류마티스 관절염, 다발성 경화증(multiple sclerosis), 자기면역 갑상선 질환, 포도막염), 이전의 실험적 암 백신 치료(예를 들면, 수지상 세포 치료, 열 충격 백신), 연구 개시 3달 내에 사이클로스포린(cyclosporine), 항흉선세포 글로불린(antithymocyte globulin), 또는 타크로리무스(tacrolimus)를 포함하는 면역억제 치료, 수혈 반응 이력, 진행성 박테리아 또는 바이러스 감염, 대증적 유형의 심장병 또는 심장 박출 계수(cardiac ejection fraction)〈 45%, 대증적 폐질환(symptomatic pulmonary disease) 또는 1초간 노력성 호기량(FEV1), 노력성 폐활량(FVC) 및 일산화탄소 확산능(DLCO)≤예측치의 50%, 인간 면역결핍 바이러스(HIV) 양성 또는 에이즈(AIDS)의 이력(B형 간염 바이러스 및/또는 C형 간염 바이러스 양성은 허용되었다)이다. 대부분의 환자들은 증식 시기에 부적절한 칼로리 및 액체 섭취를 하였으며 이러한 이류로 제외되지 않았다.
42명의 환자가 평가되었다. 40%의 남성과 60%의 여성으로 평균 연령은 60.2세(50-89세의 범위)였다. 환자들은 평균 2.7 화학치료 라인 및 라인당 평균 7 과정으로 사전에 심하게 치료되었다. 45%가 이전에 화학치료를 받았으며 90%가 이전에 종양 병변의 외과 적출을 받았다. 환자들은 또한 환자당 평균 22 전이성 병변을 갖는 높은 종양 부담을 가졌다. 가장 일반적인 징후는 유방암(42%)이었으며 그 다음으로 대장암(19%)이었으며 또한 난소, 육종, 편평 상피암(squamous cell carcinoma). 폐, 방광/수뇨관, 췌장, 흑색종(melanoma) 및 식도 전이암을 포함한다.

)
CD3/CD28이 코팅된 마이크로비드와 결합된 동종 헬퍼 T1 세포들을 포함하는 약의 피내 주사가 1×107 내지 4×107 세포 사이의 용량에서 투여되었다. 세포들은 ㎖당 1×107 세포의 밀도에서 플라즈마라이트 A(PlasmaLyte A) 및 인혈청 알부민(human serum albumin)을 포함하는 완충액 제제(formulation buffer)에 침전되었다. 1회 및 4회 사이에서 1㎖ 주사가 서로 다른 위치(상박(uper arm), 허벅다리 및 복부)에서 한번에 투여되었다. 피내 주사는 매 2일마다 또는 매 9일마다의 빈도로 투여되었는데, 바람직하게는 주사는 최소 3주 동안 매주 마다 투여되었다.
종양내

약의 종양내 주사는 절제 한 시간 내에, 정제된 종양의 괴저 중심에 발생하는데, 절제 후 일주일 내일 수도 있다. 만일 다중 종양이 존재하면, 하나의 종양만이 제거되었다. 일부 케이스에서 제거 수술이 반복되었다.
정맥 내, 복강 내, 흉막 내, 정맥 내, 경막 외 투여
1×107 내지 1×109 세포 범위의 용량에서, 1×108 세포의 일반적인 용량으로, 약의 정맥 내, 복강 내, 흉막 내, 정맥 내 투여가 실행되었다. 복강 내 약의 투여는 암종증(carcinomatosis) 및 악성 복수(ascites)를 치료하기 위하여 사용될 수 있다. 유사하게, 흉막 내 투여는 악성 흉막 삼출(pleural effusion)을 치료할 수 있으며 경막 외 투여는 뇌척수강(cerebral-spinal space) 내의 악성종양을 치료할 수 있다. 이러한 투여는 종양이 완전히 근절될 때까지 필요에 딸 반복되었다.
프로토콜의 첫 번째 단계는 "프라이밍(priming)" 단계라 불린다. 프라이밍 단계는 1×107부터 4×107 세포 범위의 용량에서 세 번 이상의 약의 피내 주사로 구성된다. 적어도 2일 이상의 간격, 바람직하게는 8일 이하의 간격을 두고 투여되었다. 부작용을 위하여 주사 후 적어도 30분 동안 환자들을 관찰하였다.
방법의 두 번째 단계는 "인시튜 백신접종(in-situ vaccination)"으로 불린다. 이러한 단계는 프라이밍 단계의 완료 후 2일 및 8일 사이에 수행된다. 이러한 과정은 약의 1×107 내지 6×107 세포 용량의 종양 내 주사에 의해 한 시간 후 내에 선택된 종양 병변의 제거를 포함한다. 대안으로서, 악성 복수(malignant ascites)를 갖는 환자들은 종양 냉동제거술과 함께 또는 종양 냉동제거술 없이 복강내 주입을 받을 수 있고 뚜렷한 병변을 갖는 환자들은 냉동제거술과 함께 또는 종양 냉동제거술 없이 알코올 절제술을 받을 수 있다. 복막 암종증(peritoneal carcinomatosis)을 갖는 환자들은 복강내로 1×108 내지 1×109 세포의 바람직한 약제 용량이 전달되었다.
냉동절제술을 위하여 사용된 방법은 CryoCare-28 경피 프로브 시스템(엔도케어(Endocare)사, 미국 캘리포니아)이다. 이러한 시스템은 폐쇄 시스템에서 저온프로브(cryoprobe)의 끝을 냉각하기 위하여 줄-톰슨 효과(Joule-Thomson effect)를 사용한다. 노즐의 가스 계수 및 크기에 따라, 서로 다른 가스 성분들이 노즐에 가까운 영역에서 서로 다른 열교환 이벤트를 발생시킨다. 냉각(-187℃)을 위하여 아르곤 가스가 사용되고, 가열(67℃)을 위하여 헬륨이 사용된다.
필요할 때, 계획된 타겟 종양 병변이 확인되고 전산화 단층촬영 이미지 가이드 하에 위치되었다. 멸균 부위가 생성되고 국부 마취가 계획된 프로브(probe) 삽입 위치에 투여되었다. 가이드 프로브가 경피적으로 삽입되었고 전산화 단층촬영에 의해 타겟 종양 병변 내에 존재하는지가 확인되었다. 1 또는 2회의 동결-융해 사이클(freeze-thaw cycle)이 실행되었다. 타겟 병변의 크기에 따라 2㎜ 또는 5㎜의 단일 프로브가 사용되었다. 동결 시간은 전산화 단층촬영 상에 볼 수 있는, "아이스-볼(ice-ball)"의 달성에 따라 대략 15-20분이었다. 융해는 두 번째 냉동 과정(사용될 때)이 개시되기 전의 냉동 시간과 동일한 기간 동안 헬륨의 주입에 의해 달성된다. 과정은 단지 종양 병변 샘플의 제거만을 필요로 하며, 종양이 없는 주변부 내의 완전한 종양 제거를 필요로 하지는 않는다.
절제된 병변은 바람직한 약의 주사 전에 냉동 사이클 후에 약 10분 내지 한 시간 동안 냉각하도록 허용되었다.
방법의 마지막 단계는 면역 자극 단계로 냉동절제술(cryoablation)과 같은 날 내지 냉동절제술 과정 후 8일 내에 수행되었다. 이러한 단계는 1×107부터 1×108 세포 범위에서의 용량에서 2일 이상의 간격으로 투여되는 약의 1회 또는 그 이상의 정맥 내 주사로 구성된다. 대부분의 환자들은 추가 주사로서 매달 Ⅳ 투여를 받았다.
반응
본 발명의 방법에 의해 치료된 환자들은 마지막 치료로부터 약 30일 후에 전산화 단층촬영에 의해 평가되었다. 정맥 조영제(contrast)가 없는 전산화 단층촬영 상에서, 종양은 일반적으로 중간 밀도이다. 종양, 혈관, 근육, 및 림프절은 모두 동일한 밀도를 가질 것이다. 요오드 함유 조영제(iodinated contrast medium)의 정맥내(Ⅳ) 투여 후에, 종양은 다양하게 개선된다: 혈관성인, 부신경졸종(paraganglioma)이 매우 개선되었으며, 반면에 더 세포성인, 편평 상피암(squamous cell carcinoma)은 상당히, 또는 거의 또는 전혀 개선되지 않을 것이다. 괴사 반점(foci) 또는 이전 출혈은 전선화 단층촬영 상에서 어둡게(저밀도) 나타난다. 혈액 공급의 부족, 괴사 반점은 조영제 투여 후에 개선되지 않는다.
성공적인 치료상에서, 30일에서의 전산화 단층촬영 스캔은 베이스라인과 비교하여 저밀도된(어두운) 모든 종양 병변의 팽창(swelling, 크기의 증가)을 나타내었다. 전산화 단층촬영상의 커다란 종양의 출현은 동종의 저밀도 시스트(cyst) 또는 중앙의 괴사 및 생존가능한 앞의 가장자리 부위를 갖는 진행 종양과 대조적으로 이질성의(heterogenous) 저밀도의 점들로 얼룩덜룩하게 나타난다. 저밀도의 이질성 출현은 종양이 액화되었다는 것을 나타낸다.
결과:
도 1은 2009년 6월 간에서 전이성 질병으로 나타나고 폴폭스(FOLFOX)와 폴피리(FOLFIRI) 화학치료 및 아바스틴(avastin)을 갖는 폴피리의 라인으로 치료된 난 89명의 전이성 결장암 환자의 관상면(coronal view)을 도시한다. 2010년 6월 진행중이고 화학치료에 난치성이었으며 2010년 9월에 11의 전이성 병변으로 나타났다. 환자들은 여기에 설명된 약의 1×107의 피내 용량으로 매주마다 3회 시술되었으며, 그리고나서 1주일 후에 간 전이 중 하나의 냉동절제술 과정 및 21일째에 종양 내 바람직한 약 투여 및 28일째에 1×109 세포의 정맥 내 투여가 수행되었다.
도 1은 간에서 전이 병변의 선택된 조각의 베이스라인 출현을 도시한다. 60일 후에 종양은 커지고 더 고밀도화되며, 액화 반응과 일치한다. 90일째에 종양은 커다란 크기를 유지하나, 아마 물 재흡수에 의해 고밀도를 잃는다. 그리고 나서 환자는 95일째에 추가 4 투여가 실행되고 120일째에 또 다른 전산화 단층촬영을 얻는다. 이미지는 증가된 크기뿐만 아니라 고밀도 복귀를 나타낸다. 이는 실제로 단지 진행중인 종양이 아니라 액화라는 것을 나타내기 위하여, 병리학자에 의해 종양이 생체검사되었으며 평가되었다. 도 2에 도시된 것과 같이 생체검사는 응고성 괴사 및 섬유증(fibrosis)의 넓은 부위가 면역 매개 종양 액화와 일치한다는 것을 나타낸다.
비록 본 발명이 바람직한 예들을 참조하여 설명되었으나. 통상의 지식을 가진 자들은 본 발명의 정신 및 범위를 벗어나지 않고 형식과 내용에서의 변경이 만들어질 수 있다는 것을 인정할 것이다.
Claims (46)
- 항-CD3/항-CD28 단일클론 항체가 코팅된 마이크로비드와 결합된 동종 헬퍼 T1 세포를 포함하는, 화학 요법에서 진행성 전이 단계 IV 암 고형물의 융해에 사용하기 위한 종양 치료 조성물에 있어서,
상기 조성물은 다음의 형태로 환자에게 투여되어, 치료 전 기준선과 비교하여 30일째 CT스캔에서 저밀도되거나(hypodense) 또는 어두운 종양의 크기가 증가하도록 하는 것을 특징으로 하는 종양 치료 조성물:
a) 2일 이상 8일 이내의 간격으로 1×107 내지 4×107 세포 용량으로 3회 이상의 피 내 주사;
b) 단계 a)의 완료 후 2일 내지 8일 사이, 종양의 절제 1시간 이내에 1×107 내지 6×107 세포의 종양 내 주사; 및
c) 절제 8일 이내에, 2일 이상의 간격으로 1×107 내지 1×108 세포 용량으로 1회 이상의 정맥 내 주사.
