사람의 IGFBP-2를 인코딩하는 유전자와 IGFBP-5를 인코딩하는 유전자의 부분에 상보적인 염기서열을 필수적으로 함유하는 양특이성 안티센스 올리고뉴클레오티드는 내분비선-조절 암의 치료에 있어서의 안티센스 치료요법에 유용하다.
Description
본 발명은 IGFBP-2 및 IGFBP-5를 억제하는 양특이성 안티센스(antisense) 올리고뉴클레오티드 및 이를 내분비선-조절 종양(예를 들면, 유방암, 전립선암, 난소암 및 결장암)의 치료에 사용하는 방법에 관한 것이다.
전립선암은 성인 남성에게 걸리는 가장 흔한 암이며, 서방 세계에서 성인 남성의 암 사망의 두 번째 원인이 된다. 전립선암은 남성호르몬-민감성 종양이기 때문에, 전립선암이 상당히 진행된 환자들에 대한 치료 요법으로 예컨대 거세에 의한 남성호르몬 퇴행(withdrawal)법이 이용되고 있다. 남성호르몬 퇴행법은 전립선암에서 광범위한 아폽토시스(apoptosis)를 유발시켜, 병을 퇴화시키는 것이다. 그러나 거세에 의해 유도된 아폽토시스는 완전하지 않으므로, 살아남아 있는 암세포가 결국에는 남성호르몬 비의존성(hormone independence)으로 진행되는 현상이 일어난다. 이러한 진행은 생존성 및 삶의 질을 향상시키는데 대한 주요 장애물이며, 따라서 남성호르몬 비의존성 암세포를 표적화하는 것에 대한 노력이 행해져 왔다. 이러한 노력은 남성호르몬 비의존성 종양세포를 표적으로 하는 비호르몬 치료에 초점이 맞추어져 있으나, 1998년 보고서에 의하면, 비호르몬제는 생존성을 향상시키지 못한다(Oh et al., J. Urol 160:1220 ~ 1229(1998)). 그러므로 대체 방법들이 예시되어 있다.
인슐린양 성장인자(Insulin-like Growth Factor) IGF-I 및 IGF-II는 많은 정상 세포 및 악성 세포에 대한 강력한 미토겐(mitogen)이다. 여러가지 증거 자료가 IGF가 전립선 질환과 유방암의 병리생리학에서 중요한 역할을 한다는 것을 시사한다. Boudon et al., J. Clin. Endocrin. IGF에 대한 생물학적 반응은 IGFBP를 포함한 여러 가지 인자에 의해 조절된다. 그 기능이 높은 친화 상호작용을 통해 IGF의 생물학적 활동을 변화시키는 것이라 생각되는 여섯 IGFBP를 확인하였다(Rajaram et al., Endocrin. Chem. 271: 1016 ~ 1021(1996); Damon et al., Endocrinology 139: 3456 ~ 3464(1998). 따라서 IGFBP의 상세한 기능은 논쟁의 여지가 남아있다. 이러한 이유로, 보고된 결과는 전립선암 및 유방암에서 IGF의 관계성을 암시하고 있으나, 이러한 관여도를 기본으로 하는 치료요법에 대해서는 명확하게 제시하지 못하고 있다.
본 명세서에서 참고문헌으로 소개되는 PCT 공보 WO 01/05435는 IGFBP-5를 인코딩(encoding)하는 유전자의 일부에 상보적인 안티센스 올리고데옥시뉴클레오티드를 투여함으로써, 사람을 포함한 포유동물의 호르몬-조절 종양(예를 들면, 유방암 및 전립선암)을 치료하는 방법에 대해 기술하고 있다. 본 명세서에서 참고문헌으로 소개되는 PCT 공보 WO 02/22642는 IGFBP-2를 인코딩하는 유전자의 일부에 상보적인 안티센스 올리고데옥시뉴클레오티드를 투여함으로써, 사람을 포함한 포유동물의 전립선암 및 기타 내분비성 종양을 치료하는 방법에 대해 기술하고 있다.
본 발명은 내분비선-조절 암의 치료에 IGFBP-2 및 IGFBP-5 둘 다에 표적화되는 양특이성 안티센스 올리고데옥시뉴클레오티드를 사용하는 것에 관한 것이다. 안티센스 올리고데옥시뉴클레오티드는 표적 유전자의 mRNA 부분에 상보적인 화학적으로 수정된 직쇄형 단일가닥 DNA이며, 그러므로 DNA/RNA 이중가닥을 형성시킴으로써 유전자 발현을 효과적으로 억제시킨다. Figueroa et al., J. Urol., 159: 1379 ~ 1383(1998). 포스포로티오에이트 올리고데옥시뉴클레오티드는 DNA의 논브리징(nonbridging) 포스포릴 산소 하나가 황으로 치환됨으로써 안정화되어 뉴클레아제 소화를 제지한다. 최근에는 유전자에 대해 특이적으로 표적화되는 몇몇 안티센스 올리고데옥시뉴클레오티드가 종양 분야 진보에 관여한다는 것이 시험관내 및 생체내 실험을 통해 인정되고 있으며, 잠재적인 치료제로서의 안티센스의 효능이 입증되고 있다. 본 발명은 IGFBP-2를 인코딩하는 유전자 및 IGFBP-5를 인코딩하는 유전자의 부분에 상보적이며, 유효량의 IGFBP-2 및 IGFBP-5의 억제제로서 역할하기에 충분한 길이(일반적으로 적어도 15개의 염기)를 가지는 염기 서열을 함유하는 양특이성 안티센스 올리고뉴클레오티드를 제공한다. 본 명세서의 상세한 설명과 청구 범위에서 사용되는 실질적으로 모든 안티센스 올리고뉴클레오티드는 각 유전자 서열의 일부에 상보적인 것을 의미한다. 그러나, 본 발명은 하나 또는 두개의 말단 염기의 추가와 같은 서열의 사소한 변경이나, 완전한 상보성에서는 벗어나 있으나 여전히 유효량의 IGFBP-2와 IGFBP-5의 억제제로서 기능을 할 수 있는 단일 염기 치환을 배제하는 것은 아니다.
유효량의 억제제라는 것은 안티센스 올리고뉴클레오티드가 다른 메커니즘에 의해 기능을 할 수 있다는 사실을 고려하는 것이다. 유효량의 IGFBP-2 또는 IGFBP-5는 세포내에서 기능을 할 수 있는 상태로 존재하는 양을 말한다. 안티센스 올리고뉴클레오티드의 투여에 의한 상기 양의 감소는 IGFBP의 생성을 제한함으로써(해독 및 전사 단계에서), 또는 생성되는 양보다 빠른 비율로 IGFBP를 분해함으로써 이루어질 수 있다. 또한 여기서 언급되는 억제라는 용어는 안티센스 올리고뉴클레오티드가 투여되지 않는다면 IGFBP가 존재하게 되는 경우에 이루어진다는 것을 이해해야 할 것이다. 후술하는 바와 같이, 잠재적인 치료 표적인 세포 모두가 IGFBP-2와 IGFBP-5 둘다를 발현하는 것은 아니다.

사용된 안티센스 올리고뉴클레오티드는 생체 내에서 안티센스 올리고뉴클레오티드의 안정성을 증가시키기 위해 수정될 수 있다. 예를 들면 안티센스 올리고뉴클레오티드는 뉴클레아제 소화에 대한 저항성을 강화시킨 포스포로티오에이트 유도체(논브리징 포스포릴 산소 원자 하나가 황 원자로 치환된 치환체)로서 사용될 수 있다. 또한 본 명세서에서 참고문헌으로 소개되는 미국 특허 제 6,451,991 호 및 미국 특허출원 제 10/080,794 호에 기술되어 있는 바와 같이 2-메톡시에틸(MOE) 치환 백본(backbone)을 가진 분자를 사용함으로써 안티센스 올리고뉴클레오티드의 안정성을 증가시킬 수 있다.
본 발명의 조성물은 본 발명에 따르는 양특이성 안티센스 올리고뉴클레오티드를 투여함으로써, 사람을 포함한 포유동물의 내분비선-조절 종양(예를 들면, 유방암, 전립선암, 난소암 및 결장암)을 치료하는데 사용할 수 있다. 안티센스 올리고뉴클레오티드의 투여는 네이키드 투여(naked administration) 및 의약적으로 허용가능한 캐리어 내에서의 투여를 포함해서 당해 분야에 알려진 다양한 메커니즘을 이용하여 수행될 수 있다. 예를 들면, 안티센스 전달을 위한 지질 캐리어에 대해서는 본 명세서에서 참고문헌으로 소개되는 미국 특허 제 5,855,911 호 및 미국 특허 제 5,417,9778 호에 기술되어 있다. 일반적으로 안티센스는 정맥내 , 복강내 , 피하 또는 경구 등의 경로에 의해 투여된다. 올리고뉴클레오티드가 의약적으로 허용가능한 캐리어 내에서 투여되는 경우, 이 캐리어는 일반적으로 사람에게 투여했을 때 독성이나 기타 다른 유해한 반응을 유발시키는 물질을 포함하지 않는다.
안티센스 올리고뉴클레오티드의 투여량은 문제의 내분비선-조절 종양 세포내의 유효량의 IGFBP-2 및/또는 IGFBP-5 양을 감소시키는데 효과적인 양이다. 상술한 바와 같이, 본 발명의 명세서에서, 본 발명은 안티센스 분자가 mRNA의 해독을 방해하여 IGFBP-2 및/또는 IGFBP-5의 발현이 제한되는 결과로서 또는 RNase 매개 메커니즘을 통해 감소가 달성될 수 있다 하더라도, 이러한 감소가 달성될 수 있는 어떠한 특정 메커니즘에 의해 제한되는 것은 아니다. 또한 적절한 치료양은 사용되는 특이성 안티센스 올리고뉴클레오티드의 유효성에 따라, 또 사용되는 캐리어의 특성에 따라 달라질 것이다. 주어진 조성물에 대한 적절한 양의 측정은 적절한 치료양을 평가하기 위해 의도된 일련의 표준 테스트를 통해 당해 분야에서 이루어진다.
또한 본 발명에 따르는 내분비선-조절 암을 치료하는 방법은 화학요법 약제 및/또는 다른 표적에 향하는 추가적인 안티센스 올리고뉴클레오티드의 투여를 또한 포함한다. 예를 들면, 탁솔(taxol)(파크리탁셀(paclitaxel) 또는 도데탁셀(dodetaxel)) 및 미토크산트론(mitoxanthrone)과 같은 종래의 화학요법 약제가 사용될 수 있다. 마찬가지로, 본 발명에 따르는 비특이성 안티센스 올리고뉴클레오티드와 기타 다른 안티센스 배열, 예컨대 Bcl-2 올리고뉴클레오티드, TRPM-2(클루스테린(clusterin)) 올리고뉴클레오티드, IGFBP-2 및/또는 IGFBP-5 올리고뉴클레오티드와의 배합물을 사용할 수 있다.
본 발명의 유효성은 솔리드 이론적 실험적 베이스 상에서 달성된다. 남성호르몬 퇴행에 따르는 대체 증식 인자 패쓰웨이(pathway)의 활성화는 상당히 진행된 전립선암에 있어서 남성호르몬 비의존성(AI)으로의 진행을 매개로 하는 한 메커니즘이다. 인슐린양 성장인자-I(IGF-I) 활성은 인슐린양 성장인자 결합 단백질(IGFBP) 패밀리에 의해 모듈화된다. IGFBP-2 및/또는 IGFBP-5가 호르몬 전립선암에서 가장 흔히 과발현되는 유전자이지만, AI로의 진행과정에서 IGF-I 시그널링의 변화의 기능적 중요성이 명확하게 명시되어 있지 않다. 전립선암의 AI로의 진행은 치료를 저항하게 만들고, 결과적으로 전립선암이 상당히 진행된 환자에게 가장 불길한 임상적인 징후인 골전이로 이어진다. 이러한 진행은 보통 사망 전 12 ~ 18개월까지 나타난다. 특히 IGFBP-5가 많이 존재하는 골 환경은 전립선암 전이를 촉진하는 매력적인 “토양(soil)"으로서 오래동안 확인되어 왔다. 중요한 IGF-I 시그널링 구성요소 IGFBP-2 및 IGFBP-5는 AI로의 진행 뿐만 아니라 전립선암의 자리-특이성 골전이에 키 역할을 할 수 있다. 이러한 문제에 초점을 맞추기 위한 시도에서, 본 발명자는 남성호르몬 퇴행 후의 전립선 암 모델내의 IGFBP-2 및 IGFBP-5의 변화를 특성화하고, AI로의 진행에서 이들의 기능적인 중요성을 기능 획득(gain-of function) 및 기능 상실(loss-of function)분석법을 이용하여 평가하였다.
IGFBP-2 mRNA 및 단백질 양은 생체내에서 AI로의 진행과정 동안 LNCaP 종양세포에서, 시험관의 LNCaP 세포에서 남성호르몬 퇴행 후 2 ~ 3배 증가한다. 또한, 거세 후 증가된 IGFBP-2 양은 비처리된 그리고 사후-호르몬 요법으로 처리된 전립선절제술 표본의 사람 전립선 조직 미세배열법(microarray)을 이용하여 확인되었다. 안정하게 과발현된 IGFBP-2를 가지는 LNCaP 세포 트랜스펙탄트(LNBP-2)는 남성호르몬 부재하에서 아폽토시스의 비율이 낮고 세포 더블링-시간도 짧다. 보고서 분석은 증식 잠재력의 증가가 남성호르몬-반응 요소의 IGFBP-2 매개 전사(transcription)로부터 초래되지 않았다는 것을 입증한다. 생체내에서, LNBP-2 세포 계열은 대조용 세포 계열에 비해 생쥐의 거세시 보다 상당히 많은 종양을 형성하였고, 보다 급속하게 남성호르몬 비의존성으로 진행되었다. IGFBP-2를 표적으로 하는 안티센스 올리고뉴클레오티드는 사용량-의존성 배열-특이성 방식으로 IGFBP-2 mRNA 및 단백질 발현을 >70% 까지 감소시켰다. 안티센스 올리고뉴클레오티드로 인한 IGFBP-2 감소는 LNCaP 세포 증식률을 감소시키고 아폽토시스를 3배 증가시켰다. 거세되고 에져반트(adjuvant) IGFBP-2 안티센스 올리고뉴클레오티드로 처리된 생쥐에 있어서 LNCaP 암 증식률 및 혈청 PSA 양은 불일치 대조용 올리고뉴클레오티드에 비하여 현저하게 감소되었다.
IGFBP-5 발현 전립선암 모델에서 IGFBP-5의 기능 획득 및 기능 상실분석법을 이용하여 비슷한 결과를 얻었다. IGFBP-5를 트랜스펙트(transfect)한 LNCaP 세포의 증식률은 디히드로테스토스테론의 존재 하에 그리고 부재 하에 모체 또는 벡터만 트랜스펙트된 LNCaP 세포에 비해 상당히 높았다. IGFBP-5로 인한 LNCaP 세포 증식의 증가는 IGF-I 의존성 및 비의존성 패쓰웨이를 통해 이루어지며, 이는 시클린 D1 mRNA 발현의 증가 및 세포 주기의 S+G2/M 상태에서의 세포의 분율(fraction)의 증가와 일치한다. LNCaP 서브라인에서의 Akt/프로테인키나제 B(PKB), 포스파티딜리노시톨 3‘-키나제(PI3K)의 다운스트림 성분의 패쓰웨이의 변화도 또한 그 증식률과 평행하였다. PI3K 억제제로 처리함으로써 대조용 및 IGFBP-5-과발현 LNCaP 세포에서 아폽토시스를 유도하였지만, 이러한 PI3K 억제제로 인해 유도된 아폽토시스는 IGFBP-5 트랜스펙탄트에서 외인성 IGF-I 처리에 의해 방해받으며, 이는 IGFBP-5-과발현이 IGF-I의 안티아폽토시스 효과를 가능하게 할 수 있다는 것을 시사한다. 또한 거세 이전 원상태의 생쥐에서 대조용으로 처리시 유사한 종양 발생률과 종양 증식률을 가졌으나, 거세 후 IGFBP-5 트랜스펙트된 LNCaP 종양을 가진 생쥐에서 종양 증식률 및 혈청 PSA 양은 수배 증가하였다. 이러한 데이터는 거세후 전립선암 세포에서의 IGFBP-5-과발현이 IGF-I의 안티아폽토시스 효과 및 미토겐 효과를 가능하게 하며, 그럼으로써 PI3K-Akt/PKB 시그널링 패쓰웨이의 활성화를 통해 남성호르몬 비의존성으로의 진행이 가속화되는 것을 돕는 적응적 세포 생존 메커니즘이라는 것을 시사한다. IGFBP-5 안티센스 올리고뉴클레오티드의 계통적 투여는 남성호르몬 비의존성으로 진행하는 시간을 현저히 지연시키고 AI 재발성 종양의 증식을 억제시켰다. 사람의 뼈에서 IGFBP-5 발현이 확인되었다.
표적 안티센스 기술을 이용한 IGFBP-2 및 IGFBP-5 발현 억제는 전립선암 환자 및 IGF-I 시그널링의 키 조절자를 발현시키는 기타 악성 종양 환자에 있어서 AI로의 진행 및 골전이를 지연시키는 트리트먼트 플랫폼을 제공한다. 단일 안티센스 올리고뉴클레오티드로 IGFBP-2와 IGFBP-5를 동시에 표적화함으로써 상기 화합물을 별도로 각각 표적화하는 혼합물 안티센스 올리고뉴클레오티드를 사용함으로써 생기는 번거로움을 피할 수 있다.
본 발명은 다음의 비한정적 실시예에서 보다 상세히 설명될 것이다.
도 1은 3가지 양특이성 안티센스 올리고데옥시뉴클레오티드, 불일치 대조용(MM) 또는 올리고뉴클레오티드 비함유(대조용)의 각각 투여시, LNCaP 및 PC3 세포에서 관찰되는 IGFBP-2와 IGFBP-5 발현의 양을 도시한 도면.
도 2는 남성호르몬 비의존성으로 진행 중인 전립선 암세포내에서의 IGFBP-2 발현의 증가를 보여주는 그래프.
도 3a 내지 3e는 다양한 안티센스 올리고뉴클레오티드의 사용에 의해 전립선 암세포 및 골세포에서 IGFBP-2와 IGFBP-5가 억제됨을 보여주는 그래프.
도 4 및 5는 본 발명의 안티센스 올리고뉴클레오티드로 처리한 후 RT4 방광암 세포내의 IGFBP-2 및 IGFBP-5의 양에 대한 실시간 PCR 측정 결과를 도시한 그래프.

실시예

500nM 농도의 서열 번호 1, 2 또는 3의 안티센스 올리고뉴클레오티드 혹은 불일치 대조용으로 LNCaP 세포를 처리하였다. IGFBP-2의 양을 측정하였다. 그 결과는 도 1a에 요약하였다.
500nM 농도의 서열 번호 1, 2 또는 3의 안티센스 올리고뉴클레오티드 혹은 불일치 대조용으로 PC3 세포를 처리하였다. IGFBP-5의 양을 측정하였다. 그 결과는 도 1b에 요약하였다.
실시예

사람의 전립선암 세포 샘플을 절단하고 면역염색하여 IGFBP-2를 검출하였다. 조직미세배열법 스코어링은 양성이나 암이 아닌 조직에서 IGFBP-2의 발현이 실질적으로 없었으나, 시간에 따라 남성호르몬 비의존성으로 된 이후에는 높은 비율로 증가하였다.
실시예

15개의 표본으로 사람의 전립선암의 골전이를 평가하였다. 사람의 전립선암은 매우 높은 비율의 IGFBP-2를 발현하였으나, 또한 IGFBP-5도 발현하였다. 사람의 골세포는 높은 비율의 IGFBP-5와 낮은 비율의 IGFBP-2를 발현하였다.
실시예

500nm 농도의 양특이성 안티센스 올리고뉴클레오티드를 사용하여 PC3, LNCaP 및 골세포를 처리하였으며, 실시간 PCR을 이용하여 IGFBP-2 또는 IGFBP-5의 억제 정도를 측정하였다. 서열 번호 2, 4, 5 및 7의 올리고뉴클레오티드를 테스트하였는데, 모든 올리고뉴클레오티드가 측정된 IGFBP의 검출량을 감소시키는데 효과적이었다(도 3a). 또한 500nM의 서열 번호 4 및 5를 사용했을 때 A549 폐세포내의 IGFBP-2의 양이 70%까지 감소된 것이 관찰되었다(도 3b). 또한 서열 번호 4(500nM)는 LNCaP 세포의 증식을 억제하고 세포수를 90%이상까지 감소시키는데 효과적이라는 것이 확인되었다(도 3c). 도 3d 및 도 3e는 각각 500nM의 서열 번호 1, 2, 4, 5, 6 및 7에 의해 PC3 세포내의 IGFBP-5의 양이 억제되고, 500nM의 서열 번호 1, 2, 4, 5, 6 및 7에 의해 사람의 태아 골섬유아세포내의 IGFBP-5의 양이 억제되었다는 결과를 보여준다.
실시예

안티센스 올리고뉴클레오티드 및 리포펙틴(LIPOFECTIN)(4 μg/ml)으로 처리한 후 MSF 사람의 태아 섬유아세포내의 IGFBP-5의 양을 측정하기 위해 실시간 PCR을 이용하였다. 시험관내 실험에서 세포를 플레이트하고 500nM의 올리고뉴클레오티드로 4시간 펄스 후 5% 혈청이 함유된 노멀미디엄에서 20시간동안 처리하였다. 둘째 날에 다시 4시간 펄스를 반복하고, 셋째 날에 그 세포 수를 카운트하였다. 가장 활성인 안티센스 올리고뉴클레오티드는 서열 번호 4 및 5였다(도 6).
실시예

양을 달리한 안티센스 올리고뉴클레오티드(서열 번호 4 및 5) 및 4 μg/ml의 리포펙틴으로 처리한 후 사람의 방광암세포(RT4) 내의 IGFBP-2 및 IGFBP-5의 양을 측정하기 위해 실시간 PCR을 이용하였다. 도 4 및 도 5에서 보여주는 바와 같이, 안티센스 올리고뉴클레오티드의 농도를 50에서부터 500nM으로 달리했을 때 사용량에 좌우되는 반응이 관찰되었다.

LNCaP/msf 혼합 종양세포 및 온전한 태아 골편내의 FBP-2 및 IGFBP-5의 양을 측정하기 위해 실시간 PCR을 이용하였다. 그 결과는 도 7에 나타내었다.

- 제1항에 있어서, 상기 올리고뉴클레오티드는 포스포로티오에이트 백본 구조를 갖는 것인 올리고뉴클레오티드.
- 제2항에 있어서, 상기 올리고뉴클레오티드는 포스포로티오에이트 백본 구조를 갖는 것인 올리고뉴클레오티드.
