神経刺激による内分泌障害の治療
発明の背景
本発明は、一般的には、患者の選択された1つまたは複数の神経に変調電気信号
を印加することによる医学、精神または神経障害を治療または制御するための方
法及び装置に関するものである。より具体的には、本発明は、埋め込み型神経刺
激装置を使用して変調電気信号を脳神経に印加することによって、さらに詳しく
言えば迷走神経電気活性を選択的変調することによって、糖尿病及び他の全身性
膵臓疾患を有する患者を治療する技術に関するものである。
全身性疾患は、脳半球、脳幹、小脳、を髄、末梢神経及び筋肉含む神経系の多病
巣性または拡散関与の共通パターンを共有する。特定の病理学的な実体または病
理学的なプロセスによる疾患とは対照的に、器官不良または機能障害に起因する
そのような疾患としては内分泌障害がある。内分泌腺の障害と神経異常とは、様
々な臨床的な症候群において関係している。内分泌障害としては、低血糖症と真
性糖尿病の膵臓障害がある。低血糖症は、一般的に、血中ブドウ糖濃度が100
1にあたり40a+g未満であると定義され、低血糖症の開始年齢及び進行度に
よって変化する示唆的な兆候及び症状を伴う。低血糖症がヒトに起こる最も共通
した状態は、インシュリンかまたは長期間作用する低血糖剤を摂取している糖尿
病患者に現れる。通常、糖尿病患者は、低血糖の症状を認識し、それを予防する
ために必要な行動をとることができる(例えば、インシュリンの投与量を減らす
)。
糖尿病は、抗利尿ホルモンの不足の結果(腎性尿崩症)または高血糖症の結果と
して尿の過度の排出と、血液中のブドウ糖が異常に高いレベル(真性糖尿病)に
あることによって特徴付けられる状態である。本発明は、主として、真性糖尿病
の治療に関するものである。真性糖尿病とは、膵臓のβ細胞によるインシュリン
分泌の相対的なまたは完全な欠如またはインシュリンレセプタの欠陥に主に起因
する炭水化物、脂肪及び蛋白質の代謝の複雑な障害であり、典型的な家族性疾患
である。本明細書の以下の部分及び請求の範囲において、「糖尿病」という語は
、真性糖尿病を示すものとする。
ナショナル インスティテユート オブ ヘルス(NationalInsti
tutes or 1IealLh)によって指定された糖尿病の種類には、■
型とII型がある。■型糖尿病は、以前には若年性発症糖尿病と呼ばれており、
インシュリンレベルが異常に低く、ケト−シスを防ぐためにインシュリンに依存
している患者を含む。治療せずに放置すると、ケト−シスは、ケトアシド−シス
、昏睡及び死に至ることがある。11型糖尿病は、インシュリン非依存性糖尿病
であり、この疾患の患者のインシュリンレベルは正常かまたは高いが、末梢器官
の感覚が減退した結果高血糖症が起きる。高血糖症から起きる神経性合併症も同
様に糖尿病性ケトアシド−シス、昏睡及び死に進行することがある。他の合併症
は、末梢神経障害、多発性単神経炎、神経根底、自律神経障害、頭蓋骨神経障害
、網膜症、を髄障害、筋障害、腎障害、奇形発生及び早発アテローム硬化症であ
る。
糖尿病の治療の目的は、インシュリン−ブドウ糖ホメオスタシスを維持すること
であり、インシュリン−ブドウ糖ホメオスタシスは、疾患の軽い早期または後期
の発症の場合、食餌療法だけで制御することができる。より悪化した場合は、血
液中のブドウ糖レベルをケトアシド−シスが起こるレベルより低く保持するため
にインシュリンホルモンの薬理調製物を投与することが習慣的に行われている。
インシュリン調製物の処方は、即効性、強さ及び作用持続期間が異なり、過剰な
投与から低血糖症及びインシ、 IJン衝撃、及び、不十分な投与から高血糖症
及び糖尿病性ケトアシド−シスを含む有害な反応を引き起こすことがある。禁忌
は、他の薬剤との起こりうる有害な相互作用を含む。すなわち、熱、ストレス及
び感染の存在下で増大するインシュリン要求、インシュリンまたはそれが含まれ
るビヒークルの成分に対するアレルギー反応及び肝臓または腎臓が存在する箇所
での減少したインシュリン要求である。
本発明の主な目的は、インシュリン−ブドウ糖ホメオスタシスを維持することが
必要な時はいつでも体内のインシュリン分泌を選択的に増加させて、糖尿病を治
療する方法を提供することにある。
通常、内生(自然に生成する)インシュリンホルモンは、血液中のブドウ糖レベ
ルの増大に応答して膵臓内の小島のβ細胞によって分泌される。しかしながら、
上記のように、末梢器官感覚の減退によって、内生インシュリンレベルが正常ま
たは高いことさえある人の場合でさえも、高血糖症が起きることがある。さらに
、血中ブドウ糖レベルは、インシュリン分泌の唯一の刺激ではない。自律神経系
も、迷走神経を介してインシュリン放出に影背する(ラスムラセン(Rasmu
ssen)その他「糖尿病治療(Diabetic Care) J 1990
年、13 (6)号、655〜666頁)。
ジ(J、Physiol、)J 1967年、192号、317〜327頁で、
ヒヒにおいて迷走神経の刺激下で膵臓のインシュリン放出が著しく増加すること
を発見したが、恐らく刺激のレベルが低いため低血糖症は生じなかったことを報
告している。
本発明の更に具体的な目的は、患者の脳神経、特に迷走神経の電気活性の選択的
変調技術を応用して、内生インシュリンの分泌を増加または減少させ、それによ
って、膵臓疾患、特に糖尿病と低血糖症をそれぞれ治療及び制御することにある
。
人体の神経は、一般にA、B及び0群と表示される寸法の異なる数千の線維によ
って構成されていることが知られている。
その線維は、各々、脳から体の他の部分に、及び、体の他の部分から脳に信号を
伝送する。迷走神経は、例えば、3つの異なる型の約ioo、 ooo本の線維
(軸索)を有し、その各々がそのような信号を伝送する。その神経の各軸索は、
正常な状態では1つの方向にだけ伝送する。A線維及びB線維はミニリン化され
ており(すなわち、各々が主に脂肪によって構成されたミニリン鞘を有する)、
一方、C線維はミニリン化されていない。通常、ミニリン化された線維は、ミニ
リン化されていない線維より大きく、より速く電気信号を伝送し、かなり低い閾
値で電気的に刺激される。そのような線維は、特定の幅及び振幅の刺激パルスに
応答して特定の強さ一持続期間曲線を示す。
A線維及びB線維は、比較的狭いパルス幅、例えば、50〜200マイクロ秒(
μS)で刺激される。B線維よりA線維の方が僅かに速い電気伝導性を示し、電
気刺激閾値も僅かに低い。C線維は、相対的にかなり小さく、電気信号を極めて
ゆっくりと伝送し、通常活性化のために広いパルス幅(例えば、300〜100
0μS)と高い振幅を必要とする高い刺激閾値を有する。A線維及びB線維は、
C線維を刺激しなくても選択的に刺激され、C線維を刺激するために必要な大き
さと幅のパルスはさらにA線維及びB線維も活性化させる。
神経線維の電気刺激は、通常、両方向(双方向)に神経信号を活性化するが、特
定の神経電極と刺激波形を使用して選択的な一方向刺激を行うことができる。
ウッドバリー(Wood[1ury)は、ラットの実験的に誘導された発作にお
ける迷走神経刺激の作用についての研究(「層側(Epilepsia) J
1990年、第31号(補遺)、87〜S9)で、迷走神経は、体腔及び内臓の
心性線維(インパルスを脳またはを髄等の神経中枢に伝送する内側に向かって伝
導する神経線維と遠心性線維(すなわち、インパルスをエフェクタに伝送してそ
れを刺激し、活性を生じさせる外側に向かって伝導性する神経線維)によって構
成されていることに注目した。迷走神経線維の大部分はC線維であり、大部分が
頚部の塊すなわち節に位置する細胞体を有する内臓求心性線維である。中央突起
は、線維を脳の様々な領域(例えば、視床下部、視床及び扁桃体)に送る単独路
の核で主に終端している。他のものは、骨髄、小脳、けつ状東核及びその他の領
域等の内側網状形成に続く。
ウッドバリーは、さらに、動物における迷走神経求心性線維の刺激によって、こ
れらの全ての領域で検出可能なEEGの変化が起こり、これらのEEGの変化の
性質と範囲は、刺激パラメータによって決定されることに気づいた。チェース(
Chase)は、また、迷走神経活性化により脳のある部分のEF、G活性化に
影響することができることを観察した(Exp Neurol (1966)1

びある不随意の運動に広範な抑制作用を生じることができることを観察した。
迷走神経の生理学外電気刺激は、既に、癲II+及び様々な形態の不随意運動障
害の治療に提案されている。特に、ジエー、ザバラ(J、 Zabara)によ
るアメリカ合衆国特許第4.702.254号(以下、特許°254と略記)に
は、脳な異常な神経放電パターンによって特徴付けられる*m発作を軽減または
予防するための方法及び埋め込み可能な装置が記載されている。特許゛254は
、埋め込み可能なニューロサイバネティク補綴(Neurocybernet
1cprosthesis) (NCP)を記載しているが、このNCPは、N
CPジェネレータの外部電流を脳の網状体系に作用する特定の群の抑制神経の電
気化学特性に合わせることによるニューロサイバネティクスベクトル識別を使用
する。これらの神経は、他の神経の東向に埋め込まれており、痙彎または発作を
制御するために脳神経放電の状態を増加させるようにNCPを合わせることによ
って、直接的または間接的に、選択的に活性化される。
この特許によると、スペクトル識別分析は、NCPパルスジェネレータのある電
気パラメータは活性化されるのが望ましい神経の電気化学特性に基づいて選択さ
れることを示している。さらに、この特許は、所望の効果を生じさせるためにN
CPジェネレータ出力の印加の最適な位置は、一般的に脳神経、特に迷走神経で
あることを指摘している。
特許゛254に記載されたNCPは、手動または自動的に作動して、発作期間の
間治療を行う。手動作動は、患者が発作の始まりにアウラを感じた時に実行され
る。また、自動作動は、発作の直前または始まりで状態パラメータの瞬間的な変
化の検出によりトリガされる。さらに、予防または防止方法は、N C))が周
期的に作動化され、発作の発生及び/または発作の強さを減少するために使用さ
れる。特許゛254のNCP刺激装置は、患者の胸部に埋め込まれており、迷走
神経に沿って脳により近い位置に配置されたより負の電極と脳から遠い正の電極
と共に神経部位で信号を印加する選択された位置に配置された電極に接続されて
いる。
発明の要約
本発明は、選択的迷走神経変調による全身性膵臓障害及び特に糖尿病を治療する
ための方法及び装置(それらはまた低血糖表の治療にも有用である)に関するも
のである。迷信神経の刺激は、膵臓の小島でインシュリンを生成することができ
る患者において膵臓のインシュリン放出を刺激することによって糖尿病を治療及
び制御するために使用できると考えた。神経は、組織に栄養上の影警を有し、迷
走神経刺激の結果、膵臓のインシュリン合成を増加させることができる。さらに
、迷走神経活性の抑制によってそのような放出を抑制することによって低血糖症
に苦しむ患者の体内で放出されるインシュリン量を減少させることができる。
本発明によって使用する装置は、迷走神経活性の所望の変調を生じさせるために
プログラム可能なパターンの電気パルスを生成する神経刺激装置にューロスティ
ミュレータ)を備える。
本発明の主な特徴によると、糖尿病は、迷走神経の電気活性を変調して、膵臓に
よるインシュリン分泌を増加させる刺激方法によって治療される。現在の好まし
い実施例では、1日の間に1回または複数の規定された回数に患者が神経刺激装
置を作動させる手段を備え、及び/または、刺激は食事時間の後すぐに作動する
ように、特定の患者の生物学的サイクルのリズムによって刺激がプログラムされ
ている。
刺激をトリガする他の技術は、外付けまたは埋め込み型の血中ブドウ糖センサの
使用を含む。迷走神経の電気活性を変調する神経刺激装置は、血液中のブドウ糖
濃度レベルを検出するに適したセンサによって作動されることが望ましい。別の
実施例では、自動的に刺激パラメータを調節するためにプログラマに接続された
外部(体外)テスト装置によって実行される標準的なブドウ糖テスト方法を使用
する。また別の実施例では、そのような検出が可能な埋め込み型センサを体内に
内蔵させる。これは、最近特に開発努力がされている領域であるが、残念ながら
、現在のところ長期間利用できるブドウ糖センサは存在しない。ブドウ糖検出及
び監視の現在の技術状態では、使用可能な期間が約2週間しかない皮下センサを
使用してブドウ糖の変化を測定する外部装置と、より最近になって開発された指
先プローブを介して変化を測定する赤外線センサとがある。しかしながら、糖尿
病患者に現在使用されている最も優勢な技術は、指先を刺して、血液中のブドウ
糖濃度の測定に使用される血液を採取することである。
本発明は、長期間埋め込み型センサが利用できるようになると、それらのセンサ
を使用できる可能性を有し、そのような構成は、手動(患者)でも生物学的サイ
クルによる作動でも好ましい。
神経刺激装置は、必らずではないが、好ましくは、埋め込み可能な型であり、あ
−らかしめ決定されたように患者の迷走神経の電気活性を変調することによって
糖尿病(または低血糖症)を治療するために所望の療法を選択的に採用するよう
に配置及び利用される。この装置は、治療のために望ましい治療法を実行するよ
うに、治療する医師によって最初プログラムされ、上述したように、検出方法の
結果によってプログラミングを変更できるように構成されている。
糖尿病を治療するための迷走神経変調の様々の方法からの選択は、多数のファク
タによる。これらのファクタには、(i)どの神経線維が刺激を受けるべきかの
考慮、(11)生成される生理学的信号の特別な性質、そして変調をトリガする
ために使用されるその検出のための技術、及び/または、(iii)変調後に変
調の利点が維持される「持ち越し」すなわち無反応の期間が起きるかどうか等が
ある。これらは、刺激計画を選択する時に考察すべきファクタの全てでもなく、
必ずしも重要度に応じて指摘したわけではないが、特定の場合に考慮されるもの
を示している。
本発明による治療では、患者の迷走神経の様々な線維を活性化して、その電気活
性を選択的に変調するために、種々の信号パラメータと閾値曲線を使用する。患
者の迷走神経に神経刺激装置によって印加される電気信号のパルス幅と振幅を適
切に設定することによって、神経線維は選択的に刺激され、例えば、B線維でも
C線維でもなくA線維が、または、C線維ではなくA線維及びB線維が、または
、A線維、B線維及びC線維が刺激される。しかしながら、選択過程では様々な
関係するファクタを考察しなければならない。例えば、C線維は極めてゆっくり
と信号を伝送し、この線維は、比較的短い刺激期間後無反応になるので、速い刺
激技術に対する応答性はあまり高くない。
従って、特定の患者でC線維を刺激しようとする時、刺激のために短かいパルス
の列を使用することに注意深くあるべきであろう。長いパルス列の使用は、刺激
の短い期間後線維の無反応性によって無効になり、その結果、より長いパルス列
のパターンを辿ることは不可能である。適切な回復時間後、別の短いパルス列を
印加して、さらに治療を実行する。
この現象は、また、C線維活性を抑制するためにも利用される。この方法では、
高周波数パルスを使用して、神経を「疲労」させ、それによって、その活性を抑
制する。いずれの場合にせよ、使用される正確なパターン、例えば、オン及びオ
フの期間の長さは、個々の患者及び糖尿病または低血糖症のどちらを治療するか
によって決定され、調節される。
迷走神経に印加される変調信号は、興奮性または抑制性ニューロトランスミッタ
放出を生じさせるために神経信号を刺激または抑制し、この明細書の目的では、
両方の状況とも「刺激する」という語に含まれる。迷走神経は、変調信号を印加
するために好ましい神経部位であるが、かなり効果は落ちるが、他の1つまたは
複数の神経に刺激をLlj加することによって糖尿病また低血糖症の治療を行う
ことができ、そのような治療は本発明の範囲内であると考えられる。迷走神経変
調のために神経電極が埋め込まれる特定の部位は、頚部または膵臓に近接した胃
またはその近傍である。
従って、本発明の別の目的は、埋め込まれるものは回路の小さい部分だけであり
、または皮下的に埋め込まれるものは1つまたは複数の神経電極とそれに接続さ
れたリード線だけである、埋め込み可能なまたは体外に配置される神経刺激装置
を使用して、迷走神経の電気活性の選択的変調技術を糖尿病及び他のブドウ糖異
常の治療及び制御に使用することにある。
本発明のさらに別の目的は、患者の迷走神経にあらかじめプログラムされた電気
刺激を印加して、糖尿病及び低血糖症の治療及び制御を目的とする療法の一部と
して選択された神経線維の電気活性を変調するだめの、糖尿病及び低血糖症に特
徴的なまたは関係する検出された兆候に応答する方法及び装置を提供することで
ある。

現在の所好ましい実施例及び方法の説明添付図面を参照すると、神経刺激装置の
刺激ジェネレータの基本構成要素とその相互関係のブロック図が第1図に示され
ており、さらに埋込み可能な型の装置とそれに組み合わされたり−を線/電極装
置との位置の詳細がvJ2図及び第3図に図示されている。本発明の装置に使用
される一般的に適切な形状の神経刺激装置は、本出願と同じ譲渡人に譲渡された
、アンソニージェー、ヴアリキt (Anthony J、 Varrichi
o)その他の名で1989年11月10日に出願された係属中のアメリカ合衆国
特許出願第07/434.985号(以下゛985出願と略記する)に記載され
ている。

、読者の便宜のためのその幾つかの部分が要約して示す。
神経刺激装置は、従来のマイクロプロセッサと他の標準的な電気及び電子機器を
使用し、更に、埋め込み型の装置の場合は、装置の状態を制御または表示するた
めに、非同期シリアル通信によって、患者の体の外部に置かれるプログラマ及び
/またはモニタと通信する。パスワード、ハンドシェーキング及びパリティチェ
ックをデータの完全性のために使用する。神経刺激装置は、また、電池で作動す
る装置ならいずれでも、及び、特にその装置が疾患の医学治療のために埋めこま
れている場合には重要なエネルギーを保持する手段と、装置の偶発的なリセット
を防ぐ等の様々な安全機能を提供する手段とを備える。
刺激ジェネレータ10(第1図)は、好ましくは、患者34の体内に、胸の皮膚
のすぐ下に、または、第2図に図示したように腹腔の皮膚の下に、外科医によっ
て形成されたポケット内に埋め込み可能にされる。所望ならば、その代わりに、
主に外付けの神経刺激装置を使用することができる。神経刺激装置はさらに、刺
激ジェネレータの出力信号を患者の迷走神経に印加するために、リード線22と
共に埋め込み可能な刺激電極(下記に記載)を含む。患者の体外の構成要素は、
刺激ジェネレータに対するパラメータ変化の遠隔測定と刺激ジェネレータからの
信号を監視するためのプログラミングワンド53と、パラメータの調節並びに刺
激ジェネレータとプログラミングワンドとコンピュータとの間の通信の制御のた
めのコンピュータ55及びその付属のソフトウェアとを備える(第2図を参照)
。
そのマイクロプロセッサをベースとする論理及び制御回路と共に、刺激ジェネレ
ータ10は、連続的または周期的に患者の血中ブドウ糖レベルを測定する、分析
回路または装置23と共に使用される検出器を備えるか、または、検出器と共に
使用される。
検出器は、センサ部分63(i2図)が刺激ジェネレータの分析装置に接続され
た静脈リードまたはカテーテルに血管内において配置されてもよい埋め込み可能
な長期間使用可能の血中ブドウ糖濃度モニタ60(そのような装置は、現在の所
入手できないが、本発明と使用可能になる)の形態をとることがある。ともかく
、膵臓によるインシュリン分泌が増加すべきまたは抑止されるべきである範囲は
、その分析から決定され、患者の迷走神経への変調信号の自動的印加をトリガす
ることによって適切な応答が実施される。
その目的のため、そのような埋め込み可能なまたは部分的に埋めこまれる検出装
置が十分に完成され、単純化され、及び/または検出装置のコストとそれらを埋
め込みために必要とされる外科的処置をなくするまで、血液サンプルの採取及び
分析等の従来の方法によって血中ブドウ糖濃度を測定した後、患者が神経刺激装
置装置を手動作動することが好ましい。また、患者が最近開発された型の赤外線
センサを使用することができる。
このセンサでは、指先プローブまたはマンセッテ65を使用してブドウ糖濃度変
化が測定される(第2図)。後者の場合、患者34は、マッセッテ65を指先に
配置し、手動でその内部の光装置(図示せず)を作動させる。そこからの感知信
号は、電気リード線を介して、血中ブドウ糖濃度を決定するためのマイクロプロ
セッサをベースとする手持ち式の分析器/プログラマ70に印加される。分析器
/プログラマ70のサイズは、第2図では、下記に記載するようにその装置を機
能を明らかに示すために他の部分に対して意図的に誇張されている。分析の結果
に基づいて、患者は、膵臓によるインシュリ゛/の分泌を刺激するために神経刺
激装置を作動させる必要を知らされる。
別の実施例は、患者臼らの操作によって手動で、または(例えば、次の)食事時
間に対応する一日の適切な時間での生物学的サイクルのプログラミングによって
自動的に、神経刺激装置を作動させることである。かかる場合、プログラミング
は、患者が選択するように医師によって設定されるか、自動選択は、朝食、昼食
、夕食または軽食などの典型的な食物摂取およびそれに伴うブドウ糖レベルの増
加を考慮して行われる。手動作動の場合、患者はプログラムキー75を押し下げ
、次に、摂取すべき食物の種類と量について適切にラベル付けされた、手持ち式
のプログラマ70上の他のキー78のjつを押し下げる。プログラマを作動させ
る時、プログラミングワンド53は埋め込まれた刺激ジェネレータ10に近接し
て配置される。
刺激ジェネレータは、そのような瞬間の度に膵臓による適切な量のインシュリン
の分泌を刺激し、それによって、糖尿病の治療及び制御を行うための迷走神経の
電気活性を変調する選択的にパターン化された刺激信号を生成するように設計、
構成及びプログラムされている。低血糖に苦しむ患者の場合、迷走神経に印加さ
れるための制御信号は、迷走神経活性を抑制するようにプリセットされ、選択さ
れている。
第1図に図示したように、刺激ジェネレータ10は、埋め込み可能な型の医学電
子装@(埋込み可能な型の心臓ベースメーカまたは細動除去器等)に給電するた
めに従来使用されている信頼性の高い長期間持続型のものである電池(または電
池の組)12を備える。刺激ジェネレータの現在の好ましい実施例では、電池は
単一のリチウムチオニルクロライドセルである。セル12の端子は、電圧調節器
13の入力側に接続されている。電圧調整器は、電池の出力を平滑して、完全な
安定した出力電圧を発生し、特別な用途で必要ならば電圧の昇圧または分圧等の
その強化を提供する。
電圧調節器13は、マイクロプロセッサを備えて装置のプログラム可能な機能を
制御する論理及び制御部15に給電する。これらのプログラム可能な機能とは、
出力電流または電圧、出力信号周波数、出力信号パルス幅、出力信号オン時間、
出力信号オフ時間、迷走神経活性の連続的または周期的変調のための毎日の処理
時間、出力信号開始遅延時間等である。このようなことがプログラム可能である
ことによって、治療法のための迷走神経活性の所望の変調を得ることができるよ
うに、刺激電極の組25(第2図及び第3図)に印加するための出力信号を選択
的に形成することができる。刺激ジェネレータの論理及び制御機能のためのタイ
ミング信号は、水晶発振器16によって生成される。
磁気作動式リードスイッチ14は、電子パッケージに内蔵され、その電子パッケ
ージまたはその埋め込み位置の直ぐ近くに外部磁石を選択的に配置することによ
り、患者がその刺激ジェネレータを作動させる機能を刺激ジェネレータに持たせ
ている。
組み込みアンテナ17によって、埋め込まれた刺激ジェネレータと外部電子機器
(プログラミングワンドモニタ装置の両方を含む)との間の通信が可能であり、
装置がパラメータ変更のためのプログラミング信号を受け、プログラミングワン
ドからまたはプログラミングワンドへ遠隔測定情報を転送することを可能にする
。装置が一旦プログラムされると、その装置は、外部コンピュータ及びプログラ
ミングワンドによって(治療に携わっている医師によって)再プログラムされる
まで、プログラムされた設定で連続的に作動する。
パワーダウン回路18は、電気的にリードスイッチ14及び論理/制御部15に
接続され、水晶発振器16からクロックパルスによってタイミングを合わせられ
て、論理及び制御部15のマイクロプロセッサ及び/または発振器への電力を、
装置が実質的に睡眠状態にあるが命令で目覚めるほど十分には敏捷な点まで減少
させることができる。パワーダウンモードまたは睡眠状態は、言わば、装置がそ
のプログラムされた刺激出力信号を生成するように作動化された後2時間内に自
動的に開始され、2〜5時間の間そのモードのままであることがある。これらの
期間は、特定の患者の必要性に応じて長くなったり短くなったりする。
または、装置は、患者による装置の手動作動によってマイクロプロセッサを目覚
めさせるようにパワーダウン回路が不作動にされるまで、減少した電力状態に留
まるようにすることもできる。CMO3半導体回路内に作り込まれたパワーダウ
ン回路は、集積回路の分野でよく知られており、そのような回路は神経刺激装置
に容易に内蔵される。
ある間隔における装置の減少された電力の要求は、十分な電池の電力を使用を保
証して、そうでない場合よりかなり長い電池の寿命に渡って治療を可能にするこ
とができる。その結果、要求される装置の外科手術による交換の間の間隔は、か
なり大きくなり、装置の寸法は電池の寸法が小さくなったことによってかなり縮
小される。
刺激ジェネレータ10の論理及び制御部15は、適切なプログラムされた信号レ
ベルを生成する出力回路すなわち出力ff1s19を制御する。論理及び制御部
15のプログラムされた出力信号は、刺激ジェネレータケース(ハウジング)2
1上の電気コネクタ20を介して、末端部が刺激電極に接続されたリード線22
に送られる(第2図及び第3図)。本明細書で前述したように、分析回路23は
、刺激ジェネレータハウジング21内に設けることができ、論理及び制御部15
内のマイクロプロセッサと埋め込み型ブドウ糖濃度検出器60に接続された検知
電極への接続を備える。
しかしながら、好ましくは、装置は、通常の方法では血液サンプルの採取及び分
析(図示せず)によって血中ブドウ糖を直接測定した後患者によって、まjこは
、上記の記載した方法のように外部指先マンセッテ赤外線センサ65と組み合わ
されたブドウ糖濃度のマイクロプロセッサをベースとする分析器70との使用に
よって、装置を作動させて、膵臓によるインシュリン分泌を引き起こすようにプ
ログラムされている(糖尿病患者の大部分を占める、膵臓小島でインシュリンを
生成することができる患者において)。別の方法は、選択的に装置を作動させ、
生物学的サイクル毎(または24時間−1日毎)に予め選択された時または複数
の時に、例えば、患者の習慣的な食事時間に続く数分間から1時間の範囲内で、
迷走神経を刺激することである。
自動的な作動は、水晶発振器16によって制御されたタイマによって提供される
。血中ブドウ糖の直接の測定または症状に応答した患者自身による手動作動は、
プログラマ70及びプログラミングワンド53を使用して上記に記載した方法で
実行できる。
埋め込み型装置の刺激信号のパラメータは、特定の患者ごとの必要性に従って遠
隔測定(プログラミングワンドを介して)によって較正され、規定された療法に
従って刺激ジェネレータを作動することによって治療を行うようにマイクロプロ
セッサ内にプログラムされる。刺激ジェネレータは、例えば、著作権登録簿(R
egister of CopyrightsSLibrary of Con
gress)に本出願の譲受人が著作権を保有する種類のプログラミングソフト
フェアまたはこの記載に基づいた他の適切なソフトウェアを使用スるIBM互換
パーソナルコンピュータ55及びプログラミングワンド53でプログラムされる
。プログラミングワンドとソフトウェアによって、刺激ジェネレータが埋め込ま
れた後、その刺激ジェネレータとの非侵入的な通信を可能にすることができる。
プログラミングワンドは、好ましくは、内部電池によって給電され、通信に十分
な電力を表示する「ノ切−オン」ライトを備える。データ転送がプログラミング
ワンドと刺激ジェネレータとの間で行われていることを示すために、好ましくは
、もう1つの表示ライトが備えられている。
刺激ジェネレータ10が内蔵されているハウジング21は、気密式に密封されて
おり1、患者の体の流体及び組&’L!:生物学的に適合するチタン等の従来の
適切な材料によって形成されている。
神経刺激装置の本明細書に記載した疾患を治療するために適したもの以外のさら
に詳細な構造及び動作は、゛985出願に記載されており、参照される。
第2図は、埋め込み型装置の、コネクタ20を備えるケース21内の刺激ジェネ
レータの2つの配置を図示したものである。どちらの場合でも、刺激ジェネレー
タは、患者の胸部に、皮膚のすぐ下に外科医によって形成されたポケット内に埋
め込まれる。
刺激ジェネレータの1つの適切な位置は、ベースメーカパルスジェネレータが埋
め込まれるような患者の胸部内であり、神経電極アレー25とそれに接続された
リード線22は、患者の頚部に埋め込まれる。刺激神経電極の組25(第3図)
は、絶縁された電導性のリード線22の末端部に接続されており、そのリード線
22の基部の端部はコネクタ20に接続されている。電極の組25は、好ましく
は、ブッララ([1ullara)の1986年3月4日に許可されたアメリカ
合衆国特許′iC4,573,481号に記載の型の双極型刺激電極である。こ
の特定の例での電極装置は、患者の頚部の迷走神経27に外科的に埋め込まれる
。2つの電極25−1及び25−2は、迷走神経に巻きつけられており、装置は
好ましくは1990年12月25日にリースニス、テリー、ジュニア(Rees
e S、 Terry Jr、 )に許可され、本譲渡人に譲渡されたアメリカ
合衆国特許第4,979、511号に記載のような螺旋係留鎖によって神経に固
定されている。リード線22は、固定されているが、近傍の組織への縫合接続3
0によって胸部及び頚部の振動により屈曲する能力を保持している。
自己サイジング及び可撓性の電極装置25の開いた螺旋の構成(上記ブック、う
(Bul)ara)の特許に詳細に記載されている)によって、神経の物理的外
傷を最小にし、神経との体液の相互交換を可能にする。電極装置は、神経の形状
に適合しており、より広い刺激接触面積を可能にして低い刺激閾値を与える。構
造的には、電極装置は、電極を構成する2つのプラチナリボンを備え、それらの
電極は、各々3巻きの螺旋装置の最初の2つの螺旋ループ25−1及び25−2
の各々の内側表面に接着されており、2つのリード線は各々導体リボン電極に溶
着されている。各ループの残りの部分は、シリコーンゴムによって構成されてお
り、第3のループは電極装置のための固定用類28として働く。螺旋状双極型電
極装置の内径は、通常、約2mmであり、個々の螺旋的7m口の長さである(神
経の軸線に沿って測定)。
患者の頚部内に神経電極装置を埋め込む代わりに、装置を膵842により近い胃
40上の迷走神経上に埋め込み、迷走神経活性の変調に対する感度を高めること
ができる。電極の組25の埋め込みは、頚部の位置の場合とほとんど同様に実施
される。しかしながら、ここでは、刺激ジェネレータ10は腹腔に沿って配置さ
れている。
埋め込み可能な血中ブドウ糖1度センサが使用できる時、装置は、内部分析回路
23と共に使用される検出器60と同様な埋め込み型検出器を使用して、較正さ
れた閾値レベルとの比較を実行し、過度の血中ブドウ糖レベルが正常(所定の閾
値レベル)に戻るまで徐々に間隔が短くなるように設計された信号で迷走神経を
刺激することによって量が増加するインシュリン分泌を開始させるように配置さ
れる。低血糖症の場合は、治療は、また、迷走神経を抑制して、インシュリン分
泌を抑止するように実施される。
糖尿病及び低血糖症を制御するだめの刺激ジェネレ″−夕10の動作は、第4図
に示した信号波型及びパラメータを参照して説明される。この第4図は、電極装
置25に対して神経刺激装置の出力部19によって生成された出力信号を理想的
に図示したものである。この図面は、主に、出力信号オン時間、出力信号オフ時
間、出力信号周波数、出力信号パルス幅及び出力信号電流または電圧を含む用語
を明らかにするために示されている。糖尿病の治療のための刺激パラメータの適
切な範囲及び刺激出力信号の各パラメータの代表的な値は、下記の第1表に記載
する。
低血糖症の治療のために迷走神経に印加される刺激(抑制)信号のパラメータの
適正範囲及び各パラメータの代表値を下記第2表に示した。

患者の安全性と快適性のために、神経刺激装置には様々な特徴が備えられている
。例えば、快適性は、出力刺激が突然に与えられるよりむしろ刺激の最初の2秒
間で立ち上がるようにブロクラミングすることによって高められる。また、埋め
込み型刺激ジェネレータは、迷走神経に印加される最大電圧を制限する(例えば
14V)ためのクランプ回路を備えることがある。最大限界は、患者の迷走神経
の負傷または外傷を防ぐように設定される。
神経刺激装置のプログラム可能な機能及び能力は、機能の作動化とモニタに有効
な、埋め込み後の刺激ジェネレータとの非侵入性通信を可能にするように設計及
び形成される。装置の主な機能の他に、プログラミングソフトウェアは、直進型
メニュドライブ動作、HE L P機能、プロンプト及びメツセージを提供して
、シーケンスの各段階で起きている全てのことをユーザに完全に知らせながら、
単純且つ迅速なプログラミングを容易にするように構成される。プログラミング
能力は、刺激ジェネレータの調節可能なパラメータとその出力信号を変更し、装
置の診断をテストし、遠隔測定されたデータを記憶及び検索する能力を含む。埋
め込み型装置を調べる時、調節可能なパラメータの現在の状態が外部PCのモニ
タにディスプレイされ、従って、次にプログラマがそれらのいずれかまたは全て
のパラメータを同時に容易に変化させることができることが望ましい。特定のパ
ラメータが変化するように選択された時、そのパラメータに許容できる全ての値
をディスプレイし、その結果、プログラマ(通常、治療に携わっている医師に限
られる)が神経刺激装置に入力するための適切な所望の値を選択することができ
る。
診断のテストは、装置の適切な動作を確認し、通信、電池またはリード線/電極
インピーダンス等の問題の存在を示すために使用される。例えば、低い電池続出
は、電池の寿命の終りが差し迫ってふり、新しい装置の埋め込みが必要なことを
示している。神経電極は、診断のテストについて観察された神経電極に問題の表
示がない限り、無期限に使用が可能である。
ここでは内分泌障害を制御する好ましい実施例及び方法について説明したが、当
業者には上記の記載から本発明の精神及び範囲内で変更及び修正を行うことがで
きることは明らかである。
例えば、完全に埋め込み可能な神経刺激装置を使用する必要はない。代わりに、
電子的なエネルギー発生パッケージを主に体外に置き、刺激は、必要なエネルギ
ーレベルを出力するように構成されたRF電力装置で実行される。埋め込まれる
構成要素は、リード線/電極装置、コイル、直流整流器に限られ、所望のパラメ
ータによってプログラムされたパルスは、RFキャリアと共に皮膚を介して送ら
れる。信号は、迷走神経活性を変調するために迷走神経に対する刺激として印加
されるパルス化された信号を再発生させるように整流される。これによって、実
質的に電池の取替えを排除することができるが、患者が外部トランスミッタを運
ぶ必要があり、作動に必要とされる電力がより大きく、神経に対する出力電流の
安定性が低下するという問題がある。
外部刺激ジェネレータは、埋め込み型神経電極の組に皮下に延びているリード線
と共に使用される。ここで大きな問題は、感染の可能性であるが、そのような一
時的な構成は、特定の患者の内分泌障害の神経刺激装置による制御及び治療が成
功しているかどうかを決定する短い期間の検査を実施するためには有効である。
その結果が決定的または期待できるものであらば、さらに恒久的なインブラント
が提供される。
従って、本発明は、添付の請求の範囲と適用される法の規則及び原則によって要
求される範囲にのみ限定されるものである。
Claims (26)
- 1.患者の異常な血中ブドウ糖濃度を検出し、そのような検出に応答して、その 患者の迷走神経に所定の電気信号を選択的に印加して、その活性を変調して、患 者の膵臓のインシュリン分泌を調節して、血中ブドウ糖濃度を所定の正常範囲に して、内分泌障害を制御する ことを含む、患者の膵臓内分泌障害を治療する方法。
- 2.上記の所定の電気信号は、プログラム可能な信号パラメータを有するパルス 波形であることを特徴とする請求項1に記載の方法。
- 3.上記電気信号は、患者によって手動で開始されることを特徴とする請求項1 に記載の方法。
- 4.上記電気信号は、患者の生物学的サイクルの間に所定の1回または複数の回 数生成するようにプログラムされていることを特徴とする請求項1に記載の方法 。
- 5.血中ブドウ糖濃度は、体外で検出されることを特徴とする請求項1に記載の 方法。
- 6.内分泌障害は、真性糖尿病であり、上記電気信号は、迷走神経活性を刺激し て、膵臓のインシュリン分泌を増加させるように印加されることを特徴とする請 求項1に記載の方法。
- 7.内分泌障害は、低血糖症であり、上記電気信号は、迷走神経活性を抑制して 、膵臓のインシュリン分泌を抑制するように印加されることを特徴とする請求項 1に記載の方法。
- 8.上記電気信号は、患者が摂取する食物の種類及び量に応じた範囲に迷走神経 活性を変調するようにプログラムされていることを特徴とする請求項1に記載の 方法。
- 9.上記のプログラムされた電気信号は、患者によって、手動で選択的に活性化 されることを特徴とする請求項8に記載の方法。
- 10.作動されたとき、患者の迷走神経にプログラムされた電気信号を印加し、 その迷走神経を刺激して、患者の血中ブドウ糖レベルの所定の正常範囲に適切に 膵臓のインシュリン分泌を調節するように応答するプログラム可能な応答性装置 を設け、上記装置を周期的に作動させ、患者の血中ブドウ糖レベルを上記の所定 の範囲に維持する ことを含む、患者の糖尿病を治療及び制御する方法。
- 11.上記の周期的作動は、患者によって手動で行われることを特徴とする請求 項10に記載の方法。
- 12.上記の周期的作動は、患者の生物学的リズムに応じて信号を印加するよう に装置をプログラミングすることによって実行されることを特徴とする請求項1 0に記載の方法。
- 13.上記電気信号は、パルス幅、出力電流または電圧、周波数、オン時間また はオフ時間の少なくとも幾つかを含むプログラム可能な信号パラメータを有する パルス波形であることを特徴とする請求項10に記載の方法。
- 14.ヒトである愚者の膵臓内分泌障害を治療するための装置であって、 作動されると、プログラム可能な電気信号波形を生成するように応答する刺激手 段と、 上記プログラム可能な電気信号波形を患者の迷走神経に伝送するための、上記刺 激手段に電気的に接続可能な埋め込み可能な電極手段と、 上記電極手段と上記刺激手段が電気的に接続されているとき、迷走神経を刺激し て、その電気活性を変調し、それによって、血中ブドウ糖濃度が所定の正常範囲 になるように患者の膵臓による内性インシュリンの分泌を調節して、内分泌障害 を制御するように、上記プログラム可能な電気信号波形を選択したパラメータ値 でプログラミングするためのプログラミング手段と、上記刺激手段を選択的に作 動させる作動手段とを備えることを特徴とする装置。
- 15.上記作動手段は、上記刺激手段を手動で作動させる手段を備えることを特 徴とする請求項14に記載の装置。
- 16.上記作動手段は、上記刺激手段を自動的に作動させる手段を備えることを 特徴とする請求項14に記載の装置。
- 17.上記膵臓内分泌障害は糖尿病であり、上記作動手段は、インシュリン分泌 を刺激するために患者の生物学的サイクルの間に1回または複数の所定の回数作 動させるように刺激ジェネレータをプログラミングする手段を備えることを特徴 とする請求項16に記載の装置。
- 18.上記作動手段は、患者の血中ブドウ糖レベルを検出するセンサ手段と、患 者の血中ブドウ糖レベルが所定の値と異なる時上記刺激手段を作動させるトリガ 手段とを備えることを特徴とする請求項14に記載の装置。
- 19.上記センサ手段は、患者の体内に埋め込み可能であることを特徴とする請 求項18に記載の装置。
- 20.上記刺激手段は、患者の体内に埋め込み可能であることを特徴とする請求 項14に記載の装置。
- 21.上記埋め込み可能な電極手段は、患者の頸部において迷走神経に埋め込ま れることを特徴とする請求項14に記載の装置。
- 22.上記埋め込み可能な電極手段は、患者の胃において迷走神経に埋め込まれ ることを特徴とする請求項14に記載の装置。
- 23.上記膵臓内分泌障害は糖尿病であり、上記手動作動手段は、患者か摂取す る食物の種類及び量に応じてインシュリンの分泌を増加させるように、迷走神経 の電気刺激の複数の異なるレベルを患者が選択する手段を備えることを特徴とす る請求項15に記載の装置。
- 24.上記センサ手段は、愚者の指先で血液中に検出されたブドウ糖レベルに応 答することを特徴とする請求項18に記載の装置。
- 25.上記膵臓内分泌障害は糖尿病であり、上記トリガ手段は、インシェリン分 泌を増加させて、患者の血中ブドウ糖レベルを上記の値に戻すように、迷走神経 活性を刺激する刺激手段を作動させることを特徴とする請求項18に記載の装置 。
- 26.上記膵臓内分泌障害は低血糖症であり、上記トリガ手段は、インシュリン 分泌を減少させて、患者の血中ブドウ糖レベルを上記の値に戻すように、迷走神 経活性を抑制する刺激手段を作動させることを特徴とする請求項18に記載の装 置。
