以下の実施例は、本発明のいくつかの実施形態および態様を説明する。関連分野の当業者には、本発明の趣旨および範囲を逸脱することなく、様々な改変、添加、置換などを実施することができること、そのような改変および変形が、続く特許請求の範囲に規定されるような本発明の範囲内に包含されることが明らかであろう。以下の実施例は、本発明に何らかの限定を加えるものではない。
この実施例では、移植前にCXCL12ポリペプチドを用いて被覆している同種膵島が、膵島の生存および機能の延長、ならびに移植部位での制御性T細胞(Treg)の蓄積をもたらすことができるか否かを決定しようとした。膵島カプセルは、概して、フィブロネクチンを含有し、また、理論に拘束されることを望むものではないが、CXCL12は、このマトリックスタンパク質に安定に結合すること、およびそこから溶出することのどちらも可能である(15)。したがって、BALB/Cマウス由来の膵島を、緩衝溶液(例えばPBS)に曝露するか、または約100ng/mLもしくは約1μg/mLの濃度のCXCL12を用いて被覆し、ストレプトゾトシン(STZ)処置した糖尿病C57BL/6マウスの左腎被膜下に移植した。>250mg/dLの血中グルコースが2回連続して記録される糖尿病状態に戻った時点で、マウスを屠殺した。およそ1μg/mLのCXCL12を用いて被覆された同種の膵島移植片は、PBSのみに曝露された同種膵島と比べて有意に長い時間にわたって、非糖尿病状態のレシピエントマウスの維持をもたらした(p=0.012、カプラン−マイヤー・ログ・ランク検定)(図1A)。100ng/mLのCXCL12ポリペプチドを用いて被覆された膵島は、PBS曝露された膵島と同等の速さで拒絶された(p=0.31)。移植片内への単核細胞の浸潤は、約1μg/mLでCXCL12被覆を行った場合、PBS対照と比べて減少した(図1B)。PBS処理された同種移植片と比べ、CXCL12被覆された同種膵島にインスリンが発現し、CXCL12が存在することも示された(図1B)。移植片内へのCD3 T細胞およびFoxP3 T細胞の浸潤を定量した。免疫組織化学によって判定したところ、膵島移植片内へのCD3+T細胞の浸潤は、CXCL12では、PBS曝露された同種膵島と比べて有意に減少した(図1C、p=0.001)。さらに、膵島の同種移植片のCXCL12被覆は、PBS曝露された同種膵島と比べて、CXCL12被覆された移植片内またはその周囲におけるFoxP3+T細胞の浸潤の有意な増加を伴った(図1D、p=0.0016)。
次に、全身免疫抑制を、例えば、低用量のシクロスポリンA(CsA)(例えばおよそ2mg/kg)処置の形で同時に使用することによって、CXCL12被覆された同種膵島の生存を高めることができるか否かを決定しようとした。CXCL12被覆のみだと、PBS曝露された対照と比べて同種膵島の生存が延長されることがこれらの実験で示されたが(p=0.0245)、低用量のCsA処置と組み合わせたCXCL12被覆では、膵島の生存の延長は認められなかった。したがって、CXCL12被覆とCsA処置とを組み合わせた結果、CXCL12被覆のみと比べて膵島の生存の減少を生じる可能性がある(p=0.046)(図2A)。膵島の生存についての対照群と実験群との間の有意な差は、移植後23日目まで示された。CXCL12被覆に加えてCsA処置された動物では、CXCL12被覆のみ(p=0.0002)またはCsA処置のみ(p=0.0027)に比べて、膵島移植片の染色が、重度のCD3+細胞の浸潤を示した(図2B〜図2C)。CXCL12被覆された膵島を受けた動物の処置に、低用量のCsAを加えることもまた、CXCL12被覆のみの膵島に比べた際の、移植片内へのFoxP3細胞の浸潤の有意な減少につながった(p=0.0188)(図2C)。CsAが、CXCL12に関するシグナリング経路、例えば、ケモカイン媒介性の細胞遊走の特異的な因子を阻害することは、以前に考察されていた(16)。本明細書に提示された知見は、CXCL12被覆と低用量のシクロスポリンAとの同時使用による、同種膵島移植での免疫保護効果の増大は認められないことを表す。
同一遺伝子の膵島を移植した場合、CXCL12被覆が、拒絶を低減または予防する役割を果たすことができるか否かを決定するために、糖尿病NOD/LtJマウスを使用した。このモデルでは、非糖尿病NOD/LtJマウス由来の同一遺伝子の膵島を、STZ処置された糖尿病NOD/LtJマウス内に移植した。同一遺伝子の膵島をCXCL12被覆することによって、PBS曝露された膵島よりも有意に長い期間の正常血糖がもたらされた(図4A)(p=0.017)。組織病理学的および免疫組織化学的な試験では、PBS処理された膵島内には重度の単核細胞の浸潤があったものの、CXCL12被覆された膵島にはなかったことが示された(画像示さず)。すなわち、H&E染色では、およそ1μg/mLのCXCL12を用いて被覆された膵島移植片内への単核細胞の浸潤の減少が示され、インスリンおよびCXCL12の免疫蛍光染色では、CXCL12被覆された移植片での両タンパク質のレベルの増加が示された。この実験でのCXCL12およびインスリンの染色では、PBS曝露された膵島と比べて、健康なCXCL12陽性のインスリン産生膵島であることが示された。同一遺伝子の膵島をCXCL12被覆することはまた、PBS曝露された同一遺伝子のNOD/LtJの膵島に比べて、ドナー膵島内へのCD3+T細胞の浸潤を低減した(図4B、p=0.0081)。同一遺伝子のNOD/LtJの膵島をCXCL12被覆することは、PBS処理された対照に比べて、膵島移植片中のFoxP3+細胞の数の有意な増加につながった(p=0.0019)(図4C)。自然発生糖尿病NOD/LtJマウスでは、同一遺伝子の膵島をCXCL12被覆することによる、糖尿病が再発する速さの低減は認められなかった一方で(図5A)(p=0.24)、ヘマトキシリンおよびエオシン染色ならびにインスリン/CXCL12染色では、対照に比べて、CXCL12被覆された膵島で単核細胞の浸潤が減少し、インスリン発現が増加することが示された(図5B〜図5C)。さらに、PBS対照に比べて、一貫して、CXCL12被覆された移植片内へのCD3+T細胞浸潤の減少およびFoxP3+細胞浸潤の増加があった(図5D)。これらを合わせると、同一遺伝子の膵島をCXCL12被覆することによって、前糖尿病モデルでは拒絶を防ぐことができるが、糖尿病モデルでは必ずしもそうではない。この同一遺伝子移植の状況で、このようなCXCL12被覆の生存推進効果の不足は、例えば、確立した体液性の抗膵島免疫応答は、移植する膵島をXCL12被覆することによって効率よく途絶されないことに起因する可能性がある。
早期のインスリン自己抗体の発現は、糖尿病の進行に相関し、おそらくは膵島炎を本質的に反映するのではないかと、以前に考察された(17〜19)。実施例3に示されるように、移植のための膵島のCXCL12被覆の効能は、抗膵島抗体が予め形成された状況における介入として、いっそう有効である。この実施例では、アルギン酸マイクロカプセル中にCXCL12を組み込むことが、細胞誘導性かつ体液性の抗膵島免疫に対し物理的障壁および生物学的障壁の両方をもたらすことによって、移植された膵島を保護することができるかを決定しようとした。
被包体へのCXCL12被覆または組込みによって異種膵島または同種膵島の移植片の免疫隔離が持続される機構が、CD8+T細胞の選択的な化学反発と、移植部位へのCD4+Treg細胞の誘引とに関係するか否かを決定するために、NOD/LtJマウス由来のT細胞サブ集団上のCXCR4の発現、および該サブ集団の遊走を、フローサイトメトリーおよび遊出アッセイそれぞれによって検討した。培地のみ、組換えCXCL12、CXCL12被覆された膵島、またはCa−LVM−CXL12被包された膵島に応答した、NOD/LtJマウスに由来するT細胞を、ボイデンチャンバーベースのアッセイで検討した。各条件について、上部チャンバーに、精製CD3+CD8+T細胞(図7Aおよび図7C)またはCD4+CD25hi+Treg細胞(図7Bおよび図7D)を充填した。上部および下部のウェルには、培地、CXCL12(およそ1μg/mL)、またはCXCL12(およそ1μg/mL)を用いて被覆された膵島、もしくはCa−LVM−CXCL12(およそ1μg/mL)を用いて被包された膵島を充填した。CD3+CD8+T細胞とCD4+CD25Hi Tregのどちらも、CXCL12、CXCL12被覆された膵島、もしくはCa−LVM−CXCL12被包された膵島に応答して化学走性を示した。しかし、CD8+細胞を、CXCL12、CXCL12被覆された膵島、またはCa−LVM−CXCL12被包された膵島と共にインキュベートした際に、Treg細胞よりも有意に大きな化学反発性応答が検出された。実施した条件では、Treg細胞の検出されうる化学反発性のレベルは測定されなかった。
実施例1〜5に提示された知見は、CXCL12を用いて膵島を被覆することによって、STZ処置された糖尿病NOD/LtJマウスで、同種または同一遺伝子の膵島移植での膵島拒絶の遅延をもたらしうることを示す。具体的には、糖尿病または感作されたNOD/LtJマウス内に移植する場合、CXCL12を組み込んだCa−LVMベースのアルギン酸被包体が、既に存在する体液性および細胞誘導性の両方の抗膵島応答について、同種膵島を保護したことが観察された。
動物および糖尿病の誘導。6週齢の雌BALB/c(H2d)マウス、6週齢の雌C57BL/6(H2b)マウスおよび6〜8週齢の雌NOD/LtJ(H2g7)マウスを使用した。動物は、例えばJackson Laboratory(バーハーバー、メイン州)から購入することができる。全ての手順は、実験動物の人道的管理に関する公衆衛生局方針に従って実施し、マサチューセッツ総合病院で研究動物管理に関する小委員会によって承認された。高血糖は、18〜20週齢のNOD/LtJマウスには自然発生し、4週齢および6週齢のNOD/LtJマウス、ならびに6週齢のC57BL/6マウスには、200mg/kgのストレプトゾトシン(Sigma−Aldrich、セントルイス、ミズーリ州)の腹腔内(IP)注射によって誘導された。3回連続して血中グルコースが250mg/dLを上回る読み取りであったマウスは、高血糖であるものと考えた。
2種類の遊走アッセイを実施した。第1に、以前にPoznansky et al.,Journal of clinical investigation,109,1101 (2002)に報告されているように、ボイデンチャンバー(96ウェルフォーマット、3μm孔径、ChemoTx System、Neuro Probe Inc、ゲイザースバーグ、メリーランド)を使用して、細胞遊走を測定した。例えば、0.5%FBSを補充したおよそ30μLのRPMI1640を、50個の対照の膵島または1μg/mL CXCL12(PeproTech Inc、ロッキーヒル、ニュージャージー州)を用いて被覆した膵島と併せて、下部および上部のチャンバーに添加した。CD4、CD25および/またはCD8 MACS分離キット(Miltenyi Biotec、オーバーン、カリフォルニア州)をそれぞれ用いて単離した7,000個のT−reg細胞またはCD8 T細胞を、ボイデンチャンバーの上部チャンバー内に充填した。37℃、5%CO2で3時間インキュベートした後、膜の上部表面の細胞を除去し、下部チャンバー中の遊走細胞を3連で計数した。これとは別に、CXCL12がカプセルに組み込まれたまたは組み込まれない被包化された膵島に対する遊走も、Transwell system(24ウェルフォーマット、3μm孔径、Corning)を使用して測定した。端的に述べれば、0.5%FBSを補充した500μLのRPMI1640を単独で、または10個の対照の膵島もしくはおよそ1μg/mLのCXCL12を組み込んだカプセルと共に、下部チャンバー内に充填した。100μLの培地を単独で、または様々に処理したカプセルと共に、所望の集団の105個の細胞と併せて、上部チャンバー内に充填した。細胞を37℃、5%CO2で3時間インキュベートし、遊走細胞の数を3連で計数した。どちらのアッセイでも、遊出の指標を、CXCL12存在下で計数された細胞数と、培地のみの対照で計数された細胞数との比として算出した。
脾細胞を非処置のNOD/LtJマウスから採取し、Mouse Erythrocyte Lysing Buffer (R&D Systems)を使用して赤血球を溶解した。次いで、細胞を表面マーカーCD3ε、CD4、CD8α、CD25およびCXCR4(CD184)(CD3εはクローン145−2C11、CD4はクローンRM4−5、CD8αはクローン53−6.7、CD25はクローンPC61、CXCR4はクローン2B11、BD Biosciences)について染色し、固定し、BD Cytofix/Cytoperm (BD Biosciences)を用い製造業者の取扱説明書に従って浸透させ、細胞内マーカーFoxP3(クローンMF23、BD Biosciences)について染色した。フローサイトメトリーは、4 Laser LSR II(BD Biosciences)にて実施した。FlowJoソフトウェア(Tree Star)を使用してデータを解析し、ゲーティングは、蛍光1項目除外ゲートストラテジーを使用して達成した。
群間の膵島移植片の生存率は、カプラン−マイヤー法を使用して比較し、生存データは、GraphPad Prism 5統計ソフトウェアを使用することによって解析した。数値変数は、スチューデントのt検定を使用して比較した。0.05を下回るp値を、統計的に有意であるものと考えた。

Claims (38)
- 対象に埋め込むのに適した溶出マトリックスであって、前記溶出マトリックスが、タンパク質を発現する少なくとも1つの細胞を含み、
前記細胞が、前記タンパク質について浸透性であるCXCL12ポリペプチド溶出多孔性ポリマーマトリックスに被包されており、かつ、
埋め込み後少なくとも1カ月にわたり前記マトリックス周囲のエフェクターT細胞を退けるために、前記マトリックス中のCXCL12ポリペプチドの濃度が少なくとも100ng/mLである、
溶出マトリックス。 - 前記CXCL12ポリペプチドが、100ng/mLから1μg/mLの濃度で、マトリックス中に存在する、請求項1に記載の溶出マトリックス。
- 前記マトリックスがカプセルを形成する、請求項1または2に記載の溶出マトリックス。
- 前記マトリックスの厚さが、200〜500ミクロンである、請求項1〜3のいずれか1項に記載の溶出マトリックス。
- 前記少なくとも1つの細胞が、前記対象に対して同種細胞または異種細胞である、請求項1〜4のいずれか1項に記載の溶出マトリックス。
- 前記少なくとも1つの細胞が、少なくとも1カ月間活性を保持する、請求項1〜5のいずれか1項に記載の溶出マトリックス。
- 前記活性が、対象の血中グルコースレベルを調節している、請求項6に記載の溶出マトリックス。
- 前記少なくとも1つの細胞が、膵島細胞である、請求項1〜7のいずれか1項に記載の溶出マトリックス。
- 前記膵島細胞が、インスリン産生細胞、iPS細胞に由来する膵島細胞、ブタ膵島細胞またはヒト膵島細胞である、請求項8に記載の溶出マトリックス。
- 前記少なくとも1つの細胞が、筋細胞、線維芽細胞、軟骨細胞、脂肪細胞、線維筋芽細胞、腎臓細胞、肝細胞、膵臓細胞、腸細胞、骨芽細胞、造血細胞、神経細胞、平滑筋、骨格筋細胞、筋細胞、上皮細胞、肝実質細胞、または幹細胞である、請求項1〜6のいずれか1項に記載の溶出マトリックス。
- 前記マトリックスが、アルギン酸ゲルを含む、請求項1〜10のいずれか1項に記載の溶出マトリックス。
- 前記アルギン酸ゲルが、2%w/vである、請求項11に記載の溶出マトリックス。
- 前記アルギン酸ゲルが、共有結合で架橋されている、請求項11または12に記載の溶出マトリックス。
- 前記アルギン酸ゲルが、1.5から2%w/vの高マンヌロン酸のカルシウム架橋アルギン酸塩を含む、請求項11〜13のいずれか1項に記載の溶出マトリックス。
- 前記アルギン酸ゲルが、75kDa未満の平均分子量のアルギン酸マトリックスのアルギン酸ポリマーを含む、請求項11〜14のいずれか1項に記載の溶出マトリックス。

- 前記期間が、少なくとも1か月である、請求項29に記載の溶出マトリックス。
- 前記膵島細胞が、前記対象の空腹時血清グルコース濃度を、80mg/dLから120mg/dLの間の血中レベルに維持する、請求項29または30に記載の溶出マトリックス。

- CXCL12ポリペプチド溶出多孔性ポリマーマトリックスに被包された同種移植または異種移植の膵島細胞を含む、対象に埋め込むのに適した溶出マトリックスであって、前記マトリックスが、
a)200〜500ミクロンのマトリックス厚、および100ng/mL〜1μg/mLのマトリックス中のCXCL12ポリペプチド濃度と、
b)I型糖尿病である対象の血清グルコース濃度を調節する薬剤が、前記マトリックスを通じて拡散するような多孔性と、
c)前記薬剤の前記膵島細胞との相互作用に基づく、前記膵島細胞によるインスリン産生と、ここで、前記インスリンは、前記マトリックスを通じて、前記対象の前記血清グルコース濃度を調節するのに充分な速さで放出され、
前記マトリックス中のCXCL12ポリペプチドの濃度が、埋め込み後少なくとも1カ月にわたり前記マトリックス周囲のエフェクターT細胞を退けるのに十分な濃度である
ことを特徴とし、前記マトリックス厚、CXCL12ポリペプチド濃度および溶出の速さが、少なくとも4カ月の期間にわたって前記膵島細胞の分解を阻害するように選択され、それによって、前記期間の間、前記対象の血中グルコースレベルの制御が実現される、溶出マトリックス。 - 前記期間が6カ月間である、請求項37に記載の溶出マトリックス。
