여드름을 유발하는 원인중에는 피지가 모공밖으로 배출되지 않는 혐기적 조건에서 피지를 먹고 사는 Propionibacterium acnes 라는 균이 피지의 주성분인 트리글리세라이드를 분해하여 유리 지방산으로 변하게 한다. 이런 유리 지방산은 피부에 심한 자극을 주는 물질이어서 모낭벽이나 모공 주위의 세포에 염증을 일으킨다. 그 결과 각질이 두터워지고 모공 둘레가 부어서 모공이 막히면 피지가 원활하게 빠져나가지 못해 여드름이 생긴다.
