前記円筒状支持体の前記下流部分から延在する複数のS字形状の支持アームであって、前記心臓弁プロテーゼが拡張構成にあるとき、前記S字形状の支持アームが、上流方向に延在して、前記天然僧帽弁の弁輪上又は下の心臓組織と係合するように構成されている、S字形状の支持アームと、
前記円筒状支持体の前記上流部分から延在し、前記第1の断面寸法よりも大きい前記第2の断面寸法を有する半径方向に延在するセグメントであって、前記天然僧帽弁の前記弁輪上又は上方の心臓組織と係合するように構成されている、半径方向に延在するセグメントと、備える、プロテーゼ。
(項目19)
前記心臓弁プロテーゼが前記拡張構成にあり、かつ前記天然僧帽弁で展開されたとき、前記弁輪が、前記S字形状の支持アームの上流湾曲セグメントと前記半径方向に延在するセグメントとの間に位置付けられる、項目18に記載の心臓弁プロテーゼ。
(項目20)
各S字形状の支持アームが、その上流湾曲セグメント上に着地ゾーンを含み、前記着地ゾーンが、前記天然僧帽弁の前記弁輪上又は下の心臓組織と非外傷的に係合するように構成されている、項目19に記載の心臓弁プロテーゼ。
S.A.R.L.(Wilmington、DE)から市販)、他の布材料、ナイロンブレンド、ポリマー材料、及び真空蒸着ニチノール製造材料が挙げられる。更に別の実施形態では、弁尖132は、Royal DSM(オランダ)から商品名DYNEEMAで市販されている超高分子量ポリエチレン材料で製造されてもよい。特定の弁尖材料では、弁尖の片面又は両面を、過成長を防止又は最小限に抑える材料で被覆することが望ましい場合がある。弁尖材料に耐久性があり、また、弁尖材料が伸張、変形、又は疲労を受けないことが更に望ましい場合もある。
Claims (5)
- 心臓弁修復デバイスであって、
フレームを備え、前記フレームは、
左心房に近接して位置付けられるように構成された上流部分と、左心室に近接して位置付けられるように構成された下流部分と、第1の断面寸法とを有する支持体と、
半径方向に延在するセグメントであって、前記半径方向に延在するセグメントは、前記支持体の前記上流部分から延在し、前記第1の断面寸法よりも大きな第2の断面寸法を有し、前記半径方向に延在するセグメントは、下流方向に下向きに湾曲する第1の弓状セクションと、前記第1の弓状セクションから延在し、上流方向に上向きに湾曲する第2の弓状セクションとを有し、前記半径方向に延在するセグメントは、前記左心房の心臓組織と係合するように構成されている、半径方向に延在するセグメントと、
前記支持体の前記下流部分から延在する支持アームであって、前記心臓弁修復デバイスが拡張構成にあるとき、
前記支持アームは、前記下流部分から前記支持体の上流部分に向かって戻るように半径方向外向きに延在し、
前記支持アームは、天然僧帽弁の天然弁尖の後ろに延在し、前記左心室内の弁輪下組織と係合するように構成され、
前記支持アームは、曲線形状を有し、対向する第1の弓状領域および第2の弓状領域が、それらの間に延在する略直線領域によって長手方向に分離されており、前記第1の弓状領域は、前記支持体の前記下流部分から延在し、前記支持体に向かって湾曲し、前記略直線領域は、前記支持体の長手方向軸に対して前記支持体に向かって内向きに傾斜し、前記第1の弓状領域および前記第2の弓状領域を接合し、前記第2の弓状領域は、前記上流部分に近接する前記支持体から離れて外向きに湾曲し、前記第2の弓状領域は、前記天然弁尖の後ろの弁輪下組織に非外傷的に係合するように構成されている、支持アームと
を備え、前記支持体は、複数のストラットを含み、前記複数のストラットは、複数のダイヤモンド形状の開口部を画定し、前記支持アームは、第1の端部および第2の端部を含み、前記支持アームの前記第1の端部は、少なくとも1つの取設点を介して前記支持体に結合され、前記第2の端部は、前記支持体から半径方向に離間されている、心臓弁修復デバイス。 - 前記第2の弓状領域は、前記天然弁尖の後ろの前記弁輪下組織に非外傷的に係合するように構成されている着地ゾーンを有し、前記着地ゾーンは、前記支持アームの他の部分の幅よりも大きな幅を有する、請求項1に記載の心臓弁デバイス。
- 前記第1の弓状領域、前記第2の弓状領域、および、前記略直線領域はともに、実質的にS字形状を画定する、請求項1に記載の心臓弁修復デバイス。
- 前記第2の弓状領域は、前記支持アームの頂点に到達するように、移行点から外向きかつ上流方向に湾曲する第1の湾曲セグメントを画定し、前記第2の弓状領域は、前記頂点から外向き方向に延在し、前記支持アームの自由端の方へわずかに下向きに湾曲する第2の湾曲セグメントをさらに画定する、請求項1に記載の心臓弁修復デバイス。
- 前記半径方向に延在するセグメントは、前記支持体の前記上流部分から前記支持体から離れる上流方向に、半径方向外向きに延在する複数の相互接続されたストラットを備え、前記半径方向に延在するセグメントは、非外傷的端部分を形成する、請求項1に記載の心臓弁修復デバイス。
