本発明の実施形態は、肩の人工関節に概ね関し、より具体的に反転型人工肩関節に関する。

大きい(例えばより大きい曲率半径を有する)上腕骨頭12’を有するインプラント10’’を示す。異なるサイズのインプラント10、10’及び10’’は、外科医に、関節内における柔軟な組織間張力にインプラントを適合させることができ、各骨頭12、12’、12’’は、異なる厚み/曲率半径を提供する。より厚い骨頭12’’は、関節の軟部組織においてより大きい張力をかけ、一方、より薄い骨頭12は、関節の軟部組織においてより小さい張力をかける。インプラント12の底面120は、ステム4の正面41と同一平面にあるように構成される。ステム4に上腕骨頭を組み合わせることは、周知のモールステーパ部11を形成する上腕骨等における雄凸部とステム内の雌凹部とを介して達成され得る。
Claims (18)
- 内部側壁を含む近位穴を有する上腕骨構成要素と、
遠位端及び近位端を有するインサートと、
前記近位穴の前記内部側壁から内側に突出するフィンとを備え、
前記遠位端は、前記上腕骨構成要素の前記近位穴に係合するように構成され、インサート軸の周囲に形成された外部側壁を有し、該外部側壁は前記上腕骨構成要素の前記内部側壁に係合するように構成されており、
前記近位端は、窪んだ関節面を含み、
前記フィンは、前記外部側壁の外径よりも小さい内径に突出し、
前記フィンが前記上腕骨構成要素に対する前記インサート軸の周囲の回転に対して前記インサートを守るように、前記フィンは、前記上腕骨構成要素の前記近位穴に配置されていることを特徴とするモジュール式反転型人工肩関節のためのシステム。 - 前記フィンは、前記インサートが前記上腕骨構成要素に挿入されるときに、前記フィンは、前記インサート軸の周囲の前記インサートの回転位置にかかわらず、前記インサート軸の周囲の回転に対して前記インサートを守るように構成されていることを特徴とする請求項1に記載のシステム。
- 前記外部側壁は、少なくとも部分的に円筒状であり、前記インサート軸の周囲に形成された外径を有することを特徴とする請求項1又は2に記載のシステム。
- 前記フィンは、前記インサートが前記上腕骨構成要素に挿入されるときに、前記インサートをカットすることにより前記インサートを変形させることを特徴とする請求項1〜3のいずれか1項に記載のシステム。
- 前記フィンは、前記インサートが前記上腕骨構成要素に挿入されるときに、前記インサートを非弾性的変形により前記インサートを変形させることを特徴とする請求項1〜3のいずれか1項に記載のシステム。
- 前記フィンは、前記インサート軸の周囲において回転対称に前記内部側壁に配置された少なくとも3つのフィンであることを特徴とする請求項1〜5のいずれか1項に記載のシステム。
- 前記少なくとも3つのフィンは、前記インサート軸の周囲において回転対称に前記内部側壁に配置された少なくとも6つのフィンであることを特徴とする請求項6に記載のシステム。
- 前記フィンを形成する第1の材料は、前記インサートの遠位端を形成する第2の材料よりも硬いことを特徴とする請求項1〜7のいずれか1項に記載のシステム。
- 前記フィンは、前記近位穴の底部から内側に突出していることを特徴とする請求項1〜8のいずれか1項に記載のシステム。
- 前記インサートは、前記遠位端の周囲に形成された第1のチャンネルを含み、
前記インサートは、前記第1のチャンネル内に少なくとも部分的に配設されたスナップフィットリングをさらに含み、
前記上腕骨構成要素は、前記近位端の周囲に形成された第2のチャンネルを含み、
前記スナップフィットリングは、前記上腕骨構成要素から分離することに対して前記インサートを守るために、前記インサートが前記上腕骨構成要素に挿入されるときに前記第1のチャンネル及び第2のチャンネルの両方に配置されるように構成されていることを特徴とする請求項1〜9のいずれか1項に記載のシステム。 - 前記スナップフィットリングは、前記スナップフィットリングの内径が第1のチャンネル内にあり、前記スナップフィットリングの外径が前記第1のチャンネルの外側にある静止形態と、前記スナップフィットリングの外径が前記第1のチャンネル内に完全にある圧縮形態と、前記外径が前記圧縮形態における前記スナップフィットリングの外径よりも小さいが、前記第1のチャンネル内に完全には配置されていない部分圧縮形態とを有し、
前記スナップフィットリングは、前記上腕骨構成要素へのインサートの挿入の前に前記静止形態になり、前記インサートが前記上腕骨構成要素に最初に挿入されると圧縮形態になり、前記スナップフィットリングが前記第1のチャンネル及び前記第2のチャンネルの両方に配置されるときに部分圧縮形態になるように構成されていることを特徴とする請求項10に記載のシステム。 - 前記スナップフィットリングは、前記インサートが前記上腕骨構成要素に最初に挿入されるときに、前記第1のチャンネル内に完全に受け入れられるために径方向に圧縮するように構成されていることを特徴とする請求項10又は11に記載のシステム。
- 前記スナップフィットリングは、前記インサートが前記上腕骨構成要素に最初に挿入されるときに、前記スナップフィットリングの径方向の圧縮を促進する切れ目を含むことを特徴とする請求項10〜12のいずれか1項に記載のシステム。
- 前記スナップフィットリングは、前記上腕骨構成要素からの分離に対して前記インサートを可逆的に守ることを特徴とする請求項13に記載のシステム。
- 前記上腕骨構成要素は、前記上腕骨構成要素の外面から前記近位穴の前記第2のチャンネル内にまで形成された1つ又はそれ以上の穴を含み、
前記1つ又はそれ以上の穴は、前記上腕骨構成要素からインサートの解放を可能とするように前記第1のチャンネル内に前記スナップフィットリングを圧縮するための手段の挿入を可能とすることを特徴とする請求項14に記載のシステム。 - 前記スナップフィットリングは、径方向に圧縮するように構成され、
前記スナップフィットリングは、前記インサートが前記上腕骨構成要素に挿入されるときに、前記スナップフィットリングの圧縮を促進するために面取りされた遠位縁部を含むことを特徴とする請求項10〜15のいずれか1項に記載のシステム。 - 前記上腕骨構成要素は、前記スナップフィットリングの面取りされた遠位縁部と相互作用するように構成されて且つ面取りされた近位縁部を含むことを特徴とする請求項16に記載のシステム。
- 前記インサートが前記上腕骨構成要素に挿入されるときに前記フィンが前記インサートを変形させるように、前記フィンは、前記上腕骨構成要素の前記近位穴に配置されていることを特徴とする請求項1〜17に記載のシステム。
