がんの治療法。
a.少なくとも1用量の放射線療法を投与するステップと;
b.少なくとも1つのPD−1および/またはPD−L1拮抗薬を投与するステップと
を含んでなり、少なくとも1つのPD−1および/またはPD−L1拮抗薬は、放射線療法の投与と同日にまたは4日後までに投与される。

I.治療法
本方法は、少なくとも1用量(one dose)の放射線療法投与と、少なくとも1用量の少なくとも1つのPD−1および/またはPD−L1拮抗薬の投与を用いる、がん治療を包含する。例えば、患者においてがんを治療する方法は、少なくとも1用量の放射線療法を投与するステップと、少なくとも1つのPD−1および/またはPD−L1拮抗薬を投与するステップとを含んでもよく、少なくとも1つのPD−1および/またはPD−L1拮抗薬は、放射線療法の投与と同日にまたは4日後までに投与される。
一実施形態では、本方法で使用するための少なくとも1つのPD−1および/またはPD−L1拮抗薬は、抗PD−1および/もしくは抗PD−L1抗体またはその機能性部分である。
高線量イオン化照射としてもまた知られている放射線療法は、検討中の治療的アプローチの構成要素である。
本方法は、多様ながん型を治療するために使用されてもよい。一態様では、方法は、メラノーマ、結腸直腸がん、または乳がんを治療するのに使用されてもよい。一実施形態では、乳がんは、トリプルネガティブ乳がんである。
A)マウスおよび細胞株
BALB/cおよびC57Bl/6マウスは、Harlan、U.K.から入手された。全ての動物実験は、地方倫理委員会によって承認され、英国内務省免許の下で実施された。CT26マウス結腸がん細胞(ATCC)および4434細胞は、BRafV600E p16-/-マウス(Richard Marias,Cancer Research UK,Manchester Institute)から単離されて、DMEM中で維持され、4T1トリプルネガティブ乳がん(ATCC)は、10%FCS、1%L−グルタミン(Invitrogen,U.K.)が添加されたRPMI−1640中で維持された。全ての細胞株は慣例的にスクリーニングされ、マイコプラズマ属(Mycoplasma)混入の不在が確認された。
マウスには、5×105個のCT26、1×105個の4T1または5×106個の4434細胞のいずれかが皮下接種(s.c.)された。照射は、Pantak HF−320320 kV X線ユニット(Gulmay Medical,U.K.)を使用して、接種の7〜10日後に(腫瘍が少なくとも100mm3の時点で)実施された。装置は300kV、9.2mAで操作され、X線ビームに濾過装置が装着されて2.3mm Cu半価層の放射線の質を与えた。マウスはX線焦点から350mmの距離に配置されて、投与量速度は0.80Gy/分であった。αPD−1(クローンRMPI−14)、αPD−L1(クローン10F.9G2)(どちらもBiolegend)またはアイソタイプ対照mAb(それぞれIgG2aおよびIgG2b)投与は、(特に断りのない限り)分割RT周期の1日目に開始されて、3qwで最高3週間までのPBS中の100μl/10gの投与量中の10mg/kg用量で腹腔内投与(i.p.)された。細胞およびサイトカイン枯渇実験マウスには、αCD8 mAb;クローンYTS169(M.Glennie,Southampton Universityからの贈与)、αCD4 mAb;クローンGK1.5(Biolegend)、αAsialo−GM1(Wako Chemicals)またはαIFNγ;クローンXMG1.2(BioXcell)のいずれかが投与された。末梢血が処置中に採取され、細胞枯渇が確認された。腫瘍再チャレンジ実験では、長期生存(LTS)マウスに、以前の腫瘍移植の最低限100日後に、腫瘍細胞が対側性に移植された。追加的な対照マウスもまた移植を受けて、腫瘍増殖が確認された。実験群は、少なくとも5匹/群のマウスを含み、少なくとも2つの独立した実験を代表する。
生体外刺激では、50Gyで照射された1×106個の腫瘍細胞または1μmol/mlのH2−Ld拘束性ペプチドSPSYVYHQF(配列番号91)(AH1)/TPHPARIGL(配列番号92)(β−ガラクトシダーゼ)(Anaspec,U.K.)のどちらかの存在下で、LTSまたは対照マウスのどちらかからの3.5×106個の脾細胞が、10%FCS、100U/mlのペニシリン、100μg/mlのストレプトマイシン、1%L−グルタミン、50μMの2−MEおよび10IU/mlのヒト組換えIL−2が添加されたRPMI−1640中で、5日間にわたり培養された。実験群は3〜5匹のマウスを含み、2つの独立した実験を代表する。5日間の培養後に、3μg/mlのBrefeldin A(BD Pharmingen,U.K.)および100IU/mlのヒト組換え体IL−2(Chiron,NL)の存在下において、1:1の比率で16時間にわたり50Gy照射腫瘍細胞で細胞が再刺激された。FACS分析のために、細胞は洗浄されてラット抗−CD16/32(eBioscience,U.K.)と共に培養され、非特異的結合がブロックされて、次にFITCコンジュゲート抗CD8αmAb(eBioscience、U.K.)で染色された。次に細胞は、固定/透過処理されて、APCコンジュゲートmAb(eBioscience,U.K.)を使用して、IFNγの発現について染色された。
単細胞懸濁液を得るために、gentleMacs Dissociatorおよびマウス腫瘍分離キット(Miltenyi Biotec,U.K.)を使用して、腫瘍が処理された。分析のために、上述されるように非特異的結合がブロックされ、多重パラメータフローサイトメトリーによって、CD4、CD8(BD Biosciences,UK)、CD45、NKP46、PD−1およびPD−L1の発現が調べられた(特に断りのない限り全てeBioscience)。
腫瘍細胞は、上述されるようなフローサイトメトリーによるPD−L1発現の評価に先だって、20ng/mlのIFNγおよび/またはTNFαの存在下で24時間にわたって培養された。共培養アッセイでは、(それぞれPBSまたはホルボール12−ミリステート13−アセテートおよびイオノマイシン細胞刺激混合物、(eBioscience,UK)で処理された)休止または活性化脾細胞のどちらかが、腫瘍細胞と1:1の比率で共培養されて、PD−L1の腫瘍細胞発現が、上述されるように評価された。IFNγR1発現のサイレンシングは、ShRNAがある細胞のレンチウイルス形質導入によって達成された(細胞はまた、対照として非標的化ShRNAでも形質導入された)(Thermo Scientific,UK)。脾細胞サイトカイン産生(IFNγおよびTNFα)の測定値は、上述されるような細胞内フローサイトメトリーによって測定された。
放射線療法は、腫瘍細胞の免疫原性を調節することが示されているが、それは全身性の抗腫瘍免疫のみをもたらす、耐久性のある治療応答をめったに生じ得ない。本発明者らは、5回の分割量で約10Gyとして送達される低用量の局所性分割投与RTが、腫瘍細胞のPD−L1発現の増大をもたらし、発現の上昇は、RTの最終投与の1、3、および5日後に、時間を整合させた未処置(NT)マウスと比較して、明白であることを示す(図1AおよびB)。この腫瘍細胞PD−L1発現のRT媒介増大は、RTの最終投与の72時間後にピークに達し、それはRTの7日後に有意に低下するが(1および3日目における発現との比較;それぞれP<0.05およびP<0.01、マンホイットニー検定)、NTマウスと比較して上昇したままである(P<0.05、マンホイットニー検定)。
次に本発明者らは、組み合わせRTおよびαPD−L1 mAb療法に続いて観察された、長期腫瘍制御の基礎となる機序を調べた。最初にコロニー形成アッセイを使用して、αPD−1およびαPD−L1 mAbが、腫瘍細胞との直接相互作用を通じて、放射線増感剤として作用しないことを確認した(図9A〜C)。抗体枯渇を使用して、次に本発明者らは、RT/αPD−L1 mAb併用療法に続く、抗腫瘍活性の仲介におけるエフェクターT細胞およびNK細胞の役割を調査した。本発明者らのデータは、(RT周期の1日目に開始するαPD−L1 mAb療法による)5日間の分割RT周期開始の早くも7日後に、NTマウスと比較して、併用療法に続いて、マウスにおける腫瘍量低下が明白であることを実証する(それぞれ207.5±29.2mm2対409.4±86.88mm2;P=0.067、マン・ホイットニーU検定)(図2A)。しかし、体積を低下させるこの統計学的傾向は、CD8+T細胞またはNK細胞のどちらかの枯渇に続いて失われ、腫瘍体積には、NTコホート(それぞれP=0.52およびP=0.70、マン・ホイットニーU検定)からの有意差がなかったが、免疫細胞枯渇なしで併用療法を投与されたマウスよりも有意に大きかった(P<0.01;併用療法と対比したCD8枯渇、およびP<0.05;併用療法と対比したNK細胞枯渇、マン・ホイットニーU検定)。処置後11日目までに、併用療法は、NT対照と比較して腫瘍量を有意に減少させた(P<0.001、マン・ホイットニーU検定)。この時点におけるCD8+T細胞またはNK細胞のどちらかの枯渇は、併用療法(それぞれP<0.001およびP<0.05、マン・ホイットニーU検定)の有効性を低下させる一方で、CD8+T細胞およびNK細胞の相対的貢献度はより明白になり、CD8対NK細胞枯渇マウスで、腫瘍制御を有意に低下させる(P<0.05、マン・ホイットニーU検定)。本発明者らのデータはまた、CD4+T細胞の枯渇が、併用療法に続く局所腫瘍制御を改善したことも示す(それぞれ153.2±27.0mm2対72.7±17.3mm2;P<0.05、マン・ホイットニーU検定)。
次に本発明者らは、RTおよびαPD−L1 mAbによる処置に続いて、免疫記憶が生じたかどうかを調べた。本発明者らは、最初にRTおよびαPD−L1 mAbで処置されたLTSマウスが、対側性再チャレンジ(図3A)に続いて、腫瘍を完全に拒絶できることを示す。この記憶応答を定量化するために、100日を超える無病生存期間を有するLTSマウスから脾細胞が収穫されて、CT26腫瘍関連抗原に由来するペプチド(AH1:SPSYVYHQF(配列番号91))、対照ペプチド(β−ガラクトシダーゼ:TPHARIGL(配列番号93))、または照射CT26細胞との共培養に続いて、IFNγを産生するCD8+T細胞の能力が評価された(図3BおよびC)。本発明者らのデータは、AH1ペプチドとの共培養に続いて、LTSマウスに、未感作マウスよりも有意に高いIFNγ産生CD8+Tリンパ球の出現頻度が与えられたことを示す(それぞれ6.6%±0.8対2.3%±0.2;P<0.05、マンホイットニー検定)。同様の応答が、脾細胞のCT26細胞との共培養に続いて観察された。ペプチドおよび腫瘍細胞の同時培養の比較は、記憶CD8+T細胞の出現頻度が、LTSマウスでは腫瘍細胞との共培養に続いて、AH1ペプチドとの共培養の約3倍低かったことを明らかにし、T細胞活性化の腫瘍細胞媒介抑制を反映するかもしれない。総合するとこれらのデータは、マウスモデルにおいて、RTがPD−1/PD−L1系の遮断と組み合わされた場合に、それが長期生存者において保護的免疫記憶を生じ得ることを示す。
最初に本発明者らは、生体外における一連のRT投与による腫瘍細胞の治療が、PD−L1の発現にいかなる直接的影響も有しないことを確認した(図4A)。腫瘍微小環境内のどの細胞集団が、PD−L1の腫瘍細胞発現の調節に関与するのかを同定するために、分割RT周期が、CD8、CD4またはNK細胞枯渇抗体との併用でマウスに投与された。本発明者らのデータは、NK細胞でなくCD8+T細胞の枯渇が、腫瘍細胞上のPD−L1のRT媒介上方制御を完全に抑止することを示す(図4BおよびC)。興味深いことに、CD4+T細胞の枯渇は、腫瘍細胞上のPD−L1のRT媒介上方制御をさらに増強することが分かった(RTのみによる処置と比較して約2倍)。
腫瘍微小環境内のIFNγとPD−L1発現(16)の間の臨床相関、およびこの応答に対するTNFαの影響(22)を所与として、本発明者らは、本発明者らの細胞株内のPD−L1に対するこれらのサイトカインの影響を評価した。腫瘍細胞と組換えIFNγの共培養は、生体外でPD−L1の細胞表面発現に顕著な20倍の増大をもたらす(図5A)。さらに、組換えTNFαのみの添加が、腫瘍細胞のPD−L1発現に影響を及ぼさない一方で、IFNγおよびTNFαの双方の混合物は,IFNγのみによる発現と比較して、腫瘍細胞PD−L1発現をさらに増強し得る(約2倍)(図5A)。本発明者らは、ShRNAを使用したCT26腫瘍細胞上のIFN−γ−受容体1(IFNγR1)のサイレンシングが、組換えIFNγとの、またはIFNγおよびTNFαの双方との共培養に続いて、PD−L1の上方制御を完全に抑止することを示す(図5A)。
本発明者らは、確立されたCT26腫瘍を有するマウスに、分割RT周期の1日目(スケジュールA)、周期の5日目(スケジュールB)またはRT完了の7日後(スケジュールC)のいずれかに開始するαPD−L1 mAbの投与と共に、5回の分割量で約10Gyの分割RT周期が投与される、3つの異なる組み合わせスケジュールを評価した(図6A)。スケジュールAおよびBの間で全生存期間の有意差は見られず、LTSは、それぞれ60%および57であった(P>0.05ログランク;マンテル・コックス検定)(図6Bおよび図10A)。対照的に、検群全体にわたる同様の腫瘍量にもかかわらず、RTとそれに続く7日後のαPD−L1 mAbによる逐次処置(スケジュールC)は、RTのみと比較して、全生存期間を向上させる上で完全に無効であった(それぞれ30日間および35日間の生存期間中央値数;P>0.05ログランク;マンテル・コックス検定)(図10BおよびC)。
低用量の分割RTは、CD8+T細胞のIFNγ産生に続発して、腫瘍細胞上におけるPD−L1発現の上方制御をもたらす。メラノーマ、結腸直腸、および乳がんのマウスモデルにおいて、本発明者らは、αPD−L1 mAbとの併用を通じてRTの活性が向上され得て、長期生存マウスにおいて、腫瘍再発から保護できる免疫記憶の発生をもたらすことを示す。さらに、本発明者らのデータは、投与計画が予後に影響を及ぼしてもよく、同時のしかし逐次でない治療法が、局所腫瘍制御および生存期間を改善するのに効果的であることを示す。
患者Aは、結腸直腸がんを有する。1週目に、患者Aには、5回の分割量(fractopms)で、有効量の放射線療法が投与される。1週目に、患者Aには、1日目、3日目、および5日目に、MEDI4736の治療用量もまた投与される。患者Aは、このスケジュールを2、3、4、および5週目に反復する。

Claims (17)
- 少なくとも1つのPD−1および/またはPD−L1拮抗薬を含む医薬であって、前記少なくとも1つのPD−1および/またはPD−L1拮抗薬が分割放射線療法の分割投与と同日にまたは4日後までに投与される、患者におけるがんの治療に使用するための医薬であり、ここで、前記少なくとも1つのPD−1および/またはPD−L1拮抗薬が、少なくとも1つの抗PD−1抗体および/もしくは抗PD−L1抗体、または抗原もしくはエピトープに結合できるその機能性部分であり、分割放射線療法の総用量が約11Gy以下である、医薬。
- 分割放射線療法の総用量が約10Gy以下である、請求項1に記載の医薬。
- 前記分割放射線療法が、2〜7回の分割量を含む、請求項2に記載の医薬。
- 前記分割放射線療法が、5回の分割量を含む、請求項3に記載の医薬。
