【発明の詳細な説明】
成長因子による口内結膜炎の治療および予防本発明は、同日付出願のSon i
sの特許出願(発明の名称、急性ヒト化学療法誘発粘膜炎の動物モデルおよび
治療効果の評価におけるその使用、合衆国出願番号、o7/337,988)に
開示された発明に関連するものである。
口内結膜炎は、化学療法および放射線療法のいずれの癌療法にも認められる、珍
らしくない副作用である。粘膜炎は患者にかなりの痛みと不快感をもたらし、重
篤度は発赤および腫張から順序の潰瘍性病変にまで及ぶ。白血病、乳癌のような
癌に対してまたは腫瘍摘出後の補助療法としての化学療法、頭頚部癌に対する放
射線療法、ならびに骨髄移植のための化学療法と放射線療法の併用を受けている
患者か主にかかりゃすい。骨髄移植患者のすへておよび頭頚部癌患者の約半数に
粘膜炎か発症する(それぞれ、年間約10,000例および15,000例)。
長期にわたる5−フルオロウラシル投与か意図される乳癌患者(年間約50,0
00例)でも、この療法か原因で粘膜炎を発症しやすい。化学療法剤および放射
線は口腔を内張すする上皮lH胞を殺滅し、傷害を与える。
この傷害には、化学療法剤か上皮の有糸分裂に対して示す阻害作用も関係する。
その結果、基底上皮の再生速度は低下し、それか粘膜の萎縮性変化、最終的には
潰瘍形成を起こすことになる。傷害の重篤度は化学療法剤の種類および用量、な
らびに併用療法たとえば放射線療法に関係する。さらに、慢性的な刺激の原因、
たとえば欠陥を残した歯科治療、歯の破損および合わない義歯の装着かあると、
潰瘍形成は促進される。
粘膜炎は、癌療法の開始後はぼ1〜2週で、頬、唇、軟口蓋、下の腹面および0
床の非ケラチン化粘膜を冒すことかとくに多い。病変は二次感染を起こすことか
多く、治療はさらに困難になる。現在のところ、粘膜炎には有効な治療法かない
。治療は痛みに対する投薬と二次感染の処置に限られている。とくに、キシロカ
イン、ペンシカインおよびコカインのような局所麻酔剤による処置、−潰瘍性病
変を多糖ゲルで被覆する溶液剤による処置、ならびにクロルヘキソジンのような
消毒液の使用等か薦められてはいる。これらの処置はすべて、ある程度の緩解は
もたらすか、いずれも口内粘膜炎の実際の治癒を指図するものではなく、粘膜上
皮細胞の直接的な治療の課題を残すものである。
化学療法および放射線療法によって誘発される口内粘膜炎の処置には、きわめて
独特な問題慨がある。本明細書て用いられる「口内」の語は、口腔および食道の
両者を意味して使用される。第一に、粘膜表面それ自体の性質かある。本明細書
で用いられる「口内粘膜Jの語は、唇の粘膜皮膚連結部、頬側および唇側粘膜、
歯槽粘膜、a底部、舌の背面および腹面、硬および軟口蓋、後口腔−咽頭、なら
びに食道を含めた意味である。口腔粘膜表面は、完全な角質化を受けない粘膜扁
平上皮からなり、その組織学的構築は独特である。すなわち、角質化された扁平
上皮ではあるか現実に粘膜ではない皮膚組織とは異なり、また粘膜ではあるか扁
平上皮ではなく円柱上皮である胃および腸組織とも異なる。第二に、口腔の性質
かある。舌の可動性と物理的活動は、治療剤を含有する保護材の口腔表面の付着
性を著しく低下させる。歯の存在は絶えず粘膜の外傷を生しさせる。さらに、唾
液の組成はかなり複雑で、たとえば、多数の酵素たとえばリゾチーム、アミラー
ゼおよびリガーゼ、カリクレイン、免疫グロブリン、潤滑剤分子たとえば耳下腺
プロリン富糖蛋白質−アルブミン複合体、亜鉛結合蛋白質たとえばガスチンおよ
びムチン糖蛋白質を含んでいる。さらに、患者の免疫状態によって影響される独
特で絶えず変動する細菌叢か存在する。最後に、口腔粘膜表面を覆って治療剤の
上皮表面への浸透能を低下させる粘液の物理的障壁効果かある。すなわち、口腔
の粘膜組織および環境は、他の上皮表面とは明らかに異なっている。
本発明は、口腔結膜炎の成長因子による予防および治療に関する。創傷の治癒に
対する上皮成長因子の使用は、胃、十二指腸、回腸および結腸組織を含む胃腸管
において広範に研究されてきた。しかしながら、口腔上皮組織に対して行われた
研究は、無傷の粘膜に対する作用を調へるための上皮成長因子の皮下注射にほぼ
限られている(S(eidlerら: Arch 0raI Biol、25
: 37〜43. ]980)。創傷の治癒や、化学療法または放射線療法の結
果として上皮層やその下の間質組織が傷害または破壊された場合の口腔粘膜組織
に関する研究はなかった。顆粒球コロニー刺激因子の静脈内注入か、癌患者で口
腔結膜炎の発症と重篤度を低下させるといわれている(N。
Eng、 J、 Med、318 (2) :1414〜1422. 1988
)。しかしながら、その機構は、根底の上皮または間質細胞に対する成長促進作
用を介するというよりも、末梢白血球の増加、その結果としての二次感染の低下
による効果にあると思われる。
本発明は、化学療法または放射線療法に誘発される口腔結膜炎による病変の処置
に成長因子を予防的および治療的に使用して、粘膜炎の発症を低減、遅延または
阻止するための方法および組成物に関する。処置は局所的でも全身的でもよく、
成長因子は、単独で投与しても、1種もしくは2種以上の成長因子と併用投与し
ても、また治療剤と一緒に投与してもよい。
とくに、本発明は、生物活性ペプチドであり、細胞増殖を引き起こす成長因子お
よびその類縁体の使用に関する。さらに詳しくは、本発明は、上皮成長因子受容
体に結合する成長因子を使用し、その成長因子は6個の必須なンステイン残基を
−Cys −(AA)、−Cys−(AA)、−Cys−(AA)。−Cys−
AA−Cys −(AA) d C’lS −(この場合、AAはアミノ酸残基
てあり、aは7、bは4もしくは5、Cは10、dは8である)のパターンの間
隔て含有する。
本発明はさらに詳しくは、形質転換成長因子−α(TGF−α)の、口腔結膜炎
の予防および処置のための使用に関する。
第1図は、対照およびTGF−α処置動物についての12日間にわたる平均口腔
結膜炎評点(粘膜炎の進展程度の評価)を示す。
第2図は、日6〜■0に重篤な口腔結膜炎を表した動物数を示す。
本発明は、動物とくにその口腔に化学療法および放射線療法で誘発された口腔結
膜炎の治癒を成長因子が急速に促進する可能性を育することを確立したものであ
る。
本明細書において用いられる「成長因子」の語は、細胞増殖を起こさせる生物活
性ポリペプチドを意味し、成長因子およびそれらの類縁体の両者を包含する。こ
れらには、上皮成長因子、形質転換成長因子、神経成長因子、酸性および塩基性
線維芽細胞成長因子、脈管形成成長因子、血小板由来成長因子、インスリンおよ
びインスリン様成長因子たとえばソマトメジン、粘液腫およびワクシニアウィル
ス由来成長因子が包含されるが、これらに限定されるものではない。
したかって、本発明は、その最も広い態様のひとつにおいて、すべての成長因子
を指図するものである。本発明の他の態様は、上皮成長因子受容体に結合する成
長因子を指図するものであり、その成長因子は、好ましい分子量範囲5,000
〜35.000を有し、式ニーCys−(JILA ) 、−CYIK−(AA

るペプチド配列を存する成長因子を指図する。
本発明の池の態様は、式■
(式中、RはAspまたはLysであり、R′はPheまたはTyrであり、R
″はSerまたはPheてあり、R”PheまたはTyrである)のポリペプチ
ド化合物またはそのオリゴマーを包含し、形質転換成長因子特性を有する、特定
の群の成長因子を指図するものである。
本発明のさらに他の態様は、式■においてRかAspてあり、R′かPheであ
り、R″かPheてあり、R′かTyeである配列を有する形質転換成長因子−
α(TGF−α)の使用を指図するものである。本発明の方法および組成物に使
用されるTGF−αは米国特許第4.816,561号に開示されている既知化
合物である。この特許明細書の開示を参考として本明細書に導入する。
成長因子は治療的には存在する病変の処置に可動な量を投与することかでき、予
防的には口腔結膜炎の発症前に病変の発症を遅延させるかまたは完全に防止する
のに可動な量を投与することかできる。本発明の好ましい使用は予防的処置であ
る。有効量は0.01−100μg/回、好ましくは0.1〜50μg/回の範
囲内てあり、上記用量か1日2〜4回投与される。
本発明は、成長因子の単独でのまたは他の成長因子との併用での両使用を意図す
る。適当な併用には、上皮成長因子とインスリン様成長因子、形質転換成長因子
−αおよび形質転換因子−β、ならびに血小板由来成長因子とインスリン様成長
因子が包含されるか、これらに限定されるものではない。
使用は全身的でも局所的でもよいか、好ましい実施態様は局所投与である。本明
細書において用いられる「全身的」投与の語は、成長因子を医薬的に許可される
非毒性ビヒクルまたは担体中、創傷部位から離れたある部位に、成長因子か血流
を介してその創傷部位に送達されるように投与することを意味する。全身的投与
は、粘膜表面および口腔の性質に伴う問題を克服する必要かないという利点かあ
る。成長因子は、成長因子とともに非経口投与に適した任意の慣用の医薬的に許
容される非毒性ビヒクルもしくは担体たとえば生理食塩水、または粘膜炎の予防
もしくは処置に適当な血清レベルを維持する成長因子の徐放を実現するように適
用されたポリマーマトリックス材料からなる組成物の形態で全身的に使用するこ
とかできる。この組成物は、静脈内に、筋肉内に、または他の任意の適当な様式
での経皮的導入によって投与できる。
本明細書において用いられる「局所的」の語は、成長因子の直接的な創傷部位へ
の投与を意味する。これは多くの方法で遂行できる。成長因子を含有する液体溶
液または懸濁液はマウスウオツツユと類似の方法で使用でき、この場合、液体は
病変の処置を最大限にするように口内の周囲で含撤させる。粘膜表面との接触を
長く(するには、粘膜の被覆か可能な適当なビヒクルを選択できる。代表的な例
はペクチン金回組成物、たとえば0raba、se ”(Colgate−Ho
yt Laboratories、Norwood、MA) 、スクラルフェー
ト懸濁液、カオペクテートおよびマグネシア乳剤である。組成物は、医薬的に許
容される非毒性ビヒクルまたは担体を含む、拡散可能なりリーム、ゲル、ローシ
ョンまたは軟膏とすることもてきる。本発明の成長因子はまた、緩徐溶解性のロ
ゼンジもしくはトローチ、チューインガムベース、またはバッカルもしくは奥歯
に掛けた徐放性の補綴に、たとえば導入することもてきる。
本発明はまた、成長因子とともに他の治療剤を投与することも意図する。成長因
子は病変の治癒を促進する機能を存するか、口腔結膜炎に伴う痛みを緩和するた
めには、鎮痛剤および麻酔薬のような治療剤を投与することかできる。さらに、
病変の二次感染を予防または処置するために、抗感染剤、抗細菌剤、抗かび剤お
よび消毒剤を投与することもてきる。
口腔における化学療法および放射線療法の急性作用を研究するための動物モデル
かBrigham and iVomen’5Hospita1. Bosto
n、 Massachusettsの5tephen T、5onis博士によ
って開発され、係属中の相互に参照された特許出願に開示されている。このモデ
ルはとくに、化学僚法粘膜炎に対する成長因子の有効性の試験に適していた。
このモデルでは、結膜炎を誘発するために、5−フルオロウラシル(5−FU)
の急性投与基準と機械的刺激を併用する。さらに詳しくは、5−フルオロウラシ
ルの腹腔内(i、p、)注射に粘膜表面の機械的刺激をゴールデンノリアンハム
スターに併用し、潰瘍性結膜炎で特徴づけられる口腔粘膜の臨床的破壊を生成さ
せた。臨床的および組織学的評価によって、これらの変化かヒトに認められる変
化に類似すること、また骨髄抑制の程度によって影響されるパターンをたどるこ
とか明らかにされた。
これらの理由から、このモデルは成長因子による結膜炎の処置の評価にきわめて
有利と考えられた。
ハムスターにおいて、5−FU投与によって誘発される結膜炎の予防および治癒
に対する成長因子とくにTGF−αの効果を測定するため、以下の実験を実施し
た。
若齢の雄ゴールデンシリアン/%ムスター100匹を各群25匹の4群に分けた
。日0から開始して、動物に182回、頬袋の頬側粘膜に以下の薬剤投与を実施
した。

a CC20u/回)投薬は各頬袋当たり0. 27nl容量の0rabase
中で適用された。右側の頬袋は日0に開始して毎日、紅斑性の変化か認められる
まで表面を引っ掻いて機械的に刺激した。
大部分の動物で、このよ・うな変化は日2まてに認められ、機械的刺激は中止さ
れた。残りの動物については、日3に機械的刺激を中止した。日0に開始して、
全動物に5−FU 60■/kg体重の1 p 注射を行い、さらに日5および
10にもさらに付加投与を行った。動物は毎日観察し、秤量した。日2.4.6
.8.10.12゜1.4,16.18および20に、各田無作為に2匹の動物
を屠殺した。頬袋の標本を採取した。ついて組織を10%ホルマリン中に漬けて
確立された方法での組織学的検に付し、次にヘマトキシリンおよびニオシンで染
色した。以下のパラメーターについて評価し、採点した。すなわち、炎症性浸潤
、上皮萎縮の程度、上皮破壊/潰瘍化の程度、創傷縁における育糸分裂活性およ
び表面/二次感染である。各群から無作為に2匹のハムスターを選択し、毎日そ
の頬袋を写真撮影した。動物は毎日観察し、秤量した。全群について平均15〜
20%の体重減少か認められた。結膜炎は以下の確立された尺度を用いて採点し
た。
重症度O4潰瘍または結膜炎の形跡なし重症度l:紅斑たたし潰瘍化なし
重症度21個もしくは2個の小さな(2<28)単一潰瘍
重症度31個の大きなもしくは3個以上の小さな潰瘍
重症度4 多数の大きな潰瘍および/または頬袋粘膜表面積の50%の罹患
動物については、結膜炎の発症の遅延または重篤度の低下を起こさせるTGF−
αの能力についても評価した。
本明細書に示したデータは臨床的観察に基つくものである。生検頬袋標本の組織
学的評価は、これらの観察結果を確証するものであった。
結膜炎の重症度を全4群について18日間にわたり平均したところ、TGF−α
を投与されていた群で低し1総評屯を示し、したかってTGF−αは結膜炎の重
症度を低下させることか確立された。最善の結果は、低用量群の群■て認められ
た。日0〜12のデータを図1に示す。
日12〜日18には、全群で結膜炎は消退した。
重症度3および5の結膜炎の発症は数日間にわたって認められ、TGF−αか結
膜炎の発症を遅延させたことか見出された。これは図2のデータから明らかであ
る。

成長因子による口内粘膜炎の治療および予防口内粘膜炎は、化学療法および放射
線療法のいずれの癌療法にも認められる、珍らしくない副作用である。粘膜炎は
患者にかなりの痛みと不快感をもたらし、重篤度は発赤および腫張から顛症の潰
瘍性病変にまで及ぶ。白血病、乳癌のような癌に対してまたは腫瘍摘出後の補助
療法としての化学療法、頭頚部癌に対する放射線療法、ならびに骨髄移植のため
の化学療法と放射線療法の併用を受けている患者か主にかかりやすい。骨髄移植
患者のすへておよび頭頚部癌患者の約半数に粘膜炎インスリン様成長因子からな

Claims (1)
