Description

【発明の詳細な説明】 〔産業上の利用分野〕 本発明は、湾曲又は屈曲した骨の接骨用固定具であっ
て、スクリューや釘のような固定手段がそれぞれ備えら
れた一対の支持板が連結手段によって位置固定可能に連
結され、前記固定手段が各支持板の長手方向及びその垂
直方向に位置調節可能なものに関する。
〔従来の技術〕
接骨用固定具、特に金属製の固定具が骨接部を固定する
ために用いられる。例えば顎骨の手術において、屈曲し
た又は湾曲した骨部分の骨折端部を所望の位置に正確に
固定することは非常に困難である。特に、今日普通に行
われているように固定具を完全に覆い隠す場合に困難が
大きい。
この種の接骨用固定具として、米国特許第1789060号に
開示されているものがある。骨折部の固定は人体の外部
に配置された固定具によって行われ、この固定具は基本
的に２つの支持板からなる。各支持板にはその長手方向
及び直角方向に位置調節可能な固定部材が保持されてい
る。支持板上での固定部材の回動は許容されていない。
２つの支持板は複雑な連結機構によって連結されてい
る。この連結機構は、一対の相対位置調節可能な半球
と、各の支持板に形成された細長い案内溝からなる。２
つの支持板の案内溝が直交する位置関係により、一方の
支持板の縦方向位置及び他方の支持板の横方向位置が調
節可能である。
又、特開昭60−77747号には別の従来例が開示されてい
る。この固定具では２本の支持棒がボール継手で連結さ
れ、各支持棒には長手方向に沿って固定具を保持するた
めの複数の孔が配設されている。いずれの孔に固定具を
挿入するかによって支持棒の長手方向における固定具の
位置を段階的に調節することができる。又、挿入された
固定具はそれに直交する固定螺子との摩擦によって、支
持棒の長手方向に垂直な方向に位置調節されて固定され
る。
〔発明が解決しようとする課題〕 上記の第１従来例の固定具は、骨の位置合わせを不連続
に行うことしかできず、例えば顎骨や胸椎のような複雑
に湾曲している骨の骨折の場合は、固定具の限られた自
由度に起因してかなり制限されたものとなる。第２従来
例にあっても、一対の支持棒のなす角度の可動範囲が限
られており、又、支持棒の軸方向における固定具の位置
を連続的に調節することができない点で改善の余地があ
る。
そこで、本発明の目的は、一対の支持棒（支持板）のな
す角度の可動範囲を広げて、骨の位置合わせを連続的に
行うことができる接骨用固定具を提供することにある。
〔課題を解決するための手段〕
本発明による接骨用固定具は、スクリューや釘のような
固定手段がそれぞれ備えられた一対の支持板が連結手段
によって位置固定可能に連結され、前記固定手段が各支
持板の長手方向及びその垂直方向に位置調節可能なもの
であって、その特徴構成は、前記連結手段が、中間部材
で連結された２つのボールヘッドを備える二連ボール継
手で構成され、しかも、前記固定手段が前記支持板の長
手方向軸に平行な平面上で回動可能で、且つ、任意の回
動角度で固定可能である点にある。
〔作用効果及び実施態様〕
上記特徴構成によれば、まず、一対の支持板が二連ボー
ル継手で連結されているので、一対の支持板がなす角度
の可動範囲はかなり広くなる。さらに、骨に挿入固定さ
れるスクリューや釘のような固定手段が、各支持板の長
手方向及び垂直方向に位置調節可能であるのみならず、
支持板に対する角度（姿勢）をも調節可能であるので、
固定具全体の自由度は従来に比べてかなり大きくなる。
この結果、接合すべき骨折部分を位置合わせする作業の
作業性が大幅に改善される。この作業は、例えば、まず
接合すべき骨に固定手段を取り付け、支持板を連結し、
例えばレントゲン写真に基づいて支持板のなす角度を望
ましい角度にセットし、最終に接合すべき２つの骨部分
を正確に位置合わせして固定具全体の可動部分を固定す
るといった手順で行われる。
好ましくは、二連ボール継手のボールヘッドに対応する
凹面のほぼ四分球面であるボール受けが支持板の端部に
形成され、ボール受けと同様の四分球面を有するボール
受け板を前記支持板に固定することによってボール受け
内での前記ボールヘッドの固定が行われる。例えば、ボ
ール受け板を前記支持板に固定するねじを緩めることに
より、ボール受けと及びボール受け板で形成される半球
凹面内でボールヘッドが摺動回転可能となり、上記ねじ
を締め付けることにより半球凹面内でボールヘッドが固
定される。
また、固定手段を保持する保持部材が支持板の長手方向
に位置調節自在に前記支持板に備えられ、保持部材が、
固定手段を回動自在に且つ支持板の長手方向と垂直な方
向に位置調節自在に保持するためのクランプ部材を備え
ている構造が好ましい。
さらに具体的には、保持部材が、支持板に形成された案
内溝内を摺動自在な摺動台を備え、この摺動台に形成さ
れた雌ねじ孔に螺合する螺子軸が支持板に回転自在に支
持される構造によって、螺子軸を回転することによる支
持板の長手方向における摺動台の位置調節が可能とな
る。
摺動台のクランプ部材との合わせ面を平らに形成し、ク
ランプ部材の摺動台との合わせ面に固定手段を保持する
ための凹面を形成することにより、例えばクランプ部材
を摺動台に締め付け固定する一本のねじを緩めれば、ク
ランプ部材が保持する固定手段が摺動台に対して、即ち
支持板に対して連続的に回動可能で、且つ、ねじを締め
付けることにより任意の回動角度で固定手段を固定可能
な構造が簡単に得られる。
螺子軸の軸方向の移動規制は、例えば螺子軸に嵌め込ま
れる係止リングによって簡単に実現できる。
〔実施例〕
以下に本発明の実施例を図面に基づいて説明する。
図１に示すように、２つの支持板1,2が二連ボール継手
３によって連結されている。各支持板1,2には一対のス
クリュー（ねじくぎ）6,7からなる固定手段4,5がそれぞ
れ保持されている。固定手段4,5（スクリュー6,7）は保
持部材8,9を介して支持板1,2に保持され、保持部材8,9
は螺子軸10,11により支持板1,2の長手方向に移動自在に
支持されている。
二連ボール継手３は中間部材12で連結された２つのボー
ルヘッド14,15を備え、各ボールヘッド14,15に対応する
凹球面が各支持板1,2の端部に備えられている。つま
り、各支持板1,2の端部にボールヘッド14,15に対応する
凹面のほぼ四分球面であるボール受け16,17が形成さ
れ、同様の四分球面20がボール受け板18,19に形成さ
れ、ボール受け板18,19を支持板1,2の端部に固定したと
きに二つの四分球面が合わさって形成される半球面内に
ボールヘッド14,15が収容される。もっとも実際にはボ
ールヘッド14,15の脱落を防止すべく半球よりやや広い
球面とする必要がある。ボール受け板19,18は、固定用
ねじ21の挿通孔があけられ、支持板1,2の対応箇所には
ねじ孔22が形成されている。
ボールヘッド14,15が支持板1,2の端部とボール受け板1
9,18とで形成された凹球面に嵌合している状態で固定用
ねじ21を強く締めればボールヘッド14,15と凹球面とが
固定され、固定用ねじ21を緩めればボールヘッド14,15
と凹球面とが摺動可能となる。従って、固定用ねじ21を
緩め、又は締め付けることにより、二連ボール継手３を
介して支持板１と支持板２とが成す角度を広い範囲にわ
たって変更することができる。
支持板1,2には前述の保持部材8,9の移動を案内する案内
溝23,24が形成されている。保持部材8,9は案内溝23,24
内を摺動する摺動台25,26と、これにねじ29で固定され
るクランプ部材27,28とからなる。クランプ部材27,28の
摺動台25,26との合わせ面にはスクリュー6,7を保持する
ための一対の溝30が形成され、摺動台25,26の合わせ面
は平らに形成されている。ねじ29を締め付けることによ
ってスクリュー6,7（固定手段4,5）が固定される。ねじ
29を緩めればクランプ部材27,28が摺動台25,26に対して
回転でき、これによってスクリュー6,7の方向を調節で
きる。
摺動台25,26には螺子軸10,11に螺合する雌ねじ孔が形成
されている。螺子軸10,11は、その両端部が支持板1,2に
形成された孔31,32に挿通されて案内溝23,24内で回転自
在に支持される。又、拡径頭部35,36とその近くに嵌め
込まれる係止リング34,33によって螺子軸10,11の軸方向
の移動が規制されている。このような構造により、螺子
軸10,11を回転することによって摺動台25,26（保持部材
8,9）即ちスクリュー6,7（固定手段4,5）を案内溝23,24
に沿って支持板1,2の長手方向に移動させ、その位置を
調節することができる。

Claims (6)

    【特許請求の範囲】
  1. 【請求項１】湾曲又は屈曲した骨の接骨用固定具であっ
    て、スクリュー（6,7）や釘のような固定手段（4,5）が
    それぞれ備えられた一対の支持板（1,2）が連結手段に
    よって位置固定可能に連結され、前記固定手段（4,5）
    が各支持板（1,2）の長手方向及びその垂直方向に位置
    調節可能なものにおいて、 前記連結手段が、中間部材（12）で連結された２つのボ
    ールヘッド（14,15）を備える二連ボール継手（３）で
    構成され、しかも、前記固定手段（4,5）が前記支持板
    （1,2）の長手方向軸に平行な平面上で回動可能で、且
    つ、任意の回動角度で固定可能である接骨用固定具。
  2. 【請求項２】前記支持板（1,2）端部に前記ボールヘッ
    ド（14,15）に対応する凹面のほぼ四分球面であるボー
    ル受け（16,17）が形成され、そのボール受け（16,17）
    内での前記ボールヘッド（14,15）の固定が、前記ボー
    ル受け（16,17）と同様の四分球面（20）の有するボー
    ル受け板（18,19）を前記支持板（1,2）に固定すること
    によって行われる請求項１記載の接骨用固定具。
  3. 【請求項３】前記固定手段（4,5）を保持する保持部材
    （8,9）が前記支持板（1,2）の長手方向に位置調節自在
    に前記支持板（1,2）に備えられ、前記保持部材（8,9）
    が、前記固定手段（4,5）を回動自在に且つ前記支持板
    （1,2）の長手方向と垂直な方向に位置調整自在に保持
    するためのクランプ部材（27,28）を備えている請求項
    １又は２記載の接骨用固定具。
  4. 【請求項４】前記保持部材（8,9）が、前記支持板（1,
    2）に形成された案内溝（23,24）内を摺動自在な摺動台
    （25,26）を備え、この摺動台（25,26）に形成された雌
    ねじ孔に螺合する螺子軸（10,11）が前記支持板（1,2）
    に回転自在に支持され、前記螺子軸（10,11）を回転す
    ることにより前記支持板（1,2）の長手方向における前
    記摺動台（25,26）の位置調節が可能な請求項1,2又は３
    記載の接骨用固定具。
  5. 【請求項５】前記摺動台（25,26）の前記クランプ部材
    （27,28）との合わせ面は平らに形成され、前記クラン
    プ部材（27,28）の前記摺動台（25,26）との合わせ面に
    は前記固定手段（4,5）を保持するための凹部（30）が
    形成されている請求項1,2,3又は４記載の接骨用固定
    具。
  6. 【請求項６】前記螺子軸（10,11）の軸方向の移動が、
    前記螺子軸（10,11）に嵌め込まれる係止リング（33,3
    4）によって規制されている請求項1,2,3,4又は５記載の
    接骨用固定具。
