【発明の詳細な説明】
【０００１】
【産業上の利用分野】本発明は、補綴関節窩部材，　特
に股関節窩の補綴部材に関するものである。
【０００２】
【従来の技術】股関節を代替するための補綴装具（プロ
テーゼ）は、従来からよく知られており、関節球体を支
えるための関節窩部材が、大腿骨に取り付けられるもの
である。中空外部シェルおよび内部シェルからカップ（
杯状窩）部材を形成し、関節球体を収容するための凹部
を内部シェルに形成することは、ドイツ特許第２３０１
８０１号により周知である。単一の力が作用するため、
該カップ部材は、大腿骨中でゆるみぐらついてくる。ヨ
ーロッパ特許第０３０３００６号には、プロテーゼ中で
の骨材の体内成長を向上させるためにカップ部材の外部
表面に複数の小さな球体を取り付ける方法が開示されて
いる。またヨーロッパ特許第０３２９０１９号では、大
腿骨にネジ込む外部シェルと内部シェルから構成される
カップ部材を開示している。該外部中空シェルには、ス
ポンジ又は固定されていない骨物質が、外部と内部シェ
ル間の中間の空間に導入し、そこに確実に保持するため
の、円形又は延長された開口（長孔）がある。この部材
は、手術中に導入でき、新しい強い骨物質がその中の空
間で成長することができる。該外部シェルの外部表面は
、タッピングするねじ山を形成する溝により遮られるネ
ジ山部を有する。関節窩部材は、この関節窩体を骨物質
の中へセルフタッピングネジによってネジ込むことによ
り取り付けられるのが有効な方法である。そしてこのプ
ロテーゼの固定度は、骨材の内部成長により増加する。
【０００３】
【発明が解決しようとする課題】従来のプロテーゼでは
、骨の中に効果的にネジ込むことが難しく、且つその中
での骨組織の内部成長が大きくない。よって、効果的に
ネジ込め、骨自体の成長を無理なく受容する関節窩部材
が要望されていた。
【０００４】
【課題を解決するための手段】本発明のカップ部材（補
綴関節窩部材）は、外部シェルの略軸長さに等しい延長
孔を有し、更に、その孔は、セルフタッピングスクリュ
ーのネジ山のための溝にもなる。周囲方向及び同軸的に
延びるネジ山を横断する開口の位置は、単にスポンジ等
を中間スペースに挿入できるだけでなく、カップ部材の
載置中に発生する骨チップがこの中間スペースに入って
いけるようにしている。該中間スペース内の該骨材チッ
プが手術中に漏れることを防止するようになっている。
【０００５】本発明において、該延長開口または孔は、
骨材のカップ部材の中への成長が大きな面積でなされる
ように、カップ部材の総軸長さとほぼ等しくされ、カッ
プ部材を確実に且つ永久に定着させるように構成されて
いる。
【０００６】本発明の好ましい実施態様では、セルフタ
ッピングスクリューのネジ山は、二重になっており、そ
れによってさらに有効に保持できるようになっている。
【０００７】さらに本発明の他の実施態様によると、外
部シェルにおいて該延長開口は、該軸に対してある角度
、好ましくは、約３０°の角度で穿設される。この開口
は、外部シェルの放射方向に対してある角度を持って穿
設されている。即ち、穿設中における装置の軸は、回転
方向に傾斜、好ましくは１０°、している。
【０００８】ネジ山部の高さと長さは、カップ部材の下
端部に向かって増大しており、ネジ込み力を減少させて
いる。この実施態様は、延長開口と二重ネジ山を組み合
わせて、カップ部材の挿入を助け、また、その載置後の
定着を向上するようになっている。
【０００９】外部シェル及び内部シェルは、一体的に成
形されている（鋳物にしてある）ものでもよい。或いは
、両シェルは、別々に成形されていてもよい。成形後、
内部シェルを外部シェルに嵌合した後、例えば、コール
ドウェルデイングで、結合する。
【００１０】本発明の好ましい実施態様によれば、内部
及び外部シェルは、円錐状に構成されており、内部シェ
ルは、大腿骨にカップ部材をネジ込むための工具によっ
て係合される溝を有する。内部シェルに対応して形成さ
れる嵌着具は、内部シェルに挿入されるもので、内部シ
ェルの上記溝に係合する突起を有しており、内部シェル
の嵌着物をロックさせることができる。
【００１１】カップ部材の嵌着具は、内部シェルのフラ
ンジと係合する環状放射フランジを有し、内部シェルの
該フランジに外部シェルを係合させる。

【実施例】以下本発明を図面に示す実施例に基づいてよ
り詳細に説明する。図１は、本発明のカップ部材の外部
シェルの断面図、図２は、図１の外部シェルの側面図、
図３は、図１の外部シェルの平面図、図４は、本発明の
カップ部材の内部シェルの断面図、図５は、本発明のカ
ップ部材の嵌着具の断面図、図６は、図５の嵌着具の平
面図、及び図７は、外部シェル、内部シェル及び嵌着具
からなるカップ部材の断面図である。
【００１３】カップ部材１は、装着後大腿骨に面する外
部シェル１０と嵌着具が挿入される内部シェルとから構
成される。
【００１４】カップ部材１の円錐状外部シェル１０は、
外部シェルの軸長さにほぼ等しく、この軸に対して、あ
る角度で延びており、周方向に間隔を持った延長開口１
１を有している。図１に示すように、この開口１１は、
軸に対して３０°の角度を持って、また放射方向に対し
て１０°の角度で穿設されている。すなわち、延長開口
１１の壁は外部シェル１０の回転方向に向かい傾斜して
いる。外部シェル１０の外側には、複数のネジ山部１２
がある。図２から明らかなように、ネジ山部は、延長開
口１１間に外部シェルの軸に対してある角度で延びる複
数の列１３を形成する。図３に示す通り、ネジ山部の高
さと長さは、外部シェル１０の上端部１４に向かって減
少する。ネジ山部１２の列１３間の延長開口１１は、タ
ッピング工具の溝の働きをする。周溝１６は、外部シェ
ルの下端部１５で内側に形成されている。この上端部１
４で、外部シェル１０は、１７の部分でアール取りされ
、開口１８が設けられている。
【００１５】図４によると、カップ部材１の内部シェル
は、フランジ２２を有する円錐部２１と、ショルダー２
３を介して円錐部２１とつながるシリンダー部２４を有
する。この円錐部２１は、ショルダー２３で限定される
円錐空間２５を形成する。円錐部２８は、下部は円錐空
間２５とつながり、上部はシリンダー部２４の内部空間
２９とつながっている。溝３０は、内部空間２９に隣接
して形成されている。内部シェルは上端部に、円形エッ
ジ３１で限定されている開口を有する。
【００１６】図５の嵌着具４０には、関節球体を収容す
る半球空間凹部４１が設けられており、円錐部４２の内
部表面がこの凹部４１を限定している。放射状フランジ
ネジ４３は、円錐部４２の下端部に形成されている。そ
の反対側の上端部には、ショルダー４４から上部に延び
ている円錐部４５が設けられている。図６から明らかな
ように、円錐部４５の外周に一体的に６個の突起４６が
形成されている。
【００１７】図７は、内部シェル２０と外部シェル１０
を組み立てたものである。内部シェル２０のフランジ２
２は、外部シェルの溝１６に収容され、一方シリンダー
部２４は、外部シェル１０の開口１８に適切に収容され
る。環状円錐中間スペース５０は、カップ部材の装着前
に、延長開口１１を介しルーズ骨材が挿入される外部シ
ェルおよび内部シェル１０，２０の間に形成される。セ
ルフタッピングネジ山部１２によって、カップ部材１は
工具を用いて、大腿骨に効果的にネジ込まれる。
【００１８】内部シェル２０中に嵌着具４０を挿入する
際、嵌着具４０を内部シェル内で固定するため、突起４
６が溝３０に係合する迄、内部シェル２０のシリンダー
部２９により、突起４６は一時的に変形する。
【００１９】嵌着具４０のショルダー４４は、内部シェ
ル２０のショルダー２６に係合し、内部シェルの放射状
フランジ２２の下側は、嵌着具４０のフランジ４３に係
合する。
【００２０】嵌着具４０のシリンダー部４５は、内部シ
ェル２０のシリンダー部２７内に嵌入され、内部シェル
２０のシリンダー部２４に環状に取り囲まれる。その内
部シェル２０のシリンダー部２４は、外部シェル１０の
開口１８とぴったり係合する。そのため、開口１８は、
実質的には、外部シェルの略外側の高さで嵌着具４０の
上端面と内部シェル２０により閉鎖されている。
【００２１】関節窩部材を装着した後、骨材は、延長孔
１１を介して中間空間５０の中に成長可能である。そし
て、大腿骨中にカップ部材を安全に定着させるために、
中間空間中に骨物質を結合することができる。
【００２２】
【発明の効果】本発明補綴関節窩部材によると、装着が
確実であり、且つ骨組織の成長を妨げることがない。

Claims (13)

    【特許請求の範囲】
  1. 【請求項１】　　大腿骨に固定可能な構造の中空外部シ
    ェルとベアリング部材（嵌着具）を収容する凹部が形成
    された内部シェルとから構成されるものであって、該外
    部シェルと該内部シェルが所定の間隔を持って位置し、
    それによって中間に空間を形成し、該外部シェルの外表
    面には、それを横切って延びている溝によって遮られる
    同軸状のネジ山が設けられており、該ネジ山は骨にネジ
    込めるようにセルフタッピングネジとして形成され、又
    該溝は、手術で装着される時に、該空間への新しい骨組
    織の成長を促進するため、スポンジ部材が該空間に導入
    でき、そこで保持できるように、上記外部シェルの略軸
    長さに等しい複数の延長開口であることを特徴とする補
    綴関節窩部材。
  2. 【請求項２】　　該タッピングネジが、二重ネジ山であ
    る請求項１記載の補綴関節窩部材。
  3. 【請求項３】　　該延長開口が、外部シェルの軸に対し
    てある角度で、好ましくは３０°の角度で設けられてい
    る請求項１又は２記載の補綴関節窩部材。
  4. 【請求項４】　　該延長開口が、外部シェルの半径に対
    して、回転方向に傾斜してある角度で、好ましくは１０
    °の角度で設けられている請求項１記載の補綴関節窩部
    材。
  5. 【請求項５】　　該ネジ山の高さが、外部シェルの上端
    部から下端部まで増加している請求項１記載の補綴関節
    窩部材。
  6. 【請求項６】　　該ネジ山の長さが、外部シェルの上端
    部から下端部まで増加している請求項１記載の補綴関節
    窩部材。
  7. 【請求項７】　　該カップ部材が一体的に成形されてい
    る請求項１記載の補綴関節窩部材。
  8. 【請求項８】　　該外部シェルと内部シェルが別々に成
    形されており、該空間を形成するように内部シェルが外
    部シェルに挿入され、両シェルは、例えばコールドウェ
    ルディングによって接合されている請求項１記載の補綴
    関節窩部材。
  9. 【請求項９】　　該外部シェルと内部シェルが円錐状に
    形成されている請求項１記載の補綴関節窩部材。
  10. 【請求項１０】　　該内部シェルが放射状フランジを有
    する円錐部と、もう一つの円錐部とからなり、後者の円
    錐部は上記外部シェルの開口１８と係合し、上記放射状
    フランジは、上記外部シェルの上端部で周状に延びてい
    る溝と係合している請求項８又は９記載の補綴関節窩部
    材。
  11. 【請求項１１】　　該溝が上記カップ部材を上記大腿骨
    内にネジ込むためのツールを収容するためのシリンダー
    部の内表面に形成されている請求項１０記載のの補綴関
    節窩部材。
  12. 【請求項１２】　　該嵌挿物が放射状フランジ付きの円
    錐部ともう一つの円錐部によって形成されており、上記
    嵌挿物の上記放射状フランジが上記内部シェルの上記放
    射状フランジの下側に係合するものである請求項９記載
    の補綴関節窩部材。
  13. 【請求項１３】　　複数の周方向に間隔を有する突起が
    上記嵌挿物の上記シリンダー部に形成されており、該突
    起は上記嵌挿物を内部シェル内にロックするために該内
    部シェルのシリンダー部の上記溝に係合するものである
    請求項１２記載の補綴関節窩部材。
