45セットのヒトの肺を、灌流液としてSTEEN溶液を用いて体外で灌流した。肺を移植するか、または移植に適していないと考えられる肺を捨てるために、ほとんどの場合、3時間または4時間後に灌流を停止したが、最大7時間まで灌流を行った。灌流液を集め、SpDのその後の免疫学的分析のために凍結した。サンプルを通常、最初とその後1時間ごとに収集した。しかしながら、すべての場合に、サンプルをすべての時点で収集したわけではない。これは、サンプル収集が行われる臨床状況に起因するものであった。臓器の処理が主要な作業であり、サンプルの収集は、必然的に2番目の作業であった。
異なる分析を、以下の示唆されたカットオフ値を用いて一緒に分析した:
TF>120pg/ml
tPA>4000かつ<7400pg/ml
dsDNA>3000ng/ml
SpA>40000ng/ml
SpD>130ng/ml。
Claims (13)
- 摘出された臓器を摘出臓器灌流中に評価する方法であって、
評価後、前記臓器が由来する身体に前記臓器を移植しないものであり、
前記摘出された臓器を灌流液で灌流して得られた灌流液中のdsDNA、ミトコンドリアDNA、またはリボソームである細胞破壊マーカーを含む1つ以上のバイオマーカーの濃度を測定するステップ、
を含んでなる前記方法。 - 前記方法が、TF、tPA、SpA、またはSpDからなる群から選択される、さらなるバイオマーカーの前記灌流液中の濃度を測定するステップをさらに含む、請求項1に記載の方法。
- 前記摘出された臓器が、動物またはヒト由来であり、肺、腎臓、肝臓、心臓、膵臓、または腸である、請求項1または2に記載の方法。
- 前記臓器は体内で循環的に摘出されたか、または体外で循環的に摘出されたものである、請求項1に記載の方法。
- 前記1つ以上のバイオマーカーの前記灌流液中の前記濃度の測定を、所定の期間にわたって連続的に、または所定の間隔で繰り返し行う、請求項1〜4の何れか一項に記載の方法。
- 前記1つ以上のバイオマーカーの前記灌流液中の測定濃度を基準値と比較し、比較に基づいて前記臓器を移植用に選択するか否かを決定するためのパラメータを提供するステップをさらに含んでなる、請求項1〜5の何れか一項に記載の方法。
- 前記1つ以上のバイオマーカーの前記灌流液中の前記測定濃度を基準値と比較し、比較に基づいて前記臓器を治療するために前記灌流液に添加する成分を決定するためのパラメータを提供するステップをさらに含んでなる、請求項1〜6の何れか一項に記載の方法。
- 前記灌流液中の前記1つ以上のバイオマーカーの濃度を測定し続けることによって、前
記摘出された臓器に対する前記成分の治療効果をモニタリングするステップをさらに含んでなる、請求項7に記載の方法。 - TFの測定濃度が基準値を超える場合に、TF抑制分子を前記灌流液に添加する、請求項7に記載の方法。
- 前記TF抑制分子が、HMG−CoAレダクターゼインヒビター、シクロオキシゲナーゼ(COX)インヒビター、パクリタキセル、リゾホスファチジルコリン、インスリン、ニコチンアミド、一酸化窒素(NO)/または可溶性グアニル酸シクラーゼ活性化因子、ヒドロキシ尿素、ピルビン酸エチル、ジメチルスルホキシド(DMSO)、アンギオテンシン変換酵素(ACE)インヒビター、アディポネクチン、レチノイン酸、オールトランスレチノイン酸、ビタミンD3、PGJ2、PPARαアゴニスト活性化因子(WY14643およびエイコサテトラエン酸)、肝臓X受容体アゴニスト、ペントロキシフィリン(pentroxifylline)、フェノール/レスベラトロール誘導体、インドブフェン、アミオダロン、メトホルミン、高濃度細胞内cAMP、およびPI3K/Akt/PKBシグナリング、miR−19、ショートヘアピンRNA、ヘアピンリボザイム、またはアンチセンスODNからなる群から選択される、請求項9に記載の方法。
- 1つ以上の細胞破壊マーカーの測定濃度が基準値を超える場合、抗アポトーシス剤を前記灌流液に添加する、請求項7に記載の方法。
- tPAの測定濃度が基準値未満である場合、線維素溶解剤を前記灌流液に添加する、請求項7に記載の方法。
- 前記線維素溶解剤がストレプトキナーゼまたはtPAである、請求項12に記載の方法。
