Description
-γ-Abu-Phe-Trp-D-Trp-Lys-Thr-Phe-N-カルバモイルメチル
式中、Abuは、アミノブチリルである。
(i)環状多糖若しくはデキストロース、又はそれらの組み合わせを含む、賦形剤と、ペプチド又はその薬学的に許容される塩とを水中で混合して、第1の水性混合物を形成するステップであって、ペプチド又はその薬学的に許容される塩が、脱イオン水における溶解性が100~350mg/mlであり、脱イオン水に溶けやすいが、生理学的条件下において、又は別様に塩化物イオンを有する溶液において溶解性が2~3mg/mlであり、溶けにくく、ペプチド又は薬学的に許容される塩が、3mg/ml超の濃度で塩析することによって沈殿する、ステップ、
(ii)ポリマーを、ジクロロメタンなどの有機溶媒中に混合して、ポリマー溶液を形成するステップ、
(iii)第1の水性混合物を、ポリマー有機溶液中に混合して、油中水型一次エマルションを含む第1の分散体混合物を形成するステップ、
(iv)ポリビニルアルコール(PVA,polyvinyl alcohol)を、リン酸緩衝生理食塩水又は生理食塩水中に、0.1~3重量%の量で混合して、第2の水性混合物を形成するステップ、
(v)一次エマルションを、PVAの第2の水性混合物中に混合して、水中油中水型の二重エマルションを形成して、二次分散体混合物を提供するステップ、
(vi)二次分散体混合物中の有機溶媒を蒸発させて、固体高分子ミクロスフェアを形成するステップであって、ペプチド又はその薬学的に許容される塩が、高分子ミクロスフェア内にカプセル封入されている、ステップ、
(vii)高分子ミクロスフェアを洗浄及び単離するステップ、並びに
(viii)乾燥過程中に、界面活性剤及びマンニトールの混合物を添加して、又は添加せずに、制御条件下でミクロスフェアを乾燥させるステップ
を含む、前記方法も提供する。
賦形剤は、Vベルドレオチド酢酸塩などの注射用ペプチドの溶解性、分散、及びバイオアベイラビリティを改善することができる。そのような賦形剤としては、疎水性捕捉剤、糖、及びアミノ酸、例えばヒドロキシプロピル-β-シクロデキストリン、L-リジンHCl、及びデキストロースなどを挙げることができる。
β-シクロデキストリン、例えばヒドロキシプロピル-β-シクロデキストリン(HP-β-CD又はHPBCD,hydroxypropyl-β-cyclodextrin)などは、本発明において使用するための、例示的部類の賦形剤である。HPBCDは、1-4結合を通じて接合された7個のデキストロース単位で構成される環状オリゴ糖であり、比較的親油性の内側と比較的親水性の外側を保有し、包接複合体を形成する傾向がある(Chang、米国特許第7,259,153号明細書)。
ベルドレオチド酢酸塩の溶解性は、塩化物イオンを含有する溶液において低減されることが観察されている。例えば、ベルドレオチド酢酸塩は、0.1NのHClに対しては溶けるが、0.9NのHClに対しては溶けない。同様に、ベルドレオチド酢酸塩は、0.9%生理食塩水及びリン酸緩衝生理食塩水を含む、塩化物を含有する等張溶液に対しては、制限された溶解性を有する。
また、予想外なことに、等張溶液及び生理学的溶液中のベルドレオチド酢酸塩のバイオアベイラビリティが、賦形剤としてデキストロースを添加することによって改善されることも発見された。ベルドレオチド酢酸塩は、5%デキストロース(USP(米国薬局方))に対して約400mg/mlの溶解性を有し、溶けやすいことが分かっている。驚くべきことに、5%デキストロースは、生理学的媒体(生理食塩水又はリン酸緩衝生理食塩水)中の高濃度のベルドレオチドの溶解性を改善しない。これは、5%デキストロース水溶液に対するベルドレオチドの非常に高い溶解性とは反対である。LARの薬物動態学的プロファイル及びバイオアベイラビリティの改善は、使用されたデキストロース:ペプチドの比率が低いにもかかわらず(1:8)達成される。そのような低い比率が、生理学的媒体に対するペプチドの溶解性を改善することは予想されていないが、ここでは、表面張力の挙動によって裏付けられるように、このペプチドの物理的性質を調節することが示されている。
ミクロスフェア製剤
環状ソマトスタチン類似体の薬物動態を考慮すると、多くの場合、インビボでの持続放出用の製剤を提供することが望ましい。持続放出を実現するために、例えばPLGAミクロスフェア及びヒドロゲルの使用などの、様々なポリマー系製剤の使用を含む、いくつかの方法を使用することができる。また、エマルション及びリポローム製剤も有用であり得る。エマルション、ゲル、ミクロスフェア、及びリポソームを含む持続放出性製剤が、注射部位反応を減少させ、バイオアベイラビリティを改善し、持続放出性プロファイルを提供することができる一方で、即時放出性製剤用の賦形剤に関する所見に基づき、そのような賦形剤の持続放出性製剤における使用について調査すると、驚くべきことに、そのような賦形剤の使用は、インビボでの環状ソマトスタチン類似体の薬物動態を改善し、注射部位反応を低減させることが分かった。
また、ベルドレオチド酢酸塩は、ベルドレオチド酢酸塩の血漿濃度が治療有効レベルにある、持続放出性プロファイルを少なくとも48時間呈するリポソームへと製剤化できることも分かった。
先述されたように、ベルドレオチド酢酸塩は、界面活性剤として作用し、水を油中に乳化できることが分かった。加えて、その両親媒性の性質を考慮すると、ベルドレオチド酢酸塩は、ヒドロゲルへと製剤化することもできる。ビヒクルは両方とも、持続放出性製剤を提供することができる。

水及び等張5%デキストロースに対するCOR005酢酸塩の溶解性(下を参照されたい)は、比較的高い(少なくとも300mg/ml)。一方で、水に対するその高い溶解性と比較すると、COR005酢酸塩の溶解性は、等張PBS pH7.4及び生理食塩水において観察されるように、生理学的(水性)条件においては有意に低い(1~2mg/mlの範囲内)。両方の媒体(PBS又は生理食塩水)は生理学的濃度の塩化物を含んでおり、このことは、塩化物イオンの存在下におけるペプチドの不適合性及び塩析を示していることに留意されたい。
デキストロース:500mg/ml超という、(水中)5%デキストロースに対するCOR005酢酸塩の比較的高い溶解性は、デキストロースを伴う水に対するペプチドの改善された溶解性を示している。等張生理食塩水に対するデキストロースの添加は、2.5%のデキストロース及び0.45%の生理食塩水(NaCl)という最終濃度を有する、通常生理食塩水中に希釈された5%デキストロースに対して、2mg/mlから約6~7mg/mlというペプチドの溶解性における増加をもたらす。なお、生理食塩水中に1:1で希釈されたこのデキストロースを、ラット及びミニブタにおけるCOR005酢酸塩のIR研究のビヒクルとして使用した(DEX:PEPの比率は5:1であった)。
表4のデータは、WFI中の、添加剤を伴う又は伴わない、高度に濃縮されたCOR005酢酸塩(200mg/ml)の表面活性を示しており、これは、ミクロスフェアの一次エマルションの調製条件を模している。試験1及び2の結果(表4)は、水の表面張力に対するCOR005酢酸塩の効果を示している。COR005酢酸塩が水に溶解された場合、表面張力は、水単独の場合の68.5ダイン/cmから、200mg/mlのCOR005酢酸塩を伴う水溶液の場合の44ダイン/cmまで、約36%低減される。
HPB:PEPの比率1:2でのHPBの添加は、ペプチドの表面活性の低減をもたらす。観察された、添加物を伴わない水中におけるペプチドの表面活性(44ダイン/cm)は、HPBが1:2の比率で添加されると、水(68ダイン/cm)へと増加する方向に戻される(52ダイン/cm)。この結果は、HPBが、水に対するペプチドの溶解性を増加させることによって、界面活性剤として作用するペプチドの潜在性を低減していることを示している。低減されたペプチドの表面活性及びより高い水溶性の結果は、境界面に存在するペプチドがより少なくなることである。
試験4の結果(表4)は、HPB:PEPの比率を1:2(製剤B13)から1:4(製剤B14)に低減したことの効果を示している。表面張力の結果は、HPB:PEP 1:4の低減された濃度(45ダイン/cm)が、水中におけるCR005酢酸塩と同じ表面活性(44ダイン/cm)を有することを示しており、この低めの比率においては、ペプチドの界面活性剤の性質に対するHPBの効果はないことを示している。
試験5の結果(表4)は、DEX:PEPの比率が1:8でMS製剤に添加された場合のDEX:PEPの効果を示している。表面張力の結果は、DEX:PEP 1:8の溶液の見かけの表面張力(54ダイン/cm)が、試験3のHPB:PEP 1:2の場合(51ダイン/cm)に匹敵することを示している。この効果は、より多くのペプチドが、境界面ではなくバルク溶液中に集中することを示唆している。実際、ペプチドに対するこれらのDEX及びHPBの比率の下でのペプチドの表面活性は、ペプチド単独の場合(44ダイン/cm)と比較して、低減される。水中に集中する添加剤は、一次エマルションの間、ペプチドの周囲に引き付けられる水の増加につながり得る。このことは、B13製剤で起こることが予想されたような、内部マトリックスにおける比較的高めの含水量に起因する、ミクロスフェアの多孔性の増加につながり得る。
COR005の薬物動態の分析を、PK Solutions 2.0ソフトウェア(Summit Research Services, CO.社 USA)を使用して行った。このソフトウェアは、いかなる特定のコンパートメントモデルも推定することなく、非コンパートメント(面積)及びコンパートメント(指数項)の方法を使用して、結果を計算する。複数回用量及び定常状態パラメーターは、単回用量の結果から自動的に算出される。PKパラメーターの計算は、2つの標準的分析方法:(1)血液レベル曲線を説明する指数項を導出するための曲線回帰(又は残差法)、及び(2)曲線化面積の計算に基づいた。PK Solutions 2.0は、該当する場合、両方の方法を適用し、結果を並べて比較する。
SEM画像撮影の第1の目的は、様々なMS製剤の外部形態を評定及び比較し、液滴サイズ及び表面張力に対する添加剤HPB及びDEXの効果を検証することであった。第2の目的は、様々なMSマトリックスの内部構造を評定及び比較することであった。より具体的には、COR005、添加剤(DEX及びHPB)、及び疎水性PLGAポリマーの間の、示唆される独特な相互作用(一次エマルションの間、及び最終的MSから注射部位においてペプチドが放出される間)、並びに(添加剤を伴う又は伴わない)COR005の観察されるIVR及びPKプロファイルに対する、これらの相互作用の効果と相関する、様々な内部マトリックス間における可能な差異を特定することであった。
図21及び22は、それぞれ、製剤B12(PLGA-MS-DEX)の外部形態及び内部形態のSEM画像を描写している。図21に描写されている画像は、基礎的B10製剤において観察された非常に少ない細孔と比較して、表面に見える多くの細孔の結果としての外部表面の有意に高い多孔性を有する、製剤B12の代表的MSを示している。図22においては、MSの低温切断後の、製剤B12の内部マトリックスが曝露されている。この画像は、比較的薄い組織内の高分子境界によって分離された有意に多数の空隙を示しており、これらの境界が、内部マトリックス内の高密度の小さな空隙と、したがって空隙容積の増加とをもたらす。この増加された空隙容積による、第1の予想される影響は、内部マトリックスにおけるポリマー密度の低減である。所定の体積のMSに関して、MS内部の空隙がより多いことは、空隙の内部空間に起因して、ポリマーがより少なくなることに等しい。この高い空隙容積による、第2の予想される結果は、MSにおけるペプチド含有量の低減である。マトリックス内のポリマーがより少ないことは、MS内部におけるペプチドのカプセル封入がより少なくなることにつながり得る。この高い空隙容積による、第3の予想される影響は、製剤B10の基礎的MSの中実かつより密集したマトリックスと比較して、放出持続期間が短くなり、製剤B12 MSからのカプセル封入されたペプチドの放出が急速になることである。
図23及び24は、製剤B13 MSの外部形態及び内部形態のSEM画像を描写している。図25は、製剤B13において使用されたものと同量のHPBを伴う、対照として調製されたプラセボMS(COR005酢酸塩を添加しなかった)の外部形態及び内部形態を描写している。図23に描写されているように、製剤B13の外部形態は、PLGA-MSにおける多孔性薬剤としてのHPBの予想された効果を支持している。幅広い範囲の細孔径を有する多くの細孔が、表面に存在している。しかしながら、図24は、低温切断によって切断された後の、製剤B13 MSの内部構造を示しており、これは、製剤B13の内部形態と製剤B12との間の有意な差異を指摘している。製剤B13の内部構造は、製剤B12における空隙のサイズと比較して、空隙サイズにおける有意な増加を呈している(図22)。また、製剤B13における内部のより大きい空隙の数は、製剤B12において見られた高密度の多くの小さな空隙と比較して、低減されている。これらの結果は、製剤B13の場合、内部空隙の形成に起因する総表面積の増加は、製剤B12の場合よりも少ないことを示している。
カプセル封入されたペプチド含有量の大部分が即時放出された後、有意に低い血漿レベルが経時的に続くことを示している、製剤B13 IVR及びそのPKプロファイルの予想外なデータは、低減された比率のHPB:PEPを有するMS製剤の設計に関する基礎をもたらした。反直観的なIVR及びPKの結果は、1:2から1:4、さらには1:8へとHPB:PEPの比率を低減させることで、MSからのペプチド放出における予想外な増加がもたらされることを示している。これらのIVR及びPKの結果と、COR005の独特な表面活性との間の結び付きを調査するために、低減されたHPB:PEPの比率を有するMSの詳細なSEM分析を、それらの内部形態を評定することを目的として始めた。図27は、様々な比率のHPB:PEPを有するCOR005 MS製剤の内部マトリックスの形態を比較するSEM画像を示している。
ミクロスフェアの表面積は、Micromeritics ASAP 2020計器において、窒素ガス吸着を使用して測定した。15~25mgのミクロスフェアを、この機械の分析管に添加し、特殊なシールフリットを用いて管の内部に密封する。分析を、2段階:脱ガス及び窒素吸着測定で始めた。脱ガスは、熱及び真空を適用して、ミクロスフェアの内側に湿分及び他の閉じ込められたガスを除去する過程である。ミクロスフェアは、真空(100μmHg)下で脱ガスし、35℃で150分間加熱した。その後、試料管を、計器の分析バルブに移した。試料管を、液体窒素のデュワ内に下ろして、試料を77°Kに冷却する。窒素ガスを制御された様式で添加し、吸着等温線を生成するために、管の圧力をモニタリングする。等温線から、単位質量当たりの表面積を計算するために、ブルナウアー・エメット・テラー(BET)理論を適用する。分析時間はおよそ4時間であり、試料当たりの総実行時間は6.5時間である。

- 高分子ミクロスフェアを製造するための方法であって、
(i)ヒドロキシプロピル-β-シクロデキストリン(HPBCD)若しくはデキストロース、又はそれらの組み合わせを含む、賦形剤と、ベルドレオチド又はその薬学的に許容される塩とを水中で混合して、第1の水性混合物を形成するステップ、
(ii)ポリマーを、ジクロロメタンなどの有機溶媒中に混合して、ポリマー溶液を形成するステップ、
(iii)前記第1の水性混合物を、前記ポリマー溶液中に混合して、一次エマルションを
含む第1の分散体混合物を形成するステップ、
(iv)ポリビニルアルコール(PVA)を、リン酸緩衝生理食塩水又は生理食塩水中に混合して、第2の水性混合物を形成するステップ、
(v)前記一次エマルションを、前記PVAの第2の水性混合物中に混合して、二重エマルションを形成し、それによって二次分散体混合物を提供するステップ、及び
(vi)前記二次分散体混合物中の前記有機溶媒を蒸発させて、ベルドレオチド又はその薬学的に許容される塩を含む、固体高分子ミクロスフェアを形成するステップ
を含む、前記方法。 - 賦形剤が、ヒドロキシプロピル-β-シクロデキストリン(HPBCD)である、請求項34に記載の方法。

- ポリマー溶液が、ポリ(乳酸-co-グリコール酸)(PLGA)を含む、請求項34~40のいずれかに記載の方法。
- 高分子ミクロスフェアを製造するための組成物であって、ベルドレオチド又はその薬学的に許容される塩;ヒドロキシプロピル-β-シクロデキストリン(HPBCD)若しくはデキストロース、又はそれらの組み合わせを含む、賦形剤;及びポリマーを含む、前記組成物。
- 賦形剤が、ヒドロキシプロピル-β-シクロデキストリン(HPBCD)である、請求項43に記載の組成物。
