Abstract
상피성 종양 질환의 치유적 치료에서 화학요법 약물에 대한 내성을 감소시키기 위한 약제의 제조를 위한, EphA1, EphA2, EphA8, EphB2, CSF1R, VEGFR2, RAMP2, RAMP3, CLRN1, MAPK4, PIK3C2A, PIK3CG, GSK3alpha, GSK3beta, IRAK3, DAPK1, JAK1, PIM1, TRB3, BTG1, LATS1, LIMK2, MYLK, PAK1, PAK2, CDC2, BTK, PNRC2, NCOA4, NR2C1, TPR, RBBP8, TRPC7, FXYD1, ERN1, PRSS16, RPS3, CCL23 및 SERPINE1로 구성된 군으로부터 선택된 하나 이상의 유전자를 기능적으로 차단할 수 있는 화합물의 용도가 개시된다. 또한, 종양 세포에서 약물 내성을 결정하는 방법, 및 종양 줄기 세포를 확인하는 방법이 개시된다.

Description
본 발명은 주요 청구항의 전제부에 기재된 특징에 따라, 인간 상피성 종양 세포(epithelial tumor cell)에서 약물 내성을 조절, 특히, 감소시킬 수 있는 약제의 제조를 위한 화합물의 용도에 관한 것이다. 본 발명은 또한, 종양 세포에서 약물 내성을 결정하는 방법 및 종양 줄기 세포(tumor stem cell)를 식별하는 방법에 관한 것이다.
종양성 질환(neoplastic pathology)의 치료에서 가장 일반적으로 따르는 전략 중 하나는 종양 세포에서 아폽토시스(apoptosis)의 자연적인 과정을 유도하기 위해, 종양 세포의 DNA를 손상시킬 수 있는 약물(화학요법)의 이용을 고려한다.
그럼에도 불구하고, 종양 세포들은 예기치 못한 방식으로 반응하여, 반대로, 약물 요법에 대해 강한 내성을 보일 수 있는 것으로 알려져 있다. 또한, 종양 세포에 의해 보여진 약물 내성에 대한 주요 이유 중 하나는 상당한 DNA 손상의 존재 하에서도, 세포가 아폽토시스의 과정을 개시할 수 없기 때문인 것으로 알려져 있다.
이 현상은 더 이상 아폽토시스 과정을 개시할 수 없어서, 세포들이 약물 작 용에 내성을 갖게하는, 유전자 p53의 기능적 변형(돌연변이 또는 결실)에서 기인되는 것으로 규명되었다. 종양 세포의 약물 내성 수준은 매우 높을 수 있다. 예를 들면, 진행기(advanced phase)에 있는 결장-직장의 종양 세포의 경우, 5-플루오로우라실(5FU)에 기반한 약물 요법은 세포들의 10 내지 15%에 대해서만 유효한 반응을 보이고, 이리노테칸 및 옥살리플라틴과 같은 신규한 약물과 5FU의 조합은 세포 사망률의 40-50%까지의 증가를 가져오나, 이는 종양성 질환에 대한 효과적인 치료 작용에 대해 여전히 완전히 불만족스럽다.
최근년에, 유전자 발현이 특이적 방식으로 침묵화되는 것인 "RNA 간섭(RNA interference)"(RNAi)라고 불리는 세포 현상이 발견되었다. 그와 같은 과정을 이용하여, 미지의 기능을 갖는 유전자의 선택적 침묵화(selective silencing)를 수득할 수 있고, 따라서, 수득된 표현형의 연구를 통해 그의 특이적 기능을 정의할 수 있다. RNAi 기법을 적용하고, 표현형 결과를 연구하는 것에 의해, 이미 공지된 유전자에 새로운 기능을 부여하는 것도 가능하다.
종양 세포의 약물 내성의 현상에 관여된 유전자는 현재 거의 알려져 있지 않다.
따라서, 약물-내성에 관여하는 신규한 유전자를 식별(identify)하고 종양 세포의 약물에 대한 내성을 실질적으로 감소시킬 수 있는 방법 및 화합물을 설계하는 것에 대한 매우 큰 요구가 존재한다.
본 발명이 해결하고자 하는 근본적인 문제는 관련된 종양성 질환의 치료를 위한 약물의 제조를 가능하게 하기 위해, 상피성 종양 세포의 약물 내성을 감소시킬 수 있는 화합물을 이용가능하게 하는 것이다.
이 문제는 첨부된 청구항에서 확인된 그룹으로부터 선택된 하나 이상의 유전자를 기능적으로 차단할 수 있는 화합물의 이용에 의한 현재의 발견에 의해 해결된다.
본 발명의 제2 양태에서, 본 발명은 상피성 종양 세포주의 약물 내성을 측정하는 방법을 제공한다.
본 발명의 추가적인 양태에서, 본 발명은 또한 종양 줄기세포를 식별하는 방법을 제공한다.
상피성 종양 세포에서 약물 내성을 조절하는 유전자를 식별하는 방법 및 관련된 검증 테스트
전술된 기술적 문제는 여러 상피성 종양 세포주를 대상으로 목적 표현형 발현(phenotypic expression of interest), 즉, 화학요법 약물에 의해 유도되는 아폽토시스에 대한 내성의 역전(reversion)을 가져올 수 있는 유전자를 식별하고 규명하도록 준비된 일련의 테스트 및 분석을 적용하는 것에 의해 해결되었다.
그와 같은 확인(identification)은 RNAi에 의해 확장된 그룹의 유전자의 선택적 침묵화 후에 상피성 종양 세포들의 대표적인 세포주의 표현형 스크리닝에 의해 수행되었다.
이 복잡한 스크리닝 작업은 최근에 Bernards 연구소에 의해 제조된 레트로바이러스성 pRetroSuper(pRS) 벡터의 라이브러리 때문에 가능했다. 그와 같은 벡터들은 특정한 mRNA(messenger RNA)의 상응하는 단백질로의 번역 과정을 차단할 수 있는, 작은 간섭 RNA(small interference RNA), 줄여서 siRNA로 알려진 특정한 올리고뉴클레오티드 분자를 안정한 방식으로 발현할 수 있는 것으로 알려져 있다. 그와 같은 메카니즘은 요약하면, siRNA(또는 그의 단편)가 RISC 효소 복합체와 회합하여, 이를 활성화시켜, 상기 복합체가 그에 회합된 siRNA에 상보적인 mRNA를 인식하여 결합하고 이를 분해할 수 있다는 것을 예견한다. 상기 메카니즘에 따르면, siRNA에 의해 특이적 방식으로 확인된 mRNA가 상응하는 아미노산 사슬로 번역될 수 없어서, 상기 mRNA의 전사가 유래된 유전자의 침묵화가 수득된다. 간섭 과정은 특이적이어서, siRNA 분자는 정상적으로 하나의 mRNA만을 분해할 수 있고, 따라서, 하나의 단일 유전자를 침묵화시킬 수 있다. 반면에, 동일한 mRNA가 RISC를 통해 상이한 siRNA에 의해 분해될 수 있다.
이 메카니즘은 유전자를 기능적으로 차단할 수 있는 상이한 방식들 중 하나이다(기능적 넉아웃). 그러나, RISC를 통한 siRNA의 결과적인 분해는 단백질 수준에서 일시적인 효과만을 생성하며, 따라서, 그 결과가 장기적으로는 관련되지 않을 수 있는, 단기적 실험만을 가능하게 한다. 이 문제를 극복하기 위해, 헤어핀(hairpin)의 형태로 폴딩된 RNA의 서열인 짧은 헤어핀 RNA(short hairpin RNA, shRNA)가 기능적 넉아웃을 위한 메카니즘으로 이용되었다. 요약하면, shRNA를 세포 염색체 내로 도입하고 내포시키기 위해 벡터가 이용된다. DNA의 전사는 뒤이어 세포 기구(cellular machinery)인 DICER에 의해 siRNA로 절단되는 shRNA를 생성한다. 그 후, 상기 siRNA는 전술된 바와 같이 기능한다. shRNA 벡터의 세포 염색체로의 내포는 유전자 침묵화가 딸세포에 의해 유전되게 한다. 따라서, shRNA의 이용은 장기적인 실험을 위해 이용될 수 있는 세포의 기능적 넉아웃을 가능하게한다.
또한, 문헌에서 siRNA에 대한 형질감염의 효율은 100%일 수 없다는 것이 알려져 있다. 따라서, 형질감염된 siRNA의 이용이 모든 처리된 세포들에서 성공적으로 표적 단백질이 결핍되게 하는 것을 보장하는 것은 아니다. 선택성 마커의 부재는 또한 세포들의 균일한 집단으로 작업하는데 있어서 문제를 야기한다. 레트로바이러스 라이브러리와 함께, 푸로마이신 내성 유전자와 같은 선택성 마커를 이용하는 것은 shRNA를 갖는 세포만의 회수를 가능하게 한다. 또한, shRNA 발현 선택 후에, 그의 침묵화가 정상 세포 생존 및 증식과 양립할 수 있는 유전자들만이 선택된다. 이 유전자들의 부재는 정상적인 세포 생리에 영향을 미치지 않고, 항암 약물에 대한 반응에만 영향을 미치며, 이는 항암 요법 개발시 매우 중요한 고려사항이다.
그럼에도 불구하고, 일반적으로 예를 들면, 유전자의 mRNA로의 전사 단계, 또는, mRNA 형질도입(transduction)의 단계와 같은, 유전자의 단백질 코딩 과정의 단계에서 작용하는 것에 의해, 또는 상기 코딩 과정으로부터 초래된 단백질의 저해에 의해 동일한 효과가 달성될 수 있다는 것이 중요하다.
상기 문제의 해결에서 필수적이고 중요한 단계는 원인 유전자(responsible gene) 또는 원하는 표현형의 유전자의 확인이다.
Bernards 연구소에 의해 배열된 레트로바이러스 라이브러리는 인간 게놈의 약 8,300개의 유전자를 침묵시킬 수 있는 약 25,000개의 상이한 요소들로 구성되며, 이는 각 유전자당 약 3개의 상이한 벡터의 비율이다.
먼저 테스트는 인 비트로에서 수행되었고, 뒤이어 유전자 p53의 부재 또는 돌연변이를 가지며, 따라서 화학요법 약물에 대한 현저한 내성을 갖는 것을 특징으로 하는, 상이한 상피성 종양 세포주에서 엑스 비보(ex vivo)로 검증되었다.
상세하게, 침묵화를 위한 8,300개의 표적화된 유전자로 구성된 인간 RNAi 라이브러리(NKi library)가 구축되었다. 표적화된 유전자는 키나아제, 포스파타아제, 종양 유전자, 종양 억제자, 전사 인자, 형질전환, 전이, 세포 주기, 분화, 아폽토시스, 대사 및 동화 과정에 관여하는 유전자를 포함했다. 프로토콜은 선행 문헌(Berns et al., NATURE vol. 428, 25 March 2004)에 개시된 바와 유사하게 진행되었다. 상기 문헌의 내용은 참조에 의해 본 출원에 포함된다. 각각의 표적화된 유전자의 mRNA 서열은 UniGene으로부터 선택되었다. 서열들은 반복적 서열을 제거하기 위해 RepeatMasker를 이용하여 마스킹(mask)되고, 벡터 오염에 대한 마스킹을 위해, UniVec에 대해 NCBI BLAST로 검색되었다. 짧은 헤어핀 RNA(short hairpin RNA, shRNA)를 특정하는 총 약 25,000개의 59-mer에 대해, 각각의 표적화된 유전자를 침묵시키기 위한 3개의 상이한, 19개의 뉴클레오티드로 구성된 서열(19-mer)을 설계하였다. 상기 19-mer 서열은 Berns 공개문헌에 개시된 선택 기준을 이용하여 선택하였고, 상기 기준은, a) (조기(premature) 폴리머라아제 III 전사 종결 신호를 피하기 위해), 4개 이상의 연속된 T 또는 A 잔기의 스트레치(stretch)가 없고; b) 30-70%의 총 GC 함량을 가지며; c) 표적 유전자의 코딩 서열 내에 존재하고; d) G 또는 C 잔기로 개시되며(스트랜드 바이어스(strand bias)에 대한 최근에 확립된 규칙과 일치함); e) 5' 플랭킹(flanking) 서열 내의 AA 이량체(dimer) 다음에 개시되고; f) 3' 플랭킹 서열 내의 TT, TG 또는 GT 더블릿(doublet) 직전에 종료되며; g) 넉다운 카세트의 벡터 백본으로의 후속 셔틀링(shuttling)을 촉진하기 위해 XhoI 또는 EcoRI 제한효소 부위를 포함하지 않고; h) 다른 유전자와 최소한의 서열 동일성(sequence identity)을 공유하며; i) RefSeq mRNAs에 의해 대표되는 모든 전사체 변이를 표적화하고; 및 j) 동일한 표적 서열로부터 선택된 다른 19-mer와 중첩되지 않는 것이었다. 상기 59-mer 올리고뉴클레오티드는 19-mer 서열, 그의 상보적인 19-mer 서열, polIII 전사 개시 부위, polIII 종결 부위, 및 HindIII/BglII 클로닝 부위를 포함하도록 설계되었다. 실시예 1
처음에 사용된 종양 세포주는 결장 종양과 관련된 HCT116p53KO(유전자 p53의 부재 때문에, 야생형 wt HCT116과 상이함)였고, 특정한 유전자의 추가적인 상세한 연구는 DLD-1 및 SW480과 같은, 결장의 다른 약물-내성 종양 세포주, 및 폐와 난소의 다른 종양 세포주에서 수행하였다. 특히, 분석의 대상인 모든 세포주는 상피성 종양과 관련되었다.
예비적으로, 약물-유도 아폽토시스에 대한 내성을 확인하기 위해, 세포주 HCT116p53KO, DLD-1 및 SW480을 공통된 화학요법 약물로 처리하였다.
본 발명에 따라 사용가능한 화학요법 약물은 영향받은(affected) 종양 세포에서 아폽토시스 과정을 유도할 수 있는 임의의 종류, 예를 들면, 항대사산물, 또는 토포이소머라아제 I의 저해제, 토포이소머라아제 II의 저해제, 백금 배위 화합물(platinum coordination compound) 및 알킬화제를 포함한 DNA-손상 유발제(DNA-damaging agent)일 수 있다.
전술된 예비 테스트는 200 uM 5FU에서 72시간의 처리 후에, 95% 이상의 wt HCR116 세포 사망률 대비, 세포 사망률(cell mortality)이 10% 미만이라는 것을 보여주었다.
콜로니 형성 분석법(colony forming assay, CFA)의 보충 테스트는 또한 그와 같은 약물 내성이 비-일과성(non-transitory)이라는 것을 입증하였다.
화학요법 약물에 의해 유도된 아폽토시스에 대한 내성이 확인된 후, 200x106개의 HCT116p53KO 세포들을 Bernards 연구소에 의해 제공된, 전술된 pRetroSuper 라이브러리로 감염시켰다. 이 라이브러리의 각 벡터는 유리하게 푸로마이신 내성 유전자에 대한 선택 카세트를 갖추어서, 푸로마이신에 의한 처리(2일 동안 배양 배 지 중 2 mg/l)를 통해 상기 라이브러리의 벡터에 의해 실제로 감염된 HCT116p53KO 세포를 선택할 수 있었다.
항생제 처리의 완료 시, 여전히 살아있는 세포들을 회수하였고, 따라서, 이들은 그의 침묵화된 유전자들이 세포 생존과 양립할 수 있는 라이브러리의 레트로바이러스에 의해 감염된 모든 세포들을 포함했다.
상기에서 회수된 세포들을 72시간 동안 200 uM 5FU로 처리하고, 동시에, 대조군으로서, wt HCT116 세포들 및 비감염(uninfected) HCT116p53KO 세포들에도 동일한 처리를 수행하였다.
처리의 완료 시에, 세포들의 약 절반이 사망하여 배양 배지 중에 부유되었다. 그와 같은 세포들이 원하는(sought-after) 표현형을 나타내며, 따라서, 이들을 회수하고 레트로바이러스 라이브러리에 의해 침묵화된 유전자의 확인을 위해 필요한 처리를 수행하였다.
요약하면, 그와 같은 처리는 DNA 추출 및 특정한 프로모터의 영역 H1 및 목적 뉴클레오티드 서열을 코딩하는 인접 영역(adjacent region)을 포함하는 643 bp 영역의 PCR(Polymerase Chain Reaction)에 의한 증폭을 포함했다. PCR 증폭은 pRS-fw 프라이머: 5'-CCCTTGAACCTCCTC GTTCGACC-3' 및 pRS-rev 프라이머: 5'-AGACGTGCTACTTCCATTTGTC-3'를 이용하여 수행하였다. 상기 증폭에 의해 수득된 산물을 pRS 레트로바이러스 벡터에 결합시키고 상기 새로운 벡터에 의한 HCT115p53KO 세포의 감염 과정을 완전히 반복하여, 스크리닝을 정립(refine)하였다. 상기 산물을 EcoRI/XhoI로 처리하고 pRS에 재클로닝시켰다.
5FU에 의한 새로운 처리 후에 죽은 세포들로부터 수집된 DNA에 대한 PCR에 의한 제2 증폭 처리로부터 수득된 산물을 새로 분리하고 레트로바이러스 벡터 pRS에 결합시키고 DH5alpha 세균을 형질전환시키기 위해 이용하고, 상기 세균의 개별적인 플라스미드의 서열결정을 수행하여, 목적 표현형 발현을 가져오는 유전자를 확인하였다.
단일 유전자를 수득하고 RNA 간섭에 의한 침묵화가 5FU에 대해 테스트된 종양 세포의 내성의 역전을 가져오는 유전자를 확인한 후, 상기 단일 유전자들의 별개의 독립적인 검증을 수행하였다.
먼저, 선행 스크리닝에 의해 표시된 유전자들 중 하나를 특이적이고 안정한 방식으로 침묵시키고 약물에 대한 내성을 조절하는 능력을 검증하기 위해, 각각 미리 제조된 플라스미드 중 하나로 감염시킨, HCT116p53KO 종양 세포 시료에 대해 인 비트로에서 검증을 수행하였다. 그 후, 상기 시료를 푸로마이신으로 선택하고, 페트리 디쉬(Petri dish)에 50% 합류점(confluence)으로 배치하고 200 uM의 5FU로 12시간 동안 처리하였다.
약물 내성의 역전의 평가는 CFA에 의해 정의된 프로토콜에 따른 콜로니의 형성의 관찰과 wt HCT116 시료와 비감염 HCT116p53KO 시료와의 비교에 의해 수행하였다.

실시예 2

항대사산물 유형의 화학요법 약물의 패밀리의 대표적인 예인 5FU 외에, 옥살리플라틴과 같은, 다른 종류의 화학요법 약물에 대해 약물에 대한 내성의 역전도 테스트하였다.
도 4a에서, 옥살리플라틴(50 uM) 처리에 의해 유도된 죽은 세포의 분획이 각각 빈 벡터 및 유전자 GSK3beta를 침묵화시키는 벡터에 의해 감염된 결장 종양 세포주 DLD-1 및 SW480의 시료에 대해 보고된다.
GSK3beta가 침묵화된 시료에서 옥살리플라틴에 의해 유도된 아폽토시스에 대한 내성의 상당한 감소가 명백하다. 도 4b에서, 사용된 약물이 5FU와 옥살리플라틴의 조합인, 세포주 SW480에 대한 유사한 테스트의 결과가 표시된 그래프가 보고된다.
앞서 설명되고 도 2a 및 도 2b에 표시된 바와 같이, GSK3alpha 침묵화는 5FU 및 옥살리플라틴에 대한 아폽토시스 반응을 조절하는데 있어서 GSK3beta와 동일한 효과를 갖는다.

실시예 3
전술된 유전자 그룹의 또 다른 특히 대표적인 예는 여러 연구가 수행되었던 유전자 BTK로 구성된다.
BTK 키나아제는 B-세포 발달 및 분화를 위해 중요한 세포질의 단백질 티로신 키나아제이다. BTK 돌연변이는 실제로 성숙 B 세포의 부재 및 낮은 수준의 면역글로불린을 특징으로 하는 주요한 면역결핍증인, X-연관 무감마글루불린증(X-linked agammaglobulinemia, XLA)이다. B 세포에서, BTK는 아폽토시스-촉진(pro-apoptotic) 기능 또는 아폽토시스-저해(anti-apoptotic) 기능을 갖는 것으로 보고되었다. 또한, BTK는 현재까지 B 세포 및 비만 세포와 같은 일부 골수-유래 계통, 적혈구 전구세포(erythroid progenitor), 혈소판에서만 발현되는 것으로 간주되었다. 또한, BTK 저해가 옥살리플라틴(도 10a)에 대한 내성을 DLD-1(도 10b)에서 역전시킨다는 것을 확인하였다.
LFM-A13에 의한 유전자 BTK의 기능적 차단 후에 화학요법 약물에 대한 내성의 감소가 결장의 상피성 종양 세포주와 상이한 상피성 종양 세포주에 대해 인 비트로에서 수행된 테스트에 의해 더 확인되었다. 도 11에서, BTK의 수준이 여러 상이한 상피성 육종 세포주에서 웨스턴 블롯에 의해 조사되어, 상기 키나아제가 대부분에서 발현된다는 것을 보여주었다. 특히, 도 12a 및 도 12b에서, SKOV 세포주(난소암과 관련됨) 및 A549 세포주(폐암과 관련됨)에 대해 수득된 역전 테스트의 그래프가 보고되었다.
상세하게, 도 12a는 LMFA13의 이용을 통한 BTK 저해를 갖거나 또는 갖지 않는 내성 난소(resistant ovarian) (SKOV) 세포에서 200uM 5FU, 50uM OxPt, 및 상기 두 물질의 조합의 부재(기호 "NT" 및 "-") 및 존재시 세포 사망의 비율을 비교한다. 두 경우 모두에서, BTK 유전자의 기능적 저해제인 LFM-A13에 의한 세포주의 처리가 5FU 또는 옥살리플라틴 또는 상기 두 약물의 조합에 대한 노출 후에 세포 사망률의 상당한 증가를 가져왔다는 것이 주목된다.
또한, 이 세포주들에서 약물-내성의 감소 작용에 대한 높은 효과는 전술된 테스트에서 수득된 결과의 유효성이 적어도 폐 종양, 난소 종양 및 유방 종양과 같은, 모든 종류의 상피성 종양까지 확대될 수 있다는 것을 확인한다.
전술된 최적 결과는 상피성 인간 종양 시료에 대한 엑스-비보 분석을 통해서 유전자 BTK를 검증할 것을 시사했다.
먼저, 질병의 진행된(advanced) 단계에 있고 및/또는 화학요법 약물에 대한 내성을 갖는 환자로부터 유래된 난소 종양 세포의 시료의 30%에서 BTK 단백질 수준이 높다는 것을 웨스턴 블롯을 통해 확인하였다.
둘째, 조사된 유전자들이 또한 결장 종양 줄기세포에서도 발현되는지(및 어느 정도로 발현되는지)를 확인하기 위해 환자들로부터 분리된 결장 종양 줄기세포에 대해 조사를 수행하였다. 실제로, 최근의 연구(Dean et al., 2005)에 따르면, 이 줄기 세포들이 약물 내성에 대한 주요한 원인(responsible principal)일 것이다.
상이한 환자들로부터 분리되어, 분석된 4개의 모든 종양 줄기 세포주에서, 단백질 BTK의 발현은 매우 높았고, 기능성 실험에서 이용되었던 결장암 세포주에서 검출된 발현보다 4-5배 이상 높았으며, 이는 BTK 수준의 측정이 조사된 종양 세포의 줄기 세포 특성, 및 결과적으로 그들의 화학요법 약물에 대한 내성을 정의하는 방법으로서 유리하게 이용될 수 있다는 것을 시사한다.
결과는 BTK 수준이 종양 세포의 5FU에 대한 감수성(sensitivity)을 결정하고 BTK 저해는 약물-내성을 역전시킨다는 것을 보여주었다. 5FU-처리된 세포에서 항-시토크롬 C 면역염색(도 13)은 BTK가 저해되었을 때에만 5FU 처리 후 내성 세포에서 세포질내 축적을 보여주었고, 이는 BTK 저해 후 약물-내성의 역전은 아폽토시스의 활성화 때문이라는 것을 시사한다. 동일한 발견은 또한 BTK 저해제 LFM-A13의 존재 또는 부재시 HCT116p53KO 내성 세포에서 5FU 처리시 카스파아제 3/7 활성화의 수준을 평가하는, 도 14에 보고된 그래프에 의해 뒷받침된다. 세포의 수 당 RLU(RLU/number of cells)로 측정된, 카스파아제 3/7 활성화의 높은 수준은 BTK가 저해되었을 때만 5FU-처리된 HCT116p53KO 세포에서 관찰되었다.
BTK 단백질의 예측되고 보고된 분자량은 77 kDa이다. 대조적으로, HCT116p53KO 및 테스트된 모든 상피성 육종 세포주(6종의 유방 세포주, 3종의 난소 세포주, 7종의 폐 세포주, 5종의 결장 세포주)에서 웨스턴 블롯에서 특정한 항체(sc-1696, Santa Cruz Biotechnology)에 의해 BTK로 확인된 단백질은 약 65-68 kDa(도 11, 도 15a)의 겉보기 분자량을 가져서, 상피 세포주에서 보다 짧은 BTK 이소형이 발현된다는 것을 시사했다.
이 결과를 확인하기 위해, 두 개의 상이한 특이적 BTK 항체(전술된 sc-1696 및 영국 버밍햄 대학교(University of Birmingham)의 Mike Tomlinson 박사로부터 제공된 BL7)를 이용하여 면역침전 분석을 수행하였다. 도 15b의 결과는 이 신규한 이소형만 상피 세포주에서 발현되고, 전형적인 77 kDa 형을 발현하는 것으로 이미 문헌으로부터 공지된 백혈병 세포주(Nalm6)에는, 두 개의 이소형이 모두 존재한다 는 것을 보여주었다.
따라서, BTK 코딩 서열(cds)의 생물정보학적 분석(bioinformatic analysis)을 수행하였고, 따라서, BTK (nt 164-166)를 번역하기 위해 이용되는 것으로 알려진 것과 동일한 프레임 상에 있고(in frame), 단백질의 번역을 개시할 수 있는 제2의 뉴클레오티드 트리플렛 ATG (nt 428-430)를 확인하였다. 이 제2의 ATG로부터 개시되어 번역된 추정적인(putative) 단백질의 예상되는 분자량은 상피성 육종 세포에서 상이한 BTK 항체들에 의해 확인된 밴드의 겉보기 분자량과 일치하는 67 kDa이다.
BTK 단백질의 보다 짧고 신규한 이소형을 코딩하는 mRNA에서 결실된 부분을 확인하기 위해, 상이한 프라이머 쌍(도 16의 상단 도면에 표시된 바와 같이 cds의 상이한 부분에서 어닐링됨)을 이용한 PCR 실험을 수행하였다. 도 16에 표시된 바와 같이, 이 실험들은 HCT116p53KO 세포에서 202번 뉴클레오티드의 상류인 5' 말단이 결실되거나(absent) 또는 상이하다는 것을 보여주었다.
미지의 5' 말단의 정체(identity)를 결정하기 위해, HCT116p53KO 세포로부터의 mRNA에 대해 5'RACE/서열결정 실험을 수행하였다. 뒤이어, ClustaIW 컴퓨터 프로그램을 이용한, HCT116p53KO 세포로부터의 mRNA로부터 유래된 cDNA와 GenBank accession #NM_00061에 의해 확인된 표준 BTK mRNA로부터 유래된 cDNA 간의 정렬(alignment) 분석(도 17에 결과가 도시됨)은 제2 엑손(cds의 134번 뉴클레오티드로부터 개시됨)의 상류에 있는 서열이 문헌에서 보고된 서열과 상이하다는 것을 보여주었고, 즉, 상피성 결장 육종 세포는 상이한 제1 엑손을 발현한다는 것을 보여 주었다.
게놈 데이터베이스를 이용한 BLAST 분석(도 18)은 BTK 좌(locus)의 개시와 일치하는, 염색체 X contig 상의 101160K에 NM_000061 BTK 전사체의 제1 엑손을 위치시켰다. 변이에서, 신규한 BTK 전사체의 제1 엑손은 RPL36A 좌의 바로 하류에 존재한는, 제1의 공지된 BTK 엑손의 15192 bp 5'과 정렬되었고, 이는 현재까지 인식되지 않았던 BTK 엑손에 해당한다는 것을 시사한다. 또한, HCT116p53KO에서 제1 "전형적인(classical)" 엑손 대신 이 신규한 엑손이 존재하며, 이는 그 사용이 이 보다 짧은 BTK 이소형을 발현하는 세포에서 전사되는 상이한 BTK mRNA를 생성하는 "대안적인(alternative)" 제1 엑손이라는 것을 시사한다.
PCR/서열결정 실험은 테스트된 모든 결장암 세포주(HCT116p53KO, DLD-1 및 SW480)와 결장암 환자로부터의 9/9 FFPE 시료에서 이 "대안적인" BTK mRNA의 발현을 보여주었다(도 19).
GenBank accession # NM_000061에 의해 식별되는, BTK의 제1 엑손은 mRNA의 5'UTR에 상응하고, ATG 트리플렛(triplet)이 제2 엑손 내에 위치한 BTK 단백질의 제1 Met 아미노산이라는 것에 유의해야 한다. 5'UTR은 통상적으로 cap-의존적 또는 IRES-매개 cap-독립적 번역을 지시하는 것과 같은 조절 기능을 수행한다. 따라서, 신규한 전사체에서 확인된 바와 같은, 상이한 제1 엑손은 상이한 ATG(이 경우, 제4 엑손에 위치한 ATG)가 BTK 단백질의 번역을 개시하기 위해 사용되어야 하고, 따라서, 상이한 이소형의 발현을 조절해야 하는지 여부를 지시할 수 있다.

전술된 결과들은 유전자 BTK의 발현은 BTK 단백질의 신규한 이소형의 존재를 확인하는 단계를 포함하는 것인, 화학요법 약물에 대한 종양 세포의 내성을 결정하는 방법 및 종양 줄기 세포의 존재를 식별하는 방법을 시사한다.
BTK 단백질의 신규한 이소형의 존재는 예를 들면, 웨스턴 블롯, 또는 면역침전(immunoprecipitation) 또는 면역화학(immnunochemistry) 또는 면역형광(immunofluorescence) 분석을 이용하여 상기 단백질의 존재를 확인하거나, 또는 바람직하게는 cDNA가 서열번호 1로 정의된 뉴클레오티드 서열을 갖는, 대안적인 제1 엑손을 갖는 mRNA의 존재를 확인하는 것에 의해 제어될 수 있다. 후자는 유리하게 PCR 분석에 의해, 바람직하는 서열번호 1에 포함된 서열을 갖는 프라이머를 이용한 PCR 분석에 의해 수행될 수 있다.

실제로, 인간 환자로부터 채취된 종양 조직은 BTK 단백질을 발현할 수 있는 림프구의 유효량을 포함하여, 종양 세포에 의해 발현되는 BTK 단백질의 탐색을 교란시킬 수 있다는 것이 잘 알려져 있다.
그러나, 림프구에 의해 발현되는 BTK 단백질은 77 KDa의 분자량을 갖는 BTK 단백질의 "고전적인(classical)" 이소형이어서, BTK 단백질의 신규한 이소형의 탐 색이 간섭 없이, 대안적인 제1 엑손을 갖는 BTK mRNA의 존재를 찾는 단순한 PCR 분석에 의해 수행될 수 있다.
