【発明の詳細な説明】
グルタチオンのエーロゾル製剤及び肺におけるグルタチオン濃度を増加する方法
技術の分野
本発明は一般に、肺の障害の管理および治療に関する。
特に、本発明は、吸入によるグルタチオンの直接エーロゾル投与による下部の気
道組織中の還元されたグルタチオン(GSH)濃度を増加することにより、肺の
機能不全、障害または疾病を治療することに関する。
発明の背景
肺において、GSHは下部の気道の上皮内液(ELF)中、高濃度で存在し、ヒ
トELF中の標準濃度は血漿中の標準濃度より40倍以上も高い。ELF GS
Hは、炎症性細胞により放出されたオキシダント、並びに吸入されたオキシダン
トから肺の上皮を保護する酸化防止スクリーンの主成分である。さらに、ELF
GSHは、肺上皮の宿主防御システムの免疫成分の標準機能を維持する手助け
をする。しかし、突発性繊維症、及びAIDS患者のようなある条件において、
実質的なELF GSH欠乏症であることが見い出された。
肺のGSH濃度の増加における問題は、GSHの経口投与が肺でのGSH濃度の
有為な高さに届かず、GSHの静注投与が分子の非常に短い血漿半減期と関連す
ることである。
それゆえ、本発明の目的は、肺にGSHを届ける新規で効きめのある方法を提供
することである。
本発明の他の目的は、ヒトの肺に直接投与するためのGSHの実質的に無毒のエ
ーロゾル組成物を提供することである。
本発明のその他の目的は、肺へエーロゾルGSHを直接投与することにより、増
加されたGSHa1度が有益であると考えられる肺の機能不全、障害または疾病
を治療する方法を提供することである。

液(ELF) 、及び尿中のGSH濃度は、静注でのGSH投与の前、および後
のある間隔で評価された。各データ点は11匹の羊での二重測定の平均値を表わ
す。
第3図は、グルタチオンのエーロゾル化の効果を評価したものである。エーロゾ
ル発生器の発生物は、リン酸エステル緩衝塩水中に集められた。エーロゾル前の
グルタチオン(″前″)及び部屋の空気で発生されたエーロゾル(”後”)のた
めの還元形態の全グルタチオンの比率が示されている。各データ点は三重測定の
平均値を表わす。
第4A図から第4D図は、600■のGSHをエーロゾル投与した後の、羊にお
ける還元されたグルタチオン(GSH)濃度を示す。肺上皮内液(ELF) 、
肺リンパ液、静脈血漿、及び尿中のGSH濃度は、エーロゾルでのGSH投与の
前、および後のある間隔で評価された。
各データ点は12匹の羊での二重測定の平均値を表わす。
第5図は、GSH投与前、及び3日間毎日2回エーロゾルにより600■のGS
Hを投与した1時間後に、2人のHIV感染した、徴候のない患者の肺における
GSH濃度を示す。
発明の詳細な説明
本発明の上記の種々の目的および利点は、吸入によりヒトの肺へ直接投与するの
に適当なGSHの無毒なエーロゾル組成物により達成される。GSHのエーロゾ
ル組成物は、吸入により肺へ安全に投与するのに適するエーロゾルの形態で、薬
剤的に許容可能な担体中に有効量のGSHを含む。
別な方法で定義されなければ、ここで使用された技術的および科学的な用語のす
べては、本発明の属する分野の当業者により一般に理解されるのと同じ意味を有
する。
ここで記載されたものと類似または等しいいずれの方法および材料が、本発明の
実施または試験で使用されうるにもかかわらず、好ましい方法および材料を記載
している。以下に記載された全刊行物は参照文献としてここに導入される。別な
方法で記載されなければ、ここで使用され、または行われた技術は、この分野の
当業者によりよく知られた標準の方法論である。材料、方法および実施例は例証
であるだけで、制限するものではない。
ここで使用される用語”エーロジノじは、空気中に懸濁化された液体の小滴を意
味する。小滴の大きさは、GSHを含むエーロゾルが使用されるべき臨床条件に
必要と考えられる上部の、及び下部の気道におけるエーロゾルの沈着に適当であ
ると理解される。
材料および方法
グルタチオン製剤
グルタチオンの還元形態は遊離酸(組織培養段階;Sigma)として得られ、
4℃で貯蔵された。各実験前の、この製剤中の還元されたグルタチオンのパーセ
ントは常に〉96%である(以下を見よ)。
静注チオベンタールナトリウムで麻酔された。静注カテーテルは頚静脈中、及び
後ろ足の静脈中に設置され、カテーテルは膀胱に挿入された。気管は、正圧ベン
チレーターに取り付けられた、折り返し付きの気管内チューブで挿管された。動
物は16/分の速度におけるH2O正の最終呼気圧5cmを有する体重12m1
’/kgの一回換気量で換気された。
心電図および吸気圧は研究の間モニターされた。麻酔は1.0−1.5%のフル
オツン及び50%の酸素で継続された。肺リンパ液は、ヘパリン処理されたシラ
スティックなカテーテルでカニユーレ挿入された尾部の輸出性リンパ管から、S
taub等の技術(1975゜J、 Surg、 Res、 19,315−
320) 、肺の間隙を排液する肺リンパ液の選択的サンプリングを可能にする
方法により集められた。
下部の気道ELFは、0.9%のNaCl1の単一な50一部分を使用し、光フ
ァイバーの気管支鏡(長さ:1m、外径: 5 、 3 in ; Machi
da)を通して気管支の肺胞の洗浄により得られた。血液試料は頚静脈中のカテ
ーテルから得られ、そして尿試料は膀胱カテーテルから得られた。静脈血漿、肺
リンパ液、ELF及び尿は、GSH投与の前、そして600■のGSHの静注ま
たはエーロゾル投与の後180分までの間で、得られた。
23匹の羊の全てが評価された。各実験セットで使用された羊の数は、データで
示される。全ての静注およびエーロゾルの研究は、閉じられた胸およびうつ伏せ
状態の動物で行われた。基準の気管支肺胞の洗浄液、血液、肺リンパ液、及び尿
の試料が得られた後、4Tn1の塩水に溶解された600■のグルタチオンは、
後ろ足の静脈中への静注によるか、または25分間かけて換気回路の吸息性脚へ
のエーロゾルによるどちらかで投与された。
その後、気管支肺胞の洗浄液、血液、リンパ液、及び尿の試料は3時間かけて種
々の間隔で得られた。異なる時点の洗浄試料は、異なる部位で得られた。全ての
測定は全く同一に行われた。リンパ液、血液および尿の値は、分析された液体容
量に関係する。下部の気道の値は、尿素法により評価されながら、回収された上
皮内液の容量に関係する( Rennard等著、1986゜J、 Appl、
Physiol、 60.532−538)。
全データは、平均値の平均上標準誤差として表され、全ての統計的な比較は、学
生用の両側を検定を使用して行われた。
エーロゾル発生系
還元型グルタチオンを、肺胞領域における沈着のために適当な大きさのエーロゾ
ル小滴の発生可能な噴霧器(Ultravent 、Mallinckrodt
社)を使用して、下方気道への到達が可能な形態にする〔バーバードら(Hub
bard et al)、1989.Proc、 Natl、 Acad、 S
ci、 USA 、 86,680−684〕。還元型グルタチオンを含む二一
口ゾルを発生させるため、0.9%NaC1中150mg/ml濃度のGSH溶
液溶液4奢l霧器のレザーバーに入れ、そして噴霧器を圧縮空気を使用して40
psiで運転する。レーザー粒子径分析により測定されたエーロゾル小滴の寸法
分布は、1.3μmの幾何学標準偏差を有する2、8μmの小滴のマスメディア
ンエアロダイナミック径(massmedian aerodynamic d
iameter)を示す。還元型のままのGSH製剤の相対的割合は、既に示さ
れている(バーバードら、上記)ようにpH7,4のリン酸緩衝された生理食塩
水中にエーロゾル化された小滴を収集することによって評価される。
グルタチオン濃度および形態
静脈プラズマ、肺リンパ、気管支肺胞洗浄液および尿におけるグルタチオン濃度
は標準的な方法の僅かな改良により定量される〔キャンティンら(Cantin
et al)、1945−451 、マーテンソンら(Martensson
et al)1989. Proc。
Natl、 Acad、 Sci、 USA、86.5296−5300 )
、手短にいえば、総グルタチオン濃度〔即ち、還元型グルタチオン+グルタチオ
ンジスルフィド(GSSG))を測定するため、それぞれ溶液を等量の17.5
mMのエチレンジアミン四酢酸(EDTA)を含有するpH7,5のO,1Mリ
ン酸カリウム中の10mM5.5’−ジチオビス(2−ニトロ安息香酸)(DT
NB)と混合する。試料を遠心分離しく2000g、10分間)、および上滑の
一部分(50μm)を5mM EDTAを含有するpH7,5の0゜1Mリン酸
カリウム中の、0.5U G55Gリダクターゼの入っているキュベツトに加え
る。25°C11分間のインキュベーションの後検定反応を、最終的に1mlに
なるように、5mM EDTAを含有する、pH7゜5の0.1Mリン酸カリウ
ム中の220nM還元型β−ニコチンアミドアデニンジヌクレオチドリン酸(N
ADPH)を加えて、開始する。DTNBの減少割合を412nmの波長におけ
る分光光度計(Beckman社製、DU−70型分光光度計)を使用して記録
する。総グルタチオン濃度の測定はpH7,4、リン酸緩衝生理食塩水中の既知
の濃度のG55G (0,125ないし4μM)から作成した標準曲線に基づく
。
グルタチオンジスルフィドを定量するために、それぞれの溶液を回収後直ちに、
等量の17.5mM EDTAを含有するpH6,5の0,1Mリン酸カリウム
中の10mMN−エチルマレイミド(NEM)と混合する。
試料を次に遠心分離しく2000g、10分間)、そして上清250μmを、メ
タノール3ml、続いて蒸留水3mlで洗浄した5EP−PAK C18カート
リッジ(Waters As5ociates製)を通過させ、そして流出分を
集める。G55Gは5mM EDTAを含有する、pH7,5の0.1Mリン酸
カリウム1mlでカラムから溶離される。集められた流出分および溶離分の一部
分(750μm)を5mM EDTAを含有する0、1Mリン酸カリウム、pH
7,5,250μmと混合する。800μM DTNB、2 U/m 1グルタ
チオンレダクターゼおよび1mM NADPH,およびDTMBの減少割合を4
12nmで分光光度計で記録する。標準曲線を10mM NEMと混合し、そし
て上記に示した様に5EP−PAK C18カートリッジでクロマトグラフ化し
た既知の濃度のG55G (0,125ないし4μM)の希釈液から得る。標準
曲線の範囲より上の値が見出された場合は、0.9%NaC1で希釈した試料の
平行実施(Parallel run)を行う。GSHの量は総グルタチオン濃
度からG55G量を差し引(ことによって得られる。
総グルタチオンおよびG55G分析の両方に対して、それぞれの溶液から生じた
標準曲線は平行であり、即ち、全ての溶液からの値は類似している。
葭玉
羊におけるベースライングルタチオン濃度#脈プラズマおよび肺リンパにおける
還元型グルタチオン濃度は、プラズマ濃度が僅かに高い(p>0.1゜図1)が
、同様の範囲である。尿濃度は非常に低い。著しくは、ヒトにおけるように〔キ
ャンティンら(Cantinet al)、既出:プールら(Buhl et
al)、1989.Lancet II。

これらのプラズマまたは肺リンパ濃度より数倍高い。
プラズマおよび肺リンパ両方においては、GSHは総グルタチオン〉95%(プ
ラズマ97±2%還元型;リンパ95±3%還元型)の割合を占める。ELFに
おいてはベースライン総グルタチオンの75±4%は還元型であるが、尿におい
ては59±6%が還元型である。
グルタチオンの静脈内投薬
肺上皮内液のGSH濃度を増加させる可能な方法としての静脈内経路を評価する
ために、600mgGSHの各投与量を羊に静脈内投与する。プラズマにおける
GSH濃度は注射によって増加するが、45分でベースラインに戻る(p<0.
05、ベースライン15分間と比較、他の値のすべてp>0.1;図2Aから2
F)。GSHの幾らかは肺に分散し、一時的に肺リンパ濃度を高める(pro、
05.15分間、ベースラインと比較)が、プラズマGSHのように30分で値
がベースラインになる(p>0.1.15分以外の全ての時間)。しかし、肺上
皮内液値の平均は多様であるがこれは観測期間に顕著な変化がない(p>0.1
、全てベースラインに比較)。予期された通り、尿排泄物は15分には迅速に高
濫度を示す(p<Q、02.ベースラインと比較)。これはまたその後はベース
ライン値になる一時的なものである(p>0.1)。プラズマ、リンパ、ELF
および尿中の総グルタチオン濃度の評価は還元型グルタチオンのパターンに類似
のパターンを示す(データは示さない。
)。全体として、これら結果は静脈内経路が肺の上皮面へのGSH配達に有効な
方法でないことを示している。
GSHにおけるエーロゾル投与の効果
下方気道へのGSHを配達するためのエーロゾル投与する使用のためには、少な
くとも3つの主な規準が満たされなければならない。第一にGSHの溶液は肺胞
表面上への沈着に最適な寸法の小滴からなるエーロゾル中になければならない。
この必要条件はこれらの研究で使用されたUltravent噴霧器で満たされ
るが、他の噴霧器または通常の当業者によく知られているような同様の特性を持
つ二一口ゾルを作るものによって行うことができる。
第二に、エーロゾルはGSHを変化させるべきではな(、即ち還元されたおよび
それゆえ機能的なままでなければに適当であるように、非毒性であるべきである
。体外でのエーロゾル化されたGSHの評価はエーロゾル化方法自体グルタチオ
ン分子の構造を変えないことを示した。
これに関しては、総グルタチオンのエーロモ弗化前製剤は98.2±0.1%還
元型グルタチオンを含み、一方、集められたエーロゾル化後の製剤は97.0±
0.6%の還元型である(P>0.1)。全体として、これらの結果はエーロゾ
ルが肺の肺胞領域に達するのに最適な寸法の小滴内にある充分機能する還元型グ
ルタチオンからなることを示している。
グルタチオンのエーロゾル投薬
エーロゾルにより25分かけて羊(n=12)に投与された0、9%NaC14
ml中の還元型グルタチオン600mgの各投与量は30分内に下方気道の上皮
内液中のGSH濃度を7倍を越えて増加させる(p<0゜001、エーロゾル化
30分後に対して比較したエーロゾル化前;図4)。ELFのGSH濃度は2時
間まではベースラインよりも大きい状態のままである(p < 0゜01、エー
ロゾル化60分および90分後、両方についてベースラインと比較)。肺リンパ
の濃度は30分においてのみ一時的に増加するが、ベースラインとの著しい違い
はなく (p>0.1)、そして静脈プラズマおよび尿の濃度はエーロゾル化に
より著しく変化しない(p>0.1、全てベースラインと比較)。対照として、
生理食塩水のみをエーロゾル化した。ELFのGSH濃度は3時間にわたりベー
スラインから変化していないままである(pro、1、全て比較)。
ELFのGSH濃度が著しくベースラインより上になる各々の時点では、ELF
における総グルタチオン濃度は還元型グルタチオンよりも高い濃度を示す。例え
ば30分においてELFにおける還元型グルタチオン濃度は総グルタチオン濃度
の65±7%である(pro、001)。全体にわたり、60.90.120分
において還元型グルタチオン濃度は、総グルタチオン濃度の62ないし65%で
ある(p<0.005全て比較)。エーロゾル化工程はそれ自体はGSHの相対
的割合をエーロモ弗化前製剤におけるそれからから変化させないし、(図3)、
回収されたGSHの割合も減少しないので、多分羊肺の上皮表面上に衝突した時
、GSHは酸化剤負荷と遭遇したものと思われる。しかし、この観察とは別に、
GSHのエーロゾル化がELFのGSH濃度の顕著な増大を起こすことは疑念を
越えて明らかである(114Aないし4D)。
全体として、これらのデータはグルタチオンのエーロゾル投与が実行可能であり
および下方気道のELFへの還元型グルタチオンを目的とするための有効な方法
であることを示している。療法の安全に関しては、平均およびピークの吸気圧は
手順の間中安定のままである。GSHのエーロゾル投与は、気道の粘膜の視覚検
査により判断されるように、気道下部の炎症を引き起こさない。さらに、エーロ
ゾル化前および後の気管支洗浄液のELF容積の測定により決定されるように、
上皮関門の明らかなr漏れ1はない(p>0.1、全てベースラインに対する比
較)。
ヒトにおけるエーロゾル化されたグルタチオンの研究例えば特発性疾患性肺繊維
症(IPF)であるような患者において、ELFグルタチオン濃度は約4倍低(
、IPFに見られる増加したオキシダント負荷の状況において肺表面における著
しい酸化剤−抗酸化剤欠乏のあることを示している(キャンティンら、既出)。
同様に、GSH欠乏がHIV−漿液陽性(HIV−sero−positive
)の患者で見られている。IPFの患者のおよびHIV−漿液陽性の患者の下方
気道の上皮表面の欠乏性抗酸化剤スクリーンと共に炎症性の細胞から誘導された
増加された酸化剤負荷がある。全体として、これらの作用はこれらの患者におい
てみられる上皮損傷の役割をしているようである。肺におけるGSHの増加した
濃度を投薬する他の状態は望ましくは嚢胞性の繊維症、急性のおよび慢性の気管
支炎、成人呼吸困難症候群、細菌性ならびにウィルス性肺炎、日和見性感染、マ
イコプラズマ、レジオネラ、マイコバクテリアおよび真菌性感染を含む肺感染の
全ての形態等を包含するものと考えることができる。
本研究は以下の主な目的に従って行われる。
1、特発性疾患性肺繊維症またはHIVウィルスによる漿液陽性患者の患者への
エーロゾル化したGSHの増加する量の投与の安全性を示すため、病歴ならびに
身体検査、化学的作用、血漿学的および凝固プロフィールおよび肺機能を監視し
、ならびに肺における全ての異常な炎症性反応を評価するため気管支肺胞洗浄を
使用すること。
2、これらの疾患に対し臨床的に効果的であるらしいグルタチオンの投与量を決
定すること、それはエーロゾルによる肺への安全な投薬および次に気管支肺胞洗
浄を利用した下方気道のGSHのバイオアベイラビリティ−を評価できる。
A、パート1.GSHの増加する投与量の評価。各々の患者は4回の連続的な(
受は入れ難い毒性のない)投与量濃度で、4回の連続的な投与に対して24時間
ごとに還元型グルタチオンの各エーロゾルを受ける。
投与量管理の表を以下に示す:

4 600mg
注目すべきは、しかし、投与量は患者にとって実質的に薬害反応のない耐許容性
のあるGSHのいずれの濃度にも調整できることである。このため2.500m
gまでまたはより高い投与量はこのような高い濃度が患者に好ましい結果を生む
かどうか内科医師により検討され得る。
すべての患者は10mgでの処置から始め、もし段階II毒性または段階III
毒性を伴わない場合は各々連続的な処置において投与量を拡大できる。2人の患
者までが同時に処置できる。いずれかの患者において毒性が起こった場合、次の
投与量の拡大が生じ得る前の投与濃度において、4人の追加の患者が処置される
。毒性が第二の患者に起こった場合は、投与量の拡大は毒性源が決定および除か
れるまで行われない。
B、パートII−GSHのバイオアベイラビリティ−の評価
各々の患者は600mgまたは最大許容投与量を12時間、6回の投与量に分け
て受ける。
C,GSHの投薬
各々の患者は特別な投与量におけるエーロゾル化したGSHを受ける。エーロゾ
ル投薬中、すべての薬害反応を管理するため内科医が付き添う。
エーロゾル化したグルタチオンの投薬の使用方法。
人、希釈賦形剤
希釈賦形剤は滅菌0.9%塩化ナトリウムである。
B、投薬説明
バイアルを使用直前に再溶解しそして過剰公金ては廃棄する。分けたグルタチオ
ンのバイアルを患者の投与量毎に配分する。各々の事例における望ましい投与量
は以下に示すように再溶解溶液4mlである。
10mg : 20[Dlで再溶解した50mg /)くイアル50+ng :
4mlで再溶解した50mg / /クイアル200mg : 12m1で再
溶解した600mg / ノ<イアル600mg : 4mlで再溶解した60
0mg /ノくイアル再溶解に従って、溶液4 m lを空気エーロゾル発生器
(Ultravent ;Mallinckrodt社製)のレザーノ(−にし
Aれる。噴霧器を圧縮空気を使用して4“Ops iで運転し、10リットル/
分のエーロゾルを発生させる。一方向)くルブ、鼻クリップおよびマウスピース
の一連を使用して系を閉鎖する;系は吸いこまれまたは吐き出されたすべてエー
ロゾル化されたGSHを含む気体であり、フィルターを通して全て吐き出された
薬を回収する。

図5はHIVウィルスに対して漿液陽性の2患者〔しかしながら無症候性、即ち
、かれらは【ブレーエイズ(pre−AIDS) Jである〕に対し、グルタチ
オンのエーロゾル投与をおこなった結果を示している。図5に示されるように、
これら2患者はグルタチオンエーロゾル600mgを日に2回、3日間、計6投
与量受けた。肺上皮内液はいずれも療法以前、次に6回目の投与後に本明細書に
上述した方法に従って、気管支肺胞の洗浄により評価される。両患者は通常の範
囲以下の肺液内のグルタチオン濃度で開始したが、療法により著しく増加し、即
ち「正常な1欠乏状態になった。副作用または毒性作用は処置した患者には見ら
れなかった。
要約すれば、動物およびヒトの両方の研究から、GSHのエーロゾル化した投薬
により肺のELF GSH濃度は著しく増大されたことは明らかである。ここで
記述された実施例および具体例は説明の目的のためだけであり、ここでの見解に
おける多様な改良または変法が当業者から提案されるであろうしおよびこの発明
の精神および範囲ならびに付加した請求項の範囲内に含まれるものと理解すべき
である。

Claims (6)
- 1.吸入により肺へ安全に投与するのに適するエーロゾルの形態で、薬剤的に許 容可能な担体中に有効量の還元されたグルタチオンを含む、還元されたグルタチ オン(GSH)のエーロゾル。
- 2. 無菌のGSHを約10mgないし約2, 500mg、塩化ナトリウム溶 液を約0.9%〔容量に対する重量の比〕を含み、無菌空気のエーロゾル発生貯 蔵器中に入れられ、そのため、該GSHのエーロゾルが約30−50psiの圧 縮空気において約8−12リットル/分の速度で作られる請求の範囲第1項記載 のエーロゾル。
- 3.約40psiの圧縮空気において約10リットル/分で作られる請求の範囲 第2項記載のエーロゾル。
- 4.肺への吸入により請求の範囲第1項記載のエーロゾルを投与することからな る、肺における還元されたグルタチオン濃度を増加する方法。
- 5.肺への吸入により請求の範囲第1項記載のエーロゾルを、肺中のGSH濃度 を増加させるのに十分な量で投与することからなる、GSHの増加された濃度が 望ましいと考えられる肺の機能不全、障害または疾病を治療する方法。
- 6.肺の機能不全、障害または疾病を治療するため、肺中のGSH濃度を増加さ せるのに十分な量の請求の範囲第1項記載のエーロゾルを使用する方法。
