Description
本発明は、放射性ポリマー粒子を用いたがんの撮像および治療に関する。特定の局面では、本発明は、放射性核種を有する微小球の使用に関する。
肝細胞がん(「HCC」)は、毎年約750,000の新たな症例が診断されている6番目に多くみられるがんであり、世界中で約700,000の死者数をもたらす3番目に多いがん関連死の原因である。Axelrodおよびvon Leeuwenは、過去20年間にわたってHCCの発生率が「人口100,000人あたり2.6人から5.2人まで2倍以上に増え」、死亡率は100,000人あたり2.8人から4.7人まで増加したと報告している。HCC症例の80%は、危険性の高い行為に関連したB型およびC型肝炎の早期獲得によるものである。加えて、肥満の蔓延が、最終的に線維症、肝硬変、およびHCCまで進行し得る非アルコール性脂肪性肝炎(NASH)の増加に寄与している。

説明の目的のために図面に示されているように、発明は、放射線塞栓術での使用のための新規な放射性微小球において具体化される。本発明の好ましい態様では、新規な放射性微小球は、肝細胞がん(HCC)としても公知の原発性肝臓がんの患者の全生存期間を著しく延ばし得る一方で、先行技術に対して明確な利点および特徴を提供する。しかしながら、身体の別の部分におけるがんを含む別の病状のために発明のさらなる態様が用いられてもよいことが認識されるであろう。

本発明の好ましい態様によれば、放射性微小球の製作は、225Ac(アルファ放出体)および89Zr(陽電子放出体)が両方とも結合した樹脂微小球と、生理的pHおよび温度でのその安定性の検証とからなる。許容基準は、樹脂に対する同位体の結合効率が97%以上である(つまり遊離同位体が<3%)。
用具および化学品:
1)225Ac(0.01M HCl中100μCi、凡その体積=50〜60μl)
2)陽イオン交換樹脂(直径約30ミクロン)、0.01M HCl溶液
3)ガラス器具、磁気撹拌器、実験器具、注射用滅菌水(SWFI)、滅菌pH7.4緩衝溶液(好ましくは無リン酸)
4)放射線防護のために適切に装備された実験室
5)線量較正器およびGeガンマ線分光システム
手順:
1)0.5グラムの陽イオン交換樹脂(直径約30ミクロン)を適切な容器中の0.01M HCl溶液に加え(はじめに5mLそして必要であれば1mLずつ加える)、適度なスラリー懸濁液を形成するために磁気撹拌子を用いて30分間撹拌して確実に樹脂を完全に湿らせる。
2)アクチニウム-225溶液(活性=100μCi、0.01M HCl中、凡その体積50〜60μl)をフラスコに加える。溶液のpHを確認し(約pH2.0になると予想される)、撹拌を90分間再開して確実に微小球における放射活性を均等に分布させる。
3)100uLの一定量を取り出すことによって試料の遠心分離後の遊離Ac-225を確認する。
4)これらの標識微小球を次いで適切なろ過機構(すなわち22ミクロンフィルタ)を用いてろ過し、SWFIで5ml×6回洗浄する。アルファ計数のために各洗浄物を別々に保管する。最後の洗浄液のpHを確認する(6.5〜7.0の間になると予想される)
5)滅菌pH7.4緩衝溶液(5mL)に小球を再懸濁して懸濁液を作る。それを適切なクリンプキャップバイアルに移す。適切な方法を用いて微小球の放射活性を測定し標識量を決定する。
6)アクチニウムについて溶出試験を次のように実施する:
微小球を含有する上記バイアルを37℃の水浴中で20分間かき混ぜる(振動台)。かき混ぜた後はバイアルを37℃でかき混ぜないままにしておくのがよい。遠心分離後、100μLの試料をバイアルから取り、上清のAc活性を測定し記録する。樹脂のAc活性も測定し記録する。溶出試験の期間中は試料を37℃に保つのがよい。この試験を1、2、および3日目に実施する。結果が許容範囲内であれば、7、10、および20日目に溶出活性を測定する。
用具および化学品:
1)89Zr(pH8緩衝溶液中およそ10mCi)
2)陽イオン交換樹脂(直径約30ミクロン)、pH8緩衝溶液(好ましくは無リン酸)、滅菌pH7.4緩衝溶液
3)ガラス器具、磁気撹拌器、実験器具、注射用滅菌水(SWFI)
4)放射線防護のために適切に装備された実験室
5)線量較正器およびGeガンマ線分光システム
手順:
1)0.5グラムの陽イオン交換樹脂(直径約30ミクロン)を適切な容器中のpH8緩衝液に加え(はじめに5mLそして必要であれば1mLずつ加える)、適度なスラリー懸濁液を形成するために磁気撹拌子を用いて30分間撹拌して確実に樹脂を完全に湿らせる。

4)これらの標識微小球を次いで適切なろ過機構(すなわち22ミクロンフィルタ)を用いてろ過し、SWFIで5ml×6回洗浄する。すべての洗浄物を個別の容器に集め各同位体毎に標識効力を測定する。
5)二重標識微小球を集め、5mlの滅菌pH7.4緩衝溶液を入れたバイアルに再懸濁する。
6)Ac-225の溶出試験で記載したよう、規定した時間間隔でZr-89の溶出試験を実施する。
用具および化学品:
1)225Ac(0.01M HCl中100μCi、凡その体積=50〜60μl)、89Zr(滅菌pH7.4緩衝液中10mCi)
2)陽イオン交換樹脂(直径約30ミクロン)、0.01M HCl溶液、滅菌pH7.4緩衝溶液(好ましくは無リン酸)
3)ガラス器具、磁気撹拌器、実験器具、注射用滅菌水(SWFI)
4)放射線防護のために適切に装備された実験室
5)線量較正器およびGeガンマ線分光システム
手順:

7)Zr-89標識効率は、この手順では約60%になると予想される。
8)これらの標識微小球を次いで適切なろ過機構(すなわち22ミクロンフィルタ)を用いてろ過し、滅菌pH7.4緩衝溶液で5ml×6回洗浄する。すべての洗浄物を個別の容器に集め、同位体毎に標識効力を測定する。
9)二重標識微小球を集め、5mlの滅菌pH7.4緩衝溶液を入れたバイアルに再懸濁する。規定した時間間隔で両方の放射性同位体の溶出試験を実施する。
第一部)2倍の比活性を備えたAc-225標識微小球の作製
用具および化学品:
1)225Ac(0.01M HCl中200μCi、凡その体積=60μl)
2)陽イオン交換樹脂(直径約30ミクロン)、0.01M HCl溶液
3)ガラス器具、磁気撹拌器、実験器具、注射用滅菌水(SWFI)、滅菌pH7.4緩衝溶液(好ましくは無リン酸)
4)放射線防護のために適切に装備された実験室
5)線量較正器およびGeガンマ線分光システム
手順:
1)0.5グラムの陽イオン交換樹脂(直径約30ミクロン)を適切な容器中の0.01M HCl溶液に加え(はじめに5mLそして必要であれば1mLずつ加える)、適度なスラリー懸濁液を形成するために磁気撹拌子を用いて30分間撹拌して確実に樹脂を完全に湿らせる。

4)これらの標識微小球を次いで適切なろ過機構(すなわち22ミクロンフィルタ)を用いてろ過し、SWFIで5ml×6回洗浄する。アルファ計数のために各洗浄物を別々に保管する。最後の洗浄液のpHを確認する(6.5〜7.0の間になると予想される)
5)10mlのバイアルにAc-225標識微小球を移し、それらを無菌pH7.4緩衝溶液(3mL)に再懸濁して懸濁液を作る。適切な方法を用いて微小球の放射活性を測定し標識量を決定する。バイアルにバイアル番号1Acとラベルを付ける。
用具および化学品:
1)89Zr(pH8緩衝液中20mCi)
2)陽イオン交換樹脂(直径約30ミクロン)、pH8緩衝溶液(好ましくは無リン酸)、滅菌pH7.4緩衝溶液
3)ガラス器具、磁気撹拌器、実験器具、注射用滅菌水(SWFI)
4)放射線防護のために適切に装備された実験室
5)線量較正器およびGeガンマ線分光システム
手順:
1)0.5グラムの陽イオン交換樹脂(直径約30ミクロン)を適切な容器中のpH8緩衝液に加え(はじめに5mLそして必要であれば1mLずつ加える)、適度なスラリー懸濁液を形成するために磁気撹拌子を用いて30分間撹拌して確実に樹脂を完全に湿らせる。
2)pH8緩衝液中のZr-89溶液(20mCi)を懸濁液に加え、20分間撹拌を継続する。
3)Zr-89標識効率は、この手順では約70%になると予想される。
4)これらの標識微小球を次いで適切なろ過機構(すなわち22ミクロンフィルタ)を用いてろ過し、SWFIで5ml×6回洗浄する。すべての洗浄物を個別の容器に集め各同位体毎に標識効力を測定する。
5)二重標識微小球を集め5mlの滅菌pH7.4緩衝溶液を入れた20mlバイアルに再懸濁する。バイアルにバイアル番号2とラベルを付ける。バイアルに蓋をする。
適切なシリンジ機構を用いて、バイアル番号1Acの内容物をすべて取り出しそれらをバイアル番号2に移す。バイアル番号1Acを1mlの滅菌pH7.4緩衝溶液で2回すすいで確実にすべてのAc-225標識微小球をバイアル番号2に移す。

- ポリマーマトリクスへのイオンのアクセシビリティを有するイオン交換樹脂ポリマーを含む、樹脂微小球と、
崩壊するにつれて主にアルファ粒子を放出する、該樹脂微小球に結合した殺腫瘍効果のための少なくとも1つの同位体と
から形成された放射性微小球を含む、生物組織の治療における放射線塞栓術での使用のためのデバイスであって、
該放射性微小球は、該少なくとも1つの同位体の崩壊の娘放射性核種が該放射性微小球によって捕捉されることを特徴とする、該デバイス。 - 少なくとも1つの同位体が、アクチニウム-225(225Ac)である、請求項1記載のデバイス。
- 樹脂が、多官能性である、請求項1記載のデバイス。
- 樹脂が、陽イオン結合のための少なくとも3つの異なる種類の官能基を有する、請求項3記載のデバイス。
- 樹脂に結合した少なくとも3つの官能基が、カルボン酸基、ジホスホン酸基、およびスルホン酸基を含む、請求項4記載のデバイス。
- 処置後の線量測定のために用いられる、放射性微小球に付着した第二同位体をさらに含む、請求項1記載のデバイス。
- 第二同位体が、PET線量測定のための陽電子放出体である、請求項6記載のデバイス。
- 第二同位体が、ジルコニウム-89(89Zr)である、請求項6または7記載のデバイス。
- 放射線塞栓術後の腫瘍細胞および正常な肝細胞の両方に吸収された放射線量の量が、PET線量測定走査の開始5分以内にデバイスから決定され得る、請求項6記載のデバイス。
- 各放射線治療が、総数3700万前後の放射性微小球を含む、請求項6記載のデバイス。
