Description
第1の実施形態は、図1に示すような少なくとも3つの構成要素を有するSESデバイスおよびシステムまたはSESデバイスもしくはシステムを備える。第1の構成要素は、ベースライン(プレ)、刺激中(ペリ)および刺激後(ポスト)のバイオマーカーレベルをモニタするためのバイオマーカーモニタ101からなる。(複数の)センサ102は、頭(頭蓋骨の外皮表面、または身体内部の他のバイオマーカーフィードバック感知位置)に取り付けられるか、または健康な被験体および罹患した被験体が装着することができるスマートキャップ103に組み込まれた1つもしくは複数の電極であってもよい。センサシグナルは、周波数およびEEG電流の振幅であってもよい。

四肢および指の動きを検出するためのセンサを備えたデバイスの他の形状因子として、様々な指に配置することができるリングが挙げられる。リングは、リストバンドのPNS刺激部およびスマートフォンまたはタブレットと連携して動作する(または物理的にまたは無線で接続することができる)センサを含有する。患者がスマートフォンまたはタブレット上のゲームに基づいて指を動かしている間に、SESまたはPNSがリストバンド電極を介して刺激される。指輪センサは、動作を検出し、刺激を測定し、セッション毎のデータを記録する。刺激パラメータおよび得られた動きに関するフィードバックに基づいて、数週間から数ヶ月続く物理的治療およびリハビリテーションの間に、単一のセッションまたは複数のセッションで所望の結果または肢の動きを得るように刺激を設定することができる。同様のシステムを使用して、様々なビデオゲームを使用して訓練することを伴うか、または伴わずに、SESによってビデオゲーマーの応答時間を改善することができる。9-hole peg testのような様々なテストを使用して、応答時間を測定することができる。
システムのためのEEGデバイスおよび電極は、ユーザが使用しやすいように設計されており、図2に既に示されているように様々な実施形態で利用可能である。これらのEEGフィードバックおよび刺激システムまたはEEGフィードバックもしくは刺激システムは、可能な限り多くの処理をソフトウェア/電話に転送することによって、嵩(bulk)を最小にするように、Bluetooth Low Energy(BLE)または電話による類似の無線通信技術と統合することができ、これはまた、インタープリテイション(interpretation)を改善することができる。刺激用の電極システムは、皮膚に対する負傷または損傷を引き起こす可能性がある導電性ゲル(標準的なEEGペースト)を使用する。そのような損傷を最小限に抑えるために、システムは、(例えば、再使用可能な糸くずローラに使用される材料もしくは3M製の携帯電話のダッシュボード吸着マウントに使用されている材料、またはほこりを洗い流すことができる「粘着ハンド」のおもちゃとおそらく同様の)残留物が最小限である高凝集接着ゲルを含んでもよい。
訓練の主要な構成要素は、運動タスクに関連しているが、追加の構成要素(光、音声、画像、スクリーン上の指標、振動)は、前述のようなグローブにおける触覚フィードバックと同様に、認知の負担を減らすための追加的な合図をもたらすことができる。バイオフィードバックシステムはまた、慢性疼痛のモニタおよび管理に使用することができる。訓練/ゲームプレイ中のパフォーマンスを向上させるためのフィードバックシステムは、携帯電話/タブレット上での視覚、振動、および音声を含む。図9に示されるように、タッチスクリーンを使用して、微細運動(ピンチング、タッピング、独立化)能力を訓練することができる。図9において、ユーザは、ゲームによって指示されたように指をタッピングし、各指は、タブレットまたは電話のスクリーンを個別にタッピングする。加えて、特に、粗大動作/大きい筋肉のための、追加のセンサの必要性を排除することができる場合、タブレットおよび電話を有する既存のモーションセンサを使用することができる。例えば、本態性振戦において、到達している間に電話を握ることを使用して、スマートフォンのジャイロスコープを使用して振戦を測定することができる。同様に、既存の振動/音声センサおよび画像/光を使用して、グローブからの触覚フィードバックと同様に、訓練セッション中に追加のフィードバックを提供することができる。ハードウェアを使用して、他のデバイス/センサ/電極からの入力および出力を調整することができる。ソフトウェアおよびインターネット接続を、アルゴリズムの実装、クラウドリレーなどに使用することができる。
上記のデバイス、システム、およびアルゴリズムを使用した訓練およびフィードバックと組み合わせたPNSによって様々な疾患および状態を治療することができる。様々な疾患および状態としては、脳卒中、疼痛、神経痛、片頭痛、歩行障害、パーキンソン病(Parkinson’s disease : PD)などが挙げられる。PNSは、疾患の経過を変えることは期待されていないが、PD患者は、理学療法が症状(運動性、姿勢、歩行、こわばり)を緩和することを見出すことができる。しかしながら、理学療法が中止されると、効果は失われる。本発明は、在宅で長期治療を可能にすると考えられる。PNSは、大小の筋肉、特に、大きい筋肉の神経に効果をもたらすことができる。PNSはまた、血管以外の原因によるインポテンスを治療することができる。様々な状態、標的PNS部位、ならびに訓練およびフィードバックを含む治療計画は、図10~図13に詳述されている。PNSを使用して、健康な被験体のパフォーマンスを高めることもできる。プロの運動選手、ダンサー、ミュージシャン、内科医、ゲーマー、転記者、および速記者は、PNSおよびフィードバックアルゴリズムによる反復的な動作により、専門能力もしくは職業能力を向上させるか、または運動機能の低下を防ぐことができる。さらに、PNSを使用して、記憶力および睡眠を改善することもできる。治療から効果を得ることができる被験体、標的PNS部位、および訓練/フィードバックシステムを図14に詳述する。
尿失禁、痙性膀胱または過活動膀胱については、刺激は、S1~5に関連する皮神経ならびに会陰神経、陰部神経およびその枝を伴い得る。評価は、痙縮の軽減、膀胱の緊張の改善、または排尿の頻度に関連する自己申告の症状を伴うと考えられる。
不眠症および他の睡眠障害については、脳神経5および脳神経7または脳神経5もしくは脳神経7ならびに関連する感覚神経を標的とする接着性顔面電極、後頭神経、耳介神経、および脳卒中について以前に同定された上下肢の標的の刺激を使用してもよい。刺激は、ユーザの睡眠サイクルに合わせて調整されてもよい。評価は、自己申告またはEMG、EOGもしくはEEGを伴い得る。
運動選手、ダンサー、ミュージシャン、医師、外科医、ビデオゲームプレーヤー、およびタイピストはすべて、本明細書に記載のデバイスから効果を得ることができる。電極を、腕、脚、手首などのスリーブ(sleeves)に配置して、使いやすく快適な送達システムにおいて必要な刺激を与えることができる。
PNS電極刺激は、EEGからのフィードバックおよび訓練/フィードバック「ゲーム」に基づいて調整される。これは、類似のEEGプロファイルおよび応答を有する人々が過去にどのように実行したかに基づいてさらに調整および最適化され得る。例えば、後期患者の治療アルゴリズムは、早期に治療された患者からのフィードバックデータによって調節または通知される。あるいは、同じ患者に対する治療アルゴリズムは、PNSに対する患者の応答に基づいてカスタマイズされてもよく、および/または治療もしくはパフォーマンスをさらに向上させるように、後続のセッションにおいて調節もしくは設定されてもよい。長時間の刺激(例えば、日中または夜間に間欠的に付与される感覚閾値下またはほぼ感覚閾値下の刺激)では、EEGを装着したままで刺激がアルゴリズムで最適化されるが、EEGを装着せずに継続することができる。例えば、患者は、刺激パラダイムを更新/最適化するために1時間、EEGを装着することができ、その後、EEGキャップを除去して継続する。集中型PNS治療アルゴリズムは、EEGキャップを着用して処方することができ、低集中型PNS治療は、EEGキャップを除去して送達することができる。
後天性脳損傷および遠位上肢運動障害の病歴を有する8人の参加者が、経皮的電気神経刺激を用いたSESの単一の2時間セッションを受けた。治療介入前後の評価は、安静時のEEGモニタリングとともに、アクションリサーチアームテスト(Action Research Arm Test : ARAT)、指の分別、挾持力、および改訂アシュワーススケール(modified Ashworth scale : MAS)で構成された。
実施例1における上記の研究を基に、手のリハビリテーションのための療法の組み合わせの相乗効果を、同じ被験体において試験した。影響を受けた手における、モーションセンサデータグローブ、リアルタイムの振動触覚/触覚フィードバックおよび末梢神経のSESを組み合わせて、運動機能および手/指の動きの変化を測定した。この研究は、手のリハビリテーションへの集学的アプローチが、脳卒中後に慢性的な運動障害のある被験体の手および指の機能を改善できるか否かを評価した。2つの重要な目標は、最先端技術を使用して、リハビリテーション中にリアルタイムのフィードバックを提供すること、およびモダリティ毎に改善を予測した患者の特徴を完全に明確にすることであった。
技術の一実施形態は、SES刺激を使用するインポテンスの治療に関する。このケースにおいて、患者または医療サービス提供者は、電極を会陰に付与する。図24に示すように、神経毎の1個のパッチまたは構成部品に一体化されるか、あるいは、会陰全体の1個のパッチまたは構成部品に一体化されたかのいずれか一方である、陰部神経毎に正極および負極が存在することができる。ユーザは、電極またはパッチを付与する前に、毛をトリミングする必要がある場合がある。電極がワイヤを有する場合、電極を、制御モジュールに差し込まれなければならない。他の実施形態では、電極は、制御モジュールと無線通信する。次に、ユーザは、ヘッドバンド形状のEEGを着用してもよく、その後、電話でアプリを開く。アプリは、最後の刺激セッションおよび最後の訓練セッションからの以前にアップロードされたデータを有する。サーバは、刺激アルゴリズムを改訂し、そのアルゴリズムをアプリに戻している。次に、ユーザは、刺激セッションを開始する。次に、電話は、Bluetooth Low Energyを使用して、EEGモニタ、および電極制御モジュールと通信する。EEGデータがEEGモニタから受信される間、刺激シグナルは制御モジュールに送られる。EEGデータを使用して、アルゴリズムに従い、リアルタイムで刺激を調整する。EEGデータを分析することにより、EEGセンサが適切に位置づけられていない場合、電話は、患者に、EEGセンサを調節するように促す。その後、ユーザは、電極を取り外し、電極のプラグを抜き、捨てるか、または再使用してもよい。その後、マイクロUSB充電器を使用して、制御モジュールを充電し、マイクロUSB充電器を使用してEEGモニタを再充電する。バックグラウンドでは、電話は、最新の刺激データをクラウドサーバにアップロードする。
通常のゲームコンソールに基づくか、あるいは電話/タブレットに基づくかにかかわらず、視覚運動協応、手先の巧緻性、および指の独立化は、ビデオゲームをプレイするために重要である。PNSは、健全な被験体における肢の動きの運動表象を変化させることが知られている。重要なことに、PNSが知覚能力および手先の巧緻性に役立つことができるという示唆を行う文献が存在する。デバイスの一実施形態は、視覚運動協応、手先の巧緻性、ならびに指の独立化、反射神経、正確な動き、(タッピングなどの)速度、指の動きの円滑性、および手と目の協応を改善するように設計されたゲームスキルを向上させるように設計されている。
睡眠中の神経処理の調整は、宣言的記憶および運動の記憶を向上させることが知られている。重要なことに、複雑な問題への洞察を提供すること(例えば、一見無関係なデータ点において隠れたルールを発見すること)も知られている。これらのプロセスはすべて、新しい運動能力の習得、知覚能力の向上、新しい事実および知識の習得、ならびに困難な問題に対する洞察の獲得など日常生活にとって非常に重要である。前述のデバイスを使用して、これらのプロセスを調整することができ、それ故に、睡眠依存処理の全ての要素を強化することができる。
Claims (15)
- 閉ループ電気刺激によって、指の独立化を伴う手作業タスクのパフォーマンスを向上させるための装置であって、
ユーザの腕、手または腕および手に装着されるように構成された刺激部であって、前記刺激部は、1つまたは複数の電極と、電源と、コントローラと、を有し、前記刺激部は、装着時に前記1つまたは複数の電極から前記ユーザの橈骨神経、尺骨神経および正中神経のうちの1つまたは複数に電気刺激を送達するように構成されている、前記刺激部と、
プログラムを記憶している非一時的なコンピュータ可読記憶媒体と、を備え、
前記プログラムは、プロセッサに、
指の独立化を伴う手作業訓練タスクを提示すること、
提示された前記手作業訓練タスクに対する前記ユーザの少なくとも2つの指の独立した動きに関連するパフォーマンス測定基準を測定することであって、前記パフォーマンス測定基準は、前記少なくとも2つの指の全体的な動きおよび前記少なくとも2つの指の連動した動きの尺度のうちの少なくとも一方である、前記パフォーマンス測定基準を測定すること、
前記パフォーマンス測定基準に基づいて前記手作業タスクのパフォーマンスを向上させるように付与された刺激の閉ループ制御によって前記電気刺激の複数のパラメータを調節すること、
調節された前記電気刺激の複数のパラメータを前記刺激部に送信すること、を行わせる、装置。 - 前記刺激部は、
リストバンド、パッチ、またはスマートグローブを含む、請求項1に記載の装置。 - 前記刺激部は、
