本開示は、少なくとも1種類のヒト化CD123結合ドメインを有し得るCD123に特異的に結合する分子に関する。これらの分子は、癌などのCD123過剰発現によって特徴付けられる障害の特徴付け又は治療に有用である。CD123に結合するタンパク質治療薬は、単一特異性タンパク質治療薬又は多特異性タンパク質治療薬であり得る。多特異性タンパク質治療薬は、CD123発現細胞とT細胞上のT細胞受容体複合体の両方に結合し、標的依存的にT細胞の細胞傷害、活性化及び増殖を誘導することができる。
CD123は、ヒトインターロイキン3(IL-3)受容体のアルファ鎖としても知られている。CD123は、I型膜貫通糖タンパク質であり、サイトカイン受容体スーパーファミリーのメンバーである。インターロイキン3受容体は、CD123及びベータ鎖(CD131)によって形成されたヘテロ二量体である。IL-3はCD123に結合し、シグナル伝達はCD131によって提供される。IL-3は、造血細胞及び免疫細胞の機能並びに生成を調節し、内皮細胞の増殖を刺激する(Testaら、Biomark Res.2:4(2014))。

実施例1:抗CD123抗体のヒト化変異体の作製並びに単一特異性及び二重特異性CD123結合分子の構築
ファージディスプレイによる単一特異性CD123結合分子の単離
抗CD123特異的scFv結合分子を、Distributed Bio社のプロトコルに従って、かつ以前にAyriss,Jら,(2007)J Proteome Res.,p1072-82に記載されている通りに、ビオチン化組み換えCD123タンパク質(配列番号200)に対する可溶性ファージディスプレイパニング技術を用いて、Distributed Bio社(South San Francisco,CA)のスーパーヒューマン(SuperHuman)ライブラリーから単離した。クローンの結合特異性を、同一の精製タグを有するCD123タンパク質及び無関係の組み換えタンパク質を用いてELISAによって特徴付けた。それぞれ配列番号201及び配列番号203のCD123のヒト変異体及びカニクイザル変異体でトランスフェクトしたHEK293細胞を用いるフローサイトメトリーを用いて、細胞表面の状況でCD123に結合するクローンを特定した。これらの実施例で言及する結合分子のヌクレオチド配列及びアミノ酸配列は、表5の中で見つけることができる。
抗CD123特異的抗体を、組み換えヒトCD123エクトドメイン(配列番号200)でOmniMice(Ligand社,San Diego,CA)を免疫した後に作製されたハイブリドーマライブラリーから単離した。個々のクローンの結合特異性を、それぞれ配列番号201及び配列番号203のCD123のヒト変異体及びカニクイザル変異体でトランスフェクトしたHEK293細胞にてフローサイトメトリーを用いて結合を試験することによって確認し、さらに親のHEK293細胞への結合の欠如によって確認した。配列を、製造業者のプロトコルの改変版に従って、OneStep RT-PCRキット(QIAGEN社,Valencia,CA)を用いるRT-PCRによって取得した。簡単に説明すると、各クローンのからの細胞を、凍結細胞バンクのバイアルから擦り取り、リボヌクレアーゼを含まない水に再懸濁した。次いで、この細胞懸濁液を、重鎖、カッパ又はラムダ可変ドメインに隣接する遺伝子特異的プライマーのセットを用いるRT-PCR反応の鋳型として使用した。これらの断片の各々の定常ドメイン内のリバースプライマーを用いて配列決定を行った。次いで、配列を、重複配列を含有するプライマーを用いて可変ドメインを増幅することによってscFvフォーマットに変換し、NEBuilder(登録商標) HiF DNAアセンブリークローニングキット(New England Biolabs,Beverly,MA)を用いて哺乳類発現ベクター内にアセンブルした。

本明細書に開示される二重特異性CD123結合分子を、ヒトHEK293細胞の一過的トランスフェクションと、場合によっては、またCHO細胞の安定なトランスフェクションの両方によって生成した。トランスフェクト細胞を、プロテインAアフィニティークロマトグラフィーによって細胞培養物の上清から精製した。凝集物がアフィニティークロマトグラフィー後に検出された場合、タンパク質の均一性を確実にするために、第2の分子ふるいクロマトグラフィーも実行した。
癌細胞の表面上のCD123への結合活性が、抗CD123×抗CD3ε分子を保持し、カニクイザルCD123に対する交差反応性を示すことを確認するために、フローサイトメトリーを用いて、構築したCD123結合分子のCD123発現細胞株への結合を定量化した。
CD123(+)Molm-13(Matsuo,Y.ら,1997,Leukemia 11,1469-1477)癌細胞株及びカニクイザルCD123発現CHO細胞上での結合試験を、標準的なフローサイトメトリー系の染色手順によって行った。Molm-13細胞株をDSMZ(Braunschweig,Germany)から取得した。Molm-13細胞株を、提供されたプロトコルに従って培養した。全長カニクイザルCD123タンパク質を安定発現するチャイニーズハムスター卵巣(CHO)細胞を、研究室内で開発した。典型的な実験は、氷上で30分間、0.2%のBSA及び2mMのEDTAを有するPBS緩衝液100μl中0.012nM~1,000nMの範囲の結合分子で、96ウェルプレート中の1ウェルあたり100,000細胞を標識し、その後洗浄し、氷上で30分間、PE標識した最小交差種反応性二次抗体、ヤギ抗ヒトIgG Fcγ、F(ab’)2(Jackson Laboratory)とインキュベートするものである。結合分子からのシグナルを、LSR-II(商標)フローサイトメーター(BD Biosciences)を用いて検出し、FlowJoフローサイトメトリー分析ソフトウェアによって分析した。細胞上の結合分子の平均経口強度(MFI)を、二重線の除外後に決定した。EC50値の決定のための非線形回帰分析を、GraphPad Prism 7(登録商標)グラフ化及び統計用ソフトウェアで行った。
CD123とCD3εの両方への二重特異性分子結合がCD123(+)細胞株に対してT細胞の細胞傷害をリダイレクトできるかを確認するために、クロム51放出アッセイを用いて、T細胞によって誘導される標的細胞の溶解を定量化した。
Molm-13ヒト腫瘍細胞株を、提供されたプロトコルに従って培養した。末梢血単核球(PBMC)を、標準的なフィコール勾配を用いて、ヒト血液から単離した。単離細胞を生理食塩水緩衝液で洗浄した。PBMCを、製造業者のプロトコルを用いて、T細胞を活性化するために、ヒトTアクチベーターCD3/CD28 Dynabeads(登録商標)(カタログ番号11131D、Gibco Life Technologies、Carlsbad、California、USA)を用いて24時間培養した。培養24時間後、PBMCを収集し、磁場に置いて、ダイナビーズを除去した。単離細胞を、RPMI培地+10%ヒト血清で洗浄した。アッセイ中に、0.061pM~1000pMの範囲の最終濃度で二重特異性分子の濃度を活性化PBMCに添加した(1ウェルあたり約100,000)。
標的依存性T細胞増殖の誘導時の異なる二重特異性CD123結合分子の有効性を比較するために、TRI123、TRI126、TRI129、TRI130、TRI132及びTRI139を含む6種の異なる抗CD123×抗CD3ε二重特異性分子を、2回の独立した実験で試験した。
関連する非臨床種における薬物動態を決定するために、二重特異性抗CD123×抗CD3分子を、抗CD123×抗CD3分子の薬物動態(PK)評価及び毒性評価について非ヒト霊長類(NHP)種(例えば、カニクイザル)において試験する。NHP CD123への抗CD123結合ドメインの交差反応性を、ELISA又は表面プラズモン共鳴アッセイによって試験する(実施例2と同様)。NHPとヒトCD123の間に85.7%の配列同一性があるので、等しい結合が期待される。PK及び免疫原性アッセイが開発され、これを用いて、カニクイザル血清の存在下で抗CD123×抗CD3分子を検出及び定量化し、かつNHPにおける免疫原性反応を評価することができる。CD123標的分子の関連毒種としてカニクイザルを正当化するこれらの作業が正常に完了したら、単回用量のPK/耐容性実験を行う。研究は、有資格臨床研究機関(CRO)で行うことができる。研究結果を、PKパラメータ、及び薬物の作用機序又は抗薬物抗体(ADA)に起因し得ない説明できない有害事象を含む任意の有害事象について調べる。毒物学者は、研究結果を検証及び解釈し、リードの継続的な開発を支持する。この研究由来のPKパラメータを用いて、非ヒト霊長類におけるさらなる毒性研究及び/又はヒト患者における臨床研究を計画する。
CD123及びCD3を標的とするリード二重特異性分子のヒトにおける安全性及び耐容性を決定するために、以下のフェーズ1臨床試験を実施することができた。抗CD123×抗CD3二重特異性分子の初のヒトでの研究は、ヒト投与における最大耐量(MTD)を特定するための用量漸増研究であり得る。
CD4+及びCD8+T細胞の標的依存性T細胞活性化の誘導時の異なる二重特異性CD123結合分子の有効性を比較するために、TRI123、TRI126、TRI129、TRI130、TRI132及びTRI139を含む6種の異なる抗CD123×抗CD3ε二重特異性分子を、2回の独立した実験で試験した。
CD123への二重特異性タンパク質の動態及び親和性を決定するために、TRI-129及びTRI-130二重特異性タンパク質を、HEK293細胞において一過的トランスフェクションによって発現させ、親和性精製とサイズ排除精製の組み合わせを用いて精製した。CD123の細胞外ドメインを、C末端上にアフィニティタグをつけて一過的に発現させ、親和性精製とサイズ排除精製の組み合わせを用いて精製した。分析を、BIACORE(商標) T200装置(Biacore社、Piscataway,NJ)を用いて行った。BIACORE(商標) T200は、リアルタイム動態結合データを提供するように設計されている表面プラズモン共鳴(SPR)ベースのバイオセンサーシステムである。
CD123結合ドメインの熱安定性を、示差走査熱量測定(DSC)を用いて評価した。DSCは、一定速度で加熱した時に、分子の熱変性と関連する熱容量変化を測定する。CD123結合ドメインscfvのTm(熱転移の中点)値は、安定性の尺度であり、融解曲線から導くことができる。
二重特異性分子の薬物動態活性を決定するために、雌Balb/cマウスに200mg(約10mg/kg)のTRI129又はTRI130のいずれかを静脈内(IV)注入した(各分子についてn=30、TRI129又はTRI130)。各時点で、血液を3匹のマウスから心臓穿刺によって採取した。時点は、15分、2、6、24、48、72、96、168、336及び504時間目であった。血液を血清に処理し、分注し、凍結した。血清試料中のTRI129及びTRI130の濃度を、酵素結合免疫吸着測定法(ELISA)によって決定した。経時的血清濃度を用いて、非コンパートメント解析(NCA)及びコンパートメント解析によってPKパラメータ推定値を決定した。経時的血清濃度プロファイルを、Phoenix 64ソフトウェアを用いて解析した。GraphPad Prism 7.0を用いてグラフをプロットした。得られたデータのNCAパラメータ解析を表10に提供する。
TRI129及びTRI130が、インビボで腫瘍成長を阻害し、生存を延長することができるかどうかを決定するために、ヒト急性骨髄性白血病のげっ歯類モデルを利用した。MOLM-13/LUC細胞をドナーT細胞及びマトリゲルと混合し、研究の0日目にNOD/SCIDマウスの側腹部に移植した。グループあたり10匹のマウスのグループの動物に、研究0、4、及び8日目に全容量200μl中15、3及び0.6μgのタンパク質の用量で、ビヒクル、TRI129又はTRI130と共に1匹のドナーからのT細胞で処置した。研究中、経時的に腫瘍成長をノギスで測定し、マウスがエンドポイントに達した各時点で生存事象を記録し(腫瘍体積1500mm3以上)、安楽死させた。図8及び図9に示すように、ドナーT細胞の非存在下でTRI129及びTRI130の影響は最小限であり、ドナーT細胞のみによる腫瘍成長に対するT細胞の影響は最小限であった。ドナーT細胞の存在下で試験した全用量で、TRI129とTRI130の両方での処置後に、腫瘍成長の顕著な阻害が見られた。生存曲線の比較のためのログランク検定と共にカプラン・マイヤー生存解析を用いて、マウスの生存期間中央値の顕著な違いを決定した。図10に示すように、共移植ヒトT細胞の存在下で、TRI129及びTRI130の全ての試験用量で処置したマウスの生存期間は、全対照グループに対して顕著に延びた。ビヒクル処置でのT細胞の共移植も、ヒトT細胞の非存在下でのTRI129又はTRI130での腫瘍の処置も、腫瘍細胞のみを移植し、ビヒクルで処置したマウスと比較して、生存期間は延長しなかった。この結果から、TRI129とTRI130の両方が、急性骨髄性白血病の異種移植モデルにおいて腫瘍成長の阻害及び生存期間の延長を促進できることが実証される。
TRI130の特異性を確認し、異なるレベルのCD123を発現する腫瘍細胞株に対するその細胞傷害活性を比較するために、クロム51放出アッセイを用いて、T細胞によって誘導される標的細胞溶解を定量化した。CD123陽性MOLM-13及びKG-1a AML腫瘍細胞株に対するCD123発現レベルを、フローサイトメトリーによってCD123陰性Daudi Burkittリンパ腫細胞株と比較した。CD123発現を、市販のヒトCD123フィコエリトリン(PE)コンジュゲート抗体を用いて検出した。陽性CD123発現は、MOLM-13及びKG-1a上で検出されたが、Daudi細胞上では検出されなかった。MOLM-13及びKG-1a細胞は、それぞれ平均して細胞あたり10,000及び2,000個の受容体を発現した(データ示さず)。異なる濃度のTRI130を、MOLM-13、KG-1a又はDaudi細胞株とインキュベートし、ヒト末梢血から新たに単離したヒトT細胞を精製し、次いで、24時間、抗CD3×抗CD28結合ビーズと事前活性化した。TRI130は、CD123発現について陽性である細胞株においてのみ細胞傷害を誘導し、MOLM-13とKG-1a AML腫瘍細胞株の間で同等の効力を実証した(図12)。細胞傷害を、4時間及び24時間の時点でクロム51放出アッセイを用いて推定した特異的腫瘍細胞溶解の割合として測定した(データ示さず)。これらの結果から、TRI130によって誘導されたリダイレクトT細胞細胞傷害が、CD123発現標的細胞に特異的であることが確認され、異なるレベルのCD123タンパク質を発現するAML腫瘍細胞株に対して強力なTRI130細胞傷害を実証する。
異なるエフェクター対標的細胞比でTRI130誘導性細胞傷害活性を調べるために、中程度のレベルのCD123を発現するKG-1a細胞を、異なる濃度のTRI130とインキュベートし、事前活性化されていない5人の異なるヒトドナーからT細胞を精製した。KG-1a標的細胞を、10:1、5:1及び1:1のエフェクター対標的細胞比でT細胞と培養した。全てのエフェクター対標的細胞比でのTRI130誘導性細胞傷害活性は、全5人のドナー間で同等であった(データ示さず)。細胞傷害を、特異的腫瘍細胞溶解の割合として測定し、24時間のクロム51放出アッセイを用いて推定した。これらの結果から、有力なTRI130は、未処理T細胞を用いて、異なるエフェクター対標的細胞比で細胞傷害を誘導し、正常ドナー間での活性の変動は最小限であることが実証される。
単回用量の薬物動態研究及び耐容性研究を行い、TRI130の静脈内投与後の耐容性、免疫原性及び薬物動態学的特性を決定した。研究計画を表11に示す。意図されたヒト投与経路であるので、静脈内投与経路を本研究のために選択した。
b グループ1及び2の投与を、グループ間で遅れることなく、継続的に開始した。投与は、グループ2の第3の動物の終わり及びグループ3の第1の動物の開始から2時間遅れた。グループ4の投与は、グループ1の投与の開始から24時間遅れた。
TRI130を捕捉するためのCD123の細胞外ドメイン、及び結合したTRI130を検出するための抗CD3結合ドメインを認識するビオチン化抗ID抗体(5H5)を用いる標準的なELISA法を用いて、血清濃度を決定した。ほとんどの動物にとって、遅い時点での血清濃度の急速な低下は、抗薬物抗体(ADA)の存在に相当した。TRI130+/-ビオチンを用いる標準的な架橋ELISAを用いて、遅い時点で収集した血清試料中のADA力価を測定した。TRI130で処置した9匹中7匹の動物のADA力価は比較的低く、力価の値は、経時的に増加する傾向にあった(データ示さず)。しかしながら、力価はまた、TRI130の用量の増加と共に減少するように思われ、ADAの開始は高用量では遅く、通常、予想されるように、より高い用量は免疫原性のレベルの増加と相関しないことを意味した。TRI130はヒトタンパク質であるので、NHP血清試料における遅い時点でのADAの検出は正常応答とみなされ、NHPにおける免疫原性はヒト応答を予測しない。
グループ平均を、まばらなサンプリング及び適切な重み付けで、WinNonlin(登録商標)事前コンパイルモデル番号7を用いて解析した。

カニクイザルへのTRI130の単回用量投与は、1mg/kg以下の用量レベルで臨床的に、十分に耐容性を示した。これらの用量では、臨床所見又は体重の変化、摂餌量、臨床化学又は凝固パラメータに関連する試験品はなかった。
**動物4002のTRI130の血清濃度Tmaxは6時間であり、皮下投与経路が示唆され、該動物においてTRI130のより遅い蓄積が示された。この動物のピークサイトカインレベルは、他の動物と比較していくぶんか遅れた。
確立した播種性腫瘍を有するマウスにおけるTRI130の治療有効性を調べるために、ヒト急性骨髄性白血病のげっ歯類モデルを利用した。ホタルルシフェラーゼを発現するように改変されたMOLM-13細胞を用いて、生物発光イメージングによる腫瘍定量化を可能にした。本研究のために、100,000 MOLM-13 Luc細胞を、24匹のNSG雄マウスの外側尾静脈に静脈内注射した。MOLM-13 Luc細胞は、処置開始前の4日間、マウス内で確立させた。マウスを各8匹の3グループに割り当てた。1つのグループに、対照としてさらなる処置を施さなかった。残りの2グループに、第1の処置で700万個のヒトT細胞を投与した。処置は、4、8及び12日目で、PBSビヒクル対照又は3μgのTRI130からなっていた。処置は、尾静脈を介して静脈内に施した。分子を含有しない(ビヒクル対照)又は3μgのTRI130を含有する0.2mLのダルベッコリン酸緩衝生理食塩水(PBS)で処置剤を送達した。注射液には、4日目の処置のみ、700万個の精製ヒトT細胞を含んでいた。TRI130処置は、骨格腫瘍量に急激で顕著な低下をもたらした(図16、図17及び表16)。残存非骨格腫瘍量は、TRI130処置後も増えた。剖検及び本研究の終了時に、この残存腫瘍量が雄の生殖管と関連し、ビヒクルとTRI130の両方のグループにおいて骨格腫瘍量の有無の間の主な違いとして観察されたと決定付けた。雄マウスにおける確立された播種性MOLM-13 Luc腫瘍のTRI130での処置は、T細胞単独の対照に対して、腫瘍量の有意な(p<0.0001)低下をもたらした(表16)。MOLM-13 Luc単独のグループとビヒクル対照グループの間に顕著な違いはなかった。腫瘍量分布は、TRI130処置によって変わり、骨格部位では生物発光シグナルが消えた。
TRI130活性と別の二重特異性形式を比較するために、Chichiliら、Sci Transl Med.2015 May 27;7(289):289ra82に報告された形式で、精製を可能にするためにアビジン-Flag-HIS配列が付加されたWO 2015/026892から取得したMGD006のCD123及びCD3結合ドメイン配列(核酸配列は配列番号2及び4に対応し、アミノ酸配列は配列番号1及び3に対応する)を含有する抗CD123×抗CD3二重特異性分子を作製した。得られたMGD006様構築物をTRI168と呼ぶ。
インビトロでTRI130及びTRI168タンパク質の活性を比較するために、T細胞を、正常ドナーの末梢血単核球(PBMC)から単離し、CD123+腫瘍細胞(MOLM-13)の存在下で様々な濃度のTRI130及びTRI168とインキュベートした。
TRI130が原発性AML細胞において細胞傷害を誘導できるかどうかを決定するために、AML対象からのPBMC試料を、様々な濃度のTRI130と数日間培養した。AMLの細胞傷害を、培養4日後に、フローサイトメトリーによって評価した。遠心分離し、上清を除去した後、細胞ペレットを、以下の色素標識抗体:CD69-FITC、CD5-APC、CD19-パシフィックブルー、CD25-PE-Cy7、及び7AADの混合物に50μl容量で再懸濁し、氷上で30分間インキュベートした。細胞を2回洗浄し、BD LSRIIフローサイトメーターでの各ウェルの50%取得の直前に120μl容量に再懸濁した。FlowJoソフトウェアを用いて、試料ファイルを分析し、前方散乱対側方散乱で、7AAD-、CD5-、CD19-、CD25-細胞を順次ゲーティングすることによって、非B細胞、非T細胞コンパートメントにおいて細胞傷害を定量化した。GraphPadプリズム7.0を用いてグラフをプロットした。TRI130は、試験した両方のドナーにおいて、非B細胞、非T AML細胞試料の用量依存性喪失を誘導した(図22)。この図は、培養96時間後、生存可能な非B細胞、非T細胞の合計数を示す。類似の結果が、24時間及び48時間の時点で得られた(データ示さず)。これらの結果から、TRI130が、原発性ヒトAML PBMC試料の培養物において細胞傷害を誘導できることが実証される。
