Description
本出願は、2017年3月8日出願の米国仮特許出願第62/468,417号明細書、および2018年2月14日出願の米国特許出願番号第15/896,302号に対する優先権の利益を主張するものであり、この開示は参照によりその全体が本明細書に組み込まれる。
よって、前に開発されたデュアルルーメンガス密封アクセス装置(米国特許第7,854,724号、米国特許公開公報2014/0358070A1に開示されるものなど)に関連した不利益を克服しつつ、従来的なシングルルーメン弁密封型アクセス装置と比較して実質的な利益を維持する、ガス密封外科手術アクセス装置を提供できれば有益である。
本発明は、こうした新規のアクセス装置および内視鏡外科手術で使用するための装置用にフィルター付き管セットを提供するが、これは以下で詳細に記述されている。
図2〜4を参照すると、本発明のガス循環システム10は、デュアルルーメン部分22およびシングルルーメン部分24を含む、全体が参照番号20で示されるフィルター付きマルチルーメン管セットを含む。デュアルルーメン部分22は、患者の外科手術空洞に対するガス再循環を促進するための加圧ガスライン30および戻りガスライン32を有する。シングルルーメン部分24は、患者の外科手術空洞に送気用ガスを送達するための、また患者の外科手術空洞内の圧力を定期的に感知するための、ガス供給および感知ライン34を有する。
図1および図2を図5〜9と併せて参照し続けると、循環システム10は、内視鏡外科処置中に患者の外科手術空洞へのガス密封アクセスを提供するよう適合され構成されたガス密封シングルルーメンアクセスポート26を含む。これに関して、アクセスポート26は、例えば、同一出願人による米国特許第7,854,724号(その全体が参照により本明細書に組み込まれる)に開示されているデュアルルーメントロカール組立品と類似して機能する。しかしながら、アクセスポート26は、一つの中央ルーメンのみが存在するという点で、米国特許第7,854,724号に開示されているトロカール組立品とは有意に異なる。
アクセスポート26の近位ハウジング部分50は、経路として管セット20の加圧ガスライン30と連通するための入口経路56と、管セット20の戻りガスライン32と連通するための出口経路58とのみを有する。
図11〜24を参照すると、上述のシングルルーメンガス密封アクセスポート26は、外科手術器具がそれを通過するためのガス密封、安定した空洞圧力および外科手術空洞の煙排出を実施するように適合され、かつ構成され、空気の混入および緊急時の空洞圧力放出を許容するように構築されている一方で、本発明の実施形態は、必ずしも外科手術空洞への器具のアクセスを提供しなければならないわけではなく、むしろ器具の通路をもたないシングルルーメンガス密封トロカールとして構成されうることも想定されており、十分に本開示の範囲内である。
図25〜30を参照すると、全体が参照番号710によって示される本発明のガス循環システムの別の実施形態が図示されており、これはロボット支援の低侵襲外科処置で使用するように構成されている。より具体的には、ガス循環システム710は、Intuitive Surgical, Inc.によって製造・販売されるDa Vinci Xiタイプのロボットシステムと併せて使用するように適合される。
ここで図31〜34を参照すると、内視鏡外科処置における使用のために適合および構成された、全体が参照番号810で示される本発明のガス循環システムの別の実施形態が図示されている。システム810は、デュアルルーメン部分22、シングルルーメン部分24、およびマルチパスフィルターカートリッジ組立品36を持つマルチルーメン管セット20を含む。
図35〜38を参照すると、本発明は、患者の外科手術空洞内で内視鏡外科処置を実施するために、分離可能二部品弁密封型外科手術アクセスポートを改良する新規の方法も対象とする。図35に示すように、方法は、シングルルーメン管状体部分852に分離可能に係合された弁密封型近位ハウジング部分950を有する分離可能二部品外科手術アクセスポート926を取得する工程を含む。近位ハウジング部分950は、機械的ダックビル弁995および従来のルアータイプの取り付け具925を含む。
Claims (5)
- 患者の腹腔内で内視鏡外科処置を実施するための外科手術アクセスポートであって、
内部チャンバーを画定する近位ハウジング部分と、前記近位ハウジング部分から遠位に延び、中央ルーメンを画定する細長いシングルルーメン管状体部分とを有し、前記中央ルーメンを囲む環状ルーメンを有さず、前記近位ハウジング部分が、経路として管セットの加圧ガスラインと連通する入口経路と前記管セットの戻りガスラインと連通する出口経路とのみを画定するマニホールドコネクターを有し、
前記近位ハウジング部分が、前記入口経路からの加圧ガスを受けるため、かつ前記加圧ガスを用いて前記管状体部分の前記中央ルーメン内にガス密封ゾーンを生成し、前記患者の前記腹腔内での安定した圧力を維持し、前記中央ルーメンを通って前記患者の前記腹腔にガス密封された器具アクセスを提供するための環状ジェット組立品を前記内部チャンバー内に収容する、外科手術アクセスポート。 - 別のアクセスポートと併せて、前記患者の前記腹腔からの煙排出を実施するように適合され構成される、請求項1に記載の外科手術アクセスポート。
- 前記管状体部分の前記中央ルーメンへの空気の混入を許容するように適合され構成されている、請求項1に記載の外科手術アクセスポート。
- 前記入口経路と前記出口経路が前記マニホールドコネクター内に同心に配置されている、請求項1に記載の外科手術アクセスポート。
