Description
図1は、ハンドル組立体(20)と、シャフト組立体(30)と、エンドエフェクタ(40)と、を含む、例示的な外科用ステープル留め及び分断器具(10)を表す。エンドエフェクタ(40)及びシャフト組立体(30)の遠位部分は、外科処置を実行するために、トロカールカニューレを通して患者の手術部位まで、図1に表すような関節動作していない状態で挿入するようにサイズ設定されている。単なる例示として、患者の腹部内に、患者の2本の肋骨の間に、又はその他の部位に、かかるトロカールを挿入してもよい。一部の状況では、器具(10)は、トロカールなしで使用される。例えば、エンドエフェクタ(40)及びシャフト組立体(30)の遠位部分は、胸壁切開又は他の種類の切開を通して直接挿入されてもよい。本明細書では、「近位」及び「遠位」といった用語は、器具(10)のハンドル組立体(20)を握っている臨床医を基準として用いられることを理解されたい。したがって、エンドエフェクタ(40)は、より近位にあるハンドル組立体(20)に対して遠位にある。便宜上、また説明を明確にするため、本明細書では「垂直」及び「水平」といった空間的な用語が、図面に対して使用されている点も更に認識されるであろう。しかしながら、外科用器具は、多くの向き及び位置で使用されるものであり、これらの用語は、限定的及び絶対的なものであることを意図するものではない。
図1〜2に示すように、本実施例のハンドル組立体(20)は、ピストル型グリップ(22)と、閉鎖トリガー(24)と、発射トリガー(26)と、を備える。各トリガー(24、26)は、以下でより詳細に説明するように、ピストル型グリップ(22)の方に及びそれから離れる方に選択的に枢動可能である。ハンドル組立体(20)は、アンビル解放ボタン(25)と、発射ビーム逆転スイッチ(27)と、取外し可能な電池パック(28)と、を更に含む。これらの構成要素についても、以下でより詳細に説明する。勿論、ハンドル組立体(20)は、上記のもののうちのいずれかに加えて又はその代わりに、様々なその他の構成要素、機構、及び操作性を有してもよい。ハンドル組立体(20)についてのその他の好適な構成は、本明細書の教示を考慮することで当業者には明らかとなるであろう。更に、ハンドル組立体(20)は単に、器具(10)内に含まれることができる例示的な種類の本体組立体あり、器具(10)は、ハンドル組立体(20)に加えて又はその代わりに、器具(10)がロボット支援治療及び手技中に使用されることを可能にするように構成された本体組立体を含むがこれに限定されない任意のその他の好適な本体組立体を備えてもよいことを理解されたい。
やはり図1〜3に示すように、本実施例のエンドエフェクタ(40)は、下部つかみ具(50)と、枢動可能なアンビル(60)と、を含む。アンビル(60)は、下部つかみ具(50)の対応する湾曲したスロット(54)の中に配設された1対の一体的な外向きに延在するピン(66)を含む。ピン(66)及びスロット(54)を図5を示す。アンビル(60)は、開位置(図2及び4に示す)と閉位置(図1、3及び7A〜7Bに示す)との間で下部つかみ具(50)の方に及びそれから離れる方に枢動可能である。用語「枢動可能である」(及び基礎としての「枢動する」と同類の用語)の使用は、固定軸の周りでの枢動運動を必ずしも必要とするとして読まれるべきではない。例えば、本実施例では、アンビル(60)は、ピン(66)によって画定された軸の周りを枢動し、これらのピンは、アンビル(60)が下部つかみ具(50)の方に動くときに、下部つかみ具(50)の湾曲したスロット(54)に沿って摺動する。そのような変形例において、枢動軸は、スロット(54)によって画定された経路に沿って並進し、同時にアンビル(60)は、その軸の周りを枢動する。加えて又は代わりに、枢動軸は、最初に、アンビル(60)と共にスロット(54)に沿って摺動し、次に、枢動軸がスロット(54)に沿ってある距離だけ摺動した後に、枢動軸の周りを枢動してもよい。そのような摺動/並進枢動運動は、「枢動」、「枢動する」「枢動の」、「枢動可能な」、「枢動している」などの用語内に包含されることを理解されたい。勿論、一部の形態は、固定したままスロット又はチャネル内を移動しない軸を中心としたアンビル(60)の枢動運動を提供してもよい。
本実施例において、アンビル(60)は、閉鎖リング(36)をエンドエフェクタ(40)に対して遠位に前進させることによって、下部つかみ具(50)の方に駆動される。閉鎖リング(36)は、カム作用によってアンビル(60)と協働することにより、閉鎖リング(36)のエンドエフェクタ(40)に対する遠位の並進に応じて、アンビル(60)を下部つかみ具(50)の方に駆動する。同様に、閉鎖リング(36)は、アンビル(60)と協働することにより、閉鎖リング(36)のエンドエフェクタ(40)に対する近位の並進に応じて、アンビル(60)を下部つかみ具(50)から離れる方に開くことができる。単に例として、閉鎖リング(36)及びアンビル(60)は、その開示が参照により本明細書に組み込まれている、2013年2月28日に出願された「Jaw Closure Feature for End Effector of Surgical Instrument」と題する米国特許出願第13/780,120号の教示の少なくとも一部分に従って、及び/又は以下の教示の少なくとも一部に従って相互作用してもよい。エンドエフェクタ(40)に対する閉鎖リング(36)の長手方向並進を提供するために使用され得る例示的な機構については、以下で更に詳細に説明する。

エンドエフェクタの構成は、アンビルと下部つかみ具との相互作用に関して、精度及び安定性の望ましくない欠如をもたらし得ることを理解されたい。例えば、一部の実施形態において、特定のエンドエフェクタ構成要素の構成、及びそれらが相互作用する態様が、アンビルが完全に開いた位置にあるときに、エンドエフェクタの下部つかみ具に対するアンビルの好ましくない動き即ち「パタパタした動き」を生じさせることがある。したがって、一旦アンビルが完全に開いた位置に到達すると、少なくとも閉鎖ストロークが開始されるまで、アンビルが完全に開いた位置に留まり、それによって、好ましくないパタパタした動きを低減するように、エンドエフェクタを構成することが望ましい場合がある。弾力性部材の様々な実施形態を含む様々なエンドエフェクタが以下で説明されるが、その弾力性部材は、アンビルを完全に開いた位置に留めるよう促して好ましくないパタパタする動きを低減することによって、対応するエンドエフェクタの精度及び安定性を改善するように構成されてよい。

図45は、外科用ステープル留め及び分断器具(10)に容易に組み込まれ得る別の例示的なエンドエフェクタ(900)を表す。エンドエフェクタ(900)は、上記のエンドエフェクタ(40、240)に概ね類似している。その結果、エンドエフェクタ(900)の構成要素は、上記のエンドエフェクタ(40、240)のものに概ね類似し、概ね類似の機能を果たす。したがって、エンドエフェクタ(900)の先に論じた構成要素についての説明は、ここでは繰り返さない。エンドエフェクタ(900)は、アンビル(960)と、下部つかみ具(950)と、閉鎖リング(970)と、を含むことを理解されたい。アンビル(960)は、上記のアンビル(260)と同じである。下部つかみ具(950)は、上記の下部つかみ具(250)と同じである。上記の閉鎖リング(270)と同様に、本実施例の閉鎖リング(970)は、アンビル(960)を下部つかみ具(950)の方に閉じるために遠位に前進し、そして、アンビル(960)を下部つかみ具(950)から離れる方に開くために近位に後退するように動作可能である。エンドエフェクタ(900)は、シャフト組立体(30)と連結されてよく、上記のエンドエフェクタ(40、240)のように駆動され得る。

(1) 外科用ステープラであって、
(a)エンドエフェクタであって、前記エンドエフェクタは、
(i)下部つかみ具と、
(ii)開位置と閉位置との間で前記下部つかみ具に対して枢動可能であるアンビルと、
(iii)弾力性部材であって、前記弾力性部材は、前記アンビルが前記開位置にあるときに、前記アンビルを前記開位置に留まるよう偏向させるように構成されている、弾力性部材と、を備える、エンドエフェクタと、
(b)本体組立体であって、前記エンドエフェクタは、前記本体組立体に対して遠位に配置されている、本体組立体と、
を備える、外科用ステープラ。
(2) 前記エンドエフェクタは、前記アンビルと連結された閉鎖リングを更に含み、前記閉鎖リングは、前記下部つかみ具に対して並進可能であり、前記閉鎖リングは、前記閉鎖リングが前記下部つかみ具に対して近位に並進するときに境界面において前記アンビルと係合することにより前記アンビルを前記下部つかみ具に対して前記開位置の方に枢動させるように構成されている、実施態様1に記載の外科用ステープラ。
(3) 前記弾力性部材は、前記アンビルと前記閉鎖リングとの間の前記境界面に配置されている、実施態様2に記載の外科用ステープラ。
(4) 前記弾力性部材は、エラストマー材料を含む、実施態様3に記載の外科用ステープラ。
(5) 前記閉鎖リングは、タブを備え、前記弾力性部材は、前記閉鎖リングの前記タブに取り付けられたエラストマー材料の層を備える、実施態様2に記載の外科用ステープラ。
(7) 前記エラストマーインサートの前記開口部は、星形の外形を有する、実施態様6に記載の外科用ステープラ。
(8) 前記アンビルは、タブを備え、前記弾力性部材は、前記アンビルの前記タブに取り付けられたエラストマー材料の層を備える、実施態様1に記載の外科用ステープラ。
(9) 前記アンビルは、内面を備え、前記弾力性部材は、前記アンビルの前記内面と係合する、実施態様1に記載の外科用ステープラ。
(10) 前記弾力性部材は、ばねを備える、実施態様1に記載の外科用ステープラ。
(12) 前記ばねは、上側脚部を備え、前記ばねは、前記上側脚部が前記アンビルの内面と係合するように配置されている、実施態様10に記載の外科用ステープラ。
(13) 前記弾力性部材は、1対のばねを備える、実施態様1に記載の外科用ステープラ。
(14) 前記弾力性部材は、前記アンビルの内面と係合する弾力性腕部を備える、実施態様1に記載の外科用ステープラ。
(15) 前記エンドエフェクタは、トレーを備えるステープルカートリッジを更に備え、前記弾力性腕部は、前記トレーと一体的である、実施態様14に記載の外科用ステープラ。
(a)下部つかみ具と、
(b)開位置と閉位置との間で前記下部つかみ具に対して枢動可能であるアンビルと、
(c)前記アンビルと連結される閉鎖リングであって、前記閉鎖リングは、前記下部つかみ具に対して並進可能であり、前記閉鎖リングは、前記閉鎖リングが前記下部つかみ具に対して近位に並進するときに前記アンビルと係合することにより前記アンビルを前記下部つかみ具に対して前記開位置の方に枢動させるように構成されている、閉鎖リングと、
(d)弾力性部材であって、前記弾力性部材は、前記アンビルが前記開位置にあるときに、前記アンビルを前記開位置に留まるよう偏向させるように構成されており、前記弾力性部材は、前記アンビルが前記開位置にあるときに、前記弾力性部材の少なくとも一部分が前記アンビルと前記閉鎖リングとの間で圧縮されるように配置されている、弾力性部材と、
を備える、エンドエフェクタ。
(17) 前記アンビルは、タブを備え、前記閉鎖リングは、タブを備え、前記閉鎖リングの前記タブは、前記閉鎖リングが前記下部つかみ具に対して近位に並進するときに、前記アンビルの前記タブと係合し、前記アンビルが前記開位置にあるときに前記アンビルと前記閉鎖リングとの間で圧縮されている前記弾力性部材の前記少なくとも一部分は、前記アンビルが前記開位置にあるときに前記閉鎖リングの前記タブと前記アンビルの前記タブとの間で圧縮されるように配置され、構成されている、実施態様16に記載のエンドエフェクタ。
(18) 外科用器具と共に使用されるためのエンドエフェクタであって、前記エンドエフェクタは、
(a)下部つかみ具と、
(b)開位置と閉位置との間で前記下部つかみ具に対して枢動可能であるアンビルと、
(c)弾力性部材であって、前記弾力性部材は、前記アンビルを前記開位置の方に偏向させるように構成されており、前記弾力性部材は、前記アンビルが前記閉位置にあるときに、前記アンビルによって圧縮されるように配置され、構成されている、弾力性部材と、
を備える、エンドエフェクタ。
(19) 前記弾力性部材は、ばねを備える、実施態様18に記載のエンドエフェクタ。
(20) 前記弾力性部材は、前記アンビルの内面と係合する弾力性腕部を備える、実施態様18に記載のエンドエフェクタ。
Claims (4)
- 外科用ステープラであって、
(a)エンドエフェクタであって、前記エンドエフェクタは、
(i)下部つかみ具と、
(ii)開位置と閉位置との間で前記下部つかみ具に対して枢動可能であるアンビルと、
(iii)前記アンビルと連結された閉鎖リングであって、前記閉鎖リングは、前記下部つかみ具に対して並進可能である、閉鎖リングと、
(iv)第1の弾力性部材であって、前記第1の弾力性部材は、前記アンビルが前記開位置にあるときに、前記アンビルを前記開位置に留まるよう偏向させるように構成されている、第1の弾力性部材と、を備える、エンドエフェクタと、
(b)本体組立体であって、前記エンドエフェクタは、前記本体組立体に対して遠位に配置されている、本体組立体と、
を備え、
前記閉鎖リングは第1のタブを備え、前記アンビルは第2のタブを備え、前記第1の弾力性部材は、前記第1のタブまたは前記第2のタブに取り付けられたエラストマー材料の層を備え、前記閉鎖リングが前記下部つかみ具に対して近位に並進するときに前記第1のタブが前記第2のタブに前記第1の弾力性部材を介して係合することにより前記アンビルを前記下部つかみ具に対して前記開位置の方に枢動させるように構成されている、外科用ステープラ。 - 前記第1の弾力性部材は、開口部を画定するエラストマーインサートを備え、前記第2のタブの少なくとも一部分は、前記エラストマーインサートの前記開口部の少なくとも一部分を通って延在する、請求項1に記載の外科用ステープラ。
- 前記エラストマーインサートの前記開口部は、星形の外形を有する、請求項2に記載の外科用ステープラ。
- 前記アンビルは、内面を備え、
前記エンドエフェクタは、前記アンビルの前記内面と係合して前記アンビルを開いた位置の方に偏向させる力を前記アンビルに及ぼす第2の弾力性部材を更に備える、請求項1に記載の外科用ステープラ。
