Description

図13に示すように、SNSは、脳が身体と通信することを可能にする神経のネットワークを提供する。交感神経は、脊柱の内側から中間帯外側細胞柱の脊髄の中央(または側角)に向かって、脊髄の第1の胸部セグメントから始まり、第2または第3の腰部セグメントまで伸びていると考えられている。その細胞は脊髄の胸部領域および腰部領域で始まるので、SNSは胸腰部流出を有すると言われている。これらの神経の軸索は、前部小根/根を通って脊髄を離れる。これらは脊髄(感覚)神経節の近くを通り、そこで脊髄神経の前部分枝に入る。しかし、体性神経支配とは異なり、これらは脊柱に沿って伸びる傍脊椎神経節(脊柱の近くにある)または前椎骨神経節(大動脈分岐部の近くにある)のいずれかに接続する白い分枝コネクタを介して迅速に分離する。
図14が示すように、腎臓は、腎動脈と密接に関連している腎神経叢(RP)によって神経支配されている。腎神経叢(RP)は、腎動脈を囲み、かつ腎動脈の外膜内に埋め込まれている自律神経叢である。腎神経叢(RP)は、腎臓の実体に到達するまで腎動脈に沿って延びている。腎神経叢(RP)に寄与する線維は、腹腔神経節、上腸間膜神経節、大動脈神経節神経節および大動脈神経叢から生じる。腎神経とも呼ばれる腎神経叢(RP)は、主に交感神経成分からなる。腎臓の副交感神経支配はない(または少なくともごくわずかである)。
メッセージは、双方向フローでSNSを通過する。遠心性メッセージは、身体の様々な部分で同時に変化を引き起こす可能性がある。例えば、交感神経系は心拍を加速させ、気管支の気道を広げ、大腸の運動性(運動)を低下させ、血管を収縮させ、食道の蠕動運動を増加させ、瞳孔の拡大、立毛(鳥肌)および発汗(汗)を引き起こし、血圧を上げる可能性がある。求心性メッセージは、体内の様々な臓器や感覚受容体から他の器官、特に脳に信号を伝える。
腎臓への交感神経は血管、傍糸球体装置および腎尿細管で終わる。腎臓交感神経の刺激は、レニン放出の増加、ナトリウム(Na+)再吸収の増加、および腎血流の減少を引き起こす。腎機能の神経調節のこれらの要素は、交感神経緊張の高まりを特徴とする病状においてかなり刺激され、高血圧患者における血圧の上昇に明らかに寄与する。腎遠心性交感神経刺激の結果としての腎血流および糸球体濾過率の減少は、心腎症候群における腎機能不全、すなわち慢性心不全の進行性合併症としての腎機能障害の礎石である可能性が高く、これは臨床的に通常、患者の臨床状態および治療によって変動する。腎遠心性交感神経刺激の影響を阻止するための薬理学的戦略には、中枢作用性交感神経阻害薬、β阻害薬(レニン放出を減少させることを意図)、アンジオテンシン変換酵素阻害薬および受容体阻害薬(アンジオテンシンIIの作用およびレニン放出によるアルドステロン活性化を阻害することを意図)、および利尿薬(腎臓交感神経によるナトリウムおよび水分貯留に対抗することを意図)が含まれる。しかしながら、現在の薬理学的戦略は、限られた有効性、コンプライアンス問題、副作用などを含む著しい制限がある。
腎臓は腎感覚求心性神経を介して中枢神経系の一体構造と通信する。いくつかの形態の「腎臓損傷」は、感覚求心性信号の活性化を誘発し得る。例えば、腎虚血、一回拍出量もしくは腎血流の減少、または多量のアデノシン酵素は、求心性神経伝達の活性化を引き起こし得る。図15および図16に示すように、この求心性伝達は腎臓から脳へのものでも、あるいは一方の腎臓から他方の腎臓へのもの(中枢神経系を介する)でもよい。これらの求心性信号は集中的に統合されており、交感神経流出の増加をもたらし得る。この交感神経駆動は腎臓に向けられ、それによってRAASを活性化し、そしてレニン分泌の減少、ナトリウム保持、体積保持および血管収縮を誘導する。中枢交感神経過活性はまた、心臓および末梢血管系などの交感神経によって神経支配される他の器官および身体構造にも影響を及ぼし、説明したような交感神経活性化の有害作用をもたらし、そのいくつかの局面は血圧の上昇にも寄与する。
上記のように、腎臓神経支配除去は、高血圧、メタボリックシンドローム、インスリン抵抗性、糖尿病、左室肥大、慢性末期腎臓病、心不全における不適切な体液貯留、心腎症候群、および突然死などの全体的および特に腎交感神経活性の増加を特徴とするいくつかの臨床症状の治療に価値があると思われる。求心性神経信号の減少は交感神経の緊張/駆動の全身的減少に寄与するので、腎臓の神経支配除去はまた、全身性交感神経活動亢進に関連する他の状態を治療するのに有用であり得る。したがって、腎臓の神経支配除去はまた、図13で特定されるものを含む、交感神経によって神経支配される他の器官および身体構造にも利益をもたらし得る。例えば、前述のように、中枢性交感神経刺激の減少は、メタボリックシンドロームおよびII型糖尿病を患っている人々を苦しめるインスリン抵抗性を減少させる可能性がある。
さらに、骨粗鬆症の患者もまた交感神経的に活性化されており、腎臓神経支配除去を伴う交感神経駆動の下方制御から恩恵を受ける可能性がある。
本技術によれば、左および/または右の腎動脈と密接に関連している左および/または右の腎神経叢(RP)の神経調節は、血管内アクセスを介して達成され得る。図17が示すように、心臓の収縮によって動いた血液は、大動脈によって心臓の左心室から運ばれる。大動脈は胸部を通って下降し、左右の腎動脈に分岐している。腎動脈の下で、大動脈は左右の腸骨動脈で分岐する。左右の腸骨動脈は、それぞれ左右の脚を通って下降し、左右の大腿動脈を結ぶ。

- 前記血管の回避領域を識別するステップが、小孔、気管分岐部、テーパ領域、石灰化、線維筋形成異常、動脈瘤、分岐部、またはそれらの組み合わせ上でまたはその近傍で前記血管の部分を識別するステップを含むことを特徴とする請求項1から16のいずれか一項に記載のシステム。
- 前記血管の前記計算流体力学モデルを生成するステップが、経験的データまたは集塊データから導出される血行動態パラメータを適用するステップを含むことを特徴とする請求項1から17のいずれか一項に記載のシステム。
- 前記メモリが、前記プロセッサによって実行されたときに、前記システムに、前記識別された標的領域および前記識別された回避領域を示す視覚的な指標を含む前記血管の前記CFD表現をユーザインタフェース上に表示するステップを含む動作を実行させる命令をさらに格納することを特徴とする請求項1から18のいずれか一項に記載のシステム。
- 前記メモリが、前記プロセッサによって実行されたときに、前記システムに、前記血管内の前記現在の神経調節カテーテル位置で神経調節療法を進めるかどうかについて前記プロセッサによって作成された推奨を、前記ユーザインタフェースを介して提供するステップを含む動作を実行させる命令をさらに格納し、前記推奨が、前記神経調節カテーテルの前記現在位置と前記血管の前記識別された標的領域および/または前記識別された回避領域との比較に基づくことを特徴とする請求項1から19のいずれか一項に記載のシステム。
- 前記回避領域を識別するステップが、低い壁剪断応力(WSS)、高いWSS、高いWSS勾配、小孔、気管分岐部、テーパ領域、石灰化、線維筋形成異常、動脈瘤、分岐、血流分離領域、渦、衝突血流領域、乱流領域、二次血流領域、またはそれらの組み合わせを有する前記血管の部分を識別するステップを含むことを特徴とする請求項1から20のいずれか一項に記載のシステム。
- 神経調節療法について血管を評価するための方法であって、前記方法は、
プロセッサで、前記血管の三次元画像に関するデジタルデータを受信するステップ、
前記プロセッサで、前記血管に関する血行動態データを受信するステップ、
前記画像データおよび前記血行動態データに少なくとも部分的に基づいて、前記血管の計算流体力学(CFD)モデルを前記プロセッサで生成するステップ、
前記CFDモデルを参照して、神経調節療法の前記血管の回避領域を識別するステップであって、前記回避領域は、流れの分離、渦形成、流れの衝突、低い壁剪断応力(WSS)、または高いWSS勾配のうちの少なくとも1つの領域を含む、ステップ、および、
前記識別された回避領域を示す視覚的マーカを含む前記血管の表現をユーザインタフェース上に表示するステップ
を含むことを特徴とする方法。
- 前記血管が腎動脈、肺動脈、肝動脈、冠状動脈、または大動脈であることを特徴とする請求項22に記載の方法。
- 前記血管が主血管、前記主血管の少なくとも1つの分岐血管、または前記少なくとも1つの分岐血管に直接結合された少なくとも1つの補助血管、または前記主血管に結合された別の血管であることを特徴とする請求項22に記載の方法。
- 前記プロセッサを介して前記推奨を提供するステップが、前記回避領域を示す信号を前記ユーザに生成するステップを含み、前記信号が、音声信号、視覚信号、触覚信号、またはそれらの組み合わせを含むことを特徴とする請求項22に記載の方法。
- 前記血管の前記表現を表示するステップが、神経調節療法を施す前記血管の標的領域のマーカを表示するステップをさらに含むことを特徴とする請求項22に記載の方法。
- 前記血管の前記表現を表示するステップが、
前記表現上に前記血管の第1の部分を表示するステップと、
前記表現上に前記血管の第2の部分を表示するステップとをさらに含み、該表現は前記第2の部分が前記血管の1つ以上の回避領域に対応し、前記表示された第1の部分が前記表示された第2の部分と比較して低い解像度を有することを特徴とする請求項22に記載の方法。
- 前記血行動態データを受信するステップが、血圧、血流、および/または血液インピーダンスに関するデータを受信するステップを含むことを特徴とする請求項22に記載の方法。
- 前記血行動態データを受信するステップが、外部圧力カフを介して血圧データを受信するステップ、および/または磁気共鳴画像法(MRI)、非侵襲性超音波ドップラシフト流量計、またはそれらの組み合わせを介して血流データを受信するステップを含むことを特徴とする請求項22に記載の方法。
- 前記血管の前記三次元画像に関するデジタルデータを受信するステップが、血管造影法、X線撮像、コンピュータ断層撮影(CT)、MRI、またはそれらの組み合わせを使用して取得されたデータを受信するステップを含むことを特徴とする請求項22に記載の方法。
- 血行動態データを受信するステップが、少なくとも概して層流を有する前記血管の主要部分内の位置で測定された血圧データおよび/または血流データを受信するステップを含むことを特徴とする請求項22に記載の方法。
- 前記推奨を提供するステップが、閾値血行動態パラメータを超え、それによって少なくとも1つの局所的な流れ異常を示す前記血行動態データ内の少なくとも1つの血行動態パラメータを有する前記血管の1つ以上の部分における神経調節療法の回避を推奨するステップをさらに含むことを特徴とする請求項22に記載の方法。

前記プロセッサを介して、神経調節療法に適した前記血管の標的領域および神経調節療法にあまり適していない前記血管の回避領域を識別するステップ、および、
前記識別された標的領域および前記識別された回避領域を示す視覚的な指標を含む前記血管の前記CFD表現をユーザインタフェース上に表示するステップ
を含むことを特徴とする方法。
- 前記回避領域を識別するステップが、低い壁剪断応力(WSS)、高いWSS、高いWSS勾配、小孔、気管分岐部、テーパ領域、石灰化、線維筋形成異常、動脈瘤、分岐、血流分離領域、渦、衝突血流領域、乱流領域、二次血流領域、またはそれらの組み合わせを有する前記血管の部分を識別するステップを含むことを特徴とする請求項35に記載の方法。
- 前記血管内に配置された前記センサが前記神経調節カテーテルのセンサであることを特徴とする請求項35に記載の方法。

神経調節療法に適した前記血管の標的領域および神経調節療法中に回避する前記血管の回避領域を識別するステップであって、前記識別ステップは前記血管の前記CFDモデルに基づく、ステップ、および
ユーザインタフェース上に、前記識別された標的領域および/または前記識別された回避領域を示す視覚的マーカを含む前記血管の表現を表示するステップ
を含むことを特徴とする非一時的コンピュータ可読メモリ。
