Description
本願は、2017年6月3日に出願された米国特許仮出願第62/514,793号、2017年8月11日に出願された米国特許仮出願第62/544,680号、2017年11月24日に出願された米国特許仮出願第62/590,513号、2018年1月27日に出願された米国特許仮出願第62/622,871号、および2018年4月9日に出願された実用特許出願第15/949,005号の権益主張出願である。これら出願の各々を参照により引用し、これらの記載内容全体を本明細書の一部とする。
第1のルーメン、ドレナージルーメン、拡張可能な第1のバルーンおよび温度センサを有するカテーテルを用意するステップと、
尿道を通って患者の膀胱中にカテーテルを挿入するステップと、
圧力変換器を収納したハブを第1のルーメンに連結してカテーテルの第1のルーメンを通ってガスを自動的に送り進めて第1のバルーンを萎ませ状態からより拡張された状態に拡張するとともに温度センサをハブ内のコネクタに自動的に接続するステップと、
流体を膀胱中に注入しないで、バルーンの変形に基づいて膀胱の第1の圧力読みを得るステップと、
第1の圧力読みをハブに連結された外部モニタに伝送するステップと、
流体を膀胱中に注入しないで、バルーンの変形に基づいて膀胱の第2の圧力読みを得るステップと、
第2の圧力読みをハブに連結された外部モニタに伝送するステップと、
流体を膀胱中に注入しないで、膀胱の逐次連続した圧力の読みを得るステップとを含む。
カテーテル50は、同一の仕方で用いられることに注目されたい。バルーン26を膨らませてルーメン20と流体連通状態にある側部ポート28を通って流体(液体または気体)を注入することによって、カテーテル10を手技中、定位置に固定する。このシステムは、バルーン16の膨らましによって、すなわち、好ましくは上述した理由で部分的膨らましよって、ルーメン14と流体連通状態にあるポート15を通って注射器による空気の注入によってチャージされる。上述したように、カテーテル10は、ルーメン14を通ってバルーン16中に挿入される空気がバルーン16を通って逃げ出ることができないよう封止されたバルーン16を備えた閉鎖系である。かくして、密閉チャンバが形成され、この密閉チャンバは、バルーン16の内部スペース16aおよびバルーン16の内部スペース16aと連通した内部ルーメン14を有する。バルーン16を膨らませた状態で、圧力モニタリングが始まるのが良い。圧力を膀胱壁に加え、かくしてバルーン16の壁に加える外向きの腹部内圧力により生じる外部圧力がバルーン16の外面16bに加えられると、カテーテル内のガス、例えば空気が圧縮される。ルーメン14の遠位端部のところに位置するセンサ30は、ルーメン14の遠位端部内に設けられた変換器によって電気信号に変換される連続した圧力の読みを提供し、次に、ルーメン14を通って延び、近位側部ポート16を通って出て外部モニタに接続されるワイヤ32を通って電気的に連絡する。ワイヤは、プラグインコネクタ内の近位端部のところで終端するのが良く、プラグインコネクタは、モニタに直接接続されても良く、あるいは変形例として、コンバータ中に差し込まれて、それによりコンバータは上述のグラフィックディスプレイをもたらすようワイヤとモニタとの間に介在している(例えば、図2のシステムを参照されたい)実施形態では変換器からの信号を変換する。システムは、連続圧力および温度モニタリングを実施することができるが、このシステムはまた、所望ならば、定期的にモニタするよう改造でき、したがって、圧力および温度の読みを、間隔を置いてまたは臨床医による要求に応じて得ることができる。
細長い部材302は、これを貫通して延びていて、圧力波が圧力センサまで伝播することができるようにするチャネルまたはルーメン305をさらに有する。好ましい実施形態では、追加の空気がハブ300の取り付け後にバルーン204中に注入される必要はないが、ポートまたは開口部が追加の空気の注入のための注入装置を受け入れるようハブ300に設けられることが良いこともまた想定される。かかる追加の空気は、細長い部材302のチャネル305と連絡し、これを通って膨らましのために空気ルーメンおよびバルーン204中に流れるのが良く、あるいは変形例として、細長い部材302の下流側(遠位側)で角度付きポートに設けられた側部ポートまたは開口部が設けられても良い。また、ハウジング290へのハブ300の取り付けにより、サーミスタコネクタ308がサーミスタピン292に自動的に接続されてケーブルを介する温度モニタとの連通のために温度センサをハブ300に自動的に接続する。
Claims (14)
- 圧力をモニタするために患者の体内中に挿入可能なマルチルーメン型カテーテルであって、前記カテーテルは、
前記カテーテルの遠位部分のところに設けられた外側バルーンを有し、前記外側バルーンは、拡張可能であり、第1の外壁を有し、
前記外側バルーンを膨らませるための第2のルーメンを有し、
前記外側バルーン内に位置決めされた内側バルーンを有し、前記内側バルーンは、拡張可能であり、第2の外壁を有し、
前記内側バルーンを膨らませるための第1のルーメンを有し、前記第1のルーメンは、前記外側バルーンを膨らませるための第2のルーメンと独立であり、前記内側バルーンと連通しており、前記内側バルーンと前記第1のルーメンは、膀胱内の圧力をモニタするためにガスを収容するガス充填チャンバを形成し、前記外側バルーンは、前記内側バルーンの円周方向面積よりも大きい円周方向面積を有し、前記膀胱内で拡張された前記外側バルーンの前記第1の外壁に及ぼされた圧力に応答して、前記外側バルーンは、変形し、そして圧力を拡張状態の前記内側バルーンの前記第2の外壁に及ぼして前記内側バルーンを変形させると共に前記内側バルーン内および前記第1のルーメン内の前記ガスを圧縮し、それにより精密測定をもたらし、
前記第1のルーメンおよび第2のルーメンと独立しており、流体を前記膀胱から取り出すよう前記膀胱と連通したドレナージルーメンを有し、
前記第1のルーメンおよび第2のルーメン並びに前記ドレナージルーメンは、半径方向に間隔を隔てており、および/または、前記第1のルーメンおよびドレナージルーメンは、前記第2のルーメンの外側にあり、
膀胱内圧力が、拡張された前記外側バルーンの変形によって変形された拡張された前記内側バルーンの変形によって引き起こされるガスの圧縮に基づいて測定される、カテーテル。 - 前記ガス充填チャンバは、前記膀胱内の圧力をモニタし、それにより前記患者の腹部内の圧力をモニタする、請求項1記載のカテーテル。
- 前記ガス充填チャンバと連通したセンサが、前記膀胱中への水の注入を必要としないで、前記カテーテルの挿入全体を通じて平均圧力を連続的に測定する、請求項1または2記載のカテーテル。
- 前記ドレナージルーメンは、前記内側および前記外側バルーンの近位側に位置する側部開口部を有する、請求項1~3のうちいずれか一に記載のカテーテル。
- 前記カテーテルは、追加のルーメンおよび中核体温を測定するよう前記追加のルーメン内に位置決めされた温度センサを有する、請求項1~4のうちいずれか一に記載のカテーテル。
- 前記外側バルーンは、前記膀胱の壁の平均圧力の計算のために多数の基準個所を提供するよう多数の接触領域のところで前記膀胱壁と係合可能な周囲、および前記外側バルーンを膨らませるよう前記外側バルーンと連通した第3のルーメンを有する、請求項1~5のうちいずれか一に記載のカテーテル。
- ハブ内に収容された圧力センサをさらに含み、前記カテーテルの第1のポートへの前記ハブの連結により、自動的に空気を前記内側バルーン中に送り進めて前記内側バルーンを拡張させる、請求項1~6のうちいずれか一に記載のカテーテル。
- 前記カテーテルは、膨らましルーメンおよび安定化バルーンをさらに有し、前記膨らましルーメンは、前記安定化バルーンを膨らませて前記カテーテルの位置を安定化するために前記安定化バルーンと連通し、前記安定化バルーンは、前記外側バルーンの近位側に位置決めされている、請求項1~7のうちいずれか一に記載のカテーテル。
- 細長い部材が、前記ハブから遠位方向に延びており、前記第1のポートは、弁を有し、前記細長い部材は、前記ハブが前記カテーテルの前記第1のポートに連結されると、前記弁を通って挿入可能である、請求項7記載のカテーテル。
- 前記カテーテルは、中核体温を測定する温度センサを有し、ワイヤが前記温度センサから別のルーメンを通りかつ前記カテーテルの外側で、前記カテーテルに連結された前記ハブ中に延びており、前記ハブの前記カテーテルへの連結により、自動的に電気的に前記温度センサを電気モニタケーブルに接続する、請求項7または9記載のカテーテル。
- 前記ドレナージルーメンは、前記第1のルーメンと独立している、請求項1~10のうちいずれか一記載のカテーテル。
- 前記第2のルーメンは、前記外側バルーンを膨らませるための開口部を有し、前記第1のルーメンおよび前記ドレナージルーメンと独立している、請求項1~11のうちいずれか一記載のカテーテル。
- 前記第1のルーメンは、前記ハブが前記第1のポートに連結されているときに、外気に空気抜けされていない、請求項7記載のカテーテル。
- 前記外側バルーンは、部分的に膨らまされる、請求項1~13のうちいずれか一記載のカテーテル。
