7,976号および同第5,452,836号明細書に記載されてい

【０００６】別の望ましい線状ステープラーの特徴は、
外科医がクランピングトリガを中間または一部閉鎖した
位置に引くことができる、という特徴である。この特徴
は、完全に閉じ、そしてステープルで止めることが成さ
れる前に、外科医がステープラーを都合よく、かつ正し
く配置することを可能にする。いったんステープラーの
位置が設定されれば、次に外科医は組織を締めるため
に、一部閉じた位置から完全に閉じた位置へとクランピ
ングトリガを引けることが望ましい。したがってまた組
織をステープルで閉じた後に、ステープラーのクランピ
ングトリガがその元の完全に開いた位置に、一部閉じた

【０００７】したがって、外科用具、特にハンドグリッ

しかしこの用具は、第一の外科的機能が組織を締めるこ
とであり、そして行われるべき第二の外科的機能が締め
た組織にステープルを打ち込む時に特に適合している。
理想的には、本発明の外科用具は、線状ステープルに使
用される。

【実施例】図１に関して、図２−４と組み合わせて、線
状外科用ステープラー20を示している。このステープラ
ーは第一近位端にフレイム21および反対の遠位端にエン
ドエフエクター80を有する。右および左手構造板、また
はそれぞれホック34および35は、用具のフレイムをエン
ドエフエクターに連結する(左のハンドホックは図１に
示していない)。フレイムは、左手側板に連結した右手
側板22を有する（左手側板は図１に示していない）。こ
のフレイムはまた、ステープラーを握り、そして誘導す
るための本体部23も有する(図２−４を参照にされた
い)。エンドエフエクターは、カートリッジ24およびア
ンビル25を有する外科的固定アッセンブリーである。こ
のカートリッジは、垂直列に複数のステープルを含有す

照にされたい）。このハンドグリップは、右手の側板ハ
ンドル29(図１を参照にされたい）を左手の側板ハンド
ル（左手の側板ハンドルは図１に示していない）につな
ぐことから形成される。フレイムの下側から旋回可能に
延びているのは、閉鎖トリガ30および発射トリガ31であ
る。図１に描いた線状の外科用ステープラーは、未作動

よりフレイムに取り付けられ、そしてボタンバネポスト
66により解放ボタンに取り付けられている解放バネ64に
より、解放パラはクランプスライド面に向かって下側に
片寄っている。スロット化クランピングアームリンク40
は、中間およびクランピング止めの間に位置する弓形の
凹部67を有する。回転運動のためにこの弓形の凹部内に
置くのは、右手トグルに統合的に連結した左手トグル68
である（右手トグルは示さず）。各トグルは、パラ突出
部60に係合可能なトグルアーム69を有する。このパラ突
出部はトグルアームとパラ突出部との間にクリアランス
を提供するための凹状の近位面70を有する。
【００２７】クランピングおよび発射機構の操作、なら
びに中間クランピング止めおよび解放機構を、図６−１
０と関連させてより詳細に説明する。図６では、クラン
ピングトリガ30は、図１および５に描かれている解放の
未作動位置から一部引かれている。クランピングトリガ
が一部引かれる時、これはクランピングトリガピボット
ピン41について、ハンドグリップに向かって反時計回り
の方向に旋回する。回転すると、スロット化アームリン
ク40および閉鎖板クランピングリンク36および37が前方
に向かって移動し、その結果、右および左手閉鎖板32お
よび33が移動し、そして発射棒43が遠方に移動する。ス
ロット化アームリンクが前方に移動すると、解放パラ59
のパラ突出部60はクランプスライド面56上に移動する。
このパラ突出部60はトグルのトグルアームの遠位端を係
合し、そしてその結果、トグルを時計回りの方向に旋回
させる。スロット化アームリンクが、ハンドグリップに
向かうクランピングトリガの旋回運動に反応して前方に
移動し続けると、解放パラ59のパラ突出部60は最終的に
中間止め57中に入る。いったん中間止めに位置すると、
閉鎖バネはクランピングトリガをその元の未作動位置に
戻すことができない。このクランピングトリガは今、中
間の一部閉じた位置にあり、図２に示すように組織をカ
ートリッジとアンビルの間に正しく配置し、そして保持
する。さらに、右および左手閉鎖板および発射棒は遠方
に移動し、弓形の発射トリガリンク51の先端52が発射棒
の近位端区分のスライド面48に移動する。
【００２８】特に図７を参照にして、クランピングトリ
ガが中間止め位置からハンドグリップに向かって引かれ
る時、トグルのトグルアームはパラ突出部から外れる。
その結果、トグルが時計回り方向に回転し続けるので、
解放パラ突出部60はトグルアームを上げ、そしてクラン
ピングトリガの連続運動でクランピング止め58に落ち
る。解放パラがトグルアームを上げるので、これは解放
ボタンをピボット63について時計回りに回転させる。解
放パラ60がクランピング止め58に落ちると、これは外科
医にクランピング位置に到達したことを変えさせるクリ
ック音を生じる。さらに、発射棒が前方に向かって移動
し続けると、弓形の発射トリガリンクの先端が発射棒の
近位端区分の側連結面49と接触するようになる。その結
果、発射トリガは組織が完全に締められた後にステープ
ルを打つために、発射棒を遠方で動かし続けられる位置
に移動する。弓形の発射トリガリンクの先端が、近位端
区分の係合面と係合するように移動する時、発射トリガ
はトリガの右手側上のトーションバネの作用に反応し
て、ハンドグリップに向かって反時計回り方向に旋回し
始める（トーションバネは示していない）。発射トリガ
ピボットはクランピングトリガの旋回運動とは独立して
旋回するが、この旋回は発射棒が遠方に移動して、発射
トリガリンクを発射棒の末端係合面と係合させることが
できるまで阻止される。
【００２９】特に図８を参照にして、クランピングトリ
ガが完全に引かれ、そしてハンドグリップに隣接した
時、解放パラの遠位末端でパラ突出部はクランピング止
めの中に入る。クランピング止め位置で、組織が完全に
カートリッジとアンビルとの間で締められ、そして閉鎖
バネはクランピングトリガをその元の位置に戻すことが
できない。したがって、クランピングトリガは図３に示
す位置に保持される。クランピングトリガの反時計回り
の運動と同時に、発射トリガは発射トリガがステープラ
ーのフレイムに関して比較的垂直方向になるまで、トー
ション発射バネの作用により反時計回りに回転し続け
る。完全に締めた位置で、弓形の発射トリガリンクの先
端は完全に発射棒の近位端区分の係合面に完全に係合
し、したがって発射トリガはさらに発射棒を遠方に移動
させてステープルを組織に打ち込む位置にある。図９に
説明するように、発射トリガはクランピングトリガと隣
接する位置になるまで、引かれてハンドグリップに向か
って旋回することができる。発射トリガが旋回している
間、発射棒はステープルを打つために遠方に移動する。
図４に示す位置と同様であるが図９に説明する位置で、
ステープラーは完全に打たれた。
【００３０】最後に図１０を参照にして、外科医が解放
ボタン61を押す時、解放パラ59は解放トラニオン63につ
いて中心軸で時計回り方向に旋回して、パラ突出部をク
ランピング止め位置からはずす。これがはずされると、
パラ突出部60はトグルアーム69上に乗り、クランプリン
ク40上の中間止め位置57をバイパスする。このようにク
ランピングおよび発射トリガは、閉鎖バネ42および発射
棒戻しバネ55から生じる片寄りに反応して、それらの元
の位置、未作動位置に戻ることができる。パラ突出部が
トグルのトグルアーム上に乗る時、トグルアームはクラ
ンピングのように反時計回り方向に回転し、そして発射
トリガが時計回りに回転して元の未作動位置に戻る。し
たがって、クランピングおよび発射トリガが中間止め位
置に不必要に戻ることなく図５に描かれている位置に戻
れるように、外科医はクランピングおよび発射トリガを
解放することができる。
【００３１】本発明を最も好適な態様を用いて記載して
きたが、多数のさらなる態様が当業者には明らかであろ
う。この記載は説明を目的とするのみであり、以下に記
載する請求の範囲のような特許請求する本発明の範囲お
よび精神を限定するものではない。例えば、本発明は特
に線状ステープラーに関して記載したが、本発明を解放
および内視鏡的な外科処置に使用するための多数の様々
な外科用具に応用できることは容易に明らかである。
【００３２】本発明の主な特徴および態様は、次のとお
りである。
【００３３】１．ａ）用具の第一端における握りのため
のそして該用具を操作するためのフレイムであって、本
体部および該本体部から下がったハンドグリップを有す
るフレイム； ｂ）該用具の反対端で外科的処置を行うエンドエフエク
ター； ｃ）該ハンドグリップから間隔が空いた未作動の第一ト
リガ位置から、該ハンドグリップに隣接する完全に作動
した第一トリガ位置へ反時計回り方向に旋回するよう
に、該フレイムの該本体部内の第一ピボットピンに旋回
可能に設けられた第一トリガ； ｄ）近位および遠位端を有し、近位端は該第一トリガに
つながれ、そして遠位端は該エンドエフエクターにつな
がれた第一伝導アッセンブリーであって、（i）該第一
トリガが、該未作動の第一トリガ位置からハンドグリッ
プに向かって反時計回り方向に旋回するとき、該第一伝
導アッセンブリーは該エンドエフエクターが外科的処置
の第一外科的機能を果たすように、遠方に移動する第一
伝導アッセンブリー； ｅ）該第一トリガ位置から間隔が空いた未作動の第二ト
リガ位置から、該第一トリガに隣接する完全に作動した
第二トリガ位置へ反時計回りの方向で旋回するように、
該フレイムの該本体部内の第二ピボットピンに旋回可能
に取り付けられた第二トリガであって、第二ピボットピ
ンは第一ピボットピンから間隔が空いており、該第二ト
リガの旋回は、該第一トリガの旋回とは独立しており、
該第一トリガは該第二トリガと該ハンドグリップとの間
に位置する第二トリガ；ならびに ｆ）近位および遠位端を有し、この近位端は該第二トリ
ガと係合でき、そして遠位端は該エンドエフエクターと
係合できる該第二伝導アッセンブリーであって、（i）
該第一トリガが該未作動第一トリガ位に位置するとき、
該第二伝導アッセンブリーの近位端は、該第二トリガの
反時計回り方向の旋回を阻止し；（ii）該第一トリガ
が、該未作動第一トリガ位置からハンドグリップに向か
って反時計回りの方向に旋回するとき、該第二伝導アッ
センブリーは該第二トリガの旋回の阻止を防止するよう
に、該第一伝導アッセンブリーと同時に遠方に移動し；
そして（iii）該第一トリガが、該完全に作動した第一
トリガ位置に位置し、そして該第二トリガが該第一トリ
ガに向かって反時計回りの方向に旋回するとき、該第二
伝導アッセンブリーは、該エンドエフエクターが外科的
処置の第二の外科的機能を果たすように遠方に移動し続
ける、第二伝導アッセンブリーを含んで成る外科用具。
【００３４】２．上記第一トリガが、上記の完全に作動
した第一トリガ位に対して旋回したとき、上記第二トリ
ガが上記未作動位から上記ハンドグリップに向かって旋
回する、上記１に記載の外科用具。
【００３５】３．上記第二トリガが、上記第二伝導アッ
センブリーの近位端に接する第二トリガリンクに付いて
いる、上記２に記載の外科用具。
【００３６】４．第二伝導アッセンブリーの近位端が、
スライド面および上記側係合面を有し、そして上記第二
トリガリンクは、第一トリガが上記未作動第一トリガ位
置から上記ハンドグリップに向かって旋回するとき、該
スライド面上に移動し、そして該第一トリガが該完全に
作動した第一トリガ位に旋回するとき、該第二トリガリ
ンクが該側係合面を係合する、上記３に記載の外科用
具。
【００３７】５．上記第一トリガが、中間止めおよび最
終止めを持つアームスライド面を有するアームリンクに
付いている、上記４に記載の外科用具。
【００３８】６．さらに、上記フレイムの上記本体部に
旋回可能に取り付けられた解放パラを含んで成り、該パ
ラは上記第一トリガが旋回するとき、上記アームスライ
ド面上に移動するために該アームスライド面に向かって
傾いている、上記５に記載の外科用具。
【００３９】７．上記第一トリガが、上記未作動第一ト
リガ位置から反時計回り方向に旋回するとき、上記パラ
が上記アームスライド面に移動し、そして上記中間止め
に入り、そして該第一トリガが該未作動第一トリガ位置
と該作動第一トリガ位置との間の中間作動位置に保持さ
れる、上記６に記載の外科用具。
【００４０】８．さらに上記アームスライド面に回転で
きるように固定され、そして上記中間止めと上記最終止
めとの間に位置するトグルを含んで成り、該トグルがそ
こから伸びるトグルアームを有し、該トグルアームは上
記第一トリガが未作動第一トリガ位置に位置するとき、
該パラと係合できる、上記７に記載の外科用具。
【００４１】９．上記第一トリガが上記未作動第一トリ
ガ位置から、上記中間作動位置に旋回する時、該パラが
該トグルアームを第一方向に回転させる、上記８に記載
の外科用具。
【００４２】１０．上記第一トリガが、上記中間作動位
置から上記の完全に作動した第一トリガ位置に旋回した
とき、上記パラは最初に該トグルアーム上の該最終止め
をライドオーバーし、そして次に該アームから外れて該
最終止めに入るようになり、ここで該第一トリガは該完
全な第一トリガ位置に保持されている、上記９に記載の
外科用具。
【００４３】１１．さらに、上記フレイムの上記本体部
に弾力的に取り付けられ、そして上記解放パラに連結さ
れた解放ボタンを含んで成り、ここで該パラが上記最終
止めに入ったとき、該解放ボタンが押され、該ボタンは

【００４４】１２．上記エンドエフエクターが外科的固
定アッセンブリーを含む、上記１に記載の外科用具。
【００４５】１３．上記アッセンブリーが、組織−接触
面を上に持つステープルカートリッジ、およびステープ
ル−形成面を上に持つアンビルを有し、該表面はその間

【図１】ステープラーの内部構成を見せるために取りは

瞭性のために取り外されている。

Claims (1)

    【特許請求の範囲】
  1. 【請求項１】 ａ）用具の第一端における握りのための
    そして該用具を操作するためのフレイムであって、本体
    部および該本体部から下がったハンドグリップを有する
    フレイム； ｂ）該用具の反対端で外科的処置を行うエンドエフエク

    リガ位置から、該ハンドグリップに隣接する完全に作動
    した第一トリガ位置へ反時計回り方向に旋回するよう
    に、該フレイムの該本体部内の第一ピボットピンに旋回
    可能に設けられた第一トリガ； ｄ）近位および遠位端を有し、近位端は該第一トリガに
    つながれ、そして遠位端は該エンドエフエクターにつな
    がれた第一伝導アッセンブリーであって、（i）該第一
    トリガが、該未作動の第一トリガ位置からハンドグリッ

    に位置する第二トリガ；ならびに ｆ）近位および遠位端を有し、この近位端は該第二トリ

    そして（iii）該第一トリガが、該完全に作動した第一
    トリガ位置に位置し、そして該第二トリガが該第一トリ
    ガに向かって反時計回りの方向に旋回するとき、該第二
    伝導アッセンブリーは、該エンドエフエクターが外科的
    処置の第二の外科的機能を果たすように遠方に移動し続
    ける、第二伝導アッセンブリーを含んで成る外科用具。
