Description
[付記項1]

Claims (22)
- 複数回発射クリップ装置であって、
遠位クリップ固定位置と外面と遠位シャフト端部と内部とを有している中空のシャフトであって、側方開口部を前記遠位クリップ固定位置の近傍に形成しており、前記側方開口部が、前記内部と前記外面の外部環境との間に連通している、前記シャフトと、
少なくとも1つの縫合糸を受容するように形成されている内部空洞をそれぞれ有している複数の縫合糸固定クリップであって、前記内部に重ねられており、第1のクリップを形成しており、且つ、長手方向軸線に沿って移動するように構成されている、複数の前記縫合糸固定クリップと、
前記シャフトの前記内部に配置されたクリップ移動組立体であって、長手方向軸線において第1の近位位置から前記遠位クリップ固定位置に前記第1のクリップを移動させることによって、前記第1のクリップを前記遠位クリップ固定位置に搬送するように構成されている、前記クリップ移動組立体と、
長手方向において前記シャフトに沿って移動可能とされるスネア組立体であって、
遠位管端部を具備する中空のスネア案内管を備えているスネア移動本体、
前記スネア移動本体に機能的に接続されているスネアであって、遠位スネア位置を有している前記スネア、及び
スネア進行経路を形成するように前記スネア案内管と位置合わせされたスネア案内経路の一部分を形成している内部本体であって、前記スネアが、前記スネア移動本体から前記スネア案内経路の前記内部本体を通じて前記スネア案内管に至る前記スネア案内経路の内部に配置されており、前記内部本体が、前記スネアによって少なくとも1つの前記縫合糸の少なくとも一部分を一時的に固定するために、前記遠位管端部から前記側方開口部、前記遠位クリップ固定位置、及び前記第1のクリップの前記内部空洞を通じて外側に、且つ、前記遠位シャフト端部から遠位に、所定距離に至るまで前記遠位スネア位置を延長させるように、前記スネア移動本体に対して移動可能に配置されている、前記内部本体、
を具備する前記スネア組立体と、
を備えていることを特徴とする複数回発射クリップ装置。 - 前記スネアが、近位端部を有しており、
前記スネア移動本体が、前記スネア案内経路の前記スネア移動本体を有しており、
前記スネア案内経路の前記スネア移動本体が、前記スネア進行経路を形成するように、前記スネア案内経路の前記内部本体及び前記スネア案内管と位置合わせされており、
前記スネア移動本体が、前記スネアの前記近位端部が固定されている基礎領域を備えており、
前記スネアが、前記基礎領域から前記スネア案内経路の前記スネア移動本体を通じて前記スネア案内管に至る経路の内部に配置されており、
前記内部本体が、前記スネア進行経路を短縮するように、すなわち前記遠位管端部から外方に前記遠位スネア位置を延長させるように、前記スネア移動本体に対して移動可能に配置されていることを特徴とする請求項1に記載の複数回発射クリップ装置。 - 前記スネア案内経路の前記内部本体がU字状の第1の曲げを有しており、前記スネア案内経路の前記スネア移動本体がU字状の第2の曲げを有していることを特徴とする請求項2に記載の複数回発射クリップ装置。
- 前記スネア案内管が、近位案内管開口部を備えており、
前記スネア案内経路の前記内部本体が、前記第1の曲げの近位端と位置合わせされている近位部分と、前記第1の曲げの遠位端と位置合わせされている遠位部分とを備えており、
前記スネア移動本体が、対応して前記スネア移動本体と共に移動するように前記内部本体に固定されている第1の管を備えており、前記第1の管の遠位端部分が、前記第1の管が、前記スネア案内経路の前記内部本体の内部に延在するように、且つ、前記内部本体が移動するに従って前記スネア案内経路の前記内部本体の近位部分から後退するように、前記スネア案内経路の前記内部本体の前記近位部分と位置合わせされており、前記第1の管が、前記スネアの近位端が固定されている近位スネア基部を具備する近位部分を有しており、
前記スネア案内経路の前記スネア移動本体が、前記第2の曲げの近位端と位置合わせされている近位部分を有しており、
第2の管の遠位端部分が、前記スネア案内経路の前記スネア移動本体の近位部分の内部に配置されており、前記第2の管の遠位端部分が、前記スネア案内経路の前記内部本体の遠位部分の内部に配置されており、前記第2の管が、延出可能とされ、前記内部本体の移動に伴って引き込み可能とされ、
前記スネアが、U字状の前記第1の曲げを通じて、前記第2の管を通じて、U字状の前記第2の曲げを通じて、前記近位案内管開口部を通じて、且つ、前記スネア案内管の少なくとも一部分を通じて、前記近位スネア基部から前記第1の管の外側に遠位に向かって延在していることを特徴とする請求項3に記載の複数回発射クリップ装置。 - 前記スネアが、近位端を有しており、
前記内部本体が、前記スネアの前記近位端が固定されている近位スネア基部を有していることを特徴とする請求項1に記載の複数回発射クリップ装置。 - 前記スネア移動本体の前記スネア案内経路の一部分が、Uターンの曲げを有していることを特徴とする請求項5に記載の複数回発射クリップ装置。
- 前記スネア移動本体の前記スネア案内経路の前記一部分が、近位部分と遠位部分とを備えており、
前記内部本体が、前記内部本体に固定されている近位部分と、前記スネアが固定されている前記近位スネア基部と、を具備する管を備えており、前記管が、前記スネア移動本体の前記スネア案内経路の内部に延在するように、且つ、前記内部本体が移動するに従って前記スネア移動本体の前記スネア案内経路の近位部分から後退するように、前記スネア移動本体の前記スネア案内経路の前記一部分の前記近位部分と位置合わせされており、
前記スネア案内管が、近位案内管開口部を備えており、前記近位案内管開口部が、前記スネア移動本体の前記スネア案内経路の内部に延在するように、且つ、前記スネア移動本体が移動するに従って前記スネア移動本体の前記スネア案内経路の遠位部分から後退するように、前記スネア移動本体の前記スネア案内経路の前記一部分の前記遠位部分と位置合わせされており、
前記スネアが、前記Uターンの曲げを通じて、前記近位案内管開口部を通じて、且つ、前記スネア案内管の少なくとも一部分を通じて、前記近位スネア基部から前記スネア案内管の外側に遠位に向かって延在していることを特徴とする請求項6に記載の複数回発射クリップ装置。 - シャフトが、前記側方開口部と前記遠位シャフト端部との間に遠位シャフト部分を備えており、
前記スネア組立体が、前記第1のクリップの内部空洞を通じて、且つ、前記遠位シャフト部分を通じて、前記遠位シャフト端部の遠位から、前記側方開口部を貫通する前記シャフトの側面の外方に近位に向かって、前記スネアに一時的に固定されている少なくとも1つの前記縫合糸の一部分を引っ張り出すように構成されていることを特徴とする請求項1に記載の複数回発射クリップ装置。 - 前記スネアが、近位端部を有しており、
前記スネア移動本体が、最近位位置と最遠位位置との間において滑動し、
前記内部本体が、前記スネアの前記近位端部が固定されている近位スネア基部を有しており、これにより、前記スネア移動本体が前記最遠位位置に移動するに従って、遠位スネア部分が、前記遠位シャフト部分を通じて、前記遠位クリップ固定位置を通じて、且つ、前記第1のクリップの前記内部空洞を通じて、前記遠位シャフト端部の遠位に延在することを特徴とする請求項8に記載の複数回発射クリップ装置。 - 前記スネア移動本体が、最近位位置と最遠位位置との間において滑動距離に沿って移動し、
スネア組立体が、前記最遠位位置に至る前記内部本体の移動に応答して、少なくとも前記遠位スネア位置を遠位に向かって前記滑動距離より長い距離で移動させるように構成されていることを特徴とする請求項1に記載の複数回発射クリップ装置。 - 前記スネア案内経路が、前記スネア移動本体が約1インチ移動することによって、前記スネアが約2.5インチ伸長するように構成されていることを特徴とする請求項1に記載の複数回発射クリップ装置。
- 前記スネアが、外部環境から前記側方開口部を通じて前記シャフトの前記内部を横方向に通過するように構成されており、前記第1のクリップの前記内部空洞を通過するように構成されており、且つ、前記シャフトの前記内部から遠位に向かって前記遠位クリップ固定位置を通過するように構成されており、
前記スネア組立体が、前記側方開口部を通じて前記シャフトの側面の外側に少なくとも1つの前記縫合糸の縫合糸部分を引っ張り出すように構成されており、これにより、前記縫合糸部分の少なくとも一部分が、利用者がアクセスするための前記側方開口部から飛び出すことを防止されていることを特徴とする請求項1に記載の複数回発射クリップ装置。 - 前記スネアに一時的に固定されている前記縫合糸部分が、外科用縫合糸の2つの自由端を備えており、
前記第1のクリップを通じて近位に向かって引っ張られる前記縫合糸部分が、一時に4つの長さの前記外科用縫合糸を前記第1のクリップを通じて引っ張る2つの前記自由端とされ、
利用者がアクセスするための側方開口部を通じて引っ張り出される前記縫合糸部分の少なくとも一部が、前記外科用縫合糸の2つの前記自由端とされることを特徴とする請求項12に記載の複数回発射クリップ装置。 - 前記内部空洞が、ルーメン又はスロットとされることを特徴とする請求項1に記載の複数回発射クリップ装置。
- クリップ移動組立体が、長手方向において前記第1の近位位置から前記遠位クリップ固定位置に前記第1のクリップを移動させ、前記第1のクリップを介さずに第2の近位位置と略同一であるか又は前記第1の近位位置とは異なる前記第2の近位位置に復帰させることによって、前記第1のクリップを前記遠位クリップ固定位置に搬送するように構成されていることを特徴とする請求項1に記載の複数回発射クリップ装置。
- 前記クリップ移動組立体が前記第1のクリップを介さずに前記第2の近位位置に戻る際に、又はその後に、前記クリップ移動組立体が、新しい第1のクリップを形成するために、2番目のクリップをクリップ装填位置に移動させることを特徴とする請求項15に記載の複数回発射クリップ装置。
- 前記第1のクリップが、近位側を有しており、
前記スネア案内管が、前記シャフトの外部から前記側方開口部を通じて前記内部に至るまで、且つ、前記第1のクリップの前記近位側に至るまで移動可能とされる遠位案内部分を有しており、
遠位スネア部分が、前記遠位案内部分が前記シャフトの外部に位置する場合に前記スネア案内管に対して固定されており、
前記遠位スネア部分が、前記遠位案内部分が前記内部に配置されたことに応答して、前記スネア案内管から延在するように構成されている請求項1に記載の複数回発射クリップ装置。 - 前記スネア移動本体が、前記シャフトを囲んでいるルーメンを形成していることを特徴とする請求項1に記載の複数回発射クリップ装置。
- 遠位スネア部分が、前記第1のクリップを通過するために、前記シャフトの前記内部から遠位に向かって前記遠位クリップ固定位置を通過するために、及び、少なくとも1つの前記縫合糸の少なくとも一部分をループの開口部の内部に一時的に固定するために、外部環境から前記シャフトの前記内部を通じて且つ前記側方開口部を通じて延在するように形成されている前記ループを有していることを特徴とする請求項1に記載の複数回発射クリップ装置。
- 前記スネア移動本体が、遠位側を有しており、
前記内部本体が、最近位位置すなわち作動位置と最遠位位置すなわち非作動位置とを有しており、
前記遠位管端部が、前記スネア移動本体が前記最遠位位置すなわち前記非作動位置に位置している場合に、前記スネア組立体の前記遠位側に隣り合っており、
前記スネアが、先端と、前記内部本体が前記最近位位置すなわち前記作動位置に位置している場合に前記スネア移動本体の前記遠位側のすぐ外側に前記先端を位置決めするように設定されている全長とを有していることを特徴とする請求項19に記載の複数回発射クリップ装置。 - スネア延出管が、スネア組立体が前記遠位シャフト端部に向かって移動するに従って、前記スネア延出管が前記シャフトの前記側方開口部に落下するように構成されていることを特徴とする請求項20に記載の複数回発射クリップ装置。
- 外面と、縫合糸固定クリップの重なりを受容するように構成されている内部と、少なくとも1つの前記縫合糸固定クリップを射出するように形成されている遠位端開口部と、を備えている中空のシャフトであって、前記シャフトが、前記内部と前記外面の外側の外部環境との間に側方開口部を形成しており、前記シャフトが、前記側方開口部と前記遠位端開口部との間に遠位シャフト部分を形成している、前記シャフトと、
前記シャフトに接触した状態で長手方向に移動可能とされるスネア組立体であって、
前記シャフトを囲んでいるルーメンを形成していると共に遠位側を備えているスネア移動本体、
前記スネア移動本体に機能的に接続されていると共に遠位スネア部分を有しているスネア、及び
内部本体、
を有している前記スネア組立体と、
を備えている複数回発射クリップ装置であって、
前記内部本体が、
前記スネアが前記スネア移動本体から前記内部本体を通じて且つ前記スネア移動本体の少なくとも一部分を通じて配置されているスネア進行経路を形成するように、スネア案内経路の一部分を形成しており、
前記内部本体が、滑動距離及び遠位端を形成する所定の範囲に沿って、前記スネア移動本体に対して相対的に移動可能に配置されており、
前記内部本体が前記遠位端に移動することに応答して、前記スネア進行経路を短縮し、これにより遠位スネア位置が、前記スネアによって少なくとも1つの縫合糸の少なくとも一部分を一時的に固定するために、前記所定の範囲より長い距離で、前記遠位側から前記側方開口部を通じて且つ前記遠位シャフト部分を通じて遠位に向かって延長されることを特徴とする複数回発射クリップ装置。
