Description
本開示は、特発性肺線維症(IPF)を撮像および/または処置するための結合体およびその使用法を提供する。具体的には、線維芽細胞活性タンパク質(FAP)を標的とするイメージング剤または治療剤をIPFに送達し、IPFの診断の指標とするか、またはIPFの病的細胞外マトリックス沈着を有意に減少させる。
線維性疾患は、多数の個人に影響を及ぼす世界的な健康上の主要な問題を構成するものである。

一般
H-Cys(Trt)-2-Cl-Trt樹脂および保護アミノ酸は、Chem-Impex Intlから購入した。2-(ヒドロキシメチル)ピリジン-5-ボロン酸ピナコールエステルは、Combi-Blocksから購入した。6-ブロモ-4-ヨードキノリン、2-4-ジフルロベンゼン-l-スルホニル-クロリド、および5-ブロモ-2-メトキシピリジン-3-アミンは、ArkPharmから購入した。他の化学物質は全て、SIGMA-AldrichおよびFisher Scientificから購入し、受領したままで使用した。薄層クロマトグラフィ(TLC)は、Merckシリカゲル60 F254 TLCプレート上で行った。シリカゲルカラムクロマトグラフィは、シリカゲル(粒径60~120μm)を用いて実施した。分取逆相高性能液体クロマトグラフィ(RP-HPLC)は、Waters、XBridgeTM Prep C18、5μm;19×100mmカラム、移動相A=20mM酢酸アンモニウム緩衝液、pH5または7、B=アセトニトリル、30分の勾配システム、13mL/分、λ=254/280nmで実施した。LRMS-ESI(LC-MS)は、Agilent LCMS 1220システム、およびWaters、XBridgeTM RP18、3.5μm;3×50mmカラム、移動相A=20mM炭酸水素アンモニウム緩衝液、pH5または7、B=アセトニトリル、12~15分の勾配システム、0.75mL/分、λ=254/280nmで記録した。高分解能質量測定は、LTQ Orbitrap XL質量分析計で、エレクトロスプレーイオン化(ESI)を用いて記録した。
FAPリガンド(11)、FAP-FITCおよびFAP-S0456を以前に発表された手順に従って合成した。国際公開公報第2018111989A1号参照。
2-4-ジフルロベンゼン-l-スルホニル-クロリド2(1当量)を、ピリジン中の5-ブロモ-2-メトキシピリジン-3-アミン1(1当量)の冷溶液にゆっくり加えた。反応混合物を周囲温度で16時間撹拌し、その時点で反応混合物を水で希釈し、固体をろ過し、大量の水で洗浄した。沈澱を高減圧下で乾燥して化合物3を得、それ以上精製せずに次の段階で用いた。LRMS-LC/MS (m/z): [M + H] + C12H9BrF2N2O3S、計算値377.9;実測値378.9)

6-ブロモ-4-ヨードキノリン6(212.37、0.636mmol)および2-(ヒドロキシメチル)ピリジン-5-ボロン酸7(150mg、0.636mmol)を、無水1,4-ジオキサン(15mL)に溶解した。これにPd(dppf)2Cl2(19.9mg、0.024mmol)と、続いて2M Na2C03(2.5mL)を加えた。次いで、混合物を6時間加熱還流した。室温まで冷却した後、固体をろ過し、蒸発させた。粗生成物をフラッシュクロマトグラフィ(EtOAc:MeOH)で精製して、化合物8を得た。LRMS-LC/MS (m/z): [M + H] + C15H11BrN2O、計算値314;実測値315)
化合物5(136mg、0.32mmol)および化合物8(110mg、0.32mmol)を無水1,4-ジオキサン(50mL)に溶解した。これにPd(dppf)2Cl2(10mg、0.012mmol)と、続いて2M Na2C03(8mL)を加えた。次いで、混合物を6時間加熱還流した。室温まで冷却した後、固体をろ過し、残渣を蒸発させた。粗生成物をフラッシュクロマトグラフィ(EtOAc:MeOH)で精製して、PI3KI1を得た。LRMS-LC/MS (m/z): [M + H] + C27H20F2N4O4S、計算値534.1;実測値535.1)
SUH78017(50mg、0.094mmol)および化合物9(32.7mg、0.094mmol)をDMF(1mL)に溶解し、撹拌した。反応の進行を、分析LRMS-LCMSでモニターした。反応完了後、粗生成物を分取RP-HPLC[A=2mM酢酸アンモニウム緩衝液(pH7.0)、B=アセトニトリル、溶媒勾配5%Bから80%B/35分]で精製して、80%の化合物10を得た。LRMS-LC/MS (m/z): [M + H] + C35H27F2N5O6S3、計算値747.1;実測値748.1)
化合物10(22.3mg、0.019)および化合物11(10mg、0.018mmol)を無水DMSOに溶解し、不活性雰囲気下で撹拌した。反応の進行を、分析LRMS-LCMSでモニターした。反応完了後、粗生成物を、分取RP-HPLC[A=2mM酢酸アンモニウム緩衝液(pH7.0)、B=アセトニトリル、溶媒勾配5%Bから80%B/35分]で精製して、80%の最終生成物FAP-PI3KI1を得た。LRMS-LC/MS (m/z): [M + H] + C52H48F4N10O12S3、計算値1177.2 実測値1179.1)。
IPF患者細胞株は、Dr. Ivan O. Rosas、M.D. Brigham and Women's Hospital、Boston、MAからご供与いただいた。C57BL6/6-NCrl(系統コード(Strain code):027)マウスはCharles Riverから購入し、通常の齧歯類飼料で飼養した。マウスは試験期間中、標準の12時間明暗周期で滅菌環境に収容した。すべての動物手順は、NIHガイドラインに従い、Purdue Animal Care and Use Committee(PACUC)によって承認された。
FAP-FITC内部移行の生細胞撮像
HLF1-hFAP細胞をガラス底の皿に播種し、充分な量のエンドソームトラッカー(Rab7a-RFP、ThermoFisher)と共に終夜インキュベートした。次いで、細胞をFAP-FITC(10nM)と共に4℃で1時間インキュベートした。その後5nM DRAQ5核色素(ThermoFisher)で染色し、PBSで3回洗浄し、FAP-FITCの空間局在を周囲温度下、共焦点顕微鏡(FV 1000、Olympus)により、所与の時点でモニターした。共焦点画像をFV10-ASW、Olympusソフトウェアによりさらに加工した。
HLF1細胞、一次IPF線維芽細胞および非IPF線維芽細胞を、免疫蛍光染色のためにガラス底の皿で培養し、固定し、透過処理した。hFAPに対する一次抗体(1:200、FAB3715R、R&D Systems)またはαSMAに対する一次抗体(1:1000、ab2l027、Abcam)を、4℃で終夜インキュベートした。PBS洗浄後、Alexa Fluor(登録商標) 488 anti-Goat抗体(Abcam、1:400)の二次抗体と共にインキュベートした。共焦点顕微鏡で画像を取得し、分析した。
コンフルエントなHLF1細胞を、10%FBS含有DMEM培地に播種し、次いで0.4%血清飢餓培養を終夜行った後、コラーゲン分泌を刺激した。TGFb1(0.1ng/ml)を細胞に、PI3K阻害剤と共に、または阻害剤なしで加えた。同時インキュベーション後2日の時点で、全分泌コラーゲンレベルを定量するために、培地を回収した。全コラーゲンレベルを、Sirius Red Total Collagen Detection Kit(Chondrex, Inc)で定量した。基本的に、濃縮試料を500mlのシリウスレッド溶液と共に室温で20分間インキュベートした。10,000rpmで3分間遠心沈降してペレットを回収し、続いて500mlの洗浄溶液で3回洗浄した。シリウスレッド染色したペレットに250mlの抽出緩衝液を加え、510~550nmでODを読み取った。
血清飢餓培養したコンフルエントなHLF1細胞を、1ng/ml TGFb1含有培地中、指定の濃度のPI3K阻害剤と共に、または阻害剤なしで24時間同時インキュベートした。ウェスタンブロット分析のために細胞を回収し、溶解した。ドデシル硫酸ナトリウムポリアクリルアミドゲル電気泳動およびブロッキングの後、膜をpSMAD2Ser465/467(#3101、Cell Signalling Technology)、またはpAktSer473(#4060、Cell Signalling Technology)を検出するための抗体と共にインキュベートし、シグナルをECL Western Blotting Detection Reagents(GE Healthcare)で可視化した。ストリッピングの後、膜をブロックし、全SMAD2(#3103、Cell Signalling Technology)または全Akt(#4060、Cell Signalling Technology)に特異的な抗体でリプローブした。
ブレオマイシン誘導肺線維症モデル
8~10週齡のC57BL/6-NCrl(系統コード:027)雄マウス(Charles River)を麻酔(キシラジン/ケタミンの混合物)し、続いて50□Lの滅菌リン酸緩衝化食塩水(PBS)中で新しく調製した0.75mg/Kgの硫酸ブレオマイシン(Cayman Chemicals、Cat N13877)を、1回気管内注射した。対照マウスには、50□Lの滅菌PBSを注射した。試験期間中、体重をモニターした。縦断的試験においてFAP発現および線維症にアクセスするために、ブレオマイシン注入後7日、14日、および21日に肺を回収した。FAP-PI3K阻害剤による治療のために、同じ手順を実施してブレオマイシンを投与し、第21日に肺を回収して治療効果にアクセスした(第0日をブレオマイシン投与の日とした)。
全肺コラーゲンを、以前に記載された通りにヒドロキシプロリンの分析によって定量した30。このアッセイのために右肺を常に供して比較を可能にした。簡単に言うと、回収した右肺をPBS(PH7.4)中でホモジナイズし、12N HClにより120℃で3時間消化した。クエン酸/酢酸緩衝液(PH6.0)およびクロラミン-T溶液を室温で20分間で加え、試料をエールリッヒ溶液と共に65℃で15分間インキュベートした。試料を室温まで冷却し、550nmで読み取った。0~100μg/mlの濃度のヒドロキシプロリン標準(Sigma、MO)を用いて標準曲線を作成した。
左肺を膨張させ、4%パラホルムアルデヒドで固定した。肺組織をパラフィン中に包埋し、10□mの切片を調製し、H&Eおよびマッソントリクローム染色を用いて染色した。ブレオマイシン誘導線維症の重症度を、ブレオマイシン投与後の指定の日時に、半定量的病理組織学的スコアリングにより判定した。29
マウスを、5nmolのFAPを標的とするNIR色素結合体(FAP-S0456)の尾静脈(i.v.)注射により処置し、注射の2時間後にSpectral AMI光学撮像システムを用いて撮像した。競合実験のために、100倍過剰の基本のFAPリガンドを用いた。設定は以下のとおりであった:対象物の高さ、1.5;励起、745nm;発光、790nm;FOV、25;ビニング、2;f-ストップ、2;取得時間、1秒。全身撮像の完了後、動物を解剖し、選択した臓器を回収し、完全な生体分布について再度撮像した。縦断的撮像試験のための条件は、マウスをブレオマイシン投与後7日、14日、および21日に撮像した以外は、同じままであった。
全肺のマイクロCT分析を、ブレオマイシン投与後7日、14日、および21日に実施した。簡単に言うと、動物をイソフルランで麻酔し、腹臥位で固定した。マイクロCT画像を、Quantum FX マイクロCTシステム(Perkin Elmer、Waltham、MA)で心ゲーティングを併用し(呼吸ゲーティングなし)、以下のパラメーターを用いて取得した:90kV;160μA;FOV、60×60×60mm;空間分解能、0.11mm、全取得時間は4~5分となった。
本実施例において、FAP標的指向リガンド(FAPL)およびFITCなどのフルオレセインを含むイメージング剤結合体を、図1に示すスキームに従って生成した。
本実施例において、ヒトIPF患者細胞株および非IPF対照細胞株を、それぞれFAP抗体およびaSMA抗体で染色した。共焦点顕微鏡法は、FAPおよびaSMAはいずれもIPF肺線維芽細胞で主に発現されることを示している。図3A参照。図3Bにおいて、IPF患者細胞株をFAPL_FITCと共にインキュベートした場合、フローサイトメトリー分析は、FAPL_FITCで染色された試料を示す。
本実施例において、本発明者らは、PI3KI1と命名した新規汎PI-3キナーゼ-mTOR阻害剤を提供した。この可能性のあるIPF薬は、標的指向リガンド、例えば、実施例2~3のFAPリガンドに結合するための遊離可能リンカーを組み込むための良好なハンドルを有する。
本実施例において、本発明者らは、新規PI-3キナーゼ-阻害剤PI3KI1は、そのGSK対応物オミパリシブよりもAktリン酸化を良く阻害することを示した。後者はIPF治療薬のための臨床試験中である。PI3KI1はHLF-1細胞株でコラーゲン分泌およびコラーゲンゲル収縮も抑制する。新規PI-3キナーゼ阻害剤PI3KI1は遊離ヒドロキシル基を有するため、実施例2および3に示す、FAPリガンドへの容易な結合を可能にする。新規薬物は毒性を有さず、GSK薬オミパリシブと同様にはたらく。図6およびその説明文参照。
本実施例において、本発明者らは、FAPを標的とするPI-3キナーゼ阻害剤FAP_PI3KI1は、IPF患者細胞において遊離PI3KI1および遊離GSK薬オミパリシブよりも良好に、TGFβ誘導コラーゲン分泌を低い薬物濃度で抑制することを確立した。図7およびその説明文参照。
本実施例において、本発明者らは、FAPL_FITCはマウスFAPを認識し得ることを示した。簡単に言うと、NIH-3T3細胞をTGFβでFAPを発現するように誘導した(マウスにおける線維症の発病を模倣)。FAPL_FITCは、マウスFAPに対する特異的モノクローナル抗体に結合する細胞集団のほぼ98%を認識することができる。図8およびその説明文参照。
本実施例において、薬物または他のエフェクターに結合したFAPLは、急性肺傷害後14日のマウスで、線維化肺を特異的に標的とする。簡単に言うと、食塩水処置およびFAP競合ありの線維化肺はいずれも、NIRシグナルの最小限の保持を示した一方で、競合なしの病的肺は、FAP_S0456色素の高い取り込みを示した。急性肺傷害後の異なる時点(7、14、21日)のマイクロCTおよびNIR撮像はいずれも、線維化の進行を示した。
本実施例において、FAPL_PI3KI-SUH処置マウスで、非処置マウスに比べて、線維化の減弱が、生存率の増大およびヒドロキシプロリンの減少を伴って示された。図10およびその説明文参照。
本実施例において、実験的肺線維症の放射性物質を用いた撮像を図19A(様々な臓器)および肺のFAPを標的とする99mTC(図19B)で示す。
