Abstract
꾸불하게 감긴 래더 스텐트는 코일이 셀의 감긴 스트립으로부터 형성되어, 셀의 측면이 꾸불하게 제공된다. 그러므로 스텐트는 일련의 감긴 와인딩 속에 나선형으로 감긴 스트립으로 구성되고, 스트립은 적어도 예를 들어 연속적인 십자가형 기둥(cross struts)에 의해 서로 연결되는 두 개의 측면 밴드로 구성된다. 각각의 측면 밴드는 일련의 굴곡(bend)으로 구성된 꾸불한 형태를 띠고, 스텐트 팽창시 측면 밴드의 굴곡은 나선형 방향에서 각각의 셀의 길이를 증가시키기 위해 개방되고, 그것에 의하여 스트립의 상당한 풀림(unwinding)없이 스텐트가 팽창하도록 나선형 방향에서 스트립을 증가시킨다.
Description
본 발명은 일반적으로 혈관과 같은 신체내에 혈관(vessel)속으로 이식되는 내부보철(endoprostheses)인 혈관을 지지하고 열린채로 유지하거나 혈관속에 다른 내부보철을 지지하고 안정화하기 위한 스텐트에 관한 것이다.
다양한 스텐트가 공지되어 있다. 전형적으로 스텐트는 관상(tubular)형이고, 비교적 작고 팽창하지 않은 직경에서 더 크고 팽창된 직경으로 팽창한다. 이식하는 동안, 스텐트는 전형적으로 카테테르(catheter)의 말단에 놓여지고 비교적작고 팽창하지 않은 직경으로 카테테르 위에 지지된다. 카테테르에 의하여 팽창하지 않은 스텐트는 관강(lumen)을 통하여 지정된 이식자리(implantation site)로 보내진다. 스텐트는 지정된 이식자리로 보내지면, 일반적으로 예를 들어 스텐트의 내부로 벌룬(balloon) 부풀게하는 내부힘이나 자체-팽창하는 스텐트 주변으로부터 슬리브(sleeve)를 제거하는 스텐트의 자체-팽창이거나 스텐트를 외부로 팽창하여 스텐트는 팽창된다. 어느 경우에나, 팽창된 스텐트는 혈관이 좁아지는 것을 막고 그것에 의해서 혈관의 개방성을 유지시킨다.

여기에 본 발명에 인용된 미국 특허 제4,553,545호(Maass et al., 'Maass '545 특허')는 감긴 스프링 형태의 다양한 스텐트를 개시하고 있다. 상기 특허의도 1 내지 도 7은 스텐트의 직경을 스프링 끝의 회전에 의해 접촉하고 팽창하여 나선형으로 감긴 와이어의 형태인 감긴(coiled) 스프링 스텐트를 나타내고 있다. 그러한 감긴 스프링 스텐트들은 비틀린(tortuous) 관강을 쉽게 찾을 수 있고 배치 후에 혈관의 컴플리언스(compliance)에 비교적 가깝게 따르기 위해 매우 탄력적이다. 그러나, 이 스텐트들이 매우 탄력적이나, 또한 팽창후에 비교적 불안정한 상태를 유지하게 한다. 코일의 각각의 와인딩(winding)은 서로 관계하여 움직일 것이고, 또한 때때로 인접 와인딩 사이에 상당한 틈이 생길수 있고, 이것은 혈관벽의 상당한 부분을 지탱할 수 없게 한다. 또한, 코일의 감김은 약간 구부리거나 기울일 수 있고, 관강을 가로막을 수도 있으며 심각하게 손상시킬 수 있다. 본 발명의 목적은 비뚤어진 관강을 탐지 할 수 있는 장기적으로 탄력적이고, 배치후에 혈관(vessel)의 컴플리언스(compliance)를 현저히 변화시키지 않는 스텐트를 제공하는 것으로, 스텐트는 관강을 막을 수 있는 굽음(bending)이나기움(tilting)을 어느정도 피할 수 있고 혈관 벽의 상당한 부분이 지지되지 않은 것을 피할 수 있도록 비교적 안정하다.
본 발명의 또 다른 목적은 팽창시 말단의 약간의 비틀림 또는 회전이 있거나 거의 없고, 팽창시 약간 또는 거의 없는 축소를 행하며, 팽창시 스텐트의 인접 와인딩 사이에 생성되는 상당한 틈을 동시에 생성하지 않는 스텐트를 제공하는 것이다.

본 발명에 따른 꾸불하게 감긴 래더 스텐트는 감긴 스프링 스텐트와 관련된 유연성을 지니고 있으며, 이동이나 기움에 비교적 안정하고 영향을 받지 않는 와인딩(winding)을 지니고 있다. 그러므로 본 발명에 따른 꾸불하게 감긴 래더 스텐트는 관강을 차단하는 불편함 없이 혈관 조직의 연속적인 지지를 제공하는 것이다.

감긴 래더 스텐트 20에서, 스트립은 십자가형 기둥 26에 의해 다리가 놓아진 측면 밴드들 24, 25를 가진다. 측면 밴드 24, 25 및 십자가형 기둥 26은 연속의 개방(opening) 27을 형성하고, 각각의 개방 27은 두 개의 측면 밴드 24, 25 및 두 개의 십자가형 기둥 26에 의해 결합된다. 감긴 와이어와 리본 스트립과 비슷하게, 래더 스트립은 연속이 와인딩을 형성하여 나선형으로 감긴다.
