Description
液体血液サンプルと比べたDBS採取の利点は多数である。DBSの採取は容易である。指またはかかとを刺すだけですみ、静脈穿刺の必要が回避される。瀉血のスキルが不要である。採取装置は最小である。サンプルカードは通常十分なサンプルの大きさを確保するためのスポットサイズ(直径)の表示を有している。通常約70μlのサンプル容量で十分である。サンプルを周囲温度で風乾する。DBSを保存のために冷凍する必要がない。DBSは密閉容器(例えば、タッパウェア(R)または密閉エンベロープ)を用いて保存または移送し得る。サンプルは簡単に移送され、周囲条件下で長期間(数週間から数ヶ月)安定である。よって、サンプルを外部の場所で採取し、集中検査施設に移送することができる。DS/DBSサンプルは低いバイオハザードしか与えないために、適切に包装されているサンプルは検査施設に郵送され得る(乾燥血液スポット標本の発送ガイドライン,CDC,www.cdc.gov/labstandards/pdf/nsqap/Bloodspot_Transportation_Guidelines.pdf及びその中に含まれている文献;Clinical Laboratory and Standards Institute. Blood collection on filter paper for newborn screening programs;Approved standard−Fifth edition.CLSI document LA4−A6.Wayne、PA: Clinical and Laboratory Standards Institute;2012)。検査施設において一度、サンプルは所望により自動化システムまたは手動手順を用いて抽出され得る。
以下の定義は本明細書に該当する。
標的配列を当業界で公知の技術を用いて増幅する。最適な技術はポリメラーゼ連鎖反応(PCR)である。PCR増幅は標準のPCR技術により製造業者の説明書に従って実施され得る。Abbott m2000システムは使用者からのインプットに基づいてサンプル調製及びPCR反応を自動化するデバイスを含む。
本発明は好ましくは自動化プログラマブルPCRデバイスを用いて実施され、これらのデバイスの幾つかは当業者に公知であり、本発明の発明概念から逸脱せずに特定システムで使用するために必要であり得る手順の変化を加えて本発明で使用するのに適している。他の場合には、本発明は手動で実施され得る。しかしながら、本発明を手動で実行すると、時間への投資が高まり、操作者のエラーのために精度が低下する可能性が生ずる。
下記されている方法は乾燥血液スポット(DBS)標本のHIV−1ウイルス量検査に適用される。下記されているこの方法はAbbott m2000sp及びm2000rt装置をAbbottリアルタイムHIV−1試薬と一緒に使用する。他のシステム及びデバイスは当業界で市販されており、当業者は本明細書の発明概念を逸脱することなく、本明細書の教示に基づいて他の市販されているシステム及びデバイスで使用するために以下に開示されている方法を修飾することができる。
アッセイを実施する前に、HIV−1 DBSウイルス量検査のためのアプリケーションファイル(すなわち、ソフトウェア)をAbbott m2000sp及びAbbott m2000rtシステムにインストールしなければならない。
DBSを以下のステップに従ってMunktell TFN(スウェーデン)ペーパーカード(または、当業者に公知の均等なペーパーカード)上に作成し得る:
・全血をMunktell TFNペーパーカード(または、均等物)上の0.5インチ(12mm)の円にスポットして、全円をカバーすることを確実にする。完全適用範囲を確実にするために各円につき少なくとも70μlの血液(約3から5滴;圧搾したり、指で絞ったりしない)を使用することが推奨される。全血を採血チューブ中に採取したならば、新たに採血した血液を2〜8℃(冷蔵温度)から15〜30℃(周囲温度)でスポットする前に最長24時間保持し得る。加えて、血液をピペットを用いてスポットする前に混合しなければならない。
・カードを周囲温度が風乾させる。
・移送または保存のために、各カードを乾燥剤パックを入れたバッグまたは他の密閉容器に詰める。カードは周囲温度で最長12週間保存し得る。或いは、カードは2〜8℃、または−10℃またはそれより低い温度で最長24週間保存可能である。
・所要または所望により、標本を上に挙げた推奨される保存温度及び時間に従って発送する。国内及び海外発送の場合、標本は臨床、診断または生物学的標本の移送に該当する適用可能な州、連邦及び国際規制に従って包装し、ラベルを貼らなければならならい。
1.この典型的なプロトコルはAbbottリアルタイムシステムを使用した。当業者は過度の実験をしなくてもこのプロトコルを他の類似のデバイス及びシステムに適応させることができる。各ランで全部で96個のサンプルを処理することができる。各ランにネガティブコントロール、低ポジティブコントロール及び高ポジティブコントロールを含め、従ってキャリブレーターを含んでいないときには1ランあたり最大93個のDBS標本を処理することができる。これらのステップは直接液体サンプルとして処理されなければならないAbbottコントロール及びキャリブレーターに適用しない。DBS標本をこれらのステップに従って処理する:
・1.3mlのDS/DBS溶出バッファー(溶出バッファーはおよそpH6.0で、およそ3.M GITC(グアニジニウムチオシアネート)、およそ5% Tween(R)20、およそ50mM KOAc(酢酸カリウム)を含む)を用いてAbbott移送チューブを作成する。Tween(R)はニュージャージ州ブリッジウォーターのICI Americas,Inc.の登録商標である。Tween(R)20はポリソルベート20の商品名である。ポリソルベート20の他のブランドも本発明の方法で機能する。[GITCは1.0〜5.5M、2.0〜4.5M、3.0〜4.0M及び約3.5Mで使用され得る;Tween 20は0〜20%、2%〜8%、4%〜6%及び約5%で使用され得る;酢酸カリウムは10〜500mM、20mM〜300mM、30mM〜200mM、40mM〜100mM及び約50mMで使用され得る;pHは5〜10、5.2〜8、5.6〜7、5.8〜6.5及び約6.5であり得る。]
・Munktell TFNペーパーカード(または、均等物)から各標本あたり1つの全DBSを分離する。各DBSの直径はおよそ1/2インチ(12mm)でなければならない。注:妥当な場合、切断表面がDBS標本と直接接触するのを避けなけれはならない。所要により標本間のDBSを切断するために使用する装置を優良試験所基準に従って清潔にする。DS/DBS溶出バッファーを収容しているAbbott移送チューブにDBSを入れる。DBSがDS/DBS溶出バッファー中に完全に没していることを確かめる。注:このDBS移動ステップ中、有孔Munktell TFNペーパーカードをAbbott移送チューブの上に置き、DBSをカードから清潔なピペットチップを用いてチューブに直接押出してもよい。
・サンプルをAbbott m2000sp装置または他のロボットシステムに装入する(ステップ7)前に、断続的にゆっくり混合しながら室温で約20分間インキュベートするか、または55℃で30分間インキュベートする。
・一旦解凍したら、アッセイコントロール、IC及びキャリブレーターは使用前2から8℃で最長24時間保存し得る。
・使用前、各アッセイキャリブレーター及び各コントロールを2から3秒間3回ボルテックスする(すなわち、ボルテックスミキサーまたは均等物を用いて非常に激しく混合する)。ボルテックス後、液体がバイアルの底部に届くようにバイアルをベンチ上でタップすることにより各バイアルの内容物が底部にあることを確かめる。泡またはフォームが発生してないことを確かめる。泡が存在しているならば、バイアル毎に新しいチップを用いて泡を滅菌ピペットチップで除去する。
・インターナルコントロール(IC)を当業界で公知のように製造業者の説明書に従って作成する。
・一旦解凍したら、すぐに使用しない場合には増幅試薬は2から8℃で最長24時間保存し得る。
・当業界で公知のように、増幅試薬(PCR試薬)を製造業者の説明書に従って作成する。
・低及び高ポジティブコントロール、ネガティブコントロール、妥当な場合にはキャリブレーター、及びDBS標本をAbbott m2000spサンプルラック上のAbbott移送チューブ中に入れる。注:Abbott m2000spサンプルラックがこのHIV−1 DBSウイルス量手順のために特に較正されていることを確認する。
・飛び散るのを避けるためにサンプルラックを注意深く装入する。使用する場合、チューブラベル上のバーコードをスキャンニングのために右に向けなければならない。チューブの底がラックの内底に達するように各チューブがサンプルラック中にしっかり置かれていることを確認する。
・装入したサンプルラックをAbbott m2000sp上に連続サンプルラック位置に、第1ラックは作業台の右に対して最も遠い位置に、追加のラックは第1ラックの左に漸次載せる。
・キャリブレーター(検量線がAbbott m2000rtに蓄積されていないならば、必要である)及びコントロールロット特性値をサンプル抽出:作業台セットアップ、キャリブレーター及びコントロールフィールドに入れる。ロット特性値は各AbbottリアルタイムHIV−1キャリブレーター及びコントロールキットカードにおいて特定されている。
・サンプル調製の終了から1時間以内にAbbott m2000spマスターミックス添加プロトコル(ステップ9)を開始しなければならない。
注:増幅試薬を取り扱う前に手袋を交換する。
・サンプル抽出が終了したら、ラン内で処理したサンプル数が48個以上のときにはAbbott m2000spはAbbott 96ウェルオプティカルリアクションプレート中に空ウェルを自動的に充填する。48個以下のサンプルを収容しているランに対してはプレート充填を実施しない。
標本またはコントロール中のウイルスHIV−1 RNAの濃度を蓄積検量線から計算する。Abbott m2000rt装置は自動的にAbbott m2000rtワークステーションに結果を報告する。アッセイ結果はコピー/ml、log[コピー/ml]、国際単位(IU)/mL、またはlog[IU/mL]で報告され得る。結果を解釈するために、以下の表1を参照されたい。
本実施例は、本発明の自動化DBSアッセイ方法を従来方法と比較して改善された効率及び感度で実施し得る設計要件を示す。
1.DBSをDBSカードから分離する。
注:DNアーゼの使用が所望される特定用途は、プロウイルスDNAからの干渉を排除する/減少させるHIV RNA特定PCRである。
1.DS/DBSから核酸を高効率で溶出させる処理バッファー。この開示は、AbbottのmWash 1バッファー(3.5M GITC;5% Tween 20;50mM KOAc,pH6.0)のDBS処理/溶出バッファーとしての使用を含む。注:Abbottは以前商業的HIV DBS VLプロトコル、及びAbbott mLysisバッファーを処理バッファー(4.66M GITC;10% Tween 20;100mM Trizma,pH7.8)として使用する商業的CE−IVD HIV定性DBSアッセイを提供していた。これら2つの方法の比較を以下に提示し、本発明の方法の予期せぬ優位性を示す。
1.DBS含有チューブからの液体のロボット移動は以下のステップからなる:
・DBSを「チューブボトム検出」アルゴリズムを用いて使い捨てチップによりサンプルインプットチューブの底部に押し進める。
・使い捨てチップをサンプルインプットチューブの底部から短距離(例えば、3mm)だけゆっくり引っ込め、DBSが使い捨てチップを妨害していないことを確認するために少量(例えば、50μl)の吸引を実施する。少量の吸引が終了したら、使い捨てチップを液体の表面上のポイントまで引っ込める。
・同一の使い捨てチップを用いて、液体の表面を検出し、部分容量追跡吸引(例えば、1mLに達するまで450μl)をその場所から実施する。最初の部分容量吸引が完了したら、更なる核酸抽出ステップのために使い捨てチップ中に含まれている液体を反応チューブに移動させる。
・全サンプル移動容量が得られるようにこれらのステップを繰り返す。
・Promega(ウィスコンシン州マデイソン)RQ1 RNアーゼフリーDNアーゼ(カタログ番号M6101);2U/反応物;室温;30分間。
・Ambion(ニューヨーク州グランドアイランド)DNアーゼI(RNアーゼフリー)(カタログ番号AM2222);2U/反応物;室温;30分間。
・Roche(スイス国バーゼル)DNアーゼI組み換体RNアーゼフリー(カタログ番号04716728001);20U/反応物;室温;30分間。
・他の適当なDNアーゼは公知であり得、過度の実験なしで本明細書中に記載されている方法を用いて当業者により同定され得る。本明細書中にリストされている基準を満たしている限り、本発明は特定のDNアーゼに限定されない。以下の図面に記載されているアッセイは例示にすぎず、本発明を特定のDNアーゼまたは特定のスクリーニング方法に限定するのに役立たない。
・Promega RQ1 RNアーゼフリーDNアーゼ(カタログ番号M6101);2U/反応物;室温;10分間。
・Ambion DNアーゼI(RNアーゼフリー)(カタログ番号AM2222);2U/反応物;室温;30分間。

感度及びアッセイロバスト性改善について実施した研究
初期オープンモードアッセイで、およそ2〜3%のインターナル研究サンプル及び>5%のエクスターナル研究サンプルはm2000rt 4450または4442エラーを有しており、よって無効であった。残留グアニジンにより、高い抑制の頻度及び4450及び4442エラーが生じたと確認された。グアニジンのキャリーオーバー量及び頻度を減らすためにHIV−1 DBSアプリケーション仕様ファイルを改変した。廃棄物除去中の撤回速度を遅くすると、ピペットチップから垂下している液滴の分散が減る。サンプルインプットとして1mlでDBSサンプルがWash1バッファー(DBS溶出バッファー)中に存在しているので、反応物中の溶解バッファーの容量は2400から1600μlに減少し、各反応物中に存在するGITCの量が減少する。洗浄有効性はWash2容量を700から750μlに増加させることにより向上した。これらの変化を実行すると、4450及び4442エラーの頻度はおよそ0.2%(データ示さず)に減少した。この最適化により、アッセイロバスト性が大きく改善された。
Claims (12)
- 血液サンプル中のHIV−1核酸を検出するための自動化方法であって、
a)i)固体担体上で乾燥させた、HIVに感染していると疑われる血液サンプル、ii)溶出バッファー、iii)自動化プログラマブルサンプル調製装置、iv)自動化プログラマブルPCR装置、v)DNアーゼ、及びvi)HIV−1核酸を検出するのに適したPCR試薬を用意するステップ;
b)前記溶出バッファーを用いて、前記固体担体から前記血液サンプルを溶出させて、溶出サンプルを作成するステップ;
c)更なる核酸抽出及び精製のために、前記溶出サンプルを前記自動化プログラマブルサンプル調製装置に装入して、処理サンプルを作成するステップ;
d)前記自動化プログラマブルPCR装置に前記PCR試薬を装入するステップ;
e)自動化プログラムを開始して、前記PCR試薬を前記処理サンプルにアリコートするステップ;
f)前記自動化プログラマブルPCR装置を用いて、前記処理サンプル中の抽出核酸に対してPCRを実施するステップ;
g)前記自動化プログラマブルPCR装置より得たPCR結果を分析して、サンプルがHIV−1核酸を含んでいるかを決定するステップ;
を含み、
h)前記溶出バッファーはおよそpH6.0でおよそ3.5M GITC、およそ5% ポリソルベート20、およそ50mM KOAcを含み、ここで用語「およそ」は指定されている値から±10%の変動を指す
方法。 - 前記処理サンプルに前記PCR試薬を添加した後に前記処理サンプル、前記PCR試薬、または完全PCR反応物の1つ以上に、DNアーゼを添加する、請求項1に記載の方法。

- 自動化方法がソフトウェアコマンドによりプログラムされている、請求項1に記載の方法。
- ステップi)が実施され、DNアーゼが特定DNアーゼ反応バッファーを必要とせず、周囲温度またはPCRサイクリング段階中の温度で有効であり、30分以内にDNAを効果的に分解し、DNAを効果的に分解した後に不活化する必要がなく、RNA配列の検出に悪影響を及ぼさない、請求項1に記載の方法。
- 固体担体が濾紙である、請求項1に記載の方法。
