Description
本願は、2012年10月29日に出願された米国仮特許出願第61/719835号の優先権を主張し、その全体は、全ての目的のために本明細書に参照として組み込まれる。
本発明の実施形態は、肩の人工関節に概ね関し、より具体的に反転型人工肩関節に関する。
Claims (37)
- 近位テーパ部及び中心主軸を有するステムと、
前記ステムの前記近位テーパ部と嵌合するように構成された遠位テーパ部を有するトレイと、
遠位端及び近位端を有するインサートとを備え、
前記近位テーパ部は、テーパ軸に沿って前記ステム内に延び、
前記トレイは、内部側壁を含む近位穴を有し、
前記遠位端は、前記トレイの前記内部側壁に係合するように構成された外部側壁を有し、
前記外部側壁は、少なくとも部分的に円筒状であり、インサート軸の周囲に形成され、
前記近位端は、回転中心の少なくとも部分的に周囲に形成された関節面を有し、
前記回転中心は、前記インサート軸からオフセットされており、
前記インサートは、前記インサート軸の周囲の前記インサートのいずれの回転位置においても前記トレイの前記近位穴と係合可能であり、
前記ステムは、該ステムの近位面に、深さ方向に向かうに従って互いに向かうように傾斜された2つの溝を含むインサータインターフェイスを含み、該インサータインターフェイスは上腕骨内に前記ステムを挿入するためのインサータに連結するように構成されていることを特徴とするモジュール式反転型人工肩関節。 - 前記回転中心は、前記インサートの最も薄い部分に向かう方向に、前記インサート軸からオフセットされていることを特徴とする請求項1に記載のモジュール式反転型人工肩関節。
- 前記回転中心は、前記インサート軸から5mm〜8mmオフセットされていることを特徴とする請求項1又は2に記載のモジュール式反転型人工肩関節。
- 前記回転中心は、前記トレイが前記ステムと嵌合して且つ前記インサートが前記トレイ
と係合する場合に、前記中心主軸から22mm〜28mmオフセットされていることを特徴とする請求項1〜3のいずれか1項に記載のモジュール式反転型人工肩関節。 - 前記インサートは、最終インプラント部傾斜角が110°〜155°の範囲となるようにステム傾斜角に従って曲げられることを特徴とする請求項1〜4のいずれか1項に記載のモジュール式反転型人工肩関節。
- 前記トレイの厚みは、3mm〜4mmであることを特徴とする請求項1〜5のいずれか1項に記載のモジュール式反転型人工肩関節。
- 前記テーパ軸は、前記トレイの中心軸から1.5mm程度オフセットされていることを特徴とする請求項1〜6のいずれか1項に記載のモジュール式反転型人工肩関節。
- 前記テーパ軸は、前記トレイの中心軸から3.5mm程度オフセットされていることを特徴とする請求項1〜6のいずれか1項に記載のモジュール式反転型人工肩関節。
- 前記インサートは、17.5°の傾斜角を有することを特徴とする請求項1〜8のいずれか1項に記載のモジュール式反転型人工肩関節。
- 前記インサートは、12.5°の傾斜角を有することを特徴とする請求項1〜8のいずれか1項に記載のモジュール式反転型人工肩関節。
- 前記インサートは、7.5°の傾斜角を有することを特徴とする請求項1〜8のいずれか1項に記載のモジュール式反転型人工肩関節。
- 前記ステムは、上腕骨の切除面の上に前記インサータの把持のためのカラー又は他のメカニズムが無いように構成されていることを特徴とする請求項1〜11のいずれか1項に記載のモジュール式反転型人工肩関節。
- 前記ステムは、第1の近位テーパ部及び第1の中心主軸を有する第1のステムである、請求項1〜12のいずれか一項に記載のモジュール式反転型人工肩関節と、
第2の近位テーパ部及び第2の中心主軸を有する第2のステムと、を備え、
前記第1の近位テーパ部は、第1のテーパ軸の周囲の前記第1のステムに延び、
前記第1のステムの傾斜角は、前記第1の中心主軸と前記第1のテーパ軸との間に形成され、
前記第2の近位テーパ部は、第2のテーパ軸の周囲の前記第2のステムに延び、
前記第2のステムの傾斜角は、前記第2の中心主軸と前記第2のテーパ軸との間に形成され、
前記第1のステムの傾斜角は、第2のステムの傾斜角と異なり、
前記第1のステム、前記トレイ及び前記インサートの第1のインプラント可能な組合せは、前記第1の中心主軸と前記インサート軸との間に形成された第1の最終インプラント部傾斜角を有し、
前記第2のステム、前記トレイ及び前記インサートの第2のインプラント可能な組合せは、前記第2の中心主軸と前記インサート軸との間に形成された第2の最終インプラント部傾斜角を有し、前記第1の最終インプラント部傾斜角は、前記第2の最終インプラント部傾斜角と異なることを特徴とするモジュール式反転型人工肩関節のためのシステム。 - 前記近位穴から内側に突出するフィンをさらに備え、
前記インサートが前記トレイに挿入されるときに前記フィンが前記インサートを変形させ、前記フィンが前記トレイに対する前記インサート軸の周囲の回転に対して前記インサートを守るように、前記フィンは、前記トレイの前記近位穴に配置されていることを特徴とする請求項1に記載のモジュール式反転型人工肩関節。 - 前記フィンは、前記インサートが前記トレイに挿入されるときに、前記フィンは、前記
インサート軸の周囲の前記インサートの回転位置にかかわらず、前記インサート軸の周囲の回転に対して前記インサートを守るように構成されていることを特徴とする請求項14に記載のモジュール式反転型人工肩関節。 - 前記フィンは、前記トレイの前記内部側壁から内側に突出し、
前記フィンは、前記インサートの外部側壁の前記外径よりも小さい内径に突出することを特徴とする請求項14又は15に記載のモジュール式反転型人工肩関節。 - 前記フィンは、前記インサートが前記トレイに挿入されるときに、前記インサートをカットすることにより前記インサートを変形させることを特徴とする請求項14〜16のいずれか1項に記載のモジュール式反転型人工肩関節。
- 前記フィンは、前記インサートが前記トレイに挿入されるときに、前記インサートを非弾性的変形により前記インサートを変形させることを特徴とする請求項14〜16のいずれか1項に記載のモジュール式反転型人工肩関節。
- 前記フィンは、前記インサート軸の周囲において回転対称に前記内部側壁に配置された少なくとも3つのフィンであることを特徴とする請求項14〜18のいずれか1項に記載のモジュール式反転型人工肩関節。
- 前記少なくとも3つのフィンは、前記インサート軸の周囲において回転対称に前記内部側壁に配置された少なくとも6つのフィンであることを特徴とする請求項19に記載のモジュール式反転型人工肩関節。
- 前記フィンを形成する第1の材料は、前記インサートの遠位端を形成する第2の材料よりも硬いことを特徴とする請求項14〜20のいずれか1項に記載のモジュール式反転型人工肩関節。
- 前記遠位テーパ部は、テーパ軸の周囲に形成され、
前記遠位テーパ部が前記ステムの前記近位テーパ部に挿入されるときに、前記テーパ軸の周囲における前記トレイの偏心性回転を可能とするように、前記テーパ軸は、前記トレイの中心軸からオフセットされていることを特徴とする請求項14〜21のいずれか1項に記載のモジュール式反転型人工肩関節。 - 前記フィンは、前記近位穴の底部から内側に突出していることを特徴とする請求項14〜22のいずれか1項に記載のモジュール式反転型人工肩関節。
- 前記インサートは、前記遠位端の周囲に形成された第1のチャンネルを含み、
前記インサートは、前記第1のチャンネル内に少なくとも部分的に配設されたスナップフィットリングをさらに含み、
前記トレイは、前記近位端の周囲に形成された第2のチャンネルを含み、
前記スナップフィットリングは、前記トレイから分離することに対して前記インサートを守るために、前記インサートが前記トレイに挿入されるときに前記第1のチャンネル及び第2のチャンネルの両方に配置されるように構成されていることを特徴とする請求項14〜23のいずれか1項に記載のモジュール式反転型人工肩関節。 - 前記スナップフィットリングは、前記インサートが前記トレイに最初に挿入されるときに、前記第1のチャンネル内に完全に受け入れられるために径方向に圧縮するように構成されていることを特徴とする請求項24に記載のモジュール式反転型人工肩関節。
- 前記スナップフィットリングは、前記インサートが前記トレイに最初に挿入されるときに、前記スナップフィットリングの径方向の圧縮を促進する切れ目を含むことを特徴とする請求項24又は25に記載のモジュール式反転型人工肩関節。
- 前記スナップフィットリングは、前記トレイからの分離に対して前記インサートを守ることを特徴とする請求項26に記載のモジュール式反転型人工肩関節。
- 前記トレイは、前記トレイの外面から前記近位穴の前記第2のチャンネル内にまで形成された1つ又はそれ以上の穴を含み、
前記1つ又はそれ以上の穴は、前記トレイからインサートの解放を可能とするように前記第1のチャンネル内に前記スナップフィットリングを圧縮するための手段の挿入を可能とすることを特徴とする請求項27に記載のモジュール式反転型人工肩関節。 - 前記スナップフィットリングは、径方向に圧縮するように構成され、
前記スナップフィットリングは、前記インサートが前記トレイに挿入されるときに、前記スナップフィットリングの圧縮を促進するために面取りされた遠位縁部を含むことを特徴とする請求項24〜28のいずれか1項に記載のモジュール式反転型人工肩関節。 - 前記トレイは、前記スナップフィットリングの面取りされた遠位縁部と相互作用するように構成されて且つ面取りされた近位縁部を含むことを特徴とする請求項29に記載のモジュール式反転型人工肩関節。
- 前記トレイの前記近位穴内において円周方向に形成される第1のチャンネルと、
前記インサートの前記遠位端の周囲において形成される第2のチャンネルと、
前記第2のチャンネル内に少なくとも部分的に配置されたスナップフィットリングと、をさらに備え、
前記スナップフィットリングは、前記トレイからの分離に対して前記インサートを守るために、前記インサートが前記トレイに挿入されるときに、前記第1のチャンネル及び前記第2のチャンネルの両方に配置されるように構成されることを特徴とする請求項1に記載のモジュール式反転型人工肩関節。 - 前記スナップフィットリングは、前記インサートが前記トレイに挿入されるときに、前記スナップフィットリングの径方向の圧縮を促進するための切れ目を含むことを特徴とする請求項31に記載のモジュール式反転型人工肩関節。
- 前記スナップフィットリングは、前記トレイからの分離に対して前記インサートを守ることを特徴とする請求項31又は32に記載のモジュール式反転型人工肩関節。
- 前記トレイは、前記トレイの外面から前記近位穴の前記第1のチャンネル内までに形成された1つ又はそれ以上の穴を含み、
前記1つ又はそれ以上の穴は、前記トレイからの前記インサートの解放を可能とするために、前記第2のチャンネル内に前記スナップフィットリングを圧縮するための手段の挿入を可能とすることを特徴とする請求項31〜33のいずれか1項に記載のモジュール式反転型人工肩関節。 - 前記スナップフィットリングは、径方向に圧縮するように構成され、
前記スナップフィットリングは、前記インサートが前記トレイに挿入されるときに前記スナップフィットリングの圧縮を促進するために面取りされた遠位縁部を含むことを特徴とする請求項31〜34のいずれか1項に記載のモジュール式反転型人工肩関節。 - 前記トレイは、前記スナップフィットリングの前記面取りされた遠位縁部と相互作用するように構成されて且つ面取りされた近位縁部を含むことを特徴とする請求項35に記載のモジュール式反転型人工肩関節。
- 前記スナップフィットリングは、前記スナップフィットリングの内径が前記第2のチャンネル内にあり、且つ前記スナップフィットリングの外径が前記第2のチャンネルの外側にある静止形態と、前記スナップフィットリングの外径が前記第2のチャンネル内に完全に配置されている圧縮形態と、前記外径が前記静止形態における前記スナップフィットリングの外径よりも小さいが、前記第2のチャンネル内に完全には配置されていない部分圧縮形態とを有し、
前記スナップフィットリングは、前記トレイ内への前記インサートの挿入の前に前記静止形態にあり、前記インサートが前記トレイ内に挿入されると圧縮形態になり、前記スナップフィットリングが前記第1のチャンネル及び前記第2のチャンネルの両方に配置されるときに前記部分圧縮形態になるように構成されていることを特徴とする請求項31〜36のいずれか1項に記載のモジュール式反転型人工肩関節。
