ART感受性のF32-Tanzaniaのクローンを、用量増加型、125サイクルのアルテミシニンのレジメンで5年間培養することにより、ART耐性F32-ART5寄生虫株を選抜した18。F32-ART5とF32-TEM(アルテミシニン無しで培養した同胞種のクローン(sibling clone))の両方について、それぞれ460x及び500xの平均ヌクレオチドカバレッジで、全ゲノム配列を得た。F32-TEMと比較して、F32-ART5において欠失した遺伝子は同定されなかった。(i) 変動が大きい、マルチジーンファミリー由来の遺伝子(var、rifin、及びstevor)、(ii) 寄生虫株の平均カバレッジの25%未満のカバレッジを有する位置、(iii) F32-ART5中に混在していることが判明した一塩基多型(SNP)、獲得したART耐性変異は5年間の継続的な選抜の後はサンプル中に定着していると期待できると考えて、(iv) F32-ART5とART感受性の3D7寄生虫株とで共有されているSNP、及び(v) 同義的(synonymous)SNPを除いた後、F32-ART5及びF32-TEMのエクソームを比較した (図5)。
過去10年間にわたり、カンボジアの西部の州でART耐性の保有率は着実に増加しているが、当該国の他の地域ではそうではない2。K13プロペラ変異の時空間的な分布がART耐性の時空間的分布と相関するかどうかを試験するために、発明者らは、2001年〜2012年にマラリアに罹患したカンボジアの患者由来の保管寄生虫分離株のK13プロペラをシーケンシングした(表4)。6州からのデータを比較した(n=886):ART耐性が確認された西部のパイリン、バタンバン、及びポーサット1, 6, 8, 22、ART耐性が近年増加している南東部のクラチエ2、並びに、この期間内にART耐性のエビデンスが事実上無かった北部のプレアヴィヒア及び北東部のラタナキリ2である。この解析により、3個の高頻度(≧ 5%)アレル(C580Y、R539T、及びY493H)を含む、全部で17個の変異型アレルが明らかになる。西部の3州全てにおいて、野生型配列の頻度は、時間とともに有意に減少したが、プレアヴィヒアやラタナキリでは減少しなかった。パイリン及びバタンバンにおいて、2001年〜2002年から2011年〜2012年にかけて、C580Yアレルの頻度が有意に増加したが、これはこの集団がこれらの地域に急速に侵入し、ほとんど定着していることを示している(図3)。
K13プロペラ多型が臨床的ART耐性の分子マーカーであることを確認するために、発明者らは、最初に、2009年〜2010年に寄生虫クリアランス半減期を測定し(レンジ1.58-11.53時間)6、以前に寄生虫が全ゲノム配列データの系統解析に基づいてKH亜集団(KH1、KH2、KH3、KH4、又はKHA)に割り当てられた15、ポーサット及びラタナキリの163人の患者を同定した。混合型の遺伝子型(野生型及び1種以上の変異型K13プロペラアレル)を有する13人の患者を除外した。残りの150人の患者中、72人が野生型アレルを有する寄生虫を保有し、他の人はK13プロペラに単一の非同義的SNPのみを有する寄生虫を保有していた:C580Y (n=51)、R539T (n=6)、及びY493H (n=21) (表6)。野生型寄生虫を有する患者における寄生虫クリアランス半減期(中央値3.30時間、四分位範囲2.59-3.95)は、C580Y (7.19時間、6.47-8.31、P<10-6、マン・ホイットニーのU検定)、R539T (6.64時間、6.00-6.72、P<10-4)、又はY493H (6.28時間、5.37-7.14、P<10-6)の寄生虫を有する患者の半減期に比べて、有意に短い(図4A)。また、C580Y寄生虫を保有する患者における寄生虫クリアランス半減期は、Y493H寄生虫を有する患者の半減期に比べて、有意に長い(P=0.007、マン・ホイットニーのU検定)。これらのデータは、C580Y、R539T、及びY493Hが、ARTを用いて治療したマラリア患者においてクリアランスが遅延する(slow-clearing)寄生虫を特定することを示す。

本発明は、ART耐性プラスモジウム属原虫に感染した患者の治療に有効な新規マラリア薬剤をスクリーニングするための方法を包含する。一つの実施態様において、方法は、以下の工程を含む。
・本発明に従って、プラスモジウム属原虫に感染した患者由来の生体サンプル中のプラスモジウム属原虫を遺伝子型決定する工程、
・患者に感染しているプラスモジウム属原虫が変異型K13プロペラアレルを有している場合、プラスモジウム属原虫の寄生虫クリアランス半減期及び/又は生存率を測定する工程、
・患者に感染しているプラスモジウム属原虫が変異型K13プロペラアレルを有している場合、試験する新規薬剤を生体サンプルと接触させる工程、
・薬剤の投与後、RSA0-3hにおけるプラスモジウム属原虫の寄生虫クリアランス半減期及び/又は生存率を測定する工程、
・薬剤の投与後にプラスモジウム属原虫の寄生虫クリアランス半減期及び/又は生存率が減少する場合、ART耐性プラスモジウム属原虫に感染した患者の治療に有効なマラリア薬剤として、その薬剤を選択する工程。
ART感受性F32-Tanzaniaクローンを、アルテミシニンの用量増加型レジメンで5年間培養することにより、ART耐性F32-ART5寄生虫株を選抜した。F32-Tanzaniaをアルテミシニン曝露無しで並行して培養することによりF32-TEM株を得た。参照DNAは、熱帯熱マラリア原虫株3D7、89F5 Palo Alto Uganda、及びK1992から抽出した。リングステージ生存アッセイ(RSA0-3h)を記述された13とおりに行った。Illumina社のペアリードシーケンシング技術を用いて、全ゲノムシーケンシングを、F32-Tanzania、F32-TEM、F32-ART5(4時点で)、3株の参照株(3D7、89F5、及びK1992)、及び21株のカンボジア寄生虫分離株について行った。一連の1091株の臨床的熱帯熱マラリア原虫分離株を、2001年〜2012年のACT有効性試験に参加した患者から採集した。K13プロペラを、ネステッドPCRを用いて増幅した。PCR産物の二本鎖配列シーケンシングは、韓国のMacrogen社が行った。配列を、MEGA 5ソフトウェアバージョン5.10を用いて解析し、特異的なSNPの組み合わせを同定した。データを、マイクロソフト社のエクセル及びMedCalcバージョン12(マリアケルケ、ベルギー)を用いて解析した。P値が0.05未満の場合、差は統計的に優位であるとみなした。寄生虫分離株コレクションの倫理的クリアランスは、カンボジア国立保健研究倫理委員会(Cambodian National Ethics Committee for Health Research)、ナバル医療研究センター治験審査委員会(the Institutional Review Board of the Naval Medical Research Center)、WHO西太平洋地域事務局技術審査グループ(the Technical Review Group of the WHO Regional Office for the Western Pacific)、及び国立アレルギー・感染症研究所治験審査委員会(the Institutional Review Board of the National Institute of Allergy and Infectious Diseases)より得た。
参照DNAを、実験室順応させた熱帯熱マラリア原虫株3D7(MR4、マナサス、バージニア州)、89F5 Palo Alto Uganda (Palo Alto株からのクローンであり、1978年のウガンダに由来し、コモンリスザル(Saimiri sciureus)の実験的宿主においてアーテメター(artemether)治療への高感受性を呈する[O. Mercereau-Puijalon、H. Contamin、及びJ.C. Barale、未発表データ])、及びK1992、すなわち1992年にパイリンにおいて、この地域におけるARTの大規模な展開の前に採集された分離株(フランス国立マラリアレファレンスセンター(French National Reference Center of Malaria)より提供)より抽出した。寄生虫のDNAは、凍結血液分注物(200 μl)より、Mini blood kit (Qiagen社、バレンシア、カリフォルニア州)を用いて、メーカーの取扱説明書に従って抽出した。
カンボジア由来の臨床的熱帯熱マラリア原虫分離株50株(2010年及び2011年に採集)を、Witkowskiら45により記述されたとおりにインビトロ培養に順応させた。これらの地理的起源を、表8に示す。これらの患者分離株について寄生虫クリアランス速度は決定されなかったが、これはこれらがそのような測定を含まない現地試験中に採集されたためである。Mini blood kit (Qiagen社)を用いて、寄生虫DNAを凍結血液分注物(200 μl)より抽出した。
リングステージ生存アッセイ(RSA0-3h)を、Witkowskiら13によって記述されたとおりに、高度に同期した寄生虫培養を用いて実行した。簡潔に言えば、0〜3時間の侵入後のリングステージの寄生虫を700nM DHA [ジヒドロアルテミシニン、WWARN (www.wwarn.org/research/tools/qaqc)より入手]又はその溶媒であるDMSOに6時間曝露し、洗浄してその後さらに66時間、薬剤無しで培養した。ギムザ染色した薄層塗抹標本中、正常な形態で第二世代のリング又はトロホゾイトへと成長した、生存している寄生虫の比率を顕微鏡で数えることにより、生存率を評価した。
2001年から2012年までに、カンボジアの国立マラリア制御プログラム(Cambodia’s National Malaria Control Program)の定期的な抗マラリア薬の効果モニタリングの一環として行われたACTの治療効果試験に参加した患者から、及びNAMRU-2により行われた試験から、一連の臨床的熱帯熱マラリア原虫分離株941株を採集した(表4)。EDTA又はACD中に回収した静脈血サンプル(5 ml)を、採集の48時間以内に4℃でプノンペンのカンボジアパスツール研究所(Institut Pasteur du Cambodge)へ輸送し、その後−20℃でDNA抽出まで保存した。Mini blood kit (Qiagen社)を用いて、寄生虫DNAを凍結血液分注物(200 μl)より抽出した。
既報6, 13のとおり、2009年及び2010年、ポーサット州及びラタナキリ州において、合併症が無いか、又は重篤な熱帯熱マラリア原虫性マラリアに罹患し、初期寄生虫密度が≧10,000/μlである患者を登録した。ARTを用いて患者を治療し、寄生虫血が検出されなくなるまで、血液厚層塗抹より、寄生虫密度を6時間毎に測定した。これらの寄生虫計数から、WWARNのオンラインParasite Clearance Estimator (http://www.wwarn.org/toolkit/data-management/parasite-clearance-estimator)を用いて163人の患者における寄生虫クリアランス半減期を得た。本試験は、ClinicalTrials.govに登録されている(第NCT00341003号)。
llumina社のペアリードシーケンシング技術を用いて、全ゲノムシーケンシングを、F32-Tanzania、F32-TEM、F32-ART5系統(4時点)、参照株3株(3D7、89F5、及びK1992)、及びカンボジア由来の寄生虫分離株21株について行った。Illuminaライブラリーの調整及びシーケンシングは、供給者により開発された標準的なプロトコルに従った。簡潔に言えば、ゲノムDNAを噴霧によりせん断し、せん断した断片を末端修復及びリン酸化した。平滑末端にA付加し、シーケンシングアダプターを断片にライゲーションした。インサートを、Agencourt AMPure XP Beads (± 500 bp;Beckman Coulter Genomics社、ダンバース、マサチューセッツ州)を用いてサイズを揃え、10サイクルのPCRを用いて濃縮した後、ライブラリー定量化及びバリデーションを行った。ライブラリーのフローセルへのハイブリダイゼーション及びブリッジ増幅を行ってクラスターを生成し、100サイクルのペアエンドのリードをHiSeq 2000機器(Illumina社、サンディエゴ、カリフォルニア州)に回収した。シーケンシングが完了した後、Illumina Analysis Pipelineバージョン1.7を用いて画像解析、ベースコーリング、及び誤差推定を実行した。
選択した遺伝子のPCR増幅を、表1に列挙したプライマーを用いて行った。2 μlのDNAを、1 μMの各プライマー、0.2 mM dNTP (Solis Biodyne社、タルトゥ、エストニア)、3 mM MgCl2、及び2 U Taq DNAポリメラーゼ(Solis Biodyne社)と共に、以下のサイクルプログラムを用いて増幅した:94℃で5分間、その後94℃で30秒間、60℃で90秒間、72℃で90秒間を40サイクル、及び最終伸長72℃で10分間。PCR産物を、2%アガロースゲル電気泳動及びエチジウムブロマイド染色により検出した。PCR産物の二本鎖シーケンシングを、フランスのBeckman Coulter Genomics社により行った。特定のSNPの組み合わせを同定するために、MEGA 5ソフトウェアバージョン5.10を用いて配列を解析した。

カンボジアのポーサット州及びラタナキリ州のマラリア患者から得られた臨床的寄生虫分離株のDNA抽出、Illumina(登録商標)シーケンシング、及びSNP遺伝子型決定は、既に記述されている15。これらの寄生虫の集団構造解析により、4つの亜集団が同定された:KH1、KH2、KH3、及びKH4。これら4つの群のいずれにも<80%の起源を有する寄生虫を、混合されている(KHA)とみなした。
0時点(元のF32-Tanzaniaクローン株)、196日目(0.2-μMの圧力サイクル#23)、385日目(1.8-μMの圧力サイクル#39)、618日目(9-μMの圧力サイクル#56)、及び2250日目(9-μMの圧力サイクル#120)に採取したサンプルを用いて、F32-ART5系統を全ゲノムシーケンシングにより探索した。F32-TEMのサンプルを2250日目に採取した。30回目、33回目、34回目、36回目、68回目、及び98回目の圧力サイクルの時点で採取した追加のサンプルを、PCRにより試験した。寄生虫培養由来のDNAを、High Pure PCR Template Preparation Kit (Roche Diagnostics社、マンハイム、ドイツ)を用いて、メーカーの取扱説明書に従って抽出した。
886個の保管血液サンプルから増幅したPCR産物をシーケンシングすることにより、K13プロペラを遺伝子型決定した。解析した各州由来の各年のサンプルの数を、図3に示す。フィッシャーの正確確率検定を用いて、各州における野生型K13プロペラ配列を含む分離株の頻度を経時的に比較した。この10年の間で、西部の州において野生型K13プロペラアレル頻度の有意な減少が観察された。パイリンでは、2001年〜2002年の30.0%(12/40)から2011年〜2012年の4.8%(4/84)、P=0.0002、フィッシャーの正確確率検定;バタンバンでは、2001年〜2002年の71.9%(46/64)から2011年〜2012年の7.0%(5/71)、P<10-6;ポーサットでは、2003年〜2004年の50.0%(5/10)から2011年〜2012年の10.5%(2/19)、P=0.03;及びクラチエでは、2001年〜2002年の93.3%(14/15)から2011年〜2012年の29.4%(5/17)、P=0.0003、と減少した。野生型アレル頻度の有意な減少は、プレアヴィヒア[2001年〜2002年の92.6%(25/27)から2011年〜2012年の84.2%(16/19)、P=0.63]やラタナキリ[2001年〜2002年の96.4%(54/56)から2011年〜2012年の94.3%(33/35)、P=0.63]では観察されなかった。C580Yの頻度は、パイリンでは、2001年〜2002年の45.0%(18/40)から2011年〜2012年の88.1%(74/84) (P<10-6)、バタンバンでは、2001年〜2002年の7.8%(5/64)から2011年〜2012年の87.3%(62/71) (P<10-6)に増加し、これはこの集団がこれらの州に急速に侵入し、ほとんど定着していることを示している。
PF3D7_1343700のkelchプロペラドメイン(「K13プロペラ」)の3次元構造モデルを、Modeller v9.11 (Fiser A、Sali A (2003). "Modeller: generation and refinement of homology-based protein structure models". Meth. Enzymol. 374: 461−91.)を用いて、空間的制限を充足するホモロジーモデリングにより得た。K13プロペラを構成する295アミノ酸は、ヒトKeap1のkelchプロペラドメイン[Protein Data Bank (PDB) 2FLU]、KLHL12 (PDB 2VPJ)、及びKLHL2 (PDB 2XN4)タンパク質と、それぞれ22%、25%、及び25%同一であり、これらをK13プロペラの3次元構造をモデリングするための鋳型として用いた。得られたモデルの信頼性を、2つの古典的な基準を用いて評価した。第一に、K13のkelchドメインと1つの鋳型との間の配列アライメントの有意性を、Built-Profileの定型操作を用いてModellerによって計算された期待値(E-value) = 0によって確認した。第二に、モデルはGA341モデルスコア = 1 (≧0.7のスコアが、高信頼性のモデルに相当)を達成した。K13の3次元モデルにおける変異の位置の特定を、PyMOL Molecular Graphics System、バージョン1.5.0.4 (Schrodinger社、ポートランド、オレゴン州)を用いて、作成した。
マイクロソフト社のエクセル及びMedCalcバージョン12(マリアケルケ、ベルギー)を用いてデータを解析した。定量データは中央値、四分位範囲(IQR)として表した。マン・ホイットニーのU検定(独立サンプル、両側)を用いて2つの群を比較し、クラスカル・ウォリス検定(H検定、両側)を用いて2より多い群を比較した。スピアマンのロー順位相関係数(及び相関係数について95%信頼区間)を用いて、野生型K13プロペラアレルの保有率と3日目の陽性率(アルテミシニンに基づく組み合わせ療法の3日目における顕微鏡で検出可能な寄生虫の持続として定義される)との間の関係の強さを測定した2。フィッシャーの正確確率検定を用いて、頻度データを比較し、比率についてはベータ分布に基づくクロッパー-ピアソン(Clopper-Pearson)の正確法を用いて95%信頼区間を決定した。P値が0.05未満の場合、差は統計的に優位であるとみなした。
実施例5に記述したカンボジアの患者由来の941株の臨床的熱帯熱マラリア原虫の最初の採集の後、熱帯熱マラリア原虫に感染した患者由来のさらなる血液サンプルを、ベナン、アンゴラ、及びカメルーン等の複数のアフリカの国々、及び南アメリカにおいて、実施例5と同様の実験的手法に従って採集した。合計で9523個の血液サンプルを採集した。
本実施例は、K13プロペラドメイン中の全ての変異を同時に検出するための方法に関する詳細を記述する(図13)。SNP検出を3つの工程で行う:

- G449A、N458Y、A481V、Y493H、R539T、I543T、P553L、R561H、V568G、P574L、C580Y、及びD584Vアレルをコードする変異型K-13プロペラ核酸を検出する、請求項8〜12のいずれか一項に記載のキット。
- 以下のSNPの1つとハイブリダイズする少なくとも1個のプローブを含む、請求項8〜13のいずれか一項に記載のキット。
- プラスモジウム属原虫を含む患者由来の生体サンプルにおいて野生型K-13プロペラ核酸又は変異型K-13プロペラ核酸の存在を検出するためのインビトロの方法であって、
a)場合により、プラスモジウム属原虫のDNAを含む生体サンプルからDNAを抽出する工程、
b)前記生体サンプル又はプラスモジウム属原虫のDNAを、100 bp〜300 bpの範囲の距離をおいてK-13プロペラ核酸と特異的にハイブリダイズする少なくとも一組のプライマーと接触させ、インターカレート染料の存在下でPCR反応を行う工程、
c)増幅産物を融解ステップに供する工程、
d)前記増幅産物の融解プロファイルを解析することにより変異型アレル又は野生型アレルの存在を判定する工程であって、変異型K-13プロペラ核酸の存在が、前記患者がアルテミシニン誘導体耐性のプラスモジウム属原虫に感染していることを示す、工程、及び
e)場合により、G449A、N458Y、A481V、Y493H、R539T、I543T、P553L、R561H、V568G、P574L、C580Y、及びD584Vアレルをコードする変異型K-13プロペラ核酸を検出する工程
を含む、方法。 - 検出されるK-13プロペラ核酸の変異が非同義的SNPであり、前記非同義的SNPが、以下:
- 変異型K-13プロペラ核酸又はタンパク質の存在が、アルテミシニン誘導体耐性のプラスモジウム属原虫に感染している前記患者に、通常のプロトコルよりも長期のアルテミシニン誘導体に基づく治療を施すこと及び/又は別の抗マラリア薬、好ましくはキニーネ、クロロキン、又はメフロキンを投与することが有効である可能性があることをさらに示す、請求項2に記載の方法。
- アルテミシニン誘導体耐性のプラスモジウム属原虫に感染していることが判明し、新規抗マラリア薬に基づく治療を施された患者の治療後の血液サンプルにおいて、請求項2に規定する工程を繰り返す工程をさらに含み、変異型K-13プロペラ核酸又はタンパク質が存在しないことが、前記患者がもはやアルテミシニン誘導体耐性のプラスモジウム属原虫に感染していないこと、及び前記治療がアルテミシニン誘導体耐性のプラスモジウム属原虫に対して有効であることを示す、請求項2に記載の方法。
- 前記プローブが蛍光ビーズに結合している、請求項14に記載のキット。
