Description

を測定する工程;

PAMELA(NCT01973660)は、HER2陽性乳癌の女性における、無作為化されていない、多施設で行われる、期待される非盲検の第2相試験である(図1)。全ての適格である患者は、一元的に確認されたHER2、免疫組織化学染色により一元的に行われたエストロゲン受容体及びプロゲステロン受容体、直径が1cmより大きい原発性腫瘍を持つステージI~IIIAの乳癌を有しており、18歳以上の年齢であり、これまでにいずれの癌治療も受けていなかった。腫瘍はHER2免疫組織化学染色3+又は2+を有していなければならず、インサイチュ(in-situ)ハイブリダイゼーションでは色素生産性について陽性でなければならなかった。注目に値するのは、HER2、ER、及びPRの試験をIS015189の認定のもとで行ったことである。
ラパチニブを1000mgの1日量で経口投与した。トラスツズマブは、8mg/kgの負荷投与量で3週間おきに、続いて6mg/kgをIV投与した。HR陽性の患者には、閉経状態に従ってレトロゾール(1日2.5mg)又はタモキシフェン(1日20mg)を投与した。全処置期間は18週間であった。2週目に、コア針生検(core-needle biopsy)を強制的に行った。6週目に、超音波による初期反応の評価を強制的に行った。この研究の間に、又は6週目で腫瘍の大きさがいくらか増大していることを処置の失敗とみなし、患者を主要エンドポイント(即ち、二重の遮断を用いたpCR)について反応しないと分類した。これらの患者を、トラスツズマブと、12服用量の1週間に1回の80mg/m2のパクリタキセル、及び750mgのラパチニブで、経口で処置した。二重のHER2の遮断の最後の用量の後1週間から3週間の間、又はパクリタキセルの最後の用量の後2週間から3週間の間に、外科手術を行った。医師の裁量にしたがって標準的な免疫賦活剤による化学療法を投与した。
ホルマリン固定パラフィン包埋(FFPE)乳房組織の切片を、浸潤性の腫瘍細胞の存在(≧10%)を確認するため、及び最小の腫瘍表面積(>4mm2)を決定するために、ヘマトキシリン及びエオシン染色で最初に試験した。遺伝子発現の分析のための最小限の組織要件が満たされない限りは、患者を入れることはできなかった。浸潤性の腫瘍細胞を含まなかった15日目の試料についてもまた、プロファイリングを行った。RNA精製(Roche(登録商標)High Pure FFPET RNA単離キット)には、≧1~5の10μmのFFPEスライドを各腫瘍検体について使用し、必要である場合には通常の汚染を避けるためにマクロダイセクションを行った。最低限約100ngのトータルRNA(total RNA)を、nCounterプラットフォーム(Nanostring Technologies,Seattle,WA,US)を用いて乳癌について選択した555種類の遺伝子及び5種類のハウスキーピング遺伝子(ACTB、MRPL19、PSMC4、RPLP0、及びSF3A1)の発現を測定するために使用した。データを底2の対数に変換した。5種類のハウスキーピング遺伝子の幾何平均を各試料について得、各試料中の各遺伝子について正規化係数として使用した。標的配列を含む560のコードセット(CodeSet)の設計は表8に見ることができる。
主要エンドポイントは、外科手術時の原発性腫瘍の顕微鏡検査で浸潤性の腫瘍性細胞が存在しないこととして定義される、乳房におけるpCRであった。インサイチュ病変が残っていることは許容した。
2013年10月から2015年12月まで、151人の患者をスペインの19の場所に集めた。集めた151人の患者のうち、137人の患者は計画したとおり処置を完了し、14人の患者は処置を中断した(図2)。視触診での乳房検診によるベースラインの腫瘍の大きさの中央値は2.4cmであり、ほとんどの患者は陰性の腋窩を有しており(64.9%)、閉経後であった(59.6%)(表1)。HR陽性疾患の患者(n=77)のうち、52%及び48%に、閉経状態に従ってタモキシフェン及びレトロゾールをそれぞれ、投与した。外科手術を行った全ての患者が病理学的反応について有効な評価を有していた。
555種類の乳癌関連遺伝子及び5種類のハウスキーピング遺伝子の発現は、全てのベースライン試料中で良好に行われていた(n=151)(表8を参照のこと)。最良のモデルを選択するために、4種類の変数選択方法(t検定、Welchのt検定、ランダムフォレスト[rf]、及び再帰的特徴量削減[rfe])、様々な数の選択した遺伝子(1、2、3、4、5、10、12、15、20、及び30種類)、並びに3種類の分類方法(対角線形判別分析[dlda]、線形判別分析[Ida]、及び二次判別分析[qda])を使用して交差検証法による分析(10分割、25回の繰り返し)を行った。図3及び表3に示すように、最良の「バランスのとれた精度解析」(分類性能の尺度)を、1つの遺伝子を用いて得た。1つの遺伝子は行った全ての分析においてERBB2であった。
全体として、このデータは、555種類の乳癌関連遺伝子のうち、ERBB2が、化学療法を用いない二重のHER2の遮断後の反応を予測するための最も力強い遺伝子であることを示唆していた。その後、ベースラインの腫瘍試料を用いて、151人の患者のデータセット全体においてpCRを予測するERBB2発現の能力を調べた。まず、pCRを予測するERBB2の能力を推定した(図5)。結果は0.804のAUCを示した。
HRの状態は、今日までのところ、細胞毒性治療を行わない二重のHER2の遮断後のpCRを予測するための唯一の分子予測因子であった。ここでは、HRの状態を上回るpCRを予測するERBB2の能力を評価した。HRの状態とERBB2発現を含む二変量分析のロジスティック回帰モデル(表4)において、ERBB2はなおもpCRと有意な関係があったが、一方、HRの状態はその統計学的有意性を失っていることが観察された。この結果は、ERBB2がHRの状態よりも更により予測となる情報を提供することを示唆している。
全部で144のペア試料を、集めた151人の患者からPAMELAにおいて得た。これは、全ての入手できた試料の95%を占める。従って、このペアのデータセットから、ベースラインで、2週目で測定したERBB2の発現と、2つの時点の間でのERBB2発現の比の予測能力を比較することができた。能力を比較するために、3種類のバイオマーカの間のAUCを比較した(図6)。結果は、2週目及びベースラインの時点でのERBB2発現の比が、pCRについての最良の予測因子であり(図6)、AUCが0.878であったことを示していた。

本研究では、ERBB2の発現が単独で、化学療法を用いない二重のHER2の遮断後のpCRについての最良の予測因子であることが示されている。このバイオマーカは、ベースラインで、処置の2週目で、又は両方でのいずれでも評価することができる。これらの結果は、ベースラインERBB2発現の予測能力が2週目のERBB2発現と同様であるが、両方の時点によるERBB2発現データを組み合わせること(即ち、ERBB2比)が上記3つのうちの最良の予測因子であることを示唆している。従って、臨床的な将来の見通しから、ERBB2発現は、ベースラインのみ(即ち、処置開始前)、又は2週目での生検を行うことができる場合は両方の時点(即ち、ERBB2比)でのいずれを使用することもできる。いずれにしても、いずれの予測因子も、化学療法を用いない二重のHER2の遮断で処置される場合には、症例の64.9~75%においてpCRに到達するであろうHER2陽性疾患の患者の約25%(上位四分位)を同定することができる。重要であるのは、ベースライン時点でのERBB2又はERBB2比が、化学療法を行わない二重のHER2の遮断後のHER2陽性乳癌においてpCRと関係があることが今日まで一貫して明らかにされてきた唯一の分子予測因子であるHRの状態と比較して、独立した更なる情報を提供することである。

Claims (16)
- HER2陽性乳癌の患者における化学療法を行わない抗HER2治療の有効性を決定するためのインビトロ方法であって、
(a)以下の(a.i)及び(a.ii)の時期に患者から単離された乳癌の組織生検試料中のHER2の遺伝子発現産物の検出及び/又は定量する工程を含み、
(a.i)抗HER2治療の開始前、及び
(a.ii)抗HER2治療の開始後5~20日目;
ここで、工程(a.i)で検出又は定量した遺伝子発現産物量と比較して工程(a.ii)で検出又は定量した遺伝子発現産物量に減少が存在する場合には、それが、抗HER2治療が有効であることの指標となる、方法。 - 化学療法を行わない抗HER2治療の代替の医療レジメンを開始するかどうかHER2陽性乳癌の患者について決定する又は推奨するためのインビトロ方法であって、
(a)以下の(a.i)及び(a.ii)の時期に患者から単離された乳癌の組織生検試料中のHER2の遺伝子発現産物の検出及び/又は定量する工程を含み、
(a.i)抗HER2治療の開始前、及び
(a.ii)抗HER2治療の開始後5~20日目;
ここで、工程(a.ii)で検出又は定量された遺伝子発現産物量が工程(a.i)で検出又は定量された遺伝子発現産物量よりも高い場合には、それが、代替の医療レジメンが必要であることの指標となる、方法。 - 代替の医療レジメンが、化学療法、外科手術、放射線治療、又はそれらの任意の組み合わせである、請求項2に記載の代替の医療レジメンを開始するかどうかを決定又は推奨するためのインビトロ方法。
- 化学療法が、パクリタキセル、ドセタキセル、カルボプラチン、ドキソルビシン、エピルビシン、nab-パクリタキセル、ビノレルビン、カペシタビン、及びエリブリン、又はそれらの任意の組み合わせからなる群より選択される、請求項3に記載の代替の医療レジメンを開始するかどうかを決定又は推奨するためのインビトロ方法。
- 化学療法を行わない抗HER2治療の開始後のHER2の遺伝子発現産物の検出及び/又は定量が、抗HER2治療の開始後9~19日目の間に行われる、請求項1~4のいずれか1項に記載のインビトロ方法。
- HER2の遺伝子発現産物がmRNA又は配列番号2のタンパク質である、請求項1~5のいずれか1項に記載のインビトロ方法。
- 抗HER2治療が、トラスツズマブ、ラパチニブ、トラスツズマブとラパチニブ、ネラチニブ、ペルツズマブ、ado-トラスツズマブ、エムタンシン、又はそれらの任意の組み合わせからなる群より選択される、請求項1~6のいずれか1項に記載のインビトロ方法。
- 患者が女性である、請求項1~7のいずれか1項に記載のインビトロ方法。
- 患者がホルモン受容体陽性(HR+)患者、ホルモン受容体陰性(HR-)患者、又はホルモン受容体陽性(HR+)患者であり、且つ抗HER2治療が内分泌療法と組み合わせられている、請求項1~8のいずれか1項に記載のインビトロ方法。
- 患者がホルモン受容体陽性(HR+)患者であり、且つ抗HER2治療が内分泌療法と組み合わせられる場合、前記内分泌療法が、タモキシフェン、トレミフェン、アナストロゾール、エキセメスタン、レトロゾール、フルベストラント、ゴセレリン、ロイプロリド、及び/又はトリプトレリン、あるいはそれらの任意の組み合わせからなる群より選択される、請求項9に記載のインビトロ方法。
- HER2陽性乳癌の患者において化学療法を行わない抗HER2治療の有効性を決定するためのインビトロマーカとして、あるいは代わりに、化学療法を行わない抗HER2治療を受けているHER2陽性乳癌の患者において抗HER2治療の代替の医療レジメンを開始するかどうかを決定又は推奨するためのインビトロマーカとしての、抗HER2治療の開始前及び開始後5~20日目における前記患者の乳癌の組織生検試料中のHER2の遺伝子発現産物量の比の使用。
- 代替の医療レジメンが、化学療法、外科手術、放射線治療、又はそれらの任意の組み合わせである、請求項11に記載の使用。
- 抗HER2治療が、トラスツズマブ、ラパチニブ、トラスツズマブとラパチニブ、ネラチニブ、ペルツズマブ、ado-トラスツズマブ、エムタンシン、又はそれらの任意の組み合わせからなる群より選択される、請求項11又は12に記載の使用。
- 患者がホルモン受容体陽性(HR+)患者、ホルモン受容体陰性(HR-)患者、又はホルモン受容体陽性(HR+)患者であり、且つ抗HER2治療が内分泌療法と組み合わせられている、請求項11~13のいずれか1項に記載の使用。
- 請求項1~10のいずれか1項で記載された方法のいずれかにおける、HER2の遺伝子発現産物の存在を決定する又は定量するための手段の使用。
- 以下の工程を含む、HER2陽性乳癌と診断された被験者における、抗HER2処置の有効性を決定するための方法:
a)化学療法を行わない抗HER2治療の開始前の被験者から得られた、乳癌の組織生検試料を含有している第1の試料を、HER2のmRNAに結合する試薬と接触させる工程;
b)第1の試料中の前記試薬に結合したHER2のmRNAの量を測定する工程;
c)化学療法を行わない抗HER2治療の開始後5~20日目の被験者から得られた、乳癌の組織生検試料を含有している第2の試料を、HER2のmRNAに結合する前記試薬と接触させる工程;
d)第2の試料中の前記試薬に結合したHER2のmRNAの量を測定する工程;および
e)工程b)で測定されたHER2のmRNAの量を、前記工程d)で測定されたHER2のmRNAの量と比較する工程;
ここで:
(i)前記工程b)で測定されたHER2のmRNAの量が、前記工程d)で測定されたHER2のmRNAの量より多い場合は、抗HER2処置がより有効であるか又は有効性が高く;そして
(ii)前記工程b)で測定されたHER2のmRNAの量が、前記工程d)で測定されたHER2のmRNAの量よりも少ない場合は、抗HER2処置があまり有効ではない又は無効であることの指標となる、方法。
